Abstract:The COVID-19 pandemic demonstrates the critical need to reimagine and repair the broken systems of global health. Specifically, the pandemic demonstrates the hollowness of the global health rhetoric of equity, the weaknesses of a health security-driven global health agenda, and the negative health impacts of power differentials not only globally, but also regionally and locally. This article analyses the effects of these inequities and calls on governments, multilateral agencies, universities, and NGOs to engage in true collaboration and partnership in this historic moment. Before this pandemic spreads further – including in the Global South – with potentially extreme impact, we must work together to rectify the field and practice of global health.

Abstract:The COVID-19 pandemic offers an historic precedent to review and challenge the values of social, economic, environmental, and cultural belief systems. The concept of the “New Normal” and the experience of the global pandemic provide points of transition in thinking about our relationship to our planet and to each other. These include the fragility of contemporary economics, dependency on industrialized urban infrastructures, and reliance on top-down governance, vulnerability to climate disasters, dislocation from the natural world, societal inequalities, and the loss of cultural memory. The paper considers the potential role of systems thinking in attempting to manage societies’ responses to the pandemic. To provide the methodological context in which some systems thinking can be applied to alleviate the pandemic, we conduct a focused literature review of systemic frameworks, and using examples from Brazil and England, the paper questions the validity of existing disaster management systems and proposes an integrated critical systems approach. Reflecting on these experiences, questions of systems criticality are further developed and considered in relation to critical recovery from disasters and as integral critical systems (ICS) to interrogate the intention of systems. Finally, the paper reflects upon the value of systems and the values embedded in systems that may or may not promote equitable well-being in recovery from disasters such as COVID-19.

Abstract:The move towards universal health coverage is premised on having well-functioning health systems, which can assure provision of the essential health and related services people need. Efforts to define ways to assess functionality of health systems have however varied, with many not translating into concrete policy action and influence on system development. We present an approach to provide countries with information on the functionality of their systems in a manner that will facilitate movement towards universal health coverage. We conceptualise functionality of a health system as being a construct of four capacities: access to, quality of, demand for essential services and its resilience to external shocks. We test and confirm the validity of these capacities as appropriate measures of system functionality. We thus provide results for functionality of the 47 countries of the WHO African Region based on this. The functionality of health systems ranges from 34.4 to 75.8 on a 0–100 scale. Access to essential services represents the lowest capacity in most countries of the region, specifically due to poor physical access to services. Funding levels from public and out-of-pocket sources represent the strongest predictors of system functionality, compared with other sources. By focusing on the assessment on the capacities that define system functionality, each country has concrete information on where it needs to focus, in order to improve the functionality of its health system to enable it respond to current needs including achieving universal health coverage, while responding to shocks from challenges such as the 2019 coronavirus disease. This systematic and replicable approach for assessing health system functionality can provide the guidance needed for investing in country health systems to attain universal health coverage goals.

Abstract:The outburst and rapid worldwide spread of the COVID-19 pandemic have become the single greatest challenges to humankind today. The adverse impact of the pandemic on the world economy and social order raises the question of whether the existing international public health order is capable of meeting the needs of humankind facing serious infectious diseases like the COVID-19 pandemic. The WHO-led international public health order was formulated at the end of the Second World War and reflected the culture, tradition, and perception of security of Western countries. Stimulated by the process of globalization, national states’ perception of sovereignty, human rights, and interdependence has undergone profound changes. By the very nature of public health, it is closely related to trade and intellectual property, which encouraged and enabled the World Health Organization, the World Trade Organization, and the World Intellectual Property Organization to cooperate closely. Hand in hand with the ever deepening of globalization, public health has become a genuine and important international issue drawing collaboration of many international organizations and ad hoc bodies such as the G20. “There are sure to be bad years following the movement of great armies.” The current international public health order has revealed certain weaknesses and shortcomings that require actions. The system is bound to be reviewed and reformed, which must take into account the interests, cultures, and traditions of the developing countries in the context of global collaboration.

