Abstract:BACKGROUND:Uterine artery pseudoaneurysm (UAP) is a rare but potentially life-threatening cause of hemorrhage. Nonetheless, its knowledge could be insufficient among obstetricians, gynecologists, and radiologists. We aimed to clarify the clinical characteristics, management, and outcomes of UAP.METHODS:We retrospectively analyzed nine female patients diagnosed with UAP at our institute between 2013 and 2020.RESULTS:Seven cases presented with a history of traumatic surgery including cesarean section, dilation and curettage, laparoscopic myomectomy, and cervical conization. Two cases occurred after spontaneous vaginal delivery and second-trimester pregnancy termination. The main symptom was heavy/massive/prolonged vaginal bleeding. All patients were first evaluated by color Doppler ultrasonography and three cases were confirmed by magnetic resonance imaging. Severn patients underwent transarterial embolization (TAE) of the uterine arteries, and two were managed conservatively. All patients had good outcomes.CONCLUSIONS:UAP can develop after traumatic pelvic operations and non-traumatic delivery/abortion. It may be more common than previously considered. The risk of rupture may be correlated with multiple factors other than the mass size. TAE of the uterine artery could be an effective management strategy for ruptured UAP. However, some cases can resolve spontaneously without TAE, suggesting that conservative management can be employed in some women.

Abstract:Objective: Our aim is to demonstrate a rare cause of hemoperitoneum without vaginal bleeding resulting from the rupture of a uterine artery pseudoaneurysm after uncomplicated vaginal delivery. Case report: A 39-year-old woman who had experienced a normal vaginal delivery 8 days previously to being seen in our hospital, was presented to the emergency room with hypovolemic shock. Computed tomography angiography (CTA) showed massive internal bleeding and a ruptured pseudoaneurysm arising from the left uterine artery. The patient was successfully treated through transcatheter arterial embolization (TAE). Conclusion: A pseudoaneurysm is a rare disease which can occur during an uncomplicated vaginal delivery. The clinical presentation can vary from asymptomatic, vaginal bleeding or hemoperitoneum. The diagnosis can be made by using Doppler sonography, CTA or Magnetic Resonance Imaging. The use of TAE is now the most common treatment option and possesses a high success rate.

Abstract:This study reported a case of spontaneous rupture of an ovarian artery aneurysm 4 days following cesarean section. The patient mainly presented with a consistent low-grade fever and did not have symptoms, such as severe abdominal or flank pain, as is common in other similar cases. Enhanced magnetic resonance imaging revealed a massive retroperitoneal hematoma; angiography confirmed that there was a rupture in an ovarian artery aneurysm. A selective embolization was performed successfully.

Abstract:Ovarian arterial haemorrhage (OAH), a rare cause of spontaneous retroperitoneal hematoma (SRH), usually occurs in women during pregnancy or in those with a history of repeated pregnancies. The clinical manifestations of OAH are non-specific; hence, proper and timely imaging examinations are extremely important. Contrast Enhanced CT scan is the first choice for clarifying the cause of haemorrhage in patients with SRH. Trans-arterial embolisation (TAE) has become the most common treatment option for OAH. Here, we report a case of SRH in the left ovarian artery region in a patient with a congenital solitary kidney. The patient was initially misdiagnosed as left kidney haemorrhage which was successfully embolised with tissue glue. There are no previous reports of OAH in patients with a congenital solitary kidney.

Abstract:Journal of International Medical Research, Volume 52, Issue 10, October 2024. Uterine rupture during pregnancy is a complication of placenta percreta. We present the case of a woman in her early 30s with a history of incomplete abortion treated by dilatation and curettage who was admitted with abdominal pain and vomiting at 30 weeks of gestation. She was diagnosed with thrombophilia and was administered anticoagulant drugs. After 10 hours of monitoring, the patient abruptly deteriorated. An emergency cesarean delivery showed a ruptured uterus due to placenta percreta. She accepted localized excision and uterine repair, and recovered well. Rupture of an unscarred uterus due to placental percreta is an extremely rare obstetric complication with high maternal and fetal mortality. This condition should be suspected in all pregnant women who have severe abdominal pain without being in labor. The treatment of uterine rupture due to placental percreta should be individualized, and repair of the uterus is possible in the majority of women.

