Abstract:Laparoscopic inguinal herniorrhaphy has been well established for the management of primary and recurrent inguinal hernias. Single-incision laparoscopic surgery (SILS) has now been accepted as a less invasive alternative to conventional laparoscopic surgery. However, commercially available access devices for SILS had disadvantages such as rigidness and crowding. This series aimed to analyze the feasibility and safety of single-incision laparoscopic trans-abdominal pre-peritoneal hernioplasty (SILS-TAPP) by applying our self-made device for managing inguinal hernia. We collected and reviewed the medical records of patients who received SILS-TAPP using a self-made glove-port device between January 2014 and January 2016. All operations were performed by the same surgical team. The demographics and intra- and perioperative outcomes were evaluated. SILS-TAPP was successfully performed in 105 patients (131 inguinal hernia repairs). No major intra- and postoperative morbidities were encountered, and no conversion to a conventional 3-port approach or open surgery was required. The mean operative time was 73.5 min and the mean postoperative hospital stay was 2.1 days. Three minor short-term complications were noted, which were resolved without surgical intervention. One recurrence was diagnosed during follow-up and treated using a second TAPP procedure. SILS-TAPP was shown to be a feasible, safe procedure in patients with an inguinal hernia. A simple self-made glove-port device was proven as a practical method of SILS-TAPP.
Safety and efficacy of totally extraperitoneal and transabdominal preperitoneal laparoscopic inguinal hernia repair

Abstract:Objective To investigate the safety and efficacy of laparoscopic transabdominal preperitoneal (TAPP) and totally extraperitoneal (TEP) repair in the treatment of inguinal hernia.Methods This study was conducted from January 2015 to June 2016.282 patients with groin hernias were performed with either TAPP or TEP techniques.A total of 145 patients were in TAPP group and 137 were in TEP group.The operation time,hospital duration,postoperative pain,cost and postoperative complications were compared in both groups.Results The operation time of TAPP was (55.3 ± 11.3) minutes,and the operation time of TEP was (56.5 ± 15.4) minutes,there was no significant difference (P ＜ 0.05).There were also no significant differences in the hospital stay,hospital cost,recurrence,and acute and chronic postoperative pain between the two groups.While,there were significant more seroma in TAPP group.We speculated that it was due the patient selection bias in TAPP group.Conclusions The TEP and TAPP techniques of laparoscopic repair of inguinal hernia are comparable regarding most of the parameters,and they are both safe and effective.Surgeons can choose one of the techniques according to their skills and patients' characteristics.
Comparison of Open and Laparoscopic Techniques for Tension-free Hernia Repair for Elderly Patients with Inguinal Hernia

Abstract:Objective Tto compare the efficacy of open surgery with laparoscopy for tension-free hernia repair in elderly patients suffering from inguinal hernia. Methods Between January 2006 and January 2010,136 elderly patients diagnosed with inguinal hernia underwent open tension-free mesh-plug and patch(open group,n=72) or total extraperitoneal hernioplasty(TEP group,n=64).We collected and compared the clinical data between the two groups.ResultsCompared to TEP group,the open group showed significantly shorter operation time,lower hospital cost,higher pain score in 24 hours and 1 week,and longer hospital stay [(65.0±18.1) min vs.(78.1±12.6) min,t=4.742,P=0.000;(4960.2±1033.3) RMB vs.(6998.0±1632.2) RMB,t=8.796,P=0.000;Z=-7.144,P=0.000 and Z=-6.408,P=0.000;(4.9 ±2.4)d vs.(3.6±1.5) d,t=-3.733,P=0.000].No severe complications were found in the patients.The open group were followed up for 8 to 46 months with a mean of 15 months(>24 months in 11 cases);and the TEP group was followed up for 4 to 35 months with a mean of 14 months(>24 months in 9 cases).None of the patients had recurrence during the period. Conclusions Both open surgery and TEP are effective for elderly patients with inguinal hernia.TEP can be the first choice,especially for bilateral inguinal hernia,for it's less pain and quicker recovery.Open surgery for tension-free hernia repair is less expensive,and easier to perform,and thus is especially suitable for patients with cardiopulmonary diseases that cannot tolerate general anesthesia and CO2 pneumoperitoneum.

