Abstract:In recent decades, minimally invasive surgery has become the favoured surgical technique, with increasing utilisation of robotic surgery to enhance patient outcomes. However, the design complexity of surgical robotic instruments can pose challenges in maintaining adequate cleaning, disinfection and sterilisation—particularly of the device’s interior. In our hospital, robotic instruments are reused for a maximum of ten successive patients, following the manufacturer’s guidelines. To the best of our knowledge, neither the manufacturer nor ISO standards have specified any methods to determine the sterility of robotic instruments after cleaning, disinfection and sterilisation procedures. In a small pilot study, we used a locally developed protocol to evaluate the sterility of 20 da Vinci SI robotic instruments, with the aim of determining whether the recommended cleaning, disinfection and sterilisation process is adequate to achieve safe usage in subsequent patients. None of the 20 instruments showed viable micro-organisms, therefore the robotic instruments were considered sterile, and suitable for re-use. We recommend our protocol to other hospitals, to be used as an essential control element in the assessment of their unique reprocessing technique for robotic instruments.

Abstract:Objective To approach washing quality for medical instrument,and study preferred cleaning method of medical instrument.Methods To adopt test paper of occult blood for washing quality of medical instrument in spot.Results Medical instrument with soaking in enzymatic detergent plus washing of ultrasonic washing unit,eligible rate of occult blood test is 91.60%;medical instrument with soaking in hydrogen peroxide plus washing of automatic rinser,eligible rate of occult blood test is 89.47%;medical instrument with soaking in enzymatic detergent plus manual washing,eligible rate of occult blood test is 98.44%.It's showed medical instrument with dry bloodstain to amply soaking in enzymatic detergent plus washing of ultrasonic washing unit,washing quality eligible rate is 89.47%;to amply soaking in 30 g/L hydrogen peroxide plus washing of automatic rinser,washing quality eligible rate is 100%.Conclusions The washing quality is better when cleaning of contaminated medical instruments by soaking in enzymatic detergent plus ultrasonic washing unit,and good at cleaning of medical instruments with dry bloodstain by soaking in hydrogen peroxide plus automatic rinser.
Management Quality of Surgical Instrument and Influence of Cleaning and Sterilization on the Surgical Outcomes of the Patient: A Review

Abstract:When a medical device or surgical instrument comes into contact with the patient's sterile tissue or mucous membrane during the various processes, the risk of introducing infections into the patient's body increases. Furthermore, an infection may be transmitted from one patient to another, from a patient to a member of the medical staff and vice versa or from the environment to the patient via improperly sterilized or disinfected equipment. A number of outbreaks and diseases have been documented in the hospital setting as a result of poorly sanitized devices. As a result, adequate disinfection procedures for medical and surgical items are required in all healthcare facilities. It is equally the responsibility of healthcare providers to reduce and eliminate such infections. Each hospital should have its own standards for sterilization and disinfection of equipment based on the intended use of medical devices and associated infections. In order to reduce the risk of both endogenous and exogenous infections, infection control procedures must be implemented in general practice. Using a formulation containing alcohol alone or in combination with other agents to properly wash hands after each patient's checkup and before any procedure reduces the likelihood of transmitting infections to and from patients. Sterilization and disinfection are the most important aspects of infection control. The most common sterilization methods are steam sterilizers and ethylene dioxide sterilizers. Trash generated during practice should be handled according to protocol and rules, as it may be a source of nosocomial infections. Trained personnel are required to carry out these procedures.

