Abstract:Laparoscopic inguinal herniorrhaphy has been well established for the management of primary and recurrent inguinal hernias. Single-incision laparoscopic surgery (SILS) has now been accepted as a less invasive alternative to conventional laparoscopic surgery. However, commercially available access devices for SILS had disadvantages such as rigidness and crowding. This series aimed to analyze the feasibility and safety of single-incision laparoscopic trans-abdominal pre-peritoneal hernioplasty (SILS-TAPP) by applying our self-made device for managing inguinal hernia. We collected and reviewed the medical records of patients who received SILS-TAPP using a self-made glove-port device between January 2014 and January 2016. All operations were performed by the same surgical team. The demographics and intra- and perioperative outcomes were evaluated. SILS-TAPP was successfully performed in 105 patients (131 inguinal hernia repairs). No major intra- and postoperative morbidities were encountered, and no conversion to a conventional 3-port approach or open surgery was required. The mean operative time was 73.5 min and the mean postoperative hospital stay was 2.1 days. Three minor short-term complications were noted, which were resolved without surgical intervention. One recurrence was diagnosed during follow-up and treated using a second TAPP procedure. SILS-TAPP was shown to be a feasible, safe procedure in patients with an inguinal hernia. A simple self-made glove-port device was proven as a practical method of SILS-TAPP.
Primary experience in single incision totally extraperitoneal approach for inguinal hernia repair

Abstract:Objective Analysis the value of single incision totally extraperitoneal approach for inguinal hernia repair. Methods During the time of Nov 2009 to Feb 2010,20 cases of inguinal hernia were repaired by single incision totally extraperitoneal approach(SITEP) with polyethylene mesh(10cm×15cm). At the same time, 25 groin hernia patients were repaired by TEP approach. Operative time, hospital time, recurrence rates and complications were analyzed. Results The average operating time was (65.3±22.8)min (SITEP group) vs (47.4±14.9)min(TEP group), Mean postoperative hospital stay time was (2.0±1.1) days (SITEP group) vs( 1.5±0.8) days(TEP group), there were two cases with complications in each group. No recurrent case occurred. Conclusion SITEP approach with polyethylene mesh is safe and eligeable.
Retrospective study on transumbilical single-incision laparoscopic versus conventional laparoscopic surgery for inguinal hernia repair

Abstract:Objective To study the clinical data of patients who underwent transumbilical single-incision laparoscopic and conventional laparoscopic inguinal hernia repair and explore the safety and feasibility of transumbilical single-incision laparoscopic inguinal hernia repair.Methods From Dec 2009 to Mar 2011,11 patients underwent transumbilical single-incision laparoscopic inguinal hernia repair.There were 2 patients with direct inguinal hernia and 9 with indirect inguinal hernia,while 10 of whom underwent totally extraperitoneal laparoscopic hernia repair(TEP) and 1 transabdominal preperitoneal laparoscopic hernia repair(TAPP).Eighteen patients underwent conventional laparoscopic inguinal hernia repair.There were 5 patients with direct inguinal hernia and 13 with indirect inguinal hernia,while 16 of whom underwent TEP and 2 TAPP.Results Blood loss(P = 0.579)and postoperative hospital stay(P = 0.839) had no significant difference.The operative time was shorter in conventional laparoscopic group which was significantly different(P = 0.016).The postoperative complications between the two groups had no significant difference after a period of 3 to 18 months.Conclusions Transumbilical single-incision laparoscopic inguinal hernia repair is safe and feasible while its clinical application needs further study.

