This case describes the management, outcome, and problems encountered when a single lung transplant recipient developed a progressive deterioration in pulmonary function during pregnancy, attributed to accelerated obliterative bronchiolitis.
Empirical modeling of the percent depth dose for megavoltage photon beams

Abstract:Introduction This study presents an empirical method to model the high-energy photon beam percent depth dose (PDD) curve by using the home-generated buildup function and tail function (buildup-tail function) in radiation therapy. The modeling parameters n and μ of buildup-tail function can be used to characterize the Collimator Scatter Factor (S c) either in a square field or in the different individual upper jaw and lower jaw setting separately for individual monitor unit check. Methods and materials The PDD curves for four high-energy photon beams were modeled by the buildup and tail function in this study. The buildup function was a quadratic function in the form of d d 2 + n with the main parameter of d (depth in water) and n, while the tail function was in the form of e − μd and was composed by an exponential function with the main parameter of d and μ. The PDD was the product of buildup and tail function, PDD = d d 2 + n · e − μ d . The PDD of four-photon energies was characterized by the buildup-tail function by adjusting the parameters n and μ. The S c of 6 MV and 10 MV can then be expressed simply by the modeling parameters n and μ. Results The main parameters n increases in buildup-tail function when photon energy increased. The physical meaning of the parameter n expresses the beam hardening of photon energy in PDD. The fitting results of parameters n in the buildup function are 0.17, 0.208, 0.495, 1.2 of four-photon energies, 4 MV, 6 MV, 10 MV, 18 MV, respectively. The parameter μ can be treated as attenuation coefficient in tail function and decreases when photon energy increased. The fitting results of parameters μ in the tail function are 0.065, 0.0515, 0.0458, 0.0422 of four-photon energies, 4 MV, 6 MV, 10 MV, 18 MV, respectively. The values of n and μ obtained from the fitted buildup-tail function were applied into an analytical formula of S c = n E (S) 0.63μ E to get the collimator to scatter factor S c for 6 and 10 MV photon beam, while n E , μ E , S denotes n, μ at photon energy E of field size S, respectively. The calculated S c were compared with the measured data and showed agreement at different field sizes to within ±1.5%. Conclusions We proposed a model incorporating a two-parameter formula which can improve the fitting accuracy to be better than 1.5% maximum error for describing the PDD in different photon energies used in clinical setting. This model can be used to parameterize the S c factors for some clinical requirements. The modeling parameters n and μ can be used to predict the S c in either square field or individual jaws opening asymmetrically for treatment monitor unit double-check in dose calculation. The technique developed in this study can also be used for systematic or random errors in the QA program, thus improves the clinical dose computation accuracy for patient treatment.

Dependence of surface dose on atomic number in megavoltage photon beams

Abstract:Background Surface dose in megavoltage photon radiotherapy has a significant clinical impact on the skin‐sparing effect. In previously published works, it was established that the presence of medium atomic number (Z) absorbers, such as tin, decreases the surface dose. It was concluded that high‐Z absorbers, such as lead, increase the surface dose, relative to medium‐Z absorbers, due to the increased contributions from photoelectrons and electron‐positron pairs. Purpose The purpose of this investigation is to revisit these conclusions in the context of photon beams from modern linacs. Methods A metric estimating the relative intensity of charged particles emitted in the forward direction, If , was proposed using cross‐sections for the photon interactions. The If values were calculated for various absorbers using energy spectra of 6 and 10 MV photon beams from a Varian TrueBeam linac. Monte Carlo (MC) simulations were performed using TOPAS MC code to calculate the surface dose for various absorbers. Surface dose measurements were performed with 6 and 10 MV photon beams with tin and lead absorbers. Results The If values were found to decrease as a function of Z for both 6 and 10 MV photon beams indicating that the surface dose from electrons emitted in the forward direction consistently decreases with increasing Z. With the increasing Z of the absorbers, both experimental and MC‐calculated surface dose decreased without exhibiting a minimum at medium‐Z absorbers. The surface dose for lead and tin was determined to be within 1% of each other for both 6 and 10 MV photon beams using MC simulations and experimental measurements. Therefore, no statistically significant difference in surface dose was found between the tin and lead absorbers disproving the presence of any minima in the surface dose versus the Z curve as has been reported in the literature. Conclusions Surface dose for modern photon beams can be reduced using both medium and high Z absorbers since a consistent decrease in surface dose was found with increasing absorber Z.

