Abstract:Background: Community mental health services in China are in need of a brief, valid instrument to identify mental disorders. Many studies support the Self-Reporting Questionnaire 20 (SRQ-20) as a cost-effective instrument with which to measure community mental health, but there have been no previous studies conducted with the Chinese version of SRQ-20. In this paper, the psychometric properties of the Chinese version of SRQ-20 in community settings were investigated. Method: SRQ-20 was used to evaluate 959 participants from 10 communities of Hangzhou City and 60 participants from two community primary care centres for the presence of mental disorders. Discriminative, concurrent, and construct validity as well as internal consistency and screening properties of SRQ-20 were evaluated. Results: Cronbach’s α for SRQ-20 was 0.90 for primary care and 0.91 for the community. Item-total correlation coefficients ranged from 0.40 to 0.73 in primary care, and from 0.51 to 0.69 in the community. The test-retest correlation coefficient was 0.93 in primary care and 0.94 in the community. Varimax-rotated principal component analysis of the SRQ-20 items yielded three factors: depressive symptoms, anxiety symptoms and somatic symptoms. The area under the ROC curve was 0.83 (SE =0.04, 95% CI 0.75—0.90) for the community and primary care samples. The optimal cut-off point for SRQ-20 was 6/7 yielding sensitivity of 93% and specificity of 62%. Conclusion: SRQ-20 appears to be a reliable and valid measure of mental disorders in the community in China.
Validation of the Nine-Item Patient Health Questionnaire to Screen for Major Depression in A Chinese Primary Care Population

Abstract:Introduction The aim of this study was to examine the reliability and validity of the nine-item Patient Health Questionnaire (PHQ-9) for screening depression in Chinese primary care settings. Methods In 100 randomly selected primary care clinics (PCC) of Hangzhou city, we recruited 2,639 patients (aged 18 years) and applied the PHQ-9 to screen for depression. Then, 280 of them were further interviewed with the Structured Clinical Interview for Diagnostic and Statistical Manual Disorders (SCID) for the diagnosis of major depression. Statistical analysis of the demographic data, reliability, validity and receiveroperator curve were performed. Results The mean administration time was 7 minutes, and the Cronbach's was 0.89. The optimal cutoff score of 10 on the PHQ-9 revealed a sensitivity of 0.87, specificity of 0.81, and positive likelihood ratio of 5.04. The area under the receiveroperator curve in this study was 0.91 (95% confidence interval, 0.870.94). Discussion The PHQ-9 performs well and has acceptable psychometric properties for screening of patients with depression in a Chinese primary care population.
Chinese Version of the Responses to Positive Affect Questionnaire: Testing the Factor Structure, Reliability, and Validity in A College Student Sample

Abstract:This study tested the psychometrics of the Chinese version of the Responses to Positive Affect Questionnaire among 915 Chinese college students with an average age of 20.3 yr. (SD = 1.6). The original three-factor model with the factors dampening, emotion-focused positive rumination, and self-focused rumination was supported using exploratory and confirmatory factor analyses. All subscales showed good internal reliability, as well as evidence for convergent and incremental validity with measures of ego-resiliency, life satisfaction, and mental health symptoms. Finally, a series of hierarchical regression analyses indicated that the three subscales, especially dampening, accounted for additional unique variance in psychological adjustment above and beyond resilience. These findings generally suggested that the Chinese Responses to Positive Affect Questionnaire possesses acceptable psychometric properties. Implications for counseling, limitations, and suggestions for future study were presented.

