Abstract:Purpose: To assess the dosimetric effectiveness of image-guided radiation treatment (IGRT) and online adaptive radiation therapy (oART) for cervical cancer. As well as survival follow-up was conducted to validated the safety and efficacy of oART. Methods: A total of 15 cervical cancer patients were enrolled. oART was performed on a CT-integrated linear accelerator. The initial plan was revised to include the distribution of IGRT dose using daily fan-beam CT (FBCT) images, after which the distinctions between ART and IGRT in terms of target coverage and organs at risk (OARs) sparing were analyzed. Survival follow-up was conducted to validated the safety and efficacy of oART in this group. Results: PTV Dmax value decreased by 1.23 Gy in the ART plan when compared to that in the IGRT plan; PTV D95 increased by 1.34 Gy; PTV V50 coverage increased by 4.86%; CTV coverage increased by 3.02%; PTV D2cc of the colon, rectum, and small intestine decreased by 1.24 Gy, 1.29 Gy, and 1.12 Gy, respectively. The V10 and V30 of the pelvis increased by 1.27% and 0.56%, respectively, while the V30 of the left and right femoral heads dropped by 2.82% and 3.41%, respectively. Except for the pelvic changes, all other differences were statistically significant (p < 0.01). The average time for the ART procedure was 21.22 min (range: 18.72–24.90 min). The median follow-up time is 28.0 months. Median event-free survival and overall survival were not reached. EFS rate and OS rate at 3 years were 79.4% and 92.9%. Conclusion: Online ART for cervical cancer can minimize the dose of OARs and enhance the target volume coverage significantly when compared to IGRT with satisfied survival time.

Daily Online Adaptive Radiation Therapy of Postoperative Endometrial and Cervical Cancer With PTV Margin Reduction to 5 mm: Dosimetric Outcomes, Acute Toxicity, and First Clinical Experience

Abstract:Purpose: This study evaluated the first clinical implementation of daily iterative cone beam computed tomography (iCBCT)-guided online adaptive radiation therapy (oART) in the postoperative treatment of endometrial and cervical cancer. Methods and materials: Seventeen consecutive patients treated with daily iCBCT-guided oART were enrolled in this prospective study, with a reduced uniform 3-dimensional PTV margin of 5 mm. Treatment plans were designed to deliver 45 or 50.4 Gy in 1.8 Gy daily fractions to PTV. Pre- and posttreatment ultrasound and iCBCT scans were performed to record intrafractional bladder and rectal volume changes. The accuracy of contouring, oART procedure time, dosimetric outcomes, and acute toxicity were evaluated. Results: The average time from first iCBCT acquisition to completion of treatment was 22 minutes and 26 seconds. During this period, bladder volume increased by 44 cm3 using iCBCT contouring, whereas rectal volume remained stable (62.9 cm3 pretreatment vs 61.9 cm3 posttreatment). A total of 91.6% of influencers and 88.1% of CTVs required no or minor edits. The adapted plan was selected in all (434) fractions and significantly improved the dosimetry coverage for CTV and PTV, especially the vaginal PTV coverage by nearly 7% (P < .05). The adapted bladder Dmean was 104.61 cGy, and the rectum Dmean was 123.67 cGy, significantly lower than the scheduled plan of 108.24 and 128.19 cGy, respectively. The bone marrow and femur head left and right dosimetry were also improved with adaptation. Grade 2 acute gastrointestinal and genitourinary toxicities were 24% and 0, respectively. There was a grade 3 acute toxicity of decreased white blood cell count in 1 patient. Conclusions: Daily oART was associated with favorable dosimetry improvement and low acute toxicity, supporting its safety and efficacy for postoperative treatment of endometrial and cervical cancer. These results need to be validated in a larger prospective randomized controlled cohort.
Initial Experience of CT-Based Online Adaptive Radiotherapy for Nasopharyngeal Carcinoma With a Novel Integrated Platform: A Case Report

