Abstract:OBJECTIVES: Despite improvements in many domains of functioning, sexual quality of life often remains impaired following coronary artery bypass graft (CABG) surgery. This study examined associations among sexual quality of life, fear of sexual activity and receiving information from providers about sexual activity in CABG patients.METHODS: Participants completed a survey assessing sexual activity, mental health and physical health at baseline (3-5-day post-surgery; n=60) and 2-month post-surgery (n=42).RESULTS: Sexual quality of life showed moderate difficulties at baseline and did not improve by follow-up (p values≥0.09). At follow-up, greater patient fear was associated with lower sexual quality of life in some domains; receiving information was related to lower fear (p values≤0.03) and greater sexual satisfaction and interest (p values≤0.04). Suggestive of mediation, there was a significant indirect effect of information on patient fear and of patient fear on sexual interest (p=0.05).CONCLUSIONS: Though data were cross-sectional, findings suggest that fears of sexual activity may play a role in lowering CABG patients' motivation for sexual activity and that receiving information from a medical provider may assist in hastening sexual rehabilitation. Prospective and intervention studies are needed to support findings.

Impact of Clinical and Sociodemographic Factors on Quality of Life Following Coronary Artery Bypass Grafting: A Mixed-Methods Study

Abstract:Introduction: Coronary artery bypass grafting (CABG) is an essential surgical management modality for patients with coronary artery disease. Health-related quality of life (HRQoL) has become important because of the significant decrease in the mortality rate associated with CABG. We aimed to explore the factors that affect the quality of life after CABG. Methods: This study used a descriptive correlational design to assess the determinants of HRQoL using the 36-item Short Form Health Survey questionnaire (SF-36). Patients who underwent CABG at King Abdulaziz University Hospital (KAUH), Jeddah, Saudi Arabia, between March 2015 and December 2021 were enrolled in this study. Overall, 275 participants were eligible for our study, of which 84 were found to be valid for analysis. Phone contacts were made directly with the patient after briefly explaining the study. Scores and clinical data were investigated using multivariable linear regression analysis. Results: Subscales of role limitations due to physical issues had the lowest mean scores, followed by vitality and general health (57.4 ± 44.7; 60.4 ± 25.6; 64.1 ± 22.6), respectively. However, social functioning (78.9 ± 29.0) and pain (75.1 ± 29.9) had the highest scores of all subscales. A history of congestive heart failure (CHF) was independently associated with lower scores for physical role limitations (p = 0.021), vitality (p = 0.001), general health (p< 0.001), and mental health (p = 0.011). Lower mental health scores were also predicted by being a widow (p = 0.030), whereas lower general health scores were predicted by being unemployed (p = 0.001) and having a peripheral vascular disease (PVD) (p = 0.043). Additionally, the development of postoperative complications was an independent predictor of lower physical functioning (p = 0.028) and vitality (p = 0.043). Regarding the number of grafts, cardiopulmonary bypass, and cross-clamp time, no significant impact was found on any of the SF-36 subscales (p> 0.05). Conclusion: The postoperative decline in HRQoL was attributed to comorbidities such as CHF and PVD, postoperative complications including bleeding and wound infection, as well as unemployment and widowed status. Therefore, choosing the appropriate patients for surgery and post-discharge follow-up may enhance HRQoL.
Anxiety and Quality of Life Outcomes After Coronary Artery Bypass Graft Surgery - A Prospective Cohort Study

