Abstract:Objective To study the change of quality of life after operation for patients with portal hypertension in order to provide evidence for clinical nursing care.Methods The value of life quality for 37 patients with portal hypertension were studied by Gastrointestinal Quality of Life Index(GLQI)before operation and from two weeks to two years after the operation.Results The average GLQI was(101.2±12.2)before operation,while(93.7±13.5) and(93.3±9.4) in 2w and 5w after operation respectively,which were declined obviously compared with pre-operation,P0.05.There was no difference between the GLQI before operation and 10w to 16w after operation in which GLQI was 94.3 to 97.6,P0.05.The GLQI maintained between 99.5 and 102.3 6w to 2y after operation.Conclusions 2 w to 5 w after operation is the most important period for clinical nursing,when patients had the lowest GLQI.GLQI increased after 5w,and reached the level before operation at 16w.

Abstract:Abstract Background With increasing importance, health-related quality of life (HRQoL) has become a crucial outcome measure of cardiac surgery. The aim of this study was to assess the dynamics of HRQoL change within 12 months after surgery and to identify predictors of deterioration in physical and mental health. Methods The cohort of this prospective study included 164 consecutive patients who underwent elective surgery. HRQoL was assessed on the basis of the Short-Form 36 questionnaire at three different times: upon admission and at 3 and 12 months after surgery. The minimal clinically important difference (MCID) was used to determine whether the surgery resulted in deterioration of HRQoL. Results In general, physical and mental health status improved within the first year after cardiac surgery. However, after 12 months, 7.9 and 21.2% of patients had clinically significant poorer physical (PCS) and mental component summary (MCS) scores, based on the MCID approach. The results of multivariate analysis identified preoperative health status, age < 70 years, coronary artery bypass grafting, and a previous neurological event as predictors of deterioration in postoperative HRQoL. The greatest risks for deterioration were higher preoperative PCS and MCS scores. Conclusion Although we were able to demonstrate a general improvement in the HRQoL following cardiac surgery, in one-fifth of patients, there was no recovery of mental health status even after 1 year. As this effect is mainly determined by preoperative functional status, HRQoL should be an integral part of medical consultation, especially in younger patients with a positive perception of quality of life.

Abstract:Background Helicobacter pylori CagA strains could increase the risk for atrial fibrillation in patients with coronary artery disease Methods Serological status for H. pylori CagA using enzyme-linked immunosorbent assay, C-reactive protein, total leucocytic count and atrial size were determined in 185 coronary artery disease patients (with and without atrial fibrillation) and 80 healthy subjects (control). Results CagA strain showed a higher prevalence in the atrial fibrillation group. Atrial dimension and C-reactive protein (independent predictors of atrial fibrillation) were significantly increased in the CagA seropositive subgroup Conclus ons There is a strong liaison between H. pylori CagA infection and atrial fibrillation in coronary artery disease. Increased C reactive protein and atrial size in atrial fibrillation patients may reflect atrial inflammatory remodeling.
A Study on Quality of Life in Patients with Coronary Artery Bypass Grafting Surgery

Abstract:Objective To assess the perioperative change in Quality of Life(QoL)in patients who underwent CABG surgery. Methods The Chinese version of the SF-36 and SAQ were sent to participants at baseline and three and six months after CABG sur- gery.Results Angina stability score,one of the five SAQ domains,was lowest and postoperative SAQ domains scores were with sig- nificant improvement from baseline.Many of the dimensions of the SF-36 in postoperative patients were better than baseline.The SF- 36 was also used to evaluate in groups ONCAB and OPCAB,but no difference of the SF-36 subscale scores between the two groups was observed.Conclusion SAQ domains scores were significantly improved in three months and increased further in six months.Many of the dimensions of the SF-36 in postoperative patients were improved than baseline.No difference of the SF-36 subscale scores between the groups of ONCAB and OPCAB was observed postoperatively.
Severity of angina as a predictor of quality of life changes six months after coronary artery bypass surgery