Abstract:Intensity-modulated radiation therapy (IMRT) improves tumor control and reduces long-term radiation-induced complications of patients with nasopharyngeal carcinoma (NPC), contingent upon accurate contouring and precise delivery of treatment plans. Online adaptive radiotherapy (ART) involves real-time treatment plan modification based on the variations in targets and organs at risk (OARs) to uphold treatment planning accuracy. This study describes the first reported case of fan beam computed tomography (FBCT)-guided online ART for NPC using a novel integrated platform. Online ART was performed at the 25th fraction in this case, as tumors and the patient's anatomy were observed to regress inter-fractionally, necessitating adjustments to the contours based on the anatomy of the day. Online ART plan optimized target volume coverage while reducing doses to OARs. Notably, online ART significantly improved radiotherapy efficiency. This patient achieved a clinical complete response 12 weeks post-treatment, with Epstein-Barr virus DNA levels reduced to 0 copies/ml. Currently, the patient is alive without evidence of high-grade toxicity or local recurrence at approximately 10 months post-treatment. This case confirms the feasibility and dosimetric benefit of online ART for NPC using a novel integrated platform. Further research is needed to confirm its clinical benefits.

Abstract:Purpose Conventional image-guided radiotherapy (IGRT) of the prostate bed is challenged by the varying anatomy due to dynamic changes of surrounding organs such as the bladder and rectum. This leads to changed dose coverage of target and surrounding tissue. The novel online adaptive radiotherapy (oART) aims to improve target coverage as well as reduce dose exposure to surrounding healthy tissues by daily reoptimization of treatment plans. Here we set out to quantify the resulting changes of this adaptation for patients and treatment team. Methods A total of 198 fractions of radiotherapy of the prostate bed (6 patients) were treated using oART with the Ethos accelerator (Varian Medical Systems, Palo Alto, CA, USA). For each fraction, volumes and several dose–volume parameters of target volumes and organs at risk were recorded for the scheduled plan (initial plan, recalculated based on daily cone beam computed tomography [CBCT]), the adapted plan, and the verification plan, which is the dose distribution of the applied plan recalculated on the closing CBCT after the adaptation process. Clinical acceptability for all plans was determined using given dose–volume parameters of target volumes. Additionally, the time needed for the adaptation process was registered and compared to the time required for the daily treatment of five conventional IGRT patients. Results Volumes of target and organs at risk (OAR) exhibited broad variation from day to day. The differences in dose coverage D 98% of the clinical target volume (CTV) were significant through adaptation ( p < 0.0001; median D 98% 97.1–98.0%) and further after verification CBCT ( p < 0.001; median D 98% 98.1%). Similarly, differences in D 98% of the planning target volume (PTV) were significant with adaptation ( p < 0.0001; median D 98% 91.8–96.5%) and after verification CBCT ( p < 0.001; median D 98% 96.4%) with decreasing interquartile ranges (IQR). Dose to OAR varied extensively and did not show a consistent benefit from oART but decreased in IQR. Clinical acceptability increased significantly from 19.2% for scheduled plans to 76.8% for adapted plans and decreased to 70.7% for verification plans. The scheduled plan was never chosen for treatment. The median time needed for oART was 25 min compared to 8 min for IGRT. Conclusion Target dose coverage was significantly improved using oART. IQR decreased for target coverage as well as OAR doses indicating higher repeatability of dose delivery using oART. Differences in doses after verification CBCT for targets as well as OAR were significant compared to adapted plans but did not offset the overall dosimetric gain of oART. The median time required is three times higher for oART compared to IGRT.

