Abstract:Objective To discuss the middle fossa approach (MFA) for vestibular schwannoma (VS) and hearing preservation (HP). Methods Retrospectively analyze the database of HP in patients that underwent MFA for intracanalicular VS from May 2006 to May 2021. Examinations of preoperative hearing and craniocerebral imaging were performed to evaluate whether the patients met the criterion of HP. The function of the cochlear nerve was evaluated with American Academy of Otolaryngology-Head and Neck Surgery (AAO-HNS) hearing classification; the facial nerve function was evaluated with House-Brackmann (HB) grading. Different clinical characteristics regarding HP were summarized. Results A total of 22 patients with VS that met the inclusion and exclusion criteria were included in this study. Facial nerve function was HB grade Ⅰ in 21 patients, grade Ⅲ in 1 patient. The preoperative serviceable and usable HP rate were 77.3%(17/22, 95% CI: 58.3%-96.3%) and 86.4%(19/22, 95% CI: 70.8%-100%), respectively. Seventeen patients met the criterion of hearing preservation, and the other 5 got their residual hearing reserved. The tumor was completely removed in 22 patients with VS and without postoperative complications, such as cerebrospinal fluid (CSF) leakage, epidural hematoma or infection. The rates of postoperative serviceable and useful HP were 52.9%(9/17, 95% CI: 26.5%-79.4%) and 73.7%(14/19, 95% CI: 51.9%-95.5%), respectively. In the patients present and those absent of CSF cap, the rates of postoperative serviceable HP were 60.0% (3/5, 95% CI: 23.1%-96.9%), 50.0% (6/12, 95% CI: 16.8%-83.2%), and the rates of useful HP were 80.0% (4/5, 95% CI: 24.5%-100%), and 71.4% (10/14, 95% CI: 44.4%-98.5%), respectively. In the tumor originating from the superior vestibular nerve, the rates of postoperative serviceable HP and useful HP were 33.3% (2/6, 95% CI: 0-87.5%) and 57.1% (4/7, 95% CI: 7.7%-100%), respectively. Whereas in patients with tumors originating from the inferior vestibular nerve, the rates of serviceable HP and useful HP were 55.6% (5/9, 95% CI: 14.0%-96.1%) and 80.0% (8/10, 95% CI: 49.8%-100%), respectively. For the patients in the presence and those in the absence of adhesion between the tumor and the cochlear nerve, the rates of serviceable HP were 41.7%(5/12, 95% CI: 8.9%-74.4%), 80.0% (4/5, 95% CI: 24.5%-100%), and the rates of useful HP were 66.7% (8/12, 95% CI: 35.4%-98.0%), 85.7% (6/7, 95% CI: 50.8%-100%), respectively. The median follow-up was 4.9 years. None of the patients had tumor recurrence. Postoperatively, the facial nerve function was HB grade Ⅰ in 21 patients and grade Ⅳ in 1 patient. Conclusions The preoperative hearing classification and the features of imaging should be considered for complete excision of the tumor with the middle fossa approach and HP. As well, sufficient surgical skills need to be grasped in the condition of different nerve origin and the degree of adhesion between the tumor and cochlear nerve to preserve the anatomy and function of the cochlear nerve.
Microsurgery For Vestibular Schwannoma Via Retrosigmoid Transmeatal Approach With Intraoperative Monitoring Techniques