Abstract:Uterine rupture (UR) during pregnancy is a life-threatening complication. Scarred UR is not difficult to diagnose. However, it is not easy to identify unscarred uterine rupture (UUR) because the risk factors are complex. According to literature review, the risk factors include high parity (≥4 births), uterine anomalies or diverticula, fetal macrosomia, multiple gestations, abnormal placentation, uterine adenomyosis, curettage, underlying connective tissue disorder and injudicious use of oxytocin. [ 1–11 ] Here, we present a rare case of UUR in a primiparous woman at 13 weeks of gestation after in vitro fertilization embryo transfer who was diagnosed with the disease of systemic lupus erythematosus (SLE).

Abstract:Rationale: Uterine rupture is an obstetrical emergency associated with severe maternal and fetal mortality. It is rare in the unscarred uterus of a primipara. Patient concerns: A 25-year-old woman in her 38th week of gestation presented with slight abdominal pain of sudden onset 10 hours before. An emergency cesarean section was done. After surgery, the patient and the infant survived. Diagnoses: With slight abdominal pain of clinical signs, ultrasound examination showed that the amniotic sac was found in the peritoneal cavity with a rupture of the uterine fundus. Interventions: Uterine repair and right salpingectomy. Outcomes: After surgery, the patient and the infant survived. The newborn weighed 2600 g and had an Apgar score of 10 points per minute. Forty-two days after delivery, the uterus recovered well. Lessons: Spontaneous uterine rupture should be considered in patients even without acute pain, regardless of gestational age, and pregnancy with abdominal cystic mass should consider the possibility of uterine rupture.

Abstract:The spontaneous rupture of an ovarian artery aneurysm (OAA) is an extremely uncommon and life-threatening event. Here, we describe the case of a 34-year-old G6P5015 female who underwent spontaneous vaginal delivery. Following delivery, she experienced hypotension and reported right-sided abdominal pain. A contrast-enhanced computed tomography (CT) angiogram revealed an aneurysmal dilation, extravasation, pseudoaneurysms, and a large retroperitoneal hematoma attributable to a rupture of the right ovarian artery. Subsequently, an exploratory laparotomy was performed, and then a transcatheter arterial embolization (TAE) by interventional radiology (IR). At a proximal site, IR successfully embolized both the ovarian and uterine arteries. This case highlights the significance of rapid intervention in managing an OAA. Additionally, we discuss the risk factors and treatment alternatives for OAA, underscoring the importance of considering it in the differential diagnosis when encountering atypical hypotension in the postpartum period.

Abstract:This paper reports the case of a spontaneous rupture of a non-scarring gravid uterus seen four days after vaginal delivery and provides an update on this rare pathology, which can be functionally and vitally life-threatening. Uterine rupture of a healthy gravid uterus can occur as a result of structural abnormalities of the uterine tissue framework or uterine parietal fragility due to pathological phenomena such as septic states. On admission, the clinical picture is generally that of an acute abdomen with a hypogastric origin, with or without hemodynamic instability and an altered general condition, depending on the presence of an underlying advanced uterine infection. Medical imaging, mainly ultrasound and CT scan with iodine contrast, enables visualization of the uterine breach and a precise assessment of the damage. Surgery is the treatment of choice for repairing the breach and ensuring hemostasis. This case study sheds light on this pathology, familiarizing us with its clinical and radiological picture, as well as its post-treatment prognosis.