Abstract:Purpose/objectives: Retrospective analysis suggests that dose escalation to a biologically effective dose of more than 70 Gy may improve overall survival in patients with pancreatic ductal adenocarcinoma (PDAC), but such treatments in practice are limited by proximity of organs at risk (OARs). We hypothesized that CT-guided online adaptive radiotherapy (OART) can account for interfraction movement of OARs and allow for safe delivery of ablative doses. Materials/methods: This is a single institution retrospective analysis of patients with PDAC treated with OART on the Ethos platform (Varian Medical Systems, a Siemens Healthineers Company, Palo Alto). All patients were treated to 40 Gy in 5 fractions. PTV overlapping with a 5 mm planning risk volume expansion on the stomach, duodenum and bowel received 25 Gy. Initial treatment plans were created conventionally. For each fraction, PTV and OAR volumes were recontoured with AI assistance after initial cone beam CT (CBCT). The adapted plan was calculated, underwent QA, and then compared to the scheduled plan. A second CBCT was obtained prior to delivery of the selected plan. Total treatment time (first CBCT to end of radiation delivery) and active physician time (first to second CBCT) were recorded. PTV_4000 V95 %, PTV_2500 V9 5%, and D0.03 cc to stomach, duodenum and bowel were reported for scheduled (S) and adapted (A) plans. CTCAEv5.0 toxicities were recorded. Statistical analysis was performed using a two-sided T test and α of 0.05. Results: 21 patients with unresectable or locally-recurrent PDAC were analyzed, with a total of 105 fractions. Average total time was 29 min and 16 s (16:36-49:40) and average active physician time was 19:41 min (9:25-39:34). All fractions were treated with adapted plans. 97 % of adapted plans met PTV_4000 V95.0 % >95.0 % coverage goal and 100 % of adapted plans met OAR dose constraints. Median follow up was 6.6 months. Only 1 patient experienced acute grade 3+ toxicity directly attributable to radiation. Only 1 patient experienced late grade 3+ toxicity directly attributable to radiation. Conclusions: Daily CT-based OART was associated with significantly reduced dose OARs while achieving superior PTV coverage. Given the relatively quick total treatment time, radiation delivery was generally well tolerated and easily incorporated into the clinic workflow. Our initial clinical experience demonstrates OART allows for safe dose escalation in the treatment of PDAC.

Abstract:Importance: Patients with newly diagnosed locally advanced cervical carcinomas or recurrences after surgery undergoing radiochemotherapy whose tumor is unsuited for a brachytherapy boost need high-dose percutaneous radiotherapy with small margins to compensate for clinical target volume deformations and set-up errors. Cone-beam computed tomography-based online adaptive radiotherapy (ART) has the potential to reduce planning target volume (PTV) margins below 5 mm for these tumors. Objective: To compare online ART technologies with image-guided radiotherapy (IGRT) for gynecologic tumors. Design, setting, and participants: This comparative effectiveness study comprised all 7 consecutive patients with gynecologic tumors who were treated with ART with artificial intelligence segmentation from January to May 2022 at the West German Cancer Center. All adapted treatment plans were reviewed for the new scenario of organs at risk and target volume. Dose distributions of adapted and scheduled plans optimized on the initial planning computed tomography scan were compared. Exposure: Online ART for gynecologic tumors. Main outcomes and measures: Target dose coverage with ART compared with IGRT for PTV margins of 5 mm or less in terms of the generalized equivalent uniform dose (gEUD) without increasing the gEUD for the organs at risk (bladder and rectum). Results: The first 10 treatment series among 7 patients (mean [SD] age, 65.7 [16.5] years) with gynecologic tumors from a prospective observational trial performed with ART were compared with IGRT. For a clinical PTV margin of 5 mm, IGRT was associated with a median gEUD decrease in the interfractional clinical target volume of -1.5% (90% CI, -31.8% to 2.9%) for all fractions in comparison with the planned dose distribution. Online ART was associated with a decrease of -0.02% (90% CI, -3.2% to 1.5%), which was less than the decrease with IGRT (P < .001). This was not associated with an increase in the gEUD for the bladder or rectum. For a PTV margin of 0 mm, the median gEUD deviation with IGRT was -13.1% (90% CI, -47.9% to 1.6%) compared with 0.1% (90% CI, -2.3% to 6.6%) with ART (P < .001). The benefit associated with ART was larger for a PTV margin of 0 mm than of 5 mm (P = .004) due to spreading of the cold spot at the clinical target volume margin from fraction to fraction with a median SD of 2.4 cm (90% CI, 1.9-3.4 cm) for all patients. Conclusions and relevance: This study suggests that ART is associated with an improvement in the percentage deviation of gEUD for the interfractional clinical target volume compared with IGRT. As the gain of ART depends on fractionation and PTV margin, a strategy is proposed here to switch from IGRT to ART, if the delivered gEUD distribution becomes unfavorable in comparison with the expected distribution during the course of treatment.