Abstract:OBJECTIVE:To explore the feasibility of single-incision laparoscopic totally extraperitoneal hernioplasty (SILS-TEP) with self-made port for repairing of inguinal hernia.METHODS:SILS-TEP was performed in 7 inguinal hernia patients (9 sides) with conventional laparoscopic instruments and self-made port, which composed of a wound retractor, surgical gloves and 3 ordinary trocars. The clinical data and follow-up results of 7 cases were retrospectively collected and analyzed.RESULTS:The self-made port was applied for SILS-TEP uneventfully without the need of additional ports in all 7 patients (9 inguinal hernias). The median operating time was 90. 0 (70-125) min, intraoperative blood loss was 10. 0 (5. 0-20. 0) mL and postoperative hospital stay was 2.0 (2. 0-4. 0) d. The median pain scores of visual analog scale (VAS) at 6 h,12 h, 24 h and 14 d were 3(2~4), 2(1~2), 1(0~2) and 0(0~1), respectively. There were no intraoperative complications reported, and all patients were satisfied with wound healing. No hernia recurrence was observed during the 3-months of follow-up.CONCLUSION:Our initial experiences show that SILS-TEP with self-made port is a safe and feasible surgery, which can simplify the procedure with available equipments and reduce the cost, therefore can be applied in grass-root hospitals.

Abstract:Objective To explore the surgical skills and evaluate the efficacy of posterior approach herniorrhaphy for bilateral inguinal hernia via single ventral midline incision.Methods From July 2009 to December 2011,47 patients with bilateral inguinal hernia received posterior approach herniorrhaphy via single ventral midline incision,and the clinical data,including the operation time,postoperative complications,the hospital stay and the recurrence,were analyzed retrospectively.Results In 47 cases of bilateral inguinal hernia,the operation time was(51.6±10.5)min,the postoperative hospital stay was(4.7±1.2)d,the incidence of postoperative complications was 6.4%(3/47).No recurrence occurred during the follow-up of 6-37 months.Conclusion The posterior approach herniorrhaphy for bilateral inguinal hernia via single ventral midline incision is proved to be effective,safe and convenient,and it has few postoperative complications.This method is worth popularizing in the clinical practice.

Abstract:AIMS:Various single incision laparoscopic surgeries (SILS) and natural orifice transluminal endoscopic surgeries (NOTES) have been reported recently. Herein we performed SILS for totally extraperitoneal inguinal hernia repair (TEP) on three cases.CASES:Three males of 72, 49, and 73 years old with the diagnoses of bilateral primary inguinal hernia underwent single incision TEP. The operative steps of single incision TEP are very similar to those of a traditional laparoscopic TEP. The difference between them is a 2 cm infraumbilical incision for the placement of three (5 mm) trocars in single incision TEP. We preferred to use a 30° 5 mm laparoscope with some routine laparoscopic instruments during the surgical procedure. All the hernia defects were repaired with VyproII of 15 × 10 cm (Ethicon, NJ, USA). The operations took 32, 26, and 65 min, respectively, with no obvious inconvenience.RESULTS:All three patients were discharged on the second postoperative day uneventfully. The postoperative follow-up showed no recurrence in the three patients up to now.CONCLUSION:The single incision TEP using an access port device is safe and feasible. Meanwhile SILS may reduce medical costs and complication rates through practice and improvement of SILS instruments.
Laparoscopic Experience for Recurrent Inguinal Hernia Repair in a Single Center for 14 Years

Abstract:Laparoscopic techniques are now well pervading in the treatment of inguinal hernia. This study aims to investigate the laparoscopic strategy for recurrent inguinal hernia repair. Laparoscopic technique was retrospectively applied to 330 patients with 352 recurrent inguinal hernias in the past 14 years. The surgical strategies were further evaluated. There were 22 cases with bilateral recurrent hernias, whereas the rest 308 cases with unilateral disorders. Patients were further categorized by previous repair approaches as high ligation, sclerotherapy, conventional suture repair, Lichtenstein repair, plug and patch repair, and preperitoneal repair. All cases were successfully repaired by laparoscopic approaches including transabdominal preperitoneal (TAPP) (288 cases), totally extraperitoneal (50 cases), and intraperitoneal onlay mesh (14 cases). The median operation duration was 39.5 ± 13.4 minutes. The average Visual Analog Scales score on postoperative day 1 was 2.4 ± 1.1. The median follow-up time was 36 (14–61) months. There was one case of recurrence during the follow-ups. One severe complication, i.e., bowel injury, was observed and cured, whereas other complications were as follows: 22 seroma, 8 urinary retention, 3 transient paresthesia, and 1 ileus. Laparoscopic procedures for recurrence inguinal hernia are safe and applicable. A surgeon can choose to reinforce the myopectineal orifice or only fix the hernia defect accordingly. The strategy of choosing TAPP and/or totally extraperitoneal depends on the type of previous repair, the exact anatomical position of the previous implanted mesh, and more importantly, the surgeon's experience. Moreover, the intraperitoneal onlay mesh technique can be regarded as a backup option for TAPP in certain cases.

