Abstract:An intracranial serpentine aneurysm (SA) is a clinically rare entity, and very few multi-case studies on SA have been published.The present study reviewed the relevant literature available on PubMed.The studied information included the formation mechanism and natural history of SA as well as its clinical manifestation, imaging characteristics, and current treatments.After reviewing the literature, we conclude that intracranial SA can be managed surgically and by endovascular embolization, but the degree of blood flow in normal brain tissue distal to the SA must be evaluated.A balloon occlusion test (BOT) or cross compression test is recommended for this evaluation.If the collateral circulation is sufficiently compensatory, direct excision or embolization can be performed.However, if the compensatory collateral circulation is poor, a bypass surgery is necessary.Satisfactory results can be achieved in the majority of SA patients after treatment.However, the size of the aneurysm may increase in some patients after endovascular treatment.Special attention should be paid to cases exhibiting a significant mass effect to avoid subsequent SA excision due to an intolerable mass effect.Satisfactory results can be achieved with careful treatment of SA.
Clip Reconstruction of a Giant Internal Carotid Artery Bifurcation Aneurysm in Hybrid Operating Room: 2-Dimensional Operative Video

Abstract:Giant internal carotid artery (ICA) bifurcation aneurysms are technically difficult to treat because of lack of a definable neck, which makes direct clipping impossible. In addition, these lesions are related to multiple perforating arteries, posing a different challenge to surgical operators. This video demonstrates clip reconstruction of a giant ICA bifurcation aneurysm in a hybrid operating room. A 32-year-old female presented with dizziness for one year. Angiography demonstrated a giant left ICA bifurcation aneurysm. The patient was consented for both clip reconstruction and extracranial-intracranial bypass if needed. A left pterional craniotomy was performed. After dural opening, the sylvian fissure was opened widely until the aneurysm was identified. The proximal ICA was then identified. The aneurysm projected posteriorly, involving the origins of both the A1 and M1 segments. After temporary clipping, the critical perforators were dissected free from the aneurysm wall. Tandem fenestrated clipping was used to reconstruct the parent artery. Finally, curved clips were used to occlude the distal part of the neck. Vessel patency was evaluated using intraoperative Doppler and indocyanine green angiography. Intraoperative somatosensory and motor evoked potential monitoring remained unchanged after final clipping. Intraoperative angiography confirmed complete obliteration of the aneurysm and reconstruction of the parent artery. The patient recovered well without any complications. Giant ICA bifurcation aneurysms are complex lesions that require preoperative planning and intraoperative monitoring. Adherence to general principles, including careful dissection of aneurysmal wall from adjacent perforators and clip reconstruction using tandem fenestrated clipping, these lesions can be treated safely and effectively.
Surgical Treatment for Posterior Inferior Cerebellar Artery Aneurysms

Abstract:Objective To explore the clinical characteristics and surgical managements of posterior inferior cerebellar artery (PICA) aneurysms. Methods Retrospective analysis of the clinical features, imaging characteristics and clipping results of 21 PICA artery aneurysms. This series included 9 men and 12 women, mean age was 40.6 years. One presented with mass pressure effects of infrotentoria fossa; 20 patients presented with severe and sudden onset of symptoms, reflecting a hemorrhage from a PICA aneurysm. Hunt-Hess and Hess grades at presentation were: Grade Ⅰ in 1 patient, Grade Ⅱ in 14 patients, Grade Ⅲ in 5 patients, Grade Ⅳ in 1 patient. The bleeding was confined to the infrotentoria subarachnoid space (SAH) and/or the fourth ventricle. Arteriographies revealed 13 proximal PICA aneurysms and 8 distal PICA aneurysms. Results Lateral suboccipital and midline suboccipital surgical approaches were used to secure the aneurysms in all patients, either by direct clipping in 19 cases, parent artery's occlusion in 1 case and parent artery's sacrifice in 1 case. Transient CSF leak occurred in 3 patents; paresis and lower cranial nerve dysfunction and long-term coma occurred in 1 patient; mild lower cranial nerve dysfunctions occurred in 2 patients, and had good recovery at 3 months of follow-up; no complication in other patients. Conclusions The favorable outcomes and low postoperative morbidity in our patients argues that surgical clipping be considered a first-line therapeutic option. The protection of the low cranial nerves and the preservations of PICA are major contributors to good long-term results.
Diagnosis and treatment of intracranial aneurysm presenting with cerebral ischemia

