Abstract:Background Tumors involving the jugular foramen region are challenging for surgical resection. With the development of endoscope in the past decade, surgical approaches assisted by endoscope have been widely emerged in the treatment of skull base tumors. Methods Herein, we report a case of jugular foramen schwannoma (Samii type B). Surgical resection was applied via a suboccipital retrosigmoidal craniotomy using surgical microscope assisted by endoscope. Gross total resection was achieved. And the patient recovered without obvious neurological deficits. Conclusions Samii type B schwannomas involving the jugular foramen is approachable by endoscope-assisted surgery.
The Relationship Between Jugular Foramen Schwannoma and Surrounding Membrane Structures and Its Surgical Application

Abstract:Background and objectives: Surgical resection of jugular foramen (JF) schwannomas with minimal neurological complications is challenging because of their difficult-to-access location and complex relationships with surrounding neurovascular structures, even for experienced neurosurgeons. In this article, we elucidate the membranous anatomy of JF schwannomas, with the aim of reducing iatrogenic injury to the lower cranial nerves (LCNs) during surgery. Methods: The clinical data of 31 consecutive patients with JF schwannomas were reviewed. The relationship between the tumor and the surrounding membranous structures was observed during dissection. Samples were analyzed using Masson's trichrome and immunofluorescence staining to study the membranous characteristics. Histological-radiographic correlations were also summarized. Results: In this series, we found that all 3 type B, 2 type C, and 8 type D tumors (according to the Kaye-Pellet grading system) were entirely extradural in location, whereas the 18 type A tumors could be subdural (9 cases) or extradural (9 cases), which frequently could not be predicted preoperatively based on whether the tumor had intraforaminal extension. The dural capsule, when present, could be used as an insulating layer to protect LCNs. With this subcapsular dissection technique, postoperative LCN dysfunction occurred in 10 patients (32.3%), which was usually temporary and mild. Conclusion: The different relationships between the tumor and membranous structures of the JF is related to the distinct point of tumor origin and the complex anatomy of the meningeal dura within the JF. Subcapsular dissection technique is recommended for better preservation of LCNs when the dural capsule is identified.

Abstract:Objective To investigate the surgical approach to resect the jugular foramen (JF)lesions in one-stage. Method With the aid of surgical microscope, twenty cadaver heads were used to study the microsurgical anatomy of high cervical part,jugular foramen and relative structures. Results Detailed dissection was performed on high cervical part between the I st cervical vertebra and the 4th cervical vertebra. The role and the effect of extended extracranial exposure of this approach were studied. The mastoid triangle was the mark of the area of mastoidectomy. Total exposure of lateral semicircular canal was completed, then The sigmoid sinus and jugular bulb were skeletonized. The vertical segment of facial nerve was fully skeletonized to study the necessity of the facial nerve translocation. A lateral suboccipital craniectomy was then performed, the posterior and medial one-third of the occipital condyle was removed. The jugular tubercle was drilled away as much as possible. Conclusions Total exposure of JF can be achieved through the modified approach we described, and muhidirectional approaches can be performed, including suprajugular, infrajugular, postjugular and extrajugular exposures. Both intracranial and extracranial tumors can be removed in a one-stage procedure. Tumors at the foramen magnum and high cervical region are accessible through this approach. In addition, it offers advantages in reducing even preventing the injury of facial nerve and other postoperative complications.
[Paracondylar-lateral cervical approach for resection of jugular foramen schwannomas: a retrospective analysis of 15 cases]

