Abstract:Introduction EQ-5D is one of the most frequently used health-related quality of life (HRQoL) measures but has been found to be insensitive in detecting differences in health status in some general populations and disease groups. For example, the appropriateness of applying EQ-5D in the Chinese cultural setting has been widely discussed. Adding additional HRQoL dimensions (bolt-on items) can be a solution to both retain the original descriptive system of EQ-5D, while enhancing its sensitivity to the local context. To date, no studies have proposed culturally relevant bolt-ons for China or examined the psychometric properties of such bolt-on items. This protocol documents the identification, development, selection and psychometric testing of culturally relevant bolt-on items for EQ-5D-5L in China. Methods and analysis We will identify and develop candidate bolt-on items that are most relevant in the Chinese culture, through former literature reviews on health concepts important for the Chinese population, conducting expert consultations and qualitative interviews. We will quantitatively test the acceptability and measurement properties (including distributional characteristics and construct validity) of the candidate items in both general and disease populations in a cross-sectional setting. The patient group will be followed up to collect two-time-point data to assess test–retest reliability of the candidate items. Bolt-on item selection will consider both the qualitative and quantitative evidence gathered. This protocol outlines a comprehensive mixed-methods process for identifying, developing, selecting and testing bolt-on items that are relevant and culturally appropriate in China. This study may serve as a guide for similar initiatives in other cultural contexts. Ethics and dissemination This study received ethics approval from the Institutional Review Board of School of Public Health, Fudan University (IRB number: 2022-TYSQ-03-154). Study findings will be disseminated through international peer-reviewed journal articles as well as public, academic presentations at national and international conferences.

Validation of the EQ-5D in a general population sample in urban China

Abstract:Purpose To evaluate the reliability and validity of the EQ-5D in a general population sample in urban China. Methods Thousand and eight hundred respondents in 18 communities of Hangzhou, China were recruited by multi-stage stratified random sampling. Respondents self-administered a questionnaire including the EQ-5D, the SF-36, and demographic questions. Test–retest reliability at 2-week intervals was evaluated using Kappa coefficient, the intraclass correlation coefficient. The standard error of measurement (SEM) was used to indicate the absolute measurement error. Construct validity was established using convergent, discriminant, and known groups analyses. Results Complete data for all EQ-5D dimensions were available for 1,747 respondents (97%). Kappa values were from 0.35 to 1.0. The ICCs of test–retest reliability were 0.53 for the EQ-5D index score and 0.87 for the EQ VAS score. The SEM values were 0.13 (9.22% range) and 4.20 (4.20% range) for the EQ-5D index and EQ VAS scores, respectively. The Pearson’s correlation coefficients between the EQ-5D and the SF-36 were stronger between comparable dimensions than those between less comparable dimensions, demonstrating convergent and discriminant evidence of construct validity. The Chinese EQ-5D distinguished well between known groups: respondents who reported poor general health and chronic diseases had worse HRQoL than those without. Older people, females, people widowed or divorced, and those with a lower socioeconomic status reported poorer HRQoL. Respondents reporting no problems on any EQ-5D dimension had better scores on the SF-36 summary scores than those reporting problems. Conclusions The Chinese version of the EQ-5D demonstrated acceptable construct validity and fair to moderate levels of test–retest reliability in an urban general population in China.
[Reliability and Validity of a Short-Form Health Survey Scale (SF-36), Chinese Version Used in an Elderly Population of Zhejiang Province in China].