Abstract:Background: Functional preservation of cranial nerves remains an issue in surgical treatment of vestibular schwannoma.Aims: To explore the functional outcomes of vestibular schwannoma removed by microsurgery via a retrosigmoid transmeatal approach with intraoperative monitoring techniques.Study Design: A retrospective cross-sectional study was conducted on a group of patients with vestibular schwannoma operated by microsurgery.Methods: The outcomes, including the extent of tumor removal, the anatomic positions of the facial nerve, and postoperative Karnofsky performance status score, facial nerve function, and hearing function were reviewed and were statistically compared among tumor sizes (small, medium, and giant) and intraoperative monitoring types [electrophysiological monitoring only (E), electrophysiological monitoring + intraoperative imaging examination (E+I), and electrophysiological monitoring + neuronavigation (E+N)].Results: A total of 436 patients with VS received microsurgery. The position of the facial nerve was anterior in 85.5% of cases with small vestibular schwannoma. Other position patterns, especially anterior-superior and anterior-inferior, increased in tumors > 2.0 cm. Total resections were performed in all patients with small vestibular schwannoma. A total of 98.1% and 84.8% of patients with medium and giant vestibular schwannoma, respectively, had total resections. More than 90% of patients in all of the 3 monitoring groups had total resections. More than 80% of patients had excellent Karnofsky performance status score regardless of tumor size and monitoring type. After surgery, 100%, 84.4%, and 59.8% of patients with small-, medium-, and giant-sized vestibular schwannoma, respectively, had good facial nerve function. More than 70% of patients in all of the 3 monitoring groups had good facial nerve function postoperatively. The hearing preservation rate was 26.7% and 7.7% in small- and medium-sized vestibular schwannoma, respectively, and was 21.6% and 27.3% in the E group and the E+N group, respectively. The statistical analyses showed that tumor size was significantly associated with the extent of tumor resection, facial nerve localization, complications, postoperative Karnofsky performance status score, facial nerve function, and hearing function (all P = .001). Monitoring type was significantly associated with the extent of resection (P = .001). Additionally, patients in the E+N group had higher total resection rates than those in the E group (P = .001). No cerebrospinal fluid leakage and surgery-related death occurred.Conclusion: In vestibular schwannoma microsurgery, tumor size is an important parameter that affects the localization of the facial nerve, the extent of resection, postoperative outcomes and complications. Intraoperative electrophysiological techniques combined with neuronavigation may be helpful to improve the extent of resection.
Removal of Vestibular Schwannoma and Facial Nerve Preservation Using Small Suboccipital Retrosigmoid Craniotomy.

Abstract:Background Vestibular schwannoma, the commonest form of intracranial schwannoma, arises from the Schwann cells investing the vestibular nerve. At present, the surgery for vestibular schwannoma remains one of the most complicated operations demanding for surgical skills in neurosurgery. And the trend of minimal invasion should also be the major influence on the management of patients with vestibular schwannomas. We summarized the microsurgical removal experience in a recent series of vestibular schwannomas and presented the operative technique and cranial nerve preservation in order to improve the rates of total tumor removal and facial nerve preservation.Methods A retrospective analysis was performed in 145 patients over a 7-year period who suffered from vestibular schwannomas that had been microsurgically removed by suboccipital retrosigmoid transmeatus approach with small craniotomy. CT thinner scans revealed the tumor size in the internal auditory meatus and the relationship of the posterior wall of the internal acoustic meatus to the bone labyrinths preoperatively. Brain stem evoked potential was monitored intraoperatively. The posterior wall of the internal acoustic meatus was designedly drilled off. Patient records and operative reports, including data from the electrophysiological monitoring, follow-up audiometric examinations, and neuroradiological findings were analyzed.Results Total tumor resection was achieved in 140 cases (96.6%) and subtotal resection in 5 cases. The anatomical integrity of the facial nerve was preserved in 91.0% (132/145) of the cases. Intracranial end-to-end anastomosis of the facial nerve was performed in 7 cases. Functional preservation of the facial nerve was achieved in 115 patients (Grade I and Grade H, 79.3%). No patient died in this series. Preservation of nerves and vessels were as important as tumor removal during the operation. CT thinner scan could show the relationship between the posterior wall of the internal acoustic meatus and bone labyrinths, that is helpful for a safe drilling of the posterior wall of the internal acoustic meatus.Conclusions The goal of every surgery should be the preservation of function of all cranial nerves. Using the retrosigmoid approach with small craniotomy is possible even for large schwannomas. Knowing the microanatomy of the cerebellopontine angle and internal auditory meatus, intraoperating neurophysiological monitoring of the facial nerve function, and the microsurgical techniques of the surgeons are all important factors for improving total tumor removal and preserving facial nerve function. Abstract:The effectiveness of neuramide as an analgesic was assessed in 71 patients with neuritic pain in an oncological radiotherapy ward. 1-2 vials i.m. twice a day for results in 54%, poor results in 13%, and nil results in 21%. The degree of analgesia was that the drug is useful as an analgesic, anti-inflammatory, cell membrane stabilising, and neurotrophic preparation in certain well-defined pain syndromes, especially those of a secondary nature. Attention is also drawn to its excellent tolerance and the complete absence of side-effects.