Abstract:Objective To explore the clinical characteristics and surgical management of intracranial aneurysms presenting with cerebral ischemia stroke. Methods The clinical data of 4 patients with intracranial aneurysms presenting with cerebral ischemia stroke, of whom, 1 underwent clipping of the aneurysms and reconstruction of the parent artery, 2 trapping of the aneurysms and 1 sent-assisted coils embolization, were analyzed retrospectively. Results Four aneurysms were giant and thrombotic ones. The cerebral ischemia symptoms were relieved in all the patients after the treatment. Conclusions The intracranial aneurysms presenting with cerebral ischemia stoke usually are giant and thrombotic ones. The cerebral ischemia symptoms may be relieved and aneurysm rupture may be prevented by surgical occlusion or endovascular embolization of the aneurysms.

Abstract:BACKGROUND There is no standard endovascular treatment for extracranial internal carotid artery dissecting aneurysms. In the past, stent-graft isolation and stent-assisted coil embolization were commonly used for wide-necked and fusiform aneurysms. Here, we present two cases of extracranial internal carotid artery dissecting aneurysms treated successfully using the SUPERA stent. CASE SUMMARY Case 1 was a 57-year-old male patient with sudden right limb weakness and vague speech and diagnosed with cerebral infarction in February 2019. Cervical computed tomographic angiography (CTA) revealed left internal carotid artery dissection with stenosis. CTA at 2 mo showed an eccentric wide-necked dissecting aneurysm (5 mm x 5 mm x 12 mm, 10-mm neck) that was enlarged at 4 mo (7 mm x 6 mm x 12 mm, 11- mm neck). The patient underwent SUPERA stent implantation. His condition was stable in July 2020. Case 2 was a 57-year-old man who suddenly felt dizzy and developed unsteady walking in November 2019. Cervical CTA suggested right internal carotid artery dissecting aneurysm (11 mm x 9 mm x 31 mm) complicated with severe lumen stenosis (95%). The patient underwent SUPERA stent implantation. The patient had no residual symptoms and was stable in December 2020. CONCLUSION SUPERA stent implantation might achieve good results in treating wide-necked or long fusiform internal carotid artery dissecting aneurysms.

Abstract:Background Despite advancements in endovascular techniques, microsurgical treatment for posterior circulation aneurysms remains challenging. Method This report highlights the successful clipping surgery of a 17-year-old female patient with an aneurysm located at the basilar artery (BA) bifurcation and left anterior choroidal artery (AChoA). To improve exposure, the posterior communicating artery was transected. A straight fenestrated clip was then placed to repair the BA bifurcation aneurysm, followed by a curved mini clip for the AChoA aneurysm. Conclusion This report demonstrates the nuances of microsurgery in the treatment of select complex cases, which can benefit from microsurgery to achieve optimal treatment outcomes.

Abstract:Salvage treatment of postclipping recurrent aneurysms remains challenging. 1,2 The salvage microsurgical clipping is a possible intervention but sometimes difficult because of tissue adhesions around the aneurysm and previously installed clips; therefore, salvage coil embolization may have certain advantages. 1-5 However, when coil embolization is not applicable, 6 microsurgical clipping is a stand-alone curative treatment, requiring proficient and reliable microsurgical techniques. This article describes a unique case of a 70-year-old female patient complaining of a severe headache with subarachnoid hemorrhage due to a recurrent ruptured left internal carotid-posterior communicating artery aneurysm after microsurgical clipping 23 years ago. An initial attempt at coil embolization proved unsuccessful because of the aneurysm shape. Consequently, the salvage microsurgical clipping was planned. Given the additional time for trapping the internal carotid artery for the old clip removal, a superficial temporal artery-middle cerebral artery bypass was also planned. The operation entailed a superficial temporal artery-middle cerebral artery bypass, a Sylvian fissure dissection, the old clip removal, and aneurysm clipping. Intraoperatively, the complete aneurysm neck clipping was successfully performed without any complication, and patient postoperative course was uneventful. A wide surgical field should be obtained to be able to manipulate the aneurysm and old clip safely under a microscope. It is also important to temporarily trap the main artery to ensure removal of old clips and to prepare for intraprocedural ischemia using bypass after a precise assessment of hemodynamics before surgery. The patient provided informed consent for the procedure and the publication of the case along with its pertinent imaging, and this report was approved by the institutional review board at our hospital.

