Abstract:Background: Developing guidelines for the diagnosis and treatment of common cancers in China based on the evidence-based practice, the availability of diagnosis and treatment products, and the up-to-date advances in precision medicine is one of the basic tasks of the Chinese Society of Clinical Oncology Breast Cancer (CSCO BC) Committee. Methods: Protocols with high evidence level and good availability are used as the Level I recommendations; protocols with relatively high evidence level but slightly lower expert consensus or with poor availability are used as the Level II recommendations; and protocols that are clinically applicable but with low evidence level are regarded as the Level III recommendations. Based on the findings of clinical research at home and abroad and the opinions of CSCO BC experts, the CSCO BC guidelines determine the levels of recommendations for clinical application. Results: For human epidermal growth factor receptor 2 (HER2)-positive breast cancer, a combination of trastuzumab and pertuzumab regimen were recommended as Level I recommendation for neoadjuvant and first line metastatic breast cancer. Pyrotinib is also recommended as Level I recommendation in first line and second line therapy according to the latest studies conducted in China. Antibody drug conjugates was also recommended for patients with trastuzumab progression. For triple negative breast cancer, immunotherapy in early and metastatic breast cancer was highlighted and listed as new chapters in this version of guideline. For hormone receptor (HR)-positive breast cancer, cyclin dependent kinase 4/6 (CDK4/6) was recommended in different stages, especially in adjuvant therapy. There was also a new chapter for HER2-low breast cancer stratified by HR status. Conclusions: We firmly believe that evidence-based, availability-concerned, and consensus-based guidelines will be more feasible for clinical practice in China and in other countries with similar situations.

Abstract:Breast cancer ranks as the most common female malignancy worldwide. Data from GLOBOCAN 2020 showed that breast cancer surpassed lung cancer and become the leading malignancy globally which was a serious threat to women's health. Different from Caucasian, there is a lower prevalence of breast cancer in Chinese women. But the age-standardized incidence rate of breast cancer in China has reached to 39.1 per 100,000 in 2020, with 416,000 new cases, accounting for 18.4% of global breast cancer burden, due to multiple factors such as lifestyle and dietary habits changes in recent years[1]. The treatment and prevention of breast cancer are becoming increasingly a tuff task. In addition, triple-negative breast cancer (TNBC) is a subtype with higher malignancy and worse prognosis. Due to lack of specific therapeutic targets, less treatment progress has been made in recent years. There is even less progress in the therapy of advanced TNBC, but huge unmet needs exist for further therapeutic optimization. Echoing advocacy from The Society of Breast Cancer China Anti-Cancer Association; International Medical Exchange Society, Chinese Anti-Cancer Association; Breast Cancer Group, Branch of Oncologist, Chinese Medical Doctor Association, top experts from multidisciplinary departments of breast cancer in China have consensus on this "Guidelines for Clinical Diagnosis and Treatment of Advanced Triple-Negative Breast Cancer in China (2024 Edition)" according to the latest evidence based updates. We hope this pater can guide or optimize the precise therapeutic decision-making for advanced triple-negative breast cancer in China.

Abstract:Abstract The 2023 update of the Chinese Society of Clinical Oncology (CSCO) Clinical Guidelines for Gastric Cancer focuses on standardizing cancer diagnosis and treatment in China, reflecting the latest advancements in evidence‐based medicine, healthcare resource availability, and precision medicine. These updates address the differences in epidemiological characteristics, clinicopathological features, tumor biology, treatment patterns, and drug selections between Eastern and Western gastric cancer patients. Key revisions include a structured template for imaging diagnosis reports, updated standards for molecular marker testing in pathological diagnosis, and an elevated recommendation for neoadjuvant chemotherapy in stage III gastric cancer. For advanced metastatic gastric cancer, the guidelines introduce new recommendations for immunotherapy, anti‐angiogenic therapy and targeted drugs, along with updated management strategies for human epidermal growth factor receptor 2 (HER2)‐positive and deficient DNA mismatch repair (dMMR)/microsatellite instability‐high (MSI‐H) patients. Abstract:Neoadjuvant therapy can yield clinical benefits in early-stage breast cancer with indications. The indications, treatment regimens, and efficacy of neoadjuvant therapy have been investigated, which have contributed valuable clinical evidence. [ 1,2 ] The Chinese Society of Breast Surgery convened a panel of breast disease experts to review and discuss the literature on neoadjuvant therapy for breast cancer and evaluate relevant evidence by referring to the Grading of Recommendations, Assessment, Development, and Evaluations (GRADE) system. Taking into account China's specific circumstances, the society and participating experts developed the Clinical Practice Guidelines for Neoadjuvant Therapy for Early-stage Breast Cancer (Version 2022) to serve as a reference for breast surgeons in China.

