Abstract:The present paper addresses the implications of proper comments on prescriptions based on practical experience. These implications include: first, safer, more effective and economical medical treatments; second, reinforced thoughts of evidence-based medicine and safe use of drugs; third,optimized varieties of drugs used in the hospital; and fourth, sustained improvement of quality of medical care. When implementing comments on proscriptions, attention should be given to the following aspects:a. retrospective or progressive reviews should be conducted on prescriptions on one day or a period oftime. Subjects can be prescriptions of one drug, one kind of drugs, one-disease-specific drugs, one medical record or a prescription by one physician. It should be noticed that comments on prescriptions are very important to education in use of drugs, especially those on controversial prescriptions or ones that pharmacists might consider as unjustified or having potential drug interractions, b. Criteria and process of comments should be rational and pcople-friendiy, c. Function of computers should also be emphasized for their indispensable assistance in commenting on prescriptions.
Analysis of Severe Irrational Inpatient Medical Orders Intercepted by an Online Software

Abstract:OBJECTIVE To estimate the level of rational drug use when prescribing inpatient medical orders before and after comprehensive pharmaceutical interventions. METHODS Analysis was performed on all records on automatic warning of severe irrational medical orders from workstation of the software with function of prescription screening and warning during July 2011-June 2012. Series of comprehensive pharmaceutical intervention measures were taken by pharmacists, including trainings, typical case analysis, revision of standard operation procedures and tracing method. Records during July 2012-Feb 2013 were data mined again. RESULTS Compared with the first stage, proportion of severe irrational medical orders relative to all medical orders for inpatients decreased significantly(P-0.05) and average number of cases related with pediatric contraindication, administration route contraindication, dose over the maximum dosage and incompatibility decreased significantly by 74.8%, 89.4%, 66.7% and 16.6%, respectively. CONCLUSION Comprehensive pharmaceutical interventions have silent transforming influence on physicians, irrational medical orders can be effectively decreased and the awareness of rational drug use at the stage of prescribing can be improved greatly.

Abstract:ObjectiveTo share the relevant experiences in pharmaceutical administration and promote the rationality of physician orders for discharged patients.MethodsPercentage of irrational physician orders for discharged patients was listed as an index of quality and patient safety (QPS). A PDCA tool (plan-do-check-action) was applied into continuous quality improvement. The latest monitoring data was data-mined from the perspectives of error type, medication category, department characteristics and physician qualiifcation.ResultsPercentage of irrational physician orders for discharged patients decreased from 8.5% to below 2.0%, the current value is signiifcantly less than 8.4%-25% in hospitals in developed countries. Prospective prescription auditing and online database screening could decrease the occurrence of irrational physician orders. However, training and education is necessary for sustainable control of the QPS index.ConclusionsThe rationality in physician orders for discharged patients can be achieved by performing project management, application of PDCA, multiple discipline team working and comprehensive interventions.

Abstract:AIM:To review and summarize drug metabolism and its related interactions in prescribing drugs within the similar therapeutic or structural class for gastrointestinal disease treatment so as to promote rational use of medicines in clinical practice.METHODS:Relevant literature was identified by performing MEDLINE/Pubmed searches covering the period from 1988 to 2006.RESULTS:Seven classes of drugs were chosen, including gastric proton pump inhibitors, histamine H(2)-receptor antagonists, benzamide-type gastroprokinetic agents, selective 5-HT(3) receptor antagonists, fluoroquinolones, macrolide antibiotics and azole antifungals. They showed significant differences in metabolic profile (i.e., the fraction of drug metabolized by cytochrome P450 (CYP), CYP reaction phenotype, impact of CYP genotype on interindividual pharmacokinetics variability and CYP-mediated drug-drug interaction potential). Many events of severe adverse drug reactions and treatment failures were closely related to the ignorance of the above issues.CONCLUSION:Clinicians should acquaint themselves with what kind of drug has less interpatient variability in clearance and whether to perform CYP genotyping prior to initiation of therapy. The relevant CYP knowledge helps clinicians to enhance the management of patients with gastrointestinal disease who may require treatment with polytherapeutic regimens.

Abstract:OBJECTIVE To develop a popular electronic medication record software and promote rational administration.METHODS The electronic medication record was set up by programming software,and observed its function of rational administration in clinical practice. RESULTS The electronic medication record software was successfully developed and had advantages in universality,helping clinical pharmacists to make up for knowledge lacking,wording as a platform for systematic research and information-sharing,helping doctors′ rational drug use.CONCLUSION As a widely applicable software,the eletronic medication record will promote the development of clinical pharmacy in hospitals with a low information level,and is worth being popularized and promoted.