Abstract:Digestive endoscopy is an important technique for the diagnosis and treatment of digestive system disease. To assure medical safety, a digestive endoscope must be cleaned and disinfected before its use in an operation on the next patient. The most common treatment procedure on a digestive endoscope is high-level disinfection. The potential risk associated with digestive endoscopes is always the focus of endoscopic management in clinical practice. In this study, a polluted pancreatic and biliary endoscope after surgery was cleaned and disinfected multiple times with the standard procedure but still tested positive for Pseudomonas aeruginosa culture, which is very rare and has not been reported in China or abroad. Abstract:Our study assessed the factors that influence the resistance of blood residues on dental surgical instruments to washer-disinfector-based cleaning procedures in a clinical setting. The use of 2 additional cleaning methods-presoaking and scrubbing by hand-and the use of newer and/or less structurally complex instruments significantly increased the efficacy of washer-disinfector cleaning.

Abstract:OBJECTIVE To study the effect to type WAYWIN 2000 automatic sterilizer for endoscope applying in the operative section. METHODS Put the used endoscopic instruments into the system WAYWIN 2000, and then process them with computer to realize automatic cleaning and disinfection within 30 minutes. RESULTS The system of WAYWIN 2000 was highly effective in sterilizing, easy to operate with time and labor saving in its procedures, no obvious damages or poisons were found. CONCLUSIONS Using the system WAYWIN 2000, we can clean and disinfect the endoscopic instruments effectively, especially in one after another operation. Therefore, we could increase the utilization of these expensive endoscopes, and control the cross infection and hospital infection safely and effectively.

Abstract:Objective:To improve hospital infection control and management of central sterile supply.Methods:The hospital sterilization and supply center establishes the standard workflow from equipment recovery,equipment cleaning,cleaning cleanliness inspection,device packaging,instrument sterilization to dissemination to strengthen quality monitoring,focusing on staff training,communication and coordination and sector departments.Results:Hospital infection control management level and effectiveness were improved significantly to achieve the integration of resources through saving the capital and operating costs of the relevant sections of the cleaning and disinfection of facilities to avoid re-contamination of surgical instruments on the staff and surroundings.Conclusion:The measures taken are feasible and productive.

Abstract:In this study, 897 surgical instruments used for comprehensive management of hand supply from January to December 2019 were selected as the control group. Similarly, 1086 surgical instruments administered with 6Sigma from January to December 2020 were selected as the observation group. By observing and comparing the differences between the two groups of patients in the operating room equipment cleaning pass rate and general indicators, other related pass rate, operating room equipment defects, and doctors' satisfaction with equipment, to explore the application value and influence of comprehensive nursing in operating room and disinfection supply center combined with 6Sigma management in operating room equipment management. The results show that the application of hand-supply integration combined with 6Sigma management has a good effect on operating room equipment management, which significantly improves the qualified rate of operating room equipment cleaning and the satisfaction of doctors to the equipment, and reduces the defects of operating room equipment, which has a certain reference value for operating room equipment management.

Abstract:Objective This study aimed to evaluate the current status of cleaning and disinfection management for digestive endoscopy, provide data for standardization processing techniques, and improve the quality of cleaning and disinfection. Methods Two reviewers independently and comprehensively searched the PubMed, Cochrane Library, EMBASE, Web of Science, CNKI, Wanfang, and CBM databases on February 1, 2023. The inclusion and exclusion criteria were strictly followed during the literature survey and data extraction. All observational studies detailing the current cleaning and disinfection management status for digestive endoscopy in hospitals were included. Meta-analysis was performed using STATA 16.0 software. Results After removing different articles, the meta-analysis finally included 54 articles associated with multiple countries. The authors favor auditing staffers to confirm compliance with guidelines. The meta-analysis results indicated a configuration rate of 76% (95% CI: 68–83%) for separate rooms designated for reprocessing; 79% (95% CI: 72–84%) for reprocessing rooms with adequate ventilation; 30% (95% CI: 24–36%) for automated endoscope washer-disinfectors; 68% (95% CI: 55–81%) for complete protective equipment usage; 90% (95% CI: 83–95%) for the configuration rate of endoscope and accessory storage cabinets; 50% (95% CI: 38–61%) for changing enzymatic-type detergents after each use; 51% (95% CI: 30–71%) for the use of purified or sterilized water for final rinsing; 80% (95% CI: 70–88%) for monitoring disinfectant concentration; 87% (95% CI: 80–93%) for microbial monitoring; and 44% (95% CI: 26–62%) for the usage of protective equipment. Conclusion The configuration of the automated endoscope washer-disinfector, non-standard cleaning and disinfection procedures, and a lack of occupational protection awareness among personnel responsible for cleaning and disinfecting digestive endoscopy were all apparent issues. It was suggested that all departments enhance their levels of management and supervision, standardize reprocessing procedures and quality control details, upgrade hardware facilities and spatial layouts, reinforce personnel training, and increase staff awareness of nosocomial infection risks.