Abstract:In order to characterize the epidemiology of American Cutaneous Leishmaniasis (ACL) in a periurban area of the municipality of Sabará in the metropolitan region of Belo Horizonte (MRBH), an area until then considered free of the disease, a cross sectional survey was undertaken in 1990. The survey of the population consisted of 1119 interviews and 881 clinical examinations using Montenegro's skin test (MST). A low prevalence (3.7%) of positive MST was encountered. The disease had been occurring in the area for about 20 years in the form of sporadic cases. The predominant species of sandfly both in domestic areas and nearby areas of secondary vegetation was Lutzomyia whitmani. A canine survey of delayed hypersensitivity to the antigen P10,000 identified only one dog with a positive reaction out of 113 examined. The transmission of ACL in MRBH was confirmed. The occurrence of the disease in women, children and individuals with no contact with forest areas as well as the presence of potential vector species in the domiciliar environment, suggests the transmission of the disease in this environment.
Transcanal transpromontorial approach for vestibular schwannoma: experience of a single center

Abstract:BackgroundThe expanded transpromontorial transcanal approach (ExpTTA) represents a recent addition to the surgical approaches available for the treatment of vestibular schwannoma. An initial purely endoscopic version has been complemented by the use of the microscope and it is now one of the possible surgical options for small to medium-sized vestibular schwannomas with a predominantly intracanalar development.MethodsThis is a series of 54 patients who underwent microsurgical resection of sporadic, unilateral vestibular schwannoma, mainly Koos I—II with non-serviceable hearing, between January 2016 and January 2023 using the expanded transcanal transpromontorial approach. We describe the surgical technique, focusing on anatomical landmarks, and analyzing its advantages and shortcomings. Retrospective analysis of clinical outcomes is presented, including early and late complications. The mean follow-up was 46.7 months.ResultsWe achieved gross total resection of the lesion in all cases, confirmed on the first follow-up MRI at least 6 months after each procedure. We did not record any intraoperative complication nor disease recurrence. We recorded two postoperative severe facial nerve palsies, one of which was permanent. No cases of disabling vertigo or imbalance were reported, and all patients reported full recovery of autonomy in daily activities. Three cases of otoliquorrhea were managed conservatively successfully.ConclusionsThe transcanal transpromontorial approach combines the advantages of endoscopy with the possibilities provided by microsurgery. Our experience confirms its safety in terms of surgical complications and facial nerve outcome. This approach is amongst the treatment options for small-medium schwannomas in patients with impaired hearing, especially in young patients, ensuring radical resection, disease control, and minimal morbidity.

Endoscope-assisted retrosigmoid resection of a medium size vestibular schwannoma tumor model: a cadaveric study

Abstract:Objective: To demonstrate a flexible endoscope assisted technique to perform microsurgical resection using a retrosigmoid approach of an artificial polymer tumor model that mimics a medium size (15-20mm diameter) vestibular schwannoma. Methods: Twelve bilateral retrosigmoid dissections were performed in 6 glutaraldehyde embalmed, colored silicone injected, adult cadaveric heads. Using a standard retrosigmoid approach, we first implanted the tumor model at the cerebellopontine angle (CPA) and then we resected the tumor under simultaneous endoscopic and microscopic visualizations. The resection was performed by first creating a corridor by removing the lower portion of the tumor and then by inserting through the same corridor the flexible endoscope mounted on a surgical instrument in order to accomplish early visualization of the VII-VIII complex. This early visualization of the VII-VIII complex made possible expeditious removal of the model with preservation of the VII-VIII complex. Results: We were able to successfully implant the artificial tumor in all the specimens. The post-tumor implantation CT scan confirmed the optimal CPA location of the model with its intra-porus extension. The exposure of the facial and the adjoining neuro-vascular structures was excellent during all stages of the surgical removal and was accomplished with minimal cerebellar retraction, under intermittent endoscopic-assisted control. Early visualization of the facial and vestibular cochlear nerves complex led to unhindered removal of the tumor model. Conclusions: The endoscopic-assisted microsurgical removal of a tumor model simulating a medium size vestibular schwannoma was feasible in our tumor model study emulating real surgery. Visualization of the acousticofacial bundle at the early stage of the surgical removal should theoretically decrease the risk of its inadvertent injuries as well as facilitate complete removal of the tumor. Clinical studies to validate this laboratory study are necessary.
Resection of Vestibular Schwannoma Through Middle Cranial Fossa Approach with Endoscopic Assistance

