Abstract:Desflurane at more than 1 minimum alveolar concentration (MAC) has been shown to prolong the QT interval, but it is unclear whether this is the case at lower concentrations. The aim of this study was to determine whether desflurane concentrations of < 1 MAC affect tracheal intubation-induced prolongation of the QT interval.Forty-four subjects received either inspired desflurane at 1 MAC in oxygen 100% at a fresh gas flow rate of 6 litre min(-1) (desflurane group) or only oxygen 100% (control group) beginning at anaesthesia induction with propofol, before tracheal intubation. The QT intervals were corrected by Bazett's (QTcB) and Fridericia's (QTcF) formulae. The primary outcome was the QTcB immediately after tracheal intubation. Secondary outcomes were the interval from the peak to the end of the T wave (Tp-e), mean arterial pressure (MAP), heart rate (HR), and bispectral index (BIS) score.The QTc interval immediately after tracheal intubation did not differ between the control and the desflurane groups [QTcB, 451 (sd 23) vs 456 (27) ms, P=0.56; QTcF, 422 (24) vs 429 (22) ms, P=0.31, control vs desflurane group, respectively]. There was no difference in Tp-e or HR between the two groups in this study. However, MAP and the BIS score were significantly lower in the desflurane group until 1 min after tracheal intubation.The administration of desflurane at an inspiratory concentration of 1 MAC during manually controlled ventilation after anaesthesia induction with propofol did not affect tracheal intubation-induced QTc prolongation.
Vestibular compensation: new clinical and basic science perspectives.

Abstract:This symposium on vestibular compensation was held in Baltimore, Maryland, on February 16, 2009, at the 32nd annual midwinter research meeting of the Association for Research in Otolaryngology. The meeting program and abstracts have been published online (http://aro.org/abstracts/abstracts.html). The articles, which underwent peer-review for this special issue, address different aspects of vestibular compensation, with the objective of covering a wide spectrum of topics rather than focusing on one approach. Thus, both clinical and basic science studies are presented. The roles of the vestibular nuclei, other brainstem nuclei, vestibular inner ear, cerebellum, and spinal cord in vestibular compensation are discussed. The experiments apply a wide range of approaches, from testing the subject’s balance on stationary and moving platforms to recording vestibulo-ocular reflex (VOR) activity and extracellular single units in intact animals, and whole-cell patch-clamp recordings in brain slices. The presentations emphasize new perspectives on patient diagnosis, treatment, and the basic mechanisms underlying vestibular compensation. We would like to thank the Journal of Vestibular Research for this special issue devoted to this symposium.

Abstract:In balanced anesthesia, sevoflurane and propofol are often used in combination to achieve a better anesthetic effect. However, methods for on-line monitoring of concentrations of the two anesthetics in patients are still rare in clinical. This study proposed a non-invasive method utilizing a fast gas chromatograph combined with a surface acoustic wave sensor (Fast GC-SAW) to simultaneously on-line monitor sevoflurane and propofol in patients' exhaled gas. By using the direct resistive heating capillary column, the single detection time of Fast GC-SAW system was significantly shortened to 90s, as well as the size reduced to (40cm x 30cm x 20 cm). Besides, in the calibration of sevoflurane, Fast GC-SAW system showed a good linear correlation (R-2 = 0.9925, P < 0.01) with gas chromatography-mass spectrometer (GC-MS), which ensured the reliability and accuracy of the Fast GC-SAW system. Finally, clinical exper. iments on patients under balanced anesthesia were conducted. The varied concentrations measured by Fast GC-SAW extraordinarily matched the clinical usages of these two anesthetics. (C) 2017 Elsevier B.V. All rights reserved.
Effect of Sevoflurane on Neuromuscular Blockade of Rocuronium of Anaesthesia Patients in Department of Otorhinolaryngology