Abstract:The treatment of complex intracranial aneurysms has always been a great challenge in neurosurgery. Craniotomy has a high risk of clipping, there is a risk of rupture at any time, endovascular embolization is relatively low risk, but expensive and easy to relapse, the best treatment needs to be further discussed. Cavernous sinus aneurysms with pituitary adenomas are rare. This case reports a case of complex intracranial aneurysms. Chief complaints are: (1) blepharoptosis with blurred vision for 1 year, (2) headache for 3 days, and (3) digital subtraction angiography showed right internal carotid artery cavernous sinus aneurysm. Combined with the patient’s condition and family condition, the external carotid artery-radial artery-middle cerebral artery bypass was selected. After surgical treatment, the symptoms of the patients were improved, the postoperative recovery was good, and the condition was stable. After follow-up, the patient’s brain computed tomography showed intra-aneurysm thrombosis.

Abstract:Surgical treatment of large paraclinoid aneurysms remains technically challenging due to the adjacent bony anatomy and neurovascular structures. Endovascular retrograde suction decompression using a double-lumen balloon catheter facilitates clip ligation of the aneurysm. Video 1 demonstrates a large paraclinoid aneurysm that was treated with endovascular balloon occlusion and retrograde suction decompression in a hybrid operating room. A 49-year-old woman presented with progressively worsening headache. Computed tomography angiography demonstrated a large 16-mm left paraclinoid aneurysm. Neurologic examination showed no deficits. Angiography with compression of the left carotid artery showed the collateral blood flow through the posterior communicating artery. The aneurysm was exposed via the pterional transsylvian approach. A double-lumen balloon guide catheter was placed in the left internal carotid artery. Considering the risk of ischemic complications, the "trapping-evacuation" technique was not used. After balloon inflation, a temporary clip was placed on the posterior communicating artery. Retrograde suction through the guide catheter decreased the intra-aneurysmal pressure. Tandem clipping with fenestrated clips was used to occlude the aneurysm and reconstruct the parent artery. Postoperative angiography confirmed complete obliteration of the aneurysm, and the patient recovered without any neurologic deficits. Endovascular balloon occlusion obviated the need for cervical dissection of the internal carotid artery. Retrograde suction decompression and intraoperative angiography facilitated surgical clipping for large and giant paraclinoid aneurysms.
Surgical clipping of paraclinoid carotid aneurysms

Abstract:Objective To explore the clipping strategy of paraclinoid carotid aneurysms. Methods The clinical data of 11 patients with paraclinoid aneurysms who underwent aneurysm clipping in our department from February 2017 to October 2017 were analyzed retrospectively. According to modified Barami classification,2 aneurysms were type Ⅰa,3 type Ⅰb,3 type Ⅱ,3 type Ⅲa and 1 was type Ⅲb.5 aneurysms were clipped with balloon-assisted retrograde suction decompression in hybrid operation room.Clinical outcome was evaluated by Glasgow outcome scale (GOS).Results All patients received the removal of the anterior clinoid process.5 aneurysms were clipped,and 6 aneurysms were treated with clip reconstruction.Among 4 patients received a double-lumen Merci guiding balloon and retrograde suction decompression,only 2 aneurysms were deflated, whereas the other 2 aneurysms did not collapse.Intraoperative angiography or postoperative CT angiography showed 10 aneurysms were successfully obliterated,and only 1 aneurysm was residue. One patient developed decreased vision,1 patient had oculomotor paralysis.No aphasia or any limb paralysis occurred in all patients.Before discharge,all patients had good prognosis(GOS 4-5). Conclusions Clipping strategy of paraclinoid carotid aneurysms depends on classification and aneurysm size.Straight clips were used in type I,whereas fenestrated clips were used frequently in type II and III.Balloon-assisted retrograde suction decompression in hybrid operating room seems to facilitate clipping of large and giant paraclinoid aneurysms.