Bacterial Contamination of Surgical Instruments Used at the Surgery Department of a Major Teaching Hospital in a Resource-Limited Country: An Observational Study

Abstract:Surgical instruments, be they disposable or reusable, are essential in any surgical procedure. Reusable surgical instruments should be properly sterilized or disinfected before use. However, the protocols are not always followed accordingly. This results in sterilization and disinfection failures, leading to a possible increase in the incidence of surgical site infections. This observational study report on bacterial contaminants identified instruments used for surgical procedures in a major teaching hospital in a resource-limited country. In total, 207 pre-sterilized surgical instruments and instrument parts used at three units-the general surgical theater, and the gastrointestinal (GI) endoscopy and urology endoscopy (uro-endoscopy) units-within the surgical department were randomly sampled and examined for bacterial contamination. Bacteria isolates were identified, and their antimicrobial susceptibility patterns were determined. Bacteria isolates that were identified included Citrobacter spp., Citrobacter freundii, Bacillus cereus, Staphylococcus hominis, and Staphylococcus aureus. Bacillus cereus was the most predominant bacteria isolated (30/61, 49.1%), and Staphylococcus hominis the least (1/61, 1.6%). In terms of the number of isolates from the three units examined, the uro-endoscopy unit recorded the highest followed by the general surgical theater and the GI endoscopy. However, there was no association between the various units and bacteria isolated, and no significant difference between the number of isolates among the various units (p = 0.9467, χ2 = 0.1095). In this study, even though CFU per device or device part counted was less than 20, bacteria isolated from the instruments used for a surgical procedure is of great concern considering that the setting of the study is a major teaching hospital. Multi-drug resistance was observed in almost all the isolated bacteria. Sterilization processes should be strictly adhered to, taking into consideration the length and temperature in order to reduce the risk of using contaminated instruments in these environments. It is therefore recommended that similar studies should be carried out in surgical departments at different levels of hospitals to ascertain the extent of this problem.

Abstract:The complexity of medical devices has increased over the past 10 years, and outbreaks of infections due to contaminated devices have focused attention on the need to adequately clean medical devices in order to ensure the adequacy of disinfection and sterilization. There has been a paradigm shift in reprocessing of medical devices, with increased emphasis on a quality management systems approach that requires validated cleaning instructions from manufacturers and ongoing monitoring by reprocessing personnel to ensure adequacy of cleaning. This article reviews the current issues related to medical device reprocessing and summarizes the approaches used for monitoring cleaning efficacy for surgical instruments and flexible endoscopes.
Surveillance of cleaning effects of rented surgical instruments from orthopedics department of 9 hospitals in Shanghai