Abstract:A set of conversion coefficients from kerma free-in-air to the organ-absorbed dose are presented for external monoenergetic photon beams from 10 keV to 10 MeV based on the Chinese mathematical phantom, a whole-body mathematical phantom model. The model was developed based on the methods of the Oak Ridge National Laboratory mathematical phantom series and data from the Chinese Reference Man and the Reference Asian Man. This work is carried out to obtain the conversion coefficients based on this model, which represents the characteristics of the Chinese population, as the anatomical parameters of the Chinese are different from those of Caucasians. Monte Carlo simulation with MCNP code is carried out to calculate the organ dose conversion coefficients. Before the calculation, the effects from the physics model and tally type are investigated, considering both the calculation efficiency and precision. In the calculation irradiation conditions include anterior-posterior, posterior-anterior, right lateral, left lateral, rotational and isotropic geometries. Conversion coefficients from this study are compared with those recommended in the Publication 74 of International Commission on Radiological Protection (ICRP74) since both the sets of data are calculated with mathematical phantoms. Overall, consistency between the two sets of data is observed and the difference for more than 60% of the data is below 10%. However, significant deviations are also found, mainly for the superficial organs (up to 65.9%) and bone surface (up to 66%). The big difference of the dose conversion coefficients for the superficial organs at high photon energy could be ascribed to kerma approximation for the data in ICRP74. Both anatomical variations between races and the calculation method contribute to the difference of the data for bone surface.

Abstract:Background: Proton minibeam radiation therapy (pMBRT) can deliver spatially fractionated dose distributions with submillimeter resolution. These dose distributions exhibit significant heterogeneity in both depth and lateral directions. Accurate characterization of pMBRT doses requires dosimetry devices with high spatial resolution and a wide dynamic range. Furthermore, the dependency of dosimetric measurements on Linear Energy Transfer (LET), as observed in conventional proton therapy, is also present in pMBRT depth dose measurements. Purpose: This work demonstrates the process of performing comprehensive dosimetric measurements to characterize the pMBRT collimator on a clinical single-gantry proton machine, utilizing commercially available dosimetry devices. Methods: The minibeam collimator is designed to be mounted on the clinical nozzle as a beam-modifying accessory. Three collimators, each with a slit opening of 0.4 mm, are thoroughly evaluated. The center-to-center (c-t-c) distances of the slits for these collimators are 2.8 mm, 3.2 mm, and 4.0 mm, respectively. High spatial resolution dosimetry devices are essential for PMBRT dose characterizations. To meet this requirement, two-dimensional (2D) dose measurement devices, Gafchromic films, are used to measure lateral profiles at various depths. Films are also used for depth dose profile measurements in solid water. Additionally, high-resolution point dose detectors, microDiamond, and Razor diode detectors are employed for lateral profile measurements at various depths. Percent depth dose (PDD) measurements of pMBRT in solid water, with various proton energies, collimators, and air gaps, are performed using Gafchromic films. The film's LET dependency for proton beams is corrected to ensure accurate pMBRT PDD measurements. The Monte Carlo simulation tool TOPAS is utilized to compare and validate all experimental measurements. Results: At depths where LET is not a concern, film dose measurements were consistent with microDiamond and Razor diode point measurements. The point detectors need to be orientated with the thin side aligned to the incoming beam. Comparison of the lateral dose profiles extracted from TOPAS simulations, films, microDiamond, and Razor diode detectors shows a passing rate exceeding 98% in 1D gamma analysis at 3% 0.1 mm criteria.However, when the microDiamond detector is orientated to face the pMBRT beam, its spatial resolution may not be sufficient to capture the peak and valley dose accurately. Nevertheless, an accuracy within 2% can still be achieved when comparing the average dose. The PDD measurements show that the peak valley dose ratio (PVDR) of pMBRT can be altered at different depths with different air gaps using the same collimator or different collimators of different c-t-c distances. Conclusion: Our study demonstrates that comprehensive dose measurements for pMBRT can be conducted using standard clinical dose measurement devices. These measurements are indispensable for guiding and ensuring accurate dose reporting in pre-clinical studies using the pMBRT technique.

