Abstract:Sirs, Schultz et al. (1981) reported increased prevalence of the HLA A1 antigen in HLA B27 positive patients with Reiter’s syndrome and acute anterior uveitis. Khan et al. (1980) reported increased prevalence of the HLA A2 antigen in HLA B27 positive patients with ankylosing spondylitis and acute anterior uveitis. Both studies were made in a mixed population of American Whites and Blacks. Sen Gupta et al. (1979) examined Asian patients with acute anterior uveitis. They found an increased prevalence of the HLA A9 antigen in HLA B27 positive patients. Furthermore, Khan & Braun (1982) reported a high prevalence of the HLA A9 antigen in Asian-American patients having both acute anterior uveitis and ankylosing spondylitis. We have previously (Moller et al. 1981) reported lack of any association between HLA A antigens and acute anterior uveitis. We have examined 122 patients with ankylosing spondylitis (Msller et al. 1982). Forty-eight of the 112 HLA B27 positive patients had experienced acute anterior uveitis. None of the HLA antigens Al, A2 or A9 was associated with having experienced acute anterior uveitis (Table 1). The conflicting results from different studies of patients who had acute anterior uveitis alone or in combination with other HLA B27associated diseases, may be due to one or more factors. Firstly, the findings may reflect chance variation in small samples. Secondly, genes located close to the HLA A locus may affect HLA B27-associated diseases susceptibility. Acute anterior uveitis may be due to environmental factors. Different environmental factors could have different capacities to trigger the development of acute anterior uveitis in persons with the same or different genetic predisposition. Although our
