Abstract:According to the latest global cancer statistics,colorectal cancer(CRC)is the most common malignancy of the digestive system and the second most lethal among all cancer types(1).In China,CRC is the second most prevalent cancer,following only after lung cancer(2).The first version of the Chinese Society of Clinical Oncology(CSCO)guideline was launched in 2017 and has been updated annually based on the latest findings of clinical research,drug accessibility and expert consensus(3-8).Here,we present the main updates of the 2024 version compared to the 2023 version.

Abstract:Background: Developing guidelines for the diagnosis and treatment of common cancers in China based on the evidence-based practice, the availability of diagnosis and treatment products, and the up-to-date advances in precision medicine is one of the basic tasks of the Chinese Society of Clinical Oncology Breast Cancer (CSCO BC) Committee. Methods: Protocols with high evidence level and good availability are used as the Level I recommendations; protocols with relatively high evidence level but slightly lower expert consensus or with poor availability are used as the Level II recommendations; and protocols that are clinically applicable but with low evidence level are regarded as the Level III recommendations. Based on the findings of clinical research at home and abroad and the opinions of CSCO BC experts, the CSCO BC guidelines determine the levels of recommendations for clinical application. Results: For human epidermal growth factor receptor 2 (HER2)-positive breast cancer, a combination of trastuzumab and pertuzumab regimen were recommended as Level I recommendation for neoadjuvant and first line metastatic breast cancer. Pyrotinib is also recommended as Level I recommendation in first line and second line therapy according to the latest studies conducted in China. Antibody drug conjugates was also recommended for patients with trastuzumab progression. For triple negative breast cancer, immunotherapy in early and metastatic breast cancer was highlighted and listed as new chapters in this version of guideline. For hormone receptor (HR)-positive breast cancer, cyclin dependent kinase 4/6 (CDK4/6) was recommended in different stages, especially in adjuvant therapy. There was also a new chapter for HER2-low breast cancer stratified by HR status. Conclusions: We firmly believe that evidence-based, availability-concerned, and consensus-based guidelines will be more feasible for clinical practice in China and in other countries with similar situations.

Abstract:Abstract The 2023 update of the Chinese Society of Clinical Oncology (CSCO) Clinical Guidelines for Gastric Cancer focuses on standardizing cancer diagnosis and treatment in China, reflecting the latest advancements in evidence‐based medicine, healthcare resource availability, and precision medicine. These updates address the differences in epidemiological characteristics, clinicopathological features, tumor biology, treatment patterns, and drug selections between Eastern and Western gastric cancer patients. Key revisions include a structured template for imaging diagnosis reports, updated standards for molecular marker testing in pathological diagnosis, and an elevated recommendation for neoadjuvant chemotherapy in stage III gastric cancer. For advanced metastatic gastric cancer, the guidelines introduce new recommendations for immunotherapy, anti‐angiogenic therapy and targeted drugs, along with updated management strategies for human epidermal growth factor receptor 2 (HER2)‐positive and deficient DNA mismatch repair (dMMR)/microsatellite instability‐high (MSI‐H) patients. Additionally, the guidelines offer detailed screening recommendations for hereditary gastric cancer and an appendix listing drug treatment regimens for various stages of gastric cancer. The 2023 CSCO Clinical Guidelines for Gastric Cancer updates are based on both Chinese and international clinical research and expert consensus to enhance their applicability and relevance in clinical practice, particularly in the heterogeneous healthcare landscape of China, while maintaining a commitment to scientific rigor, impartiality, and timely revisions.

Abstract:There exist differences in the epidemiological characteristics, clinicopathological features, tumor biological characteristics, treatment patterns, and drug selections between gastric cancer patients from the Eastern and Western countries. The Chinese Society of Clinical Oncology (CSCO) has organized a panel of senior experts specializing in all sub-specialties of gastric cancer to compile a clinical guideline for the diagnosis and treatment of gastric cancer since 2016 and renews it annually. Taking into account regional differences, giving full consideration to the accessibility of diagnosis and treatment resources, these experts have conducted expert consensus judgment on relevant evidence and made various grades of recommendations for the clinical diagnosis and treatment of gastric cancer to reflect the value of cancer treatment and meeting health economic indexes in China. The 2021 CSCO Clinical Practice Guidelines for Gastric Cancer covers the diagnosis, treatment, follow-up, and screening of gastric cancer. Based on the 2020 version of the CSCO Chinese Gastric Cancer guidelines, this updated guideline integrates the results of major clinical studies from China and overseas for the past year, focused on the inclusion of research data from the Chinese population for more personalized and clinically relevant recommendations. For the comprehensive treatment of non-metastatic gastric cancer, attentions were paid to neoadjuvant treatment. The value of perioperative chemotherapy is gradually becoming clearer and its recommendation level has been updated. For the comprehensive treatment of metastatic gastric cancer, recommendations for immunotherapy were included, and immune checkpoint inhibitors from third-line to the first-line of treatment for different patient groups with detailed notes are provided.