Abstract:Improvements in physical and adaptive psychosocial challenges after Coronary artery bypass grafting (CABG) is unclear to what extent these outcomes impact patients' health-related quality of life. This study was to determine the quality of life among patients 3 months after CABG surgery. More specifically, the study examines the contribution of a set of variables on the quality of life. A prospective cohort study was performed over 3 months among 219 adult patients prepared for elective Coronary Artery Bypass Grafting. The data on the baseline measurements Short Form-36 to measure self-reported health-related quality of life and the state-trait anxiety inventory scale to assess anxiety were collected two days before and three months after CABG surgery. The Pearson correlation coefficient was adopted to examine the relationship between confounding, predictor, and dependent variables. Shapiro-Wilk test tested the normality of the distribution of numerical variables. A 2-tailed level of P-value < 0.05 was set to be statistically significant for all analyses. Mean preoperative postoperative physical and mental component score was 34.57 ± 9.6, 43.53 ± 7 and 54.87 ± 1.19, 51.65 ± 9.67, respectively, indicating poor quality of life. Preoperative anxiety uniquely explained with the variation 32.1% and 29.9% and it significantly predicts postoperative physical health quality of life as (β = .535, t = 8.433, P < 0.001) and postoperative mental health quality of life as (β = .475, t = 7.147, P < 0.001) respectively. Significant improvement in physical health over the 3 months was confirmed, but mental health-related quality of life is unconvincing with the substantial contribution of anxiety.
Determinants of Sexual Health and Sexual Quality of Life after Cardiovascular Surgeries: An Integrative Review

Abstract:Background and aim: Sexual health and sexual quality of life are key components of psychosocial adjustment after cardiac surgeries and are often linked with improving the general quality of life. Reviews have been conducted to highlight the associations between cardiovascular diseases and sexual dysfunctions, but no review reported determinants of sexual health and sexual quality of life in patients after cardiovascular surgeries. We aimed to comprehensively examine the determinants of sexual health and sexual quality of life among individuals with cardiovascular surgeries. Methods: Literature was searched within PubMed, CINAHL, Scopus, Web of Science, and OVID databases. In total, 816 records were identified from database searches, 279 records were screened, and 11 empirical studies were included for review. Relevant data were extracted using literature summary tables and synthesised using an inductive approach. Results: The core determinants of sexual health and sexual quality of life were type of surgery and comorbidities, fears and uncertainties regarding sexual activity, sexual health education and counselling, spousal relationship and communication, and demographic factors such as advanced age and literacy levels. Major surgeries performed were coronary artery bypass grafting (CABG) and heart valve surgeries. The data collection tools used to collect data for sexual health and sexual quality of life were the International Erectile Function Questionnaire (IEFQ), International Index of Erectile Function (IIEF), Female Sexual Function Index (FSFI), Sexual Knowledge CABG Scale (SKS-CABG), Sexual Quality of Life Questionnaire (SQOL), SKS-Myocardial Infarction Scale (SKS-MI), and Couple Communication Scale (CCS). Conclusions: Despite their importance, sexual health and quality of life are frequently overlooked during patient rehabilitation after cardiovascular surgeries. The lack of adequate education and counselling from healthcare professionals frequently leads to increased fear and uncertainties among individuals and their partners. Therefore, more person-centred educational and counselling approaches should be developed to address the sexual concerns of individuals and their partners.

Abstract:Abstract Background Cardiovascular diseases are one of the major causes of mortality in the world. According to the World Health Organization (WHO), approximately 17.9 million people die annually due to cardiovascular diseases worldwide. The most common cardiovascular disease is coronary artery disease (CAD), which involves the narrowing or blockage of the heart arteries as a result of atherosclerosis. Coronary Artery Bypass Grafting (CABG) is a common surgical approach for patients with coronary heart disease (CHD), which improves symptoms and survival, and expected to enhance Health-Related quality of life (HR-QoL). Patients, even after recovery from GABG, may have physical and psychological limitations that reduce HR-QoL. Aim The investigation of factors that affecting the HR-QoL of people after coronary artery bypass grafting (CABG). The secondary aim of the study was the investigation of demographic or clinical factors, physical activity, stress or depression may affect the HR-QoL of people after CABG. Methods This is a descriptive correlational study with a cross-sectional design. Data were collected from the cardiac surgery department of two large general hospitals in Cyprus after consent from both, the management and their ethics committee. The study sample (N=56) included all patients who had undergone CABG surgery from January to June 2022. At, the patients completed a questionnaire at three to six months postoperatively. The data collection tool included: Demographic and Clinical Characteristics Record Form, the SF12 Health Survey Tool for assessing the Health-Related Quality of Life (HR-QoL) of patients, the HADS Tool for evaluating the anxiety and depression levels of patients, and the IPAQ tool for assessing Physical Activity. Descriptive statistic was used for the analysis and presentation of the data and ANOVA analysis to investigate the factors that affect the health-related quality of life. Results Gender (64.2±19.3 vs. 49.8±11.9, p<0.05) and education level (78.5±6.7) (P<0.05) were both found to influence HR-QoL. Other factors that affect the HR-QoL of subjects after CABG are intense physical activity, increased anxiety levels, severe depression and patient's symptoms such as dyspnea (53.3±18.6 vs. 64.6±18.4, p<0.05) and orthopnea (45.4±17.4 vs. 65.6±17.3, p<0.05). Conclusion The identification of the factors that were found to affect the HR-QoL of patients after coronary artery bypass grafting enlighten health professionals where they should pay attention when they develop rehabilitation programs.

