Abstract:An introduction to the physics and technological principles of diagnostic ultrasound is presented, including (1) a discussion of ultrasound-tissue interactions; (2) the pulse-echo technique; (3) transducers and ultrasound machine instrumentation for the detection and signal processing of echoes and image formation; (4) Doppler ultrasound; (5) B-mode and Doppler ultrasound artifacts. An overview of advanced ultrasound modes including harmonic and compound imaging, elastography, 3D imaging, ultrasound contrast, bioeffects, and safety of ultrasound is also provided.

Abstract:Ultrasound is a sound wave beyond the audible frequency. Owing to technological development, the extent of use of ultrasound in orthopaedics is expanding. Ultrasound is produced by a piezoelectric effect and matter is requires for propagation. According to the characteristics of matter, the velocity of propagation differs and the images are the overall result of the interaction of reflection, refraction, absorption, scattering, transmission, and attenuation. The most important device is the transducer, which differs according to the array of piezoelectric elements and shapes the way it used and where it is used. Mode B is currently the most common image and many images which help to make diagnosis easier, such as doppler flow imaging, extended field of view imaging, compound imaging and three-dimensional imaging, are under developments. Ultrasound produces variable artifacts and during interpretation of ultrasound images, artifacts could interfere with correct diagnosis. In order to avoid creation of artifacts, profound knowledge of each artifact is needed. Therefore, precise understanding and interpretation of the ultrasound image is essential for proper diagnosis and use of ultrasound.

Abstract:Point-of-care ultrasound demonstrates significant potential in biomedical research due to its noninvasive, real-time visualization, cost-effectiveness, and other biological benefits. Ultrasound irradiation can precisely control the mechanical and physicochemical effects on pathogenic lesions, enabling real-time visualization, tunable tissue penetration depth, and therapeutic applications. This review summarizes recent advancements in ultrasound-enabled diagnostics and therapeutics, focusing on mechanochemical effects that can be directly integrated into biomedical applications. Additionally, the structure-functionality relationships of sonotheranostic nanoplatforms are systematically discussed, providing insights into the underlying biological effects. Finally, the limitations of current ultrasonic medicine are discussed, along with potential expansions to facilitate patient-centered translations.

Abstract:At present, medical ultrasound technology is gradually developing from the traditional imagingof tissue and organ structure based on wave effect of ultrasound, and the treatment of diseased tissue based on thermal and cavitation effects, to the direction of multi-functional integration of diagnosis and treatment. Therefore, asone of the most important biological effects, the mechanical effect of ultrasound (i.e. acoustic radiation force) hasattracted more and more attention from scientific researchers and medical staffs. Benefiting from the in-depthunderstanding of the theory and mechanism of ultrasonic mechanical effect, as well as the rapid development ofinformation and electronic technology, a variety of new ultrasonic diagnosis and treatment technologies based onacoustic radiation force have emerged in recent years, which have greatly expanded the application of ultrasoundin clinics. For example, ultrasound elastography that can quantitatively measure the tissue stiffness and image itsdistribution, which can provide early screening for liver cirrhosis and breast cancer; as well as ultrasound neuromodulation that can non-invasively regulate the state of nerve cells in the brain, which can provide unique toolsfor brain disease treatment and neuroscience research. This article will systematically introduce the representativeachievements of these new medical ultrasound technologies based on the mechanical effect of ultrasound in recentyears, and analyze their future development directions.

Abstract:Recent advances in electronics and digital processing techniques have significantly improved conventional ultrasonic imaging systems and allowed the development of new and sophisticated scanning methods. As a result, ultrasonic imaging devices have become an important modality for the clinical radiologist, complimenting the images obtained from X-ray and nuclear cameras. A particular advantage of ultrasonic waves is that they are nonionizing, thus presenting less risk to both patient and examiner. This paper presents some of the basic principles of ultrasonic propagation in tissue and how those principles impact the design of imaging devices. The characteristics of both the B- scan and C-scan techniques are described along with a summary of various scan formats that are currently available. Examples of B-scan and C-scan instruments are presented along with their relative advantages, limitations, and current usage;representative images are presented whenever possible. This paper concludes with a description of some new research developments in this rapidly emerging technology.

Abstract:Ultrasound is widely used for medical diagnosis and increasingly for therapeutic purposes. An understanding of the bio-effects of sonography is important for clinicians and scientists working in the field because permanent damage to biological tissues can occur at high levels of exposure. Here the underlying principles of thermal mechanisms and the physical interactions of ultrasound with biological tissues are reviewed. Adverse health effects derived from cellular studies, animal studies and clinical reports are reviewed to provide insight into the in vitro and in vivo bio-effects of ultrasound.

Abstract:The emergence of new ultrasound technologies has improved our understanding of the brain functions and offered new opportunities for the treatment of brain diseases. Ultrasound has become a valuable tool in preclinical animal and clinical studies as it not only provides information about the structure and function of brain tissues but can also be used as a therapy alternative for brain diseases. High-resolution cerebral flow images with high sensitivity can be acquired using novel functional ultrasound and super-resolution ultrasound imaging techniques. The noninvasive treatment of essential tremors has been clinically approved and it has been demonstrated that the ultrasound technology can revolutionize the currently existing treatment methods. Microbubble-mediated ultrasound can remotely open the blood-brain barrier enabling targeted drug delivery in the brain. More recently, ultrasound neuromodulation received a great amount of attention due to its noninvasive and deep penetration features and potential therapeutic benefits. This review provides a thorough introduction to the current state-of-the-art research on brain ultrasound and also introduces basic knowledge of brain ultrasound including the acoustic properties of the brain/skull and engineering techniques for ultrasound. Ultrasound is expected to play an increasingly important role in the diagnosis and therapy of brain diseases.

