Abstract:This study evaluated the 3-dimensional (3D) fit of self-glazed zirconia monolithic crowns fabricated by wet deposition, in comparison with milled zirconia and lithium disilicate monolithic crowns. Dual-scan protocol was used to assess the fit of crowns. Root mean squares (RMS) and uniformity index (UI) were calculated to describe the gap size and uniformity. The self-glazed zirconia crowns had significantly lower RMS values for the gaps at axial wall and transition regions than the milled zirconia crowns, and for the gaps at occlusal region than both milled crowns. All 3 types of crowns had comparable RMS values for the gaps at marginal and chamfer regions and comparable UI values for both marginal and internal gaps. The 3D fit of the self-glazed zirconia monolithic crowns was clinically acceptable and they exhibited better fit at occlusal region than the milled crowns.
Outcome of Teeth Restored with CAD/CAM Zirconium Dioxide Post-Cores: a Retrospective Study with a Follow-Up Period of 3–6 Years

Abstract:Abstract Statement of problem Computer aided design/computer aided manufacturing (CAD/CAM) zirconia post-cores is one of the options of post crown restoration materials due to their esthetic properties and superior mechanical strength. However, the clinical effect on aesthetics and strength properties is unclear due to the lack of results of their long-term follow-up. Purpose This retrospective clinical study aims to analyze the survival rate, clinical manifestations, and failure factors after CAD/CAM zirconia post-core restoration. Material and methods Clinical and radiographic examinations were performed on 342 patients with 400 teeth for 3–6 years postsurgical follow-up examination. The patients were all received CAD/CAM zirconia post-cores and all-ceramic crowns at the Department of Prosthodontics in the public hospital. The retrospective outcomes were conducted after zirconia post restoration, including survival rate by Kaplan–Meier analysis and findings of manifestations and failure factors. The effects of gender and dental position on survival rate were analyzed by Cox–Mantel Test. Results This study retrospectively evaluated 261 teeth from 229 patients with a 35% drop-out rate. The survival rate was 96.0%, and the success rate was 92.4%. According to the tooth position classification, the survival rate was 100% for 101 anterior teeth, 95.4% for 69 premolars, and 88.3% for 91 molars. According to gender, the survival rate of the male group was 92.3%, while that of the female group was 98.0%, with a significant difference (P < 0.01). The complications included crown fracture (1.9%) periapical inflammation (1.9%), crown debonding (1.1%), percussion abnormal (1.9%) and root fracture (0.8%). Conclusions Within the limitations of this retrospective study, it can be concluded that CAD/CAM zirconia post-cores are clinically promising. Compared with the posterior teeth, CAD/CAM zirconia post-cores are more suitable for anterior teeth.
Effect of Surface Treatment and Cement on Fracture Load of Traditional Zirconia (3Y), Translucent Zirconia (5Y), and Lithium Disilicate Crowns

Abstract:Purpose: To determine if surface treatment and cement selection for traditional 3 mol% yttria partially stabilized zirconia (3Y-PSZ), "translucent" 5 mol% yttria-stabilized zirconia (5Y-Z), or lithium disilicate crowns affected their fracture load. Materials and methods: Crowns with 0.8 mm uniform thickness (96, n = 8/group) were milled of 3Y-PSZ (Lava Plus), 5Y-Z (Lava Esthetic), or lithium disilicate (e.max CAD) and sintered/crystallized. Half the crowns were either particle-abraded with 30 µm alumina (zirconias) or etched with 5% hydrofluoric acid (lithium disilicate), and the other half received no surface treatment. Half the crowns from each group were luted with resin-modified glass ionomer (RMGI, RelyX Luting Plus) and half were luted with a resin cement (RelyX Unicem 2) to resin composite dies. Crowns were load cycled (100,000 cycles, 100 N force, 24°C water) and then loaded with a steel indenter until failure. A three-way ANOVA examined the effects of material, cement, and surface treatment on fracture load. Post-hoc comparisons were performed with the Tukey-Krammer method. Results: Fracture load was signficiantly different for materials and cements (p < 0.0001) but not surface treatments (p = 0.77). All lithium disilicate crowns luted with RMGI failed in fatigue loading cycling; 3Y-PSZ and 5Y-Z crowns luted with resin showed a higher fracture load compared with RMGI (p < 0.001). With resin cement, there was no signficant difference in fracture load between 5Y-Z and lithium disiliciate (p = 1) whereas 3Y-PSZ had a higher fracture load (p < 0.0001). Conclusions: Cement type affected fracture load of crowns but surface treatment did not. The 0.8 mm uniform thick crowns tested benefited from using resin cement regardless of type of ceramic material. Crowns fabricated from 5Y-Z may be particle-abraded if luted with resin cement.