Abstract:Objective: To explore the effects of paracondylar-lateral cervical approach for resection of the jugular foramen schwannoma(JFS). Methods: A total of 15 patients with JFS operated via the paracondylar-lateral cervical approach between December 2011 and March 2016 at Department of Neurosurgery in Xiangya Hospital of Central South University were retrospectively analyzed. There were 7 males and 8 females, aging from 22 to 77 years with a mean age of (41.9±15.8) years.There were 12 patients who accepted primary surgery, 3 patients who accepted secondary surgery. There were 10 patients with tinnitus or hearing loss, 8 patients with dysphagia, 9 patients with hoarseness, 7 patients with tongue hemiparesis, 8 patients with ataxia, 1 patient with Pyramidal signs, 2 patients with facial hypesthesia or pain, 1 patient with facial paresis. According to Samii JFS grading system, 2 patients were type B, 4 were type C and 9 were type D. All patients were followed-up through outpatient and telephone by MRI in 3, 9, 12 months postoperation. Results: Gross total removal of tumors were achieved in 14 patients and subtotal removal in 1 patient. Two patients had new hoarseness, 2 had new dysphagia and 3 had more serious dysphagia, 1 patient had more serious facial paresis after the operation. There were 2 patients with intracranial infection, 2 with pneumonia, 1 with subcutaneous effusion, 1 with cerebrospinal fluid rhinorrhea, 5 with gastric intubation during perioperative period. There were no death, intracranial hematoma and decreased hearing patients. All patients were followed up, the follow-up time were 3 to 33 months with a mean of (26.9±11.2) months. Till to the latest follow up, dysphagia improved in 2 cases, hoarseness and tongue hemiparesis improved in 3 cases, hearing loss and tinnitus improved in 9 cases, balance function improved in 7 cases, facial hypesthesia and pain improved in 2 cases, pyramidal signs disappeared in 1 case, facial nerve function improved to normal in 1 case. There was no recurrence and progressed case. Conclusion: Paracondylar-lateral cervical approach is an alternative approach for both intracranial and extracranial JFS.

Methods With the aid of the magnification （× 3-40）, the microsurgical anatomy and extended exposure of the transjugluar approach were studied in 10 cadaveric head-neck specimens of adults. Results The exposure of the transjugular approach was directed through the jugular process of occipital bone to posterior surface of jugular bulk （JB）. After removal of the mastoid process and the petrous bone under the labyrinth, the vertical segment of facial nerve was skeletoned. The exposure was extended lateral to JF. The transverse process of C1 was removed and the segment of the VA between the transverse foramen of C2 and its entrance of the dura was seperated, the cervical exposure was extended. Abstract:OBJECTIVE:To discuss the method of microsurgical treatment for jugular foramen tumor (JFT).METHODS:Ten patients with dumbbell-shaped JFTs who were microsurgically treated by the same group were retrospectively studied, the surgical approaches includes infratemporal approach and modified far lateral approach according to tumors' size, blood feeding, hearing and growth manner. Cranial nerve function, pre- and postoperative complications, follow-up data were presented and discussed.RESULTS:Gross total tumor removal was achieved in 7 patients, subtotal removal in 2 cases, partial removal in 1 case, postoperative cerebrospinal fluid leaking in 1 cases, postoperative new cranial nerve defects in 1 cases, aggravation in 2 cases. Postoperative deficits of the cranial nerves improved in 80 percent of the patients. Favorable facial function in 6 months postoperatively (House-Brackmann grade system in Grade 1 and Grade 2) was noted in 7 of the 10 patients. The postoperative level of hearing was preserved in 3 of the 6 patients with residual hearing. Recurrence was noted in 1 case during the follow-up period.CONCLUSIONS:Surgical total removal of JFT is possible depends on microsurgical operation with the two approaches with lowly additional neurological deficits. The function of preoperative affected cranial nerves can be recovered.

Abstract:OBJECTIVE:To investigate the diagnosis and surgical treatment of lower cranial schwannomas of jugular foramen.METHODS:Retrospective review of 11 cranial schwannomas of jugular foramen which received surgical removal from November 2001 to June 2005. According to classification, 5 cases were as intracranial type, 3 as jugular foramen type, 1 as neck extension type and the other 2 as combination type. Surgical procedures composed of 3 translabyrinthine, 2 retrosigmoid and 6 subtemporal fossa approach.RESULTS:One-stage total tumor removal was accomplished in all the cases without any severe complications. Residue hearing was preserved in two cases received retrosigmoid approach. Anatomic completion of facial nerve was maintained in all the cases. Lower cranial neuropathy was found in 9 cases before surgery and occurred in all the cases after surgery. During in-hospital period complete compensation of swallowing ability was gained in 7 cases, near total, partial compensation and total decompensation was found in 1, 2 and 1 cases, respectively. Followed-up for 6 months, 10 cases caught normal swallowing function while the other one with partial compensation. Hoarseness was occurred in all the cases immediately after surgery and improved during follow-up. CSF leakage was occurred in 2 cases, no intracranial infection was found in all the cases.CONCLUSIONS:Surgical treatment was the optimal management to cranial schwannoma of jugular foramen. Surgical approach selections depends on tumor classification on magnetic resonance imaging features. Nerve protection in surgery and postoperative rehabilitation were the key to better prognosis.

