Abstract:Abstract Background The measures imposed during the COVID-19 pandemic, especially social distancing, had important effects on feelings of loneliness. The objective of this work is to assess the perception of loneliness in older adults living in nursing homes during the pandemic, how it has changed during the pandemic and its explanatory factors. Methods The data come from the cross-sectional project “Protective environments of the elderly in health crises”, carried out in the Community of Madrid (Spain) in 2021. The variables used were the frequency of feelings of loneliness and its change during the pandemic, as well as socio-demographic, health, emotional and contextual characteristics. Descriptive statistics and logistic regression analysis were calculated. Results The sample consisted of 447 people; mean age was 83.8; 63.1% were women; half of the sample were widowed; 40% had an educative level lower than primary. Almost 3 out of 10 residents stated that they often or always/almost always felt alone. In the regression model, loneliness was negatively associated with age, number of medications, emotional balance, coping ability, self-perception of aging, and engaging in rewarding activities; while having a low assessment of mobility in the residential environment was positively associated. In addition, 28% of the participants who declared loneliness, also felt lonelier than before the pandemic. The variables associated with change in feelings of loneliness were age, self-perception of aging and a low assessment of the residential environment. Conclusions The restrictions on mobility and social relationships during the COVID-19 epidemic have affected older people who were living in residences, with an increase in the feeling of loneliness related to demographic, emotional and contextual variables. Interventions on the design of residential spaces can mitigate the effects of isolation and loneliness related to health crises such as COVID-19 pandemic.

Abstract:Social isolation is an important public health issue that has gained recognition during the COVID-19 pandemic because of the risks posed to older adults based on physical distancing. The primary purposes of this article are to provide an overview of the complex interconnectedness between social isolation, loneliness, and depression while introducing the COVID-19 Connectivity Paradox, a new concept used to describe the conflicting risk/harm continuum resulting from recommended physical distancing. In this context, examples will be provided for practical and feasible community-based models to improve social connectivity during COVID-19 by adjusting the processes and modalities used to deliver programs and services to older adults through the aging social services network. The COVID-19 pandemic has highlighted the need for clinical and community-based organizations to unite and form inter-sectorial partnerships to maintain the provision of services and programs for engaging and supporting older adults during this difficult time of physical distancing and shelter-in-place and stay-at-home orders. The aging social services network provides a vital infrastructure for reaching older underserved and/or marginalized persons across the U.S. to reduce social isolation. Capitalizing on existing practices in the field, older adults can achieve distanced connectivity to mitigate social isolation risk while remaining at safe physical distances from others.

Abstract:The COVID-19 pandemic is a serious global burden. Epidemiological data suggest that the severity of COVID-19, in particular its case fatality rate, rises strongly with age. It is possible that neither a vaccine nor an effective treatment will be available for >1 year. Thus, it may be necessary for older adults to protect themselves by avoiding direct social contact and practicing social distancing for a rather long period of time. This may result in loneliness and social isolation because, for example, grandchildren cannot visit their grandparents. In turn, both loneliness and social isolation can have serious deleterious consequences (e.g., in terms of morbidity and mortality). Thus, the question arises: are there ways to mitigate loneliness and social isolation? One way to stay in contact is to use online social media such as Facebook or using video calling software such as Skype. However, there are very few studies examining whether the use of online social media or video chats are associated with loneliness and social isolation in older adults. We sum up some preliminary findings and make a call for further research on the link between online social media use/video chat and loneliness, as well as social isolation, in older adults.