Abstract:Objective The purpose of this in vitro study was to evaluate the effect of staining, glazing, and polishing on the survival probability of monolithic crowns manufactured with preshaded stabilized zirconia with 5 mol% of yttrium oxide (5Y‐TZP). Materials and Methods Monolithic crowns in the shape of an upper canine (1.5 mm of thickness) were manufactured by CAD/CAM, adhesively cemented on metallic foundation, and divided into 6 groups (n = 21): C (control), S (staining), G (glazing), P (polishing), SG (staining and glazing), and SP (staining and polishing). The survival probability was determined by step‐stress accelerated life testing with a load applied to the palatine concavity of the crown. First, the specimens were subjected to a single‐load to fracture test (SLF) and next to the fatigue test (5 Hz, thermocycling immersed in water varying 5–55°C), including the light (n = 9), moderate (n = 6), and aggressive (n = 3) loading profiles (load ranged between 20% and 60% of SLF). The survival probability was calculated considering the cycles for failure (CFF) and fatigue failure load (FFL) and illustrated using a Kaplan–Meier graph. The comparison among groups was performed using a Log‐Rank test (α = 0.05). Results The mean value of SLF was 586.7 N. There was no difference among groups in survival probability, considering CFF and FFL. Conclusion Staining, glazing, and polishing can be performed safely without damaging the mechanical behavior of 5Y‐TZP monolithic crowns. Clinical Significance Staining is used to characterize and improve the esthetic of zirconia monolithic crowns. It can be used to reproduce the color gradient in the cervical region of the crown and pigmented grooves. This study showed that staining, glazing, and polishing did not affect the survival probability and the use of finishing procedures (glazing or polishing) after staining did not improve the survival probability of zirconia monolithic crowns.

Silica infiltration on translucent zirconia restorations: Effects on the antagonist wear and survivability

Abstract:Objective To assess potential antagonist wear and survival probability of silica-infiltrated zirconia compared to glass-graded, glazed, and polished zirconia. Methods Table top restorations made of 3Y-TZP (3Y), 5Y-PSZ (5Y), and lithium disilicate (LD) were bonded onto epoxy resin preparations. Each zirconia was divided into five groups according to the surface treatment: polishing; glaze; polishing-glaze; glass infiltration; and silica infiltration. The LD restorations received a glaze layer. Specimens were subjected to sliding fatigue wear using a steatite antagonist (1.25 ×10 6 cycles, 200 N). The presence of cracks, fractures, and/or debonding was checked every one/third of the total number of cycles was completed. Roughness, microstructural, Scanning electron microscopy, wear and residual stress analyses were conducted. Kaplan–Meier, Mantel–Cox (log-rank) and ANOVA tests were performed for statistical analyses. Results The survival probability was different among the groups. Silica infiltration and polishing-glaze led to lower volume loss than glaze and glass-infiltration. Difference was observed for roughness among the zirconia and surface treatment, while lithium disilicate presented similar roughness compared to both glazed zirconia. Scanning electron microscopy revealed the removal of the surface treatment after sliding fatigue wear in all groups. Compressive stress was detected on 3Y surfaces, while tensile stress was observed on 5Y. Significance 3Y and 5Y zirconia behaved similarly regarding antagonist wear, presenting higher antagonist wear than the glass ceramic. Silica-infiltrated and polished-glazed zirconia produced lower antagonist volume loss than glazed and glass-infiltrated zirconia. Silica-infiltrated 3Y and lithium disilicate restorations were the only groups to show survival probabilities lower than 85%.

[Clinical Performance of Chairside Monolithic Lithium Disilicate Glass-Ceramic CAD-CAM Crowns].

Abstract:PURPOSE:To investigate the clinical performance of chairside monolithic lithium disilicate glass-ceramics computer-aided design(CAD)-computer aided manufacturing(CAM) crowns, and to analyze the influencing factors of cumulative survival rate.METHODS:Two hundred and fourteen patients who had chairside posterior lithium disilicate glass-ceramic CAD-CAM crowns in Peking University Shenzhen Hospital from March 2015 to March 2017 were enrolled. The crown preparations were milled using Cerec Omnicam system. The clinical and esthetic effects of the crowns were analyzed at 3, 6, 12, 24, and 36 months. The cumulative survival rate of crowns was calculated, and the effects of gender, age, pulp condition, tooth position and adhesive type on the cumulative survival rate were analyzed. SPSS 20.0 software package was used for statistical analysis.RESULTS:After a 36-month follow-up, the failed crowns were mainly caused by marginal integrity, crown fracture and loss of retention. During the observation period of 3, 6, 12, 24 and 36 months, the scores of color, shape, quality of proximal contact, and chewing ability were greater than 9. The cumulative survival rates were 100.00%, 96.17%, 94.89%, 92.77% and 91.06% at 3, 6, 12, 24 and 36 months, respectively. The cumulative survival rate had no significant difference among different gender, age, and dental pulp status(P>0.05).CONCLUSIONS:Chairside monolithic lithium disilicate glass-ceramic CAD-CAM crowns have a high 3-year cumulative survival rate and good esthetic outcome, which is not affected by gender, age, and pulp status with high clinical value.
Performance of posterior third-generation monolithic zirconia crowns in a complete digital workflow: A three-year prospective clinical study