Abstract:Objective To investigate the microsurgical anatomy of the jugular foramen and its adjacent structures so as to provide the anatomical base for the surgical approach and nerve preservation in the region. Methods With the aid of the surgical microscope, a combined approach was performed,which included transjugular approach, extreme lateral trans-condylar approach and infralabyrinthine approach. The jugular foramen and its adjacent structures were exposed from multi-directions step by step to identify the spatial relationships of important structures in this region. Results At the inner wall of skull, dural septations divided the jugular foramen into the petrosal portion, the sigmoid portion and the intrajugular compartment. The dura overlying the intrajugular compartment had two meatus, both of which were medial to the intrajugular processes. One was the glossopharyngeal meatus, and the other was the vagal meatus. The structures that traversed the jugular foramen were the sigmoid sinus, inferior petrosal sinus, jugular bulb, glossopharyngeus nervus, pneumogastric nerve, accessorius, meningeal branches of the ascending pharyngeal and occipital arteries, and the cochlear aqueduct. Conclusions It has more surgical significance to describe the petrosal, sigmoid, and intrajugular portions of the jugular foramen. The asterion can be defined as the landmark of the junction of transverse sinus and sigmoid sinus. Detailed micro-anatomic study may improve the success of surgery, protect cranial nerves and prevent unnecessary injury.

Abstract:(1) Background: Jugular foramen tumors are complex lesions due to their relationship with critical neurovascular structures within the skull base. It is necessary to have a deep knowledge of the anatomy of the jugular foramen and its surroundings to understand each type of tumor growth pattern and how it is related to the surrounding neurovascular structures. This scope aims to provide a guide with the primary surgical approaches to the jugular foramen and familiarize the neurosurgeons with the anatomy of the region. (2) Methods and (3) Results: A comprehensive description of the surgical approaches to jugular foramen tumors is summarized and representative cases for each tumor type is showcased. (4) Conclusions: Each case should be carefully assessed to find the most suitable approach for the patient, allowing the surgeon to remove the tumor with minimal neurovascular damage. The combined transmastoid retro- and infralabyrinthine transjugular transcondylar transtubercular high cervical approach can be performed in a stepwise fashion for the resection of complex jugular foramen tumors.

[Management of Jugular Foramen Schwannomas].

Abstract:OBJECTIVE:To discuss surgical approach selection and treatment strategy for preservation of the facial and lower cranial nerves' function in jugular foramen schwannomas surgery.METHODS:Retrospectively analyzed the clinical presentation, surgical approaches selection, facial and lower cranial nerves follow-up outcomes of 38 jugular foramen Schwannomas, who received operations in the Department of Otolaryngology Head and Neck Surgery, Xinhua Hospital affiliated to Shanghai Jiaotong University, School of Medicine during 2002 and 2012.RESULTS:A total of 38 patients, including 18 men and 20 women, with ages ranged from 21 to 70 years (mean 45.3). Headache or neck pain in preoperative clinical presentation was present in 27 patients (71.1%) and cervical mass in 9 patients (23.7%). Cranial nerve impairments, mainly the vagus nerve, were present in 21 patients (55.3%). MRI finding: 19 tumors were intra- and extracranial, 10 were intracranial and 9 were extracranial. According to the tumor location and region, select the surgical approaches. 25 patients used infratemporal fossa type A approach, 5 patients used petrous occipital transsigmoid approach and 8 patients used transcervial approach to remove tumors. The percentage of total resection was 92.1% (35/38). Adjunctive radiosurgery was used in the management of residual tumor in two patients. All cases obtained ultimately tumor control. Patients were followed by MRI every 6 months for 1 year. Thereafter, they performed follow-up imaging every 1-2 years, and the cranial nerve function was analyzed and evaluated. They were followed up from 26-124 months (median follow-up was 62.3 months). During follow-up period, good facial function was obtained in 36 patients (94.7%). All patients had no swallowing disorder. Complete compensation of lower cranial nerve function was achieved in all patients.CONCLUSIONS:The preoperative estimation and location of tumor in nature is of great importance in the determination of proper surgical planning of jugular foramen Schwannomas. Facial nerve and lower cranial nerve function can be preserved in maximal degree by proper surgical approaches and careful operative manipulation. Initial surgical resection followed by radiosurgery may be an effective option for some special patients.

