Abstract:The decline of cognitive function is the consequence of population aging, which might increase the risks of dementia and physical disability. It is important to identify the risk factors and mechanisms of cognitive function for older adults. Social isolation is a psychological risk caused by the lack of social networks. Numerous studies had proposed the evidence that social isolation was negatively associated with cognitive function among older adults in the western social context. However, there is a lack of evidence of the above association in China, whose patterns of social connections are quite different. This study hypothesizes that social isolation and its dimensions of family and friend isolation are all negatively associated with cognitive function among Chinese older adults.In the previous literature, few studies explored the mechanisms between social isolation and cognitive function. The psychological mechanism of cognitive function proposes the point that negative emotions trigger cognitive impairment. Loneliness and depression are typical negative emotions for older adults, and plentiful evidence verifies their negative effects on cognitive function. Furthermore, social isolation stands for fewer social networks, which is the predictor of loneliness and depression. Therefore, this study hypothesizes that loneliness and depression play mediating roles between social isolation and cognitive function. Moreover, previous studies also supported the significant association between loneliness and depression, and this study hypothesizes that loneliness and depression play a chain mediating effect between social isolation and cognitive function. In addition, cognitive reserve is a theoretical concept that could create a buffer against the cognitive decline caused by risk factors, and empirical evidence suggests that reserve could be constructed through a combination of experiences across the lifespan, such as education, occupation, and cognitively stimulating activities. This study hypothesizes that the cognitive reserve of older people moderates the effects on cognitive function of social isolation, loneliness, and depression.The China Longitudinal Aging Social Survey（CLASS） in 2014 wave was employed in this study. After excluding those samples with missing data of key variables, a total number of 5,162 older samples were selected. SPSS 20.0 and PROCESS were used to analyze data. At first, this study examined the common method deviation analysis. Secondly, the descriptive results and the correlation analysis between key variables were reported.Thirdly, this study explored mediating effects. Finally, a moderated chain mediation model was tested.The results were as follows: at first, social isolation was significantly associated with cognitive function for older people, and the effect of friend isolation was more pronounced. Second, depression played a mediating role in the association between social isolation and cognitive function, while the mediating role of loneliness was not statistically significant. In addition, loneliness and depression played a chain mediating role in the relationship between social isolation and cognitive function. Next, cognitive reserve significantly moderated the effects on cognitive function of social isolation,loneliness, and depression, and the mediating effects among older adults with the lower cognitive reserve were more pronounced than older adults with higher cognitive reserve.A series of practical contributions are proposed in this study. First, it is urgent and necessary to take measures to cope with the challenges of social isolation for older people. Both family and community should encourage older adults to expand their social networks, especially enhancing the friend connections. Secondly, the reduction of depression symptoms could protect cognitive function, especially for the older adults who have insufficient social network resources, who are easily trapped in social isolation. Furthermore, it is important to consider the differences in cognitive reserve among older adults when adopting cognitive function intervention strategies, and more attention should be paid to older adults with lower cognitive reserve. In addition, the intervention on loneliness for older people with lower cognitive reserve would also benefit the protection of their cognitive function.

Abstract:Loneliness significantly contributes to cognitive impairment and dementia in older adults. Loneliness is a distressing feeling resulting from a perceived lack of social connection (i.e., a discrepancy between desired and actual social relationships), while social isolation is a related term that can be defined by number and type of social relationships. Importantly, loneliness is distinct from social isolation in that it is associated with a distressing self-perception. The primary focus of this narrative review is the impact of chronic loneliness on cognitive impairment and dementia among older adults. Loneliness has a significant association with many factors that are related to worse cognition, and therefore we include discussion on health, mental health, as well as the physiological effects of loneliness, neuropathology, and potential treatments. Loneliness has been shown to be related to development of dementia with a hazard ratio (HR) risk comparable to having a single APOE4 gene. The relationship of dementia to loneliness appears to be at least partially independent of other known dementia risk factors that are possibly associated with loneliness, such as depression, educational status, social isolation, and physical activity. Episodic memory is not consistently impacted by loneliness, which would be more typically impaired if the mild cognitive impairment (MCI) or dementia was due to Alzheimer's disease (AD) pathology. In addition, the several longitudinal studies that included neuropathology showed no evidence for a relationship between loneliness and AD neuropathology. Loneliness may decrease resilience, or produce greater cognitive change associated with the same level of AD neuropathology. Intervention strategies to decrease loneliness in older adults have been developed but need to consider key treatment targets beyond social isolation. Loneliness needs to be assessed in all studies of cognitive decline in elders, since it significantly contributes to the variance of cognitive function. It will be useful to better define the underlying mechanism of loneliness effects on cognition to determine if it is similar to other psychological factors related to excessive stress reactivity, such as neuroticism or even depression, which are also associated with cognitive decline. It is important from a health perspective to develop better strategies to decrease loneliness in older adults.