Abstract:BackgroundInternet intervention in Chinese university students would be a possible approach to overcome the gap between high rate of depression and high rates of underdiagnosis and undertreatment. As a popular measure of screening, the feasibility and user satisfaction of Patient Health Questionnaire-9 items for online program were tested.MethodsThe subjects were enrolled based on an email list from the students' office of a Chinese university, and 300 undergraduate students were randomly invited of which 230 (76.7%) students were willing to participate in the study and completed the first test. After 2weeks, a subsample of 150 (65.2%) subjects were randomly chosen to retake the test for the test-retest reliability. And 81 (35.2%) among the 230 subjects were randomly selected to undergo the Mini International Neuropsychiatric Interview within 48hours. Among 150 subjects, 120 (52.2%) completed client satisfaction questionnaire about this online screening program.Results(1) The Cronbach alpha was 0.80 and the test-retest reliability was 0.78. (2) The optimal cutoff score of 10 revealed a sensitivity of 0.74 and specificity of 0.85, with an area under the curve of 0.897 (95% confidence interval, 0.823-0.970). (3) The mean duration of administration was 3.5minutes. (4) Satisfaction with the online screening program was highly appreciated.ConclusionsThe results indicated potential value of the online screening program for further Internet-administrated programs of depression among Chinese university students.

Abstract:Background:Emotional blunting is prevalent in patients with mood disorders and adversely affects the overall treatment outcome. The Oxford Depression Questionnaire is a validated psychometric instrument for assessing emotional blunting. We aimed to evaluate the reliability and validity of the Chinese version of the ODQ (ODQ) in Chinese patients with mood disorders.Methods:136 mood disorders patients and 95 healthy control participants were recruited at the First Affiliated Hospital of Zhejiang University, School of Medicine. Patients were assessed using the ODQ, Beck Depression Inventory-II (BDI-II), and Montgomery-Asberg Depression Rating Scale (MADRS). Internal consistency reliability and test-retest reliability were analyzed. Confirmatory factor analysis and correlation analysis were used to evaluate construct and convergent validity.Results:A total of 136 patients with mood disorders and 95 healthy controls participated in this study. Cronbach α values were 0.928 (ODQ-20) and 0.945 (ODQ-26). Test-retest reliability coefficients were 0.798 (ODQ-20) and 0.836 (ODQ-26) (p<0.05); intraclass correlation coefficient values were 0.777 (ODQ-20) and 0.781 (ODQ-26) (p<0.01). The score of ODQ was positively correlated with BDI-II and MADRS (r=0.326~0.719, 0.235~0.537, p<0.01). The differences in the ODQ scores between the patient and control groups were statistically significant.Conclusion:The reliability, structural validity, and criterion validity of the ODQ applied to patients with mood disorders meet the psychometric requirements, and the scale can be used to assess emotional blunting in Chinese patients with mood disorders.

Abstract:OBJECTIVETo evaluate the validity and reliability of a Chinese version on the Short-Form Health Survey Scale (SF-36) among elderly population.METHODSQuestionnaire including SF-36 Chinese version was administered in a cross-sectional study from October to December 2007. 4241 elderly people over 60 years old from rural and urban area in Zhejiang province were face-to-face interviewed by well-trained investigators, and then analyses on correlation, reliability, factors, t-test and one-way ANOVA were made to evaluate on reliability and validity of the scale.RESULTSThe SF-36 Chinese version had good split-half reliability (r = 0.91, P < 0.001) and all the internal consistency Cronbach's alpha coefficients exceeded 0.8 except for VT (alpha = 0.65), SF (alpha = 0.65) and MH (alpha = 0.40). The correlative coefficients between each item and its domain were all greater than the 0.4 thresholds except item 9 - 2, and they were greater than the correlation between the item and other domains for all but item 9 - 8. These results demonstrated that the SF-36 Chinese version had good convergent validity and discriminant validity. The distribution of 35 items in 6 public factors (the cumulative variance = 67.04%) extracted from them was consistent with the basic conceived concept. All domains except MH had good validity to discriminate different categories.CONCLUSIONThe SF-36 Chinese version had good reliability and validity. It was acceptable for the evaluation on quality of life in elderly population, but the reliability and validity of MH were relative low and the items such as 9 - 2, 9 - 8 in MH and 3 - 1 in PF were not suitable for Chinese elderly population.