Abstract:Colorectal cancer（CRC） is one of the major cancers threatening life and health of Chinese residents. According to the latest data released by the National Cancer Center in2015（1）, there were 387,000 new cases and 187,100 deaths of CRC in China, accounting for 9.87% and 8.01% of all malignancies. The first version of the Chinese Society of Clinical Oncology（CSCO） guidelines was released in April2017, and has been updated annually based on the latest clinical research data and the availability of new drugs（2-4）. Abstract:Background: The Chinese Society of Breast Surgery (CSBrS) Practice Guideline 2021 was published nearly one year ago. The new guideline was revised based on comprehensive feedback of the previous publication. The aim is to provide a better reference suitable for Chinese breast surgeons. Methods: Referred to GRADE (Grading of Recommendations Assessment, Development and Evaluation) system, evidences accepted by the Guideline were classified into four categories: I, II, III and IV, which reflected the reliability of the evidences quantitatively. The guideline also comprehensively considered the accessibility of the guideline in clinical practice in China and gave recommendations with different strengths. Results: The guideline emphasized the basic idea that a curative surgical approach falls under the category of radical tumor surgery. Six chapters, including "Diagnosis and treatment of patients with invasive breast cancer", "Sentinel lymph node biopsy in patients with early-stage breast cancer", "Breast-conserving surgery in patients with early-stage breast cancer", "Modified radical mastectomy of breast cancer", "Central venous access for the systemic treatment of breast cancer", and "Breast cancer in pregnancy and postpartum breast cancer" were revised. Conclusions: Compared with the 2021 edition, the new guideline has been revised in six chapters based on the latest research evidence and clinical needs.

Abstract:Breast cancer is the most common cancer among women worldwide. It has been estimated that about 416 000 new cases and over 117 000 deaths of breast cancer occurred in China in 2020. Among the new cases of breast cancer diagnosed each year, 3-10% have distant metastasis at the time of initial diagnosis. In addition, approximately 30% of patients with early-stage breast cancer may eventually experience recurrence or metastases. The 5-year survival rate of patients with advanced breast cancer is only 20% with a median overall survival of 2-3 years. Although advanced breast cancer remains incurable at present, new therapeutic options and multidisciplinary treatment could be utilized to alleviate symptoms, improve quality of life, and prolong patients' survival. The choice of treatment regimens for patients with advanced breast cancer is very important, and the optimal treatment strategy beyond the first- and second-line therapy is often lacking. Herein, the China Advanced Breast Cancer Guideline Panel discussed and summarized recent clinical evidence, updated the guidelines for the diagnosis and treatment of advanced breast cancer based on the 2020 edition, and formulated the "Guidelines for diagnosis and treatment of advanced breast cancer in China (2022 edition)" for clinicians' reference.

Abstract:There exist differences in the epidemiological characteristics, clinicopathological features, tumor biological characteristics, treatment patterns, and drug selections between gastric cancer patients from the Eastern and Western countries. The Chinese Society of Clinical Oncology (CSCO) has organized a panel of senior experts specializing in all sub-specialties of gastric cancer to compile a clinical guideline for the diagnosis and treatment of gastric cancer since 2016 and renews it annually. Taking into account regional differences, giving full consideration to the accessibility of diagnosis and treatment resources, these experts have conducted expert consensus judgment on relevant evidence and made various grades of recommendations for the clinical diagnosis and treatment of gastric cancer to reflect the value of cancer treatment and meeting health economic indexes in China. The 2021 CSCO Clinical Practice Guidelines for Gastric Cancer covers the diagnosis, treatment, follow-up, and screening of gastric cancer. Based on the 2020 version of the CSCO Chinese Gastric Cancer guidelines, this updated guideline integrates the results of major clinical studies from China and overseas for the past year, focused on the inclusion of research data from the Chinese population for more personalized and clinically relevant recommendations. For the comprehensive treatment of non-metastatic gastric cancer, attentions were paid to neoadjuvant treatment. The value of perioperative chemotherapy is gradually becoming clearer and its recommendation level has been updated. For the comprehensive treatment of metastatic gastric cancer, recommendations for immunotherapy were included, and immune checkpoint inhibitors from third-line to the first-line of treatment for different patient groups with detailed notes are provided.