Abstract:Dose distribution (depthwise and laterally) to organs outside the radiotherapy treatment field can be significant and therefore is of clinical interest from the radiation protection point of view. In the present work, measurements were performed in a locally fabricated polystyrene phantom using TLD chips (LiF-100) for different teletherapy units (Cobalt-60 gamma ray, 120 kVp X-ray and 250 kVp X-ray) to estimate the dose distribution at distances up to 40 cm from the field edge along the central axes of the field size. Finally, the dose distribution for Cobalt-60 beam energy is parameterized as a function of depth, distance from field edge, and field size and shape.

Abstract:The commissioning procedure and maintenance of safety precautions before the beam is hit the affected cells are the key factors for the success of external beam radiotherapy. The percentage depth dose (PDD), which must be measured before a linear accelerator (LINAC) is used clinically, is one of the crucial factors during commissioning. Measurements for PDDs were carried out in this study atTMSS Cancer Center, Bogura, Bangladesh, using a linear accelerator (Varian VitalBeam SN: 5199, machine completely new in Bangladesh) with 6MV,10MV and 15MV photon energies for a set of 10 field sizes keeping the same conditions such as pressure, temperature, incremental step, direction, geometry, chamber voltage and polarity. Two ionization chambers CC13 (SN:18616) and CC04 (SN:18635) were used to measure PDDs for the linear accelerator utilizing 3D water phantom (SMARTSCAN)and IBA myQA Accept software. Using a TPS beam analysis tool, PDDs were calculated (Eclipse, Version: 16.1, Algorithm: AAA and PO). The measured PDD curves for 6MV, 10MV and15MV photon beams with above mentioned field sizes and at SSD 100 cm were compared with the PDD curves of the British Journal of Radiology-25. The maximum depth doses (dmax) for reference field size 10 × 10 cm2are 15.0 mm, 24.0 mm and 29.0 mm and the PDDs at 10 cm depth (D10) are 66.80%, 73.55% and77.14% for6 MV, 10 MV and 15 MV photon energies respectively. The results for the maximum depth doses (dmax) and the PDDs at 10 cm depth (D10) are determined to be within the limit. The measured PDD curves and the PDD curves of BJR-25 exhibit good agreement.

Abstract:Dose coefficients for external photon radiation are widely used for assessment of radiation dose to public and workers due to ground surface contamination and air immersion of radionuclides released in nuclear accidents. Dose coefficients based on Chinese reference voxel phantom were presented. Photons that incident into the cylinder, which is slightly larger than phantom, is simulated using Geant4, including distributions of angle, height and energy of photons; Secondly, photons are sampled on surface of cylinder around Chinese Reference Male/Female Voxel Phantom with MCNPX 2?4?k from the above source, dose coefficients of 20 initial gamma ray energies, from 15keV to 10 MeV, are calculated. Thirdly, dose coefficients of 68 important nuclides are evaluated by combining of decay data from ICRP 107 and photon coefficients using cubic?spline fitting. GB/T 17982?2000 shows an overestimation for ground contamination while an underestimation of high Z nuclides for air immersion compared to our results.

Abstract:Synchrotron radiation (SR) represents a unique and innovative anti-cancer treatment due to its unique physical features, including high flux density, and tunable and collimated radiation generation. The aim of this work is to assess the dosimetric properties of SR in Shanghai Synchrotron Radiation Facility (SSRF) for potential applications to clinical radiation oncology. The experiments were performed with 34 and 50 keV X-rays on the BLOW biomedical beamline of SSRF and the 6 MV X-rays from ARTISTE linac for the dosimetry study. The percentage depth dose (PDD) and the surface dose of the SR X-rays and the 6 MV photon beams were performed in solid water phantom with Gafchromic EBT3 films. All curves are normalized to the maximum calculated dose. The depth of full dose buildup is about 10 mu m deeper for the monoenergetic X-ray beams of 34 and 50 keV. The beam transmits through the phantom, with a linear attenuation coefficient. The profile in the horizontal plane shows that the dose distribution is uniform within the facula, while the vertical profile shows a Gaussian distribution of the dose. The penumbra is less than 0.2 mm in the horizontal profile. Gafchromic EBT film may be a useful and convenient tool for dose measurement and quality control for the high space and density resolution. It is therefore important to gain a thorough understanding about the physical features of SR before this novel technology can be applied to clinical practice.

