Abstract:Objective Umbilical cord vascular rupture is a rare and severe condition that can occur during labor, leading to adverse outcomes for the fetus before as well as after delivery. Prompt diagnosis and intervention are crucial for improving the chances of a successful outcome. We aimed to analyze cases of umbilical cord vascular rupture during labor to provide insights into this challenging condition. Study Design This retrospective study evaluated the medical records of patients diagnosed with umbilical cord vessel rupture or umbilical cord hematoma at Fujian Maternity and Child Health Hospital from January 1, 2015, to May 31, 2023. The inclusion criteria included gestational age of ≥28 weeks, occurrence during labor, and availability of complete delivery data. Data on fetal heart rate (FHR) changes, delivery intervals, intraoperative findings, placental pathology, and neonatal outcomes were collected and analyzed. Results A total of 12 cases were analyzed. The incidence of umbilical cord vascular rupture during childbirth was 0.08%. The FHR patterns in umbilical cord rupture during delivery included baseline tachycardia, minimal or absent variability, variable or late deceleration, prolonged deceleration, and undetectable heart rate. The bradycardia-to-delivery interval (BDI) ranged from 6 to 26 minutes. Among the 12 neonates, 9 were discharged well, 2 were stillbirths, and there was 1 neonatal death. Hemorrhagic shock was common in live births. Conclusion Our study highlights the significance of continuous FHR monitoring during labor and the urgent need for medical teams to respond quickly in cases of umbilical cord vascular rupture. Despite advancements in neonatal resuscitation techniques, managing cases with undetectable fetal heart activity remains clinically challenging, and even with immediate pregnancy termination, poor neonatal outcomes may still occur. Key Points Umbilical cord vascular rupture during labor is a rare event. Its clinical management presents significant challenges. Advances in neonatal resuscitation have improved rescue success rates. In such cases, hemorrhagic shock is common in live births. Ethics approval for this study was granted by the Ethics Committee of Fujian Maternity and Child Health Hospital (approval no.: 2024KY053). Umbilical Artery Thrombosis Causing Fetal Distress: A Case Report

Abstract:The umbilical cord (UC) is vital to maintain blood circulation between the mother and the growing fetus, which is sometimes disrupted. The umbilical artery thrombosis (UAT) is an infrequent complication of pregnancy that can lead to extreme perinatal outcomes, ranging from intrauterine growth restriction stillbirth to neonatal death. The prenatal diagnosis of UAT is essential and sometimes challenging to detect in clinical practice. Once it is detected, the emergent delivery through a cesarean section is considered after the steroidal lung maturity of the fetus. We report a primigravida diagnosed with this rare pregnancy complication, the UAT at delivery, along with the nuchal cord and abnormally coiled UC. The patient had an uneventful course of pregnancy except for the premature rupture of membranes and continuous fetal distress in the second stage of labor. As the labor progression was optimal, and prioritizing the patient's desire, she was vigilantly observed under the premise of continuous electronic fetal monitoring (EFM) to facilitate any emergency, ultimately resulting in the spontaneous vaginal delivery of an alive and healthy baby boy. The fetal distress detected through EFM is an indicator of several stressors predisposing the fetus to some unknown danger that carries an increased risk of perinatal mortality. Based on our experience, it is suggested that radiologists should routinely conduct UC sonographic studies on regular antenatal scans; obstetricians should also have a brief and precise awareness of the critical lifesaving sonographic parameters to measure. The UAT, nuchal cord, and abnormal UC coiling, as found in our case, are all rare factors and related to some extent of fetal morbidity and mortality; once such complications are prenatally suspected, one should manage it through close monitoring and timely decision of appropriate delivery time.
Diagnosis and Treatment of Uterine Artery Pseudoaneurysm: Case Series and Literature Review.

Abstract:BACKGROUND:Uterine artery pseudoaneurysm (UAP) is a rare but potentially life-threatening cause of hemorrhage. Nonetheless, its knowledge could be insufficient among obstetricians, gynecologists, and radiologists. We aimed to clarify the clinical characteristics, management, and outcomes of UAP.METHODS:We retrospectively analyzed nine female patients diagnosed with UAP at our institute between 2013 and 2020.RESULTS:Seven cases presented with a history of traumatic surgery including cesarean section, dilation and curettage, laparoscopic myomectomy, and cervical conization. Two cases occurred after spontaneous vaginal delivery and second-trimester pregnancy termination. The main symptom was heavy/massive/prolonged vaginal bleeding. All patients were first evaluated by color Doppler ultrasonography and three cases were confirmed by magnetic resonance imaging. Severn patients underwent transarterial embolization (TAE) of the uterine arteries, and two were managed conservatively. All patients had good outcomes.CONCLUSIONS:UAP can develop after traumatic pelvic operations and non-traumatic delivery/abortion. It may be more common than previously considered. The risk of rupture may be correlated with multiple factors other than the mass size. TAE of the uterine artery could be an effective management strategy for ruptured UAP. However, some cases can resolve spontaneously without TAE, suggesting that conservative management can be employed in some women.