Abstract:OBJECTIVE To enhance the rational drug use of intravenous antineoplastic agents.METHODSRetrospective statistical analysis of irrational drug use of antineoplastic agents, detected by pharmacists during prescription check in Center for Pharmacy Intravenous Admixture Services (PIVAS), were conducted. RESULTSThe main problems were as follows: selection of irrational dilution solution (about 51%), incorrect implementation of doctors′ orders by nurses (about 18%), inappropriate medication route (about 15%), absence of pharmacoeconomic considerations (about 9%) and irrational dosage regimens (about 7%). The modification rate of irrational prescriptions reached more than 98% after clinical interventions by pharmacist. CONCLUSIONPharmacists in PIVAS can play pivotal roles in rational use of intravenous antineoplastic agents. It is essential to further improve the ability of pharmacist in prescription check, conduct medication education to nurses, establish guidelines for rational drug use and strengthen communications among medical staff.

Abstract:Rational or irrational drug use is actually relative.The aims of rational drug use are safety,effectiveness and economy,all of which can't be always satisfied together.Otherwise,any treatment protocol can not be fit to all of the characteristics of disease,patients and pharmacy.The judgment of rational drug use is also related owing to the relativity of the credibility and reliability of the results of clinical trials and clinical information,etc.At last,we must emphasize that the result of rational drug use is not always satisfactory,as it might be influenced by many factors.In clinical work,we must do our best to avoid irrational drug use.But if can't,we should estimate the benefits and harms,and then make scientific decision.Furthermore,pharmaceutical care and doctor-patient communication should be enhanced.

Abstract:The article analyzes the situation on irrational use of antibiotics in China and why there is irrational drug use in medicalinstitutions, it points out that the promotion of rational drug use is a long-term, tough and complicated work. It is suggested that clinically rationaldrug use be promoted by strengthening interventions and the national drug policies be worked out based on international experience. Abstract:Objective: To improve the quality of prescriptions, and to promote rational clinical use of drugs and ensure medical safety. Methods: This research analyzed the irrational prescriptions according to 1881 prescriptions which were selected from 19 Community Health centers in Mi-Yun district. Results: There were 814 irrational prescription items, and 632 irrational prescriptions, accounting for 33.6 percent of the total 1881 prescriptions, most of which are inappropriate medication prescription. Conclusions: In practice, the physicians and pharmacists should continue to study, perfect the prescription review work and improve the quality of prescriptions.

Abstract:OBJECTIVE: To analyze the irrational drug use in the outpatient emergency department of our hospital in order to improve clinical medication level in our hospital.METHODS: A total of 5 000 prescriptions between May 2007 and May 2009 were randomly sampled from outpatient emergency department of our hospital for retrospective analysis.RESULTS: Of all the prescriptions reviewed,8.6%(430/5 000) were found to be irrational.CONCLUSION: The drug use in the outpatient emergency department of our hospital is rational yet it is far from perfect,therefore,it is important for clinicians and pharmacists to strengthen their learning on pharmacy.

Abstract:Objective:To discuss the role of clinical pharmacists in the clinical treatment and promote rational use of drugs by analyzing the medication recommendations given by clinical pharmacists.Methods:The medication recommendations provided by clinical pharmacists from June 2014 to May 2015 were retrospectively collected,and the types,content and quantity of the recommendations were summarized and analyzed.Results:There were 1 254 pieces of medication recommendations in total,involving a total of 11 departments which were related to the allocation of clinical pharmacists.The number of medication recommendations showed an upward trend in the year.The medication recommendations could be classified into 29 types,involving 61 kinds of drugs,among which the top three were antibacterial drugs,nutritional drugs,and drugs for regulation and treatment of gastrointestinal tract function.Conclusion:The participation of clinical pharmacists in drug treatment is conducive to timely detection of problems in the course of medication,so that patients can get safe,effective,economic and rational treatment regimens.