Abstract:Purpose/Objective(s) The advent of commercially-available CBCT-guided online adaptive radiotherapy (oART) has made daily replanning possible in our center. We investigated the dosimetric impact and clinical feasibility of a treatment radiographer (RTT)-led oART with daily plan re-optimization service for muscle invasive bladder cancer (MIBC) at our center. Materials/Methods Between January 2021 and February 2022 ten patients with MIBC were treated with oART. The patients underwent a radiotherapy (RT) planning CT scan with an empty bladder. RT was prescribed to a dose of 55 Gy in 20 daily fractions to the planning target volume (PTV) and delivered using either 7- or 9-field IMRT. CTV to PTV margins were 1.5cm superiorly/anteriorly, 1cm posteriorly and 0.8cm inferiorly/laterally. In the oART workflow, artificial intelligence (AI) was utilized during daily plan generation for: organs at risk (OAR) contour delineation; structure-guided target contour propagation; and automated RT plan generation. RTTs were trained to perform the tasks of the oART workflow and a local competency program for RTTs was applied. For the first five patients a radiation oncologist (RO) was present at each treatment, for the second five patients, the RTTs performed the process independently with a RO present if needed. A RO reviewed the images weekly. A comparison of dose delivered using the daily adaptive plan (ADP) versus the original scheduled plan (SP) was performed using Wilcoxon signed rank test, for both PTV coverage and OAR doses. Timing measurements were recorded for the workflow. Results Seven male and three female patients with a median age of 79 years (range 66-82) were treated with oART for MIBC. Four patients received concomitant chemotherapy. A total of 186 oART treatments were planned. Of these, the SP was delivered on 10 occasions as a result of technical issues with the online ADP quality assurance. Using the ADP, PTV D98% was increased by a mean of 3.3% (p<0.05), and bowel and rectal V52 Gy were reduced by a mean of 9.1cc (p<0.05) and 2.6% (p<0.05), respectively, compared to the SP. For both the RO and RTT-led workflows, the median adaptive process duration from CBCT acquisition to beam-on was 19 minutes (range 9-35 minutes). Conclusion The use of AI has enabled routine clinical implementation of daily CBCT-guided oART which is RTT led. The use of oART increased PTV coverage and reduced OAR doses for MIBC. The development of appropriately trained RTTs to run this service increases the likelihood that this technological advancement can be maintained. Steps to reduce PTV margins are to be explored which should further enhance the dosimetric benefits of oART.

Managing Inconsistent Bladder Volumes in the Prostate Cancer Patient Using Daily Online Adaptive RT: A Case Report

Abstract:Purpose: We report the use of online adaptive radiotherapy (OART) aiming to improve dosimetric parameters in the prostate cancer patient who had lower urinary tract symptoms that caused him not to adhere to the standard bladder filling protocol. Methods and materials: The reference treatment plan for adaptive radiotherapy plan was generated for the pelvis and the solitary bony lesion using the Ethos treatment planning system. For each treatment session, high-quality iterative reconstructed cone beam CT (CBCT) images were acquired, and the system automatically generated an optimal adaptive plan after verification of contours. Image-guided RT (IGRT) plans were also created using the reference plan recalculated on the CBCT scan and were compared with adaptive plans. Results: The reference bladder volume in the planning CT scan was 173 cc, and the mean bladder volume difference over the course was 25.4% ± 16.6%. The ART offered superior target coverage for PTV 70 Gy over online IGRT (V95: 90.5 ± 3.2 % Vs 97.3 ± 0.4%; p=0.000) and the bladder was also better spared from the high dose (V65 Gy: 17.9 ± 9.1% vs 14.8 ± 3.6%; p=0.03). However, the mean rectum V65 doses were very similar in both plans. Conclusion: Managing the inconsistent bladder volume was feasible in the prostate cancer patient using the CBCT-guided OART and our analysis confirmed that adaptive plans offered better target coverage while sparing the bladder from high radiation doses in comparison to online IGRT plans. Clinical Experience with an Offline ART Head and Neck Program: Dosimetric Benefits and Opportunities for Patient Selection