Abstract:Background: Although the fact that chest pain has a negative influence on the quality of life is well known, it is not completely clear whether the preoperative severity of angina can be a predictor of the quality of life change after coronary artery bypass grafting (CABG). Methods: We studied 243 consecutive patients who underwent elective CABG. The Nottingham Health Profile Questionnaire part 1 was used as the model for determination of quality of life. We distributed the questionnaire to all patients before and six months after coronary artery bypass surgery. Two hundred and twenty-six patients filled in the postoperative questionnaire. Severity of angina was estimated by Canadian Cardiovascular Society (CCS) classification of angina. Results: Quality of life (before and after CABG surgery) in all sections was significantly worse in patients with higher CCS angina class (p < 0.001). The CCS angina class was 1.89 +/- 0.97 at baseline and improved to 0.46 +/- 0.75 (p < 0.001) after CABG. Six months after the operation, quality of life significantly improved in patients with all classes of angina (p < 0.01). The improvement in quality of life was related to higher CCS angina class in sections of physical mobility (r = 0.4, p < 0.001), energy (r = 0.31, p < 0.001), and pain (r = 0.48, p < 0.001). High CCS angina class before CABG was an independent predictor of quality of life improvement after coronary artery bypass surgery in sections of physical mobility (p = 0.005; odds ratio [OR] = 2.11; confidence interval [CI] 1.25 to 3.55), energy (p = 0.021; OR = 1.77; CI 1.09 to 2.87), and pain (p < 0.001; OR = 3.99; CI 2.2 to 7.22). Conclusions: Patients with higher CCS angina class had worse preoperative and postoperative quality of life. Patients with preoperative higher CCS angina class had greater improvement in sections of physical mobility, energy, and pain. High CCS angina class before CABG was the independent predictor of quality of life improvement six months after CABG.
Impact of post-hospital cardiac rehabilitation on the quality of life of patients after surgical treatment for coronary artery disease

Abstract:INTRODUCTION: The quality of life of cardiovascular patients is worse than that of healthy individuals. Pain, dyspnea, and fatigue impair daily activities, which in turn leads to decreased life satisfaction and emotional deterioration. The objective of comprehensive treatment, including early rehabilitation, is to reduce the risk of complications, progression, and recurrence of the disease, as well as to restore, maintain, or improve all aspects of the patient's quality of life to the greatest extent possible.AIM: To assess the quality of life of patients who underwent surgical treatment for coronary artery disease and completed the second phase of cardiac rehabilitation.MATERIAL AND METHODS: The study enrolled 100 patients undergoing early cardiac rehabilitation in stationary conditions after coronary artery bypass grafting. The group included 21 women and 79 men; their age ranged from 40 to 75 years. The study employed a questionnaire of our own design.RESULTS: Post-hospital cardiac rehabilitation resulted in quality of life improvements, especially with regard to psychological aspects, reducing the occurrence of anxiety from 44% to 21%. In addition, the rehabilitation program reduced the occurrence of subjective ailments such as chest pain (from 68% to 10%), fatigue (from 40% to 33%), and dyspnea (from 50% to 12%).CONCLUSIONS: Despite the achievement of quality of life improvements among patients after surgery coronary artery by-pass grafting and early cardiac rehabilitation, the result is not fully satisfactory. Systematic treatment and education should be continued to further improve the health of patients.

Abstract:OBJECTIVETo assess the value of European system for cardiac operative risk evaluation (EuroSCORE) in predicting quality of life in patients post coronary artery bypass graft surgery (CABG).METHODSA total of 387 patients underwent CABG in our institute from December of 2002 to December of 2007 were assessed by EuroSCORE before operation. Health-related quality of life (QoL) was estimated postoperatively with Seattle angina questionnaire (SAQ), Nottingham healthy profile (NHP) and Duke activity status index (DASI) in order to evaluate the value of EuroSCORE for predicting quality of life in patients post CABG.RESULTSThere were statistically significant but weak correlations between postoperative QoL score and preoperative EuroSCORE score (r: 0.010 - 0.276). Emotional and psychological experience subgroup analysis showed better predictive value of EuroSCORE score on postoperative QoL score in improved physical functioning subgroups (r > 0.2). Linear regression analysis showed that EuroSCORE score was significant but weakly (r(2) < 0.1) correlated with postoperative QoL score (P < 0.05).CONCLUSIONPreoperative EuroSCORE score is weakly correlated with postoperative QoL score in patients post CABG.