Abstract:Introduction: A high burden and many negative outcomes for older people were associated with the COVID-19 pandemic. Social isolation and loneliness are prevalent health problems impacting well-being and quality of life and may have increased due to pandemic-related restrictions. Methods: This study investigate the influence of the COVID-19 pandemic on loneliness in people visiting a mem40ory clinic between March 2020 and September 2022. We conducted a prospective, single-center, questionnaire-based observational follow-up study to assess potential predictors of newly occurring, pandemic-related loneliness. Next to a newly developed COVID-19 questionnaire, a comprehensive neuropsychological test battery, the Neuropsychiatric Inventory and the Geriatric Depression Scale were used. Results: In total 426 people (mean age: 76.48 years, 12.9% cognitively intact, 33.1% diagnosed with Mild Cognitive Impairment, 49.8% diagnosed with dementia, and 4.2% diagnosed with depression) completed the COVID-19 questionnaire at baseline and 166 at follow-up. Newly occurring loneliness was indicated by 22.3% of baseline participants and by 24.1% of follow-up participants. Results of logistic regression analysis showed that living alone (OR 5.452) and having less contact with friends (OR 2.771) were most predictive of the occurrence of loneliness. The use of digital communication media as an alternative strategy for social interaction was lowest in dementia patients (6-13%). Discussion: In conclusion, personal contacts and a close friendship network appear to be more decisive to prevent loneliness in older people than does the use of digital communication media. However, promoting an intensified use of digital communication media may be useful to counteract loneliness, especially in dementia patients.

Abstract:Abstract Loneliness has become a big issue in the time of the COVID pandemic. The attention to the topic also has to do with the increase in people living alone in Europe, although this also has to do with prosperity. Living alone does not yet mean being lonely. In the scientific discussion and especially in the measures, a differentiation must be made between loneliness, solitude and social disintegration. Poor social integration is easily measurable and has a lot to do with the physical health (mobility, vision and hearing) of the people concerned. However, the extent to which participation in social and cultural opportunities is possible, for example through the expansion of public transport, also plays an important role. Loneliness, on the other hand, is by definition subjective and strongly linked to mental health. It describes the subjective suffering of missing or unsatisfactory social relationships, lack of integration and security. Loneliness is often found in two peaks, among the young and the old. Political and public health campaigns often focus on social integration measures. However, measures to combat loneliness mean first and foremost recognising mental illness in old age, especially depression. Abstract:The study focused on examining the relationship between well-being and various psychological factors such as loneliness, anxiety, depression, and stress, whilst also considering changes in lifestyle. A total of 108 elderly participants, with an average age of 70.38 years, were enrolled in this quantitative cross-sectional study. The research employed a battery of assessment tools including a Sociodemographic Data Questionnaire, Mini-Mental State Examination, Positive Mental Health Scale, Stress Perception Scale, Geriatric Anxiety Inventory, Geriatric Depression Scale (reduced version), Loneliness Scale, and International Physical Activity Questionnaire. Descriptive analysis was conducted in order to understand the distribution of scores across these variables, followed by the categorization of participants based on the reported alterations in eating and physical activity behaviors. Correlations between variables were assessed using Spearman correlation and an EBIC-LASSO network analysis. The findings indicated a potential detriment to the well-being of elderly individuals practicing social distancing, evidenced by heightened symptoms of loneliness, depression, anxiety, and stress, alongside the reported changes in dietary patterns and physical activity. The study underscores the importance of understanding the pandemic's impact on the well-being of older adults and advocates for longitudinal investigations to delineate the evolving effects of social distancing measures across different phases of the pandemic.

Social Isolation and Loneliness Among San Francisco Bay Area Older Adults During the COVID-19 Shelter-in-Place Orders