Abstract:Abstract Background Pseudoaneurysms are formed when a local arterial wall ruptures, leading to hemorrhage and hematoma adjacent to the artery. Continuous perfusion of the injured artery increases the pressure in the lumen of the pseudoaneurysm. It may rupture and lead to massive hemorrhage that could be life-threatening. Cesarean scar pregnancy (CSP) is an ectopic pregnancy where the gestational sac is implanted in the cesarean scar. Uterine artery pseudoaneurysm (UAP) after CSP treatment is rare. Case presentation We report the case of a 36-year-old Chinese woman who presented with acute massive vaginal bleeding 53 days after transabdominal scar pregnancy excision. Doppler ultrasound confirmed UAP. Selective uterine artery embolization (UAE) failed because of the thin and curved blood vessels. The lesion decreased in size after transvaginal ultrasound-guided direct thrombin injection (UGTI); however, massive vaginal bleeding recurred and endangered the patient’s life. The uterus was removed thereafter. Conclusions UAP is a rare complication after CSP treatment that can lead to fatal massive hemorrhage. Ultrasound should be reexamined regularly after treatment of CSP. In case of unexplained vaginal bleeding, we should be alert to the existence of UAP and the possibility of rupture and take effective diagnosis and treatment measures promptly.

Abstract:Objective: To report the clinical presentation and treatment of spontaneous rupture of uterine surface veins during late pregnancy after in vitro fertilization (IVF).Design: Case report.Setting: Medical Unit of Third Hospital of Peking University, Beijing, China.Patient(s): A total of 573 pregnancies resulting from IVF in our hospital were reviewed for examples of spontaneous rupture of uterine veins.Intervention(s): Data were obtained from medical charts, delivery summaries, and surgical reports in the historical files.Main Outcome Measure(s): Explorative laparotomy was performed.Result(s): Three women were found experiencing significant intra-abdominal bleeding in the third trimester of pregnancy due to a lesion resulting from endometriosis and chronic pelvic inflammatory disease. The intra-abdominal bleeding occurred between 29 and 35 weeks of gestation. A major prenatal symptom was severe abdominal pain in all of the cases. The pain was not relieved by the application of tocolytics or mild analgesics. Explorative laparotomy, performed in all three cases, revealed the presence of massive intra-abdominal bleeding stemming from subserous uterine veins. Fetal death occurred in one case. Immediate institution of effective resuscitative measures and early surgical intervention were essential to both fetal and maternal survival.Conclusion(s): Patients undergoing IVF treatment may be at increasing risk for intra-abdominal bleeding in the third trimester of pregnancy. Progression of Uterine Artery Pseudoaneurysm Documented by Ultrasonography: A Case Report

Abstract:Introduction: Uterine artery pseudoaneurysm (UAP) is a rare cause of late postpartum hemorrhage. Insufficient understanding of this condition among clinicians may result in delayed diagnosis and treatment, potentially leading to incorrect interventions and poor prognosis, including fatal hemorrhage and even necessitating hysterectomy in severe cases. Case report: The patient, a 41-year-old woman with a history of three pregnancies and two deliveries, underwent cesarean section and subsequently experienced persistent small amounts of vaginal bleeding for a duration of two months. Transvaginal ultrasonography revealed a hypoechoic mass in the cervix that was initially misdiagnosed as a cervical fibroid. Approximately 12 h prior to admission, she experienced an episode of acute vaginal bleeding of significant intensity. Emergency transvaginal ultrasound demonstrated an intrauterine mass located in the posterior wall of the cervix with swirling blood flow, exhibiting a to-and-fro pattern. The mass was connected to the left uterine artery adjacent to the cervix through a tear measuring approximately 0.5 cm in diameter. Emergency bilateral uterine artery embolization was performed. After a follow-up period of ten months, there was no recurrence of abnormal vaginal bleeding, and subsequent ultrasound examination confirmed the complete resolution of the cervical lesions. Conclusion: The findings of this case suggest that the UAP undergoes a dynamic process. In the early stages, the lesion may manifest as a small hypoechoic or anechoic area within the myometrium. Color Doppler imaging might not reveal blood flow signals within the lesion, potentially leading to misdiagnosis as other common uterine lesions such as fibroids or cysts. However, considering the close association between UAP and the uterine artery, meticulous observation of the relationship between the uterine artery and its branches is crucial for identifying myometrial lesions to facilitate early detection of UAP and minimize misdiagnosis.
Acute Fetal Hemorrhagic Shock Due to Umbilical Cord Rupture in a Term Pregnancy With Single Umbilical Artery and Velamentous Cord Insertion: A Case Report and Literature Review

