Abstract:Introduction Uterine rupture represents one of the most severe obstetric affections. It is defined as a complete or a partial tearing of the uterine wall. Women with a prior cesarean section are reported to have a higher risk of having this situation. Moreover, maternal death and most of all middle- and long-term adverse consequences remain a great preoccupation. On another scale, neonatal death and ulterior deterioration remain very high, especially in low-income countries. Case Description A 24-year-old woman with a history of previous cesarean section presented at 35 weeks of gestation with pelvic pain without bleeding. Emergency cesarean section revealed a complete uterine rupture at the scar site from the previous cesarean section. Remarkably, the fetus managed to seal the rupture using the right temporal region, forearm, and right leg, avoiding significant complications. The mother had an uncomplicated postoperative course and was discharged after 48 hours of surveillance. Conclusion We present with this case an extraordinary case of a uterine rupture where both mother and child had a good outcome. This rare evolution was reported only one time in literature. For this reason, a history of caesarean delivery might present a huge challenge for obstetricians and neonates.

Abstract:Journal of International Medical Research, Volume 52, Issue 10, October 2024. Uterine rupture during pregnancy is a complication of placenta percreta. We present the case of a woman in her early 30s with a history of incomplete abortion treated by dilatation and curettage who was admitted with abdominal pain and vomiting at 30 weeks of gestation. She was diagnosed with thrombophilia and was administered anticoagulant drugs. After 10 hours of monitoring, the patient abruptly deteriorated. An emergency cesarean delivery showed a ruptured uterus due to placenta percreta. She accepted localized excision and uterine repair, and recovered well. Rupture of an unscarred uterus due to placental percreta is an extremely rare obstetric complication with high maternal and fetal mortality. This condition should be suspected in all pregnant women who have severe abdominal pain without being in labor. The treatment of uterine rupture due to placental percreta should be individualized, and repair of the uterus is possible in the majority of women.

Abstract:Rationale: Uterine rupture is an obstetrical emergency associated with severe maternal and fetal mortality. It is rare in the unscarred uterus of a primipara. Patient concerns: A 25-year-old woman in her 38th week of gestation presented with slight abdominal pain of sudden onset 10 hours before. An emergency cesarean section was done. After surgery, the patient and the infant survived. Diagnoses: With slight abdominal pain of clinical signs, ultrasound examination showed that the amniotic sac was found in the peritoneal cavity with a rupture of the uterine fundus. Interventions: Uterine repair and right salpingectomy. Outcomes: After surgery, the patient and the infant survived. The newborn weighed 2600 g and had an Apgar score of 10 points per minute. Forty-two days after delivery, the uterus recovered well. Lessons: Spontaneous uterine rupture should be considered in patients even without acute pain, regardless of gestational age, and pregnancy with abdominal cystic mass should consider the possibility of uterine rupture.

Abstract:Objective: To report the clinical presentation and treatment of spontaneous rupture of uterine surface veins during late pregnancy after in vitro fertilization (IVF).Design: Case report.Setting: Medical Unit of Third Hospital of Peking University, Beijing, China.Patient(s): A total of 573 pregnancies resulting from IVF in our hospital were reviewed for examples of spontaneous rupture of uterine veins.Intervention(s): Data were obtained from medical charts, delivery summaries, and surgical reports in the historical files.Main Outcome Measure(s): Explorative laparotomy was performed.Result(s): Three women were found experiencing significant intra-abdominal bleeding in the third trimester of pregnancy due to a lesion resulting from endometriosis and chronic pelvic inflammatory disease. The intra-abdominal bleeding occurred between 29 and 35 weeks of gestation. A major prenatal symptom was severe abdominal pain in all of the cases. The pain was not relieved by the application of tocolytics or mild analgesics. Explorative laparotomy, performed in all three cases, revealed the presence of massive intra-abdominal bleeding stemming from subserous uterine veins. Fetal death occurred in one case. Immediate institution of effective resuscitative measures and early surgical intervention were essential to both fetal and maternal survival.Conclusion(s): Patients undergoing IVF treatment may be at increasing risk for intra-abdominal bleeding in the third trimester of pregnancy. Abstract:Objective To assess the frequency of uterine ruptures, clinical characteristics, and maternal and neonatal outcomes in a tertiary referral center. Methods Information on complete uterine rupture between July 2010 and June 2022 was investigated retrospectively at a tertiary center. Results There were 42 cases of complete uterine rupture in 144 474 deliveries, with an incidence rate of 0.029%. Twenty‐seven cases had a scarred uterus and 15 had an unscarred uterus; Rupture of the lower uterine segment was predominant in the scarred uterus, whereas rupture of the body of the uterus was predominant in the non‐scarred uterus (P ≤ 0.001). Newborns with Apgar score of 7 or less at 1 min in the non‐scarred uterus group was more than that in the scarred uterus group (P = 0.001). There were no significant differences in the history of gynecologic surgery, induction of labor, mode of delivery, clinical features, maternal outcomes, neonatal weight, preterm birth rate, 5‐min Apgar score, or neonatal mortality between the two groups (P > 0.05). Conclusion The clinical manifestations of uterine rupture are mainly abdominal pain, abnormal fetal heartbeat, or vaginal bleeding. Attention should also be paid to the history of previous uterine surgery. Strict prenatal management, early identification, and aggressive management can help improve maternal and child outcomes. Hysterectomy is not imperative.