Abstract:The tension-free inguinal hernia repair can falls into three different surgical techniques including premuscular repair,plug and patch repair and preperitoneal repair. The method of first two repairs the posterior wall of the inguinal canal and the hernia defect,and the latter repairs all of the weak area of the groin area,that is myopectineal orifice. Preperitoneal repair can be achieved through open surgeries or laparoscopic approach. Actually,laparoscopic inguinal hernia repair(LIHR) is one of the preperitoneal repair,which is through "real posterior approach",under "direct vision" and by the "laparoscopic instruments". The three clinical suitable crowd:(1) The patients with preperitoneal repair indications,such as the patients with old aged and the transversalis fascia feebleness,direct hernia or complex hernia and the abdominal cavity internal pressure increased and so on;(2) The patients need recover physical activity as soon;(3) Recurrent and bilateral inguinal hernia patients. Laparoscoic and open surgery are both safe and efficient tension-free techniques for inguinal hernias repair. The reasonable choice of repair method can offer the best clinical and cost-effective results.
Tyco Mesh Plug and Preperitoneal Anterior Approach in Tension-free Repair for Inguinal Hernia: A Prospective Randomized Controlled Study

Abstract:Objective To evaluate the therapeutic effect of the two different open tension-free herniorrhaphies——Tyco mesh plug and preperitoneal anterior approach.Methods In a prospective randomized controlled study,67 cases with 69 hernias(2 cases with bilateral hernias) were allocated randomly to either the Tyco mesh plug group(34 cases) or the preperitoneal anterior approach group(33 cases).The former was received Rutkow hernia repair using Tyco mesh plug,the later was received preperitoneal tension-free hernia repair via open anterior approach with the polypropylene mesh.Results No significant differences were found between the two groups in the operation time,hospital stay,time for returning to work,incision complications or the rate of scrotal hydrocele(P0.05).But the mean cost of treatment in preperitoneal anterior approach group was significantly less than that in Tyco mesh plug group(P0.05).However,in the aspects of recurrence rate,postoperative pain in the groin,and foreign body sensation,the outcome of preperitoneal anterior approach group was also significantly lower than that in the Tyco mesh plug group.Conclusions The preperitoneal anterior approach in repairing of inguinal hernia offers advantages of reducing the operative expense(by using only single sheet mesh),relieving the postoperative groin pain and foreign body sensation.It has characteristics of simple,security and inexpensive,and deserving to be popularized.
Subxiphoid Top-Down Endoscopic Totally Preperitoneal Approach (etpa) for Midline Ventral Hernia Repair.

Abstract:Midline abdominal wall hernia repair is among the most common surgical interventions performed worldwide. However, the optimal surgical technique remains controversial. To overcome the disadvantages of both open and transabdominal procedures, we developed a totally endoscopic preperitoneal approach (eTPA) with placement of a large mesh. From December 2019 to October 2020, 20 consecutive patients with small to medium-sized midline ventral hernias underwent repair using a completely preperitoneal subxiphoid top-down approach. The preperitoneal space was entered directly below the xiphoid, and careful endoscopic development of the plane between the peritoneum and posterior sheath of the rectus fascia was then performed behind the linea alba. The hernia sac and its contents were identified and reduced. The hernia defect was closed with sutures, and a mesh with an adequate high defect: mesh ratio was placed in the newly created preperitoneal space. Twenty patients were enrolled in this study, including 14 with primary umbilical hernias, 4 with primary epigastric hernias, and 2 with recurrent umbilical hernias. 15 patients suffered from a mild concomitant diastasis recti. All operations were successfully completed without conversion to open repair. The mean operative time was 105.3 min (range, 60–220 min). Postoperative pain was mild, and the mean visual analog scale score for pain was 1.8 on the first postoperative day. The average postoperative hospital stay was 1.8 days (range, 1–4 days). One patient developed a minor postoperative seroma, but it had no adverse impact on the final outcome. No patients developed recurrence during the 3- to 10-month follow-up period. The subxiphoid top-down totally endoscopic preperitoneal approach (eTPA) technique is feasible and effective. It may become a valuable alternative for the treatment of primary small- (defect size < 2 cm) and medium-sized (2–4 cm) midline ventral hernias, particularly in presence of a concomitant diastasis recti.