Abstract:Objective: To discuss the methods and effect of sevoflurane on neuromuscular blockade of rocuronium of anaesthesia patients in the department of otorhinolaryngology.Method: Randomly selected 82 anaesthesia patients treated recently in the department of otorhinolaryngology,who were divided into two groups,44 cases in the experiment group received the inhalation anaesthesia of sevoflurane and 38 cases in the control group received the intravenous anaesthesia of propofol.After the anaesthesia was stable,injected 0.6mg/kg of rocuronium and compared the anaesthesia effect and different time indicators and adverse reactions of patients in two groups.Result: The total effective rate of anaesthesia on patients in the experiment group was 95.4%,higher than that of 81.6% in the control group(P0.05);the onset time,complete recovery time,the appearance of T2-T4 and the recovery time of TOFr in the experiment group were all better than those in the control group(P0.05);the incidence of adverse reactions in the experiment group was 9.1%,lower than the one of 21.1% in the control group(P0.05).Conclusion: Sevoflurane can extend the neuromuscular blockade of rocuronium,optimize the operation time and reduce the adverse reactions on patients in the department of otorhinolaryngology.
Titration of sevoflurane anesthesia to optimize the time to regain airway reflexes in patients undergoing elective surgery: A randomized clinical trial comparing desflurane and sevoflurane anesthesia

Abstract:Background: Desflurane has adverse environmental effects, but has clinical advantages to speed emergence and return of protective airway reflexes compared with sevoflurane. We hypothesized that weaning of the inspired sevoflurane during the final 15 minutes of surgery would eliminate differences in airway reflex recovery between these agents. Methods: After obtaining IRB approval and informed consent, 40 patients undergoing elective surgery (≥1-hour) randomly received desflurane or sevoflurane. Patients swallowed 20 mL of water without drooling or coughing, and then received sedation and PONV pre-medication. Anesthesia was induced using propofol and fentanyl and maintained with desflurane or sevoflurane through a laryngeal mask airway maintaining a bispectral index of 45-50 and 50-60 during the final 15 minutes before surgery end. Cardiorespiratory variables and age-adjusted minimal alveolar concentration were recorded. The duration between anesthetic discontinuation and first appropriate response to command was measured; the laryngeal mask airway was removed. Two minutes after responding to command, patients were positioned semi-upright and attempted to swallow water. If successful swallowing was not achieved, the test was repeated every 4 minutes after each failure until successful swallowing was achieved. Results: Average anesthetic concentration and bispectral index was similar in patients receiving desflurane vs sevoflurane. Response times after discontinuation of anesthetics were similar. There were no differences in the recovery of swallowing ability between desflurane and sevoflurane groups. Conclusion: Weaning of sevoflurane during the final 15 minutes of surgery eliminates clinical advantages of the more rapid return of airway reflexes with desflurane.
Comparison of sevoflurane and isoflurane in OPCAB surgery

Abstract:Maintenance of anaesthesia with volatile anaesthetic agents affects the perioperative course of patients undergoing off-pump coronary artery bypass (OPCAB) surgery. This facilitates adequate depth of anaesthesia, reduction in need of analgesic dosage, early extubation and transfer from Intensive Care Unit. We compared two volatile anaesthetic agents sevoflurane and isoflurane in terms of haemodynamic effects, amount of analgesic needed during surgery, quantity of agent needed for maintenance of anaesthesia and postoperative recovery in 40 patients undergoing OPCAB surgery. Anaesthesia was induced with fentanyl, midazolam and thiopentone, and vecuronium was used for muscle relaxation. An Octopus stabiliser was used and coronary anastomosis was performed using internal mammary artery and saphenous vein grafts. Routine monitoring was performed. The depth of anaesthesia was monitored using Bispectral index monitor. The inspired/expired concentration of anaesthetic agents to maintain the desired BIS and the amount of volatile anaesthetic agent needed was also noted. The amount of analgesic used intraoperatively was noted in both the groups. The 'time of awakening' defined as eye opening on verbal commands, and time of extubation were noted. There were no differences in haemodynamic parameters, depth of anaesthesia, and quantity of agent needed, but patients in isoflurane group required more intraoperative analgesics than sevoflurane group. Time of awakening (48+/-13 vs 114 +/- 21 mins; P < 0.001) and subsequent extubation (124 +/- 25 vs 177 +/- 36 mins, P<0.001) was earlier in sevoflurane group than isoflurane group. There was no evidence of perioperative myocardial infarction in both the groups. We conclude that sevoflurane and isoflurane can both be safely used in OPCAB surgery, but the awakening and extubation times are significantly less with sevoflurane.