Abstract:Objective To observe the quality of life in geriatric patients of different genders following coronary artery bypass grafting.Methods Three questionnaires about the quality of life:Seattle Angina Questionnaire(SAQ),Nottingham Healthy Profile(NHP),Duke Activity Status Index(DASI),were given to 287 patients,who had undergone coronary artery bypass grafting surgery.Two hundred and seventy-three patients sent back the questionnaires.The patients were divided into geriatric patients group(166 cases) and control group(107 cases).Results In male patients,the scores of life quality between geriatric patients group and the control group had difference in three questionnaires(P0.05),and especially in exertional scale,treatment satisfaction,disease perception scale,energy level,pain,emotional reaction,sleep,social isolation,physical abilities(P0.05,P0.01).In female patients,the scores between geriatric patients group and the control group also had differences in three questionnaires(P0.05,P0.01),and especially in exertional scale and treatment satisfaction(P0.05,P0.01).Conclusion The geriatric patients had lower score about quality of life following coronary artery bypass grafting,especially in male patients.

Abstract:Objective: To compare morphological and hemodynamic parameters of skin microcirculation in the fingertip in patients with essential hypertension and normotensive control subjects. Design: Consecutive sample of patients. Methods: Digital capillary blood flow measurements under normal and cooled conditions were assessed by nailfold video capillaroscopy using the technique of flying spot. Results: There was a significant reduction in capillary density in hypertensive patients, compared with normotensive subjects There was a correlation between capillary density and mean diastolic blood pressure. After local cooling, the frequency of the blood flow stop was significantly higher in hypertensive patients. Conclusion: The finding of abnormal vasoconstriction in finger microcirculation in essential hypertension suggests a vasospastic tendency in the disease.

Abstract:Objective To assess the perioperative change in Quality of Life(QoL)in patients who underwent CABG surgery. Methods The Chinese version of the SF-36 and SAQ were sent to participants at baseline and three and six months after CABG sur- gery.Results Angina stability score,one of the five SAQ domains,was lowest and postoperative SAQ domains scores were with sig- nificant improvement from baseline.Many of the dimensions of the SF-36 in postoperative patients were better than baseline.The SF- 36 was also used to evaluate in groups ONCAB and OPCAB,but no difference of the SF-36 subscale scores between the two groups was observed.Conclusion SAQ domains scores were significantly improved in three months and increased further in six months.Many of the dimensions of the SF-36 in postoperative patients were improved than baseline.No difference of the SF-36 subscale scores between the groups of ONCAB and OPCAB was observed postoperatively.