Abstract:BACKGROUND:Total extraperitoneal (TEP) inguinal hernia repair gained popularity in the past two decades for its advantage, especially single incision laparoscopic surgery (SILS) for TEP inguinal hernia repair (SILS TEP), however, which is a technically demanding procedure.METHODS:The study retrospectively analyzed 6 cases of the superior of inguinal ligament approach of S-SILS TEP repair from August 2015 to July 2016. A detailed review of medical records and the perioperative data including patient characteristics, operative details, and postoperative hospital stay was conducted.RESULTS:S-SILS TEP repair for inguinal hernia was successfully completed for 6 patients, and the mean operation time was 59.2 minutes (range 55-70 minutes), and the mean blood loss was 7.5 mL (range 5-15 mL) and the postoperative recovery was uneventful about 3 days after operation with a single wound over inguinal ligament.CONCLUSIONS:S-SILS TEP repair provides a new operative view, which combines the advantage of laparoscopic and open surgery, especially in surgical technique and minimal invasiveness. Our experience suggests that the S-SILS TEP repair may be a safe and feasible alternative in inguinal hernias repair.
Triangle Trocar Configuration in Laparoscopic Totally Extraperitoneal Inguinal Hernia Repair: A Prospective Randomized Controlled Study

Abstract:Background: In totally extraperitoneal (TEP) operation, when trocars are arranged with midline configuration, operative instruments can easily interfere with each other because of the small operative angle. The triangle trocar configuration, which creates a large operative angle, may minimize interference. Therefore, we evaluated the use of triangle trocar configuration in TEP inguinal hernia repair. Methods: A prospective randomized controlled study was conducted in 113 patients of laparoscopic TEP inguinal hernia repair in the Department of Hernia and Abdominal Wall Surgery, Shanghai East Hospital, between July 2016 and June 2017. Patients were randomly assigned to TEP laparoscopic inguinal hernioplasty with triangle trocar configuration (study group, n = 59) or midline trocar configuration (control group, n = 54). Perioperative outcomes (operative time, operative complications, postoperative pain, hospital stay, and costs), early postoperative complications (seroma/hematoma and uroschesis), and midterm outcomes (late postoperative complications and recurrence) were observed and compared. Results: After a mean follow-up of 10.21 +/- 2.32 mo, there was no significant difference in operative time, operative complications, postoperative pain, postoperative hospital stay, costs, postoperative complications, and recurrence rate between the two groups. The indirect hernia sac dissection time was shorter in the study group than in the control group. Conclusions: Triangle trocar configuration in TEP laparoscopic hernia repair is safe and reliable and is an option for hernia surgeons. The technique creates a large operative angle and avoids interference between endo-instruments, which facilitates TEP and decreases the indirect hernia sac dissection time. (C) 2019 Elsevier Inc. Comparison of the open tension-free mesh-plug, transabdominal preperitoneal (TAPP), and totally extraperitoneal (TEP) laparoscopic techniques for primary unilateral inguinal hernia repair: a prospective randomized controlled trial

