Abstract:We draw upon a ‘small history’ of one family to throw light on lived experience of welfare in the past, and consider how it may provide some glimpses into what Britain’s current economy of welfare trajectory could mean, where the state welfare safety net has holes and an ad hoc charitable safety net is being constructed beneath them. Using archived case notes from the Charity Organisation Society across the interwar period to the comprehensive welfare state, we discuss one family’s negotiation of poverty and the fragmented economy of welfare involving nascent state provision and a safety net of myriad charitable bodies, and the need to be judged as respectable and worthy. While lived experience of inequalities of assessment criteria, provision and distribution provide some indication for the potential trajectory of contemporary welfare in Britain, towards fragmented localised settlements, the small history also reveals a muted story of alternatives and reliability.

Abstract:In April 2024, current UK prime minister Rishi Sunak announced a clampdown on "sick note culture."1 Former Conservative MP Matthew Parris claimed people were "abusing" the disability benefits system.2 But with 2024 figures from the Department for Work and Pensions (DWP) showing that fraud for personal independence payments (PIP) and disability living allowance are 0.0% and 0.1%, respectively3—something isn't adding up. The current Conservative government has cast blame on sick and disabled people for systemic failings. Understanding welfare conditionality in the context of a generational habitus: A qualitative study of older citizens in England

Abstract:In many welfare states, 'austerity' policies have ignited debates about the fairness and cost-effectiveness of universal welfare benefits, with benefits received by older citizens a particular topic of concern. Empirical studies suggest that conditionality generates problems of access and uptake but, to date, there has been little research on how different conditions of entitlement are understood by older citizens. This study drew on interviews with 29 older citizens from three areas of England to explore how eligibility for and uptake of different kinds of welfare benefits were understood. In interviews, current entitlement was understood in relation to a generational habitus, in which 'our generation' was framed as sharing cohort experiences, and moral orientations to self-reliance, hard work and struggle. Entitlement to some welfare benefits was taken for granted as a reward owed by the state to its citizens for hard-earned lives. State transfers such as pensions, free travel and fuel subsidies were congruent with a nationalised generational habitus, and fostered recognition, self-worth and the sense of a generation as a collective. In contrast, transfers contingent on economic or need-based conditionality were more explicitly framed as 'benefits', and negatively associated with vulnerability and moral contestation. Uptake was therefore often incompatible with their generational habitus. Calls for introducing further conditionality to benefits for older adults are often based on claims that this will increase fairness and equality. Our analysis suggests, however, that introducing conditionality has the potential to promote inequality and foster differentiation and division, within the older population and between generations.
Elegant Economy in Elizabeth Gaskells Cranford: A Socialist Feminist Study

Abstract:Elizabeth Gaskell (1810-1865) was a Victorian writer who had to undergo various kinds of condescension for her writings. After bearing the stigma of being conformist, conventional, and meek as ascribed to her by the contemporary feminist critics, Gaskell’s writings are being revisited with a new feminist perspective in recent years. The present paper is also a humble attempt to rediscover the feminist dimension of her writings by exploring one of her novels, Cranford (1853), through a socialist feminist lens. Cranford presents such a social structure that is devoid of a Class system and constructed by women in a matrilineal society as against the capitalist patriarchal society of Drumble. This Matriarchal socialist social structure is based on the values of cooperation, humanity, and motherly care characteristic to the differently developed gendered subjectivity of women. The social change through the agency of woman foreshadows Gaskell’s far-sighted feminist views of the 1970s.
Social inequalities in 'sickness': European welfare states and non-employment among the chronically ill

Abstract:The aim of this paper is to examine educational inequalities in the risk of non-employment among people with illnesses and how they vary between European countries with different welfare state characteristics. In doing so, the paper adds to the growing literature on welfare states and social inequalities in health by studying the often overlooked 'sickness'-dimension of health, namely employment behaviour among people with illnesses. We use European Union Statistics on Income and Living Conditions (EU-SILC) data from 2005 covering 26 European countries linked to country characteristics derived from Eurostat and OECD that include spending on active labour market policies, benefit generosity, income inequality, and employment protection. Using multilevel techniques we find that comprehensive welfare states have lower absolute and relative social inequalities in sickness, as well as more favourable general rates of non-employment. Hence, regarding sickness, welfare resources appear to trump welfare disincentives.