Abstract:OBJECTIVETo evaluate the validity and reliability of a Chinese version on the Short-Form Health Survey Scale (SF-36) among elderly population.METHODSQuestionnaire including SF-36 Chinese version was administered in a cross-sectional study from October to December 2007. 4241 elderly people over 60 years old from rural and urban area in Zhejiang province were face-to-face interviewed by well-trained investigators, and then analyses on correlation, reliability, factors, t-test and one-way ANOVA were made to evaluate on reliability and validity of the scale.RESULTSThe SF-36 Chinese version had good split-half reliability (r = 0.91, P < 0.001) and all the internal consistency Cronbach's alpha coefficients exceeded 0.8 except for VT (alpha = 0.65), SF (alpha = 0.65) and MH (alpha = 0.40). The correlative coefficients between each item and its domain were all greater than the 0.4 thresholds except item 9 - 2, and they were greater than the correlation between the item and other domains for all but item 9 - 8. These results demonstrated that the SF-36 Chinese version had good convergent validity and discriminant validity. The distribution of 35 items in 6 public factors (the cumulative variance = 67.04%) extracted from them was consistent with the basic conceived concept. All domains except MH had good validity to discriminate different categories.CONCLUSIONThe SF-36 Chinese version had good reliability and validity. It was acceptable for the evaluation on quality of life in elderly population, but the reliability and validity of MH were relative low and the items such as 9 - 2, 9 - 8 in MH and 3 - 1 in PF were not suitable for Chinese elderly population.

Abstract:OBJECTIVES: This study aimed to understand the psychometric properties of EQ Health and Wellbeing (EQ-HWB) and to examine its relationship with EQ-5D-5L in a sample covering patients, carers, and general public.METHODS: A cross-sectional study was conducted in Guizhou Province, China. The acceptability, convergent validity (using Spearman correlation coefficients), internal structure (using exploratory factor analysis), and known-group validity of EQ-HWB, EQ-HWB-Short (EQ-HWB-S), and EQ-5D-5L were reported and compared.RESULTS: A total of 323 participants completed the survey, including 106 patients, 101 carers, and 116 individuals from the general public. Approximately 7.4% of participants had at least 1 missing response. In the EQ-HWB and EQ-5D-5L items related to activities, there were more level 1 responses. The correlations between EQ-HWB and EQ-5D-5L items ranged from low to high, confirming the convergent validity of similar aspects between the 2 instruments. Notably, EQ-HWB measures 2 additional factors compared with EQ-5D-5L or EQ-HWB-S, both of which share 3 common factors. When the patient group was included, EQ-5D-5L had the largest effect size, but it failed to differentiate between the groups of general public and carers. Both EQ-HWB and EQ-HWB-S demonstrated better known-group validity results when carers were included.CONCLUSIONS: EQ-HWB measures a broader quality of life construct that goes beyond health measured by EQ-5D-5L. By encompassing a broader scope, the impact of healthcare interventions may become diluted, given that other factors can influence wellbeing outcomes as significantly as health conditions do.

Abstract:The EQ-5D has been used to assess health related quality of life (HRQoL) in respiratory conditions. However, the core descriptive system may not be sensitive to all the HRQoL impacts of these conditions. To increase the sensitivity of the descriptive system, two respiratory specific bolt-ons, have been developed. Psychometric assessment of the bolt-ons in comparison to other validated instruments is required to facilitate their use. Therefore, the aim of this study is to test the psychometric characteristics of the EQ-5D-5L + R using a large dataset collected in Australia.

Development and Psychometric Tests of a Chinese Version of the SF-36 Health Survey Scales.