Abstract:Background/objectives: Physical distancing during the COVID-19 pandemic may have unintended, detrimental effects on social isolation and loneliness among older adults. Our objectives were to investigate (1) experiences of social isolation and loneliness during shelter-in-place orders, and (2) unmet health needs related to changes in social interactions. Design: Mixed-methods longitudinal phone-based survey administered every 2 weeks. Setting: Two community sites and an academic geriatrics outpatient clinical practice. Participants: A total of 151 community-dwelling older adults. Measurements: We measured social isolation using a six-item modified Duke Social Support Index, social interaction subscale, that included assessments of video-based and Internet-based socializing. Measures of loneliness included self-reported worsened loneliness due to the COVID-19 pandemic and loneliness severity based on the three-item University of California, Los Angeles (UCLA) Loneliness Scale. Participants were invited to share open-ended comments about their social experiences. Results: Participants were on average aged 75 years (standard deviation = 10), 50% had hearing or vision impairment, 64% lived alone, and 26% had difficulty bathing. Participants reported social isolation in 40% of interviews, 76% reported minimal video-based socializing, and 42% minimal Internet-based socializing. Socially isolated participants reported difficulty finding help with functional needs including bathing (20% vs 55%; P = .04). More than half (54%) of the participants reported worsened loneliness due to COVID-19 that was associated with worsened depression (62% vs 9%; P < .001) and anxiety (57% vs 9%; P < .001). Rates of loneliness improved on average by time since shelter-in-place orders (4-6 weeks: 46% vs 13-15 weeks: 27%; P = .009), however, loneliness persisted or worsened for a subgroup of participants. Open-ended responses revealed challenges faced by the subgroup experiencing persistent loneliness including poor emotional coping and discomfort with new technologies. Conclusion: Many older adults are adjusting to COVID-19 restrictions since the start of shelter-in-place orders. Additional steps are critically needed to address the psychological suffering and unmet medical needs of those with persistent loneliness or barriers to technology-based social interaction.

Abstract:Abstract Social connectivity is a crucial human experience. The objective experience of being disconnected (i.e., social isolation) and the subjective feeling of being disconnected (i.e., loneliness) are related but distinct experiences that have unique impacts on physical, mental, and cognitive health. The COVID-19 pandemic forced much of the global population into a prolonged state of isolation, and many older adults found themselves abruptly cut-off from important sources of social interaction. The lasting impacts of the pandemic are still unknown. This symposium brings together several projects addressing key questions about longitudinal processes of loneliness and social isolation among middle aged and older adults, both prior to and during the COVID-19 pandemic. Jackson will present results from a coordinated data analysis (CDA) showing the extent to which loneliness, social isolation, and social asymmetry are related to MCI, dementia, mortality, and life expectancy. Rule will discuss the impact of stroke on trajectories of loneliness in a CDA. Zhang and Fingerman will present findings that show how loneliness and social isolation vary among individuals with functional limitations. Zavala will present results from a CDA that explored daily patterns of loneliness during the early months of the COVID-19 pandemic. Lastly, Beck will discuss a CDA that examined trajectories of social asymmetry prior to and during the COVID-19 pandemic. Antonucci will synthesize and discuss each project within the context of broader implications for empowering older adults. In sum, this symposium presents novel evidence showing that loneliness and social isolation in older adulthood are impactful predictors of health.
Loneliness and associated factors among older adults during COVID-19 lockdown in Spain

Abstract:Considering the health outcomes of loneliness, it is important to understand the effects of the COVID-19 pandemic for older adults to facilitate detection and intervention. The aim of this study was to examine loneliness among Spanish older adults during the first wave lockdown and associated factors, in comparison to younger adults. An online survey was completed by 3508 adults (401 aged 60 or above). Older adults felt higher social loneliness than younger adults, but lower emotional loneliness. Living alone, poor mental health, and poor healthy habits were related to higher loneliness for both age groups. The results suggest that loneliness should be an important factor in primary care, and prevention efforts should be taken, e.g., by generating open and safe community spaces and contexts for social interaction and promoting the access and ability to use technologies that maintain social connections.