Abstract:♦ Objective: Encapsulating peritoneal sclerosis (EPS) is a rare but life-threatening complication of peritoneal dialysis (PD). The optimal management of patients with EPS is uncertain. In the present study, we investigated differences in the expression of nuclear receptors [progesterone (PR), androgen (AR), vitamin D (VDR), and glucocorticoid (GCR)] in the human peritoneum. We also investigated estrogen receptor (ER), matrix metalloproteinase 9 (MMP9), and transforming growth factor β1 (TGFβ1) in the context of their potential role in tamoxifen therapy. ♦ Methods: We analyzed clinical and histologic characteristics of 72 peritoneal biopsy specimens (22 from EPS patients, 11 from PD patients, 15 from uremic patients, and 24 from control subjects undergoing hernia repair). For immunophenotyping, we used antibodies against VDR, GCR, ER, PR, AR, MMP9, and TGFβ1. ♦ Results: In human peritoneum, VDR and GCR are highly expressed (98.6% and 87.3% respectively). Except in the case of VDR (p = 0.0012), we observed no significant difference in receptor expression between the groups. Expression of ER and PR was sparse (11.4% and 31% respectively), with higher expression in women, and AR was absent. Minimal MMP9 expression and moderate TGFβ1 expression were observed in all groups. The differences between the groups were nonsignificant. ♦ Conclusions: Nuclear receptors are present in human peritoneum. Except in the case of VDR, the pattern for any one group is nonspecific. Glucocorticoids, vitamin D, and angiotensin converting-enzyme inhibitors or angiotensin II receptor blockers (via the vitamin D/angiotensin II pathway) might be suitable interventions for preservation of the integrity of the peritoneal membrane. The mechanism of action of tamoxifen is still not elucidated, ER expression in the peritoneum is sparse, and data about the studied pathways (MMP9, TGFβ) are inconsistent.

Abstract:Abstract Introduction Coronary Artery Bypass Grafting (CABG) surgery is the preferred method in Coronary Artery Disease, particularly in patients with multi-vessel disease and impaired left ventricular function. Besides the success of surgical intervention, achieving a good level of Health-Related Quality of Life (HRQoL) is the primary goal of the entire process. Objective The aim of our study is to evaluate HRQoL before and after CABG surgery and to assess the adherence to pharmacotherapy and the frequency of arrhythmias postoperatively during a 6- month follow-up period. Methods The study design involves a descriptive longitudinal study. The study was conducted in one public hospital and two private hospitals of Cyprus. The follow-up period for participants was determined at 6 months post-CABG surgery. HRQoL assessment was conducted using the MacNew questionnaire, and the evaluation of arrhythmias and other clinical data was performed using a Clinical Data Evaluation Form created for the study purposes. Results Our study included 103 people who had undergone CABG surgery. HRQoL decreased at 3 months post-CABG surgery compared to the preoperative period (Mean: 5.03, SD: ±1.02 vs Mean: 4.75, SD: ±1.00) but improved at 6 months post-surgery (Mean: 6.19, SD: ±0.67). Among the total of patients, 21 (20.6%) developed Atrial Fibrillation (AF) postoperatively. All patients with AF received immediate treatment, and sinus rhythm was restored. Additionally, 6 patients developed other types of arrhythmias, among whom 1 patient experienced cardiac arrest with an initial asystole rhythm and eventually passed away. Conclusion CABG significantly affects HRQoL, showing a temporary decline postoperatively followed by improvement. Arrhythmias and compliance with pharmacotherapy are crucial aspects of postoperative care.

Abstract:Background: Daily activities have been recommended to minimize the long-term complications of coronary artery bypass as one of the strategies to return to the normal activity level, the effectiveness of which needs further investigation. This study aims to determine the quality of life and activities of daily living one year after CABG. Methods: This cross-sectional study was performed on 206 patients who had undergone CABG for more than one year in 2018 in the north of Iran. The research instrument was a questionnaire including five sections, Data were analyzed using descriptive statistics and Chi2, Mann-Whitney U, Kruskal-Wallis tests, and a Logistic regression model. Result: The mean score of quality of life was 31.7 ± 2.04 of 12 to 48 possible scores. About Activities of Daily Living results showed 99.5% and 84.7% of the samples needed help with many of these activities respectively. The mean score of quality of life was significantly different based on sex (p < 0.018) and instrumental activity of daily living (p < 0.0001). A logistic regression model was used to determine the factors related to quality of life. The final model showed cross-clamp duration (OR = 0.33,p = 0.014), length of stay(LOS)in the intensive care unit(OR = 0.42,p = 0.05), and instrumental activities of daily living (OR = 0.08,p = 0.001) predicted patients' quality of life one year after coronary artery bypass grafting. Conclusion: Although more than half of the samples had a good average quality of life score, due to the lack of definitive treatment for coronary artery disease, it is suggested to consider predictive variables to help plan to improve the quality of life of these patients.
Severity of angina as a predictor of quality of life changes six months after coronary artery bypass surgery