The Application of Chinese Health Questionnaire for Mental Disorder Screening in Community Settings in Mainland China

Abstract:OBJECTIVE:To examine the applicability of Chinese Health Questionnaire (Taiwan version) (CHQ) originated from General Health Questionnaire (GHQ) for mental disorder screening in community settings in mainland China.METHODS:A pilot study was conducted in Hangzhou (n = 377). Three thousand seven hundred and seven subjects were recruited from four cities to validate the results of the pilot study. Validation of the Screening Questionnaire was analyzed, using Relative Operating Characteristic (ROC) method.RESULTS:Cronbach's alpha coefficients were calculated to be 0.79 for the 12-items and 0.89 for 30-items to CHQ version in the first sample, and 0.74 in the second sample (12-items). Four factors were extracted from the CHQ-30, including somatic symptoms, anxiety and worry, social dysfunction, poor family relationship, and depression. CHQ-12 could be explained by a single factor in both samples. The areas under ROC were 0.80 (95% CI: 0.70 - 0.89) for 12 items and 0.72 (95% CI: 0.62 - 0.82) for 30 items. The sensitivities of CHQ-12 and CHQ-30 were found to be 76.9% and 71.8%, and the specificities were 73.8% and 67.9% with Kappa value 0.44 (P = 0.00) and 0.38 (P = 0.00), respectively. The estimated rates of mental disorder were 18.13% (95% CI: 14.16 - 22.10) by CHQ-12, and 22.80% (95% CI: 18.19 - 27.11) by CHQ-30 in the first sample and the rates were 21.72% (95% CI: 20.39 - 23.05) by CHQ-12 in the second sample.CONCLUSION:CHQ, especially CHQ-12 through slight language modification, could be used for epidemiological studies and on community health care to screen for mental disorder in the mainland of China.

Abstract:Background: The Sleep Condition Indicator (SCI), an insomnia measurement tool based on the updated Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5) criteria with sound psychometric properties when applied in various populations, was evaluated here among healthcare students longitudinally, to demonstrate its measurement properties and invariance in this particularly high-risk population. Methods: Healthcare students of a Chinese university were recruited into this two-wave longitudinal study, completing the simplified Chinese version of the SCI (SCI-SC), Chinese Regularity, Satisfaction, Alertness, Timing, Efficiency, Duration (RU_SATED-C) scale, Chinese Patient Health Questionnaire-4 (PHQ-4-C), and sociodemographic variables questionnaire (Q-SV) between September and November 2022. Structural validity, measurement invariance (MI), convergent and discriminant validity, internal consistency, and test-retest reliability of the SCI-SC were examined. Subgroups of gender, age, home location, part-time job, physical exercise, and stress-coping strategy were surveyed twice to test cross-sectional and longitudinal MI. Results: We identified 343 valid responses (62.9% female, mean age = 19.650 ± 1.414 years) with a time interval of seven days. The two-factor structure was considered satisfactory (comparative fit index = 0.953-0.989, Tucker-Lewis index = 0.931-0.984, root means square error of approximation = 0.040-0.092, standardized root mean square residual = 0.039-0.054), which mostly endorsed strict invariance except for part-time job subgroups, hence establishing longitudinal invariance. The SCI-SC presented acceptable convergent validity with the RU_SATED-C scale (r ≥ 0.500), discriminant validity with the PHQ-4-C (0.300 ≤ r < 0.500), internal consistency (Cronbach's alpha = 0.811-0.835, McDonald's omega = 0.805-0.832), and test-retest reliability (intraclass correlation coefficient = 0.829). Conclusion: The SCI-SC is an appropriate screening instrument available for assessing insomnia symptoms among healthcare students, and the promising measurement properties provide additional evidence about validity and reliability for detecting insomnia in healthcare students.
Development and Psychometric Tests of a Chinese Version of the SF-36 Health Survey Scales.