Abstract:Objectives: Long-term sickness absence from employment has negative consequences for the economy and can lead to widened health inequalities. Sick notes (also called 'fit notes') are issued by general practitioners when a person cannot work for health reasons for more than 7 days. We quantified the sick note rate in people with evidence of COVID-19 in 2020, 2021 and 2022, as an indication of the burden for people recovering from COVID-19. Design: Cohort study. Setting: With National Health Service (NHS) England approval, we used routine clinical data (primary care, hospital and COVID-19 testing records) within the OpenSAFELY-TPP database. Participants: People 18-64 years with a recorded positive test or diagnosis of COVID-19 in 2020 (n=365 421), 2021 (n=1 206 555) or 2022 (n=1 321 313); general population matched in age, sex and region in 2019 (n=3 140 326), 2020 (n=3 439 534), 2021 (n=4 571 469) and 2022 (n=4 818 870); people hospitalised with pneumonia in 2019 (n=29 673). Primary outcome measure: Receipt of a sick note in primary care. Results: Among people with a positive SARS-CoV-2 test or COVID-19 diagnosis, the sick note rate was 4.88 per 100 person-months (95% CI 4.83 to 4.93) in 2020, 2.66 (95% CI 2.64 to 2.67) in 2021 and 1.73 (95% CI 1.72 to 1.73) in 2022. Compared with the age, sex and region-matched general population, the adjusted HR for receipt of a sick note over the entire follow-up period (up to 10 months) was 4.07 (95% CI 4.02 to 4.12) in 2020 decreasing to 1.57 (95% CI 1.56 to 1.58) in 2022. The HR was highest in the first 30 days postdiagnosis in all years. Among people hospitalised with COVID-19, after adjustment, the sick note rate was lower than in people hospitalised with pneumonia. Conclusions: Given the under-recording of postacute COVID-19-related symptoms, these findings contribute a valuable perspective on the long-term effects of COVID-19. Despite likely underestimation of the sick note rate, sick notes were issued more frequently to people with COVID-19 compared with those without, even in an era when most people are vaccinated. Most sick notes occurred in the first 30 days postdiagnosis, but the increased risk several months postdiagnosis may provide further evidence of the long-term impact.
The Anathattractive s/State: A Marxist-Semiotic Analysis of the Discourse, Ideology and Practice of Neoliberal Workfare

Abstract:This paper explores the complex semiotic entanglement between discourse, ideology and practical application surrounding the neoliberal welfare policy known as workfare. It focuses particularly on the Flexible New Deal - the version of workfare trialled in the UK from 2009 to 2010 under New Labour. The Flexible New Deal was a privately administered, for-profit, payment-by-results scheme which made receipt of welfare benefits conditional upon reciprocal activity – in particular, attendance at workfare centres for “re-training”. This paper provides a Marxist-semiotic analysis of an actual Flexible New Deal centre, based on participant observation in 2010. This is framed within a wider analysis which proposes that an economic undercode dialectically interacts with class-racist ideology to semiotically sculpt the victims of structural unemployment into an apparent culture of pathological dysfunction – a theatrical illusion which nevertheless renders each unemployed person victim to a mendacious “rhyming up” with media folk-devils. In conclusion, this paper argues that via the term “underclass”, individual states of poverty are transformed into the perception of a socially-cancerous, welfare funded, expanding state (i.e. nation) within the State. This elision of personal and plural, and transubstantiation of the processes of poverty into people, is marked by the novel signifier “s/State”.

Abstract:A major aim of the Labour Government elected in May 1997 was to change British social policy away from passive income maintenance towards promoting employment, investment and opportunity. Many of its policy initiatives bear the clear imprint of a social development model. Investment in education and health are seen as paramount, community rebuilding through partnership‐based economic renewal and the promotion of social capital through neighbourhood renewal are all explicit priorities in the government's spending plans. Note: this approach to policy will continue in Britain now that Labour has been returned for a second term in the June 2001 general election.

Abstract:Modern economies rely on economic growth for stability and prosperity. Further, periods of stagnation and recession are currently associated with poor health and wellbeing outcomes for citizens. However, 50 years of research indicates that this state of dependency is ecologically unsustainable. It is therefore critical that we better understand the growth dependency of our economies and welfare systems, and how to overcome it. In this paper we first offer a working definition of growth dependency, clarifying its core dimensions. Next, taking the UK's adult social care sector as a case study, we argue that growth dependencies emerge in the welfare state as a result of three distinct dynamics: 1) growth in manifest needs; 2) labour productivity growth; 3) the pursuit of economic rents. Lastly, we propose a novel, sector-led framework for identifying, analysing and transforming growth dependencies. We make the case that the growth dependency of the welfare state is not inevitable, but is instead the product of specific social, economic and structural factors that can be disrupted and transformed, if we can find the political will to do so.

Abstract:DO ‘welfare states’ enhance or subvert economic enterprise, civic virtue, private moral character, the integrity of social life? Though these questions have a piquantly contemporary ring in modern British politics, they are nevertheless old quandaries in the history of social policy. Since the seventeenth century, if not earlier, practitioners, theorists and critics of public welfare schemes have argued for and against such schemes in contradictory and adversarial terms; claiming on the one hand that social welfare schemes would supply a humanitarian corrective to the rigours of a market economy; and on the other hand that they would support and streamline market forces by enhancing individual and collective efficiency. Similarly, for several hundred years models of civic morality which emphasize independence and self-sufficiency have jostled with alternative models which emphasize paternalism, altruism and organic solidarity. Few phases of social policy in Britain and elsewhere have not contained elements of more than one approach. Even the New Poor Law, notorious for its subordination to market pressures, nevertheless harboured certain residual anti-market principles and often lapsed into practices that were suspiciously communitarian; whilst Edwardian New Liberalism, famous for its philosophy of organic solidarism, in practice tempered social justice with the quest for ‘national efficiency’. These varying emphases have all been reflected in the fashions and phases of welfare state historiography—fashions and phases that appear to have been at least partly determined by the vagaries of prevailing political climate. Thus, in the aftermath of the Second World War, historians tended to portray the history of social policy as a series of governmental battles against private vested interests—battles in which the mantle of civic virtue was worn by an altruistic administrative elite, while civic vice was embodied in the motley crew of doctors, landlords, employers and insurance companies who viewed social welfare as a commodity in the market.