Abstract:AIM: A few studies comparing laparoscopic and open techniques have reported that open repair with mesh is the optimal operation for unilateral primary hernia. The aim of this study is to compare the outcomes of laparoscopic transabdominal preperitoneal repair (TAPP) versus open mesh plug repair (MP) for bilateral primary inguinal hernia.METHODS: This was a retrospective study of 107 patients with bilateral primary inguinal hernia between January 2008 and December 2016. Of these patients, 49 underwent TAPP and 58 underwent MP. The surgical outcomes and the long-term outcomes using a questionnaire were compared between TAPP and MP.RESULTS: In the TAPP group, the operation time was significantly longer (103 vs 91 minutes; P = .019). The postoperative complication rate was not significantly different between the two groups. One patient (1.0%) in the TAPP group and five patients (4.3%) in the MP group suffered recurrence (P = .30). Postoperative groin pain was not significantly different (14% in the TAPP group vs 31% in the MP group; P = .065), but more patients required analgesics in the MP group (4.1% vs 17%; P = .036). The long-term outcomes, according to a questionnaire, were not significantly different between the two groups. The median follow-up period was 22 (range, 0.4-52) months in the TAPP group and 40 (range, 0.5-108) months in the MP group (P < .001).CONCLUSION: TAPP for bilateral primary inguinal hernia achieved better results than MP relative to postoperative pain and the use of medication for pain relief without increasing the complication and recurrence rates.

Abstract:AbstractThe proximal phalangeal base is the most commonly fractured hand bone in children. Such fractures are rarely reported as irreducible due to flexor tendon entrapment. Here, we describe a patient who sustained a malunited fracture on the right fifth finger proximal phalanx with flexor tendon entrapment after treatment with closed reduction with K-wires fixation.A 13-year-old patient came to the clinic following a bicycle accident 6 weeks ago. He presented with flexion limitation in his small finger on the right hand. During physical examination, the patient felt no pain, and the neurovascular structures were intact. However range of motion (ROM) in his small finger was not normal. Plain radiographs displayed a Salter–Harris type II fracture of the small finger proximal phalanx base and volar angulation with callus formation. During the operation, it was established that the flexor digitorum superficialis (FDS) around the fracture had a severe adhesion, whereas the flexor digitorum profundus (FDP) was entrapped between the volarly displaced metaphyses and the epiphyses and united with the bone. We removed the volarly displaced metaphyses and freed FDP and repaired the A2 pulley. The bone was anatomically fixed with K-wires. In the treatment after the operation, on the 2nd day, the patient was permitted the DIP joint motion by wearing a dynamic splint.At the 12-months follow-up, the patient had regained full ROM with no discomfort and the proximal phalanx growth plate of the small finger closed naturally with normal alignment.When treating a proximal phalangeal base fracture in children, the possibility of flexor tendon entrapment should be considered and should be carefully dealt with in its treatment.

Abstract:Hamartomatous polyp of the cervical esophagus in a 60-year-old man was treated by local resection, and the patient did well. The histopathologic findings showed a predominance of cartilaginous tissue, but the specimen also contained glandular structures and adipose tissue. The diagnosis was choristoma or hamartoma. Review of the literature shows this to be the third case reported.

Abstract:Objective To evaluate the therapeutic effect of the two different open tension-free herniorrhaphies——Tyco mesh plug and preperitoneal anterior approach.Methods In a prospective randomized controlled study,67 cases with 69 hernias(2 cases with bilateral hernias) were allocated randomly to either the Tyco mesh plug group(34 cases) or the preperitoneal anterior approach group(33 cases).The former was received Rutkow hernia repair using Tyco mesh plug,the later was received preperitoneal tension-free hernia repair via open anterior approach with the polypropylene mesh.Results No significant differences were found between the two groups in the operation time,hospital stay,time for returning to work,incision complications or the rate of scrotal hydrocele(P0.05).But the mean cost of treatment in preperitoneal anterior approach group was significantly less than that in Tyco mesh plug group(P0.05).However,in the aspects of recurrence rate,postoperative pain in the groin,and foreign body sensation,the outcome of preperitoneal anterior approach group was also significantly lower than that in the Tyco mesh plug group.Conclusions The preperitoneal anterior approach in repairing of inguinal hernia offers advantages of reducing the operative expense(by using only single sheet mesh),relieving the postoperative groin pain and foreign body sensation.It has characteristics of simple,security and inexpensive,and deserving to be popularized.