Abstract:Background: Although the fact that chest pain has a negative influence on the quality of life is well known, it is not completely clear whether the preoperative severity of angina can be a predictor of the quality of life change after coronary artery bypass grafting (CABG). Methods: We studied 243 consecutive patients who underwent elective CABG. The Nottingham Health Profile Questionnaire part 1 was used as the model for determination of quality of life. We distributed the questionnaire to all patients before and six months after coronary artery bypass surgery. Two hundred and twenty-six patients filled in the postoperative questionnaire. Severity of angina was estimated by Canadian Cardiovascular Society (CCS) classification of angina. Results: Quality of life (before and after CABG surgery) in all sections was significantly worse in patients with higher CCS angina class (p < 0.001). The CCS angina class was 1.89 +/- 0.97 at baseline and improved to 0.46 +/- 0.75 (p < 0.001) after CABG. Six months after the operation, quality of life significantly improved in patients with all classes of angina (p < 0.01). The improvement in quality of life was related to higher CCS angina class in sections of physical mobility (r = 0.4, p < 0.001), energy (r = 0.31, p < 0.001), and pain (r = 0.48, p < 0.001). High CCS angina class before CABG was an independent predictor of quality of life improvement after coronary artery bypass surgery in sections of physical mobility (p = 0.005; odds ratio [OR] = 2.11; confidence interval [CI] 1.25 to 3.55), energy (p = 0.021; OR = 1.77; CI 1.09 to 2.87), and pain (p < 0.001; OR = 3.99; CI 2.2 to 7.22). Conclusions: Patients with higher CCS angina class had worse preoperative and postoperative quality of life. Patients with preoperative higher CCS angina class had greater improvement in sections of physical mobility, energy, and pain. High CCS angina class before CABG was the independent predictor of quality of life improvement six months after CABG.

Abstract:BACKGROUND: Selection of the optimum mode of coronary revascularization should not only be directed by technical outcomes, but should also consider patients' postprocedural health status, including symptoms, functionality, and quality of life.METHODS: Health status was analyzed and compared after percutaneous coronary intervention (PCI) and coronary artery bypass grafting (CABG) using the Seattle Angina Questionnaire (SAQ). The SAQ was administered to 475 patients (252 PCI and 223 CABG) preprocedure and then monthly for 6 months and again at 1 year. Differences in baseline characteristics were controlled by multivariable risk adjustment, and outcomes over time were compared using repeated-measures analysis of variance.RESULTS: In-hospital, 6-and 12-month clinical outcomes were not different; however, 25% of PCI patients required at least one reintervention during the study period, compared with only 1% of CABG patients (p < 0.001). Although physical function decreased for CABG patients at 1 month (p < 0.001), it improved and was better than the PCI group by 12 months (p = 0.008). Relief of angina was greater for CABG than PCI when analyzed over time (p < 0.001), principally due to the adverse effects of restenosis in the PCI group. Multivariable analysis confirmed that CABG independently conferred greater angina relief compared with PCI (p < 0.001). At 12 months postprocedure, quality of life had improved to a greater extent for CABG than PCI (p = 0.004).CONCLUSIONS: Over 12 months of follow-up, health status was improved to a greater extent for CABG patients than for PCI patients, primarily due to the adverse influence of restenosis after PCI.

[Quality of Life Five Years after Coronary Artery Bypass Grafting].

Abstract:OBJECTIVE:The quality of life (QOL) 5 years after coronary artery bypass grafting (CABG) was evaluated by NHP, SAQ and DASI questionnaires.METHODS:NHP, SAQ and DASI questionnaires were mailed to 287 patients who received CABG in our department between Jan 2001 and Jan 2002. The reliability and construct validity of the questionnaires and the influence factor of QOL were analyzed.RESULTS:Two hundred and seventy-three patients (95%) responded to the questionnaires. The reliability of three questionnaires about QOL was 0.81, 0.75 and 0.78, respectively. QOL at 5 years post CABG was significantly better in CCS I/II patients than CCS III/IV patients' according to SAQ. Exertional scale and disease perception scale according to SAQ, social isolation and physical abilities to NHP also significantly related to QOL 5 years post CABG.CONCLUSIONS:All 3 questionnaires had high reliability and statistical validity for evaluating QOL. CCS angina degree affect the quality of life in patients 5 years post CABG.