Abstract:ObjectiveTo discuss the clinical diagnosis and therapy for umbilical cord cyst in China. MethodsOne case was selected from the files of the Women's Hospital School of Medicine Zhejiang University.And others had been diagnosised and reported in China from 1979 to 2007 in Chinese literature.All were analyzed retrospectively.ResultsIn China 19 cases reported,The mean size of the cyst was 5.4 cm(range 2.0~12.0 cm),4 of 13 patients(4/13) were associated with poor pregnancy outcome(abnormality or stillbirth),when the umbilical cord cyst present single.Oppositely,4 of 6(4/6) multiple umbilical cord cysts were associated with poor pregnancy outcome.ConclusionsThe pathophysiological process of umbilical cord cyst is unclear,may be relating to the fetal abnormality or aneuploidy.The best way to diagnosis this illness is ultrasound,and the umbilical cord cyst should be differentiated to hematoma and umbilical vein varix.There was no standard treatment of this entity.we should take detail examination of the fetus and scan the cyst repeatly when the umbilical cord cyst was detected.

Abstract:Furcate cord insertion refers to the separation of umbilical vessels before reaching the placenta, where the branching vessels normally attach at the edge of the placental parenchyma or near the placental membranes. This is an extremely rare abnormal umbilical cord insertion. This paper reported a case of a furcate cord insertion, where the rupture of exposed umbilical vessels led to intrauterine fetal death at full term. Through literature review, we analyzed the prenatal ultrasound characteristics and pregnancy outcomes of furcate cord insertions, with the aim to improve detection rates and reduce the risk of adverse pregnancy outcomes.
How to substantially increase recruitment in cancer trials in New Zealand.

Abstract:Recruitment rates into cancer treatment trials are generally very low, both in New Zealand and internationally. This viewpoint article suggests that recruitment rates could be substantially increased by considering all patients newly diagnosed with cancer to be automatically eligible for randomisation if experimental treatments were available under study protocols for patients with their type of cancer. Patients randomised to be offered the experimental treatment would be approached for consent to receive it, whereas patients randomised not to be offered this treatment would continue to receive standard treatment (thus serving as the control group) and not be approached for consent. Routine adoption of this approach, known as "post-randomisation consent" or "pre-randomisation", would require public consultation and "societal consent". While this proposal is not without significant challenges and potential disadvantages, an informed public discussion on the subject would seem worthwhile given the potential for increasing patient access to new cancer treatments and advancing medical science.

Abstract:Rationale: Uterine rupture is an obstetrical emergency associated with severe maternal and fetal mortality. It is rare in the unscarred uterus of a primipara. Patient concerns: A 25-year-old woman in her 38th week of gestation presented with slight abdominal pain of sudden onset 10 hours before. An emergency cesarean section was done. After surgery, the patient and the infant survived. Diagnoses: With slight abdominal pain of clinical signs, ultrasound examination showed that the amniotic sac was found in the peritoneal cavity with a rupture of the uterine fundus. Interventions: Uterine repair and right salpingectomy. Outcomes: After surgery, the patient and the infant survived. The newborn weighed 2600 g and had an Apgar score of 10 points per minute. Forty-two days after delivery, the uterus recovered well. Lessons: Spontaneous uterine rupture should be considered in patients even without acute pain, regardless of gestational age, and pregnancy with abdominal cystic mass should consider the possibility of uterine rupture.

Abstract:Pediatric and Developmental Pathology, Ahead of Print. Umbilical cord hemangiomas are rare lesions, for which data on pregnancy outcome is lacking. This study combines a multi-institution 4-case series with a systematic literature search (n = 52) to determine possible pathologic lesion parameters which may have an effect on pregnancy outcome. Of all 56 pregnancies, lesion size ranged from 0.2 to 23.0 cm with pregnancy outcomes ranging from healthy liveborns (58.9%), liveborns with severe complications largely due to prematurity and/or fluid overload (12.5%), intrauterine/neonatal demise (25.0%), and pregnancy termination (3.6%). Of the 52 cases included for statistical analysis, there was no significant association between fetal outcome and vascular lesion location (P = .12) or fetal outcome and single umbilical artery involvement versus involvement of other vasculature (P = .29). The mean length of vascular lesions that resulted in healthy liveborns did not significantly differ from those resulting in severe fetal complications and/or demise (P = .72). Cases resulting in severe complications and/or demise were significantly earlier at delivery than those resulting in healthy liveborns (P < .001). Combined findings suggest that functional lesion characteristics, such as the degree of turbulent flow generated, have more significance than size, especially in early gestation losses. Moving forward, standardized reporting of pathologic lesion characteristics is paramount to better predict pregnancy prognosis.