Abstract:Objective To compare the efficacy and safety between transabdominal preperitoneal and total extraperitoneal laparoscopic inguinal hernia repair.Methods From August 2005 to August 2008,in our laparoscopic surgery center,the patients who had underwent laparoscopic inguinal hernia repair were randomly divided into two groups according to surgical method,via transabdominal preperitoneal laparoscopic hernia repair(TAPP) group of 55 patients,via totally extraperitoneal laparoscopic hernia repair(TEP) group of 64 cases,make a comparative analysis of clinical data between TAPP group and TEP group.Results Patients were followed up from 6 months to 3 years;TEP operative time was shorter(P=0.000),lower costs(P=0.035),blood loss and postoperative hospital stay has no significant difference;preperitoneal injury,hematoma,serum swelling,intestinal obstruction,urinary retention,intestinal injury,after 6 weeks for pain,infection and recurrence rates were not significantly different.Conclusion TAPP with TEP each have advantages,but with the TAPP comparison,TEP operation time was shorter in lower costs.There is no difference between the selection of indications.

Abstract:Introduction Laparoscopic transabdominal pre-peritoneal (TAPP) repair is a minimally invasive technique that is becoming the procedure of choice among surgeons for inguinal hernia repair and research work is still going on comparing TAPP repair with Lichtenstein open mesh repair. The objective of our study is to compare common postoperative complications in Lichtenstein mesh repair and laparoscopic TAPP repair for unilateral inguinal hernia in our unit. Methods Between August 2016 and August 2018, patients with unilateral inguinal hernia and ASA grade I/II were selected in the surgical outpatient department (OPD) and prospectively randomized into two equal groups. Lichtenstein open mesh repair was done in Group-I and laparoscopic TAPP repair in Group-II. The visual analog scale (VAS) was used for the assessment of the intensity of pain. Results A total of 100 patients with a diagnosis of unilateral inguinal hernia were included in the study. Overall, our study showed that there was less postoperative pain in those patients who underwent TAPP repair as compared to patients with Lichtenstein mesh repair (p= <0.05). There were more postoperative complications in Group-I as compared to Group-II. Conclusion Laparoscopic TAPP repair for inguinal hernia is associated with less postoperative pain and other postoperative complications in addition to a shorter hospital stay as compared to Lichtenstein mesh repair. Thus, this is helping in the early return of patients to daily life activities.
Efficacy of Laparoscopic Totally Extraperitoneal Repair for Inguinal Hernia

Abstract:Objective: To assess the treatment efficacy of laparoscopic totally extraperitoneal repair for inguinal hernia. Methods: Between November 2018 and May 2020, 130 patients with inguinal hernias diagnosed and treated in our hospital were randomly recruited and assigned to receive either tension-free hernia repair (control group) or laparoscopic totally extraperitoneal repair (study group) at the random method. All patients received routine care including external traditional Chinese medicine (TCM) application. Outcome measures included surgical indices, numeric rating scale (NRS) scores, infections, and postoperative complications. Results: Laparoscopic surgery is associated with a shorter operation duration, time-lapse before postoperative off-bed activity, and hospital stay, as well as less intraoperative hemorrhage volume compared to tension-free hernia repair in the control group. Patients in the study group had considerably lower NRS ratings after therapy than those in the control group. (P < 0.05). After treatment, the levels of blood cell count (WBC), C-reactive protein (CRP), and procalcitonin (PCT) in the study group were lower than those in the control group (P < 0.05). In the control group, there were 0 cases of hematoma, 3 cases of subcutaneous effusion, 4 cases of urinary retention, 5 cases of scrotal effusion, and 1 case of bladder injury. In the study group, there were 0 cases of hematoma, 1 case of subcutaneous fluid, 1 case of urinary retention, 0 cases of scrotal fluid, and 0 cases of bladder injury. Laparoscopic surgery resulted in a lower incidence of postoperative complications versus traditional surgery (P <0.05). Conclusion: Laparoscopic totally extraperitoneal repair for inguinal hernia improves the intraoperative indices, mitigates postoperative pain, and reduces the risks of infections and complications, with the advantages of short operation duration, less hemorrhage volume, and shorter hospital stay. It shows great potential for clinical promotion.