Abstract:Context: Low energy electron beam has been being used widely for superficial cancer treatments. In the current study a design for production of very low energy electron beam, by different thickness of Perspex spoilers, is presented that may be used for skin cancer. Aims: MCNPX Monte Carlo code was used for modeling and simulations in the current study. An energy spoiler Perspex was modeled for degrading 4 MeV electron beam of Varian 2300 CD Linac. Materials and Methods: The thicknesses of 3, 7, and 10 mm were applied before electron applicator at a distance of 42 cm from phantom surface. Dosimetric properties of new electron beams including Rp, Dmax, E0, as well as the penumbra of the beam were investigated. Results: For the 3 mm spoiler, the superficial beam output decreased to 77%, and the Dmax, R90, R50, and RP were shifted to the depths of 4, 6, 9, and 12 mm, respectively. While for 10 mm filter the results were 5.2, 3.0 and 5.0 mm for R90, R50, and Rp, respectively. In addition, the surface dose was 93% and the Dmax was shifted to the depth of 1mm for the 10mm Perspex spoiler slab. Conclusions: The presented beam provides a novel surface dose, Dmax, and RP which can be applicable for treatment of skin cancers with minimum dose to the beyond normal tissues.
On the nuclear halo of a proton pencil beam stopping in water

Abstract:The dose distribution of a pencil beam in water consists of a core, a halo, an aura and (possibly) spray. The core is due to primary protons which suffer multiple Coulomb scattering (MCS) and slow down by multiple collisions with atomic electrons (Bethe-Bloch theory). The halo is due to charged secondaries, many of them protons, from elastic interactions with H, elastic and inelastic interactions with O, and nonelastic interactions with O. We show that the halo radius is roughly one third of the beam range. The aura is due to neutral secondaries (neutrons and gamma rays). Spray denotes dose, avoidable in principle, coming in from the beam line. We have measured the absolute dose at 177 MeV using a test beam in a water tank. The beam monitor was a PPIC 'proton counter' and the field IC a dose calibrated Exradin T1. We took depth-dose scans at ten displacements from the beam axis ranging from 0 to 10 cm. The dose spans five orders of magnitude, and the transition from halo to aura is obvious. We present model-dependent (MD) and model-independent (MI) fits to these data. The MD fit has 25 parameters, and the goodness of fit (rms (measurement/fit) - 1) is 15%. The MI fit uses cubic spline fits in depth and radius. The goodness of fit is 9%. This fit is more portable and conceptually simpler. We discuss the prevalent parameterization of the core/halo originated by the PSI group. We argue that its use of T(w), a mass stopping power which includes energy deposited by nuclear secondaries, is incorrect. The electromagnetic (Bethe-Bloch) mass stopping power should be used instead. In consequence, 'Bragg peak chamber' measurements and associated corrections are, in our view, irrelevant. Furthermore, using T(w) leads to spurious excess dose on the axis of the core around midrange, which may be significant in fields involving relatively few pencil beams.

[Dosimetry of Translating Scanning Beam Irradiation].