Abstract:Introduction Uterine rupture represents one of the most severe obstetric affections. It is defined as a complete or a partial tearing of the uterine wall. Women with a prior cesarean section are reported to have a higher risk of having this situation. Moreover, maternal death and most of all middle- and long-term adverse consequences remain a great preoccupation. On another scale, neonatal death and ulterior deterioration remain very high, especially in low-income countries. Case Description A 24-year-old woman with a history of previous cesarean section presented at 35 weeks of gestation with pelvic pain without bleeding. Emergency cesarean section revealed a complete uterine rupture at the scar site from the previous cesarean section. Remarkably, the fetus managed to seal the rupture using the right temporal region, forearm, and right leg, avoiding significant complications. The mother had an uncomplicated postoperative course and was discharged after 48 hours of surveillance. Conclusion We present with this case an extraordinary case of a uterine rupture where both mother and child had a good outcome. This rare evolution was reported only one time in literature. For this reason, a history of caesarean delivery might present a huge challenge for obstetricians and neonates.

Abstract:Numerous permanent sequelae can result from infections caused by bacteria, rubella virus, herpes simplex virus, or Toxoplasma during pregnancy, including neurological defects, fetal growth restriction, and hearing loss. [ 1 ] The umbilical cord, serving as the sole connection between a mother and child, experiences a sequence of pathological alterations after fetal bacterial and viral infections. Among these changes, the umbilical artery wall sustains damage and ruptures; adhesion and fusion processes then lead to the formation of a distinctive variant known as a single umbilical artery. This variant is fundamentally different from single umbilical arteries caused by abnormal embryonic development. Not only does the umbilical artery supply oxygen and nutrients to the fetus, but it also acts as a transport channel for bacteria and viruses. Fetal growth and development can be adversely affected by changes in umbilical artery structure or the transport of harmful substances, leading to increased risks of fetal growth restriction and preterm birth. This study presents a rare case report of combined intrauterine bacterial and viral infections that led to the convergence of 2 umbilical arteries into a single umbilical artery at the free segment of the umbilical cord on the fetal side. Due to the premature rupture of the membranes at 31 weeks and 3 days gestation, the weight of the newborn was 13.8th percentile (−1.1 standard deviation). Follow-up at 1 month, 6 months, and 1 year after birth revealed that the height and weight of the infant were significantly lower than those of other babies the same age, and her responses to sound and light were slightly delayed. The above situation, as well as changes in umbilical cord structure, may be related to intrauterine infection with Toxoplasma gondii , herpes virus, and rubella virus.

Successful management of fetal hypoxia due to amniotic banding at 26 weeks of pregnancy: A case report of a rare survival

Abstract:Background: Amniotic banding is a rare condition that can lead to structural limb anomalies, fetal distress and adverse obstetric outcomes. The main hypothesis for its etiology is a rupture of the amniotic membrane in early pregnancy, with the formation of tightly entangling strands around the fetus. These strands can constrict, incise, and subsequently amputate limb parts, the neck or head. More rarely, the amniotic banding can affect the umbilical cord, leading to fetal distress or potential intra-uterine fetal demise. Objective: We present a unique case of a 26-week pregnant woman who attended a polyclinical consultation due to reduced fetal movements with concerning cardiotocography (CTG) findings. A review of the literature about amniotic banding of the umbilical cord was conducted as well, identifying diagnostic and interventional options for the obstetrician's practice. Study design: This is a case report, alongside a review of the literature. Results: The CTG indicated fetal distress, prompting an emergency caesarean section (C-section). Upon delivery, the neonate exhibited signs of amniotic band sequence, with distal phalangeal defects on the right hand and severe constriction of the umbilical cord caused by amniotic strands, the latter precipitating fetal hypoxia. Direct ultrasound diagnosis remains a challenge in the absence of limb amputation, yet indirect signs such as distal limb or umbilical doppler flow abnormalities and distal limb edema may be suggestive of amniotic banding. MRI is proposed as an adjuvant diagnostic tool yet does not present a higher detection rate compared to ultrasound. Fetoscopic surgery to perform lysis of the amniotic strands with favorable outcome has been described in literature. Conclusion: This case presents the first reported survival of an extremely preterm fetus in hypoxic distress as a cause of amniotic banding of the umbilical cord, with a rare degree of incidental timing. Ultrasound diagnosis remains the gold standard. Obstetrical vigilance is warranted, with fetal rescue proven to be feasible.
Umbilical cord entanglement in monochorionic monoamniotic twin pregnancy-case presentation and literature review