Abstract:OBJECTIVE:To evaluate the quality of life in patients who underwent laparoscopic and open cholecystectomy for chronic cholecystolithiasis. METHODS:A prospective survey was made on 25 patients receiving laparoscopic cholecystectomy (LC group) and 26 patients receiving open cholecystectomy (OC group). The quality of life was measured with the gastrointestinal quality of life index (GLQI) preoperatively, and at 2, 5, 10 and 16 weeks after the operation. RESULTS:The mean preoperative GLQI scores of all dimensions of the quality of life were 112.5 and 110.3 in the LC and OC groups respectively. In the LC group, the quality of life was not considerably reduced at 2 weeks after operation, with a mean GLQI score of 110. There was a significant improvement both in total mean score and in the aspects of symptomatology, emotional and physiological status from 5 to 16 weeks after LC operation. In the OC group, the GLQI score reduced to 102.0 at 2 weeks after surgery (P < 0.05). There were significant reductions in the aspects of symptomatology, physiological and social status as well. The GLQI scores reached to the preoperative level of 115.6 at 10 weeks after the operation (P > 0.05). The patients experienced significant improvements of GLQI at 16 weeks after OC operation (P < 0.01 or P < 0.05). The LC group showed better GLQI scores than did the OC group for up to 10 weeks postoperatively (P < 0.05). CONCLUSIONS:LC is betler or more rapidly than OC is improving the quality of life postoperatively. The assessment of the quality of life is valuable for measuring the outcome of surgical treatment.

Conclusions: LC can improve the quality of life postoperatively better and more rapidly than OC. The assessment of quality of life assessment is a valid method for measuring the effects of surgical treatment.
Anxiety and Quality of Life Outcomes After Coronary Artery Bypass Graft Surgery - A Prospective Cohort Study

Abstract:Improvements in physical and adaptive psychosocial challenges after Coronary artery bypass grafting (CABG) is unclear to what extent these outcomes impact patients' health-related quality of life. This study was to determine the quality of life among patients 3 months after CABG surgery. More specifically, the study examines the contribution of a set of variables on the quality of life. A prospective cohort study was performed over 3 months among 219 adult patients prepared for elective Coronary Artery Bypass Grafting. The data on the baseline measurements Short Form-36 to measure self-reported health-related quality of life and the state-trait anxiety inventory scale to assess anxiety were collected two days before and three months after CABG surgery. The Pearson correlation coefficient was adopted to examine the relationship between confounding, predictor, and dependent variables. Shapiro-Wilk test tested the normality of the distribution of numerical variables. A 2-tailed level of P-value < 0.05 was set to be statistically significant for all analyses. Mean preoperative postoperative physical and mental component score was 34.57 ± 9.6, 43.53 ± 7 and 54.87 ± 1.19, 51.65 ± 9.67, respectively, indicating poor quality of life. Preoperative anxiety uniquely explained with the variation 32.1% and 29.9% and it significantly predicts postoperative physical health quality of life as (β = .535, t = 8.433, P < 0.001) and postoperative mental health quality of life as (β = .475, t = 7.147, P < 0.001) respectively. Significant improvement in physical health over the 3 months was confirmed, but mental health-related quality of life is unconvincing with the substantial contribution of anxiety.

Abstract:BACKGROUND: The purpose of this study was to explore health-related quality of life (HRQOL) in patients with advanced heart failure undergoing heart transplantation evaluation. The overall aim of the study was to determine whether patients' demographic characteristics, functional status, neuroticism, social network, social support, spirituality, and time since symptom onset are related to the physical and mental components of a patient's HRQOL.METHODS: A descriptive, correlational design was used. Patients (N = 61) were recruited from 2 university-affiliated, outpatient, heart failure programs. Data were collected from chart review, a 6-minute walk, and patient-completed instruments. HRQOL, including physical and mental health components, was assessed with use of the Medical Outcomes Study 36-Item Short Form Health Survey (SF-36).RESULTS: Demographic characteristics, functional status, neuroticism, social network, social support, spirituality, and time since symptom onset explained 26% of the variability in the physical health component of HRQOL and 44% of the variability in the mental health component of HRQOL in patients with advanced heart failure. In analyzing the data for the most parsimonious model, New York Heart Association classification, 6-minute walk distance, and neuroticism explained 49% of the variability in the mental health component of HRQOL.CONCLUSIONS: New York Heart Association classification, 6-minute walk distance, and neuroticism are related to the mental health component of HRQOL and can be easily included in the assessment of patients with heart failure who are undergoing heart transplantation evaluation. The findings of the current study require replication but may be used to identify patients with heart failure who are potentially at risk for reduced HRQOL.