Abstract:Background/purpose: Translucent monolithic zirconia restorations have recently introduced. The purpose of this study was to evaluate the clinical behavior and the survival rate of the posterior third-generation monolithic zirconia crowns (MZCs) during three years of clinical service. Materials and methods: Twenty-four patients who needed thirty crowns were enrolled in this study. Digital impressions were made, and the crowns were milled and cemented with a resin cement. The crowns outcomes were assessed using the California Dental Association's (CDA) criteria. Gingival index (GI), plaque index (PI), and periodontal probing depth (PPD) for MZCs and contralateral natural teeth (control) were assessed. Margin index (MI) for MZCs was also assessed. Data analysis was conducted using the Wilcoxon signed-rank and the Friedman tests. Results: The 3-year survival rate was 100%. All MZCs were rated as satisfactory throughout the follow-up period, and no biological or mechanical complications were observed. No differences were recorded when GI, PI and PPD at 3-year of follow-up were compared to baseline. No differences were recorded between crowned and control teeth. The MI remained stable throughout the study period. Conclusion: The third-generation monolithic zirconia appears to be a good treatment option for the rehabilitation of posterior single teeth.
A two-year clinical trial of enamel wear opposing 5Y-TZP and lithium disilicate crowns

Abstract:Statement of problem: Clinical information on enamel wear opposing 5 mol% yttria-stabilized tetragonal zirconia polycrystal (5Y-TZP) or lithium disilicate crowns is lacking. Moreover, the effect of surface roughness on the wear of enamel antagonists remains unclear. Purpose: The purpose of this clinical study was to quantify the vertical wear and total volume of wear while also measuring the surface roughness of antagonists at 6-month intervals over 2 years. Material and methods: A tooth-supported first molar 5Y-TZP Lava Esthetic Fluorescent Zirconia crown or an IPS e.max CAD lithium disilicate crown was randomly provided for 24 participants (n=12). The contralateral first molar and opposite teeth were the natural enamel controls. The crowns were fabricated using computer-aided design and computer-aided manufacturing (CAD-CAM) technology and then polished. The data were gathered every 6 months at recall appointments with an intraoral scanner and a polyvinyl siloxane impression. A 3-dimensional (3D) comparative software program was used to measure the maximum vertical and volume loss on opposing enamel, as well as on the enamel control on the opposite side of the mouth. The surface of epoxy resin replicas was analyzed using a scanning electron microscope. The surface roughness of the cusp replica was measured with a 3D laser microscope. The crown's antagonist enamel and natural enamel wear were compared using a 2-way repeated measure ANOVA (α=.05). Results: No significant difference was found in the average volume loss between the crown's antagonist enamel (ZTE 0.042 ±0.005 mm3, LTE 0.048 ±0.006 mm3) and the enamel control (ZCE 0.046 ±0.004 mm3, LCE 0.050 ±0.005 mm3) (P>.05). No significant difference was found in surface roughness between the crown's antagonist enamel (antagonist enamel of zirconia group [ZTE] 0.7 ±0.3 µm, antagonist enamel of lithium disilicate group [LTE] 0.6 ±0.3 µm) and the enamel control (enamel control of zirconia group [ZCE] 0.7 ±0.3 µm, enamel control of lithium disilicate group [LCE] 0.5 ±0.2 µm) in both zirconia and lithium disilicate groups (P>.05). Also, no correlation was found between the amount of wear and the roughness of the surface. The wear patterns observed on the opposing enamel surfaces of the monolithic zirconia and lithium disilicate materials were found to be similar to those seen on natural enamel. Conclusions: A 2-year clinical investigation found that polished lithium disilicate and 5Y-TZP crowns did not affect enamel wear more than enamel against enamel, contrary to laboratory studies. Another finding revealed that surface roughness did not significantly affect wear.
Influence of zirconia abutment preparation on the fracture strength of single implant lithium disilicate crowns after chewing simulation