Abstract:Objective To conclude the perinatal outcomes and management of umbilical cord entanglement in monochorionic monoamniotic twin pregnancy with cord entanglement .Methods Two cases of monochorionic monoamniotic twin pregnancy with cord entanglement in Peking University First Hospital from 2013 to 2014 were reviewed .Literature review was also made to emphasis the theoretical background of this field .Results Two cases were diagnosed by ultrasound detection during 11~12 weeks pregnancy and were followed up with close fetal monitoring .In case 1, entanglement of the umbilical cords was not diagnosed until 37 weeks at elective cesarean section .In case 2, cord entanglement was detected from 21 weeks’ gestation and confirmed by cesarean section at 29 weeks gestation with suspected fetal distress .All the twins have since developed uneventfully .Conclusion Elective cesarean delivery at 32~34 weeks’ gestation after a prophylactic course of corticosteroids is most commonly recommended in monoamniotic twins with umbilical cord entanglement .However , clinical management should also be based on specific situation for individual cases with the emphasis of close fetal testing and ultrasound detection .

Abstract:Fused umblical arteries (FUAs) is a condition that warrants pathological confirmation following evaluation of multiple sections of the umbilical cord. The presence of FUAs may have an impact on fetal development. This report details the case of a fetus with fused umbilical arteries (FUAs), which exhibited the presence of a single umbilical artery near the site of placental insertion, as well as normal two umbilical arteries at the fetal end of the umbilical cord. In our case, as the pregnancy progressed, there was an occurrence of late‐onset fetal growth restriction and oligohydramnios. This is a possibility that FUAs might impact the development of the fetus.

Abstract:Spontaneous complete chorioamniotic membrane separation (cCMS) is rare and associated with adverse perinatal outcomes. We present a case of spontaneous cCMS in a fetus with ileal atresia and umbilical cord. When cCMS is evident in late preterm infants, the fetus should be carefully monitored and considered for delivery. Spontaneous complete chorioamniotic membrane separation (cCMS) is rare and associated with adverse perinatal outcomes, such as intrauterine fetal death. Herein, we present a case of spontaneous cCMS in a fetus with ileal atresia and umbilical cord ulcers. In our case, we detected spontaneous cCMS on ultrasonography at 35 weeks of gestation. On the same day, a female infant weighing 2134 g was delivered via cesarean section. Pathological examination of the placenta confirmed cCMS and umbilical cord ulcers. When cCMS is evident in late preterm infants, the fetus should be carefully monitored and considered for delivery.

Abstract:Transverse uterine fundal cesarean section in cases of total placenta previa reduces blood loss, but its influence on subsequent pregnancies, including the uterine rupture risk, remains unclear. We report a case of uterine rupture due to placenta percreta in the first trimester in a 43-year-old woman who underwent transverse uterine fundal incision in a previous pregnancy (at 40 years old). The patient did not undergo assessment of the uterine scare after the previous operation. Oocyte donation and in vitro fertilization at another institution resulted in the current pregnancy. At 11 weeks 3 days, she was admitted to the emergency department because of sudden severe abdominal pain. Ultrasound showed massive accumulation of free fluid in the peritoneal cavity and the fetus was outside the uterine cavity; uterine rupture was diagnosed. During emergency laparotomy, the uterine rupture was detected at exactly the previous incision site; a total hysterectomy was performed. Pregnancy after a transverse uterine fundal cesarean section is at high risk. As uterine scar dehiscence might have caused the uterine rupture, wounds should be evaluated before allowing subsequent pregnancies.

Abstract:Our review of the medical literature confirm the ethiopathogenetic role and involvement of placental and umbilical cord pathology in a wide variety of obstetrics diseases that may jeopardize the fetal well‐being. Methods Multicenter case series of women undergoing routine and extended prenatal ultrasound and perinatal obstetric care. Results Prenatal ultrasound findings, perinatal care, and pathology documentation in cases of placental pathology are presented. Conclusions Our case series review and that of the medical literature confirms the ethiopathogenetic role and involvement of placenta abnormalities in a wide variety of obstetrics diseases that may jeopardize the fetal well‐being. Some of these specific pathologies are strongly associated with a high risk of poor perinatal outcome.