Abstract:Background: The open tension-free mesh-plug hernia technique, transabdominal preperitoneal (TAPP) technique, and totally extraperitoneal (TEP) laparoscopic technique all are common surgical procedures for primary unilateral inguinal hernia repair. However, the choice of the right surgical procedure still is controversial in China. This study aimed to compare open tension-free hernioplasty with two laparoscopic hernia repairs. Methods: In this study, 164 male patients with primary unilateral inguinal hernia were randomized to undergo an open operation with mesh-plug and patch, TAPP, or TEP. Results: Completion of the study required 3 years, from February 2006 to February 2009. Of the 164 patients, 62 underwent open repair, 50 had TAPP, and 52 had TEP. The patients then were followed up for 15.6 ± 8.5 months. The average operating time for the open repair group was significantly shorter than for the other two groups (p < 0.001). The cost for the open repair group also was significantly less than for the other two groups (p < 0.001). By contrast, the pain scores in the open mesh group were significantly higher than in the other two groups (p < 0.001). The hospital stay and the recovery time both were significantly longer in open repair group than in the other two groups (p < 0.001). No major complications or recurrence was found in any of the groups. Conclusions: The findings show that open tension-free mesh-plug hernia repair, TAPP, and TEP are safe and effective for patients with primary unilateral inguinal hernia. Both TAPP and TEP are superior to open repair in terms of less postoperative pain and faster recovery time. The authors therefore recommend laparoscopic repair techniques as the preferable choice of surgical procedure. However, they think open repair will remain a practical solution in China because of its lower cost, short learning period, and need for no special equipment.
A study on the approach to laparoscopic repair for groin hernias

Abstract:Objective To compare the safety and efficiency of laparoscopic transabdominal preperitoneal (TAPP) and totally extraperitoneal (TEP) procedure for herniorrhaphy. Methods Between Jan 1997 and Jan 2005, 235 patients (274 hernias) underwent laparoscopic inguinal hernia repair with 163 TAPP in 139 patients and 111 TEP procedures in 96 patients. Postoperative patients were followed-up for 6-24 months. Results No conversion was necessary. There were no significant differences between TAPP and TEP when considering operating time, postoperative hospital stay, VAS and time to return to daily activity. The recurrence rate was 1. 8% and 0. 9% respectively in TAPP and TEP( P = 0. 524). The total incidence of postoperative complications were 15. 3% and 10. 8% respectively ( P = 0. 282 ) , with seroma, transient neurapraxia and urinary retention being 5. 5% vs. 4. 5% ( P = 0. 707 ) , 5. 5% vs. 3. 6% ( P = 0.464) and 3. 1% vs. 2.7%(P = 1.000). The inhospital cost was significantly higher in TAPP than in TEP(P=0.000). Conclusion TAPP and TEP are both safe and efficient tension-free technique. Surgeons' experience was more important than choice of approach to laparoscopic repair for groin hernias.
[The Surgical Strategy for Laparoscopic Approach in Recurrent Inguinal Hernia Repair: 213 Cases Report].

Abstract:OBJECTIVETo evaluate the surgical strategy for laparoscopic approach in recurrent inguinal hernia repair.METHODSBetween January 2001 and December 2011, the clinical data of 213 patients with 225 recurrent inguinal hernias underwent laparoscopic repair were retrospectively analyzed. There were 202 male and 11 female patients and their average age were (67 ± 14) years old (range 22-97 years old). The surgical methods included laparoscopic transabdominal preperitoneal hernia repair (TAPP, n = 174), total extraperitoneal herniorrhaphy (TEP, n = 41) and intraperitoneal onlay mesh (IPOM, n = 10). The follow-up period ranged from 15 to 60 months (median 42 months). No patient lost to follow-up during the investigation.RESULTSThe 48 TAPP and 26 TEP were applied to 77 (32.9%) recurrent hernias after conventional suture repair; 23 TAPP and 15 TEP were utilized to 38 (16.9%) recurrent hernias after Lichtenstein repair; 90 TAPP and 1 IPOM were employed for 91 (40.4%) recurrent hernias after Patch and Plug repair; and the other 22 (9.8%) recurrent hernias after preperitoneal repair were repaired by using 13 TAPP and 9 IPOM techniques. No conversion to open surgery was observed. The average operative time was (39 ± 14) minutes (range 15-90 minutes). No patients required analgesia postoperatively. The postoperative average VAS score was 2.4 ± 1.1 (range 1.2-6.4). The average hospital stay was (1.7 ± 1.5) days (range 1-9 days), and the patients returned to unrestricted activities in 2 weeks was 99.6% (212 cases). No recurrence was observed during the follow-up. The accumulative postoperative complications rates was 11.1% (25 cases), with 1 severe complication (surgical intervention was needed) as intraabdominal infection (0.4%), as well as other 24 complications including 15 cases of seroma (6.7%), 5 cases of urinary retention (2.2%), 3 cases with transient paresthesia (1.3%) and 1 case with paralytic ileus (0.4%).CONCLUSIONSTAPP and TEP are both feasible and efficacious techniques to treat recurrent hernias after suture repair and Lichtenstein repair, while the choice depends on surgeons' experience. Most recurrent hernias after Patch and Plug repair could be treated successfully by TAPP but TEP technique is not encouraged. For recurrences after preperitoneal repair, the TAPP repair should be recommended as first choice, while IPOM is a good technique to cope with the cases which TAPP failed.
Laparoscopic Hernioplasty using Autologous Tissues for Inguinal Hernia in Adults

