Abstract:Purpose: To assess the dosimetric effectiveness of image-guided radiation treatment (IGRT) and online adaptive radiation therapy (oART) for cervical cancer. As well as survival follow-up was conducted to validated the safety and efficacy of oART. Methods: A total of 15 cervical cancer patients were enrolled. oART was performed on a CT-integrated linear accelerator. The initial plan was revised to include the distribution of IGRT dose using daily fan-beam CT (FBCT) images, after which the distinctions between ART and IGRT in terms of target coverage and organs at risk (OARs) sparing were analyzed. Survival follow-up was conducted to validated the safety and efficacy of oART in this group. Results: PTV Dmax value decreased by 1.23 Gy in the ART plan when compared to that in the IGRT plan; PTV D95 increased by 1.34 Gy; PTV V50 coverage increased by 4.86%; CTV coverage increased by 3.02%; PTV D2cc of the colon, rectum, and small intestine decreased by 1.24 Gy, 1.29 Gy, and 1.12 Gy, respectively. The V10 and V30 of the pelvis increased by 1.27% and 0.56%, respectively, while the V30 of the left and right femoral heads dropped by 2.82% and 3.41%, respectively. Except for the pelvic changes, all other differences were statistically significant (p < 0.01). The average time for the ART procedure was 21.22 min (range: 18.72–24.90 min). The median follow-up time is 28.0 months. Median event-free survival and overall survival were not reached. EFS rate and OS rate at 3 years were 79.4% and 92.9%. Conclusion: Online ART for cervical cancer can minimize the dose of OARs and enhance the target volume coverage significantly when compared to IGRT with satisfied survival time.

Synthetic CT generation from cone-beam CT using deep-learning for breast adaptive radiotherapy

Abstract:We investigated the feasibility of the generation of synthetic CT (sCT) from CBCT images with deep learning and the dose evaluation for CBCT-guided breast cancer adaptive radiotherapy. A total of sixty-eight patients receiving radiotherapy after breast-conserving surgery were retrospectively included in this study. We compared the performance of three deep-learning methods in generating sCT from CBCT, including U-Net, Cycle generative adversarial network (CycleGAN) and pix2pix. The original treatment plan was transferred to sCT keeping the same parameters. The dosimetric evaluation was performed by a quick dose recalculation on sCT based on gamma analysis. The U-Net model obtained the lowest mean absolute error (MAE) within the body, clinical target volume (CTV) and organs at risk (OARs), with 62.53 +/- 9.14 HU within the body, 35.99 +/- 6.32 HU within tumor bed, and 30.15 +/- 6.36 HU within CTV. In terms of dose comparison, the gamma pass rates under 3%/3 mm and 2%/2 mm criteria were 91.40 +/- 3.52% and 85.95 +/- 4.75% for the U-Net model, whereas 89.50 +/- 3.46% and 83.65 +/- 4.00% for the pix2pix model, and 89.84 +/- 3.47% and 83.69 +/- 4.28% for the CycleGAN model, respectively. The sCT images generated by the U-Net model can provide higher image similarity and dosimetric accuracy than those generated by the pix2pix and CycleGAN models. The approach could be used to realize accurate dose calculation for breast cancer adaptive radiotherapy based on CBCT.
Daily Online Adaptive Radiation Therapy of Postoperative Endometrial and Cervical Cancer With PTV Margin Reduction to 5 mm: Dosimetric Outcomes, Acute Toxicity, and First Clinical Experience