Abstract:OBJECTIVE To understand the cleaning effects of the rented surgical instruments from orthopedics de-partments in Shanghai .METHODS In Dec 2011 ,the rented surgical instruments that were used frequently for sur-geries in the orthopedics departments of 9 hospitals in Shanghai were randomly collected ,meanwhile ,the surgical instruments cleaned by the hospitals themselves were also included ,then the monitoring was performed by using 4 methods ,including the residual blood ,residual protein ,ATP ,and bacterial culture counts .RESULTS A total of 1405 pieces of surgical instruments have been monitored ,including 1115 pieces of rented surgical instruments and 290 hospital-hold instruments .The positive rate of blood residue test of the rented instruments was 35 .7% ,the hospital-hold instruments 4 .3% (OR=12 .39);the positive rate of protein residue of the rented instruments was 59 .1% ,the hospital-hold instruments 32 .5% (OR=3 .00) .The ATP logarithm value of the rented instruments was (3 .19 ± 0 .89) ,the hospital-hold instruments (1 .71 ± 0 .47)(P<0 .001) .The median bacterial colony counts of the rented instruments was 165 .69 CFU/cm2 ,the hospital-hold instruments 12 .36 CFU/cm2 ,and the rank sum test showed that the P value was less than 0 .001 .CONCLUSION The cleaning of the rented instrument is far to be qualified before the sterilization ,and the qualified rates of the blood residue test ,protein residue test ,ATP test ,and bacterial colony counts of the rented instruments are lower than those of the hospital-hold instruments .

Abstract:Background Robot-assisted total joint arthroplasty (robotic-TJA) has become more widespread over the last 20 years due to higher patient satisfaction and reduced complications. However, robotic-TJA may have longer operative times and increased operating room traffic, which are known risk factors for contamination events. Contamination of surgical instruments may be contact- or airborne-related with documented scalpel blade contamination rates up to 9%. The robot arm is a novel instrument that comes in and out of the surgical field, so our objective was to assess whether the robot-arm is a source of contamination when used in robotic-TJA compared to other surgical instruments. Methods This was a prospective, single-institution, single-surgeon pilot study involving 103 robotic TJAs. The robot-arm was swabbed prior to incision and after closure. Pre- and postoperative control swabs were also collected from the suction tip and scalpel blade. Swabs were incubated for 24 hours on tryptic soy agar followed by inspection for growth of any contaminating bacteria. Results A contamination event was detected in 10 cases (10%). The scalpel blade was the most common site of contamination (8%) followed by the robot-arm (2%) and suction tip (0%). Discussion Contamination of the robot-arm during robotic-TJA is minimal when compared to contamination of the scalpel blade.

Abstract:OBJECTIVE:To compare the cleaning effect of automatic cleaning machine and manual cleaning on coupling type surgical instruments.METHODS:A total of 32 cleaned medical instruments were randomly sampled from medical institutions in Putuo District medical institutions disinfection supply center. Hygiena System SUREII ATP was used to monitor the ATP value, and the cleaning effect was evaluated.RESULTS:The surface ATP values of the medical instrument of manual cleaning were higher than that of the automatic cleaning machine.CONCLUSION:Coupling type surgical instruments has better cleaning effect of automatic cleaning machine before disinfection, the application is recommended.

Abstract:Surgical site infections are the most common infections acquired at hospitals, with Staphylococcus aureus being the main agent due to its ability to adhere to surgical instruments. This study aimed to develop a hydrodynamic system associating ozonated water and ultrasound to disinfect surgical instruments. The equipment consists of a reservoir with sinuous internal geometry to accommodate instruments, from where ozonated water comes out and flows through the circuit with the aid of a pump. To assess its efficacy, microbiological evaluation was performed on scalpel handles experimentally contaminated with S. aureus . The hydrodynamic system promoted effective disinfection, obtaining 3-log of bacterial reduction with an applied ozone dosage of 3 mg O 3 /cm 2 in seven minutes. The FEG-SEM images showed that the ultrasound (40 KHz, 50W) associated with the hydrodynamic circuit was sufficient to remove almost all bacteria adhered to the instrument ́s surface, enabling ozone to perform high-level disinfection of the material. Based on this preliminary study, it can be concluded that the hydrodynamic system developed was effective in disinfecting surgical instruments, demonstrating the synergy of the two techniques. In addition, it is a system that does not produce harmful chemical waste to the environment and can be utilized for thermo-sensitive materials.