Abstract:OBJECTIVE:To develop and evaluate scaling and scoring assumptions, and the reliability and the validity of a Chinese version of the SF-36 scales.METHODS:A multi-stage mixed sampling procedure was used to select a representative sample of the general population. The sample size was 1 000 households. All family members of a selected household, aged 18 and older, completed a survey by self-administration. Formal psychometric methods for testing assumptions underlying item scoring and scale construction were used according to the standard procedure of the IQOLA Project.RESULTS:Of the 1 985 collected questionnaires, 1 972 were qualified. Of them, 1 688 (85.6%) were respondents. 1 316 respondents answered all 36 items, while the remaining (372 respondents) answered with one or several missing responses. Among the non-respondents, 65.5% were illiteracy or quasi-illiteracy. The assumption of equal intervals was violated for the VT and MH scales. The recoded item values were used to calculate scale scores. The clustering and ordering of the item means were approximately the same as that of the source version and other two Chinese versions. The items in each scale had similar standard deviations except those in the PF, BP, SF scales. The correlations between an item and its hypothesized scale were identical for all except the SF and VT scales. The scaling success rates of convergent validity were 0% for the SF scale, 75% for the VT scale, and 100% for the other six scales. The scaling success rates of discriminat validity ranged from 87.5% to 100% for all scales except for the SF scale. The Cronbach' alpha coefficients of internal consistency reliability ranged from 0.72 to 0.88, which were satisfactory for group comparison except 0.39 for the SF scale and 0.66 for the VT scale. The two-weeks test-retest reliability coefficient ranged from 0.66 to 0.94. Factor analysis identified two principal components: a "physical" factor and a "mental" factor. Taken together, these two factors could be used to explain 56.3% of the total variance.CONCLUSION:The Chinese version of the SF-36 Health Survey Scale has achieved conceptual equivalence and satisfied the psychometric scaling assumptions well enough to warrant wide use in China. Known-groups validity will give more meaningful evidences of the validity of the Chinese SF-36 scales.

Abstract:The quality of life among the elderly with cognitive function impairment has been rarely studied in the Chinese population. The EuroQol-5D system (EQ-5D) is a standardized instrument to assess the health-related quality of life. We report the performance of the EQ-5D in a cross-sectional study in Shanghai, China, comparing cases of dementia with cases of MCI and normal controls. An age- and sex-matched sample of 34 dementia patients, 34 MCI patients and 32 normal controls (mean age =74.0, 49% female) was recruited into the SCOBHI pilot study. Proxies of study subjects were administered the EQ-5D. The EQ-5D describes health status according to five dimensions: mobility, pain/discomfort, self-care, anxiety/depression, and usual activities. Each dimension has three levels, namely, “no problems”, “some problems”, and “severe problems”. It also consists of a visual analogue scale (VAS) ranging from 0 (worst imaginable health state) to 100 (best imaginable health state). Trend chi-square test was used to detect the association between the severity of cognitive function impairment and problems in EQ-5D dimensions. One-way ANOVA was used to compare the means of VAS among subjects in 3 study groups. Most proxies (96%) were spouses and children of study subjects. For the dimension of “self-care”, 27% of dementia patients had problems, whereas none of MCI and control subjects had problems (trend p<0.001). For the dimension of “usual activities”, the percentage of subjects who had problems in dementia, MCI and control groups were 97%, 97% and 84%, respectively (trend p=0.049). Dementia patients had the significantly worst health state (mean VAS=51±24) compared with that of MCI (mean VAS=85±15) and control subjects (mean VAS=95±10) (p<0.001). There's no significant relation between severity of cognitive function impairment and problems in EQ-5D dimensions of “mobility”, “pain/discomfort”, and “anxiety/depression”. We found a significant association between the severity of cognitive function impairment and problems in “self-care” and “usual activities” of EQ-5D dimensions and VAS. Cognitive function impairment did not affect other aspects of quality of life among the Chinese elderly.

Abstract:The aim of this study was to develop a brief version of cognitive assessment test for evaluating the efficacy of treatments targeting cognitive impairments in Chinese schizophrenia patients, to examine its reliability, and establish normative data. Stratified according to age, gender, and educational level, healthy adult subjects were recruited from fifteen institutions in seven administrative regions of China and 723 valid samples were obtained, of which 50 were retested. Generalized Linear Models were conducted to analyze the effects of age, sex, and education. There was no significant difference between genders, while significant effects were demonstrated respectively among age and education on the normative data of C-BCT. The Cronbach α of C-BCT is 0.75, and the test-retest reliability (ICC) ranged from 0.62 to 0.76. Normative data of C-BCT were generated by gender, age and education, and the effects of these demographic factors were analyzed. It revealed good internal consistency and test-retest reliability of C-BCT.
Selecting Bolt-on Dimensions for the EQ-5D: Testing the Impact of Hearing, Sleep, Cognition, Energy, and Relationships on Preferences Using Pairwise Choices