Abstract:OBJECTIVES: This review of systems thinking (ST) case studies seeks to compile and analyse cases from ST literature and provide practitioners with a reference for ST in health practice. Particular attention was given to (1) reviewing the frequency and use of key ST terms, methods, and tools in the context of health, and (2) extracting and analysing longitudinal themes across cases.METHODS: A systematic search of databases was conducted, and a total of 36 case studies were identified. A combination of integrative and inductive qualitative approaches to analysis was used.RESULTS: Most cases identified took place in high-income countries and applied ST retrospectively. The most commonly used ST terms were agent/stakeholder/actor (n = 29), interdependent/interconnected (n = 28), emergence (n = 26), and adaptability/adaptation (n = 26). Common ST methods and tools were largely underutilized. Social network analysis was the most commonly used method (n = 4), and innovation or change management history was the most frequently used tool (n = 11). Four overarching themes were identified; the importance of the interdependent and interconnected nature of a health system, characteristics of leaders in a complex adaptive system, the benefits of using ST, and barriers to implementing ST.CONCLUSIONS: This review revealed that while much has been written about the potential benefits of applying ST to health, it has yet to completely transition from theory to practice. There is however evidence of the practical use of an ST lens as well as specific methods and tools. With clear examples of ST applications, the global health community will be better equipped to understand and address key health challenges.

Abstract:Objectives: Telehealth implementation is a complex systems-based endeavour. This paper compares telehealth responses to (COrona VIrus Disease 2019) COVID-19 across ten countries to identify lessons learned about the complexity of telehealth during critical response such as in response to a global pandemic. Our overall objective is to develop a health systems-based framework for telehealth implementation to support critical response. Methods: We sought responses from the members of the International Medical Informatics Association (IMIA) Telehealth Working Group (WG) on their practices and perception of telehealth practices during the times of COVID-19 pandemic in their respective countries. We then analysed their responses to identify six emerging themes that we mapped to the World Health Organization (WHO) model of health systems. Results: Our analysis identified six emergent themes. (1) Government, legal or regulatory aspects of telehealth; (2) Increase in telehealth capacity and delivery; (3) Regulated and unregulated telehealth; (4) Changes in the uptake and perception of telemedicine; (5) Public engagement in telehealth responses to COVID-19; and (6) Implications for training and education. We discuss these themes and then use them to develop a systems framework for telehealth support in critical response. Conclusion: COVID-19 has introduced new challenges for telehealth support in times of critical response. Our themes and systems framework extend the WHO systems model and highlight that telemedicine usage in response to the COVID-19 pandemic is complex and multidimensional. Our systems-based framework provides guidance for telehealth implementation as part of health systems response to a global pandemic such as COVID-19.

Abstract:Politics as the exercise of power always influence public policies–which reflects the multifaceted nature of decision-making–but "using politics" as a motivation for self-serving interests of government leaders and their allies poses problems. This article reviews the impacts of COVID-19 on health systems of the Latin American and Caribbean region from a political lens. We highlight the overriding influence of politics in health policies, weak governance structures that became compromised, exacerbation of corruption, and breakdowns in the communication and trust between governments and their citizens. There are many factors that did not work well-or as expected. For instance, the poor predictive ability of the Global Health Security Index, which showed that pre-pandemic assessments were deeply naive to how health systems evidence and expertise are uniquely vulnerable to politics. We argue that there is an urgent need to rethink health policy and systems frameworks-including metrics-at national and global level. There is also a need for new global health governance arrangements. The expected solidarity and collaboration among countries was trumped by the rich countries practice of gauging essential resources and vaccines and applying health diplomacy to the rest of the world, and the unchecked power of commercial corporations producing essential medical supplies and vaccines.

Abstract:How has the science system reacted to the early stages of the COVID-19 pandemic? Here we compare the (growing) international network for coronavirus research with the broader international health science network. Our findings show that, before the outbreak, coronavirus research realized a relatively small and rather peculiar niche within the global health sciences. As a response to the pandemic, the international network for coronavirus research expanded rapidly along the hierarchical structure laid out by the global health science network. Thus, in face of the crisis, the global health science system proved to be structurally stable yet versatile in research. The observed versatility supports optimistic views on the role of science in meeting future challenges. However, the stability of the global core-periphery structure may be worrying, because it reduces learning opportunities and social capital of scientifically peripheral countries -- not only during this pandemic but also in its "normal" mode of operation.