Abstract:Back ground: Volatile anaesthetics are known to cause hypotension because of its effects on the central nervous system and autonomic nervous system, on the myocardium as well as its direct action on vascular smooth muscle. Aim of study: is to compare between sevoflurane and isoflurane effects on pulse rate and mean blood pressure during general anaesthesia. Patients and methods: this prospective study was carried out at Al-Hilla Teaching Hospital/Babylon as well as Balad General Hospital/Salah Al-Din, from 2nd (March) 2021-20th (May) 2021. Thirty patients between the ages 7- 68 years were enrolled in this study, 13 Male and 17 females. All patients fasted for 8-10 hours before proposed time of surgery. In all selected patients, pulse rate and mean blood pressure was recorded at baseline, at intubation at (5 Minute), (10 Minute), (15 Minute), (20 Minute), (25 Minute), (30Minute), (40 Minute) of the time of operation, extubation, and recovery. Results: showed that non-significant differences between inhalation agent (sevoflurane and isoflurane) and hemodynamic status (pulse rate and Mean blood pressure) at different time. The Groups according to their gender, showed high percentage of 38.5% in males at age group of 36-50. While 50.0% of females at age group 21-35 years. The most patients which received Isoflurane as inhalational agent were female groups (52.9%) versus (46.2%) for male groups, the female group which received Sevoflurane inhalational agent were (47.1%) versus (53.8%) for male groups. Conclusion: it conclude that the sevoflurane and isoflurane is more anesthetic stable gas on heart rate and mean blood pressure.

Abstract:ObjectivesA meta-analysis to compare postoperative cognitive function and the time to specific recovery events in elderly patients (aged >65 years) anaesthetized with sevoflurane or desflurane. MethodsA systematic search of the PubMed (R), Embase (R), Cochrane Library and Chinese Biomedical databases was performed using the keywords sevoflurane' and desflurane'. Data and characteristics of appropriate randomized controlled trials (RCTs) were extracted. ResultsThe meta-analysis included five trials (n=300). The time taken to follow commands (mean difference [MD] -3.27; 95% confidence intervals [CI] -4.95, -1.59), extubation (MD -1.59; 95%CI -2.62, -0.55), orientation (MD -4.31; 95%CI -4.99, -3.62), and recovery room discharge (MD -9.38; 95%CI -13.43, -5.42) were significantly shorter in the desflurane group than in the sevoflurane group. There was no significant between-drug difference in the incidence of postoperative cognitive dysfunction or the time taken to open the eyes. ConclusionsDesflurane is associated with a faster recovery from general anaesthesia than sevoflurane in elderly patients.

Abstract:BACKGROUND AND AIMS: Inhalation anesthesia with sevoflurane may be enhanced by several drugs or techniques. The aim of the present study was to investigate the effect of nitrous oxide (N2O) pretreatment on the speed of anesthesia induction with sevoflurane.MATERIAL AND METHODS: Eighty patients scheduled for hysteroscopy under general anesthesia were randomly assigned to inhale for 10 min before induction 50% N2O in oxygen or air via a facemask. Anesthesia was induced with 7-8% sevoflurane in oxygen via a facemask. Bispectral index (BIS), end-tidal carbon dioxide (EtCO2) tidal volume, respiratory rate, oxygen saturation (SpO2), and heart rate were recorded every minute during the 10 min pretreatment periods and every 30 s during the first 300 s of induction with sevoflurane. During induction of anesthesia inspired and end-tidal sevoflurane concentrations were also recorded.RESULTS: During the 10 min of inspired 50% N2O or air BIS, EtCO2, tidal volume, respiratory rate and heart rate values did not differ between the two groups except for the SpO2, which was higher in the N2O group (P < 0.001). During induction of anesthesia the N2O group exhibited lower BIS values (P = 0.001), being significant at 60-150 s (P < 0.001, P < 0.001, P = 0.002, P = 0.014) as well as at 270 s (P = 0.004). EtCO2 and tidal volume were consistently lower in the N2O group (P = 0.001, P = 0.041 respectively) and respiratory rate was higher (P = 0.007).CONCLUSION: Our results show that pretreatment of the patients with 50% N2O for 10 min enhances the speed of induction with sevoflurane as assessed by the BIS monitoring.
Induction efficay of sevoflurane and sevoflurane combined with propofol for tracheal intubation without muscle relaxants