Abstract:Objective: The use of all-ceramic crowns over zirconia abutments is a well-established esthetic treatment option in implant dentistry; however, the effect of the mechanical processing due to abutment preparation has not been investigated under functional loading. The purpose of the study was to evaluate the influence of the zirconia abutment preparation depth and preparation mode on the fracture strength and fracture mode of lithium disilicate crowns after chewing simulation. Material and methods: Seventy single implant-supported lithium disilicate glass-ceramic crowns (IPS e.max Press, Ivoclar Vivadent) were adhesively cemented (Multilink Automix, Ivoclar Vivadent) onto zirconia abutments (ZirDesign, Astra Tech) using implants with a diameter of 4.5 mm and a length of 15.0 mm (Osseospeed, Astra Tech). Study design concerned the replacement of a maxillary central incisor (11.0 mm in height and 8.0 mm in width). Subgroups (n = 7) were subjected to dynamic loading (C) up to 1.2 × 10(6) loading cycles at 135° with 98N in a thermomechanical chewing simulator (Kausimulator, Willytech); followed by quasi-static loading at a cross-head speed of 0.5 mm/min until fracture in a universal testing machine (Z010/TN2S, Zwick). Additional subgroups were also subjected to quasi-static loading (S) at 135°. Lithium disilicate implant crowns were divided into five study groups (n = 14) according to the abutment preparation depth [A (control): 0.5 mm, B: 0.7 mm, C: 0.9 mm, and preparation mode [(No label): milling by the manufacturer, (P): copy-milling by the Celay System (Mikrona)]. Results: All specimens survived dynamic loading and mean fracture strengths (N) after quasi-static loading were as follows: Group SA: 384 ± 84; Group CA: 403 ± 67; Group SB: 294 ± 95; Group CB: 374 ± 75; Group SC: 332 ± 52; Group CC: 373 ± 105; Group SPB: 332 ± 80; Group CPB: 499 ± 91; Group SPC: 380 ± 101; and Group CPC: 358 ± 54. Conclusions: Statistical analysis using multiple linear regression showed that both the preparation depth and mode had no influence on the fracture strength of the implant crowns (P > 0.05); however, fracture strength increased statistically significantly after 5 years chewing simulation (P = 0.01).
Monolithic Zirconia Partial Coverage Restorations: An In Vitro Mastication Simulation Study

Abstract:Purpose: To evaluate the survival rate (fatigue resistance), bonding efficiency and marginal integrity of monolithic zirconia partial and full coverage single restorations adhesively bonded to the tooth structure using air-particle abrasion, a primer with 10-methacryloyloxydecyl dihydrogen phosphate and a composite-resin cement (APC) protocol. Materials and methods: Extracted human premolars (N = 32) were randomly divided into four groups of eight specimens each. Premolars were prepared for the following restorations: full crown (group 1, control), mesial-occlusal-distal-facial onlay (MODF, group 2) preserving 2 mm facio-lingual functional cusp width, mesial-occlusal-distal-lingual onlay (MODL, group 3) preserving 2 mm facio-lingual nonfunctional cusp width, mesial-occlusal-distal-buccal-lingual onlay (MODBL, group 4), overlay preparation. All restorations were milled from monolithic 3 mol% yttria (3Y) zirconia blocks (ZirCad, A1 LT, Ivoclar Vivadent) with CAD/CAM software presets at minimum occlusal and axial thicknesses of 1 mm. The intaglio surface of the restorations was air-particle abraded (50 µm Al2 O3 , 2-Bar pressure, 15 s, 10 mm distance) and primed. An adhesive cement system was used to bond the restorations. Each group was subjected to thermomechanical loading for 1.2 million cycles (force = 70 N, 1.4 Hz) with simultaneous thermocycling (5-55°C, 30 s dwell time) using a mastication simulator. All specimens were examined under scanning electron microscope (SEM) analysis (30, 100, and 150×) to evaluate cracks and marginal defects. Fracture of restoration and/or fracture within tooth structure, and debonding were considered modes of failure. Results: One specimen from group 2 debonded at 632,000 cycles. None of the specimens failed due to fracture. SEM analysis at 30× indicated marginal integrity issue of the remaining seven intact specimens of group 2 in the area of antagonist contact. No specimens from group 1, 3, and 4 demonstrated marginal integrity issue at 30×. None of the specimens demonstrated any microcrack at 100× and150×. Conclusions: Due to its fatigue resistance, 3Y-zirconia is a viable option for partial and full coverage single restorations. Following a strict bonding protocol, zirconia demonstrated durable adhesion to the tooth structure. Occlusal contact on restoration margins should be avoided.
The Effect of Restoration Thickness on the Fracture Resistance of 5 mol% Yttria-Containing Zirconia Crowns