Abstract:Objective Physical distancing and stay-at-home measures implemented to slow transmission of novel coronavirus disease (COVID-19) may intensify feelings of loneliness in older adults, especially those living alone. Our aim was to characterise the extent of loneliness during the first wave in a sample of older adults living in the community and assess characteristics associated with loneliness. Design Online cross-sectional survey between 6 May and 19 May 2020. Setting Ontario, Canada. Participants Convenience sample of members of a national retired educators’ organisation. Primary outcome measures Self-reported loneliness, including differences between women and men. Results 4879 respondents (71.0% women; 67.4% 65–79 years) reported that in the preceding week, 43.1% felt lonely at least some of the time, including 8.3% who felt lonely always or often. Women had increased odds of loneliness compared with men, whether living alone (adjusted OR (aOR) 1.52, 95% CI 1.13 to 2.04) or with others (2.44, 95% CI 2.04 to 2.92). Increasing age group decreased the odds of loneliness (aOR 0.69 (95% CI 0.59 to 0.81) 65–79 years and 0.50 (95% CI 0.39 to 0.65) 80+ years compared with <65 years). Living alone was associated with loneliness, with a greater association in men (aOR 4.26, 95% CI 3.15 to 5.76) than women (aOR 2.65, 95% CI 2.26 to 3.11). Other factors associated with loneliness included: fair or poor health (aOR 1.93, 95% CI 1.54 to 2.41), being a caregiver (aOR 1.18, 95% CI 1.02 to 1.37), receiving care (aOR 1.47, 95% CI 1.19 to 1.81), high concern for the pandemic (aOR 1.55, 95% CI 1.31 to 1.84), not experiencing positive effects of pandemic distancing measures (aOR 1.94, 95% CI 1.62 to 2.32) and changes to daily routine (aOR 2.81, 95% CI 1.96 to 4.03). Conclusions While many older adults reported feeling lonely during COVID-19, several characteristics—such as being female and living alone—increased the odds of loneliness. These characteristics may help identify priorities for targeting interventions to reduce loneliness.

Abstract:Objective: to unveil the experience of the elderly with social isolation in the pandemic of COVID-19. Methods: qualitative study, with 14 elderlies in social isolation. The content was recorded and processed using the software Interface de R pour les Analyses Multidimensionnelles de Textes et de Questionnaires. Results: six classes were obtained, the first focused on spirituality and pre-pandemic pleasurable activities; the second was related to missing the extra-household routine and family life; the third, to the construction of a new routine; the fourth, to the strategies adopted for the prevention of COVID-19; the fifth, to the signs/symptoms experienced during the infection; and the sixth class, to the fear of dying. Conclusion: the experience of the elderly was permeated by the adaptation of routine, adoption of preventive measures and feelings of anguish in the face of uncertainties.
Association of Loneliness With Change in Physical and Emotional Health of Older Adults During the COVID-19 Shutdown

Abstract:Objectives: To examine factors influencing loneliness and the effect of loneliness on physical and emotional health, in the context of the COVID-19 pandemic. Design: Prospective, observational cohort. Setting: Community-dwelling participants. Participants: Older adults (n = 238) enrolled in a longitudinal study. Measurements: Interviews were completed July-December 2020. Loneliness was measured with the UCLA 3-item loneliness scale. Data including age, marriage, education, cognitive functioning, functional impairment, vision or hearing impairment, depression, anxiety, medical comorbidity, social network size, technology use, and activity engagement were collected. Health outcomes included self-rated health, and physical and mental composites from the 12-item Short Form Survey. Physical function was measured by a PROMIS-scaled composite score. Results: Thirty-nine (16.4%) participants reported loneliness. Vulnerability factors for loneliness included age (RR = 1.08, 95% CI 1.02-1.14); impairment with instrumental activities of daily living (RR = 2.08, 95% CI 1.14-3.80); vision impairment (RR = 2.09, 95% CI 1.10-3.97); depression (RR = 1.34, 95% CI 1.25-1.43); and anxiety (RR = 1.92, 95% CI 1.55-2.39). Significant resilience factors included high cognitive functioning (RR = 0.88, 95% CI 0.83-0.94); large social network size (RR = 0.92, 95% CI 0.88-0.96); technology use (RR = 0.81, 95% CI 0.73-0.90); and social and physical activity engagement (RR = 0.91, 95% CI 0.85-0.98). Interaction analyses showed that larger social network size moderated the effect of loneliness on physical function (protective interaction effect, RR = 0.64, 95% CI 0.15-1.13, p <.01), and activity engagement moderated the effect of loneliness on mental health (protective interaction effect, RR = 0.65, 95% CI 0.25-1.05, p <.001). Conclusions: Resilience factors may mitigate the adverse health outcomes associated with loneliness. Interventions to enhance resilience may help to diminish the detrimental effects of loneliness and hold great importance for vulnerable older adults.
The Impact of Loneliness and Social Isolation on Cognitive Aging: A Narrative Review