Abstract:Pancreatic cancer is one of the leading causes of cancer-related mortality in both developed and developing countries. The incidence of pancreatic cancer in China accounts for about a quater of the global incidence, and the epidemiological characteristics and therapeutic strategies differ due to social, economic, cultural, environmental, and public health factors. Non-domestic guidelines do not reflect the clinicopathologic characteristics and treatment patterns of Chinese patients. Thus, in 2018, the Chinese Society of Clinical Oncology (CSCO) organized a panel of senior experts from all sub-specialties within the field of pancreatic oncology to compile the Chinese guidelines for the diagnosis and treatment of pancreatic cancer. The guidelines were made based on both the Western and Eastern clinical evidence and updated every one or two years. The experts made consensus judgments and classified evidence-based recommendations into various grades according to the regional differences, the accessibility of diagnostic and treatment resources, and health economic indexes in China. Here we present the latest version of the guidelines, which covers the diagnosis, treatment, and follow-up of pancreatic cancer. The guidelines might standardize the diagnosis and treatment of pancreatic cancer in China and will encourage oncologists to design and conduct more clinical trials about pancreatic cancer.
A comparison between a clinical decision support system and clinicians with guidelines in breast cancer.

Abstract:e13551 Background: We are building a clinical decision support system (CSCO AI) for breast cancer patients to improve the efficiency of clinical decision-making. We aimed to assess cancer treatment regimens, in neoadjuvant therapy, adjuvant chemotherapy, adjuvant endocrine therapy, first line therapy and second line therapy, given by CSCO AI and clinicians. Methods: 400 breast cancer patients were screened from the CSCO database. Clinicians with similar levels were randomly assigned one of the volumes (200 cases). After that, clinicians with guidelines were asked to answer the same cases again. CSCO AI was asked to assess all cases. Three reviewers were independently asked to evaluate the regimens from clinicians and CSCO AI. Regimens were masked before evaluation. The primary outcome was the proportion of high-level conformity (HLC), which were defined as the proportions of regimens in accordance with CSCO guidelines. Results: The overall concordance between clinicians and CSCO AI was 67.4% (2350/3500). After referring to the guideline, a total of 22.6% (792/3500) regimens were modified by clinicians, 12.9% (451/3500) had a higher grades and 9.7% (341/3500) had a lower grades. In early stage, the concordance was elevated with statistical significance from 71.3% (1497/2100) to 76.1% (1598/2100, p<0.001). In the metastatic stage, the concordance was improved form 61.7% (864/1400) to 66.0% (924/1400, p=0.018). HLC in CSCO AI was 95.8% (95%CI:94.0%-97.6%), significantly higher than that in clinicians (90.8%, 95%CI:89.8%-91.8%) and in clinicians with guidelines (92.1%, 95%CI:91.0%-93.4%). In early stage, high-level conformity in CSCO AI was 95.7%, with no statistical significance when compared with clinicians (92.7%, p=0.078) and clinicians with guidelines (92.3%, p=0.050). In metastatic stage, high-level conformity in clinicians was only 88.0%, lower than that in CSCO AI (96.0%, p=0.001). However, after referring guidelines, high-level conformity in clinicians was elevated to 91.9%, with no significant difference when compared with that in CSCO AI (p=0.058). Considering professions, the high level conformity of surgeons was 85.9%, lower than that of CSCO AI (OR=0.25,95%CI: 0.16-0.41). The most significant difference in HLC was in first-line therapy (OR=0.06, 95%CI:0.01-0.41). When clinicians were divided according to their levels, there was no statistical significance between CSCO AI and higher-level clinicians. Conclusions: Clinical decision support for breast cancer was superior for most process outcomes except for second-line therapy. The improvements in process outcomes suggest that CSCO AI can be widely used in clinical practice.

Abstract:The survival of patients with breast cancer has been greatly improved by comprehensive treatment including surgery, radiotherapy, and systemic treatment, among which surgical treatment remains the mainstay. Breast surgeons must not only master surgical skills but also develop the ability to treat disease comprehensively. To improve the ability of breast surgeons in China to diagnose breast cancer early, perform reasonable surgical treatment, and provide comprehensive treatment orientation, the Chinese Society of Breast Surgery (CSBrS) has reported the key issues in the diagnosis and treatment of invasive breast cancer through opinion collection and expert discussion. The group evaluated the relevant evidence using the grading of recommendations assessment, development, and evaluation system and developed the clinical practice guideline for diagnosis and treatment in patients with invasive breast cancer: CSBrS practice guidelines 2021 with the aim of providing guidance for the clinical practice of breast surgeons in China.