Abstract:We describe the case of a 51-year-old woman who underwent craniotomy and surgical clipping of a large internal carotid bifurcation aneurysm that was refractory to treatment with 2 rounds of endovascular coiling and internal carotid artery (ICA) to middle cerebral artery pipeline placement. The patient initially presented in 2004 with a 3-week history of sudden-onset headache with associated nausea. Computed tomography and lumbar puncture on presentation in 2004 were negative for subarachnoid hemorrhage. Digital subtraction angiography, however, demonstrated a large left ICA bifurcation aneurysm. The decision was made to proceed with endovascular coiling of the aneurysm. Six-month and 1-year follow-up imaging showed incomplete resolution of the aneurysm, and retreatment coiling was completed in 2005. After recoiling in 2005, the patient was lost to follow-up. During this period, she underwent pipeline placement from the ICA to the middle cerebral artery across the A1 segment at a different institution. Two years after flow diversion, the patient was referred back to our institution with further worsening of the aneurysm. Our surgical team opted for craniotomy and surgical clipping given the limited management options that this extremely complex case now presented. The patient consented to the procedure. Postoperative imaging demonstrated marked improvement in aneurysm severity. Although the current neurosurgical trend favors endovascular treatment over craniotomy, this case shows the vital importance of craniotomy and aneurysm clipping in the treatment of select patients. We also provide high-quality operative video of aneurysm clipping. 1-7

Microsurgical Treatment of Paraclinoid Aneurysms

Abstract:Objective: To explore the microsurgical treatment of paraclinoid aneurysms and evaluate its safety and efficacy. Methods: The data of 21 patients with 22 paraclinoid aneurysms receiving craniotomy between Jan. 2010 and Dec. 2017 in Peking University First Hospital were retrospectively analyzed. According to the Barami K classification, 2 aneurysms were type Ⅰa, 6 type Ⅰb,7 typeⅡ,6 type Ⅲa,1 type Ⅳ. Out of the 17 cases of saccular aneurysms, 16 aneurysms were clipped and one aneurysm was trapped following high-flow EC-IC bypass. Out of the 5 cases of blood blister like aneurysms, 2 aneurysms were wrap-clipped, 2 aneurysms were trapped following high-flow EC-IC bypass and 1 aneurysm was trapped following STA-MCA bypass. The patients were reexamined with CT angiography (CTA) or digital subtraction angiography (DSA) and followed up in outpatient or by phone call. Results: Seventeen patients with 18 paraclinoid aneurysms had received aneurysm clipping. Aneurysmal neck remnant was found in 2 cases, parent artery stenosis was found in 2 cases. In all of the four bypass cases, graft artery patency was confirmed and no recurrence of aneurysm was observed. The obliteration rate of the paraclinoid aneurysm was 91%(20/22). Eight cases with preoperative vision defect had recovered to some extent. New vision defect occurred in two cases. At discharge, 12 patients scored with Glasgow outcome scale 5, 6 patients scored 4, 2 patients scored 3, and one patient scored 1. Conclusion: Microsurgical treatment for paraclinoid aneurysm is a safe and effective method with high aneurysm obliteration rate and low aneurysm recurrence rate, and is thus a reasonable and effective complementary method for endovascular treatment.
[Salvage management and subsequent treatment after internal carotid artery injury during transnasal endoscopic surgery]

Abstract:Objective: To explore the cause, urgent management, further treatment, outcome and prevention of internal carotid artery (ICA) injury during transnasal endoscopic surgery. Methods: Five ICA injury happened during transnasal endoscopic surgery of sphenoidal sinus and (or) sellar region, involving 1 case of traumatic optic neuropathy, 1 case of sphenoidal cyst, 1 case of fungal sphenoid sinusitis, 1 case of sellar adenoid cystic carcinoma and 1 case of pituitary tumor. These five cases were from three different hospitals in 1990 - 2009, and the clinical data were collected and retrospectively reviewed. Results: Injury of ICA was related with improper anatomic localization and operative procedures. The locations of injury were in cavernous segment in 3 cases, lacerum segment in 1 case, and clinic segment in 1 case, respectively. The types of injury included 3 cases of bleeding of laceration, 1 case of carotid cavernous fistula and 1 case of pseudoaneurysm. Salvage sphenoid sinus packing with vaseline strip and subsequent intravascular stent graft implantation (2 cases) and endovascular embolization (2 cases) effectively controlled bleeding. Four cases were successfully treated without craniocerebral or ocular complications, only 1 case died of massive blood loss. Among 4 survival cases, 1 patient abandoned further therapy, the other 3 patients were cured of primary disease by reoperation or subsequent treatment. Conclusion: Preoperatively, reading carefully the imaging data, intraoperatively, identifying anatomical positions accurately, performing proper operation, and stopping bleeding decidedly, postoperatively, seeking interdisciplinary cooperation to repair vascular damages, all these procedures can effectively reduce the surgical risk of ICA injury.