Abstract:OBJECTIVE: We sought to report the outcome of resection of vestibular schwannomas (VSs) originating from the inferior vestibular nerve that extended into the fundus of the internal auditory canal through the middle cranial fossa (MCF) approach with endoscopic assistance. METHODS: Seven patients with VSs who underwent resection through MCF approach with endoscope assistance and 12 patients with conventional microsurgery in our department were enrolled in this study. These patients' characteristics were acquired, and the surgical outcomes, as well as postoperative complications, were evaluated. RESULTS: In the endoscope group, the VS was first removed by a conventional microscopic procedure and then an endoscope was used to find and remove any residue lesions. In 3 of 7 patients, residue tumor in the space below transverse crest was found and completely removed under endoscope. Six of 7 patients (85.7%) had House-Brackmann grade I at the time of last follow-up in the endoscope group compared with 10 of 12 patients (83.3%) in the conventional microsurgery group. Serviceable hearing was successfully maintained in 2 of 3 patients (66.7%) in the endoscope group compared with 6 of 10 patients (60`)/0) in the conventional microsurgery group. Total resections were achieved in all the patients (100%) in the endoscope group compared with 9 of 12 patients (75%) in the conventional microsurgery group. CONCLUSIONS: Using an endoscope in VS resection through the MCF approach could facilitate complete removal of the lesion while minimizing the risk of hearing loss and facial paralysis. The endoscope-assisted MCF approach is especially suitable for removing an intracanalicular VS with lateral extension involving the space below the transverse crest.

Abstract:OBJECTIVE To examine the combined effects of depressive symptoms and resting heart rate on mortality risk. METHOD Analysis was performed on data from 5,936 participants in the Whitehall II study with a mean ± SD age of 61 ± 6 years. Depressive symptoms were assessed from 2002 to 2004 using the Center for Epidemiologic Studies Depression Scale (cutoff score for depression at ≥ 16). Resting heart rate was measured at the same study phase via electrocardiogram. Participants were assigned to 1 of 6 risk-factor groups on the basis of depression status (yes/no) and resting heart rate categories (< 60, 60-80, and > 80 beats/minute [bpm]). All-cause mortality was the main outcome in our analysis. Mean follow-up for mortality was 5.6 years. RESULTS In mutually adjusted Cox regression models, depression (hazard ratio = 1.93, P < .001) and resting heart rate > 80 bpm (hazard ratio = 1.67, P < .001) were independent predictors of mortality. After adjustment for potential confounding and mediating variables, participants with both depression and high resting heart rate had a 3-fold higher (P < .001) risk of death compared to depression-free participants with resting heart rates ranging from 60 to 80 bpm. This risk is particularly marked in participants with prevalent coronary heart disease. CONCLUSIONS This study provides evidence that the coexistence of depressive symptoms and elevated resting heart rate is associated with substantially increased risk of death compared to those without these 2 factors. This finding suggests the possibility that treatments that improve both depression and resting heart rate might improve survival.
Analysis of the effect of neuroendoscopy-assisted microscopy in the treatment of Large (Koos grade IV) vestibular schwannoma

