Abstract:Background: Developing guidelines for the diagnosis and treatment of common cancers in China based on the evidence-based practice, the availability of diagnosis and treatment products, and the up-to-date advances in precision medicine is one of the basic tasks of the Chinese Society of Clinical Oncology Breast Cancer (CSCO BC) Committee. Methods: Protocols with high evidence level and good availability are used as the Level I recommendations; protocols with relatively high evidence level but slightly lower expert consensus or with poor availability are used as the Level II recommendations; and protocols that are clinically applicable but with low evidence level are regarded as the Level III recommendations. Based on the findings of clinical research at home and abroad and the opinions of CSCO BC experts, the CSCO BC guidelines determine the levels of recommendations for clinical application. Results: For human epidermal growth factor receptor 2 (HER2)-positive breast cancer, a combination of trastuzumab and pertuzumab regimen were recommended as Level I recommendation for neoadjuvant and first line metastatic breast cancer. Pyrotinib is also recommended as Level I recommendation in first line and second line therapy according to the latest studies conducted in China. Antibody drug conjugates was also recommended for patients with trastuzumab progression. For triple negative breast cancer, immunotherapy in early and metastatic breast cancer was highlighted and listed as new chapters in this version of guideline. For hormone receptor (HR)-positive breast cancer, cyclin dependent kinase 4/6 (CDK4/6) was recommended in different stages, especially in adjuvant therapy. There was also a new chapter for HER2-low breast cancer stratified by HR status. Conclusions: We firmly believe that evidence-based, availability-concerned, and consensus-based guidelines will be more feasible for clinical practice in China and in other countries with similar situations.

Abstract:Background: The Chinese Society of Breast Surgery (CSBrS) Practice Guideline 2021 was published nearly one year ago. The new guideline was revised based on comprehensive feedback of the previous publication. The aim is to provide a better reference suitable for Chinese breast surgeons. Methods: Referred to GRADE (Grading of Recommendations Assessment, Development and Evaluation) system, evidences accepted by the Guideline were classified into four categories: I, II, III and IV, which reflected the reliability of the evidences quantitatively. The guideline also comprehensively considered the accessibility of the guideline in clinical practice in China and gave recommendations with different strengths. Results: The guideline emphasized the basic idea that a curative surgical approach falls under the category of radical tumor surgery. Six chapters, including "Diagnosis and treatment of patients with invasive breast cancer", "Sentinel lymph node biopsy in patients with early-stage breast cancer", "Breast-conserving surgery in patients with early-stage breast cancer", "Modified radical mastectomy of breast cancer", "Central venous access for the systemic treatment of breast cancer", and "Breast cancer in pregnancy and postpartum breast cancer" were revised. Conclusions: Compared with the 2021 edition, the new guideline has been revised in six chapters based on the latest research evidence and clinical needs.

Abstract:Neoadjuvant therapy can yield clinical benefits in early-stage breast cancer with indications. The indications, treatment regimens, and efficacy of neoadjuvant therapy have been investigated, which have contributed valuable clinical evidence. [ 1,2 ] The Chinese Society of Breast Surgery convened a panel of breast disease experts to review and discuss the literature on neoadjuvant therapy for breast cancer and evaluate relevant evidence by referring to the Grading of Recommendations, Assessment, Development, and Evaluations (GRADE) system. Taking into account China's specific circumstances, the society and participating experts developed the Clinical Practice Guidelines for Neoadjuvant Therapy for Early-stage Breast Cancer (Version 2022) to serve as a reference for breast surgeons in China.

Abstract:In order to standardize the management of breast cancer, Chinese Anti-Cancer Association ( CACA) issued the first edition of Clinical Practice Guidelines in Breast Cancer in 2007 and updates it every other year based on latest evidences. The newly published CACA Clinical Practice Guidelines in Breast Cancer in 2015 covers the overall management of breast cancer including screening, imaging, pathology, surgery and systemic treatment. This article mainly focused on the updates of systemic treatment for breast cancer patients in 2015 version, with regard to endocrine therapy, anti-HER-2 molecular targeted therapy, chemotherapy and bone protection.