Abstract:Purpose To develop a LINAC-based adaptive radiotherapy (ART) workflow for head and neck that is informed by automated image tracking to identify major anatomic changes warranting adaptation. In this study we report our initial clinical experience with the program and an investigation into potential trigger signals for ART. Methods and Materials Offline ART was systematically performed on patients receiving radiotherapy for head and neck cancer on C-arm LINACs. Adaptations were performed at a single timepoint during treatment with resimulation approximately 3 weeks into treatment. Throughout treatment, all patients were tracked using an automated image tracking system called the IN_HOUSE_SOFTWARE*. IN_HOUSE_SOFTWARE measures volumetric changes in gross tumor volumes (GTV) and selected normal tissues via cone-beam computed tomography (CBCT) scans and deformable registration. The benefit of ART was determined by comparing adaptive plan dosimetry and normal tissue complication probabilities (NTCP) against the initial plans recalculated on resimulation CT scans. Dosimetric differences were then correlated with IN_HOUSE_SOFTWARE-measured volume changes to identify patient-specific triggers for ART. Candidate trigger variables were evaluated using receiver operator characteristic (ROC) analysis. Results N=46 patients received ART in this study. Amongst these patients, we observed a significant decrease in dose to the submandibular glands (mean ± standard deviation: -219.2±291.2cGy, p-value<10 −5 ), parotids (-68.2±197.7cGy, p-value=0.001) and oral cavity (-238.7±206.7cGy, p-value<10 −5 ) with the adaptive plan. NTCPs for xerostomia computed from mean parotid doses also decreased significantly with the adaptive plans (p-value=0.008). We also observed systematic intra-treatment volume reductions (ΔV) for GTVs and normal tissues. Candidate triggers were identified which did predict significant improvement with ART including parotid ΔV=7%, neck ΔV=2%, and nodal GTV ΔV=29%. Conclusions Systematic offline head and neck ART was successfully deployed on conventional LINACs and reduced doses to critical salivary structures and oral cavity. Automated CBCT tracking provided information regarding anatomic changes that may aid patient-specific triggering for ART. (*Anonymized for review)

Abstract:Trimodality treatment for bladder cancer, consisting of maximal transurethral resection of the tumor followed by concurrent chemoradiotherapy, is an attractive management option with curative and organ-sparing intent. However, such treatment can be associated with acute toxicities related to the large treatment margins required due to daily variation in bladder filling, with resultant bladder, bowel, and rectal toxicity. Adaptive radiation, which accounts for inter-fraction variations in bladder size, allows the confident delivery of radiation to bladder cancer with smaller margins, with the potential to reduce toxicities without the associated risk of compromising the target coverage. Herein, we present a case series of two patients with primary bladder cancer who were treated with computed tomography (CT)-based online adaptive hypofractionated radiotherapy using the Ethos system (Varian Medical Systems, Palo Alto, CA, USA). The first is an 83-year-old male with a remote history of prostate cancer treated with radiotherapy, who received adaptive radiotherapy as a means of decreasing the required margin size and optimizing planning based on adjacent bowel to reduce the risk of re-irradiation. The second patient is a 78-year-old male with node-positive bladder cancer, which necessitated whole pelvis radiotherapy, who underwent adaptive treatment (25 fractions) as a means of sparing cumulative dose to the bowel while ensuring suitable target coverage. In both cases, the clinical target volume consisted of the entire bladder (± nodes) with a planning target volume expansion of 7 mm. During treatment, daily cone-beam CT scans were acquired and used to generate adapted plans. These plans were compared to the original plans, with attention to target coverage and dose to organs at risk. For all 45 fractions, the adaptive plan was selected, primarily as a means of improving target coverage. This case series demonstrates that the adaptive Ethos system effectively delivers treatment for primary bladder cancer. Further data are needed for clinical toxicity outcomes and the efficacy of this approach.
Forecasting patient-specific dosimetric benefit from daily online adaptive radiotherapy for cervical cancer