Abstract:Aneurysms of the bifurcation of the internal carotid artery (ICA) represent approximately 5% of intracranial aneurysms and tend to occur in younger patients. 1-3 They typically have a superior orientation, in close relationship with the medial (branches of the anterior cerebral artery, segment A1) and lateral (branches of the middle cerebral artery, segment M1) lenticulostriate arteries (LSA), including recurrent artery of Heubner (RAH). RAH commonly originates in the junction of A1 and A2, courses medially to laterally between LSA and anterior cerebral artery, ICA bifurcation, and middle cerebral artery before entering the lateral portion of the anterior perforated substance. 4-7 Damage to these arteries are catastrophic. This complex vascular anatomy makes treatment challenging, either endovascular or microsurgical. 8-13 We present a video case of microsurgical clipping for the left ICA bifurcation aneurysm. This study was approved by the ethics committee of our institution. The patient, a 46-year-old man, was diagnosed with an unruptured ICA bifurcation aneurysm during workup for intermittent headache. Morphological characteristics of the aneurysm supported intervention over conservative treatment. Digital subtraction angiography showed a prominent proximal RAH crossing between the aneurysm neck and LSA, providing anatomical protection for clip application. Microsurgical clipping was chosen, and the procedure went uneventfully. The patient consented to the procedure and to the publication of his/her image. We discussed and demonstrated throughout the video how we used this anatomic variation and trajectory of the RAH to prevent clipping of LSA unwittingly and achieve complete occlusion of the aneurysm neck.

Abstract:Aim:The microscopic surgical anatomy of the cavernous sinus via the extended transsphenoidal approach was studied to establish an anatomic basis for surgical treatment to remove the tumor in the lateral area of internal carotid atery.Methods:Simulated surgery was performed on seven adult cadaver heads via extended transsphenoidal approach.The appearance feature of the cavernous segment of the ICA and its branch vessels and its relationship with cranial nerve were observed under microscope.Some anatomic parameters were measured.Results:The cavernous segments of the ICA have two important branches.The inferior artery of cavernous sinus arises from the lateral wall of the medial part in horizontal segment of internal carotid artery.The inferior hypophysial artery arises from the meningeal hypophyseal stem which comes from ICA.The abducent nerve in the lateral region of ICA goes into the superior orbital fissure.The average distances from the lateral edge of he cavernous segment of the ICA to the oculomotor nerve,trochlear nerve,abducent nerve and optic nerve that go into superior orbital fissure were 3.4 mm,3.4 mm,3.9 mm and 4.5 mm respectively.Conclusion:The complexity of the structure of the nerves and blood vessels in the cavernous sinus region,familiarity with the anatomic relationships in the region may help to protect the inferior artery of cavernous sinus and abducent nerve in cavernous sinus in the surgery via the extended transsphenoidal approach and reduce surgical complications.
Transcarotid flow reversal for proximal control during cerebral aneurysm clip reconstruction: illustrative case

Abstract:Background: Paraclinoid aneurysms can pose an operative challenge during clip reconstruction, given the complex surrounding anatomy and the aneurysmal tendency to maintain turgor despite standard approaches to proximal control. This report demonstrates the use of intraoperative retrograde arteriovenous shunting with the transcarotid artery revascularization (TCAR) system to assist in the safe clip reconstruction of an irregular paraclinoid aneurysm. Observations: A 33-year-old woman presented with perimesencephalic subarachnoid hemorrhage and was found to have an incidental 9-mm ophthalmic aneurysm. Coil embolization was not successful. During microsurgical clip reconstruction, the left common carotid artery was exposed to allow for proximal control as well as transcarotid arterial sheath placement. Flow reversal was instituted throughout the aneurysm dissection and clipping, with a visible softening of the aneurysm. Intraoperative angiography confirming successful clip reconstruction was performed utilizing the TCAR sheath. The case was complicated by the development of cerebrospinal fluid rhinorrhea postoperatively, requiring surgical repair. The patient has since made a complete recovery. Lessons: Transcarotid flow reversal utilizing the TCAR system has potential for use in the surgical treatment of paraclinoid aneurysms, as it may aid in softening the aneurysm for safer dissection and clip reconstruction, protect against aneurysm-associated emboli, and provide an avenue for intraoperative angiography. https://thejns.org/doi/10.3171/CASE24330.