Abstract:Objective To explore the diagnosis and microsurgical treatment and to observe the clinical effects of surgical treatment in patients with jugular foramen tumor. Methods Thirty-six patients with JF tumors who were mircosurgical treated by the same group were retrospectively studied. Cranial nerve function, preoperative and postoperative complications, follow-up data were presented and discussed. Results Gross total tumor removal was achieved in 29 patients,subtotal removal in 4 cases, partial removal in 3 cases, postoperative cerebrospinal fluid leakage in 1 cases, postoperative new cranial nerve defects in 7 cases,death in 1 case. KPS scale was adopted as a criterion to evaluate the patients'quality of life, more than 80 in 28 cases in 1 month and in 32 cases in 6 month postoperatively. Favorable facial function in 6 month postoperatively (House-Brackmann grade system in Grade 1 and Grade 2) was noted in 31 patients. The postoperative level of hearing was preserved in 11 patients with residuary hearing. Tumor recurrence occured in 3 cases in follow-up period. Conclusion According to the difference of tumor, the surgical total removal is possible depended on microsurgical operation with proper approach. Surgical treatment is the primary mode of treat9ment for tumors associated with big size and impairment of lower cranial nerves.

Abstract:Jugular foramen schwannomas (JFS) are rare, benign tumors arising from lower cranial nerves. Given their delicate location, surgical microdissection carries a substantial risk of neurological morbidity. Stereotactic radiosurgery (SRS) presents an alternative, particularly for smaller or residual tumors. This study aims to compare the outcomes and symptomatic improvements between these two treatment modalities. We conducted a retrospective analysis of 32 patients with JFS who underwent SRS (13 cases [40.6%]) or surgical dissection (19 cases [59.4%]) as their primary management at Stanford Medical Center between 1998 and 2024. Outcomes included progression-free survival (PFS), post-management adverse events based on common terminology criteria for adverse events (CTCAE), symptom improvement, overall survival (OS), and the necessity for secondary interventions. Significant differences were observed in baseline characteristics between the SRS and surgery groups, including mean age (62.52 vs. 46.87 years, p<0.001), mean largest diameter (34.38 vs. 53.66 mm, p=0.02), and median total tumor volume (6.50 vs. 19.70 mm3, p=0.009). There were no significant differences in sex or lesion shape (dumbbell vs. non-dumbbell shaped). After adjusting for baseline characteristics, no significant differences were noted in PFS (85.71 vs. 77.77%), OS (92.3 vs 100%), symptom improvement (61.53 vs. 52.63%) or mean CTCAE grade (1.31 vs. 1.58) between the SRS and surgery groups. SRS patients had significantly lower odds of requiring secondary treatment procedures after their primary intervention as compared to those in the surgery group (OR=0.01, 95% CI: 0.0001-0.59, p=0.02). Both SRS and surgery provide exceptional outcomes for JFS with no significant differences in PFS, OS, CTCAE grades, or symptom improvement between the two groups. However, SRS patients had lower odds of requiring secondary interventions for JFS. This is among the first studies comparing surgical microdissection and SRS for JFS. Future controlled prospective studies are necessary to derive more definitive conclusions.

Abstract:Aim:The surgical strategy and technique for treating intra-and extracranial tumors at jugular foramen area by far lateral transjugular approach were explored. Methods:Seventeen patients with intra-and extracranial tumors at jugular foramen area underwent operation to remove tumors by far lateral transjugular approach. Results:Thirteen tumors obtained total removal, three tumors obtained near total removal, one obtained subtotal removal. Most of the patients had greatly recovered from their dysfunction after operation. No residual or recurrent tumor was found at 3 to 42 months follow-up with MRI in 10 patients. Conclusion: The far lateral transjugular approach has been found to be helpful for exposuring and removing the intra-and extracraninal tumors totally at one time. Tumors were dissected extracranially and intracranially at jugular foramen area respectively. It is crucial to properly keep the tumor intact for completely removing the tumor and minimizing the injury of jugular bulb and the low cranial nerves. The sigmoid sinus could not be opened until it was verified occulted by examination of DSA and MRV preoperatively, along with the judgment of the operator during the operation. The dura should be completely closed at the end of the procedure.
Surgical results, technical notes and complications of jugular foramen lesions via retroauricular infratemporal fossa approach