Abstract:Background: As one of the countries with the most serious degree of aging, the incidence of potentially inappropriate drug use among the elderly is as high as 30. 4% in Chinese communities, and the lack of effective medication management and poor medication compliance at home are the main factors. Given these situations, we constructed a Rational Medication Management Mode based on family physician service, carried out an empirical research and evaluated the implementation effect. Methods: A prospective cohort study was conducted from September to December 2021 to analyze the implementation effect of the Rational Medication Management Mode by comparing the outcome indicators between the intervention group and control group. The primary outcome of this study was medication number and polypharmacy (taking 5 or more medications) at 90 days. The secondary outcomes included the situation for behavioral self-management and knowledge-belief-behavior of rational medication use. Results: A total of 618 elderly patients (309 in the intervention group and 309 in the control group) with multimorbidity were included in this study, those were all available at follow-up at 90 days. At 90 days, the number of medications was achieved by 3.88 (1.48), and patients with polypharmacy were reduced by 59.55% in the intervention group, having a significant difference compared with the control group (P < 0.001). Patients with medication reminders, intermittent medication and adverse drug reactions were achieved in 294 (95.15%), 47 (15.21%), and 51 (16.51%) respectively in the intervention group (P < 0.001). The knowledge, belief, behavior security and behavior compliance of rational medication use of elderly patients were all greatly improved in the intervention group at 90 days (P < 0.0001). Conclusion: The Rational Medication Management Mode based family physician service, which provides the support of manuals and pillboxes, can decrease the elderly patients' number of drugs with multimorbidity, reduce the incidence of polypharmacy, enhance behavioral self-management, increase the knowledge and belief of rational medication use, and improve the security and compliance of medication usage behavior. In order to provide a practical basis for rational medication management of elderly patients with multimorbidity under the background of long-term prescriptions in China.

Abstract:According to China's Seventh National Census, 18.70% of a total of 1.41 billion people were 60 or older and 13.50% were 65 or older, so China's population is increasingly aging. In conjunction with China's socioeconomic and scientific and technological development and its promotion of medical insurance-related policies, rational medication management for the elderly is a concern in order to control the risk of polypharmacy. This paper summarizes and discusses the following five key issues inherent in rational medication management: i) an increase in serious polypharmacy and the potential risks of medication; ii) a lack of medication consultation service and medication withdraw without healthcare providers' supervision; iii) poor medication compliance among the elderly; iv) insufficient quantity and incompetence of pharmaceutical staffing; and v) limited awareness of pharmaceutical services and lack of trust in the ability of pharmacists. Based on a discussion of factors influencing these issues, suggestions have been put forward in the hopes that China emphasizes rational medication management in order to reduce the risk of polypharmacy and the disease burden of the elderly in China.

Abstract:Objective To analyze the situation of irrational drug use in our hospital .Methods The irrational prescriptions were classified into 8 kinds.The Pareto diagram analysis was used to make irrational prescrip-tions classification .The factors of primary , secondary and common were analyzed.Results Three primary factors about irrational prescriptions were in unsuitable drugs , Indication and no identifies use anti -biotics. The secondary factors were inappropriate combined drugs and repeated administration.Common factors were unsuitable usage and amount , ultra instruction, frequency and unsuitable route of administration . Conclusion Clinical pharmacist should intervene effectively according to irrational types , raising the level of rational use of medicines develop appropriate interventions , control primary and secondary factors .
Rational Drug Use in the Prescriptions for Chronic Patients in Rural Health Centers of Shandong Province Under the Essential Medicine System

Abstract:Objective To investigate the rational drug use in the prescriptions for chronic patients in rural health centers of Shandong Province under the essential medicine system. Methods In August 2013,we collected the prescriptions for chronic patients in 37 rural health centers of Shandong Province before and after the implementation of essential medicine system. Several aspects concerning with rational drug use were analyzed, including the number of drug categories of each prescription, antibiotics prescription, injection prescription, the clarity of prescription writing and the completeness of prescription content. RSR and clustering analysis were jointly employed to make comprehensive evaluation of the rational drug use in the prescriptions for chronic patients. Results After the implementation of essential medicine system, the proportion of antibiotics prescriptions decreased from 34. 0%(50/147)to 20. 5%(60/292),and the proportion of prescriptions with clear writing improved from 79. 6%(117/147)to 90. 4%(264/292),with the differences both significantly different(P<0. 05);the proportion of prescriptions with more than five drug categories decreased from 21. 1%(31/147) to 18. 5%(54/292),the proportion of injection prescriptions decreased from 33. 3%(49/147) to 27. 4%(80/292),and the proportion of prescriptions with complete content increased from 20. 4%(30/147)to 28. 4%(83/292),while the three changes were not significant(P>0. 05 ) . From the RSR and clustering analysis, we knew that the rational drug use scores of most prescriptions reached" middle level"(RSRdrug use≥0. 399),with a proportion of 79. 4%(232/292). Conclusion After the implementation of the essential medicine system,rational drug use in the prescriptions for chronic patients in rural health centers has improved,and the prescriptions are mostly with rational drug use,while it still needs further improvement.