Abstract:Alzheimer's Disease (AD) is the leading cause of dementia, observed at a higher incidence in women compared with men. Treatments aimed at improving pathology in AD remain ineffective to stop disease progression. This makes the detection of the early intervention strategies to reduce future disease risk extremely important. Isolation and loneliness have been identified among the major risk factors for AD. The increasing prevalence of both loneliness and AD emphasizes the urgent need to understand this association to inform treatment. Here we present a comprehensive review of both clinical and preclinical studies that investigated loneliness and social isolation as risk factors for AD. We discuss that understanding the mechanisms of how loneliness exacerbates cognitive impairment and AD with a focus on sex differences will shed the light for the underlying mechanisms regarding loneliness as a risk factor for AD and to develop effective prevention or treatment strategies.

Abstract:Objective: Social isolation and loneliness are associated with poor health (immunity, inflammation, etc.) in ageing. The purpose of this scoping review was to investigate the link between social isolation, loneliness (as distinct constructs, in contrast to previous published work) and cognition in cognitively healthy older adults. Method: We followed PRISMA-ScR guidelines. Our search, conducted between January 2017 and April 2021, yielded 2,673 articles, of which, twelve longitudinal studies were finally identified as meeting the inclusion criteria. Multiple cognitive functions (short-term and episodic memory, attention, and global cognitive functioning) were measured. Results: The results showed that both social isolation and loneliness were associated with poor cognition in ageing, with depression as a possible mediator between loneliness and poor cognition. Some studies also suggested that the link between social isolation, loneliness and cognitive decline may be bidirectional. Conclusion: We conclude that both social isolation and loneliness may have a different impact on cognition. While depression may be an important mediator between loneliness and cognitive decline, the lack of cognitive stimulation may be a greater mediator between social isolation and cognitive health.

Abstract:Abstract Background Social isolation and loneliness have detrimental effects on the health of older adults. Being socially isolated and lonely are likely both predictors and consequences of cognitive impairment with aging. However, isolation and loneliness may be differentially associated with cognitive impairment and may operate through both shared and unique pathways. The present study aimed to examine the association of social isolation and loneliness with concurrent cognitive impairment, and to explore potential mechanisms of the association. Methods Participants (n =1,982; Mage = 77.65 years, 1,210 (61.1%) female, 1,877 (94.7%) white, 815 (41.12% > high school education) were from a population‐based study of mild cognitive impairment and dementia. Three composite measures derived from factor analysis, representing social network structure, social connectedness, and loneliness were examined in relation to clinical dementia rating (CDR normal = 0 vs. CDR impaired >= 0.5) at baseline, adjusted for demographic characteristics. The indirect effects of the composite measures were also estimated conditioned on general health and function, lifestyle factors, vascular health, depressive symptoms, and sleep. Results Of the participants, 569 (28.71%) were cognitively impaired at baseline. Social connectedness (OR 1.48, 95% CI=1.33‐1.64) and loneliness (OR 1.38, 95% CI=1.24‐1.54), but not social network structure, were significantly associated with greater likelihood of being cognitively impaired after adjustment for demographics. These effects remained significant after separate adjustments for general health and function, lifestyle factors, vascular health, depressive symptoms, sleep, and other composites. The effect of social connectedness was appreciably attenuated by general health status and function (OR 1.37, 95% CI=1.22‐1.54), while loneliness was appreciably attenuated by depressive symptoms (OR 1.26, 95% CI=1.12‐1.42). Conclusions Older adults with lower levels of social connectedness and higher perception of loneliness are more likely to be cognitively impaired, which were partially explained by health and function and depressive symptoms, respectively. Addressing these factors may reduce risk for cognitive impairment, or conversely, reduce risk for disconnectedness and loneliness among those with cognitive impairment. These results have implications for the impact that limited social contact and feelings of loneliness during the COVID‐19 pandemic may have on older adults with or at risk for cognitive impairment.