Abstract:Background: Minimal access approaches to inguinal hernia repair have added to the ongoing debate over the "best groin hernia repair." The present prospective randomized controlled trial was done to compare the totally extraperitoneal (TEP) and transabdominal preperitoneal (TAPP) techniques of laparoscopic inguinal hernia repair. Methods: The present prospective randomized study was conducted between May 2007 and March, 2009 and included 100 patients suffering from uncomplicated primary groin hernia. Patients were randomized into group I (TEP) and group II (TAPP). Intraoperative variables and postoperative pain scores were recorded in a prestructured form. Results: One hundred patients were included in the study (TEP, 53; TAPP, 47). Both groups were comparable in terms of demographic profile and hernia characteristics. The average operative time was higher in the TAPP group (p = 0.209). The pain scores at 1 h and 24 h after surgery and at 3-month follow-up were significantly higher in the TAPP group (p < 0.05). The average follow-up was 30.5 months. In the TEP group, 37.8% of patients had seroma compared to 18.3% in the TAPP group (p = 0.021). However, there was a higher incidence of scrotal edema in the TAPP group (16 vs. 9, p = 0.009). The wound infection rates were equal (2% vs. 3%). There has been no recurrence in either group during the follow-up period of 44 months. Overall, the patients were more satisfied with TEP rather than TAPP (p < 0.05). Conclusions: In the present study, TEP had a significant advantage over TAPP for significantly reduced postoperative pain up to 3 months, which resulted in a better patient satisfaction score. The other intraoperative complications, postoperative complications, and cost were similar in both groups. In terms of results, both repair techniques seemed equally effective, but TEP had an edge over TAPP.
Comparison of the effect of two different tension free operations in recurrent inguinal hernia repair

Abstract:Objective To evaluate the effect of open total extraperitioneal hernoirrhaphy (open TEP) and Lichtenstein tension-free hernioplasty in recurrent inguinal hernia repair.Methods A total of 48 patients presenting as recurrent inguinal hernia from January 2008 to June 2014,who then exposed to either open TEP (group A,n =25) or Lichtenstein (group B,n =23) were retrospectively studied.Outcome was compared between two groups in duration of the operation,blood loss,length of hospital stay (LOS),complications (urinary retention,incisional infection,incisional paresthesia,incisional pain,hematomas,seroma and recurrent hernia).Results There was no statistically significant difference between the two groups in age,sex,body mass index (BMI),duration of the operation,LOS(P ＞0.05).Statistically significant differences were identified between group A and group B in blood loss [(8.2 ±2.3)mL vs (23.8 ± 5.5)mL,P ＜ 0.05] and total complication (8％ vs 39.1％,P ＜ 0.05).Conclusion Open TEP is superior to Lichtenstein surgery and more safe and effective for the treatment of recurrent inguinal hernia.

Abstract:UNLABELLED Malaria is one of the most important parasitic diseases in humans affecting 103 countries worldwide. AIMS The present study aims to determine the diagnostic utility of cell counter data--hemoglobin, total leukocyte count, platelet count and depolarized laser light (DLL)-based purple-coded events (PCEs) in detection of acute malaria. This is a retrospective study of 523 patient data that came for complete blood count for the first time. RESULTS One hundred thirty-five of the 523 patients showed microscopic evidence of malaria. Platelet count showed the highest sensitivity of 77.77% (105/135). PCEs (> or = 1) showed 43.7% (59/135) sensitivity. CONCLUSIONS It is concluded that a low platelet count (< 150 x 109/L) is a good hematological parameter for presumptive diagnosis of malaria. If we change the cut-off for PCEs from > or = 1 to > or = 2, the sensitivity would be 56.29% (76/135) and the specificity would be 94.58% (367/388), respectively. The sensitivity of DLL was low, particularly with a low parasitic index (PI). The number of PCEs does not correlate with the PI. The cut-off number of PCEs in DLL-based malaria detection should be modified in highly endemic areas.

Abstract:Objective:To compare the effect of open preperitoneal herniorrhaphy and trans-abdominal preperitoneal repair in the treatment of inguinal hernia.Methods:The clinical data of 202 inguinal hernia patients from October 2019 to November 2021 in Beijing Chuiyangliu Hospital were retrospectively analyzed. Patients were divided into open group (n=122) and laparoscopic group (n=80). The general characteristic, operation time, intraoperative blood loss, pain degree, length of hospital stay, hospitalization cost and postoperative complications were compared between the two groups.Results:There was no significant difference in operation time, intraoperative blood loss, length of hospital stay, pain degree in the first month after surgery, and postoperative complications between the two groups (P>0.05). The pain degree on the 1st and 7th day after operation in the laparoscopic group was significantly lower than that in the open group (P<0.001), and the hospitalization cost in the laparoscopic group was significantly higher than that in the open group (P<0.001).Conclusion:Both open preperitoneal herniorrhaphy and trans-abdominal preperitoneal repair can effectively treat inguinal hernia. The latter has less pain and faster recovery.