Abstract:Objective To evaluate the efficacy of anesthesia induction with sevoflurane and sevoflurane combined with propofol for tracheal intubation without muscle relaxants in children.Methods Sixty ASA Ⅰor Ⅱ patients,aged 1-10 year,weighing 10-40 kg,and who were scheduled for elective surgery under general anesthesia,were enrolled in this study.The patients were randomized into 3 groups (n=20 each):sevoflurane group (group A),sevoflurane combined with 2 mg/kg propofol group (group B),and sevoflurane combined with 3 mg/kg propofol group (group C).Sevoflurane was inhaled initially with high-flow oxygen (5 L/min).Sevoflurane anesthetic vaporizer dial was set of 6%.When the patients’ eyelash reflex disappeared and the submaxilla was loosed,group A continued inhalation of sevoflurane for 100 second with controlled ventilation;group B continued inhalation of sevoflurane and received 2 mg/kg propofol;group C continued inhalation of sevoflurane and received 3 mg/kg propofol.The mean arterial pressure (MAP) and heart rate were recorded before administration,before tracheal intubation,and 1 min,1 min,and 5 min after tracheal intubation.Patients’ response to tracheal intubation was also recorded.Results All patients were successfully intubated at the first attempt.Only 4 children in group A had cough and body movement.The intubation satisfactory rate of group C was 80%,which was significantly higher than those of group A(45%) and group B(70%) (P<0.05).The MAP before intubation was (75±10) mmHg(1 mmHg=0.133 kPa) in group C,which was significantly lower than that before induction ([92±11] mmHg,P<0.05).The MAP in group A was (104±16) mmHg and the heart rate was (126±14)/min 1 min after intubation,both significantly higher than those before induction ([89±9] mmHg,[106±10]/min) and those immediately before intubation([80±10] mmHg,[100±14]/min,P<0.05).Conclusion Induction with sevoflurane and propofol (2-3 mg/kg)is smooth and can effectively inhibit response to tracheal intubation in childern without muscle relaxants.(Shanghai Med J,2010,33:332-334)

Abstract:Objective To investigate the effects of β2 agonist salbutamol aerosol on the uptake of sevoflurane in elderly patients with chronic obstructive pulmonary disease(COPD). Methods A randomized,placebo-controlled and double-blinded trial was designed.Twenty-six patients were recruited and randomly allocated to salbutamol group(group E,n=13) and placebo group(group C,n=13).Eligible patients were elderly patients with ASA physical status Ⅱ-Ⅲ,a body mass index(BMI) between 18 and 30 kg/m2,well-defined clinical diagnosis of COPD.Routine monitoring(consists of a three-lead ECG,pulse oximetry,noninvasive blood pressure and expired gas analysis) was instituted and 500 mL Ringer's lactate solution was administered.Bispectral index(BIS) monitoring was initiated prior to induction.All subjects were received inhaled aerosol 200 μg according to manufacturers' recommendations 30 minutes before induction of anesthesia.Controlled ventilation was applied after the trachea was intubated.When stable hemodynamics was maintained for 5 minutes,fresh gas flow was set to 2 L/min with 2% sevoflurane in admixture,then HR,invasive arterial blood pressure(IABP),SpO2,PETCO2,bispectral index(BIS),minimum alveolar concentration(MAC),concentrations of inhaled sevoflurane(FI) and end-tidal(FE) were recorded at 1,2,3,4,5,7,10,and 15 minutes after inhalation of sevoflurne.The Ppeak and Pplat were also measured in 1,5,and 10 minutes after the successful endotracheal intubation. Results Compared to the placebo group,the FE was significantly higher at 2,3,4,and 5 minutes in experiment group.Peak airway pressure and plateau pressure in experiment group were strikingly lower than control group(P<0.05).There was no statistical significance about BIS variations between these groups in respective time in spite of increased MAC(P>0.05). Conclusions The increasing rate of alveolar concentration of sevoflurane was accelerated after the administration of inhaled salbutamol aerosol(200 μg) 30 minutes before induction of anesthesia,it might increase the uptake of sevoflurane.