Abstract:In the course of the COVID-19 pandemic, it has become clear that primary healthcare systems play a critical role in clinical care, such as patient screening, triage, physical and psychological support and also in promoting good community advice and awareness in coordination with secondary healthcare and preventive care. Because of the role of social and environmental factors in COVID-19 transmission and burden of disease, it is essential to ensure that there is adequate coordination of population-based health services and public health interventions. The COVID-19 pandemic has shown the primary and community healthcare (P&CHC) system’s weaknesses worldwide. In many instances, P&CHC played only a minor role, the emphasis being on hospital and intensive care beds. This was compounded by political failures, in supporting local community resilience. Placing community building, social cohesion and resilience at the forefront of dealing with the COVID-19 crisis can help align solutions that provide a vision of ‘planetary health’. This can be achieved by involving local well-being and participation in the face of any pervasive health and environmental crisis, including other epidemics and large-scale ecological crises. This paper proposes that P&CHC should take on two critical roles: first, to support local problem-solving efforts and to serve as a partner in innovative approaches to safeguarding community well-being; and second, to understand the local environment and health risks in the context of the global health perspective. We see this as an opportunity of immediate value and broad consequence beyond the control of the COVID-19 pandemic.

Abstract:The Covid-19 pandemic has dramatically shown the level of interconnectedness of the human population, the direct relation between human health and the ecosystem, as well as the enormous ethical challenges required for a global response. Relatedly, society has been directly confronted by issues of 'Global health,' both in terms of awareness of health conditions and health systems resiliency all around the world, as well as in terms of governance of the worldwide response and its implications at national and local levels. While Global health is often used as a cosmetic label for neocolonial approaches, it is really an interdisciplinary approach consisting of the interaction between globalization and the determinants of health. Thus, it involves the ecosystem and its transformation and implies a systemic 'One Health' decolonized approach in the definition of its strategies. The Covid-19 pandemic has highlighted the inequities and the limits of the current hegemonic Global health system governance; calling for ethics to provide a renewed, comprehensive, inclusive, and decolonized conceptualization of Global health.

From bureaucratic administration to effective intervention: Comparing early governmental responses to the COVID-19 virus across East Asian and western health systems

Abstract:The outbreak of COVID-19 in early 2020 created dangerous public health conditions which pressured governments and health systems to respond in a rapid and effective manner. However, this type of rapid response required many governments to bypass standing; bureaucratic structures of health sector administration and political governance to quickly take; essential measures against a rapidly evolving public health threat. Each government's particular; configuration of governmental and health system decision-making created specific structural and functional challenges to these necessary centrally developed and coordinated strategies. Most East Asian governments (except Japan) succeeded relatively quickly in centralizing essential disease control and treatment initiatives in a timely manner. In contrast, a number of European countries, especially those with predominantly tax-based financing and politically managed health delivery systems, had greater difficulty in escaping bureaucratic governance and management constraints. Drawing on data about these governments' early stage COVID-19 control experiences, this article suggests that structural changes will be necessary if low-performing governments are to better respond to a pandemic. This paper also summarizes other relatively successful strategies. By adopting such strategies, nations can help overcome structural bureaucratic and administrative obstacles in responding to further waves of COVID-19 or similar future pandemic events.