Abstract:Generic preference-based measures (GPBMs) such as the EQ-5D are valid across many conditions, but in some cases, "bolting on" additional dimensions may improve validity. The selection of "bolt-ons" has been based on the psychometric impact of individual dimensions, but preferences provide another important way to select them. This study aims to test the potential of using pairwise choices to inform the selection of bolt-ons for the EQ-5D-5L. General population preferences were collected using an online survey of 1040 UK residents. Three EQ-5D-5L health state pairs were selected based on pairs that had a 50:50 split in respondent preferences from a previous pairwise survey. Participants were presented with pairwise choices of EQ-5D-5L health states without and with bolt-ons of hearing, sleep, cognition, energy, and relationships, each added individually. Logistic models were used to assess the impact of bolt-ons, as well as bolt-ons at different severity levels, on the log odds of responders choosing between health states. Preferences varied according to the bolt-ons and their severity level (only levels 1, 3, and 5 were used). Additions of bolt-ons at level 1 generally resulted in nonstatistically significant differences while additions of bolt-ons at level 3 and level 5 produced a negative and statistically significant impact on preferences for the health state with the bolt-on. At level 5, hearing had the largest impact, followed by cognition, relationships, energy, and sleep. At level 3, cognition produced the largest impact, followed by hearing and sleep with similar impacts, energy, and relationships. This ordering offers information for bolt-on selection, with hearing and cognition appearing as the most important. The weight placed on the different health problems is not constant across severity levels between bolt-ons. Pairwise choices provide a cost-effective approach of generating information on preferences to support bolt-on selection.

Chinese Version of the EQ-5D Preference Weights: Applicability in a Chinese General Population

Abstract:OBJECTIVES:This study aimed to test the reliability, validity and sensitivity of Chinese version of the EQ-5D preference weights in Chinese general people, examine the differences between the China value set and the UK, Japan and Korea value sets, and provide methods for evaluating and comparing the EQ-5D value sets of different countries. METHODS:A random sample of 2984 community residents (15 years or older) were interviewed using a questionnaire including the EQ-5D scale. Level of agreement, convergent validity, known-groups validity and sensitivity of the EQ-5D China, United Kingdom (UK), Japan and Korea value sets were determined. RESULTS:The mean EQ-5D index scores were significantly (P<0.05) different among the UK (0.964), Japan (0.981), Korea (0.987), and China (0.985) weights. High level of agreement (intraclass correlations coefficients > 0.75) and convergent validity (Pearson's correlation coefficients > 0.95) were found between each paired schemes. The EQ-5D index scores discriminated equally well for the four versions between levels of 10 known-groups (P< 0.05). The effect size and the relative efficiency statistics showed that the China weights had better sensitivity. CONCLUSIONS:The China EQ-5D preference weights show equivalent psychometric properties with those from the UK, Japan and Korea weights while slightly more sensitive to known group differences than those from the Japan and Korea weights. Considering both psychometric and sociocultural issues, the China scheme should be a priority as an EQ-5D based measure of the health related quality of life in Chinese general population.
Psychometric Testing of the Chinese Version of the Coping and Adaptation Processing Scale-Short Form in Adults with Chronic Illness