Abstract:Introduction: This article aimed to investigate the effects of the endoscopic-assisted microsurgery technique on the resection of large (Koos grade IV) vestibular schwannoma (VS) and provide a prognosis analysis of the patients. Methods: A retrospective analysis of the use of the endoscopic-assisted microsurgery technique in 16 cases of large vestibular schwannoma surgery was carried out. Intraoperative nerve electrophysiological monitoring was conducted to explore the effect of neuroendoscopy on the resection of internal auditory canal tumors, protection of the facial nerve, and minimizing postoperative complications. Results: Tumors were completely removed in all 16 cases, and the facial nerve was anatomically preserved in 14 cases (87.5%). There was no postoperative cerebrospinal fluid leakage and no intracranial infection complications occurred.Following the House-Brackmann (H-B) grading system, post-operative facial nerve function was grade I in 5 cases, grade II in 6 cases, grade III in 3 cases, and grade V in 2 cases. As a result, the preservation rate of facial nerve function (H-B grade I-II) was 68.8%. All 16 patients were followed up for 3 to 24 months, and no tumor recurrence was found on enhanced MRI. Discussion: Using the endoscopic-assisted microsurgery technique in the retrosigmoid approach has many advantages over the microscopic-only approach. When compared to the microscopy-only approach, the endoscope can provide a wide-angle surgical field superior to that of a microscope in areas such as the internal auditory canal in the resection of large VS, minimize iatrogenic injuries, ensure complete removal of internal auditory canal tumors, and well as reducing postoperative complications such as cerebrospinal fluid leakage and the loss of facial and auditory nerve functions.
A Retrospective Analysis of Temporal Lobe Gliosis after Middle Fossa Resection of Small Vestibular Schwannomas

Abstract:Introduction: The middle cranial fossa (MCF) approach is a well-established procedure in surgery of the internal auditory canal, as well as with the retrosigmoid and translabyrinthine approaches. It is commonly used in the hearing-preserving microsurgery of small vestibular schwannomas (VS). The debate about the "best" approach for the microsurgery of small VS without contact to the brainstem is controversial. It has been stated that the MCF approach leads to irreversible damage to the temporal lobe, which may be evident in follow-up magnet resonance imaging (MRI) as gliosis in up to 70% of patients. Materials and Methods: This study represents a retrospective chart analysis conducted at a tertiary university hospital. Here, 76 postoperative MRIs were re-evaluated by an experienced neuroradiologist and compared with the preoperative images. Temporal lobe gliosis was classified on an ordinal scale as absent, slight, moderate or severe. Occurrence of gliosis was matched to the clinical predictors (tumor stage, tumor volume, sex, age, and side). Results: No case of severe or moderate gliosis was found in the patient group. Slight gliosis of the temporal lobe was rare and was only detected in four patients (5%). There was no relation between clinical predictors and the incidence of gliosis. Conclusions: In our cohort, postoperative MR imaging did not reveal relevant damage to the temporal lobe parenchyma. This confirms the safe concept of microsurgery of small tumors via the middle fossa approach. The severe glioses described in other studies may be caused by a forced insertion of the retractor or by more extended approaches. However, further prospective neurocognitive studies seem to be necessary in order to assess functional changes in the temporal lobe.

Abstract: To study the feasibility of reserving the endolymphatic sac in the cerebellopontine angle (CPA) and the fundus of the internal auditory canal (IAC) by the retrolabyrinthine approach. Single-center retrospective study. Through 3-dimensional preoperative computed tomography reconstruction, vestibular schwannoma (VS) resection was performed using a retrolabyrinthine approach with preservation of the endolymphatic sac in selected patients, and hearing and facial nerve functions were followed to assess the feasibility of this operation and the effectiveness of function preservation. VS was completely removed in all cases and the postoperative detectable hearing retention rate (AAO-HNS hearing rating grade A, B, and C) was 80% (4/5). Postoperative facial nerve function was well preserved (HB grade I), no leakage of cerebrospinal fluid or other cranial complications was observed in all patients, and no recurrence was observed during follow-up. With less trauma and a promising route for hearing preservation and facial nerve functions, the retrolabyrinthine approach is a potential choice for the treatment of VS located in the IAC-CPA.
Metabolic alkalosis: A less appreciated side effect of Imipenem-cilastatin use

Abstract:Acinetobacter baumannii is a common nosocomial pathogen in our intensive care unit (ICU) for which we use imipenem-cilastatin as the fi rst line antimicrobial agent. Our fi rst patient was a 29-year-old male having cervical spinal cord injury and the second patient was a 37-year-old male having blunt trauma chest. Both of the patients required prolonged mechanical ventilation and eventually had A. baumannii-associated pneumonia which was treated with imipenem-cilastatin. Both of our patients showed clinical improvement with the treatment, but during the treatment both had persistent metabolic alkalosis (after 3 and 4 days of therapy in patient 1 and 2, respectively). There were no signs and/or symptoms of cardiovascular, hepatic or renal abnormality in any of the patient and their serum cortisol, potassium, calcium, and magnesium levels were within normal limits. The urinary chloride levels were 33 mEq/L and 41 mEq/L in patient 1 and 2, respectively. Apart from antimicrobial agents both of the patients were receiving enteral nutrition and multivitamin supplements only and none received any alkaline solution or massive blood transfusions.
Complications and Their Preventions of Large Intracranial Vestibular Schwannoma via Retrosigmoid Approach: A Retrospective Review of 1,167 Cases