Abstract:Objective: The objective of this study was to evaluate the clinical effect and safety of the postauricular infratemporal fossa approach (ITFA) in resecting jugular foramen lesions. Methods: All 25 patients undergoing microsurgery via postauricular ITFA from March 2015 to May 2023 in the Department of Neurosurgery, Tangdu Hospital, Air Force Military Medical University were included. The clinical and radiological data were retrospectively analyzed. Regular follow-up was carried out. Results: The mean age of all patients was 50.5±8.9 years, and 14 of them were female and 11 were male. Among the cases, lower cranial nerve schwannoma accounted for 60 % (15/25) of all tumors, jugular foramen paraganglioma accounted for 20 % (5/25), and the remaining 20 % included meningioma, chondrosarcoma, plasmacytoma, and salivary gland tumors. Total tumor resection was performed in 18 cases, subtotal tumor resection in 7 cases and partial resection in 1 case. Seven patients underwent gamma knife radiotherapy after surgery. Transient lower cranial nerve dysfunction occurred in 8 patients, and permanent lower cranial nerve dysfunction occurred in 2 patients after surgery. One patient developed facial paralysis, and one patient presented hearing loss. Conclusions: The postauricular ITFA achieved a relatively high total tumor resection rate and a lower incidence of neurological functional disorders. It is an alternative and suitable surgical approach for resecting jugular foramen lesions. Maximizing the preservation of neurological function is preferred, especially when radical resection cannot be achieved. Stereotactic radiotherapy could be used for residual tumors.

Abstract:To analysis the clinical presentation, radiological findings, surgical techniques and outcomes of jugular foramen (JF) schwannomas.We reviewed our 10-year experience in the surgical treatment of 17 patients suffered from JF schwannomas in Hua Shan Hospital, Shanghai. A total of 8 males and 9 females with a mean age of 42 years underwent surgical procedures. A relative long period of 53-month symptomatic history was shown before surgery. The main clinical presentation are vertigo and hearing difficulty in 10 cases, atrophy of unilateral muscles of tongue in 9 cases, involvement of lower cranial nerve in 8 cases. The classification of tumors was type A (at cerebellopontine angle with minimal enlargement of the JF) in five cases, type B (JF with intracranial extension) in 3 cases, type C (extracranial tumors with JF extension) in 2 cases and type D (dumbbell-shaped with both intra-and extracranial components) in 7 cases.Far lateral approaches were used in 10 cases, retrosigmoid suboccipital approaches were used in 5 cases. Submandibular approaches were selected in other 2 cases. Gross total removal was achieved in 12 cases, and subtotal removal in 5 cases. Follow-up revealed marked improvement from preoperative symptoms in 9 cases and no additional deficits in 3 cases. 5 cases suffered from additional neurological deficits. There were two cases of temporary hoarseness and gradually improved within follow-up. Two patients suffered from swallowing problems as a new deficit. One patient had facial palsy.JF schwannomas can be surgically treated with relative good outcomes. Surgical approaches should be tailored according to the tumor extension.

Abstract:We present the case of an asymptomatic 34-year-old woman who was found to have an incidental left trigeminal schwannoma.1,2 Owing to the rapid increase in size on imaging, an atypical finding for this type of lesion,3 as well as the risk of cranial nerve involvement and need for tissue diagnosis, the patient underwent a microsurgical left transorbital approach with lateral orbitotomy to resect the lesion.4,5 Piecemeal resection was performed, and the internal contents of the cavernous sinus were preserved. Ultimately, gross total resection was achieved with no residual disease identified on postoperative imaging. No new neurologic deficits were noted after surgery. The technical considerations for the transorbital approach and the literature review on the outcomes for middle fossa lesions accessed by a lateral orbitotomy are reviewed.6-10 The patient consented to the procedure and to the publication of her images. Institutional Review Board approval was deemed unnecessary due to the retrospective, case-report nature of this work.
Comparison of Endoscopic Endonasal Approach and Lateral Microsurgical Infratemporal Fossa Approach to the Jugular Foramen: An Anatomical Study