Abstract:Breast cancer is the most common cancer among women worldwide. It has been estimated that about 416 000 new cases and over 117 000 deaths of breast cancer occurred in China in 2020. Among the new cases of breast cancer diagnosed each year, 3-10% have distant metastasis at the time of initial diagnosis. In addition, approximately 30% of patients with early-stage breast cancer may eventually experience recurrence or metastases. The 5-year survival rate of patients with advanced breast cancer is only 20% with a median overall survival of 2-3 years. Although advanced breast cancer remains incurable at present, new therapeutic options and multidisciplinary treatment could be utilized to alleviate symptoms, improve quality of life, and prolong patients' survival. The choice of treatment regimens for patients with advanced breast cancer is very important, and the optimal treatment strategy beyond the first- and second-line therapy is often lacking. Herein, the China Advanced Breast Cancer Guideline Panel discussed and summarized recent clinical evidence, updated the guidelines for the diagnosis and treatment of advanced breast cancer based on the 2020 edition, and formulated the "Guidelines for diagnosis and treatment of advanced breast cancer in China (2022 edition)" for clinicians' reference.

Abstract:Breast cancer has become the most common malignant tumor worldwide. However, the survival rate of breast cancer patients in China is 8% lower compared to that in European and American countries. Insufficient screening coverage and low rate of early detection are key problems. Therefore, it is imperative to develop a set of guidelines for breast cancer screening, early diagnosis, and treatment strategies suitable for Chinese women. The National Health Commission, the National Cancer Center, and the National Cancer Quality Control Center collectively organized a committee of specialists with multidisciplinary backgrounds in breast surgery, oncology, radiotherapy, diagnostic imaging, ultrasound, pathology, epidemiology, and health economics. The committee collectively published ''China Breast Cancer Screening, Early Diagnosis and Treatment Specification'', in which the committee outlined a set of strategies of breast cancer screening, early diagnosis, and treatment suitable for China's national conditions. The strategies were formulated based on clinical experience in breast cancer prevention and treatment, characteristics of breast cancer patients in China, and socioeconomic conditions. The purpose of this article is to provide an interpretation of the above guidelines as a reference for breast cancer screening.

Abstract:BACKGROUND:Currently, there is no standard context that conforms to the Chinese national framework for evaluating medical decisions regarding the treatment of lung cancer.METHODS:This draft was formulated after a systematic review and a focus group discussion among 20 experts, who were senior physicians with extensive clinical experience from the Chinese Thoracic Oncology Group (CTONG) task force. Subsequently, a draft and a five-point Likert scale were sent to 300 CTONG working group members. These were modified according to feedback from a four-round modified Delphi approach. Hence, the first version of the 'Therapeutic option of lung cancer: CTONG scoring system' was formulated. Afterward, a corresponding questionnaire was designed to collect opinions on the weight allocation of various indicators. This was issued through the WeChat platform, "Oncology News" application and e-mails from October 23, 2020, to November 25, 2020. Participants from numerous occupations in cancer-related fields from various regions of China were included in the study. Overall and subgroup analyses regarding weight allocations were performed. The differences between participant-allocated and reference weights were considered to adjust the framework.RESULTS:The framework contained four aspects and six indicators, including efficacy [progression-free survival (PFS)/overall survival (OS) and subsequent treatment], safety [treatment-related severe adverse event (SAE), dose adjustment], quality of life (Qol), and compensation. The reference weights were 50%, 5%, 10%, 5%, 10%, and 20% for each indicator. By November 25, 2020, 1,043 valid questionnaires had been obtained. The majority of the questionnaires were completed by physicians (86.5%). Subgroup analysis among the various groups showed an overall consistent trend. Besides, significant differences between the participant-allocated and reference weights were found among PFS/OS (difference: -11.5%), compensation (difference: -10.1%), and subsequent treatment (difference: 9.7%) indicators. After discussion, the final weight allocations were set at 45%, 10%, 15%, 5%, 10%, and 15% for PFS/OS, subsequent treatment, treatment-related SAE, dose adjustment, Qol, and compensation, respectively.CONCLUSIONS:The CTONG scoring system, as an objective evaluation model that involves multiple parameters, is a breakthrough method for evaluating the therapeutic value of lung cancer treatment options in China, which is worthy of further verification in future clinical practice.

Abstract:Ductal carcinoma in situ (DCIS), also known as intraductal carcinoma, is an in situ carcinoma limited to the mammary ducts. The gold-standard screening, diagnostic, and treatment options have long been controversial. The Chinese Society of Breast Surgery (CSBrS) has re-evaluated the quality of the clinical study evidence on DCIS. In order to standardize the diagnosis and treatment of DCIS and provide a reference for Chinese breast surgeons, the CSBrS has used the Grading of Recommendations Assessment, Development, and Evaluation handbook while referring to its feasibility in the actual clinical practice of Chinese breast surgeons to develop the CSBrS Clinical Practice Guideline for the Diagnosis and Treatment of Ductal Carcinoma In Situ (2021).