Abstract:Introduction: An ideal anaesthetic for robotic surgery would allow for a quick induction with minimal discomfort, as well as a short operation duration, quick recovery, and minimal aftereffects. Additionally, it would enable speedy recovery. Aim: To compare the haemodynamic stability and recovery characteristics of sevoflurane with desflurane for robotic surgery. Materials and Methods: The present randomised clinical study was conducted for four years from January 2019 to January 2023 and included 60 patients undergoing elective robotic radical prostatectomy and randomised into the group receiving desflurane (Group D) and that receiving sevoflurane (Group S). All patients were monitored using the Bispectral index (BIS) monitor, in addition to standard monitoring. General anaesthesia was administered using midazolam, fentanyl, propofol, atracurium, and either desflurane or sevoflurane based on the assigned group. Throughout the surgery, patients’ haemodynamic stability was monitored, and vital signs were recorded at induction, intubation, after assuming the Trendelenburg position, and at 30-minute intervals, until extubation. The inhalational agent was turned off at skin closure, and the time to spontaneous eye opening (T1), time to extubation (T2), and time to verbal response (T3) were noted. After the verbal response, patients were assessed based on the Modified Aldrete Score (MAS) with a threshold of ≥9. Pulse rate, blood pressure, BIS, and MAS were recorded during this time period. Data was presented as mean, frequency, and percentage. MAS comparison among the study groups was assessed using the Chi-square test. Demographic, haemodynamic, and BIS variables were compared using paired t-tests. Results: The mean age of patients in group S was 67.67±6.07 years, while in group D, it was 65.17±6.69 years. The time required for extubation after turning off the agent was significantly shorter in group-D compared to group-S, with a mean of 16.07±13.00 minutes in group-D and 21.71±9.07 minutes in group-S (p-value=0.0001). The percentage of patients achieving MAS >9 at five minutes was significantly higher in group D. Additionally, the use of both agents was not associated with any major complications. Conclusion: Desflurane as the inhalational agent ensures faster recovery in the early postoperative period and minimal changes in haemodynamic parameters compared to sevoflurane. However, sevoflurane has fewer complications compared to desflurane.

Abstract:Introduction: Introduction of more rapid and short-acting volatile anesthetics and intravenous anesthetics has allowed anesthesiologists to achieve a recovery profile that facilitates rapid emergence after general anesthesia. Both propofol and sevoflurane provide rapid induction and faster recovery by titrating the depth of anesthesia based on the BIS Index during maintenance. The objectives of the study were analysis of hemodynamic parameters, changes in the BIS Index, recovery characteristics, postoperative complications. Methods: Following IRB approval, an observational study was conducted in 60 patients of ASA grade I-II, aged 18-60 years undergoing surgical procedures under general anesthesia. Patients were randomly divided into two groups, with 30 patients each receiving propofol infusion (Group P) or sevoflurane (Group S) to maintain anesthesia. The requirement of anesthetic agents was titrated by BIS monitoring. BIS score was held between 40 to 60. Results: Hemodynamic parameters like heart rate and mean arterial pressure (MAP) were comparable in both groups. During recovery, the meantime to spontaneous ventilation, eye-opening, hand squeezing, following verbal command, extubation, stating the name, and mean time to achieve modified Aldrete's score of ≥ 9 from the cessation of the anesthetic drug was significantly shorter in Group S as compared to Group P. PONV was noted as a postoperative complication in the sevoflurane group. Conclusion: In our study, sevoflurane provided rapid emergence and faster recovery with stable hemodynamic. However, propofol appears to be an excellent alternative to sevoflurane for maintenance of anesthesia as it also provides fast recovery with a lower incidence of PONV