Abstract:Colorectal cancer（CRC） is one of the major cancers threatening life and health of Chinese residents. According to the latest data released by the National Cancer Center in2015（1）, there were 387,000 new cases and 187,100 deaths of CRC in China, accounting for 9.87% and 8.01% of all malignancies. The first version of the Chinese Society of Clinical Oncology（CSCO） guidelines was released in April2017, and has been updated annually based on the latest clinical research data and the availability of new drugs（2-4）. Abstract:Abstract The 2023 update of the Chinese Society of Clinical Oncology (CSCO) Clinical Guidelines for Gastric Cancer focuses on standardizing cancer diagnosis and treatment in China, reflecting the latest advancements in evidence‐based medicine, healthcare resource availability, and precision medicine. These updates address the differences in epidemiological characteristics, clinicopathological features, tumor biology, treatment patterns, and drug selections between Eastern and Western gastric cancer patients. Key revisions include a structured template for imaging diagnosis reports, updated standards for molecular marker testing in pathological diagnosis, and an elevated recommendation for neoadjuvant chemotherapy in stage III gastric cancer. For advanced metastatic gastric cancer, the guidelines introduce new recommendations for immunotherapy, anti‐angiogenic therapy and targeted drugs, along with updated management strategies for human epidermal growth factor receptor 2 (HER2)‐positive and deficient DNA mismatch repair (dMMR)/microsatellite instability‐high (MSI‐H) patients. Additionally, the guidelines offer detailed screening recommendations for hereditary gastric cancer and an appendix listing drug treatment regimens for various stages of gastric cancer. Abstract:Updates of the National Comprehensive Cancer Network (NCCN) Guidelines for breast cancer involved workup, surgical treatment, preoperative systemic therapy, systemic adjuvant treatment, radiotherapy, targeted therapy, endocrine therapy, treatment of recurrence/stage IV disease and follow-up. Since the NCCN Guidelines were introduced into China ten years ago, diagnosis and treatment of breast cancer have achieved prominent progress from group-based therapy based on TNM stage to classification treatment based on pathology and immunohistochemistry, symbolizing specialization and standardization of the diagnosis and treatment of breast cancer in China. The next-generation sequencing technology and other non-anatomical information will be used to build an individual prognostic system, which is the highlight of the 8th edition TNM cancer staging system. Seven kinds of tumor including breast cancer have been selected. A deeper understanding of the nature of breast cancer will build a bridge to the Personalized Medicine.

Abstract:Breast cancer is the most common cancer among women worldwide. It has been estimated that about 416 000 new cases and over 117 000 deaths of breast cancer occurred in China in 2020. Among the new cases of breast cancer diagnosed each year, 3-10% have distant metastasis at the time of initial diagnosis. In addition, approximately 30% of patients with early-stage breast cancer may eventually experience recurrence or metastases. The 5-year survival rate of patients with advanced breast cancer is only 20% with a median overall survival of 2-3 years. Although advanced breast cancer remains incurable at present, new therapeutic options and multidisciplinary treatment could be utilized to alleviate symptoms, improve quality of life, and prolong patients' survival. The choice of treatment regimens for patients with advanced breast cancer is very important, and the optimal treatment strategy beyond the first- and second-line therapy is often lacking. Herein, the China Advanced Breast Cancer Guideline Panel discussed and summarized recent clinical evidence, updated the guidelines for the diagnosis and treatment of advanced breast cancer based on the 2020 edition, and formulated the "Guidelines for diagnosis and treatment of advanced breast cancer in China (2022 edition)" for clinicians' reference.

Abstract:AbstractIn recent years, the incidence of prostate cancer in China has shown a trend of continuous growth. Prostate cancer has become a urinary malignant tumor that seriously affects the health of men in China. Radical surgery and precision radiotherapy are the main methods of radical treatment for prostate cancer. Standardized diagnosis and treatment via a multidisciplinary team should be the standard practice to improve cure rate. Rapid development in imaging, pathology and molecular gene diagnosis has significantly changed the management of prostate cancer. In radiation oncology, image‐guided intensity modulated radiotherapy has been widely adopted in the treatment of prostate cancer. The comprehensive approach centered with radiotherapy has achieved excellent curative effect while continuously reducing treatment‐related side effects.