Abstract:Abstract Drawing on interviews with welfare claimants living in Essex, UK, this article examines the material and symbolic effects of the UK government’s 2012 Welfare Reform Act, and it highlights the participants’ interpretations of and responses to that. In reaction to their sense of material and symbolic exclusion, participants made moral claims for their inclusion through a notion of social citizenship based on collective reciprocity and care. They claimed to have paid-in to the national purse in various material and moral ways until circumstances outside of their control meant they could no longer do so. They thus asserted a moral-economic right to social inclusion and an ensuing right to receive adequate, non-stigmatised, and non-punitive welfare. These moral-economic claims differ from other, more public, counter-narratives to welfare reform and government austerity, and they assert a clear but subtle opposition to the market-bound logic of the reform .

Abstract:Choosing Health is the first government public health white paper since Saving Lives: Our Healthier Nation, published in 1999. It aims to make the NHS ‘a health service, not a sickness service,’ and is strongly influenced by the Wanless reports. This paper presents a critical review of the main elements of the white paper that relate to mental health, and sets out the actions that policy makers and service delivery organisations need to take to build genuinely mental health promoting public services within a mentally healthy society.
Post-Fordism and Welfare: An Analysis of Change in the British Health Sector

Abstract:In this paper I consider the relevance of commentaries on the transition from Fordism to post-Fordism and specifically their applicability to the welfare state. I suggest that some such commentaries have identified interesting parallels without providing much original analysis of why the putative transitions have come about. I also note various trends in the British health sector, which cannot be explained solely by reference to changes in the production process. Then, events in the British health sector since 1979 are considered in terms of the extent to which they can be interpreted in terms of a putative transition from Fordism to post-Fordism. This is divided into two sections. In the first, measures taken between 1979 and the National Health Service (NHS) reforms are examined, and it is argued that numerous measures taken prior to the NHS reforms failed to resolve some of the perceived crises of the NHS. The NHS reforms implemented from 1991 are then discussed and a number of problems are identified with these reforms, which highlight the difficulties of implementing a neoliberal reform strategy in the context of the British NHS. In particular the political conditions have yet to be established which might facilitate the wider extension of such a strategy, though this does not rule out changes that might restrict the scope of the NHS.

He worked as a GP in the Welsh mining village of Glyncorrwg — describing himself in this book as 'a Glyncorrwgologist, the only one in the world.' He was the first GP in the world to know the blood pressure of every one of his patients, and he did this just from seeing his patients coming in for their regular health problems, without a 'health check' in sight. He is perhaps most famous for describing the inverse care law in 1971, which he describes here as 'a banal truism.' Abstract:Abstract People with intellectual disabilities face health disparities, including in high‐income countries such as the United Kingdom, despite publicly funded healthcare. This paper describes the healthcare system in England (a nation of the United Kingdom) for the general population, and more specifically for people with intellectual disabilities. Key legislation that impacts the lives of people with intellectual disabilities, such as the UK Equality Act 2010 ( https://www.legislation.gov.uk/ukpga/2010/15/contents ), the Mental Capacity Act 2005 ( https://www.legislation.gov.uk/ukpga/2005/9/contents ), and the UN Convention on the Rights of Persons with Disabilities, and its implementation in the United Kingdom, is discussed. The role of deinstitutionalization and the shift to living in the community for people with intellectual disabilities is also discussed. Programmes that have been implemented to address the health disparities experienced by people with intellectual disabilities are reviewed. Finally, the recent changes to healthcare organization in the UK, the COVID‐19 pandemic, and the implementation of the Valuing People white paper are discussed.

Abstract:Journal of Sociology, Ahead of Print. This paper explores the intricate dynamics of digital welfare within the context of late-capitalist welfare states, focusing on the advanced digitalisation initiatives of Denmark. It offers a critical analysis of the concept of 'digital welfare' – defined as the integration of digital technologies into the frameworks of social protection. Central to the research is how the Danish welfare state, recognised for its e-government leadership, engages with and incorporates critiques to sustain its approach to digital transformation. Utilising Boltanski's theories on the absorption of capitalist critique and Mathiesen's 'silently silencing' concept, the study examines the state's proficiency in assimilating criticism. The findings reveal the persistent flexibility of late-capitalist governance systems, especially in how digital welfare policies are leveraged to reconnect citizens with state mechanisms while simultaneously mitigating dissent. The research highlights the necessity for ongoing investigation into the implications of digitalisation for welfare institutions and societal structures at large.