Abstract:The influence of aspartate on lithium transport was studied with humna red blood cells (rbc) in vitro and after i.p. injection of Li-DL-asp or LiCl in different tissues of the rat. After administration of Li-asp the lithium concentrations in brain and rbc of the rats increased more slowly compared to rat treated 3 to 6 days with Li-asp were more than two times higher than those of rats treated with LiCl. After daily injection of the two salts, lithium levels in the brains of rats teated with LiCl. Results of in vitro experiments with human rbc indicate that the effect of aspartate on lithium transport is specific for L-aspartate. In addition a similar but smaller effect has been observed with l-glutamate.

Abstract:This study reported a case of spontaneous rupture of an ovarian artery aneurysm 4 days following cesarean section. The patient mainly presented with a consistent low-grade fever and did not have symptoms, such as severe abdominal or flank pain, as is common in other similar cases. Enhanced magnetic resonance imaging revealed a massive retroperitoneal hematoma; angiography confirmed that there was a rupture in an ovarian artery aneurysm. A selective embolization was performed successfully.

Abstract:Uterine rupture (UR) during pregnancy is a life-threatening complication. Scarred UR is not difficult to diagnose. However, it is not easy to identify unscarred uterine rupture (UUR) because the risk factors are complex. According to literature review, the risk factors include high parity (≥4 births), uterine anomalies or diverticula, fetal macrosomia, multiple gestations, abnormal placentation, uterine adenomyosis, curettage, underlying connective tissue disorder and injudicious use of oxytocin. [ 1–11 ] Here, we present a rare case of UUR in a primiparous woman at 13 weeks of gestation after in vitro fertilization embryo transfer who was diagnosed with the disease of systemic lupus erythematosus (SLE).

Abstract:Spontaneous splenic rupture during pregnancy can be catastrophic with most of the cases needing splenectomy. We presence an interesting case of conservatively managed spontaneous splenic rupture in pregnancy. A 24-year-old G2P1L1 with 35 weeks gestation with previous LSCS severe preeclampsia and severe anaemia was referred from a peripheral centre. On examination, she was vitally stable with marked pallor and pedal oedema. Abdominal wall oedema and ascites was present, uterus was 28 weeks with FHS localised by Doppler. After 2 hours, patient complained of continuous pain abdomen. It was tense, tender with FHS non-localised, uterine fundus could not be made out. USG revealed a retroplacental hematoma of 3.5×2.2 cm with free fluid in the abdomen. Differential diagnosis of rupture uterus and abruptio placenta were made. Emergency laparotomy was done. Intraoperatively, 800 cc fresh hemoperitoneum was present, previous scar was intact and a live baby delivered by LSCS. Uterus, bilateral tubes and ovaries were normal. After the uterus was closed, fresh intraabdominal bleeding was still present. With the help of general surgeon, exploration was done and two long superficial splenic tears (grade 2 splenic injury) with continuous oozing were identified. Contact pressure was applied by gauze. Bleeding stopped and omental wrapping around spleen was done. Abdomen was closed after putting drain and patient was shifted to ICU for monitoring. After 27 days, patient was discharged in a stable condition. Thus, spontaneous splenic rupture should be considered in the diagnostic differential of hemodynamic instability in a case of severe preeclampsia. Emergency laparotomy before the setting of collapse and DIC are vital steps to save the spleen and improve feto-maternal survival.