Abstract:OBJECTIVE:To develop and evaluate scaling and scoring assumptions, and the reliability and the validity of a Chinese version of the SF-36 scales.METHODS:A multi-stage mixed sampling procedure was used to select a representative sample of the general population. The sample size was 1 000 households. All family members of a selected household, aged 18 and older, completed a survey by self-administration. Formal psychometric methods for testing assumptions underlying item scoring and scale construction were used according to the standard procedure of the IQOLA Project.RESULTS:Of the 1 985 collected questionnaires, 1 972 were qualified. Of them, 1 688 (85.6%) were respondents. 1 316 respondents answered all 36 items, while the remaining (372 respondents) answered with one or several missing responses. Among the non-respondents, 65.5% were illiteracy or quasi-illiteracy. The assumption of equal intervals was violated for the VT and MH scales. The recoded item values were used to calculate scale scores. The clustering and ordering of the item means were approximately the same as that of the source version and other two Chinese versions. The items in each scale had similar standard deviations except those in the PF, BP, SF scales. The correlations between an item and its hypothesized scale were identical for all except the SF and VT scales. The scaling success rates of convergent validity were 0% for the SF scale, 75% for the VT scale, and 100% for the other six scales. The scaling success rates of discriminat validity ranged from 87.5% to 100% for all scales except for the SF scale. The Cronbach' alpha coefficients of internal consistency reliability ranged from 0.72 to 0.88, which were satisfactory for group comparison except 0.39 for the SF scale and 0.66 for the VT scale. The two-weeks test-retest reliability coefficient ranged from 0.66 to 0.94. Factor analysis identified two principal components: a "physical" factor and a "mental" factor. Taken together, these two factors could be used to explain 56.3% of the total variance.CONCLUSION:The Chinese version of the SF-36 Health Survey Scale has achieved conceptual equivalence and satisfied the psychometric scaling assumptions well enough to warrant wide use in China. Known-groups validity will give more meaningful evidences of the validity of the Chinese SF-36 scales.
Involving adolescents in the development of research tools for adolescent mental health in China: a qualitative research study

Abstract:Background The research tools for the assessment of adolescent mental health are mostly developed for high-income settings, and are used elsewhere without linguistic or cultural modification. Youth involvement in the development of research aimed at young people is recommended by WHO, but few studies have involved young people in the design of mental health tools. In this study we aimed to develop a self-completion questionnaire to assess adolescent mental health based on perceptions of Chinese adolescents. Methods We did a literature search to initially identify 11 scales suitable for assessing adolescent mental health and influencing factors and help-seeking behaviour. Students from rural and urban areas, and from public and private schools in Zhejiang, Anhui, Henan, and Jiangsu provinces, China, were invited to work with researchers to develop a questionnaire. Inclusion criteria were students aged 13–20 years who were willing to be involved in the study. Students initially independently completed the draft questionnaire, and provided detailed feedback in one-to-one interviews with researchers and in focus group sessions. Researchers recorded student inputs. Analysis involved inductive summarisation and factor extraction. Ethical approval was obtained from the Medical Ethics Committee of Zhejiang University. We obtained verbal consent from the volunteers and from the parents of those younger than 18 years. Findings From Feb 9, 2019, to May 16, 2019, 46 student volunteers (30 male and 16 female; mean age 16 years, SD 13–20) were recruited into our study. We finalised a self-completion questionnaire with 99 items including three psychological scales. With youth involvement, we found that standard Chinese translations of widely-used scales were not understandable. Students felt that some items would not be answered honestly, and so they were changed or removed. The students were very eager to include sensitive topics, such as suicide and self-harm, and some suggested more attention should be paid to this. At their request, questions about academic pressure were expanded, with questions on social media added, including specific questions on posting modified pictures and sharing of personal problems online. Interpretation Adolescent participation in questionnaire design improves the structure and content of tools assessing mental health, making such research tools more acceptable and understandable. Abstract:Abstract Background Depression and anxiety among general hospital patients are common and under-recognized in China. This study aimed toward developing a short questionnaire for screening depression and anxiety in non-psychiatric clinical settings, and to test its reliability and validity. Methods The item pool which included 35 questions about emotional distress was drafted through a comprehensive literature review. An expert panel review and the first clinical test with 288 general hospital patients were conducted for the primary item selection. The second clinical test was performed to select the final item in 637 non-psychiatric patients. The reliability and validity of the final questionnaire were tested in 763 non-psychiatric patients, in which 211 subjects were interviewed by psychiatrists using Mini International Neuropsychiatric Interview (MINI). Multiple data analysis methods including principal components analysis (PCA), item response theory (IRT), and receiver operating characteristic (ROC) curve were used to select items and validate the final questionnaire. Results The series selection of items resulted in a 9-item questionnaire, namely Huaxi Emotional-distress Index (HEI). The Cronbachu0027s α coefficient of HEI was 0.90. The PCA results showed a unidimensional construct. The area under the ROC curve (AUC) was 0.88 when compared with MINI interview. Using the optimal cut-off score of HEI (11/12), the sensitivity and specificity were 0.880 and 0.766, respectively. Conclusions The HEI is considered as a reliable and valid instrument for screening depression and anxiety, which may have substantial clinical value to detect patientsu0027 emotional disturbances especially in the busy non-psychiatric clinical settings in China.
Measurement properties of the Chinese version of the Youth Quality of Life Instrument-Weight Module (YQOL-W).