Abstract:Background: To determine what thickness of 5 mol% yttria zirconia (5Y-Z) translucent crowns cemented with different cements and surface treatments would have equivalent fracture resistance as 3 mol% yttria (3Y-Z) crowns. Methods: The study included 0.8 mm, 1.0 mm, and 1.2 mm thickness 5Y-Z (Katana UTML) crowns and 0.5 and 1.0 mm thickness 3Y-Z (Katana HT) crowns as controls. The 5Y-Z crowns were divided among three treatment subgroups (n = 10/subgroup): (1) cemented using RMGIC (Rely X Luting Cement), (2) alumina particle-abraded then luted with the same cement, (3) alumina particle-abraded and cemented using a resin cement (Panavia SA Cement Universal). The 3Y-Z controls were alumina particle-abraded then cemented with RMGIC. The specimens were then loaded in compression at 30° until failure. Results: All 5Y-Z crowns (regardless of thickness or surface treatment) had a similar to or higher fracture force than the 0.5 mm 3Y-Z crowns. Only the 1.2 mm 5Y-Z crowns with resin cement showed significantly similar fracture force to the 1 mm 3Y-Z crowns. Conclusion: In order to achieve a similar fracture resistance to 0.5 mm 3Y-Z crowns cemented with RMGIC, 5Y-Z crowns may be as thin as 0.8 mm. To achieve a similar fracture resistance to 1.0 mm 3Y-Z crowns cemented with RMGIC, 5Y-Z crowns must be 1.2 mm and bonded with resin cement.

Comparison of retention of monolithic zirconia crowns with alumina airborne-particle abraded and nonabraded intaglio using three different cements: A clinical simulation

Abstract:Statement of problem: The necessity of roughening the intaglio surface of zirconia crowns to achieve adequate retention is unclear. Purpose: The purpose of this clinical simulation study was to evaluate the retention of airborne-particle-abraded and nonabraded monolithic zirconia crowns using 3 different cement types. Material and methods: Extracted human molars were used and prepared with a 10-degree taper. Impressions were made of the prepared teeth with a polyvinyl siloxane (PVS) material, and dies were made with Type 4 gypsum. Each die was scanned with a NobelProcera 1G Scanner, and the standard tessellation language (STL) files were transferred electronically to the Nobel Biocare production site, where a bar was added virtually on top of each crown and parameters were set for milling. Seventy-two Procera zirconia crowns were generated, of which half were airborne-particle abraded on the intaglio surface with 50-µm alumina particles at 400 to 500 kPa for 15 seconds. The other 36 received no intaglio treatment other than cleaning. Both groups of 36 crowns were divided into 3 subgroups of 12 specimens. The area of each preparation was calculated using a computer-aided design software program. The specimens were distributed to attain similar mean surface areas among the cementation groups. The crowns were cemented onto the specimen with a controlled force of 196 N. The 3 cements used were self-adhesive, modified resin RelyX Unicem Aplicap, resin-modified glass ionomer RelyX Luting, and a composite resin, Panavia F2.0 with ED Primer A & B. All specimens were thermocycled (5 °C to 55 °C) for 5000 cycles and then removed axially with a universal testing machine (Instron Model 5585H) at a crosshead speed of 0.5 mm/min. The removal force was recorded, and stress of dislodgement was calculated for each crown. A 2-way analysis of variance was used for statistical analyses. The type of failure was analyzed with the chi-squared test of association for independent samples (α=.05 for all tests). Results: The mean dislodging force for crowns with airborne-particle abraded intaglio was 5.4 MPa, which was statistically greater than the mean of 3.2 MPa for nonabraded specimens (P<.001). No significant differences related to the dislodging stresses were detected among the 3 cements (P=.109). The mode of failure was similar whether abraded or not, with 50% of specimens retaining cement in the crown after separation. Conclusions: Alumina airborne-particle abrasion of the intaglio of zirconia to create surface roughness is beneficial in retaining the crowns, regardless of the cement type. The nonabraded crowns demonstrated significantly lower retentive stress with crown removal. The principal mode of failure was similar whether the zirconia intaglio was airborne-particle abraded or not. The most common mode of failure (>50% of specimens) was at least three-fourths of the cement remaining within the crown.

