Abstract:Extensive research is currently underway to devise effective strategies for the isolation of circulating tumor cells (CTCs) from blood samples. The utilization of magnetic particles and magnet as a means for this separation process is garnering increased attention within this field. However, when utilizing fixed magnets to capture cancer cells with attached magnetic particles, the inability to control the magnetic field may result in cellular damage due to the strong magnetic field compressing the magnetic particles. In this study, we developed a hybrid system that leverages electromagnetic coils to modulate the strength of the magnetic field from the magnets within a microfluidic channel, enabling real-time control of cancer cells with attached magnetic particles. Through the strategic application of electromagnetic coils, we achieved precise control over the magnet's position, thereby enabling real-time observation of the cancer cell within the microfluidic channel. Additionally, this approach facilitated the release of captured cancer cells from the magnet. The isolated cells could then be cultured to assess individualized drug responses, and evaluate their reactivity to the treatment. Through the implementation of this novel technique, we anticipate that the real-time capture and analysis of CTCs from blood samples will enable more efficient and effective diagnostics and treatment monitoring, ultimately improving patient outcomes.

Abstract:Circulating tumor cell (CTC) and their isolation is one potential clinically useful target within a “liquid biopsy”. Detection and characterization of CTCs provide a non-invasive approach for monitoring cancer therapy. In the present research, a magnet- deformability hybrid integrated microfluidic chip was designed to enumerate CTCs. Arrays of narrow gaps (patterned by micro-ellipse microposts) were designed to capture CTCs bonded with magnetic immune beads. Eleven samples, including Gastric of Colorectal, non-small-cell lung cancer (NSCLC), Ureter cancer, and Breast cancer, were used to validate the microfluidic device clinically. Clinical test results show a high capture efficiency (over 90% at 3ml/h) and high viability (96%) at high flow rate. Furthermore, comparisons of device performance with and without magnet beads were also conducted. This hybrid chip design demonstrated excellent performance in the clinical enumeration of CTCs, which is promising in the future cancer therapy evaluation.

Abstract:<p>Circulating tumor cells (CTCs) largely contribute to cancer metastasis and show potential prognostic significance in cancer isolation and detection. Miniaturization has progressed significantly in the last decade which in turn enabled the development of several microfluidic systems. The microfluidic systems offer a controlled microenvironment for studies of fundamental cell biology, resulting in the rapid development of microfluidic isolation of CTCs. Due to the inherent ability of magnets to provide forces at a distance, the technology of CTCs isolation based on the magnetophoresis mechanism has become a routine methodology. This historical review aims to introduce two principles of magnetic isolation and recent techniques, facilitating research in this field and providing alternatives for researchers in their study of magnetic isolation. Researchers intend to promote effective CTC isolation and analysis as well as active development of next‐generation cancer treatment. The first part of this review summarizes the primary principles based on positive and negative magnetophoretic isolation and describes the metrics for isolation performance. The second part presents a detailed overview of the factors that affect the performance of CTC magnetic isolation, including the magnetic field sources, functionalized magnetic nanoparticles, magnetic fluids, and magnetically driven microfluidic systems.</p>

Abstract:The enumeration of circulating tumor cells (CTCs) has proved valuable for early detection and prognosis in cancer treatment. In this work, an optimized design of a microfluid-based immunomagnetic chip is proposed to separate CTCs from whole blood. Unlike most reported immimomagnetic-based methods in which the magnetic field is applied perpendicular to the flow direction, here, the magnetic field is applied parallel to the flow direction, which makes the hydrodynamic force not the dominant force to hamper cell capture. Experimental validation showed that this novel chip can be operated at a flow rate of up to 6 ml/h with 90% CTC capture efficiency.

Abstract:Ultrasound‐activated microfluidics is presented for rapid modification of magnetic nanoparticles. The derived Fe3O4@SiO2 nanoparticles are hybrid with folic acid for the capture and isolation of CTCs with folate receptors like Hela cells. The usage of microfluidics to upgrade Fe3O4 nanoparticles enables to improve detection efficiency of rare cells associated with malignancies, shedding new light on future clinical applications. Circulating Tumor Cells (CTCs) enrichment technology refers to medical approaches for capturing and isolating target tumor cells, which has drawn great attention owing to its applications in the clinic such as cancer diagnosis and prognosis. Magnetic nanoparticles have emerged as an important platform for CTCs capture and isolation. However, due to the inert surface nature, the molecular phenotype analysis of CTCs and identification sensitivity remain a huge challenge. Herein, an ultrasound‐activated microfluidic method is presented for rapid basic modification of magnetic nanoparticles, laying the foundation for further covalently binding of functional groups to capture CTCs. In this work, Fe3O4@SiO2 nanoparticles obtained from microfluidics are hybrid with folic acid for the capture and isolation of CTCs with folate receptors such as Hela cells. Abstract:Significance Isolation of sufficient numbers of circulating tumor cells (CTCs) in cancer patients could provide an alternative to invasive tumor biopsies, providing multianalyte cell-based biomarkers that are not available from current plasma circulating tumor DNA sequencing. Given the average prevalence at one CTC per billion blood cells, very large blood volumes must be screened to provide enough CTCs for reliable clinical applications. By creating an ultrahigh-throughput magnetic sorter, we demonstrate the efficient removal of leukocytes from near whole blood volume equivalents. Combined with leukapheresis to initially concentrate blood mononuclear cells, this LP CTC-iChip platform will enable noninvasive sampling of cancer cells in sufficient numbers for clinical applications, ranging from real-time pharmacokinetic monitoring of drug response to tissue-of-origin determination in early-stage cancer screening.