Abstract:Background Adaptive capacity may serve as an indicator of the individuals' coping behaviors toward illness management and may contribute to day-to-day living with chronic illness and improved quality of life. Practical and well-constructed instruments for measuring adaptation have not been adequately explored. An English 15-item Coping and Adaptation Processing-Short Form (CAPS-SF) for assessing adaptation has been created and validated in line with the underlying tenets of Coping and Adaptation Processing theory, but there is no applicable Chinese version. Methods The CAPS-SF was translated and culturally adapted into simplified Chinese. Among Chinese adults with chronic illness, 81 patients were selected for cultural adaptation and 288 patients were approached for psychometric testing. Content validity was evaluated by an expert panel. Construct validity was tested by confirmatory factor analysis. Concurrent validity and predictive validity were analyzed by Spearman correlation coefficient. Reliability was assessed by internal consistency and test-retest coefficients. Floor/ceiling effect was calculated. Results Adequate content validity was ensured by the expert panel. A four-factor structure (resourceful and focused, self-initiated and knowing-based, physical and fixed, and positive and systematic) describing individuals' coping strategies was identified and verified. Concurrent validity and predictive validity were demonstrated by strong correlations with the confrontation of coping mode (r= 0.46) and a quality-of-life measure (r= 0.58). The McDonald's omega coefficient of total scale was 0.82. Split-half reliability and test-retest reliability were 0.87 and 0.87. No floor/ceiling effect was present. Conclusion The Chinese version CAPS-SF is a theoretically based and culturally acceptable instrument with sound psychometric properties. Further studies are advocated to refine its four-factor structure.

Abstract:MATRICS Consensus Cognitive Battery (MCCB), packaging 10 tests selected from more than 90 nominated tests, is a method developed by the Measurement and Treatment Research to Improve Cognition in Schizophrenia (MATRICS) group to evaluate the efficacy of treatments targeting cognitive impairments in schizophrenia. MCCB had been translated into a number of languages, but only the US and Spain had normative data reported. Inconsistency in translation and cultural differences make direct application of MCCB in China problematic. In this study, we administered the battery to a representative community sample based on Chinese population census in 2005 and obtained normative data. The effects of age, gender, education level, and scale of residence area on test performance were examined. The sample included 656 healthy volunteers from six sites in China. At each site, sample was stratified according to age, gender, and educational level, and scale of the area one was born in, grew up in and currently living in was recorded. We found age, gender, and education had significant effects on the normative data for MCCB in China, which are comparable to those found for the original standardized English version in the U.S. and the Spanish version in Spain. Remarkably, the residence scale effects on neuropsychological performance were significant, which should be taking into account when calculating the standardized T score for each subject. The practice effects were minor and test-retest reliability of MCCB was good, which suggests MCCB as an appropriate measure for clinical and research usage in China.
Establishing language and ethnic equivalence for health-related quality of life item banks and testing their efficiency via computerised adaptive testing simulations

Abstract:Purpose We aimed to (1) establish linguistic and ethnic equivalence (i.e. lack of bias) for the items in the English and Chinese versions of the Singapore Health and Well Being (SHAWS) Physical Functioning (PF), Positive Mindset (PM) and Social Relationship (SR) item banks (IBs); and (2) evaluate the preliminary efficiency of these IBs using Computer Adaptive Testing (CAT) simulations. Methods In this cross-sectional study, 671, 670, and 672 subjects answered 55, 48 and 30 items of the PF, PM, and SR IBs, respectively. Rasch analysis was conducted to assess each IB’s psychometric properties, particularly the presence of differential item functioning (DIF) for language and ethnicity. A set of performance criteria related to removing items that displayed notable DIF were employed. CAT simulations determined the mean number of items for high, moderate, and moderate-low measurement precisions (stopping rule: SEM 0.300, 0.387. 0.521, respectively). Results Half of subjects were >50 years old (40.9% PF, 42.1% PM, 41.4% SR), Chinese (50.7% PF, 51.0% PM, 50.6% SR) and female (50.0% PF. 49.4% PM, 52.8% SR) respectively. Rasch analysis revealed 4 items with DIF for the PF IB, 9 items with DIF for the PM IB and 2 items with DIF for the SR IB. In CAT simulations, the mean number of items administered was 8.5, 21.6 and 14.5 for the PF, PM and SR IBs, respectively (SEM 0.300), 5.1, 13.0, 8.0 for PF, PM and SR IBs, respectively (SEM 0.387) and 3.1, 5.3 and 4.1 for PF, PM and SR IBs, respectively (SEM 0.521). Conclusion The PF, PM and SR IBs to measure health-related quality of life revealed minimal DIF for language and ethnicity after remedial efforts. CAT simulations demonstrated that these IBs were efficient, especially when the stopping rule was set at moderate precision, and support the implementation of the SHAWS IBs into routine clinical care.