Abstract:A general understanding of ultrasound physics relative to image optimization is essential when performing breast ultrasound examinations and in accurately interpreting images of the breast. Appropriate technique is crucial to achieve accurate lesion characterization and is emphasized in the second edition of the American College of Radiology Breast Imaging Reporting and Data System for ultrasound (2013). This pictorial essay reviews clinically relevant ultrasound physics to best characterize breast lesions for guiding the clinician to the proper diagnosis. Differences in chosen parameters on ultrasound imaging can create variability in the appearance of breast lesions.

Abstract:The application of ultrasound contrast agents in clinic has improved the ability of diagnosis and differential diagnosis on diseases, especially on the qualitative diagnosis and location of tumors. In recent years, many scholars are devoting to the research of targeting imaging and therapy of ultrasound contrast agents for tumors. Great achievements have been shown to us, including the preparation technology of targeting microbubble, ultrasound-relative biologic technology, microbubble-relative gene technology, etc. It is acknowledged that the combination of neotype contrast agents with ultrasound will remarkably extend the diagnosis and treatment practice of ultrasound. The improvement of diagnostic specificity and accuracy, and the effective targeting therapy on tumor will bring the individual treatment and new hope for tumor patients.

Abstract:The use of ultrasound imaging in clinical obstetrics continues to grow at an almost exponential rate. Ultrasound imaging in developmental biology has only begun to be used to enhance the traditional methodologies to study the developing embryo/fetus. The various modalities of ultrasound imaging are reviewed as they apply to current uses in clinical obstetrics and developmental biologic research. New modalities are also discussed in both clinical obstetrics and developmental biologic research as well as the current limitations of ultrasound imaging faced in both of these fields.
Therapeutic ultrasound transducer technology and monitoring techniques: a review with clinical examples

Abstract:The exponential growth of therapeutic ultrasound applications demonstrates the power of the technology to leverage the combinations of transducer technology and treatment monitoring techniques to effectively control the preferred bioeffect to elicit the desired clinical effect.Objective: This review provides an overview of the most commonly used bioeffects in therapeutic ultrasound and describes existing transducer technologies and monitoring techniques to ensure treatment safety and efficacy.Methods and materials: Literature reviews were conducted to identify key choices that essential in terms of transducer design, treatment parameters and procedure monitoring for therapeutic ultrasound applications. Effective combinations of these options are illustrated through descriptions of several clinical indications, including uterine fibroids, prostate disease, liver cancer, and brain cancer, that have been successful in leveraging therapeutic ultrasound to provide effective patient treatments.Results: Despite technological constraints, there are multiple ways to achieve a desired bioeffect with therapeutic ultrasound in a target tissue. Visualizations of the interplay of monitoring modality, bioeffect, and applied acoustic parameters are presented that demonstrate the interconnectedness of the field of therapeutic ultrasound. While the clinical indications explored in this review are at different points in the clinical evaluation path, based on the ever expanding research being conducted in preclinical realms, it is clear that additional clinical applications of therapeutic ultrasound that utilize a myriad of bioeffects will continue to grow and improve in the coming years.Conclusions: Therapeutic ultrasound will continue to improve in the next decades as the combination of transducer technology and treatment monitoring techniques will continue to evolve and be translated in clinical settings, leading to more personalized and efficient therapeutic ultrasound mediated therapies.

Abstract:Ultrasound is a powerful tool in ophthalmology with a wide range of physical effects that can interact with biological tissue. This ranges from low-intensity linear transducers for diagnosis to high-intensity pulsed or continuous focused ultrasound for therapy. Designing devices for ophthalmological applications requires creating fine focal spots, minimizing heating, and accounting for eye movements. Ultrasound is essential for ophthalmologists to provide accurate diagnosis and quantitative information on tissue composition and blood flow. Ultrasound has revolutionized cataract surgery, making it less invasive and in an outpatient basis, while enhancing the safety and predictability of glaucoma treatment using high-intensity focused ultrasound. The article aims to review the complex and multifaceted bioeffects of ultrasound used in ophthalmology, and its current and future applications of ultrasound in ophthalmology, notably regarding cavitation-mediated drug delivery.

Several categories of mechanical bioeffects of ultrasound are then reviewed to provide insight on the range of ultrasound bioeffects in vivo, the relevance of these effects to diagnostic imaging, and the potential application of mechanical bioeffects to the design of new therapeutic applications of ultrasound in medicine.

Abstract:The main methodologies are introduced for the simulation of medical ultrasound imaging.The significance and the application prospects of simulation studies are also discussed.The existing ultrasound imaging simulation methods are divided into two groups: linear assumption based and nonlinear assumption based.Representative methods in each group as well as the differences between two groups are presented and compared.The simulation method based on the numerical solution of acoustic wave equations may provide more realistic simulation of real ultrasound imaging.It can be used as an efficient tool for understanding various phenomena in ultrasound imaging and establishing relationships between tissue properties and image features.

Abstract:Breast cancer is the most common cancer in women worldwide. Providing accurate and efficient diagnosis, risk stratification and timely adjustment of treatment strategies are essential steps in achieving precision medicine before, during and after treatment. Radiomics provides image information that cannot be recognized by the naked eye through deep mining of medical images. Several studies have shown that radiomics, as a second reader of medical images, can assist physicians not only in the detection and diagnosis of breast lesions but also in the assessment of risk stratification and prediction of treatment response. Recently, more and more studies have focused on the application of ultrasound radiomics in breast management. We summarized recent research advances in ultrasound radiomics for the diagnosis of benign and malignant breast lesions, prediction of molecular subtype, assessment of lymph node status, prediction of neoadjuvant chemotherapy response, and prediction of survival. In addition, we discuss the current challenges and future prospects of ultrasound radiomics.