Abstract:The survival of patients with breast cancer has been greatly improved by comprehensive treatment including surgery, radiotherapy, and systemic treatment, among which surgical treatment remains the mainstay. Breast surgeons must not only master surgical skills but also develop the ability to treat disease comprehensively. To improve the ability of breast surgeons in China to diagnose breast cancer early, perform reasonable surgical treatment, and provide comprehensive treatment orientation, the Chinese Society of Breast Surgery (CSBrS) has reported the key issues in the diagnosis and treatment of invasive breast cancer through opinion collection and expert discussion. The group evaluated the relevant evidence using the grading of recommendations assessment, development, and evaluation system and developed the clinical practice guideline for diagnosis and treatment in patients with invasive breast cancer: CSBrS practice guidelines 2021 with the aim of providing guidance for the clinical practice of breast surgeons in China.

Abstract:The National Comprehensive Cancer Network ( NCCN ) launched the NCCN Clinical Practice Guideline for Breast Cancer (version 1) on March 20, 2018. This guideline reviews the latest research progress and treatment concepts in the field of breast cancer. On the basis of the previous versions, they update the guidelines for the diagnosis and treatment of breast cancer, which can assist physicians in their clinical practices. This paper interpreted the important changes in the latest version, and discussed its practicability and feasibility in China based on clinical practice, including the TNM staging of breast cancer by American Joint Committee on Cancer ( AJCC) ( the eighth edition), the principles of breast conserving surgery and pathological evaluation, the recent progress of sentinel lymph node biopsy and new strategy for systematic treatment.

Abstract:In recent years, multigene tests have been recommended to guide decisions on the need of adjuvant chemo-therapy for patients with early-stage breast cancer. The comprehensive strategy of early-stage breast cancer has come into an era, when both anatomic tumor burden and multigene test results should be taken into consideration to gain a better understanding of tumor biology and personalized treatment, in order to minimize toxic effects. In July 2017, the Journal of Clinical Oncology ( JCO) published the new version of the clinical practice guideline of American Society of Clinical Oncology ( ASCO) on the use of biomarkers to guide therapeutic decision for breast cancer. The guideline provided cri-teria of clinical utility for different patient groups, and focused on the recommendation of MammaPrint ? with evidence of high quality, leading to reasonable decision-making for clinicians to avoid overtreatment.

Abstract:To standardize the prevention and clinical management of lung cancer, improve patients' survival outcomes, and offer professional insight for clinicians, the Oncology Society of Chinese Medical Association has summoned experts from departments of pulmonary medicine, oncology, thoracic surgery, radiotherapy, imaging, and pathology to formulate the Oncology Society of Chinese Medical Association guideline for clinical diagnosis and treatment of lung cancer in China (2023 edition) through consensus meetings. Updates in this edition include 1) cancer screening: deletion of high-risk traits of lung cancer based on epidemiological investigations in the Caucasian population, while preserving features confirmed by research on the Chinese population. Advice on screening institutions is also added to raise awareness of the merits and demerits of lung cancer screening through detailed illustrations. 2) Principles of histopathologic evaluation: characteristics of four types of neuroendocrine tumors (typical carcinoid, atypical carcinoid, large cell carcinoma, and small cell carcinoma) are reviewed. 3) Surgical intervention: more options of resection are available for certain peripheral lesions based on several clinical studies (CALGB140503, JCOG0802, JCOG1211). 4) neoadjuvant/adjuvant therapy: marked improvement in the prognosis of non-small cell lung cancer (NSCLC) patients receiving neoadjuvant immunotherapy are reviewed; more options for consolidation immunotherapy after radiochemotherapy have also emerged. 5) Targeted and immune therapy: tyrosine kinase inhibitors of sensitive driver mutations such as EGFR and ALK as well as rare targets such as MET exon 14 skipping, RET fusion, ROS1 fusion, and NTRK fusion have been approved, offering more treatment options for clinicians and patients. Furthermore, multiple immune checkpoint inhibitors have been granted for the treatment of NSCLC and SCLC, resulting in prolonged survival of late-stage lung cancer patients. This guideline is established based on the current availability of domestically approved medications, recommendations of international guidelines, and present clinical practice in China as well as integration of the latest medical evidence of pathology, genetic testing, immune molecular biomarker detection, and treatment methods of lung cancer in recent years, to provide recommendations for professionals in clinical oncology, radiology, laboratory, and rehabilitation.