Abstract:Vascular, Ahead of Print. BackgroundExtracranial internal carotid artery aneurysms are rare entities that arise within the carotid triangle. Although rare, they are clinically significant due to the risk of neurologic thromboembolic events, compression of cranial nerves and vasculature, rupture, and ischemia.ObjectiveSurgery is the gold-standard treatment for symptomatic patients of all ages with extracranial internal carotid artery aneurysm.MethodA 26-year-old female patient was admitted to our institution with complaints of left auricular pain, hoarseness, and uncontrollable hypertension. She had a pulsatile mass located at the left cervical region. After diagnosing the extracranial internal carotid artery aneurysm, it was decided that surgery was necessary to relieve the patient's discomfort and prevent possible complications from the aneurysm in the left carotid artery.ResultIn the operation, the aneurysmatic segment was resected and sent for histopathological evaluation. Saphenous vein graft was interposed as end to end anastomosis to the proximal and distal healthy segments of the internal carotid artery.ConclusionThis case report highlights the surgical management of a 26-year-old female patient who presented to our institution with complaints of left auricular pain, hoarseness, uncontrolled hypertension, and a pulsatile mass localized at the left cervical region and diagnosed extracranial internal carotid artery aneurysm.

Microsurgical clip reconstruction of a true PCoA aneurysm

Abstract:Approximately 25% of intracranial aneurysms originate at the internal carotid artery (ICA) and posterior communicating artery (PCoA) junction. 1 Unlike typical PCoA aneurysms, which are usually saccular, a subset known as "true" PCoA aneurysms arise directly from the PCoA. These represent about 1.3% of all intracranial aneurysms and 6.8% of PCoA aneurysms. 1 The first report of a true PCoA aneurysm dates back to 1979. 2 This video article illustrates the microsurgical clipping of a true PCoA aneurysm in a 27-year-old male with subarachnoid hemorrhage and left-sided ophthalmoplegia. CT angiography revealed a large true PCoA aneurysm, leading to the decision for microsurgical clipping. Our surgical strategy included: 1) an extended pterional approach; 2) early brain relaxation through basal cisterns and third ventricle opening; 3) Sylvian fissure dissection; 4) partial uncus resection; 5) tracing the PCoA to the aneurysm; 6) pilot clipping and thrombectomy; 7) careful aneurysm dissection and definitive clipping. The patient had an uncomplicated recovery, discharged on the fifth day with resolved third nerve dysfunction. The literature review from 2022 documented only 47 cases of true PCoA aneurysms, predominantly presenting with rupture. 3 Some studies suggest these aneurysms may have a higher rupture risk than typical ICA-PCoA junction aneurysms. 4 Microsurgical clipping is a primary treatment, often in cases associated with a fetal PCA variant. 5 Ensuring the patency of the PCoA and thalamoperforating arteries is crucial, with careful visualization of the clip's distal ends to avoid impacting nearby neurovascular structures.

Abstract:Objective:To investigate the effect of surgical treatment and preoperative assessment methods for symptomatic internal carotid artery (ICA) occlusion. <br> Methods:hTe clinical data of 11 patients with symptomatic ICA occlusion undergoing surgical treatment from January 2010 to March 2016 in Department of Vascular Surgery of Hainan General Hospital were analyzed retrospectively. Of the patients, 2 cases underwent carotid endarterectomy (CEA) and 9 cases underwent CEA plus embolectomy. Before operation, all patients underwent intracranial and cervical CTA and carotid duplex ultrasonography, and some patients underwent head CT perfusion, transcranial Doppler and whole-brain angiography. hTe perioperative and long-term results of the patients were observed. <br> Results:All patients had unilateral ICA occlusion, and 4 cases of them had a less than 50%diameter stenosis in the contralateral ICA. Occlusion was mainly located in the initial segment of ICA, and occlusive lesion extended to the skull base in 8 cases, and the length of occlusion ranged from 16 to 85 mm. hTe blood lfow of ICA was detected in the skull base by color Doppler ultrasound in all patients before operation. Recanalization was successfully achieved in 10 patients and failed in 1 case. Atfer operation, the symptoms of cerebral ischemia were signiifcantly relieved in the 10 patients, of whom 3 cases developed hyperperfusion syndrome. During the follow-up period, ICA occlusion occurred in 1 patient at 3 months atfer operation, and 1 patient died 18 months atfer operation. <br> Conclusion:For ICA occlusion, surgical treatment offers satisfactory and safe perioperative effcacy as well as good mid-and long-term results. Accurate preoperative assessment is essential for surgical success.