Abstract:Objective: Adaptive Radiotherapy (ART) is an emerging technique for treating cancer patients which facilitates higher delivery accuracy and has the potential to reduce toxicity. However, ART is also resource-intensive, requiring extra human and machine time compared to standard treatment methods. In this analysis, we sought to predict the subset of node-negative cervical cancer patients with the greatest benefit from ART, so resources might be properly allocated to the highest-yield patients. Approach: CT images, initial plan data, and on-treatment Cone-Beam CT (CBCT) images for 20 retrospective cervical cancer patients were used to simulate doses from daily non-adaptive and adaptive techniques. We evaluated the correlation (R 2 ) between dose and volume metrics from initial treatment plans and the dosimetric benefits to the Bowel V 40Gy , Bowel V 45Gy , Bladder D mean , and Rectum D mean from adaptive radiotherapy using reduced 3mm or 5mm CTV-to-PTV margins. The LASSO technique was used to identify the most predictive metrics for Bowel V 40Gy . The three highest performing metrics were used to build multivariate models with leave-one-out validation for Bowel V 40Gy . Main Results: Patients with higher initial bowel doses were correlated with the largest decreases in Bowel V 40Gy from daily adaptation (linear best fit R 2 =0.77 for a 3mm PTV margin and R 2 =0.8 for a 5mm PTV margin). Other metrics had intermediate or no correlation. Selected covariates for the multivariate model were differences in the initial Bowel V 40Gy and Bladder D mean using standard versus reduced margins and the initial bladder volume. Leave-one-out validation had an R 2 of 0.66 between predicted and true adaptive Bowel V 40Gy benefits for both margins. Significance: The resulting models could be used to prospectively triage cervical cancer patients on or off daily adaptation to optimally manage clinical resources. Additionally, this work presents a critical foundation for predicting benefits from daily adaptation that can be extended to other patient cohorts.

Abstract:Stereotactic radiation therapy (SBRT) has emerged as a promising treatment modality for locally advanced pancreatic cancer. The aim of this study is to assess the dosimetric efficacy of online adaptive radiotherapy (ART) in comparison to image-guided radiation therapy (IGRT) for pancreatic cancer. We conducted a retrospective analysis involving 8 patients diagnosed with locally advanced pancreatic cancer. The gross tumor volume (GTV) delineates the visible extent of the tumor on imaging, while the planning tumor volume (PTV) was generated by expanding 5 mm from the GTV and ensuring a 3 mm distance from the small intestine, duodenum, and stomach simultaneously. Treatment planning was executed using the United Imaging Healthcare Treatment Planning System workstation. The control group underwent evaluation based on daily validated fan-beam CT (FBCT) scans, assessing both the dose delivered to actual organs at risk (OARs) and the target volume. Radiotherapy plans were developed utilizing simulation CT, and conventional radiotherapy with daily image-guided radiation therapy (IGRT) was administered using FBCT-Linac. Conversely, patients in the study group received daily validated FBCT-guided adaptive radiotherapy plans, with a focus on mean dose assessment of both the target volume and OARs. Subsequently, we compared the average outcomes of each treatment fraction between IGRT and online adaptive radiotherapy (ART). Comparison between ART and IGRT treatment plans revealed significant differences in various dosimetric parameters: For PTV: V98%: ART (96.28%) vs IGRT (89.73%), p = 0.000, V95%: ART (96.28%) vs IGRT (89.73%), p = 0.031, V90%: ART (98.58%) vs IGRT (93.65%), p = 0.000, Dmean: ART (4912.91) vs IGRT (4804.11), p = 0.000. These results suggest significant improvements in PTV coverage and sparing of OARs with ART compared to IGRT, indicating the potential of ART in optimizing treatment outcomes for pancreatic cancer patients. Compared to conventional IGRT-guided SBRT programs, ART-based SBRT for pancreatic cancer not only enhances the dose distribution to the target volume but also mitigates the radiation exposure to critical organs-at-risk (OARs) such as the duodenum, small intestine, and stomach. This approach may offer a more favorable safety profile while concurrently enhancing treatment efficacy.