Abstract:Objective To explore the feasibility of repairing and strengthening the posterior wall of the inguinal canal with laparoscopic hernioplasty using autologous anterior abdominal wall tissues.Methods From January 2001 to September 2007,173 adult patients with inguinal hernia(141 cases of indirect hernia,28 cases of direct hernia,3 cases of femoral hernia,and 1 case of obturator hernia)were treated by laparoscopic hernioplasty with autologous tissues obtained from the median,internal,and lateral umbilical folds,and the U-shape fold.Three trocars were used to perform the laparoscopy by one surgeon,who completed the intracavity suturing using two hands.Results All the 173 operations were completed without converting to open surgery.Among the 152 cases of unilateral hernia,the mean operation time was 25 min(10-40 min),while in the 21 cases of bilateral hernia,the mean operation time was 40 min(20-60 min).Postoperative complications included retroperitoneal hematoma(1 case),scrotal swelling(3),testicular hydrocele(1),subcutaneous hemorrhage at the inguinal area(2),and scrotitis(1).The patients were followed up for 1-84 months(mean 35 months),during which 3 patients had recurrence.Conclusions Laparoscopic hernioplasty with autologous tissues is feasible and safe for adult patients with inquinal hernia with advantages of short hospitalization,minimal surgical trauma,quick recovery,and low rate of recurrence.

Abstract:Objective To investigate the safety and efficacy of laparoscopic transabdominal preperitoneal (TAPP) and totally extraperitoneal (TEP) repair in the treatment of inguinal hernia.Methods This study was conducted from January 2015 to June 2016.282 patients with groin hernias were performed with either TAPP or TEP techniques.A total of 145 patients were in TAPP group and 137 were in TEP group.The operation time,hospital duration,postoperative pain,cost and postoperative complications were compared in both groups.Results The operation time of TAPP was (55.3 ± 11.3) minutes,and the operation time of TEP was (56.5 ± 15.4) minutes,there was no significant difference (P ＜ 0.05).There were also no significant differences in the hospital stay,hospital cost,recurrence,and acute and chronic postoperative pain between the two groups.While,there were significant more seroma in TAPP group.We speculated that it was due the patient selection bias in TAPP group.Conclusions The TEP and TAPP techniques of laparoscopic repair of inguinal hernia are comparable regarding most of the parameters,and they are both safe and effective.Surgeons can choose one of the techniques according to their skills and patients' characteristics.