This study was performed to compare the clinical outcomes of modified tumescent laparoscopic TAPP (MT-TAPP) inguinal hernia repair versus conventional laparoscopic TAPP (CL-TAPP) inguinal hernia repair.MethodsWe retrospectively analyzed the perioperative data of patients with inguinal hernias who underwent either MT-TAPP repair (n = 57) or CL-TAPP repair (n = 54) at the General Surgery Department of Nanjing Yimin Hospital from November 2019 to June 2023.ResultsThe durations of the total operation and the preperitoneal space dissection were shorter in the MT-TAPP than CL-TAPP group. The estimated blood loss volume was lower in the MT-TAPP than CL-TAPP group. The visual analogue scale scores recorded at the 12- and 24-hour postoperative time points showed significantly greater reductions in the MT-TAPP than CL-TAPP group.ConclusionsUsing liquid injection and gauze dissection is both safe and practical. This technique results in a shortened total operation time, less time spent on preperitoneal space dissection, decreased estimated blood loss, and less severe postoperative pain.

[Quality control in magnetic resonance for clinical use].

Abstract:This chapter summarizes requirements, rationale and main steps in the recent evolution of protocols adopted at international level for quality controls in clinical magnetic resonance imaging (MRI) and spectroscopy (MRS). An accurate assessment of instrumental performance in MRI and MRS is essential for enhancing the clinical value of these methodologies, both in terms of anatomical and physio-pathological information. In the field of MRI, a series of Concerted Actions, carried out in the frame of the medical and health research programmes of the European Economic Communities (EEC), led to the definition of standardized methodologies and to the construction of five test objects for assessing the quality of fourteen instrumental parameters, inherent to: signal uniformity and linearity; geometry; slice position and thickness; spatial resolution; signal-to-noise and contrast-to-noise ratios; T1 and T2 precision and accuracy. The first part of this paper devotes particular attention to the protocols adopted by the EEC Concerted Actions and by the National Electrical Manufacturers Association (NEMA, US), for assessing signal uniformity, geometric distortion and signal-to-noise ratio in MRI. A brief description is also given of the rationale and protocols for measuring other MRI parameters. The second part is devoted to describe eleven parameters of instrumental performance, two test objects and the measuring protocols recently proposed by the EEC Concerted Actions for quality control in single-volume localized MRS, following their validation in a series of experimental multi-centre trials.
The economic impact of laparoscopic inguinal hernia repair: results of a double-blinded, prospective, randomized trial

Abstract:For this study, 66 patients with a preoperative diagnosis of unilateral primary inguinal hernia were randomized to undergo laparoscopic totally extra peritoneal (TEP), laparoscopic transabdominal (TAPP), or open inguinal hernia repair with polypropylene mesh (Lichtenstein type). Both the operative team caring for the patient postoperatively and the patient were blinded to the operative approach by placement of a large dressing covering the abdomen, which was not removed until postoperative day 3. The patients recorded their pain level on a visual analog pain scale daily. Medication usage also was recorded. All patients were seen at 7-day intervals until they returned to work. The patients were interviewed during their postoperative visits by an investigator blinded to the operative approach and questioned regarding their ability to return to work and their pain levels. The average number of lost work days in all the groups was 12, and there was no significant difference between the three groups (p = 0.074). The average operating time for the TAPP procedure was 59 min, less than the time required to complete either the TEP or the Lichtenstein approach, which had equivalent operative times (p = 0.027). The material cost was significantly lower for the Lichtenstein repair (1,200 dollars less) than for either of the laparoscopic approaches, a saving primarily related to consumable operating room supplies. The TEP repair costs were minimally higher than those for the TAPP repair (125 dollars more). No significant differences were noted in the postoperative pain scales, and the use of postoperative oral analgesics was equivalent. The higher operative costs noted for the laparoscopic hernia repairs were not offset by a shortened convalescence. Postoperative pain appears to be equivalent regardless of the operative approach chosen and is easily managed with oral analgesics.