Abstract:P>Objective: Screening examination for retinopathy of prematurity (ROP) is stressful and painful to the neonate. Sevoflurane has been used successfully in anesthesia for full-term and premature neonates and has been recently used for pediatric outpatient procedures. In this study, we examined the effective inspired concentration of sevoflurane to prevent movement in response to stimulation of examination in 50% of patients (EIC50) of sevoflurane, as well as the response of sevoflurane to age in the preterm outpatients undergoing fundus examination. Methods: Preterm pediatric outpatients at different postconceptual ages (duration from the mother's last menstrual period to the date when the examination for ROP was performed) with < 44 weeks (group A) and 44-64 weeks (group B) undergoing fundus examination were included. SpO(2) and ECG were monitored in operation room. In the process of anesthetic induction, the oxygen flow rates were 3 l center dot min-1, and subjects spontaneously breathed 6% sevoflurane by mask. The time to loss of movement was recorded as induction time. Six percent sevoflurane was inhaled continuously for the same duration, and then the inspired concentration of sevoflurane was adjusted to maintenance concentration. When no movement or crying was observed, the speculums were used to keep the eyelids open, and then the eye examinations were performed by the same ophthalmologist. During induction time and maintenance time, the occurrence of coughing, clenching, gross purposeful movement, breath holding or desaturation to SpO(2) < 95% was recorded. After completion of the procedure, patients were observed in the same room until spontaneous eye opening or verbalization occurred. Up and down method was used to determine subsequent maintenance concentration in each group. The initial maintenance concentration was 3%. The gradient of increase or decrease was 0.5%. If the preceding subject had not moved, the sevoflurane concentration was decreased by 0.5%; if the preceding subject had moved, the concentration was increased by 0.5%. When at least six independent alternations from no-movement to movement were observed, test was terminated. Results: Twenty-four pediatric outpatients completed the investigation in group A. The effective inspired concentration prevented movement in response to stimulation of examination in 50% of patients (EIC50) of sevoflurane was 2.5% in group A. Twenty-seven patients completed the investigation and EIC50 values of sevoflurane were 3.0% in group B. The induction time in group A was significantly lower than that in group B. The gestational ages, anesthesia time, and awake time were similar in two groups. Conclusion: Anesthesia with inhaled sevoflurane by a face mask can be accomplished in preterm outpatients undergoing fundus examination without intubation and i.v. accession. The EIC50 is lower, and the induction time is shorter in smaller aged patients compared with those in older ones.

Abstract:Background: The primary objective of this study was to compare sevoflurane inhalation anesthesia with total intravenous anesthesia (TIVA) in terms of its effectiveness in maintaining adequate depth of anesthesia during all open heart surgery procedures, including cardiopulmonary bypass. The study's secondary objective was to compare sevoflurane inhalation anesthesia with TIVA regarding the impact on the time of tracheal extubation and the incidence of postoperative acute kidney injury during open heart surgeries. Methods: A total of 58 patients undergoing open heart surgery were included, with 30 receiving sevoflurane inhalation anesthesia and 28 receiving TIVA. Demographic characteristics, intraoperative parameters, and postoperative outcomes were recorded and analyzed. Statistical analysis revealed no significant differences in Bispectral Index (BIS) monitor values, mean arterial pressure, body temperature, or other intraoperative parameters between the two groups. Notably, the time to tracheal extubation was significantly shorter in the Sevoflurane group compared to the TIVA group, although both groups exhibited similar rates of postoperative acute kidney injury (AKI). Results: None of the patients had complaints of intraoperative awareness. The mean arterial pressure, body temperature, and bispectral index values during and before cardiopulmonary bypass were similar between the groups. Postoperative variables such as intensive care unit stay duration, incidence of acute kidney injury, and immediate and 24-hour post-extubation visual analog scale values were similar between the groups. The tracheal extubation time was found to be statistically shorter in the sevoflurane group. Conclusion: We believe that sevoflurane inhalation anesthesia can achieve adequate depth of anesthesia during the intraoperative period in open heart surgery without increasing the rate of postoperative complications.