Abstract:BACKGROUND:Childhood obesity is a growing public health concern in China. It not only compromises physical health, but also has negative impacts on psychosocial well-being. As obesity rates increase, finding out what the perceptions of Chinese youth are regarding their weight is important for intervention planning and evaluation. However, there is a paucity of available obesity-specific instruments for children and adolescents in China and youth weight-specific quality of life (QOL) has been little reported. This study aimed to evaluate the measurement properties of the Chinese version of the Youth Quality of Life Instrument - Weight Module (YQOL-W). METHODS:The Chinese version of the YQOL-W was administered to 840 youth aged 11-18 from nine schools. Measurement properties including measurement model, reliability, validity and burden were evaluated. RESULTS:Confirmatory factor analysis showed that a three-factor model had acceptable model fit. The instrument had robust internal consistency reliability with Cronbach's α ranging from 0.84 to 0.96 and acceptable test-retest reliability with the intraclass correlation coefficients (ICCs) all higher than 0.7. The standard error of measurement (SEM) values for the Self, Social and Environment factors and total score were 10.352, 9.526, 12.086 and 8.425, respectively. The small real differences (SRDs) for the Self, Social and Environment factors and total score were 28.675, 26.387, 33.478, and 23.337, respectively. The Pearson's correlation coefficients between the YQOL-W and the PedsQL4.0 General Core Scales were stronger between comparable dimensions than those between less comparable dimensions, demonstrating convergent and discriminant evidence of construct validity. Significant differences were found in subscale and total scores across weight status, age and genders (P<0.01), supporting the known-groups validity of the instrument. CONCLUSION:The Chinese version of the YQOL-W has acceptable measurement properties and can be used to assess the weight-specific QOL of children and adolescents in China.

Abstract:Objective: To evaluate the psychometric properties of the Union Physio-Psycho-Social Assessment Questionnaire (UPPSAQ-70) among general hospital psychiatric outpatients. Methods: A total of 2000 participants responded to the survey. Factor analyses were used to test the construct validity of the scale. Convergent validity was evaluated by the correlation between UPPSAQ-70 and symptoms measured using the Chinese versions of Self-Rating Anxiety Scale (SAS), Self-Rating Depression Scale (SDS), Patient Health Questionnaire-15 (PHQ-15), Somatic Symptom Disorder - B Criteria Scale (SSD-12) and Pittsburgh Sleep Quality Index (PSQI). Results: The nine-factor model was supported (χ2 = 8816.395, df = 2309, χ2/df = 3.818, RMSEA = 0.053, CFI = 0.929). The UPPSAQ-70 showed significant correlation with the SAS (r = 0.396, P < .001), SDS (r = 0.451, P < .001), PHQ-15 (r = 0.381, P < .001), SSD-12 (r = 0.324, P < .001) and PSQI (r = 0.220, P < .001). UPPSAQ-70 and its subscales showed good internal consistency with Cronbach's alpha coefficients ranging from 0.79 to 0.96. Conclusions: The UPPSAQ-70 was a rating scale with good construct validity and reliability, which can measure overall health in the biological, psychological, and social domains for Chinese psychiatric outpatients, but its convergent validity still requires further empirical research.
Measurement invariances of the PHQ‐9 across gender and age groups in Chinese adolescents