Abstract:Abstract Social connectivity is a crucial human experience. The objective experience of being disconnected (i.e., social isolation) and the subjective feeling of being disconnected (i.e., loneliness) are related but distinct experiences that have unique impacts on physical, mental, and cognitive health. The COVID-19 pandemic forced much of the global population into a prolonged state of isolation, and many older adults found themselves abruptly cut-off from important sources of social interaction. The lasting impacts of the pandemic are still unknown. This symposium brings together several projects addressing key questions about longitudinal processes of loneliness and social isolation among middle aged and older adults, both prior to and during the COVID-19 pandemic. Jackson will present results from a coordinated data analysis (CDA) showing the extent to which loneliness, social isolation, and social asymmetry are related to MCI, dementia, mortality, and life expectancy. Rule will discuss the impact of stroke on trajectories of loneliness in a CDA. Zhang and Fingerman will present findings that show how loneliness and social isolation vary among individuals with functional limitations. Zavala will present results from a CDA that explored daily patterns of loneliness during the early months of the COVID-19 pandemic. Lastly, Beck will discuss a CDA that examined trajectories of social asymmetry prior to and during the COVID-19 pandemic. Antonucci will synthesize and discuss each project within the context of broader implications for empowering older adults. In sum, this symposium presents novel evidence showing that loneliness and social isolation in older adulthood are impactful predictors of health.
Correlations between Dementia and Loneliness

Abstract:This review describes associations between dementia and loneliness on the neurobiological and epidemiological levels according to the recent body of literature. The aim of this study was to highlight major lines of research in this field. Sociocognitive skills and social interactions present complex interdependencies with dementia which may be explained by two theories. According to the first one, not sufficiently engaging in social or cognitive activities results in brain atrophy. The second one claims that brain neurogenesis and synaptic density are being increased by social connections. The relationship between loneliness and dementia could be mediated by sensory loss, including hearing and visual impairment, as well as depression and psychotic symptoms. Loneliness itself might cause a depletion in sensory and cognitive stimulation which results in a decrease in neural reserve. Certain changes in the structures of the brain caused by loneliness were found in imaging examination. Loneliness appears to be a crucial risk factor for dementia in recent times due to the modern lifestyle and consequences of the outbreak of COVID-19. Additional studies are required to understand more completely the key tenets of this topic and therefore to improve the prevention and treatment of dementia.

Abstract:Abstract Background The measures imposed during the COVID-19 pandemic, especially social distancing, had important effects on feelings of loneliness. The objective of this work is to assess the perception of loneliness in older adults living in nursing homes during the pandemic, how it has changed during the pandemic and its explanatory factors. Methods The data come from the cross-sectional project “Protective environments of the elderly in health crises”, carried out in the Community of Madrid (Spain) in 2021. The variables used were the frequency of feelings of loneliness and its change during the pandemic, as well as socio-demographic, health, emotional and contextual characteristics. Descriptive statistics and logistic regression analysis were calculated. Results The sample consisted of 447 people; mean age was 83.8; 63.1% were women; half of the sample were widowed; 40% had an educative level lower than primary. Almost 3 out of 10 residents stated that they often or always/almost always felt alone. In the regression model, loneliness was negatively associated with age, number of medications, emotional balance, coping ability, self-perception of aging, and engaging in rewarding activities; while having a low assessment of mobility in the residential environment was positively associated. In addition, 28% of the participants who declared loneliness, also felt lonelier than before the pandemic. The variables associated with change in feelings of loneliness were age, self-perception of aging and a low assessment of the residential environment. Conclusions The restrictions on mobility and social relationships during the COVID-19 epidemic have affected older people who were living in residences, with an increase in the feeling of loneliness related to demographic, emotional and contextual variables. Interventions on the design of residential spaces can mitigate the effects of isolation and loneliness related to health crises such as COVID-19 pandemic.

Abstract:Introduction: A high burden and many negative outcomes for older people were associated with the COVID-19 pandemic. Social isolation and loneliness are prevalent health problems impacting well-being and quality of life and may have increased due to pandemic-related restrictions. Methods: This study investigate the influence of the COVID-19 pandemic on loneliness in people visiting a mem40ory clinic between March 2020 and September 2022. We conducted a prospective, single-center, questionnaire-based observational follow-up study to assess potential predictors of newly occurring, pandemic-related loneliness. Next to a newly developed COVID-19 questionnaire, a comprehensive neuropsychological test battery, the Neuropsychiatric Inventory and the Geriatric Depression Scale were used. Results: In total 426 people (mean age: 76.48 years, 12.9% cognitively intact, 33.1% diagnosed with Mild Cognitive Impairment, 49.8% diagnosed with dementia, and 4.2% diagnosed with depression) completed the COVID-19 questionnaire at baseline and 166 at follow-up. Newly occurring loneliness was indicated by 22.3% of baseline participants and by 24.1% of follow-up participants. Results of logistic regression analysis showed that living alone (OR 5.452) and having less contact with friends (OR 2.771) were most predictive of the occurrence of loneliness. The use of digital communication media as an alternative strategy for social interaction was lowest in dementia patients (6-13%). Discussion: In conclusion, personal contacts and a close friendship network appear to be more decisive to prevent loneliness in older people than does the use of digital communication media. However, promoting an intensified use of digital communication media may be useful to counteract loneliness, especially in dementia patients.