Abstract:Objective To study the effects of sevoflurane in general anesthesia inducing for aged patients. Methods 36 cases of ASAI-Ⅱ patients were divided into two groups randomly. Group A was induced by total intravenous anesthesia. Group B was inhaled 6% sevoflurane、the flux of O2 was 2L/min.The relative data was observed. Results There was significant difference disappearing time of respiratory and recovering reflex between both groups, group B being much shorter. Conclusion The sevoflurane might have inducing smoothly without exciting time and quick sleep. The sevoflurane affect cardiovascular system without stronger effect.
Effect of Intravenous Lidocaine on Efficacy of Sevoflurane Combined with Remifentanil for Tracheal Intubation Without Neuromuscular Relaxants

Abstract:Objective To investigate the effect of intravenous lidocaine on the efficacy of sevoflurane combined with remifentanil for tracheal intubation without neuromuscular relaxants. Methods Seventy-five ASA Ⅰor Ⅱ patients, aged 18-64 yr, scheduled for elective surgery, needing tracheal intubation under general anesthesia, were randomly divided into 3 groups ( n = 25 each) : sevoflurane + remifentanil 1 μg/kg group (group A) ;sevoflurane + remifentanil 1 μg/kg + lidocaine 1 mg/kg group (group B); sevoflurane + remifentanil 2 μg/kg group (group C) . Two minutes after inhalation of 8% sevoflurane for anesthesia induction, remifentanil 1 μg/kg, remifentanil 1 μg/kg + lidocaine 1 mg/kg, and remifentanil 2 μg/kg were injected intravenously in groups A, B and C respectively. Tracheal intubation was performed after completion of remifentanil injection. Intubating conditions were assessed based on ease of laryngoscopy, position of vocal cords, activity of vocal cords, degree of coughing and limb movement. MAP and HR were also recorded before induction and immediately before and after intubation. Results Tracheal intubations were successful in all patients. The satisfactory rates of coughing were significantly higher in groups B and C, and MAP and HR were significantly lower immediately before and after intubation in group C than in group A ( P ＜ 0.05) . The satisfactory rate of coughing was significantly higher in group B than in group C ( P ＜ 0.05) . During intubation, 3 cases developed hypotension and 1 case bradycardia in group C. Conclusion When sevoflurane combined with remifentanil is used for tracheal intubation without neuromuscular relaxants, intravenous lidocaine 1 mg/kg can not only improve intubating conditions, but also decrease the consumption of remifentanil.
Efficacy and Safety Comparison Between Sevoflurane Inhalation Combining Laryngeal Mask Airway and Ketamine Anesthesia for Anesthesia Induction

Abstract:Objective: Compared with ketamine anesthesia, to investigate sevoflurane inhalation combining laryngeal mask airway for anesthesia induction in pediatric cardiac surgery. <br> Methods:A total of 40 pediatric patients with congenital heart diseases received elective cardiac surgery in our hospital from 2013-08 to 2014-01 were studied. The children were from 6 months to 2 years of age and randomly divided into 2 groups, n=20 in each group. Sevolfurane group, the children inhaled the mixture of 8%sevolfurane and 100%O2, laryngeal mask airway was used upon losing consciousness for mechanical ventilation, the anesthesia was maintained by (3-4)% sevolfurane inhalation to facilitate central venous catheter placement. Ketamine group, the children received intramuscular injection of ketamine (7 mg/kg)+atropine (0.01 mg/kg). The peripheral venous line was established upon losing consciousness, the intravenous midazolam (0.1 mg/kg), pipecuronium (0.10 mg/kg), fentanyl (5 μg/kg) were applied, then tracheal intubation was performed for mechanical ventilation and the anesthesia was maintained by (0.5-1)%sevolfurane to facilitate central venous catheter placement. <br> Results: Sevoflurane group had the shorter time for losing the consciousness than that in Ketamine group (48.90 ± 3.93) s vs (577.85 ± 116.41) s, P<0.05 and the shorter time for ifnishing the central venous catheter placement (11.15 ± 2.48) min vs (24.15 ± 4.02) min, P<0.05. The average blood pressure and heart rate were similar between 2 groups after laryngeal mask or tracheal intubation, P>0.05. The arterial PH value, PaCO2, BE and lactatein were similar between 2 groups, P>0.05. <br> Conclusion: Sevoflurane inhalation combining laryngeal mask airway could shortening anesthesia preparation time with simple management. It provided an important anesthesia option in pediatric cardiac surgery.