Development of A Children’ s Health Cognition Questionnaire

Abstract:Objective To investigate a tool for comprehensive assessment of children’s health cognition,compile a children’s health cognition questionnaire,and conduct tests on its reliability and validity. Methods Based on the health theory defined by WHO,we collected item data by open - ended interviews( n = 13) from September to November in 2013. We selected 320 children to undertake prediction and completed the formal questionnaire by item analysis( critical ratio method, subject total correlation method,intercommunity and factor loading),exploratory factor analysis(principal component analysis, varimax rotation method,predetermined criterion) and selection by reliability analysis. Questionnaire survey was conducted on 1 336 children in 10 primary schools of Shanghai to undertake confirmatory factor analysis. After formal investigation for one month,the formal questionnaire was used to conduct survey on 140 children again to test the stability of the questionnaire. Results By open - ended interviews, 60 alternative items of children’ s health cognition questionnaire were established. Exploratory factor analysis was conducted on the rest 46 items by item analysis. The KMO value of the test of goodness of fit was 0. 740,and Bartlett’s sphericity test result was significant( χ2 = 3 574. 707,P ﹤ 0. 001);the results showed that these items were suitable for exploratory factor analysis. The exploratory factor analysis showed that 21 items and 3 dimensions were retained,and the 3 dimensions were physical health recognition,mental health cognition,and social adaptability health cognition. The number of factor loading was 0. 402 - 0. 763,which could explain the 58. 47% of variance variation of children’s health cognition score. In the test of reliability,the Cronbach’s α coefficient of all the items of the questionnaire was 0. 806,and the Cronbach’s α coefficients of physical health cognition,mental health cognition,and social adaptability health cognition were 0. 829,0. 778 and 0. 734 respectively. In validity test,the models of χ2 / df = 1. 39,GFI = 0. 92,AGFI = 0. 90,CFI = 0. 93, IFI = 0. 93,TLI = 0. 92 were all ﹥ 0. 90,and PGFI and PNFI were 0. 74 and 0. 70 respectively,higher than the standard of 0. 50,and RMSEA was 0. 04. The overall fitting effect of the questionnaire model and data was good. For the test - retest reliability of the questionnaire,the Pearson correlation of all the items of the questionnaire was 0. 78,and the correlation coefficients of physical health cognition, mental health cognition and social adaptability health cognition were 0. 72,0. 65 and 0. 74 respectively. Conclusion The children’s health cognition questionnaire has good reliability and validity,and it can be used to evaluate children’s health cognition level,while further research and verification are needed to determine if it is fit for young children and regions with underdeveloped economy or education.

Abstract:This research aimed to validate a simplified Chinese version of the Empathy Quotient (EQ; 60 items) for use with Mainland Chinese people. The original English version of the EQ was translated into simplified Chinese. Through an online survey, 588 Mainland Chinese participants completed the EQ and 3 other questionnaires: the Interpersonal Reactivity Index (IRI), the Autism-Spectrum Quotient (AQ), and the 20-item Toronto Alexithymia Scale (TAS-20). Thirty-five participants completed retesting of the EQ 3 to 4weeks later. Sex differences on the EQ scores and psychometric properties of the EQ items were examined. Confirmatory factor analysis suggested that an EQ 15-item structural model fitted the data quite well. Self-report empathy, as assessed by the current simplified Chinese version of the EQ, appeared to relate to participants' autistic and alexithymic traits but not sex.