Abstract:Although advancements in CAD/CAM technology allow for more personalized treatments, it is not clear how modifications in the CAD/CAM milling process could affect the restoration surface conditions and their mechanical behavior. The objective of this study was to evaluate the effect of different CAD/CAM milling protocols on the topography and fracture behavior of zirconia monolithic crowns (3Y-PSZ) subjected to a chewing simulation. Monolithic 3Y-PSZ premolar crowns were milled using three protocols (n = 13) (slow (S), normal (N), and fast (F)). Crowns were cemented on a dentin analog abutment and subjected to mechanical aging (200 N, 2 Hz, 1,500,000 cycles, 37 °C water). Surviving crowns were subjected to compressive load test and analyzed using fractography. Fracture load data were analyzed with two-parameter Weibull analysis. The surface topography of the crowns was examined with a stereomicroscope and a 3D non-contact profiler. All crowns survived the chewing simulation. Crowns milled using the F protocol had the greatest characteristic fracture load, while crowns produced with the S protocol showed high Weibull modulus. Groups N and S had a more uniform surface and detailed occlusal anatomy than group F. The CAD/CAM milling protocol affected the topography and mechanical behavior of 3Y-PSZ monolithic crowns.

Abstract:Objective: To investigate the load-bearing capacity of the full-coverage crowns made from lithium disilicate glass-ceramic LDGC (IPS e.max®, Ivoclar Vivadent) and 10% zirconia reinforced lithium silicate glass ceramic ZLS (Dentsply DeTrey) with different thicknesses. Methods: Forty resin dies with supporting bases were duplicated from two prepared typodont teeth for all-ceramic crowns as a maxillary first molar. Forty crowns corresponding to each die were prepared and then they were divided into four groups: Group I (n 10) made from ZLS with a restoration thickness of (1.0 mm occlusally and 1.0 mm radially) and Group II (n 10) made from LDGC with a restoration thickness of (1.0 mm occlusally and 1.0 mm radially), and Group III (n 10) made from ZLS (1.5 mm occlusally and 1.5mm radially) and Group IV (n 10) made from LDGC (1.5 mm occlusally and 1.5 radially). All crowns were fabricated by chair side CEREC CAD/CAM and crystallized with Speed fire Dentsply Furness. The virolink resin cement (VariolinkII, Ivoclar Vivadent) was used to bond the crowns to the corresponding dies. All samples were thermo-cycled (10000 cycles between 5c and 55c) and tested for fracture resistance using a Universal testing machine at 0.5 mm/minute speed until failure. ANOVA and Tukey HSD test were used to compare the fracture resistance between groups. Results: The result demonstrates that the fracture resistance means and SD of ZLS with different thicknesses ranged from 572 N ± 122.002 to 1171±217.432 N, and those of LDGC with different thicknesses ranged from 625 N ±151.676 N to 845 N ±388.222 N. Conclusions: The fracture resistance increased with increasing crown thickness using different glass-ceramic materials.

Abstract:Objectives Clinical evaluation of chairside‐fabricated lithia‐zirconia glass–ceramic (LZGC) partial crowns (CCPCs) in a multicenter practice‐based study. Materials and Methods Seventy‐one patients were restored with 92 adhesively luted CCPCs (Cerec SW 4.2/Cerec MC XL/Celtra Duo) in three private dental clinics (C1–C3). Time‐dependent (Kaplan–Meier) survival rates (SVR) and success rates (SCR) were calculated. The following possible covariates of SVRs and SCRs were evaluated in a Cox regression model: Restoration position (premolar/molar), luting material (Variolink/Calibra), and operator (C1–C3). Results Seventy‐three CCPCs were placed in 59 patients and were included in the study (mean observational period: 58.0 ± 15 months). Four complete failures (two tooth fractures, one restoration fracture, and one endodontic failure) were recorded. All failures and interventions occurred in one of the three centers (5‐year SCR: C1 + C2: 100%; C3: 71%; 95% confidence interval: [0.55; 0.87]). Additionally, three biological, and two technical complications required clinical intervention to maintain function, and all occurred in C3. Restorations placed in C1 and C2 showed a significantly reduced risk for failure/intervention (hazard ratio = 0.103, p = 0.026) compared with restorations placed in C3. Conclusions LZGC CCPCs showed good five‐year clinical performance. However, SVRs and SCRs were significantly influenced by the operator. Additional clinical data are required for a more detailed investigation of this effect.