Abstract:Purpose: This study evaluated the first clinical implementation of daily iterative cone beam computed tomography (iCBCT)-guided online adaptive radiation therapy (oART) in the postoperative treatment of endometrial and cervical cancer. Methods and materials: Seventeen consecutive patients treated with daily iCBCT-guided oART were enrolled in this prospective study, with a reduced uniform 3-dimensional PTV margin of 5 mm. Treatment plans were designed to deliver 45 or 50.4 Gy in 1.8 Gy daily fractions to PTV. Pre- and posttreatment ultrasound and iCBCT scans were performed to record intrafractional bladder and rectal volume changes. The accuracy of contouring, oART procedure time, dosimetric outcomes, and acute toxicity were evaluated. Results: The average time from first iCBCT acquisition to completion of treatment was 22 minutes and 26 seconds. During this period, bladder volume increased by 44 cm3 using iCBCT contouring, whereas rectal volume remained stable (62.9 cm3 pretreatment vs 61.9 cm3 posttreatment). A total of 91.6% of influencers and 88.1% of CTVs required no or minor edits. The adapted plan was selected in all (434) fractions and significantly improved the dosimetry coverage for CTV and PTV, especially the vaginal PTV coverage by nearly 7% (P < .05). The adapted bladder Dmean was 104.61 cGy, and the rectum Dmean was 123.67 cGy, significantly lower than the scheduled plan of 108.24 and 128.19 cGy, respectively. The bone marrow and femur head left and right dosimetry were also improved with adaptation. Grade 2 acute gastrointestinal and genitourinary toxicities were 24% and 0, respectively. There was a grade 3 acute toxicity of decreased white blood cell count in 1 patient. Conclusions: Daily oART was associated with favorable dosimetry improvement and low acute toxicity, supporting its safety and efficacy for postoperative treatment of endometrial and cervical cancer. These results need to be validated in a larger prospective randomized controlled cohort.
First Application of Demand-Triggered Online Adaptive Radiotherapy in the Treatment of Cervical Cancer: A Clinical Report

Abstract:Gynecology cancers can reap significant benefits from adaptive radiation therapy (ART) for four major reasons: organ motion, organ deformation, density change, and cavity filling. There are three recognized types of adaptive radiotherapy: offline, online, and real-time. This balance of improved dosimetry versus clinic resources, as well as the optimal timing for adaptations, is still under investigation. The emergence of on-demand online adaptive radiotherapy (OART) can solve the above problems. In this context, we introduce two patients with cervical cancer who used on-demand OART for the first time. One patient with cervical cancer received radical radiotherapy on the United Imaging uCT-ART platform, and another patient with cervical cancer received postoperative adjuvant radiotherapy. The radiotherapy process used OART, which was triggered by senior radiotherapists, assisted by artificial intelligence, and guided by fan-beam computer tomography. Patient 1, who was 54 years old with cervical squamous cell carcinoma, International Federation of Gynecology and Obstetrics (FIGO) stage ⅢC1, underwent radical concurrent chemoradiotherapy. The target volume was reduced in the late stage of radiotherapy. The target volume coverage of the OART plan was better, and the bladder and rectum doses were lower than those of the image-guided radiotherapy plan. Patient 2, who was 56 years old with cervical adenocarcinoma, FIGO stage ⅡA1, underwent postoperative concurrent chemoradiotherapy. If the fractionated treatment during radiotherapy was carried out according to the original plan, treatment off-target would occur, while the OART plan could ensure target coverage. The acute toxic reactions that occurred in both patients during radiotherapy were patient-reported outcome Common Terminology Criteria for Adverse Events 1-2, and no toxic reactions of grade 3 or above occurred. This is the first description of the successful implementation of the uCT-ART-based OART system in EBRT for cervical cancer.

Abstract:Intensity-modulated radiation therapy (IMRT) improves tumor control and reduces long-term radiation-induced complications of patients with nasopharyngeal carcinoma (NPC), contingent upon accurate contouring and precise delivery of treatment plans. Online adaptive radiotherapy (ART) involves real-time treatment plan modification based on the variations in targets and organs at risk (OARs) to uphold treatment planning accuracy. This study describes the first reported case of fan beam computed tomography (FBCT)-guided online ART for NPC using a novel integrated platform. Online ART was performed at the 25th fraction in this case, as tumors and the patient's anatomy were observed to regress inter-fractionally, necessitating adjustments to the contours based on the anatomy of the day. Online ART plan optimized target volume coverage while reducing doses to OARs. Notably, online ART significantly improved radiotherapy efficiency. This patient achieved a clinical complete response 12 weeks post-treatment, with Epstein-Barr virus DNA levels reduced to 0 copies/ml. Currently, the patient is alive without evidence of high-grade toxicity or local recurrence at approximately 10 months post-treatment. This case confirms the feasibility and dosimetric benefit of online ART for NPC using a novel integrated platform. Further research is needed to confirm its clinical benefits.
Forecasting patient-specific dosimetric benefit from daily online adaptive radiotherapy for cervical cancer