Abstract:Abstract Background Approximately 1 out of 11 patients with non-ST-segment elevation myocardial infarction (NSTEMI) undergoing percutaneous coronary intervention (PCI) have a history of coronary artery bypass (CABG) surgery. Most studies investigating patients treated with PCI with prior CABG have focused on harder outcomes such as morbidity, mortality and complications. However, quality of life (QoL) is increasingly recognized as an important outcome measure after PCI. Purpose To determine QoL after revascularisation with PCI in patients with and without a history of CABG. Methods CONCARDPCI, a prospective multicenter cohort study including 3417 consecutive patients revascularised with PCI, was conducted at seven high-volume referral PCI centres in Norway and Denmark. Of these, 312 had a history with CABG. Baseline clinical data were collected from patient ́s medical records and national registers. Sociodemographic characteristics were self-reported. Patient-reported outcomes measures were collected at baseline, 2-, 6- and 12-months after discharge. QoL and health status were measured with the World Health Organization Quality of Life Brief Version (WHOQOL-BREF), RAND-12-Item Short Form Health Survey (RAND-12) and the Seattle Angina Questionnaire 7 (SAQ-7). Results Patients with prior CABG were older (mean age 71 vs 65, p<0.001), more often men (84% vs 78%), and had more comorbidities. They were less likely to be current smokers (6% vs 18%, p=0.033) and less likely to be working full time (15% vs 33%). Patients with prior CABG reported a lower QoL on all questionnaires at all measuring time points compared with the no CABG group. Both groups had a significant improvement in the physical and mental component scores from baseline to two months on RAND-12 (p<0.001). In addition, an improvement was found on physical limitation from baseline to two months on SAQ-7 (p<0.001). For both groups QoL was best at baseline using WHOQOL-BREF. Conclusion QoL improved in patients with and without a history of CABG throughout the 12-month follow-up period. However, patients with prior CABG scored worse. These findings are important for future clinical decision-making regarding rehabilitation programs after PCI.SAQ-7 mean domain scores over time

Patients' expectation of postoperative course and satisfaction following cardiac surgery

Abstract:Introduction: In patients undergoing cardiac surgery, preoperative concerns, expectations of the impact of surgery, anticipated recovery timelines, and pre- and postoperative education, which impact recovery and quality-of-life, are not well documented. These factors are important with the increase in virtual consultations, the availability of internet-based information and increased use of minimally invasive surgical procedures. Methods: Patients who underwent cardiac surgery between January 2016 and December 2019 took part in an online survey examining preoperative concerns, information provision, use of digital channels, satisfaction with surgery, impact on health and resumption of daily activity. 80 patients completed the survey. Results: There was a high rate of overall post-surgical satisfaction (86%); 71% of respondents reported an improvement in physical health, 45% in mental health and 70% in their quality-of-life. The usefulness of information provided by the National Health Service varies across different stages of the patient experience. Although approximately 90% of respondents found the information provided at each stage at least 'somewhat' helpful, the proportion who found the information 'very' helpful was lower (68% for pre-procedure; 55% for post-discharge). The majority (79%) said that they felt prepared for their operation. Survey responses highlighted areas of lower understanding, including survival rate, levels of postoperative pain, duration of hospital stay and when the patient could return to normal physical activity. Conclusions: Levels of satisfaction with the outcomes of heart surgery are high, and the majority of patients report positive health outcomes. However, there is room for improvement in patients' understanding of survival rate and level of pain post-procedure. There is also a clear desire among patients for a more surgical team-based face-to-face consultation.
Railroad: virtual infrastructure for data dissemination in wireless sensor networks

Abstract:In this paper, we present Railroad, a data dissemination architecture for large-scale wireless sensor networks. Railroad system proactively exploits a virtual infrastructure called Rail, which is an area where all the metadata of event data are stored. There is only one Rail in the network and it acts as a rendezvous area of the events and the queries. Rail is placed in the middle area of the field so that every node can easily access it. Once a query is issued, it circulates around Rail and searches relevant data stored in Rail. When a relevant metadata is found, the source node of the data transmits the corresponding data to the sink node which has issued the query. By using Rail, Railroad achieves a scalable and energy-efficient data dissemination architecture under dynamic conditions with multiple mobile observers and targets. We evaluate and compare the communication cost and the hot spot message complexity of Railroad with previous approaches.
Factors predicting quality of life for coronary artery disease patients after percutaneous coronary intervention