Abstract:Loneliness, or the <i>subjective feeling</i> of social isolation, is an important social determinant of health. Loneliness is associated with poor physical health, including higher rates of cardiovascular disease and dementia, faster cognitive decline, and increased risk of mortality, as well as disruptions in mental health, including higher levels of depression, anxiety, and negative affect. Theoretical accounts suggest loneliness is a complex cognitive and emotional state characterized by increased levels of inflammation and affective disruptions. This review examines affective neuroscience research on social isolation in animals and loneliness in humans to better understand the relationship between <i>perceptions</i> of social isolation and the brain. Loneliness associated increases in inflammation and neural changes consistent with increased sensitivity to social threat and disrupted emotion regulation suggest interventions targeting maladaptive social cognitions may be especially effective. Work in animal models suggests the neural changes associated with social isolation may be reversible. Therefore, ameliorating loneliness may be an actionable social determinant of health target. However, more research is needed to understand how loneliness impacts healthy aging, explore the role of inflammation as a potential mechanism in humans, and determine the best time to deliver interventions to improve physical health, mental health, and well-being across a diverse array of populations.
Exploring the Differential Impacts of Social Isolation, Loneliness, and Their Combination on the Memory of Aging Population: A 6-Year Longitudinal Study by the CLSA

Abstract:Memory plays a crucial role in cognitive health. Social isolation (SI) and loneliness (LON) are recognized risk factors for global cognition, although their combined effects on memory have been understudied in the literature. This study used three waves of data over six years from the Canadian Longitudinal Study on Aging to examine whether SI and LON are individually and jointly associated with memory in community-dwelling middle-aged and older adults (n = 14,208). LON was assessed with the question: "In the last week, how often did you feel lonely?". SI was measured using an index based on marital/cohabiting status, retirement status, social activity participation, and social network contacts. Memory was evaluated with combined z-scores from two administrations of the Rey Auditory Verbal Learning Test (immediate-recall, delayed-recall). We conducted our analyses using all available data across the three timepoints and retained participants with missing covariate data. Linear mixed models were used to regress combined memory scores onto SI and LON, adjusting for sociodemographic, health, functional ability, and lifestyle variables. Experiencing both SI and LON had the greatest inverse effect on memory (least-squares mean: -0.80 [95% confidence-interval: -1.22, -0.39]), followed by LON alone (-0.73 [-1.13, -0.34]), then SI alone (-0.69 [-1.09, -0.29]), and lastly by being neither lonely nor isolated (-0.65 [-1.05, -0.25]). Sensitivity analyses confirmed this hierarchy of effects. Policies developed to enhance memory in middle-aged and older adults might achieve greater benefits when targeting the alleviation of both SI and LON rather than one or the other individually.

Abstract:INTRODUCTION: Current findings of the relative influence of social isolation and loneliness on dementia are contradictory, and the potential neurobiological mechanisms are unclear. METHODS: We utilized the UK Biobank to investigate the relationships of social isolation and loneliness with dementia (n = 462,619). Neuroanatomical correlates were identified in a subset of participants (n = 32,263). The transcriptomic signatures of related brain changes were characterized by gene enrichment analysis. RESULTS: After full adjustment, social isolation but not loneliness was associated with dementia (hazard ratio: 1.28, 95% confidence interval: 1.17-1.39). Isolated individuals had reduced gray matter volumes in temporal, frontal, occipital and subcortical regions (e.g., hippocampus and amygdala). Relevant brain changes were spatially correlated with genes involved in mitochondrial dysfunction and oxidative phosphorylation, and down-regulated Alzheimer's disease-related genes. DISCUSSION: Social isolation is an independent risk factor for dementia, which could be partly explained by related structural changes coupling with altered molecular functions.
Cognition, loneliness and social isolation: A cross-national, longitudinal exploration