Abstract:Objective: To take a new look at the preperitoneal groin anatomy and investigate the surgical approach of preperitoneal inguinal hernia repair with polyethylene mesh.Methods:(1)Five fresh male cadavers were dissected to make preperitoneal groin anatomy research(2)From May 2005 to May 2006,330 groin hernias(292 patients) were treated with an open preperitoneal approach with polyethylene mesh.From November 2002 to May 2006,240 groin hernias(211 patients) were treated with TEP approach with unstapled polyethylene mesh(10cm×15cm).Patients with recurrent hernias were excluded.During operative and hospital time,symptoms were analyzed and recorded,the recovery after operation were also observed and recorded.Results: The preperitoneal space and inferior epigastric vessels are between the anterior and posterior laminae of the transversus fascia.The mesh was placed in the preperitoneal space and covered myopectineal office.Average operating time of open preperitoneal approach with polyethylene mesh was 43 min(range: 20/65).Recurrent rate was 0.91 %(3 cases of 330 patients).Complication rate was 7.3 %(24 patients).Average hospital time was 1.7 days(range: 1/4).No recurrent case occurred in TEP group during the follow-up time of 21 months(range: 1/42).The average operating time was 57 min(range: 30/70).Complication rate was 6.7%(16 cases of 240 patients) Average postoperative hospital time was 2.8 days(range: 1/6).Conclusions: From the point of view of properties of preperitoneal groin anatomy,Using enough large mesh to mend and cover the hernia cater the principle of mechanics.So the putpreperitoneal hernia repair approach with polyethylene mesh is rational and feasible.

Abstract:Background: Incisional hernia (IH) is an abdominal wall defect due to a previous laparotomy, and surgical repair is the only treatment. IH has a negative impact on patients' quality of life. In the last decades, the approach has improved from open to laparoscopic and robotic surgery with the objective of promoting better abdominal wall function after reconstruction. Today, robotic enhanced-view totally extraperitoneal (reTEP) is one of the most advanced techniques for abdominal wall reconstruction. Aims: The aim of this study was to analyze the early results of patients with incisional hernia submitted to repair with reTEP. Methods: This is a retrospective cohort study, and all patients who underwent reTEP surgery for ventral hernia in the years 2021 and 2022 were included. The only exclusion criteria were patients who underwent another type of herniorrhaphy. Statistical analysis was performed using the Stata software. Results: A total of 32 participants were submitted to reTEP; the majority had an incisional hernia, and according to the European Hernia Society, EUS-M score 3 was the most prevalent. The mean surgical time was 170 min, and the console time was 142 min. Most patients stayed 2 days in the hospital. No intraoperative complications were reported. Conclusions: reTEP is a safe and effective technique and has favorable outcomes in the early postoperative period. Further studies with larger sample sizes and longer follow-up periods are needed to confirm these findings.

Abstract:Introduction: Bilateral inguinal hernia is a distinct entity in the inguinal hernia category. Open and minimally invasive techniques for the treatment of bilateral inguinal hernia have been previously described. If resources and surgeon expertise are available, guidelines recommend laparoscopic repair for this entity. Methods:We analyzed data from 83 patients who underwent laparoscopic inguinal hernia repair (total extraperitoneal repair - TEP) of 158 hernias (146 inguinal hernias and 12 other types). Patients had bilateral symptomatic hernias. Results:Male predominance, with a mean age of 56.7 years, was noted. Lateral hernias (according to EHS classification) were prevalent (71.08%). In the majority of cases (77.11%), meshes made up of a custom polypropylene monofilament mesh were used, followed by Bard 3D Max mesh and Ultralight mesh. Regarding postoperative complications, seroma was the most frequently encountered one in our series (7.23%), followed by urinary retention and 'feeling' of mesh (2.41%). Hydrocele, wound hematoma, cord hematoma and chronic pain were seen in 1.20% of patients. No wound infections were observed. The average operative time was 97.77 minutes (SD=17.08); when associated surgery was present, it prolonged the operative time, and we found statistical significance (p=0.002). Similarly, the presence of recurrent hernia extended the operative time, which was found to be statistically significant (p=0.003). The conversion rate in our data was 2.41%. Drainage, which was performed in 13 patients (15.66%), decreased the incidence of complications, especially seroma (p=0.026). The mean length of hospital stay was 2.93 days (SD=1.81), with most of the patients having been discharged on the second postoperative day (37.35%). Only one recurrence was identified (1.20%). Conclusion:The laparoscopic approach for bilateral inguinal hernia treatment is feasible and has been proven to be advantageous. Our study emphasizes that the TEP procedure has low rates of complications, conversion and recurrence; hence, we recommend bilateral hernia repair.