Abstract:Background. The study of psychoemotional characteristics and quality of life (QOL) in patients with CHD undergoing coronary artery bypass grafting (CABG) at different stages of treatment is crucial for enriching information about the characteristics of the psychological responses to illness and for the comprehensive analysis of the psychological aspects of recovery after CABG. The study was carried out at the Almazov National Medical Research Centre (St. Petersburg, Russia). The study sample comprised 65 patients with CHD preparing for CABG. Methods. Asthenic State Scale, Integrative Anxiety Test, The Symptom Checklist-90-R (SCL-90-R), and 36-Item Short Form Survey (SF-36). Results show that in the early postoperative periood, asthenia tends to intensify. State anxiety and its components decrease during the rehabilitation period after CABG. Both before and after CABG, patients have increased rates of psychopathological symptoms. A week after CABG, depressive and somatization symptoms intensify. 3 months after CABG, psychopathological symptoms tend to decrease, however, many indicators remain above the norm. Conclusion. The analysis of the QOL dynamics revealed the influence of the surgery on all QOL components. Psychological interventions for patients with CHD undergoing the CABG are necessary at all stages of the treatment and rehabilitation process.
Predictors of Quality of Life Improvement after 2 Years of Coronary Artery Bypass Surgery

Abstract:Purpose: The aim was to examine the predictors of improvement of quality of life after 2 years of coronary artery bypass grafting (CABG). Methods: In all, 208 patients who underwent the elective CABG at the Institute for Cardiovascular Diseases Dedinje in Belgrade were contacted and examined 2 years after the surgery. All patients completed Nottingham Health Profile Questionnaire part one. Results: Two years after CABG, quality of life (QOL) in patients was significantly improved in all sections compared to preoperative period. Independent predictors of QOL improvement after 2 years of CABG were found to be serious angina under sections of physical mobility [p = 0.003, odds ratio (OR) = 1.76, 95% confidence interval (CI) 1.21-2.55], energy (p = 0.01, OR = 1.63, 95% CI: 1.11-2.38), sleep (p = 0.005, OR = 1.65, 95% CI: 1.16-2.35), pain (p <0.001, OR = 2.43, 95% CI: 1.57-3.77), absence of hereditary load in energy section (p = 0.002, OR = 0.35, 95% CI: 0.18-0.68), male sex in the sleep section (p = 0.03, OR = 0.43, 95% CI: 0.20-0.93), and absence of diabetes in pain section (p = 0.006, OR = 0.27, 95% CI: 0.10-0.68). Conclusion: Predictors of improvement of QOL after 2 years of CABG are serious angina, absence of hereditary load, male sex, and absence of diabetes.

Abstract:Abstract Background Approximately 1 out of 11 patients with non-ST-segment elevation myocardial infarction (NSTEMI) undergoing percutaneous coronary intervention (PCI) have a history of coronary artery bypass (CABG) surgery. Most studies investigating patients treated with PCI with prior CABG have focused on harder outcomes such as morbidity, mortality and complications. However, quality of life (QoL) is increasingly recognized as an important outcome measure after PCI. Purpose To determine QoL after revascularisation with PCI in patients with and without a history of CABG. Methods CONCARDPCI, a prospective multicenter cohort study including 3417 consecutive patients revascularised with PCI, was conducted at seven high-volume referral PCI centres in Norway and Denmark. Of these, 312 had a history with CABG. Baseline clinical data were collected from patient ́s medical records and national registers. Sociodemographic characteristics were self-reported. Patient-reported outcomes measures were collected at baseline, 2-, 6- and 12-months after discharge. QoL and health status were measured with the World Health Organization Quality of Life Brief Version (WHOQOL-BREF), RAND-12-Item Short Form Health Survey (RAND-12) and the Seattle Angina Questionnaire 7 (SAQ-7). Results Patients with prior CABG were older (mean age 71 vs 65, p<0.001), more often men (84% vs 78%), and had more comorbidities. They were less likely to be current smokers (6% vs 18%, p=0.033) and less likely to be working full time (15% vs 33%). Patients with prior CABG reported a lower QoL on all questionnaires at all measuring time points compared with the no CABG group. Both groups had a significant improvement in the physical and mental component scores from baseline to two months on RAND-12 (p<0.001). In addition, an improvement was found on physical limitation from baseline to two months on SAQ-7 (p<0.001). For both groups QoL was best at baseline using WHOQOL-BREF. Conclusion QoL improved in patients with and without a history of CABG throughout the 12-month follow-up period. However, patients with prior CABG scored worse. These findings are important for future clinical decision-making regarding rehabilitation programs after PCI.SAQ-7 mean domain scores over time