Abstract:Abstract Background: A clear understanding of factors affecting patients’ perception of quality of life (QOL) would be useful for improving continuous care in coronary artery disease (CAD) patients. Objective: To examine the causal relationships between cardiac self-efficacy, social support, left-ventricular ejection fraction (LVEF), angina, dyspnea, depression, vital exhaustion, functional performance, and QOL in CAD patients experiencing postpercutaneous coronary intervention (post-PCI). Methods: We used a research survey for causal analysis design to explore the theoretical linkage, guided by the revised Wilson and Cleary model, between QOL interest variables and patient QOL. The 303 subjects were all post-PCI CAD patients. All participants completed the following surveys: (1) a demographic data questionnaire, (2) a QOL Index (Cardiac version IV), (3) the Center for Epidemiologic Studies Depression Scale, (4) the Cardiac Self-efficacy Scale, (5) the Social Support Questionnaire, (6) the Rose Questionnaire for angina, (7) the Rose Dyspnea Scale, (8) the SF-36: vitality subscale, and (9) the Functional Performance Inventory Short-Form, with reliability ranging from 0.72 to 0.98. Data were analyzed using a linear structural relationship analysis. Results: The postulated model was found to fit the empirical data and explained 54% of the variance in quality of life (χ 2 = 1.90, df = 3, p = 0.59, χ 2 /df = 0.63, root mean square error of approximation = 0.00, Goodness of Fit Index = 0.99, Adjusted Goodness of Fit Index = 0.98). Social support, depression, and vital exhaustion were found to significantly and directly affect the QOL of post-PCI CAD patients. Cardiac self-efficacy was the only variable that had an indirect effect on quality of life (β = 0.21, p < 0.001). Conclusion: Health care providers should be aware of the significant effects of social support, depression, vital exhaustion, and self-efficacy on QOL, and develop appropriate nursing interventions to improve quality of life in post-PCI CAD patients.

Health-related quality of life and arrhythmia frequency following coronary artery bypass grafting surgery: a descriptive study

Abstract:Abstract Introduction Coronary Artery Bypass Grafting (CABG) surgery is the preferred method in Coronary Artery Disease, particularly in patients with multi-vessel disease and impaired left ventricular function. Besides the success of surgical intervention, achieving a good level of Health-Related Quality of Life (HRQoL) is the primary goal of the entire process. Objective The aim of our study is to evaluate HRQoL before and after CABG surgery and to assess the adherence to pharmacotherapy and the frequency of arrhythmias postoperatively during a 6- month follow-up period. Methods The study design involves a descriptive longitudinal study. The study was conducted in one public hospital and two private hospitals of Cyprus. The follow-up period for participants was determined at 6 months post-CABG surgery. HRQoL assessment was conducted using the MacNew questionnaire, and the evaluation of arrhythmias and other clinical data was performed using a Clinical Data Evaluation Form created for the study purposes. Results Our study included 103 people who had undergone CABG surgery. HRQoL decreased at 3 months post-CABG surgery compared to the preoperative period (Mean: 5.03, SD: ±1.02 vs Mean: 4.75, SD: ±1.00) but improved at 6 months post-surgery (Mean: 6.19, SD: ±0.67). Among the total of patients, 21 (20.6%) developed Atrial Fibrillation (AF) postoperatively. All patients with AF received immediate treatment, and sinus rhythm was restored. Additionally, 6 patients developed other types of arrhythmias, among whom 1 patient experienced cardiac arrest with an initial asystole rhythm and eventually passed away. Conclusion CABG significantly affects HRQoL, showing a temporary decline postoperatively followed by improvement. Arrhythmias and compliance with pharmacotherapy are crucial aspects of postoperative care.

Abstract:Purpose: The aim was to examine the predictors of improvement of quality of life after 2 years of coronary artery bypass grafting (CABG). Methods: In all, 208 patients who underwent the elective CABG at the Institute for Cardiovascular Diseases Dedinje in Belgrade were contacted and examined 2 years after the surgery. All patients completed Nottingham Health Profile Questionnaire part one. Results: Two years after CABG, quality of life (QOL) in patients was significantly improved in all sections compared to preoperative period. Independent predictors of QOL improvement after 2 years of CABG were found to be serious angina under sections of physical mobility [p = 0.003, odds ratio (OR) = 1.76, 95% confidence interval (CI) 1.21-2.55], energy (p = 0.01, OR = 1.63, 95% CI: 1.11-2.38), sleep (p = 0.005, OR = 1.65, 95% CI: 1.16-2.35), pain (p <0.001, OR = 2.43, 95% CI: 1.57-3.77), absence of hereditary load in energy section (p = 0.002, OR = 0.35, 95% CI: 0.18-0.68), male sex in the sleep section (p = 0.03, OR = 0.43, 95% CI: 0.20-0.93), and absence of diabetes in pain section (p = 0.006, OR = 0.27, 95% CI: 0.10-0.68). Conclusion: Predictors of improvement of QOL after 2 years of CABG are serious angina, absence of hereditary load, male sex, and absence of diabetes.