Abstract:BACKGROUND & OBJECTIVES:It was difficult to get a good tissue dose distribution using fixed field irradiation technique in large field. But the translating scanning beam irradiation can make it. This study was designed to create a treatment method and to evaluate the dosimetry of translating scanning beam irradiation and the clinical indications.MATERIALS & METHODS:The dosage parameter was measured under the status of the translating scanning beam irradiation in the cobalt radiotherapy machine using a translating scanning beam irradiation device which was developed by Zhejiang Cancer Hospital and Zhejiang University. The ionization chamber method was used in the dosage measurement.RESULTS:(1) The translating scanning beam irradiation can increase the percentage depth dose. (2) The dosage homogeneous distribution is better in the translating scanning beam irradiation than fixed field irradiation. (3) There is no problem in dosage connection between two closed fields in one target volume. (4) The moving penumbra was produced in the translating scanning beam irradiation.CONCLUSIONS:According to the dosage distribution property of translating scanning beam irradiation, it has some obvious advantages in big fields irradiation (for example: total spinal cord, half body, and total body irradiation).
Challenges for the Implementation of Primary Standard Dosimetry in Proton Minibeam Radiation Therapy

Abstract:Background/Objectives: Spatial fractionation of proton fields as sub-millimeter beamlets to treat cancer has shown better sparing of healthy tissue whilst maintaining the same tumor control. It is critical to ensure primary standard dosimetry is accurate and ready to support the modality's clinical implementation. Methods: This work provided a proof-of-concept, using the National Physical Laboratory's Primary Standard Proton Calorimeter (PSPC) to measure average absorbed dose-to-water in a pMBRT field. A 100 MeV mono-energetic field and a 2 cm wide SOBP were produced with a spot-scanned proton beam incident on a collimator comprising 15 slits of 400 μm width, each 5 cm long and separated by a center-to-center distance of 4 mm. Results: The results showed the uncertainty on the absorbed dose-to-water in the mono-energetic beam was dominated by contributions of 1.4% and 1.1% ( = 1) for the NPL PSPC and PTW Roos chambers, respectively, originating from the achievable positioning accuracy of the devices. In comparison, the uncertainty due to positioning in the SOBP for both the NPL PSPC and PTW Roos chambers were 0.4%. Conclusions: These results highlight that it may be more accurate and reliable to perform reference dosimetry measuring the Dose-Area Product or in an SOBP for spatially fractionated fields.

Abstract:Superselective embolization is an effective method of treating epistaxis that is refractory to conservative treatment. Soft tissue necrosis is a rare complication owing to the extensive collateral blood supply of the head and neck. We describe the case of a patient who developed unilateral necrosis of the mucosa overlying the hard palate after undergoing bilateral internal maxillary artery embolization. The presence of a nasopharyngeal balloon and bilateral nasal packs for 2 days after embolization may have compressed collateral vessels in the soft palate and nose and contributed to this complication. Packing should be removed as soon as possible after embolization.

Abstract:The accurate measurement of three-dimensional (3D) dose distribution of carbon-ion pencil beams in water is crucial for regular quality assurance (QA) practice. Although ionization chambers scanned in a water tank is conventionally used for this purpose, these measuring methods are time-consuming and labor-intensive. In addition, the beam of Heavy Ion Medical Machine (HIMM) changes continuously in each extracted duration, these methods cannot obtain the accurate 3D dose distribution. To solve these problems, an integrated system (PBDOSE) is conceived and established, which employs multi-strip ionization chambers (MSICs) to measure 2D lateral profile dose distributions. The lateral profile dose distribution in the context of various depths is measured by a mobile MSIC and normalized by the measurement of a reference MSIC to reduce the effect of beam instability. On this basis, a 3D dose distribution is created by stacking those profiles in depth direction. By conducting a single depth scan within 3 min at HIMM, information such as the 3D dose distribution, lateral contour images, the Bragg curve, beam spot size, beam position, incident direction, and energy of the beam can be obtained. The Bragg curve acquired from PBDOSE with statistical uncertainties of 1.5% was almost identical to those measured by a commercial water phantom. Besides that, the lateral profiles revealed a remarkable agreement with Gafchromic EBT3 film measurements within differences less than 0.1 mm. PBDOSE shows great prospects in the accurate and efficient measurement of 3D dose distribution for carbon-ion therapy, and the clinical application of this system will improve the efficiency of regular QA procedures to a great extent.