Abstract:Objective: To discuss the common complications from the microsurgical treatment of large intracranial vestibular schwannoma via retrosigmoid approach. Study Design: Clinical status and complications were assessed postoperatively with 14 days and at the time of follow-up. Patients and Methods: Surgical outcomes and complications were evaluated in a consecutive series of 1,167 unilateral vestibular schwannomas treated in Shanghai Huashan Hospital from 1999 to 2013. Results: Total tumor resections were achieved in 1,006 patients (86.2%). The facial nerve was preserved anatomically in 1,083 cases (92.8%) after operation, and the functional valuation of facial nerve according to postoperative House-Brackmann show 423 patients (36.2%) in grade I-II, 534 cases (45.8%) in grade III, and 210 patients (18.0%) in grade IV-VI. The main short-term postoperative complication included disequilibrium in 250 cases (21.4%), intracranial infection in 115 (9.85%), lower cranial nerve deficit in 77 cases (6.59%). Follow-up data were available for 978 of the 1,167 patients (83.8%). Long-term postoperative complications include loss of hearing and permanent facial paralysis. Conclusions: Many of these complications are avoidable. To master the clinical anatomy of the approach, using intraoperative nerve monitoring and preoperatively study the clinical data were the keys to avoid these complications.

Abstract:OBJECTIVE: To investigate the common complications from the microsurgical treatment of large intracranial vestibular schwannoma (VS) via suboccipital retrosigmoid approach and to propose strategies for minimizing such complications.METHODS: We selected all patients with large unilateral VS from the collected database (1999-2013) who underwent microsurgical resection as their initial treatment for histopathologically confirmed VS. Tumors larger than 30 Chi 20 mm were defined as large.RESULTS: A total of 1167 patients with VS were included. Gross total tumor resection was achieved in 1006 patients (86.2%). The mortality rate is 0.77%. The facial nerve was preserved anatomically in 1083 cases (92.8%), and the functional valuation of the facial nerve according to postoperative House-Brackmann scale showed 423 patients (36.2%) in grades I-II, 534 cases (45.8%) in grade III, and 210 patients (18.0%) in grade IV-VI. The main short-term postoperative complication included new hearing loss (American Institute of Otolaryngology-Head and Neck Surgery grade D) in 634 cases (54.3%), disequilibrium in 250 cases (21.4%), labial herpes in 127 cases (10.9%), meningitis in 115 (9.85%) and lower cranial nerve deficit in 77 cases (6.59%). Follow-up data were available for 978 of the 1167 patients (83.8%). Long-term complications include hearing loss (American Institute of Otolaryngology-Head and Neck Surgery grade D) (75.8%), permanent facial paralysis (11.9%), facial numbness (10.9%), tinnitus (2.96%), chronic headache (2.25%), and taste disturbance (1.43%).CONCLUSIONS: The key factors for reducing surgical complications include careful assessment of the functions of acoustic and facial nerves as well as a thorough understanding of anatomy via the retrosigmoid approach before operation, skillful microsurgical technique, and monitoring of multiple cranial nerves during resection.
In Vitro Evaluation of the Antiviral Activity of the Synthetic Epigallocatechin Gallate Analog-Epigallocatechin Gallate (EGCG) Palmitate against Porcine Reproductive and Respiratory Syndrome Virus