Abstract:This paper presents a novel centrifugal microfluidic approach (so-called lab-on-a-CD) for magnetic circulating tumor cell (CTC) separation from the other healthy cells according to their physical and acquired chemical properties. This study enhances the efficiency of CTC isolation, crucial for cancer diagnosis, prognosis, and therapy. CTCs are cells that break away from primary tumors and travel through the bloodstream; however, isolating CTCs from blood cells is difficult due to their low numbers and diverse characteristics. The proposed microfluidic device consists of two sections: a passive section that uses inertial force and bifurcation law to sort CTCs into different streamlines based on size and shape and an active section that uses magnetic forces along with Dean drag, inertial, and centrifugal forces to capture magnetized CTCs at the downstream of the microchannel. The authors designed, simulated, fabricated, and tested the device with cultured cancer cells and human cells. We also proposed a cost-effective method to mitigate the surface roughness and smooth surfaces created by micromachines and a unique pulsatile technique for flow control to improve separation efficiency. The possibility of a device with fewer layers to improve the leaks and alignment concerns was also demonstrated. The fabricated device could quickly handle a large volume of samples and achieve a high separation efficiency (93%) of CTCs at an optimal angular velocity. The paper shows the feasibility and potential of the proposed centrifugal microfluidic approach to satisfy the pumping, cell sorting, and separating functions for CTC separation.

Abstract:Detection and isolation of circulating tumor cells (CTCs) play a pivotal role in the diagnosis and prognosis of cancer, while the high capture efficiency and purity of CTCs are difficult to achieve simultaneously among the various isolation methods. In this work, we designed an inverted microchip integrating silicon nanowires (SiNWs) and multifunctional magnetic nanocomposites (Fe3O4@C6/Ce6@silane, Coumarin 6 (C6), Chlorin e6 (Ce6)) for enhanced capture efficiency and purity of CTCs. The Fe3O4@C6/Ce6@silane conjugated with antibody can label the CTCs and pull them to the upside SiNWs capture surface by the upward magnetic field with high purity. This inverted structure was also featured with real-time detection and photodynamic therapy (PDT) of CTCs with the confocal laser scanning microscope (CLSM). The results indicate the important role of the composites labels and the magnetic field, which greatly improves the capture purity of the CTCs to 90%. Meanwhile, capture efficiency of CTCs achieve to 90.3% in culture medium and 82% in blood with 2 mL/h flow rate, respectively. Based on the structure of the device and composites, the captured CTCs could be directly inactivated by the in situ photodynamic therapy in the capture process which holds positive impact to block cancer spread.

Abstract:Surface-engineered magnetic microparticles are used in chemical and biomedical engineering due to their ease of synthesis, high surface-to-volume ratio, selective binding, and magnetic separation. To separate them from fluid suspensions, existing methods rely on the magnetic force introduced by the local magnetic field gradient. However, this strategy has poor scalability because the magnetic field gradient decreases rapidly as one moves away from the magnets. Here, we present a scalable high-throughput magnetic separation strategy using a rotating permanent magnet and two-dimensional arrays of micromagnets. Under a dynamic magnetic field, nickel micromagnets allow the surrounding magnetic microparticles to self-assemble into large clusters and effectively propel themselves through the flow. The collective speed of the microparticle swarm reaches about two orders of magnitude higher than the gradient-based separation method over a wide range of operating frequencies and distances from a rotating magnet.

Abstract:We present a method for the direct and continuous separation of red and white blood cells from plasma at the microscale. The method is implemented in a microfluidic system with magnetic functionality. The fluidic structure within the microsystem consists of an inlet and a single microfluidic channel with multiple outlets. The magnetic functionality is provided by an array of integrated soft-magnetic elements that are embedded transverse and adjacent to the microchannel. The elements are magnetized using an external field, and once magnetized they produce a magnetic force on blood cells as they flow through the microchannel. In whole blood, white blood cells (WBCs) behave as diamagnetic microparticles, while red blood cells (RBCs) exhibit diamagnetic or paramagnetic behavior depending on the oxygenation of their hemoglobin. We study the motion of blood cells through the microchannel using a mathematical model that takes into account the magnetic, fluidic and gravitational forces on the cells. We use the model to study blood cell separation, and our analysis indicates that the microsystem is capable of separating WBC-rich plasma, deoxygenated RBC-rich plasma and cell-depleted plasma into respective outlets.