Abstract:Objective: Adaptive Radiotherapy (ART) is an emerging technique for treating cancer patients which facilitates higher delivery accuracy and has the potential to reduce toxicity. However, ART is also resource-intensive, requiring extra human and machine time compared to standard treatment methods. In this analysis, we sought to predict the subset of node-negative cervical cancer patients with the greatest benefit from ART, so resources might be properly allocated to the highest-yield patients. Approach: CT images, initial plan data, and on-treatment Cone-Beam CT (CBCT) images for 20 retrospective cervical cancer patients were used to simulate doses from daily non-adaptive and adaptive techniques. We evaluated the correlation (R 2 ) between dose and volume metrics from initial treatment plans and the dosimetric benefits to the Bowel V 40Gy , Bowel V 45Gy , Bladder D mean , and Rectum D mean from adaptive radiotherapy using reduced 3mm or 5mm CTV-to-PTV margins. The LASSO technique was used to identify the most predictive metrics for Bowel V 40Gy . The three highest performing metrics were used to build multivariate models with leave-one-out validation for Bowel V 40Gy . Main Results: Patients with higher initial bowel doses were correlated with the largest decreases in Bowel V 40Gy from daily adaptation (linear best fit R 2 =0.77 for a 3mm PTV margin and R 2 =0.8 for a 5mm PTV margin). Other metrics had intermediate or no correlation. Selected covariates for the multivariate model were differences in the initial Bowel V 40Gy and Bladder D mean using standard versus reduced margins and the initial bladder volume. Leave-one-out validation had an R 2 of 0.66 between predicted and true adaptive Bowel V 40Gy benefits for both margins. Significance: The resulting models could be used to prospectively triage cervical cancer patients on or off daily adaptation to optimally manage clinical resources. Additionally, this work presents a critical foundation for predicting benefits from daily adaptation that can be extended to other patient cohorts.

Abstract:Background Lung cancer poses a significant global health challenge. Adaptive radiotherapy (ART) addresses uncertainties due to lung tumor dynamics. We aimed to investigate a comprehensively and systematically validated offline ART regimen with high clinical feasibility for lung cancer. Methods This study enrolled 102 lung cancer patients, who underwent kV iterative cone‐beam computed tomography (iCBCT). Data collection included iCBCT and planning CT (pCT) scans. Among these, data from 70 patients were employed for training the UNet++ based deep learning model, while 15 patients were allocated for testing the model. The model transformed iCBCT into adaptive CT (aCT). Clinical radiotherapy feasibility was verified in 17 patients. The dosimetric evaluation encompassed GTV, organs at risk (OARs), and monitor units (MU), while delivery accuracy was validated using ArcCHECK and thermoluminescent dosimeter (TLD) detectors. Results The UNet++ based deep learning model substantially improved image quality, reducing mean absolute error (MAE) by 70.05%, increasing peak signal‐to‐noise ratio (PSNR) by 17.97%, structural similarity (SSIM) by 7.41%, and the Hounsfield Units (HU) of aCT approaching a closer proximity to pCT compared to kV iCBCT. There were no significant differences observed in the dosimetric parameters of GTV and OARs between the aCT and pCT plans, confirming the accuracy of the dose maps in ART plans. Similarly, MU manifested no notable disparities, underscoring the consistency in treatment efficiency. Gamma passing rates for intensity‐modulated radiation therapy (IMRT) and volumetric‐modulated arc therapy (VMAT) plans derived from aCT and pCT exceeded 98%, while the deviations in TLD measurements (within 2% to 7%) also exhibited no significant differences, thus corroborating the precision of dose delivery. Conclusion An offline ART regimen utilizing kV iCBCT and UNet++ based deep learning model is clinically feasible for lung cancer treatment. This approach provides enhanced image quality, comparable treatment plans to pCT, and precise dose delivery.