Influence of surface treatment of yttrium-stabilized tetragonal zirconium oxides and cement type on crown retention after artificial aging

Abstract:Statement of problem: Information about the influence of zirconia crown surface treatment and cement type on the retention of zirconia crowns is limited. It is unclear whether zirconia crowns require surface treatment to enhance their retention. Purpose: The purpose of this in vitro study was to evaluate the effect of surface treatment on the retention of zirconia crowns cemented with 3 different adhesive resin cements after artificial aging. Material and methods: Ninety extracted human molars were prepared for ceramic crowns (approximately 20-degree taper, approximately 4-mm axial length) and were divided into 3 groups (n=30). Computer-aided design and computer-aided manufacturing zirconia copings were fabricated. Three surface treatments were applied to the intaglio surface of the copings. The control group received no treatment, the second group was airborne-particle abraded with 50 μm Al2O3, and the third group was treated with 30 μm silica-modified Al2O3, The copings were luted with a self-etch (RelyX Unicem 2), a total-etch (Duo-Link), or a self-etch primer (Panavia F 2.0) adhesive cement. They were stored for 24 hours at 37°C before being artificially aged with 5000 (5°C-55°C) thermal cycles and 100,000 cycles of 70 N dynamic loading. Retention was measured on a universal testing machine under tension, with a crosshead speed of 0.5 mm/min. Statistical analysis was performed with 1-way and 2-way ANOVA. Results: Mean retention values ranged from 0.72 to 3.7 MPa. Surface treatment increased crown retention, but the difference was not statistically significant (P>.05), except for the Duo-Link cement group (P<.05). Analysis of the adhesives revealed that the Duo-Link cement resulted in significantly lower crown retention (P<.05) than the other 2 cements. Conclusion: For zirconia crowns, retention seems to be dependent on cement type rather than surface treatment.
Wear resistance of CAD/CAM one-piece screw-retained hybrid-abutment-crowns made from different restorative materials

Abstract:Introduction: The aim of this study was to measure the wear progress of three high performance polymers (HPP) materials as well as that of zirconia after artificial aging (simulated 2.5- and 5-year of clinical service with thermo-mechanical loading) and compare it with the well-documented wear of lithium disilicate. Methods: Forty implants were used to restore a maxillary first premolar, where the abutment and the crown were manufactured as hybrid-abutment-crown and connected to the implant using a titanium insert. The implants were randomly divided, according to the restorative materials used, into five groups: 3Y-TZP zirconia (Z), lithium disilicate (L), ceramic-reinforced polyetheretherketon (P), nano-hybrid composite resin (C) and polymer-infiltrated ceramic-network (E). All hybrid-abutment-crowns were produced using CAD/CAM technology. A design of a maxillary first premolar was created with an angle of 120° between the buccal and palatal cusps, which were designed as planes. The restorations were adhesively luted onto the titanium inserts, according to the manufacturers' recommendations for each material individually, by means of dual-curing luting resin with the exception of group P, where the blocks were pre-fitted (heat-pressed) with an integrated titanium insert. The suprastructures were assembled onto the implants through titanium screws. The screw channels were sealed with Teflon tape and composite resin filling material that was polished to high-gloss. All specimens underwent 1 200 000 thermo-dynamic loading cycles with 49 N in a dual-axis chewing simulator. Elastomeric impressions were made for all specimens after 600 000 and after 1 200 000 cycles. The corresponding impressions were imaged using a laser scanning microscope and then 3D-analyzed using the software (Geomagic Wrap) to measure the volume loss of the wear area for all specimens. Statistical analysis was performed using Wilcoxon-Test regarding the two different time measurements for each material. For the analysis of the material variable, Kruskal-Wallis test was conducted followed by Mann-Whitney test. Results: Group Z showed statistically the lowest volume loss compared to the other test materials, both after 600 000 and 1 200 000 cycles of artificial aging, with a median value of 0.002 mm3 volume loss after 1 200 000 cycles. In contrast, group E showed the highest volume loss with median values of 0.18 and 0.3 mm3 after 600 000 and 1 200 000 cycles, respectively. Artificial aging had significantly negative effect on the volume loss for all test materials. In addition, the choice of material had statistical influence on the outcome. Conclusion: Monolithic zirconia ceramic demonstrated lower wear than that reported for enamel after simulated 5-year of clinical service, whereas all other test materials showed higher volume loss after artificial aging.
EVALUATION OF FRACTURE RESISTANCE AND FIT ACCURACY OF THREE TYPES OF CAD/CAM FABRICATED CROWNS USING CONE BEAM COMPUTERIZED TOMOGRAPHY