Abstract:Introduction: The EQ Health and Wellbeing (EQ-HWB) is a new questionnaire for measuring quality of life (QoL) from a broad perspective. The items of the EQ-HWB were derived based on a 'qualitative review' of literature, which reported primarily on Western studies. It can be argued that the QoL is a cultural-related concept and therefore people from China have a different understanding of the QoL. This study aimed to explore whether Chinese citizens could understand the EQ-HWB's candidate items and what they thought of those items. In doing so, we wanted to examine the face validity of the candidate items and explore if further cultural adaptation is necessary. Methods: This research was part of the E-QALY project, in which 36 candidate items were selected for the EQ-HWB from a 97-item pool. In China, three interviewers investigated the face validity of these EQ-HWB candidate items in semi-structured qualitative face-to-face interviews. Respondents were invited to report 'problems' with regard to the interpretation of the items and these problems were grouped into themes. We explored to what extent those themes related to specific cultural aspects in China. We also classified the rates of reported problems for each item into three groups: 1) less than 20%, 2) from 20-50%, and 3) over 50%. Results: For 17 items the rate of reported problems was less than 20%, 15 items fell into the second group (with 20 - 50%) and for 4 items the rate of problems reported was more than 50%. The thematic analysis revealed eight themes: ambiguous problems in the interpretation of 16 items; difficult to understand (11); contained a complex negative expression (10); examples used seemed inappropriate (7); misleading connotation in Chinese (2); long and complex (2); complex response options (1); and use of non-colloquial language (1). Discussion: Our research shows that EQ-HWB candidate items require careful examination to make them more comprehensible. Most of the reported problem themes were generic problems related to the items, and only a few face validity issues appeared to relate to specific cultural aspects in China, even though most of the items were based on Western studies. Our findings are reassuring for the instrument's international application, especially in China.

Abstract:Background: Depression and anxiety symptoms among medical students are often a concern. The Patient Health Questionnaire-Four (PHQ-4), an important tool for depression and anxiety screening, is commonly used and easy to administer. This study aimed to assess and update the longitudinal measurement invariance and psychometric properties of the simplified Chinese version. Methods: A three-wave longitudinal survey was conducted among healthcare students using the PHQ-4. Structural validity was based on one-factor, two-factor, and second-order factor models, construct validity was based on the Self-Rated Health Questionnaire (SRHQ), Sleep Quality Questionnaire (SQQ), and Rosenberg Self-Esteem Scale (RSES), and longitudinal measurement invariance (LMI), internal consistency, and test-retest reliability were based on structural consistency across three time points. Results: The results of the confirmatory factor analysis indicated that two-factor model was the best fit, and LMI was supported at three time points. Inter-factor, factor-total, and construct validity correlations of the PHQ-4 were acceptable. Additionally, Cronbach's alpha, McDonald's omega, and the intraclass correlation coefficient demonstrated acceptable/moderate to excellent reliability of the PHQ-4. Conclusions: This study adds new longitudinal evidence that the Chinese version of the PHQ-4 has promising LMI and psychometric properties. Such data lends confidence to the routine and the expanded use of the PHQ-4 for routine screening of depression and anxiety in Chinese healthcare students.

Abstract:Functional impairments are common in schizophrenia patients, but few performance-based instruments are available to evaluate their functional capacity, especially those that are culturally adapted for use in China. This study aimed to modify the items of the original Beijing Performance-based Functional Ecological Test (BJ-PERFECT) due to limited tasks in a particular scenario and examine the psychometric properties of the newly modified BJ-PERFECT in schizophrenia patients. Participants of the study comprised 105 schizophrenia patients and 45 healthy controls (HCs). All of them were administered the modified BJ-PERFECT, the University of California, San Diego, Performance-based Skill Assessment-Brief version (UPSA-B) and the Global Assessment of Functioning (GAF). The modified BJ-PERFECT retained seven original items and added 4 new items. The validity of the modified BJ-PERFECT was supported by its significant correlation with the UPSA-B but not the GAF and by the significant difference on the instrument's scores between the schizophrenia patients and HCs. The internal consistency reliability was found to be good. Level of education was found to have a significant relationship with functional capacity. The modified BJ-PERFECT was found to be psychometrically valid to assess functional capacity in schizophrenia patients.