Abstract:The paper examines the health system’s response to COVID-19 in Sierra Leone. It aims to explore how the pandemic affected service delivery, health workers, patient access to services, leadership, and governance. It also examines to what extent the legacy of the 2013–16 Ebola outbreak influenced the COVID-19 response and public perception. Using the WHO Health System Building Blocks Framework, we conducted a qualitative study in Sierra Leone where semi-structured interviews were conducted with health workers, policymakers, and patients between Oct-Dec 2020. We applied thematic analysis using both deductive and inductive approaches. Twelve themes emerged from the analysis: nine on the WHO building blocks, two on patients’ experiences, and one on Ebola. We found that routine services were impacted by enhanced infection prevention control measures. Health workers faced additional responsibilities and training needs. Communication and decision-making within facilities were reported to be coordinated and effective, although updates cascading from the national level to facilities were lacking. In contrast with previous health emergencies which were heavily influenced by international organisations, we found that the COVID-19 response was led by the national leadership. Experiences of Ebola resulted in less fear of COVID-19 and a greater understanding of public health measures. However, these measures also negatively affected patients’ livelihoods and their willingness to visit facilities. We conclude, it is important to address existing challenges in the health system such as resources that affect the capacity of health systems to respond to emergencies. Prioritising the well-being of health workers and the continued provision of essential routine health services is important. The socio-economic impact of public health measures on the population needs to be considered before measures are implemented.

Abstract:The COVID-19 pandemic has caught many nations by surprise and has already caused millions of infections and hundreds of thousands of deaths worldwide. It has also exposed a deep crisis in modeling and exposed a lack of systems thinking by focusing mainly on only the short term and thinking of this event as only a health crisis. In this paper, authors from several of the key countries involved in COVID-19 propose a holistic systems model that views the problem from a perspective of human society including the natural environment, human population, health system, and economic system. We model the crisis theoretically as a feedback control problem with delay, and partial controllability and observability. Using a quantitative model of the human population allows us to test different assumptions such as detection threshold, delay to take action, fraction of the population infected, effectiveness and length of confinement strategies, and impact of earlier lifting of social distancing restrictions. Each conceptual scenario is subject to 1000+ Monte-Carlo simulations and yields both expected and surprising results. For example, we demonstrate through computational experiments that maintaining strict confinement policies for longer than 60 days may indeed be able to suppress lethality below 1% and yield the best health outcomes, but cause economic damages due to lost work that could turn out to be counterproductive in the long term. We conclude by proposing a hierarchical Computerized, Command, Control, and Communications (C4) information system and enterprise architecture for COVID-19 with real-time measurements and control actions taken at leach level.

Abstract:The coronavirus disease 2019 (COVID-19) pandemic continues to claim lives around the world and, to some extent, reflects the failure of international cooperation. Global health diplomacy (GHD)can be a bridge for international cooperation for tackling public health crises, strengthening health systems through emphasizing universal health coverage for sustainable and equitable development, and rebuilding multilateral organizations. It can be a catalyst for future global health initiatives. Health should not be used as a political tool at the cost of people’s lives, nor should it become a proxy for geopolitics but can be used to diffuse tensions and create a positive environment for political dialogue. Health diplomacy’s focus should be to mitigate inequality by making available diagnostics, therapeutics, and vaccines as a global public good. The implications for the lack of international cooperation will lead to increased global disparities and inequities as the countries that cannot procure vaccines will find their population more vulnerable to the pandemic’s repercussion. Though the international cooperation on trade has suffered the impact of geopolitical shifts and competition, through engaging in GHD, the governments can align the trade and health policies. Amid this global health crisis, the World Health Organization (WHO) has faced an increase in International Health Regulations violations, limiting its influence and response during this COVID-19 pandemic. Nations need to develop a sense of cooperation that serves as the basis for a mutual strategic trust for international development. The priorities of all the countries should be to find the areas of common interest, common operational overlap on development issues, and resource allocation for this global fight against COVID-19.
An Urban Population Health Observatory System to Support COVID-19 Pandemic Preparedness, Response, and Management: Design and Development Study