Abstract:Purpose: It has been long known that patients treated with radiation carry a risk of developing second cancers in their lifetimes as a result of their treatments. In addition to hosting a symposium on “Secondary Cancer Risks for Emerging Radiation Treatments,” the AAPM has recently formed Task Group 158 that will provide recommendations on the dosimetry protocols for quantifying scattered radiation dose outside the treatment volume. Measurements are useful in providing a relative comparison of different treatment modalities or beam characteristics. On the other hand, more precise organ dose equivalent values need to be calculated from Monte Carlo simulations and anatomically realistic computational phantoms. This study outlines a computational platform that integrates anatomically‐refined virtual patient models with a detailed model of a medical accelerator head to calculate scattered doses to patients treated with radiation. Method and Materials: A detailed model of a 6‐MV Varian Clinac 2100C was developed in MCNPX. Several virtual patient models have been integrated with the accelerator head model in MCNPX. Various conventional and IMRT treatment plans have been considered. Results: Average organ doses are significantly different than point‐wise measurement values. The differences can be attributed to various sources of scattered radiation that deposits dose in the organ. Large dose gradients will be present throughout organs at short and intermediate distances from the tumor volume, while organs at longer distances will have a more uniform dose distribution. Conclusion: A computational platform to calculate organ dose from scattered radiation during radiation treatments has been demonstrated. Results indicate that average organ/tissue doses are significantly different than point‐wise measurements. Organ doses calculated in this manner will likely provide more accurate organ dose information for epidemiologic studies and for assessing the risk during the treatment planning.
Machine characterization and central axis depth dose data of a superficial x-ray radiotherapy unit

Abstract:Objectives. The purpose of this study is to present data from the clinical commissioning of an Xstrahl 150 x-ray unit used for superficial radiotherapy,Methods. Commissioning tasks included vendor acceptance tests, timer reproducibility, linearity and end-effect measurements, half-value layer (HVL) measurements, inverse square law verification, head-leakage measurements, and beam output calibration. In addition, percent depth dose (PDD) curves were determined for different combinations of filter/kV settings and applicators. Automated PDD water phantom scans were performed utilizing four contemporary detectors: a microDiamond detector, a microSilicon detector, an EDGE detector, and a PinPoint ionization chamber. The measured PDD data were compared to the published values in BJR Supplement 25,Results. The x-ray unit's mechanical, safety, and radiation characteristics were within vendor-stated specifications. Across sixty commissioned x-ray beams, the PDDs determined in water using solid state detectors were in excellent agreement with the BJR 25 data. For the lower (<100 kVp) and medium-energy (≥100 kVp) superficial beams the average agreement was within [-3.6,+0.4]% and [-3.7,+1.4]% range, respectively. For the high-energy superficial (low-energy orthovoltage) x-rays at 150 kVp, the average difference for the largest 20 × 20 cm2collimator was (-0.7 ± 1.0)%,Conclusions. This study presents machine characterization data collected for clinical use of a superficial x-ray unit. Special focus was placed on utilizing contemporary detectors and techniques for the relative PDD measurements using a motorized water phantom. The results in this study confirm that the aggregate values published in the BJR 25 report still serve as a valid benchmark when comparing data from site-specific measurements, or the reference data for clinical utilization without such measurements,Advances in knowledge. This paper presents comprehensive data from the acceptance and commissioning of a modern kilovoltage superficial x-ray radiotherapy machine. Comparisons between the PDD data measured in this study using different detectors and BJR 25 data are highlighted.
Nuclear halo of a 177 MeV proton beam in water: theory, measurement and parameterization

Abstract:The dose distribution of a monoenergetic pencil beam in water consists of an electromagnetic "core", a "halo" from charged nuclear secondaries, and a much larger "aura" from neutral secondaries. These regions overlap, but each has distinct spatial characteristics. We have measured the core/halo using a 177MeV test beam offset in a water tank. The beam monitor was a fluence calibrated plane parallel ionization chamber (IC) and the field chamber, a dose calibrated Exradin T1, so the dose measurements are absolute (MeV/g/p). We performed depth-dose scans at ten displacements from the beam axis ranging from 0 to 10cm. The dose spans five orders of magnitude, and the transition from halo to aura is clearly visible. We have performed model-dependent (MD) and model-independent (MI) fits to the data. The MD fit separates the dose into core, elastic/inelastic nuclear, nonelastic nuclear and aura terms, and achieves a global rms measurement/fit ratio of 15%. The MI fit uses cubic splines and the same ratio is 9%. We review the literature, in particular the use of Pedroni's parametrization of the core/halo. Several papers improve on his Gaussian transverse distribution of the halo, but all retain his T(w), the radial integral of the depth-dose multiplying both the core and halo terms and motivating measurements with large "Bragg peak chambers" (BPCs). We argue that this use of T(w), which by its definition includes energy deposition by nuclear secondaries, is incorrect. T(w) should be replaced in the core term, and in at least part of the halo, by a purely electromagnetic mass stopping power. BPC measurements are unnecessary, and irrelevant to parameterizing the pencil beam.