Abstract:Definitive treatment of lung cancer with radiotherapy is challenging, as respiratory motion and anatomical changes can increase the risk of severe off-target effects during radiotherapy. Online adaptive radiotherapy (ART) is an evolving approach that enables timely modification of a treatment plan during the interfraction of radiotherapy, in response to physiologic or anatomic variations, aiming to improve the dose distribution for precise targeting and delivery in lung cancer patients. The effectiveness of online ART depends on the seamless integration of multiple components: sufficient quality of linear accelerator-integrated imaging guidance, deformable image registration, automatic recontouring, and efficient quality assurance and workflow. This review summarizes the present status of online ART for lung cancer, including key technologies, as well as the challenges and areas of active research in this field.

Abstract:Stereotactic radiation therapy (SBRT) has emerged as a promising treatment modality for locally advanced pancreatic cancer. The aim of this study is to assess the dosimetric efficacy of online adaptive radiotherapy (ART) in comparison to image-guided radiation therapy (IGRT) for pancreatic cancer. We conducted a retrospective analysis involving 8 patients diagnosed with locally advanced pancreatic cancer. The gross tumor volume (GTV) delineates the visible extent of the tumor on imaging, while the planning tumor volume (PTV) was generated by expanding 5 mm from the GTV and ensuring a 3 mm distance from the small intestine, duodenum, and stomach simultaneously. Treatment planning was executed using the United Imaging Healthcare Treatment Planning System workstation. The control group underwent evaluation based on daily validated fan-beam CT (FBCT) scans, assessing both the dose delivered to actual organs at risk (OARs) and the target volume. Radiotherapy plans were developed utilizing simulation CT, and conventional radiotherapy with daily image-guided radiation therapy (IGRT) was administered using FBCT-Linac. Conversely, patients in the study group received daily validated FBCT-guided adaptive radiotherapy plans, with a focus on mean dose assessment of both the target volume and OARs. Subsequently, we compared the average outcomes of each treatment fraction between IGRT and online adaptive radiotherapy (ART). Comparison between ART and IGRT treatment plans revealed significant differences in various dosimetric parameters: For PTV: V98%: ART (96.28%) vs IGRT (89.73%), p = 0.000, V95%: ART (96.28%) vs IGRT (89.73%), p = 0.031, V90%: ART (98.58%) vs IGRT (93.65%), p = 0.000, Dmean: ART (4912.91) vs IGRT (4804.11), p = 0.000. This approach may offer a more favorable safety profile while concurrently enhancing treatment efficacy.

Abstract:Online adaptive radiotherapy (oART) dose calculation relies on synthetic computed tomography (sCT), which notably influences anatomical changes. This study elucidates how sCT may respond to significant inter-fractional tumor volume reduction and its subsequent impact on dose distribution. In this case report, we exported sCT and cone-beam CT (CBCT) images from each treatment session. We retrospectively analyzed 20 adaptive and scheduled plans of a patient receiving oART for large pleural metastases with notable inter-fractional tumor regression. By overriding the CT number of the dissipated tumor volume with that of the lungs on each sCT, we recalculated each plan. We compared the dose distribution between the adaptive and scheduled plans. Percentage dose difference and 3D gamma analysis were employed to assess dose variability. Results of the dose analysis showed that, compared to the online (non-overridden) plans, the recalculated plans using overridden sCT demonstrated right-shifted dose-volume histogram curves for the targets and right lung, with a slight but statistically significant increase of no less than 1.5% in D mean and D max for the targets and right lung. The location of hotspots shifted in alignment with tumor shrinkage and beam arrangement. Both recalculated adaptive and scheduled plans achieved ideal GTV, CTV, and PTV coverage, with adaptive plans significantly reducing the dose and irradiated volume to the right lung. In conclusion, as the pleural tumor volume decreased, online plans slightly underestimated the dose distribution and shifted the location of hotspots, though this remained clinically acceptable. Importantly, adaptive plans significantly minimized the irradiated volume of the critical OAR (right lung) while ensuring optimal dose coverage of the target volume, demonstrating the potential of sCT and adaptive oART to enhance treatment precision and efficacy in dynamically changing tumor environments.
Moderated Hypofractionated Online Adaptive Radiotherapy in Locally Advanced Cervical Cancer: A Case Report