Abstract:Umbilical cord rupture, though rare, is a severe obstetric complication with significant implications for neonatal morbidity and mortality. We present the case of a 38-year-old primiparous female diagnosed with a single umbilical artery (SUA) and velamentous cord insertion (VCI) in late pregnancy. At 40 weeks of gestation, during labor induction, the patient suddenly experienced massive vaginal bleeding and fetal bradycardia, necessitating an emergency cesarean section. Postoperatively, it was confirmed that the umbilical cord had ruptured. The neonate required immediate and intensive resuscitation, including blood transfusion and therapeutic hypothermia. Remarkably, despite the critical initial condition, the neonate exhibited no neurological deficits and was discharged in stable condition on the 27th day. The presence of SUA and VCI likely increased the vulnerability of the umbilical cord, predisposing it to rupture. This case emphasizes the importance of prenatal ultrasound in detecting umbilical cord abnormalities such as SUA and VCI. The early detection of these abnormalities allows for proactive management, including closer monitoring and timely surgical intervention, which are crucial for optimizing neonatal outcomes. This report provides valuable insights into the pathophysiology and management of umbilical cord rupture.

Abstract:Uterine rupture is a rare but severe obstetric complication that significantly affects maternal and perinatal outcomes. It can occur due to various factors, with uterine scarring from cesarean sections and uterine-related surgeries being the most common causes. Ruptures during pregnancy are more critical than those during delivery, necessitating increased physician attention. Despite increased awareness of uterine rupture prevention after cesarean sections, prevention strategies for pregnancies following hysteroscopy remain insufficient. Therefore, this paper retrospectively examines 2 cases of spontaneous uterine rupture during pregnancy after hysteroscopy procedures at our hospital over the past 2 years.

Internal iliac artery balloon occlusion as a hemostatic method for spontaneous rupture of vulvar hematoma during delivery: A case report

Abstract:Spontaneous rupture of a vulvar hematoma during delivery is a relatively uncommon event and may cause excessive hemorrhage. Exact identification of anatomic structures and bleeding points is challenging. We herein present a case involving a pregnant woman at 39 weeks' gestation with a large vulvar hematoma that spontaneously ruptured during the second stage of labor, likely due to rupture of varices in the vulva or vagina. It was difficult to accurately expose and suture the deep bleeding points. The estimated blood loss volume was 1591 mL, and the hemoglobin concentration dropped from 132 g/L before delivery to 84 g/L after delivery. To prevent hemorrhagic shock, bilateral internal iliac artery balloon occlusion was performed and proved to be an effective way to achieve hemostasis. Once hemostasis was established, ligation of the bleeding sites and suturing of all dead space were rapidly completed. Bilateral internal iliac artery balloon occlusion can be used as an effective treatment for excessive vaginal bleeding. The presence of varices or hemangiomas in the vulva or vagina should be carefully checked during antenatal care.

Abstract:Ectopic pregnancies are a frequently encountered cause of first-trimester metrorrhagia. They occur when an embryo is implanted and grows outside the normal uterine space. Uncommonly, the embryo can be implanted in the intramural portion of the uterine tube, a condition referred to as interstitial localization. This specific type of ectopic pregnancy may have an unpredictable course, potentially leading to severe uterine rupture and catastrophic bleeding if not promptly diagnosed and managed. We present a rare case of a multiparous 36-year-old female patient who underwent pelvic ultrasonography in the emergency department for intense pelvic pain associated with hypotension and moderate anemia. A history of right salpingectomy for a ruptured tubal ectopic pregnancy 10 years previously was noted. High beta-HCG levels were also detected. A pelvic ultrasound allowed us to suspect a ruptured ectopic interstitial pregnancy at 8 weeks of amenorrhea. An association with hemoperitoneum was suspected, and an emergency laparoscopy was performed. The condition was confirmed intraoperatively, and the patient underwent a right corneal wedge resection with suture of the uterine myometrium. The postoperative course was uneventful, and the patient was discharged on the fourth day postoperatively. Interstitial ectopic pregnancy is a rare yet extremely perilous situation. Timely ultrasound-based diagnosis is crucial as it can enable conservative management with Methotrexate. Delayed diagnosis can lead to uterine rupture with consecutive surgery based on a transection of the pregnancy and cornual uterine resection.