Abstract:Purpose The purpose of this study is to characterize the dosimetric properties of a commercial brass GRID collimator for high energy photon beams including 15 and 10 MV. Then, the difference in dosimetric parameters of GRID beams among different energies and linacs was evaluated. Method A water tank scanning system was used to acquire the dosimetric parameters, including the percentage depth dose (PDD), beam profiles, peak to valley dose ratios (PVDRs), and output factors (OFs). The profiles at various depths were measured at 100 cm source to surface distance (SSD), and field sizes of 10 × 10 cm2 and 20 × 20 cm2 on three linacs. The PVDRs and OFs were measured and compared with the treatment planning system (TPS) calculations. Results Compared with the open beam data, there were noticeable changes in PDDs of GRID fields across all the energies. The GRID fields demonstrated a maximal of 3 mm shift in dmax (Truebeam STX, 15MV, 10 × 10 cm2). The PVDR decreased as beam energy increases. The difference in PVDRs between Trilogy and Truebeam STx using 6MV and 15MV was 1.5% ± 4.0% and 2.1% ± 4.3%, respectively. However, two Truebeam linacs demonstrated less than 2% difference in PVDRs. The OF of the GRID field was dependent on the energy and field size. The measured PDDs, PVDRs, and OFs agreed with the TPS calculations within 3% difference. The TPS calculations agreed with the measurements when using 1 mm calculation resolution. Conclusion The dosimetric characteristics of high‐energy GRID fields, especially PVDR, significantly differ from those of low‐energy GRID fields. Two Truebeam machines are interchangeable for GRID therapy, while a pronounced difference was observed between Truebeam and Trilogy. A series of empirical equations and reference look‐up tables for GRID therapy can be generated to facilitate clinical applications.

Biophysical characterization of collimated and uncollimated fields in pencil beam scanning proton therapy

Abstract:Objective. The lateral dose fall-off in proton pencil beam scanning (PBS) technique remains the preferred choice for sparing adjacent organs at risk as opposed to the distal edge due to the proton range uncertainties and potentially high relative biological effectiveness. However, because of the substantial spot size along with the scattering in the air and in the patient, the lateral penumbra in PBS can be degraded. Combining PBS with an aperture can result in a sharper dose fall-off, particularly for shallow targets.Approach. The aim of this work was to characterize the radiation fields produced by collimated and uncollimated 100 and 140 MeV proton beams, using Monte Carlo simulations and measurements with a MiniPIX-Timepix detector. The dose and the linear energy transfer (LET) were then coupled with publishedin silicobiophysical models to elucidate the potential biological effects of collimated and uncollimated fields.Main results. Combining an aperture with PBS reduced the absorbed dose in the lateral fall-off and out-of-field by 60%. However, the results also showed that the absolute frequency-averaged LET (LETF) values increased by a maximum of 3.5 keVμm-1in collimated relative to uncollimated fields, while the dose-averaged LET (LETD) increased by a maximum of 7 keVμm-1. Despite the higher LET values produced by collimated fields, the predicted DNA damage yields remained lower, owing to the large dose reduction.Significance. This work demonstrated the dosimetric advantages of combining an aperture with PBS coupled with lower DNA damage induction. A methodology for calculating dose in water derived from measurements with a silicon-based detector was also presented. This work is the first to demonstrate experimentally the increase in LET caused by combining PBS with aperture, and to assess the potential DNA damage which is the initial step in the cascade of events leading to the majority of radiation-induced biological effects.