Abstract:Cervical cancer is one of the most frequent malignant tumors in females. Concurrent chemoradiotherapy is one of the treatment options for cervical cancer. The treatment time of conventional radiotherapy is long. Moderately hypofractionated radiotherapy (MHRT) offers the advantage of shortening the overall treatment duration and enhancing the radiobiological effects on tumors. MHRT shortens the overall treatment duration while enhancing the radiobiological effects on tumors. Previous studies have reported that MHRT of cervical cancer has relatively high toxicity. Daily online adaptive radiation therapy (oART) showed improvements in dosimetry and a decrease in toxicity. To the best of our knowledge, this case was the first reported case of moderated hypofractionated oART used in a cervical cancer patient to date in a prospective clinical trial (NCT05994300). This case serves as a critical reminder that cervical cancer is a potential tumor that may be in MHRT with iterative cone beam computed tomography-guided oART. Further data are needed to confirm the toxicity and efficacy of this technique.

Abstract:This study explores the dosimetric benefits of cone-beam computed tomography (CBCT)-based online adaptive radiation therapy (oART) for a non-small-cell lung cancer (NSCLC) patient exhibiting significant tumor shrinkage during ChemoRT. The patient was prescribed 60 Gray (Gy) in 30 fractions and was initially treated with conventional RT. After the delivery of the first four treatment fractions, the patient's treatment course was converted to oART due to tumor shrinkage seen on CBCT. Current oART dose calculations use a synthetic CT (sCT) image derived from deformable image registration (DIR) of the planning CT to the daily CBCT, and, as the tumor regressed, the discrepancy between the CBCT and the sCT increased, leading to a re-simulation after the delivery of the ninth fraction. In this case report, we first investigated dosimetric differences leveraged by converting this patient from conventional RT to oART. With oART using sCT, the patient's target coverage remained consistent with the reference plan while simultaneously changing lung V20 by 7.8 ± 1.4% and heart mean by 3.4 ± 1.5 Gy. Then, using this new simulation CT and comparing it with iterative CBCT (iCBCT) images acquired with the new HyperSight™ (HS) (Varian Medical Systems, Inc., Palo Alto, CA, USA) imaging system on the Ethos, we investigated the impact of direct dose calculation on HS-iCBCT as compared to sCT. The HS-iCBCT generated a dose distribution similar to the CT reference, achieving a 96.01% gamma passing rate using Task Group-218 (TG-218) criteria. Results indicate that HS-iCBCT has the potential to better reflect daily anatomical changes, resulting in improved dosimetric accuracy. This study highlights the advantages of oART in the presence of tumor response to therapy and underscores HS-iCBCT's potential to provide CT-level dose calculation accuracy in oART for NSCLC patients.