Abstract:Abstract It is well known that the common goal of all central sterile supply departments (CSSDs) is to prevent healthcare-associated infections. Such infections entail high costs to society, not only economic but also social. Therefore, delivering safe medical devices and guaranteeing a positive contribution to the control of healthcare-associated infections form the main responsibilities of a CSSD. The monitoring of the effectiveness of medical device cleaning processes is highly recommended. However, ensuring a flawless environment for the preparation, assembly, and packaging of medical devices and clean handling of sterilized items is crucial to achieving the goal of safe medical devices. This study analyzed not only the cleanliness of surgical instruments but also two critical aspects of the surrounding environment: the cleanliness of work surfaces and the cleanliness of workers' hands. To evaluate the cleanliness of surgical instruments, two methods were used: the adenosine triphosphate (ATP) detection method and a residual protein test. It was not the intention of this work to make an exhaustive comparison of these methods. The ATP bioluminescence method was also used for monitoring the cleanliness of work surfaces and workers' hands. The aims of this study were to establish the most suitable method of evaluating the cleanliness of reusable medical devices in the CSSD and to assess the quality of the environment. Assessing the surgical instruments, work surfaces, and staff hands for cleanliness allowed the identification of possible contamination sources and to correct them by improving cleaning/disinfection protocols. Furthermore, the use of ATP monitoring tests of workers' hands highlighted the importance of staff compliance with good practice guidelines. Thus, these results have a positive impact on the CSSD quality system and, consequently, on patient safety.

Abstract:Background: Using a rotary instrument or ultrasonic instrument for tooth preparation is a basic operation in the dental clinic that can produce a significant number of droplets and aerosols. The dental droplet and aerosol can lead to the transfer of harmful germs. The goal of this study was to analyze the properties of microbiological aerosol created by droplets and aerosol generated by three common tooth-preparation instruments. Methods: Streptococcus mutans UA159 was used as the biological tracer to visualize the droplets and aerosols. The passive sampling method was used to map the three-dimensional spatial distribution and the six-stage Andersen microbial sampler (AMS) was used as the active sampling method to catch aerosol particles at a specific time. Results: The aerosol concentration is related to instruments, three-dimensional spatial distribution, and dissipation time. Most aerosols were generated by air turbines. More microorganisms are concentrated at the 1.5 m plane. The majority of the post dental procedure contamination was detected within the 0-10-min period and it decreased rapidly within 30 min. Conclusion: This study is conducive to the proposal and improvement of relevant infection control measures in dental procedures and provides a basis for the assessment of measures, reducing the risk of nosocomial infection.

Methods: Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, we searched 4 databases (ie, PubMed, EMBASE, OVID, CINAHL) for empirical studies on operating-room cleaning and disinfection. Studies were categorized based on their objectives and designs and were coded using the SEIPS model. The quality of randomized controlled trials and quasi-experimental studies with a nonequivalent groups design was assessed using version 2 of the Cochrane risk-of-bias tool for randomized trials. Results: In total, 40 studies were reviewed and categorized into 3 groups: observational studies examining the effectiveness of operating-room cleaning and disinfections (11 studies), observational study assessing compliance with operating-room cleaning and disinfection (1 study), and interventional studies to improve operating-room cleaning and disinfection (28 studies). The SEIPS-based analysis only identified 3 observational studies examining individual work-system components influencing the effectiveness of operating-room cleaning and disinfection. Furthermore, most interventional studies addressed single work-system components, including tools and technologies (20 studies), tasks (3 studies), and organization (3 studies). Only 2 studies implemented interventions targeting multiple work-system components. Conclusions: The existing literature shows suboptimal compliance and inconsistent effectiveness of operating-room cleaning and disinfection. Improvement efforts have been largely focused on cleaning and disinfection tools and technologies and staff monitoring and training. Future research is needed (1) to systematically examine work-system factors influencing operating-room cleaning and disinfection and (2) to redesign the entire work system to optimize operating-room cleaning and disinfection.