Abstract:Objective: To examine the effect of changes in quality of life (QoL) and levels of sexual function on decisional regret after surgical treatment of localized prostate cancer. Patients and methods: Patients who decided to have a radical prostatectomy (RP) were assessed for health-related QoL using the general European Organization for Research and Treatment of Cancer C30 instrument and disease-specific prostate cancer module, and sexual function using the abbreviated International Index of Erectile Function-5 before and 1 year after RP. Decision control was measured before RP, and decisional regret 1 year afterward, using measures mailed to participants 1 year after treatment. Results: Of 130 respondents (mean age 62 years), 4% expressed regret over their decision to have surgery. Physical and social functioning, and finances, were compromised, while emotional functioning and treatment-related symptoms improved by 1 year. Higher levels of decisional regret were correlated with decreases in role and social functioning, increased pain and financial difficulty (all P < 0.01). Sexual function was decreased (P < 0.001) after treatment. Men reported feeling less masculine, having less sexual enjoyment, difficulty in getting and maintaining an erection, and discomfort when being sexually intimate after surgery. Mean scores of decisional regret were similar among patients who reported assuming either active (84%) or collaborative (11%) roles in treatment decision-making. Men who assumed a passive role reported the most variability and highest scores on decision regret. Conclusions: Few men regretted having RP at 1 year after treatment, even though some QoL functions and domains were significantly affected. Ongoing assessment of the effect of surgical treatment on sexual function, sexuality and masculinity certainly deserves further exploration with this group of cancer survivors.

Abstract:Objectives: This article identifies minimal clinically important differences (MCIDs) in quality of life (QoL) measures among patients who had coronary artery bypass grafting (CABG) and were enrolled in the arterial revascularization trial (ART). Methods and results: The European Quality of Life-5 Dimensions (EQ-5D) and the Short Form Health Survey 36-Item (SF-36) physical component (PC) and mental component (MC) scores were recorded at baseline, 5 years and 10 years in ART. The MCIDs were calculated as changes in QoL scores anchored to 1-class improvement in the New York Heart Association functional class and Canadian Cardiovascular Society scale at 5 years. Cox proportional hazard models were used to evaluate associations between MCIDs and mortality. Patient cohorts were examined for the SF-36 PC (N = 2671), SF-36 MC (N = 2815) and EQ-5D (N = 2943) measures, respectively. All QoL scores significantly improved after CABG compared to baseline. When anchored to the New York Heart Association, the MCID at 5 years was 17 (95% confidence interval: 17-20) for SF-36 PC, 14 (14-17) for the SF-36 MC and 0.12 (0.12-0.15) for EQ-5D. Using the Canadian Cardiovascular Society scale as an anchor, the MCID at 5 years was 15 (15-17) for the SF-36 PC, 12 (13-15) for the SF-36 MC and 0.12 (0.11-0.14) for the EQ-5D. The MCIDs for SF-36 PC and EQ-5D at 5 years were associated with a lower risk of mortality at the 10-year follow-up point after surgery. Conclusions: MCIDs for CABG patients have been identified. These thresholds may have direct clinical applications in monitoring patients during follow-up and in designing new trials that include QoL as a primary study outcome.