Abstract:In this study, epigallocatechin gallate (EGCG) palmitate was synthesized and its anti-porcine reproductive and respiratory syndrome virus (PRRSV) activity was studied. Specifically, EGCG palmitate was evaluated for its ability to inhibit PRRSV infection in MARC-145 cells when administered as pre-, post-, or co-treatment. EGCG and ribavirin were used as controls. The results showed that a 50% cytotoxic concentration (CC50) of EGCG, EGCG palmitate, and ribavirin was achieved at 2,359.71, 431.42, and 94.06 μM, respectively. All three drugs inhibited PRRSV in a dose-dependent manner regardless of the treatment protocol. EGCG palmitate exhibited higher cytotoxicity than EGCG, but lower cytotoxicity than ribavirin. EGCG palmitate anti-PRRSV activity was significantly higher than that of EGCG and ribavirin, both as pre-treatment and post-treatment. Under the former conditions and a tissue culture infectious dose of 10 and 100, the selectivity index (SI) of EGCG palmitate in the inhibition of PRRSV was 3.8 and 2.9 times higher than that of ribavirin when administered as a pre-treatment, while the SI of EGCG palmitate in the inhibition of PRRSV was 3.0 and 1.9 times higher than ribavirin when administered as a post-treatment. Therefore, EGCG palmitate is potentially effective as an anti-PRRSV agent and thus of interest to the pharmaceutical industry.

Abstract:Most patients diagnosed with neurofibromatosis type 2 (NF2) have bilateral vestibular schwannomas (VS). Through reviewing surgical method and clinical outcomes, we tried to find out a strategy for treatments in NF2 patients with VS.We retrospectively reviewed patients diagnosed pathological NF2 and have had microsurgery (MS) for VS in the PLA Army General Hospital. Seventeen patients were included from January 2000 to December 2016. Fifteen patients had progressive hearing impairment, and 7 ears were totally deaf. Computed tomography and magnetic resonance imaging were used for preoperative and postoperative evaluation. House-Brackmann (H-B) classification was used to evaluate facial function, and the hearing outcome was classified according to American Academy of Otolaryngology-Head and Neck Surgery (AAO-HNS) hearing classification system. The outcomes included functional hearing, facial function, and complications.In the 17 patients, 9 were men, and the mean age was 27.2 years old. The mean duration of disease was 38.4 months. Twenty-six VS were excised. Nine patients with bilateral VS and unilateral surgery had repeated surgery for the contralateral tumor after 3 to 12 months. The hearing preservation rate was 41.6%. In the 26 excisions for VS, 24 had intact facial nerve. In the other 2 tumor excision, damaged facial nerves had head-to-head adhesion using biological fibrin glue. The rate of facial nerve function preservation was 60%. No mortality or major complication was reported. The follow-up time ranged from 11 to 78 months with a mean value of 39 months.MS is an effective treatment for NF2 patients with VS. The operation for bilateral VS should be staged according to tumor size and bilateral hearing function. However, methods on how to preserve functional hearing and facial function remain the issue. Further randomized controlled studies are needed to find out a better treatment for NF2 patients with VS according to the overall condition.

Abstract:Malignant transformation of vestibular schwannomas (VSs), though rare, demands a comprehensive, multidisciplinary treatment approach to optimize patient outcomes. We present a rare case of malignant transformation in a VS after radiation treatment in a 58-year-old woman ( Video 1 ). Initially diagnosed with a right cerebellopontine angle VS, the patient underwent CyberKnife (Accuray, Madison, Wisconsin, USA) treatment 16 years ago, receiving 18 Gy across 3 fractions. Despite initial stability, the patient experienced progressive hearing loss, ultimately requiring a bone-anchored hearing aid implant. Years later, she presented with rapid-onset facial weakness, dizziness, nausea, and vomiting, leading to a wheelchair-bound state and nutritional support upon admission. Advanced imaging revealed a significantly enlarged tumor, characterized by brain edema and suspicious hemorrhage, prompting urgent surgical intervention. Our multidisciplinary approach, including otolaryngology collaboration, led to the decision for a right retrosigmoid craniotomy and meatotomy. This strategy aimed at maximal tumor resection while preparing for potential facial nerve sacrifice due to the suspected malignancy. Intraoperative findings confirmed the malignant nature of the tumor, necessitating facial nerve resection. Postoperatively, the patient demonstrated clinical improvement and was discharged with plans for physical therapy and adjuvant radiation therapy, considering the malignant peripheral nerve sheath transformation of the tumor. This case underscores the importance of vigilant monitoring for patients with VS, particularly patients with rapid symptom progression and tumor growth, highlighting the crucial role of adjuvant radiotherapy in treatment regimens. This study received institutional review board approval. The patient provided informed consent for the use of their image and information.