Abstract:Background: COVID-19 is impacting people worldwide and is currently a leading cause of death in many countries. This study sought to redefine the Healthy People 2030 SDoH taxonomy to accommodate the COVID-19 pandemic. Furthermore, we aim to provide a blueprint and implement a prototype for the Urban Population Health Observatory (UPHO), a web-based platform that integrates classified group-level SDoH indicators to individual- and aggregate-level population health data. The process of building the UPHO involves collecting and integrating data from several sources, classifying the collected data into drivers and outcomes, incorporating data science techniques for calculating measurable indicators from the raw variables, and studying the extent to which interventions are identified or developed to mitigate drivers that lead to the undesired outcomes. We generated and classified the indicators of social determinants of health, which are linked to COVID-19. To display the functionalities of the UPHO platform, we presented a prototype design to demonstrate its features. We provided a use case scenario for 4 different users. UPHO serves as an apparatus for implementing effective interventions and can be adopted as a global platform for chronic and infectious diseases. The UPHO surveillance platform provides a novel approach and novel insights into immediate and long-term health policy responses to the COVID-19 pandemic and other future public health crises. The UPHO assists public health organizations and policymakers in their efforts in reducing health disparities, achieving health equity, and improving urban population health.

Abstract:Based on a grounded theory approach to healthcare professionals’ experience, this study conducted a qualitative assessment of new opportunities and provided a conceptual justification of forward-looking trends of global health system efficiency improvement and resilience toward universal health in the post-COVID-19 era. The data were collected through semi-structured interviews, while theoretical saturation was reached after 20 interviews with international experts in Global Health System Resilience (psychologists, nurses, and community health consultants). The data were analyzed using four coding stages: initial, focused, axial, and theoretical. The results showed that to achieve global health system resilience toward universal health and safety coverage in the post-COVID-19 era, it is required to develop a strategy including the support of healthcare professionals, which allows coping with the emotional and psychological impact of the pandemic. Moreover, opportunities for professional development and collaboration can enhance their resilience and adaptability during and after crises. The findings suggest factors of global health system resilience ranked using the AHP method. The present research provides a valuable insight into the significance of telemedicine, remote care, and ensuring the safety of the healthcare industry and patients as the most crucial factors for enhancing health system sustainability. Why the COVID-19 response needs International Relations

Abstract:Abstract The COVID-19 pandemic affects all countries, but how governments respond is dictated by politics. Amid this, the World Health Organization (WHO) has tried to coordinate advice to states and offer ongoing management of the outbreak. Given the political drivers of COVID-19, we argue this is an important moment to advance International Relations knowledge as a necessary and distinctive method for inclusion in the WHO repertoire of knowledge inputs for epidemic control. Historical efforts to assert technical expertise over politics is redundant and outdated: the WHO has always been politicized by member states. We suggest WHO needs to embrace the politics and engage foreign policy and diplomatic expertise. We suggest practical examples of the entry points where International Relations methods can inform public health decision-making and technical policy coordination. We write this as a primer for those working in response to COVID-19 in WHO, multilateral organizations, donor financing departments, governments and international non-governmental organizations, to embrace political analysis rather than shy away from it. Coordinated political cooperation is vital to overcome COVID-19.

"Digital global health diplomacy" for climate change and human security in the Anthropocene

Abstract:The COVID-19 pandemic has now affected everyone, threatening every aspect of our well-being with over 617597680 confirmed cases, including 6532705 deaths globally. The context of the Anthropocene is the backdrop for the novel, interlinked, systemic, and global threats. Anthropocene is a term proposed to designate the era in which human beings have become predominant drivers of planetary change, drastically altering the planet's biosphere. The concept of global health diplomacy (GHD), which connects the domains of health and international relations, has a critical role in advancing human security. Thus, there is a need for new forms of diplomacy, which is critically important in this complex intermestic and interdependent Anthropocene era, where globalization has inevitably linked nations and population health. This paper introduces, analyzes, and attempts to define "Digital Global Health Diplomacy" (DGHD), which has gained great momentum during this COVID-19 pandemic with concurrent health and human security threats. The application of digital formats to the existing traditional structures for dialogue has become a more popular tool recently. Furthermore, digital means are being used during the COVID-19 pandemic to share the health diplomacy discourse at subnational, supranational, international, regional, and global platforms. DGHD reminds us again of the criticality of this multidisciplinary concept involving the contributions of diplomats, global health specialists, digital technology experts, economists, trade specialists, international law, political scientists, etc., in the global policymaking process. If used effectively by trained global health diplomats through innovative digital platforms, DGHD has a great scope of delivering results faster and has more reach than the traditional approach.