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Introduction</h3><p>The nine‐item Patient Health Questionnaire (PHQ‐9) is widely used to determine the severity of depression in adult populations, but its psychometric properties with regard to adolescents has been poorly explored. The present study aims to identify the factor structure and examine the measurement invariance of this instrument across genders and age groups in a Chinese adolescent sample.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>A large sample of Chinese schoolchildren completed the PHQ‐9 in a cross‐sectional survey in Hong Kong (N = 10 933). A confirmatory factor analysis (CFA) to test the factor structure and a multiple group CFA to test the gender and age invariances of the PHQ‐9 in adolescents were conducted. Cronbach alpha was used to assess the reliability of the questionnaire, and Pearson correlations with anxiety, self‐esteem, and perceived control were used to assess its construct validity.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>The CFA results indicate that a one‐factor model with three pairs of item correlations fitted the PHQ‐9 data well, and measurement invariances by age and gender were supported. The PHQ‐9 also possesses adequate internal consistency (>.84) and is strongly correlated with anxiety (>.77), self‐esteem (<−.57), and perceived control (<−.56) in the expected directions in the overall sample and in the gender and age subsamples.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Discussion</h3><p>The results support the claim that the PHQ‐9 is a reliable and valid scale and can be used to assess and compare depressive severity across ages and genders during the period of adolescence.</p></section>

Abstract:China Mental Health Survey (CMHS), which was carried out from July 2013 to March 2015, was the first national representative community survey of mental disorders and mental health services in China using computer-assisted personal interview (CAPI). Face-to-face interviews were finished in the homes of respondents who were selected from a nationally representative multi-stage disproportionate stratified sampling procedure. Sample selection was integrated with the National Chronic Disease and Risk Factor Surveillance Survey administered by the National Centre for Chronic and Non-communicable Disease Control and Prevention in 2013, which made it possible to obtain both physical and mental health information of Chinese community population. One-stage design of data collection was used in the CMHS to obtain the information of mental disorders, including mood disorders, anxiety disorders, and substance use disorders, while two-stage design was applied for schizophrenia and other psychotic disorders, and dementia. A total of 28,140 respondents finished the survey with 72.9% of the overall response rate. This paper describes the survey mode, fieldwork organization, procedures, and the sample design and weighting of the CMHS. Detailed information is presented on the establishment of a new payment scheme for interviewers, results of the quality control in both stages, and evaluations to the weighting.
Psychometric evaluation of the Mental Health Continuum-Short Form (MHC-SF) in Chinese adolescents – a methodological study

Abstract:BackgroundIn epidemiological surveillance of mental health there is good reason to also include scales that measure the presence of well-being rather than merely symptoms of ill health. The Mental Health Continuum-Short Form (MHC-SF) is a self-reported scale to measure emotional, psychological and social well-being and conduct categorical diagnosis of positive mental health. This particular instrument includes the three core components of the World Health Organization’s definition of mental health and had previously not been psychometrically evaluated on adolescents in China.MethodsIn total 5,399 students (51.1 % female) from schools in the urban areas of Weifang in China were included in the study (mean age = 15.13, SD = 1.56). Participants completed a comprehensive questionnaire with several scales, among them the MHC-SF. Statistical analyses to evaluate reliability, structural validity, measurement invariance, presence of floor and ceiling effects and to some extent external validity of the MHC-SF were carried out.ResultsThe Cronbach’s α coefficients for sub-scales as well as the total scale were all above 0.80 indicating good reliability. Confirmative factor analysis confirmed the three-dimensional structure of the Chinese version of MHC-SF and supported the configural and metric invariance across gender and age. Noteworthy ceiling effects were observed for single items and sub-scales although not for the total scale. More importantly, observed floor effects were negligible. The stronger correlation found between MHC-SF and Minneapolis-Manchester Quality of Life Instrument (as measure of positive mental health) than between MHC-SF and Hospital Anxiety Depression Scale (as measure of mental illness and distress) yielded support for external validity.ConclusionIn conclusion, the main findings of this study are in line with studies from other countries that evaluated the psychometric properties of the MHC-SF and show that this instrument, that includes the three core components of the WHO definition of mental health, is useful in assessing positive adolescent mental health also in China.