Abstract:Abstract Background One of the challenges to ageing individuals and health care systems is the decline in cognitive function with increasing age. The present study investigates in which way loneliness and social isolation is associated with cognitive decline in an aged population in Europe. Furthermore, it investigates how these relationships vary across different cultural and social settings. Methods The study employs panel data from the Survey of Health, Ageing and Retirement in Europe (SHARE). Data from respondents aged 50+ from 16 European countries were collected from 2011-2017 at up to four points in time. Loneliness was operationalized as the perception of feeling lonely, while social isolation was operationalized by a summary index of social network size and social interaction. The cognitive function measures included tests of verbal fluency, as well as immediate and delayed recall. A linear panel fixed-effects modelling approach was used, while controlling for potential time-variant confounders from the areas of demographics, mental and physical health and survey methodological characteristics. Results Preliminary results show that loneliness and social isolation are associated with cognitive decline, while the extent varied by the measure of cognitive functioning measured. Loneliness, social isolation and cognition descriptively all show substantial cross-national variation. Conclusions The study illustrates the impact of loneliness and social isolation on cognitive functioning across a number of European countries. Cognitive functioning is an important determinant to successful ageing, preventing its decline promotes self-sufficient, independent living in the elderly and good quality end-of-life. This can support the development of implementable and accessible prevention for at risk individuals. Key messages Investigating the impact of social isolation and loneliness on the cognitive functioning can help tailor easily accessible prevention measures of cognitive decline in the elderly. The study allows for the assessment of cross-cultural differences and need for targeted measures across a number of European countries.

Abstract:Background and Objectives To investigate the independent associations of social isolation and loneliness with incident dementia and to explore the potential neurobiological mechanisms. Methods We utilized the UK Biobank cohort to establish Cox proportional hazard models with social isolation and loneliness as separate exposures. Demographic (sex, age, and ethnicity), socioeconomic (education level, household income, and Townsend deprivation index), biological (body mass index, APOE genotype, diabetes, cancer, cardiovascular disease, and other), cognitive (speed of processing and visual memory), behavioral (current smoker, alcohol intake, and physical activity), and psychological (social isolation or loneliness, depressive symptoms, and neuroticism) factors measured at baseline were adjusted. Then, voxel-wise brainwide association analyses were used to identify gray matter volumes (GMVs) associated with social isolation and with loneliness. Partial least squares regression was performed to test the spatial correlation of GMV differences and gene expression using the Allen Human Brain Atlas. Results We included 462,619 participants (mean age at baseline 57.0 years [SD 8.1]). With a mean follow-up of 11.7 years (SD 1.7), 4,998 developed all-cause dementia. Social isolation was associated with a 1.26-fold increased risk of dementia (95% CI, 1.15–1.37) independently of various risk factors including loneliness and depression (i.e., full adjustment). However, the fully adjusted hazard ratio for dementia related to loneliness was 1.04 (95% CI, 0.94–1.16) and 75% of this relationship was attributable to depressive symptoms. Structural MRI data were obtained from 32,263 participants (mean age 63.5 years [SD 7.5]). Socially isolated individuals had lower GMVs in temporal, frontal, and other (e.g., hippocampal) regions. Mediation analysis showed that the identified GMVs partly mediated the association between social isolation at baseline and cognitive function at follow-up. Social isolation–related lower GMVs were related to underexpression of genes that are downregulated in Alzheimer disease and to genes that are involved in mitochondrial dysfunction and oxidative phosphorylation. Discussion Social isolation is a risk factor for dementia that is independent of loneliness and many other covariates. Social isolation–related brain structural differences coupled with different molecular functions also support the associations of social isolation with cognition and dementia. Social isolation may thus be an early indicator of an increased risk of dementia.

