Abstract:In their analysis of the UK Biobank cohort (n = 462,619 patients, with 4,998 developing all-cause dementia), Dr. Shen et al. illustrate the complex association between social isolation and loneliness and incident dementia. In addition to characterizing this relationship between clinical factors, the investigators support their findings with radiographic evidence. Socially isolated individuals not only had a higher risk of later dementia but also had smaller gray matter volumes in several frontal and temporal regions. Dr. Kawada contests the notion that social isolation may be causally related to smaller gray matter volumes and suggests other factors may be operative including male sex, metabolic changes, and genetics. In response, Shen et al. emphasize that no causation was implied by their study. The authors also performed a subgroup analysis of their data stratified by sex, with social isolation being independently associated with dementia in both women (HR 1.24, 95% CI 1.09–1.40) and men (HR 1.26, 95% CI 1.12–1.42). In a separate comment, Dr. Holwerda cites their published experience indicating loneliness (as opposed to social isolation) may be more strongly tied to incident dementia.