Abstract:Background: Young adults in Hong Kong are subject to elevated psychological distress given the societal stressors such as civil unrest and COVID-19 pandemic and suicide is a leading cause of death among them. The present study aimed to evaluate the psychometric properties and measurement invariance of the 4-item Patient Health Questionnaire-4 (PHQ-4) as a brief measure of psychological distress and its associations with meaning in life and suicidal ideation (SI) in young adults. Materials and methods: A mobile survey recruited a large and random sample of 1,472 young adults (Mean age = 26.3 years, 51.8% males) in Hong Kong in 2021. The participants completed the PHQ-4 and Meaning in Life Questionnaire–short form (MLQ-SF) for presence of meaning in life (MIL), SI, COVID-19 impact, and exposure to suicide. Confirmatory factor analysis was conducted to examine the factorial validity, reliability, and measurement invariance of PHQ-4 and MLQ-SF across gender, age, and distress subgroups. Multigroup structural equation model evaluated and compared the direct and indirect effects of latent MIL factor on SI via latent PHQ-4 factor across distress groups. Results: Both MIL and PHQ-4 supported a 1-factor model with good composite reliability (Ω = 0.80–0.86) and strong factor loadings (λ = 0.65–0.88). Both factors showed scalar invariance across gender, age, and distress groups. MIL showed significant and negative indirect effects ( αβ = −0.196, 95% CI = −0.254 to −0.144) on SI via PHQ-4. PHQ-4 showed a stronger mediating role between MIL and SI in the distress group (Δ = −0.146, 95% CI = −0.252 to −0.049) than the non-distress group. Higher MIL predicted higher likelihoods of help-seeking (Odds ratios = 1.46, 95% CI = 1.14–1.88). Conclusion: The present results support adequate psychometric properties in terms of factorial validity, reliability, convergent validity, and measurement invariance for the PHQ-4 in young adults in Hong Kong. The PHQ-4 demonstrated a substantial mediating role in the relationship between meaning in life and SI in the distress group. These findings support clinical relevance for using the PHQ-4 as a brief and valid measure of psychological distress in the Chinese context.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background</h3><p>Anhedonia, a key symptom of depression and schizophrenia, has emerged as a potential endophenotype. The aim of this study was to evaluate the psychometric properties of a Chinese version of the Snaith-Hamilton Pleasure Scale(SHAPS), a self-report anhedonia scale, in a non-clinical sample and clinical sample inclusive of major depressive disorder (MDD), schizophrenia, or a personality disorder.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>A total of 4,722 undergraduate students and 352 clinical patients participated in this study. Internal consistency was assessed by calculating Cronbach's α and mean inter-item correlation (MIC) values. Test-retest reliability and convergent validity was assessed with Pearson r coefficients. The best fitting of six potential factor-structure models was determined by confirmatory factor analysis (CFA). Measurement invariance across genders and samples was determined by multi-group CFA.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>Internal consistency of the Chinese version of SHAPS was acceptable in non-clinical (Cronbach's α = 0.90) and clinical (Cronbach's α = 0.91) samples. Four-week interval test-retest reliability was 0.60. Moreover, the Spanish four-factor structure had the best fit indexes in both samples. Scalar invariance was established across genders as well as across non-clinical sample and clinical sample. SHAPS was significantly related with the Temporal Experience of Pleasure Scale (TEPS) and Beck Depression Inventory (BDI).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Limitations</h3><p>There was a restricted scope of convergent validity and the size of clinical sample is relatively small, psychometric properties in elderly sample is also required.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>The Chinese version of SHAPS is a reliable, effective, simple and convenient tool for assessing and screening for anhedonia.</p>