Abstract:Abstract Background Retroperitoneal hematoma after vaginal delivery is rare but can lead to maternal morbidity and mortality. Diagnosis of this condition is challenging due to its complexity and its nonspecific signs and symptoms. To date, studies and case reports regarding retroperitoneal hematoma are few, particularly in low-income countries where risk factors for this condition may be more prevalent and the prognosis poorer. Case presentation We report the case of a 37-year-old multiparous african (Congolese) woman who presented to the emergency department of a large referral hospital in Bukavu, Democratic Republic of the Congo (DRC), 2 weeks after a spontaneous nontraumatic vaginal delivery. She had abdominal pain that began immediately after delivery and progressed throughout the postpartum period. The patient had anemia, hypotension, tachycardia, and a left costo-lumbar arch distorting the body shape on a soft and depressed abdomen. She had visited a private clinic on days 3 and 7 postpartum; however, signs and symptoms of retroperitoneal hematoma went unrecognized. Using abdominal ultrasound, we diagnosed an extensive hematoma in the retroperitoneal space from the left iliac fossa to the left flank. Laparotomy was performed to evacuate the hematoma, and the patient recovered. Conclusion Retroperitoneal hematoma following a nontraumatic vaginal delivery is an unusual situation in general obstetrical practice. The knowledge of this potentially life-threatening condition in resource-limited settings enables effective diagnosis and management by ultrasound and laparotomy.

Abstract:Uterine rupture can lead to significant morbidity and mortality in both women and newborns. Most uterine ruptures occur in pregnant women. [ 1 ] This condition manifests in 2 primary ways: uterine dehiscence and uterine rupture. Uterine dehiscence is characterized by incomplete division of the uterus that does not penetrate all 3 layers: the endometrium, myometrium, and perimetrium. Often, uterine dehiscence remains unnoticed and discovered incidentally in asymptomatic patients who exhibit stable fetal heart rate tracing. In contrast, uterine rupture involves complete division of all 3 layers of the uterus. Recently, there has been a growing incidence of uterine rupture driven by aspiration to extend the option of a trial of labor after cesarean section to a broader range of patients. The rupture can come to light during cesarean delivery or arouse suspicion through obstetric ultrasound. Regularly evaluating the scar in the lower uterine segment of women with a history of prior cesarean section using accessible ultrasound resources could prove to be beneficial. [ 2 ] Uterine rupture can sometimes occur silently without any apparent signs, which is an uncommon event. The accidental discovery of such silent ruptures has rarely been documented.

Abstract:OBJECTIVE:To investigate the incidence, etiology, diagnosis, treatment and outcome of uterus rupture.METHODS:From January 1999 to May 2013, clinical data of 67 cases with uterine rupture in Woman's Hospital, School of Medicine, Zhejiang University were studied retrospectively.RESULTS:A total of 67 cases of uterine rupture with 21(+2)-39(+2) gestational weeks out of 128 599 deliveries were recorded giving an incidence of uterine rupture was 0.052 1% (67/128 599) . Cesarean scar rupture were found in 59 cases (88%, 59/67) and noncesarean scar rupture were found in 8 cases (12%, 8/67). The causes of uterine rupture include 60 cases of scar uterus (59 cesarean scar cases and 1 myomyectomy scar case), 2 cases of assisted delivery operation trauma, 2 cases of malformed uterus, 3 cases of unknown causes (all with artificial abortion history).Non obstructive dystocia and improper oxytocin use were found to be related with uterine rupture.Fifty-two cases of cesarean scar spontaneous incomplete rupture were found and repaired during repeated cesarean delivery without maternal and fetal complications. The remaining 15 cases need emergency rescue operation for fetal distress or dead fetus, severe acute abdomen, prepartum or postpartum vaginal bleeding even maternal hypovolemia; 6/15 uterine rupture cases were diagnosed with the history, clinical symptoms and signs, 3/15 cases with ultrasonic found dead fetus in the peritoneal cavity before exploratory laparotomy and 6 cases were diagnosed just during laparotomy.Hysterectomy was done in 10/15 cases and uterine repair in 5/15 cases; there was no maternal death and 12 perinatal fetal death (5 cases of mid-late pregnancy termination for deformed fetus) of the 15 uterine rupture cases.One case with hysterectomy was complicated with stress pancreatitis and dysfunction of liver and kidney and discharged 20 days after operation, the remaining 14 cases were discharged 5-7 days postpartum.One case with repaired malformed uterus got pregnancy 4 years later and delivered a 2 000 g healthy baby by cesarean section at gestational age of 33(+4) weeks.CONCLUSION:Uterine scar caused by caesarean section or other operations became the leading cause of uterine rupture, assisted delivery operations, history of intrauterine manipulation and uterine malformations were the predisposing risk factors of uterine rupture.