Abstract:Social concepts such as loneliness and social isolation are fairly new factors that have been recently gaining attention as to their involvement in changes in cognitive function and association with dementia. The primary aim of this narrative review was to describe the current understanding of how loneliness and social isolation influence cognitive aging and how they are linked to dementia. Studies have shown that there is an association between loneliness, social isolation, and reduced cognitive function, in older adults, across multiple cognitive domains, as well as a heightened risk of dementia. Numerous changes to underlying neural biomechanisms including cortisol secretion and brain volume alterations (e.g., white/grey matter, hippocampus) may contribute to these relationships. However, due to poor quality research, mixed and inconclusive findings, and issues accurately defining and measuring loneliness and social isolation, more consistent high-quality interventions are needed to determine whether studies addressing loneliness and social isolation can impact longer term risk of dementia. This is especially important given the long-term impact of the COVID-19 pandemic on social isolation in older people is yet to be fully understood.

Abstract:ObjectivesTo review the impact of social isolation during COVID-19 pandemic on mental and physical health of older people and the recommendations for patients, caregivers and health professionals.DesignNarrative review.SettingNon-institutionalized community-living people.Participants20.069 individuals from ten descriptive cross-sectional papers.MeasurementsArticles since 2019 to 2020 published on Pubmed, Scielo and Google Scholar databases with the following MeSh terms (‘COVID-19’, ‘coronavirus’, ‘aging’, ‘older people’, ‘elderly’, ‘social isolation’ and ‘quarantine’) in English, Spanish or Portuguese were included. The studies not including people over 60 were excluded. Guidelines, recommendations, and update documents from different international organizations related to mental and physical activity were also analysed.Results41 documents have been included in this narrative review, involving a total of 20.069 individuals (58% women), from Asia, Europe and America. 31 articles included recommendations and 10 addressed the impact of social distancing on mental or physical health. The main outcomes reported were anxiety, depression, poor sleep quality and physical inactivity during the isolation period. Cognitive strategies and increasing physical activity levels using apps, online videos, telehealth, are the main international recommendations.ConclusionMental and physical health in older people are negatively affected during the social distancing for COVID-19. Therefore, a multicomponent program with exercise and psychological strategies are highly recommended for this population during the confinement. Future investigations are necessary in this field.

Abstract:One of the most at-risk groups during the COVID-19 crisis is older adults, especially those who live in congregate living settings and seniors’ care facilities, are immune-compromised, and/or have other underlying illnesses. Measures undertaken to contain the spread of the virus are far-reaching, and older adults were among the first groups to experience restrictions on face-to-face contact. Although reducing viral transmission is critical, physical distancing is associated with negative psychosocial implications, such as increased rates of depression and anxiety. Promising evidence suggests that participatory digital co-design, defined as the combination of user-centered design and community engagement models, is associated with increased levels of engagement with mobile technologies among individuals with mental health conditions. The COVID-19 pandemic has highlighted shortcomings of existing technologies and challenges in their uptake and usage; however, strategies such as co-design may be leveraged to address these challenges both in the adaptation of existing technologies and the development of new technologies. By incorporating these strategies, it is hoped that we can offset some of the negative mental health implications for older adults in the context of physical distancing both during and beyond the current pandemic.

Abstract:At the beginning of July 2022, when public health restrictions were lifted, we deployed a country-wide e-survey about how older people were managing now after COVID-19 pandemic-related anxiety. Our responder sample was stratified by age, sex, and education to approximate the Canadian population. E-survey responders were asked to share open-text messages about what contemporaries could do to live less socially isolated lives at this tenuous turning point following the pandemic as the COVID-19 virus still lingered. Contracting COVID-19 enhanced older Canadians’ risk for being hospitalized and/or mortality risk. Messages were shared by 1189 of our 1327 e-survey responders. Content analysis revealed the following four calls to action: (1) cultivating community; (2) making room for what is good; (3) not letting your guard down; and (4) voicing out challenges. Responders with no chronic illnesses were more likely to endorse making room for what is good. Those with no diploma, degree, or certificate least frequently instructed others to not let their guard down. While COVID-19 is no longer a major public health risk, a worrisome proportion of older people across the globe are still living socially isolated. We encourage health and social care practitioners and older people to share messages identified in this study with more isolated persons.
Cumulative effect of loneliness and social isolation on health outcomes among older adults