Abstract:This article presents a case of a patient with advanced head and neck cancer, characterized by a large and protruding tumor. The patient was treated with an innovative on-demand online adaptive radiotherapy (ART) technology, guided by cone beam computed tomography (CBCT), on the Ethos adaptive radiotherapy platform (version 1.0, Varian Medical Systems, Palo Alto, CA). A solution was provided for this special case to address the issue where part of the target volume could not participate in the optimization due to exceeding the external contour boundary during online adaptive radiotherapy. The treatment outcome was satisfactory in terms of tumor regression, while only grade 1 radiodermatitis and grade 2 oral mucositis at the end of radiotherapy. This article discusses the clinical diagnosis, treatment process, and follow-up of this case, aiming to provide clinical references for a broader application of this technology.

Abstract:Oropharyngeal candidiasis is associated with defects in cell-mediated immunity, and is commonly seen in immunocompromised patients. We have previously shown that T-cell-deficient BALB/c nude (nu/nu) mice are extremely susceptible to oropharyngeal candidiasis, and that recovery from a chronic infection is dependent on CD4 T lymphocytes. In this study we describe the local tissue cytokine profile in lymphocyte-reconstituted immunodeficient mice and their euthymic counterparts. Mice were infected orally with 108 cells of the yeast Candida albicans, and oral tissues sampled on days 0, 4, 8, and 14. Nude mice were reconstituted with 3 x 107 naïve lymphocytes following oral inoculation. Interleukin (IL)-6, interferon (IFN)-gamma and tumour necrosis factor (TNF)-alpha were identified in the oral tissues of infected euthymic mice recovering from oral infection, as well as naïve controls. TNF-alpha was identified in nude oral tissue on days 4 and 8, but only after lymphocyte reconstitution. No IL-2, IL-4 or IL-10 was detected in either euthymic or athymic mice at any time-point throughout the experiment. This study confirms the functional activity of T lymphocytes in reconstituted nude mice, and suggests that TNF-alpha may be an important mediator in the recovery from oropharyngeal candidiasis.

Abstract:Aims: Variability in the target and organs at risk (OARs) in cervical cancer treatment presents challenges for precise radiotherapy. Adaptive radiotherapy (ART) offers the potential to enhance treatment precision and outcomes. However, the increased workload and a lack of consensus on the most suitable ART approach hinder its clinical adoption. This systematic review aims to assess the current use of adaptive strategies for cervical cancer and define the optimal approach. Materials and methods: A systematic review of current literature published between January 2012 and May 2023 was conducted. Searches used PubMed/Medline, Cochrane Library, and Web of Science databases, supplemented with the University of Manchester, Google Scholar, and papers retrieved from reference lists. The review assessed workflows, compared dosimetric benefits, and examined resources for each identified strategy. Excluded were abstracts, conference abstracts, reviews, articles unrelated to ART management, proton therapy, brachytherapy, or qualitative studies. A narrative synthesis involved data tabulation, summarizing selected studies detailing workflow for cervical cancer and dosimetric outcomes for targets and OARs. Results: Sixteen articles met the inclusion criteria; these were mostly retrospective simulation planning studies, except four studies that had been clinically implemented. We identified five approaches for ART radiotherapy for cervical cancer: reactive and scheduled adaptation, internal target volume (ITV)-based approach using library of plans (LOP), fixed-margin approach using LOP, and real-time adaptation, with each approach reducing irradiated volumes without compromising target coverage compared to the non-ART approach. The LOP-based ITV approach is the most used and clinically assessed. Conclusion: Identifying the optimal strategy is challenging due to dosimetric assessment limitations. Implementing cervical cancer ART necessitates strategic optimization of clinical benefits and resources through research, including studies to identify the optimal frequency, and prospective evaluations of toxicity.