Abstract:Uterine arteriovenous fistula (UAVF) is a rare but serious cause of heavy uterine bleeding that can lead to significant morbidity due to abnormal connections between the uterine arterial and venous supply. [ 1 , 2 ] UAVF was first described by Dubreuil and Loubat in 1926. [ 3 ] The incidence of UAVF after delivery or abortion is 0.63%. [ 4 ] UAVF usually manifests as heavy or irregular vaginal bleeding. A massive sudden gush of vaginal blood is typical after miscarriage, uterine surgery, or cesarean section. [ 5 ] Management options include observation, medical treatment, uterine artery embolization (UAE), resection, hysterectomy, or combinations of these. [ 6 , 7 ]

Abstract:Postpartum hemorrhage (PPH), a leading cause of maternal mortality, can occur within 24 h of delivery (primary PPH), or during the period from 24 h after delivery to Week 6 of puerperium (secondary PPH). It requires health professionals to be alert to the symptoms to ensure prompt diagnosis and treatment, especially in the case of rupture of a uterine artery pseudoaneurysm (UAP) due to its life-threatening consequence (Baba et al., 2014). Most of the published case reports or case serials describe UAP as a possible cause of delayed PPH after traumatic procedures during delivery or pregnancy termination, including cesarean section (CS), manual removal of the placenta, or dilation and curettage (D&C) (Wald, 2003). Herein, we report a case of prior CS-related UAP manifesting as primary PPH after an uncomplicated vaginal delivery. This case required emergency embolization and is notable for several reasons. Antepartum hemorrhage of the previously scarred uterus was a potential sign of the ruptured UAP, and color Doppler sonography sometimes deceived the physician as the characteristic features of UAP did not appear to be present.

Abstract:Objective Umbilical cord vascular rupture is a rare and severe condition that can occur during labor, leading to adverse outcomes for the fetus before as well as after delivery. Prompt diagnosis and intervention are crucial for improving the chances of a successful outcome. We aimed to analyze cases of umbilical cord vascular rupture during labor to provide insights into this challenging condition. Study Design This retrospective study evaluated the medical records of patients diagnosed with umbilical cord vessel rupture or umbilical cord hematoma at Fujian Maternity and Child Health Hospital from January 1, 2015, to May 31, 2023. The inclusion criteria included gestational age of ≥28 weeks, occurrence during labor, and availability of complete delivery data. Data on fetal heart rate (FHR) changes, delivery intervals, intraoperative findings, placental pathology, and neonatal outcomes were collected and analyzed. Results A total of 12 cases were analyzed. The incidence of umbilical cord vascular rupture during childbirth was 0.08%. The FHR patterns in umbilical cord rupture during delivery included baseline tachycardia, minimal or absent variability, variable or late deceleration, prolonged deceleration, and undetectable heart rate. The bradycardia-to-delivery interval (BDI) ranged from 6 to 26 minutes. Among the 12 neonates, 9 were discharged well, 2 were stillbirths, and there was 1 neonatal death. Hemorrhagic shock was common in live births. Conclusion Our study highlights the significance of continuous FHR monitoring during labor and the urgent need for medical teams to respond quickly in cases of umbilical cord vascular rupture. Despite advancements in neonatal resuscitation techniques, managing cases with undetectable fetal heart activity remains clinically challenging, and even with immediate pregnancy termination, poor neonatal outcomes may still occur. Key Points Umbilical cord vascular rupture during labor is a rare event. Its clinical management presents significant challenges. Advances in neonatal resuscitation have improved rescue success rates. In such cases, hemorrhagic shock is common in live births.