Abstract:Loneliness and social isolation are described similarly yet are distinct constructs. Numerous studies have examined each construct separately; however, less effort has been dedicated to exploring the impacts in combination. This study sought to describe the cumulative effects on late-life health outcomes.Survey data collected in 2018–2019 of a randomly sampled population of US older adults, age 65+, were utilized (N = 6,994). Survey measures included loneliness and social isolation using the UCLA-3 Loneliness Scale and Social Network Index. Participants were grouped into four categories based on overlap. Groups were lonely only, socially isolated only, both lonely and socially isolated, or neither. Bivariate and adjusted associations were examined.Among participants (mean age = 76.5 years), 9.8% (n = 684) were considered lonely only, 20.6% (n = 1,439) socially isolated only, 9.1% (n = 639) both lonely and socially isolated, and 60.5% (n = 4,232) neither. Those considered both lonely and socially isolated were more likely to be older, female, less healthy, depressed, with lower quality of life and greater medical costs in bivariate analyses. In adjusted results, participants who were both lonely and socially isolated had significantly higher rates of ER visits and marginally higher medical costs.Results demonstrate cumulative effects of these constructs among older adults. Findings not only fill a gap in research exploring the impacts of loneliness and social isolation later in life, but also confirm the need for approaches targeting older adults who are both lonely and socially isolated. As the COVID-19 pandemic continues, this priority will continue to be urgent for older adults.

Abstract:Background/Objectives: Social isolation (SI) and loneliness (L) are the long-standing critical concerns impacting the mental well-being of older adults. The COVID-19 pandemic has exacerbated existing vulnerabilities, leading to a notable rise in perceived social isolation (PSI) and its associated risks among an aging population. Reportedly, nearly 35% of older Americans felt lonely before the pandemic, with the pandemic further intensifying these feelings. This commentary examines the multifaceted factors contributing to PSI, including demographic and socio-economic characteristics. Methods: We outline the health risks associated with PSI, including cardiovascular diseases and mental health conditions. Results: This commentary addresses the potential of information and communication technology (ICT) to alleviate loneliness, despite significant barriers such as the digital divide and technological anxiety among older adults. Conclusions: This commentary advocates targeted digital literacy interventions and theoretical frameworks to enhance technology adoption and mitigate PSI, ultimately aiming to improve health outcomes and quality of life for the aging population.

Abstract:Objectives: There is little research conducted to systematically synthesize the evidence on psychological interventions for social isolation and loneliness among older adults during medical pandemics. This systematic review aims to address this information gap and provides guidance for planning and implementing interventions to prevent and reduce loneliness and social isolation for older adults, especially during medical pandemics. Methods: Four electronic databases (EMBASE, PsychoInfo, Medline and Web of Science) and grey literature from 1 January 2000 to 13 September 2022 were searched for eligible studies on loneliness and social isolation. Data extraction and methodological quality assessment on key study characteristics were conducted independently by two researchers. Both qualitative synthesis and meta-analysis were used. Results: The initial search yielded 3,116 titles. Of the 215 full texts reviewed, 12 intervention articles targeting loneliness during the COVID-19 pandemic met the inclusion criteria. No studies were found concerning intervention with respect to social isolation. Overall, interventions targeting social skills and the elimination of negativities effectively alleviated the feelings of loneliness in the older population. However, they had only short-term effects. Conclusion: This review systematically summarised the key characteristics and the effectiveness of existing interventions addressing loneliness in older adults during the COVID-19 pandemic. Future interventions should focus on social skills and eliminating negativities and be tailored to the needs and characteristics of older people. Repeated larger-scale randomized controlled trials and long-term effectiveness evaluations on this topic are warranted.