Abstract:Aim: The aim of this study was to evaluate the internal and marginal fit accuracy using cone beam computerized tomography as an innovative nondestructive method and to assess the fracture resistance of the recently introduced zirconia reinforced lithium silicate glass ceramic in comparison to lithium disilicate and monolithic zirconia. Materials and Methods: Three types of CAD/ CAM ceramic blocks were used; zirconia reinforced lithium silicate glass ceramics (Vita Suprinity), lithium disilicate glass ceramic (IPS e.max CAD) and partially sintered translucent zirconia (In-Coris TZI). A ceramic master die was fabricated and scanned using the Cerec Omnicam to produce a total number of 30 crowns, which were divided into 3 groups (n = 10) according to the ceramic material used . Group I: Vita Suprinity. Group II: IPS e.max CAD and Group III: In- Coris TZI. A highly computerized cone beam tomography (CBCT) was used to evaluate the fit accuracy of the crowns on the master die. For fracture resistance test, epoxy resin dies were optically scanned using Omnicam scanner and 30 crowns were fabricated using the CAD/ CAM ceramic materials (n = 10) for each tested material. The crowns were cemented using self-adhesive resin cement. A compressive load was applied at a cross head speed of 1mm/min and the fracture load in Newtons was recorded for each specimen. Fractured crowns were examined to determine the mode of fracture. Results: Marginal and internal fit evaluation showed that group I (Vita Suprinity) recorded the least mean in respect to all measuring points (103.6±10.4μm), while group III (In Coris TZI) showed the highest mean in respect to all points (146.7±9.23μm). For fracture resistance, In Coris TZI zirconia showed the highest fracture load (4548.9±200.9 N), while the lowest mean value was recorded in Vita Suprinity (3484.8±22.17 N). There was no significant difference between Vita Suprinity and IPS e.max CAD. Conclusion: Vita Suprinity crowns showed the best marginal and internal fit when compared to the other two materials. In- Coris TZI exhibited the highest fracture strength of all three groups. All values were within a clinically acceptable range for the three ceramic materials tested

Abstract:Purpose This study compared the fracture strength of single lithium disilicate implant‐supported crowns fabricated on two‐piece abutments with various materials: ceramic‐reinforced PEEK, zirconia, and lithium disilicate. Materials and Methods Thirty‐six implants were embedded in acrylic cylinders. A two‐piece abutment and a crown were designed following a pre‐operation scan for a maxillary left central incisor. The designed crown was used to fabricate 36 lithium disilicate crowns. The designed abutment was used to manufacture 36 abutments from 3 materials, 12 each: (A) zirconia; (B) lithium disilicate; and (C) ceramic‐reinforced PEEK. Abutments were surface treated and bonded on the titanium base abutments with resin cement. Then, lithium disilicate crowns were bonded on the assigned abutments. Specimens were then subjected to dynamic loading for 1,200,000 cycles. The fracture strength (N) of the assembly was assessed using a universal testing machine. One‐way ANOVA followed by multiple comparison tests was used to evaluate the effect of abutment material on the fracture strength of single implant‐supported restorations at a significance of .05. Results The average fracture strength for the groups with zirconia, PEEK, and lithium disilicate two‐piece abutments were 1362N ± 218N, 1235N ± 115N, and 1472N ± 171N, respectively. There was a significant (p 0.05) difference between the other groups. Conclusions Two‐piece abutments restored with lithium disilicate crowns investigated in the study have the potential to withstand the average physiological occlusal forces in the anterior region.

Abstract:Objectives: To investigate survival rates, technical and biologic outcomes of buccally micro-veneered all-ceramic single implant crowns. Material and methods: Sixty subjects randomly received immediately or early placed implants. Crowns out of lithium-disilicate (n = 30) and zirconia-ceramic (n = 30) were bonded to titanium-base-abutments. Restorations were inspected at baseline (BL) and during follow-up visits (6, 12 months). Technical and biologic parameters were recorded. Data were analysed descriptively. Differences between groups were tested with Student's t-test. Paired T-test was used when comparing data from the same implant or tooth over time. Linear model repeated measures were used to test differences between materials over time. Differences in counts were evaluated using Pearson Chi-square test. The level of significance was at p < .05. Results: After a mean observation time of 13.2 ± 2.4 months, 54 restorations were re-examined. The implant survival rate was 98.3%, and the restoration survival rate was 100%. One early implant failure occurred. Two minor chippings occurred in lithium-disilicate restorations. No chippings or fractures occurred in any zirconia restorations at 1 year (0%). The technical complication rate was 3.7%, with 7.7% complications among the lithium-disilicate restorations and no differences between the two materials (p = .558). At 1-year follow-up, mean Bleeding on Probing (BOP) was higher at implants (0.27 ± 0.3) than adjacent teeth (0.17 ± 0.18) (p = .046) with no differences between materials (p = .36). Differences in pocket probing depth (PPD) between implants and adjacent teeth were significant (p < .01). Jemt Index improved significantly from BL to 1-y-follow-up (p < .001). Conclusions: Preliminary results were good, suggesting that the ceramic type does not impact technical and biologic parameters.