Abstract:Objective The jugular foramen is one of the most challenging surgical regions in skull base surgery. With the development of endoscopic techniques, the endoscopic endonasal approach (EEA) has been undertaken to treat some lesions in this area independently or combined with open approaches. The purpose of the current study is to describe the anatomical steps and landmarks for the EEA to the jugular foramen and to compare it with the degree of exposure obtained with the lateral infratemporal fossa approach. Materials and Methods A total of 15 osseous structures related to the jugular foramen were measured in 33 adult dry skulls. Three silicone-injected adult cadaveric heads (six sides) were dissected for EEA and three heads (six sides) were used for a lateral infratemporal fossa approach (Fisch type A). The jugular foramen was exposed, relevant landmarks were demonstrated, and the distances between relevant landmarks and the jugular foramen were obtained. High-quality pictures were obtained. Results The jugular foramen was accessed in all dissections by using either approach. Important anatomical landmarks for EEA include internal carotid artery (ICA), petroclival fissure, inferior petrosal sinus, jugular tubercle, and hypoglossal canal. The EEA exposed the anterior and medial parts of the jugular foramen, while the lateral infratemporal fossa approach (Fisch type A) exposed the lateral and posterior parts of the jugular foramen. With EEA, dissection and transposition of the facial nerve was avoided, but the upper parapharyngeal and paraclival ICA may need to be mobilized to adequately expose the jugular foramen. Conclusion The EEA to the jugular foramen is anatomically feasible but requires mobilization of the ICA to provide access to the anterior and medial aspects of the jugular foramen. The lateral infratemporal approach requires facial nerve transposition to provide access to the lateral and posterior parts of the jugular foramen. A deep understanding of the complex anatomy of this region is paramount for safe and effective surgery of the jugular foramen. Both techniques may be complementary considering the different regions of the jugular foramen accessed with each approach.
Surgical Treatment of Jugular Foramen Tumors

Abstract:Objective To evaluate the key surgical points for treatment of jugular foramen tumors.Methods Twelve patients with jugular foramen tumors treated surgically from February 2008 to December 2010 in Peking Union Medical College Hospital were reviewed retrospectively.All patients underwent CT or MRI.Their tumors were removed using microsurgical techniques and they were followed up for 14-45 months.Results Among 12 patients with jugular foramen tumors,11 underwent complete surgical resection of the tumor,1 underwent near-total resection of the tumor and postoperatively treated by gamma knife.One-stage facial nerve reconstruction was carried out in 4 cases,including 2 cases of auricular nerve grafting and 2 cases of facial-hypoglossal nerve anastomosis.The pathological results of 12 cases showed that 8 were paraganglioma,1 hypoglossal nerve sheath tumor,1 meningioma,1 chondrosteoma,and 1 middle ear cholesterol granuloma.Five cases achieved improved symptoms postoperatively,5 cases had no improvement in symptoms,and 2 cases had worsened symptoms with severe dysphagia and hoarseness,mainly for Ⅻ,Ⅹ cranial nerve permanent palsy.One of the cases with worsened symptoms underwent cricopharyngeal myotomy and typeⅠthyroplasty 5 months after operation,and the other underwent esophageal sphincter resection and typeⅠthyroplasty 7 months after operation.Both achieved significantly improved postoperative condition.Conclusions The anatomical environment of the region of jugular foramen tumors is complex,and we should choose the appropriate surgical approach based on patients' different situations and image data for individualized treatment.To completely remove the tumor and retain or reconstruct the major blood vessels and nerve function is the key point for surgical management.
Tension‐Free Anterior Rerouting of the Facial Nerve in Management of Jugular Foramen Paragangliomas

Abstract:INTRODUCTION Jugular foramen paragangliomas (JFPs) are highly vascularized tumors, although JFPs grow slowly and are considered histologically benign, the infiltrative nature of tumors tend to destroy vital neurovascularities adjacent to tumor. Since the infratemporal fossa type A approach (IFTA-A) was developed by Prof. Fisch, the IFTA-A with total anterior transposition of the facial nerve (FN) and high cervical exposure has been the standard surgical procedures for removing JFPs. Traditionally, during anterior FN translocation, the FN was dissected from the geniculate ganglion to the main trunk of FN in the parotid gland to achieve permanent translocation, and this procedure sacrificed the blood supply of the FN from the stylomastoid and deep petrous arteries, the postoperative FN function (H-B I–II) was 67.2%. To preserve more feeding arteries of the FN, Brackmann dissected the FN, the posterior digastric muscle, and soft tissues around the stylomastoid foramen anteriorly. Postoperatively, preserved FN function (H-B I–II) was noted in 73% of patients. However, it is still challenging to consistently achieve tension-free total FN anterior rerouting while applying Brackmann’s technique. In the present study, we modified the Brackmann’s total anterior FN rerouting technique to achieve tension-free FN anterior transposition and explored the surgical outcomes postoperatively.