Abstract:To introduce the initial clinical experiences of online Kilovoltage Cone-beam CT (KVCBCT) guided lung cancer radiation in our department. From March 2007 to January 2008, 30 patients with pathologically confirmed lung cancer were included in this study. After patients were positioned using their skin marks, KVCBCT scans were performed before treatment and aligned to planning CT scans using the self-developed semi-automatic alignment method. Treatment table were repositioned according to the alignment results. A second KVCBCT scan was acquired immediately after the table correction. A total of 308 KVCBCT images from 154 fractions were analyzed. The reproducibility of the semi-automatic image alignment method, workflow of the online correction, translational and rotational setup error in three dimensional conditions and residual setup error after online correction were evaluated. The reproducibility of the semi-automatic alignment method was evaluated. The alignment difference was smaller in intraobserver evaluation than in interobserver. The reproducibility of different physicians was better than the physician and radiation therapist in anterior-posterior (AP) and left-right (LR) directions. In superior-inferior (SI) direction, the percentage of alignment difference larger than 3 mm between different physicians, the physician and radiation therapist were both near 15%. The online correction protocol adds 7.6 min to the total treatment time and the workflow proved to be feasible in clinical practice. The total systematic error (∑) before online correction was 3.5 mm, 3.5 mm and 3.0 mm, the overall random error (σ) was 2.7 mm, 2.9 mm and 3.0 mm in the LR, SI, and AP direction, respectively. After online correction, the total systematic error (∑) was 0.7 mm, 0.7 mm and 0.6 mm, the overall random error (σ) was 1.2 mm, 1.5 mm and 1.3 mm in three directions. The average rotational setup error (± 1 SD) was -0.3 ± 1.5°, 0.2 ± 0.9 ° and 0.3 ± 0.8 ° in pitch, roll and yaw, respectively. According to the formalisms suggested by van Herk et al, 3mm can apply to the margin of clinical target volume (CTV) to PTV, when using the online correction. The KVCBCT guided radiation system can be applied successfully in lung cancer patients with reproducible image alignment method. With the help of the system, we have an opportunity of new realization for patient setup error, including both translational and rotational. We can improve patient setup accuracy and reduce the CTV to PTV margin from 11 mm to 3 mm with limited workload.
A Clinically Relevant Online Patient QA Solution with Daily CT Scans and EPID-basedin Vivodosimetry: a Feasibility Study on Rectal Cancer.

Abstract:Objective.Adaptive radiation therapy (ART) could protect organs at risk (OARs) while maintain high dose coverage to targets. However, there is still a lack of efficient online patient quality assurance (QA) methods, which is an obstacle to large-scale adoption of ART. We aim to develop a clinically relevant online patient QA solution for ART using daily CT scans and EPID-basedin vivodosimetry.Approach.Ten patients with rectal cancer at our center were included. Patients' daily CT scans and portal images were collected to generate reconstructed 3D dose distributions. Contours of targets and OARs were recontoured on these daily CT scans by a clinician or an auto-segmentation algorithm, then dose-volume indices were calculated, and the percent deviation of these indices to their original plans were determined. This deviation was regarded as the metric for clinically relevant patient QA. The tolerance level was obtained using a 95% confidence interval of the QA metric distribution. These deviations could be further divided into anatomically relevant or delivery relevant indicators for error source analysis. Finally, our QA solution was validated on an additional six clinical patients.Main results.In rectal cancer, the 95% confidence intervals of the QA metric for PTV ΔD95(%) were [-3.11%, 2.35%], and for PTV ΔD2(%) were [-0.78%, 3.23%]. In validation, 68% for PTV ΔD95(%), and 79% for PTV ΔD2(%) of the 28 fractions are within tolerances of the QA metrics. one patient's dosimetric impact of anatomical variations during treatment were observed through the source of error analysis.Significance.The online patient QA solution using daily CT scans and EPID-basedin vivodosimetry is clinically feasible. Source of error analysis has the potential for distinguishing sources of error and guiding ART for future treatments.

Abstract:This article presents a case of a patient with advanced head and neck cancer, characterized by a large and protruding tumor. The patient was treated with an innovative on-demand online adaptive radiotherapy (ART) technology, guided by cone beam computed tomography (CBCT), on the Ethos adaptive radiotherapy platform (version 1.0, Varian Medical Systems, Palo Alto, CA). A solution was provided for this special case to address the issue where part of the target volume could not participate in the optimization due to exceeding the external contour boundary during online adaptive radiotherapy. The treatment outcome was satisfactory in terms of tumor regression, while only grade 1 radiodermatitis and grade 2 oral mucositis at the end of radiotherapy. This article discusses the clinical diagnosis, treatment process, and follow-up of this case, aiming to provide clinical references for a broader application of this technology.