Abstract:Background: Social isolation has been suggested to increase the risk to develop cognitive decline. However, our knowledge on causality and neurobiological underpinnings is still limited. Methods: In this preregistered analysis, we tested the impact of social isolation on central features of brain and cognitive ageing using a longitudinal population-based magnetic resonance imaging (MRI) study. We assayed 1992 cognitively healthy participants (50-82years old, 921women) at baseline and 1409 participants after~6y follow-up. Results: We found baseline social isolation and change in social isolation to be associated with smaller volumes of the hippocampus and clusters of reduced cortical thickness. Furthermore, poorer cognitive functions (memory, processing speed, executive functions) were linked to greater social isolation, too. Conclusions: Combining advanced neuroimaging outcomes with prevalent lifestyle characteristics from a well-characterized population of middle- to older aged adults, we provide evidence that social isolation contributes to human brain atrophy and cognitive decline. Within-subject effects of social isolation were similar to between-subject effects, indicating an opportunity to reduce dementia risk by promoting social networks. Abstract:<span>Loneliness and social isolation are described similarly yet are distinct constructs. Numerous studies have examined each construct separately; however, less effort has been dedicated to exploring the impacts in combination. This study sought to describe the cumulative effects on late-life health outcomes.Survey data collected in 2018–2019 of a randomly sampled population of US older adults, age 65+, were utilized (<i>N</i> = 6,994). Survey measures included loneliness and social isolation using the UCLA-3 Loneliness Scale and Social Network Index. Participants were grouped into four categories based on overlap. Groups were lonely only, socially isolated only, both lonely and socially isolated, or neither. Bivariate and adjusted associations were examined.Among participants (mean age = 76.5 years), 9.8% (<i>n</i> = 684) were considered lonely only, 20.6% (<i>n</i> = 1,439) socially isolated only, 9.1% (<i>n</i> = 639) both lonely and socially isolated, and 60.5% (<i>n</i> = 4,232) neither. Those considered both lonely and socially isolated were more likely to be older, female, less healthy, depressed, with lower quality of life and greater medical costs in bivariate analyses. In adjusted results, participants who were both lonely and socially isolated had significantly higher rates of ER visits and marginally higher medical costs.Results demonstrate cumulative effects of these constructs among older adults. Findings not only fill a gap in research exploring the impacts of loneliness and social isolation later in life, but also confirm the need for approaches targeting older adults who are both lonely and socially isolated. As the COVID-19 pandemic continues, this priority will continue to be urgent for older adults.</span>

Abstract:Understanding others is a key component of successful social interactions, and declines in social abilities during later life can lead to social isolation and loneliness. We investigated the relationship between different sub-components of social cognition and loneliness in a large sample of older adults. We tested perspective-taking and mentalizing skills, alongside self-reported loneliness and social functioning. Results revealed a significant effect of loneliness on older adults’ ability to resist egocentric interference when taking others’ perspectives. However, this effect was eliminated when age was added to models, which suggests that egocentric tendencies increase with age, and people experience increasing levels of loneliness and feelings of social isolation with increasing age. Mentalizing and interference from others’ perspectives were not influenced by loneliness or age.

Abstract:Journal of Applied Gerontology, Ahead of Print. In this study, we examined associations of social isolation and loneliness with cognitive impairment among older adults from a Rust Belt region in Southwest Pennsylvania. We used data from the population-based Monongahela-Youghiogheny Healthy Aging Team (MYHAT) study. We found that (a) 11 items combined into two reliable composites of social isolation and loneliness; (b) unique to this study, providing unpaid help to others was an indicator of reduced social isolation; (c) social isolation and loneliness were positively associated with cognitive impairment; and (d) these associations were appreciably attenuated by general health and physical functional status and depressive symptoms, respectively. We concluded that social isolation and loneliness are differentially associated with older adults' cognitive health, and that their effects might operate through separate pathways. Approaches to address social isolation and loneliness should consider the community context and its implications for older adults' cognitive health.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Objectives</h3><p>To test whether loneliness is associated with risk of cognitive impairment up to 11 years later in a European sample of middle‐aged and older adults. The study examines whether this association is independent of measures of social isolation, depression and other risk factors for cognitive impairment and dementia.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>Participants (N = 14 114) from the Survey of Health, Ageing and Retirement in Europe (SHARE) answered a single item on loneliness at baseline and were assessed for cognitive impairment every two‐to‐three years for 11 years. Participants who scored at least 1.5 standard deviations below the age‐graded mean on both a memory recall task and verbal fluency task were classified as impaired. A 3‐item measure of loneliness was available for a sample of respondents followed up to 4 years.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>Feeling lonely was associated with increased risk of incident cognitive impairment (HR = 1.31, 95%CI = 1.19‐1.44), after accounting for age, sex, education, and SHARE country strata. The association was robust but reduced in magnitude when controlling for clinical and behavioral risk factors, health‐related activity limitations, social isolation, social disengagement and depressive symptoms. The association was not moderated by socio‐demographic factors and was also apparent when using the 3‐item loneliness scale instead of the single‐item measure.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions</h3><p>These findings expand the extant literature on loneliness and risk of cognitive impairment in older adulthood.