Abstract:In order to standardize the management of breast cancer, Chinese Anti-Cancer Association ( CACA) issued the first edition of Clinical Practice Guidelines in Breast Cancer in 2007 and updates it every other year based on latest evidences. The newly published CACA Clinical Practice Guidelines in Breast Cancer in 2015 covers the overall management of breast cancer including screening, imaging, pathology, surgery and systemic treatment. This article mainly focused on the updates of systemic treatment for breast cancer patients in 2015 version, with regard to endocrine therapy, anti-HER-2 molecular targeted therapy, chemotherapy and bone protection.

Abstract:The National Comprehensive Cancer Network ( NCCN ) launched the NCCN Clinical Practice Guideline for Breast Cancer (version 1) on March 20, 2018. This guideline reviews the latest research progress and treatment concepts in the field of breast cancer. On the basis of the previous versions, they update the guidelines for the diagnosis and treatment of breast cancer, which can assist physicians in their clinical practices. This paper interpreted the important changes in the latest version, and discussed its practicability and feasibility in China based on clinical practice, including the TNM staging of breast cancer by American Joint Committee on Cancer ( AJCC) ( the eighth edition), the principles of breast conserving surgery and pathological evaluation, the recent progress of sentinel lymph node biopsy and new strategy for systematic treatment.
Focused updates of American Society of Clinical Oncology Guideline on the use of multigene tests in clinical practice for breast cancer

Abstract:In recent years, multigene tests have been recommended to guide decisions on the need of adjuvant chemo-therapy for patients with early-stage breast cancer. The comprehensive strategy of early-stage breast cancer has come into an era, when both anatomic tumor burden and multigene test results should be taken into consideration to gain a better understanding of tumor biology and personalized treatment, in order to minimize toxic effects. In July 2017, the Journal of Clinical Oncology ( JCO) published the new version of the clinical practice guideline of American Society of Clinical Oncology ( ASCO) on the use of biomarkers to guide therapeutic decision for breast cancer. The guideline provided cri-teria of clinical utility for different patient groups, and focused on the recommendation of MammaPrint ? with evidence of high quality, leading to reasonable decision-making for clinicians to avoid overtreatment.

Abstract:Triple-negative breast cancer (TNBC) is a subtype of breast cancer characterized by its highly aggressive behavior, early recurrence, and poor outcomes, when compared with other subtypes. Due to the absence of the estrogen receptor, progesterone receptor, and human epidermal growth factor receptor 2 expression, TNBC lacks meaningful biomarkers and an effective therapeutic strategy. Chemotherapy remains the main adjuvant treatment for patients with TNBC. Anthracycline/taxane-based regimens are the standard of care in adjuvant settings. The addition of capecitabine or platinum may offer extra benefits to patients with TNBC, but at the cost of increased toxicity or adverse events. Dose-dense chemotherapy may enhance treatment efficacy in patients who are able to tolerate the treatment regimen, especially in high-risk patients. As a heterogenous disease, TNBC can be classified into several molecular subtypes according to genomic or transcriptional features, which may indicate potential targets for more precise and individualized treatment strategies. With our increased understanding of signal pathways associated with TNBC, as well as the discovery of novel biomarkers indicative of TNBC prognosis, several new therapeutic options are under investigation, and some have already reported good results. In this review, we summarized the current conventional therapeutic strategies and emerging clinical trials regarding adjuvant treatment for TNBC. Furthermore, we evaluated the prognostic value of several potential targets and the progress of targeted therapy in TNBC, both in neoadjuvant and adjuvant settings.