Abstract:INTRODUCTION Static bone histomorphometry was applied to existing iliac bone sections originating from a 370-d 5 degrees head-down bed rest experiment. This bed rest experiment is the longest ever to have been conducted. We hypothesized that bed rest would decrease cancellous bone volume fraction and that this effect would be reversed by countermeasures. METHODS Eight healthy male subjects underwent 370 d of 5 degrees head-down bed rest. Three subjects were treated with bisphosphonate (Xidifon, potassium salt of ethane-1-hydroxy-1-disphosphonate, EHDP) combined with an exercise regimen (1-2 h x d(-1)) for the entire study period. Five subjects underwent 120 d of bed rest without countermeasures followed by 250 d of bed rest with the exercise regimen. Transiliac bone biopsies were obtained either at baseline and day 366, or at baseline, day 116, and day 366 at alternating sides of the ileum. Static histomorphometry was performed using a computerized method. RESULTS The 120 d of head-down bed rest without countermeasures resulted in decreased bone volume fraction BV/TV (-6.3%, p = 0.046) and trabecular number (Tb.N; -10.2%, p = 0.080) and increased trabecular separation (Tb.Sp; 14.7%, p = 0.020), whereas the 250 d of subsequent head-down bed rest with exercise treatment prevented further significant deterioration of the histomorphometric measures. DISCUSSION The 120 d of 5 degrees head-down bed rest without countermeasures induced significant deterioration of iliac crest cancellous bone histomorphometric properties. On average, the countermeasures consisting of either bisphosphonate and exercise, or exercise alone were able to either prevent or stop immobilization-induced changes of the iliac cancellous bone structure.

Abstract:A silent rupture of an unscarred uterus is a rare phenomenon. Accidental diagnosis of silent rupture during sterilization procedure in a previous vaginal delivery is rarely reported. We present a case of uterine rupture in an unscarred uterus in a 40-year-old gravida 10 para 9 with intrauterine fetal demise terminated with prostaglandin E2. She was asymptomatic and hemodynamically stable. Hemoperitoneum was observed during a tubal ligation procedure performed on the third day after the abortion. A right-sided broad ligament hematoma was noticed, and surgical treatment was initiated when the patient's condition clinically deteriorated during the operation. Our article aims to raise obstetricians' awareness of an important causative factor of hemoperitoneum found during postpartum tubal ligation surgery.

Abstract:The first-trimester rupture of a bicornuate uterus (BU) is a rare obstetrical emergency, especially following previous normal vaginal deliveries where it is often misdiagnosed. A 24-year-old G3P2002 woman presented at 11 weeks of gestation with sudden onset of severe left iliac fossa pain without other symptoms. On examination, she was fully conscious and hemodynamically unstable with signs of peritoneal irritation, a distended pouch of Douglas and a slightly enlarged uterus and a tender left adnexal mass. The diagnosis of a ruptured ectopic pregnancy was made and a laparotomy was done. Intra-operative findings were hemoperitoneum, a left ruptured BU and a dead fetus. Surgical management entailed hysterorrarphy, left salpingectomy and conservation of both ovaries. Her postoperative course was uneventful and future fertility was preserved. We recommend a high index of suspicion of ruptured BU as a differential diagnosis of acute abdomen in the first trimester in women with previous term vaginal deliveries.