Abstract:Magnetic nanotechnology represents a major and promising frontier with great potential to significantly advance the field of liquid biopsies. The last decade has witnessed considerable progress in the research and development of magnetic nanosystems for circulating tumor biomarkers screening. With the emergence of microfluidics, both rational design of magnetic nanomaterials from microfluidic reactors and efficient magnetic screening of circulating tumor biomarkers from microfluidic chips become available. This review focuses on recent advances of magnetic nanoparticles for the screening of circulating tumor biomarkers including circulating tumor cells, exosomes, and nucleic acids. We summarize the established conventional magnetic nanosystems for circulating tumor biomarkers screening, highlight microfluidic reactors-enabled magnetic nanoparticles synthesis, and discuss the emerging roles of microfluidic chips in magnetic screening of circulating tumor biomarkers. In addition, the current challenges and opportunities are provided for guiding future studies.

Abstract:The heterogeneity of circulating tumor cells has a significant impact on the diagnosis, treatment, and monitoring of cancer. Research on the subtypes of circulating tumor cells can bring better treatment outcomes for cancer patients. Here, we proposed a microfluidic chip for the magnetic capture of subtypes of circulating tumor cells from the whole blood and phenotypic profiling by stacking laminar flow vertically. Circulating tumor cells were sorted and captured by the three-dimensional regulation of both magnetic fields in the vertical direction and flow fields in the lateral direction. Using EpCAM-magnetic beads, we achieved sorting and sectional capture of target cells in whole blood and analyzed the surface expression levels of the captured cells, confirming the functionality of the microfluidic chip in sorting and capturing subtypes of circulating tumor cells. This microfluidic chip can also aid in the subsequent subtype analysis of other rare cells.

Abstract:Circulating tumor cells (CTCs) released from the primary tumor to peripheral blood are promising targets for liquid biopsies. Their biological information is vital for early cancer detection, efficacy assessment, and prognostic monitoring. Despite the tremendous clinical applications of CTCs, development of effective separation techniques are still demanding. Traditional separation methods usually use batch processing for enrichment, which inevitably destroy cell integrity and affect the complete information acquisition. Considering the rarity and heterogeneity of CTCs, it is urgent to develop effective separation methods. Microfluidic chips with precise fluid control at the micron level are promising devices for CTC separation. Their further combination with micro-/nanostructure arrays adds more biomolecule binding sites and exhibit unique fluid barrier effect, which significantly improve the CTC capture efficiency, purity, and sensitivity. This review summarized the recent advances in micro-/nanostructure array integrated microfluidic devices for CTC separation, including microrods, nanowires, and 3D micro-/nanostructures. The mechanisms by which these structures contribute to improved capture efficiency are discussed. Two major categories of separation methods, based on the physical and biological properties of CTCs, are discussed separately. Physical separation includes the design and preparation of micro-/nanostructure arrays, while chemical separation additionally involves the selection and modification of specific capture probes. These emerging technologies are expected to become powerful tools for disease diagnosis in the future.

Abstract:Efficient capture data on circulating tumor cells (CTCs) determines early-stage cancer diagnosis and contributes to timely clinical treatment. We present an enhanced method of capturing CTCs accomplished by a microfluidic device integrated with magnetic field to activate the kinetic motion of in-device magnetic beads. The device, consisting of a microfluidic chamber and two electrode chips applied with pulsatile alternating current in both, is designed based on simulations on the periodicity characteristic of magnetic beads as well as the effect of heat dissipation on cell culture medium manifest. Using MEMS technologies, the prototype is fabricated and assembled. The cell capture experiments based on active magnetic beads are achieved in separation of rare cancer cells (MCF7 cells) with low concentration. The capture rate is estimated up to 88 %, with great potential of dramatically improving detection efficiency in disease diagnostics.

Abstract:The development of sensitive and convenient methods for detection, enrichment, and analysis of circulating tumor cells (CTCs), which serve as an importance diagnostic indicator for metastatic progression of cancer, has received tremendous attention in recent years. In this work, a new approach characteristic of simultaneous CTC capture and detection is developed by integrating a microfluidic silicon nanowire (SiNW) array with multifunctional magnetic upconversion nanoparticles (MUNPs). The MUNPs were conjugated with anti-EpCAM antibody, thus capable to specifically recognize tumor cells in the blood samples and pull them down under an external magnetic field. The capture efficiency of CTCs was further improved by the integration with a microfluidic SiNW array. Due to the autofluorescence free nature in upconversion luminescence (UCL) imaging, our approach allows for highly sensitive detection of small numbers of tumor cells, which afterward could be collected for further analysis and re-culturing. We have further demonstrated that this approach can be applied to detect CTCs in clinical blood samples from lung cancer patients, and obtained consistent results by analyzing the UCL signals and the clinical outcomes of lung cancer metastasis. Therefore our approach represents a promising platform in CTC capture and detection with potential clinical utilization in cancer diagnosis and prognosis.

Creating these microposts with interacting magnetic beads makes it possible to tune the geometry in size, position and shape. We developed a simulation tool that combines micromagnetics and discrete particle dynamics, in order to design micropost arrays made of interacting beads. The simulation takes into account the viscous drag of the blood flow, magnetostatic interactions between the magnetic beads and gradient forces from external aligned magnets. We developed a particle-particle particle-mesh method for effective computation of the magnetic force and torque acting on the particles.
