Abstract:Background: COVID-19 is impacting people worldwide and is currently a leading cause of death in many countries. This study sought to redefine the Healthy People 2030 SDoH taxonomy to accommodate the COVID-19 pandemic. Furthermore, we aim to provide a blueprint and implement a prototype for the Urban Population Health Observatory (UPHO), a web-based platform that integrates classified group-level SDoH indicators to individual- and aggregate-level population health data. The process of building the UPHO involves collecting and integrating data from several sources, classifying the collected data into drivers and outcomes, incorporating data science techniques for calculating measurable indicators from the raw variables, and studying the extent to which interventions are identified or developed to mitigate drivers that lead to the undesired outcomes. We generated and classified the indicators of social determinants of health, which are linked to COVID-19. To display the functionalities of the UPHO platform, we presented a prototype design to demonstrate its features. We provided a use case scenario for 4 different users. UPHO serves as an apparatus for implementing effective interventions and can be adopted as a global platform for chronic and infectious diseases. The UPHO surveillance platform provides a novel approach and novel insights into immediate and long-term health policy responses to the COVID-19 pandemic and other future public health crises. The UPHO assists public health organizations and policymakers in their efforts in reducing health disparities, achieving health equity, and improving urban population health.

What problem does this paper attempt to address?
The problem that this paper attempts to solve is to support the preparation, response, and management of the COVID - 19 epidemic by designing and developing a web - based knowledge - based monitoring platform named Urban Population Health Observatory (UPHO). Specifically, the study aims to: 1. **Redefine the Social Determinants of Health (SDoH) classification of Healthy People 2030**: In order to adapt to the COVID - 19 pandemic, re - classify the SDoH indicators into six domains to describe specific barriers related to the COVID - 19 pandemic. 2. **Design and develop a prototype of the UPHO monitoring platform**: This platform integrates multi - dimensional heterogeneous data, including SDoH indicators and population health data, and provides near - real - time analysis and dashboards for monitoring COVID - 19 - related comorbidities and mortality. ### Detailed Problem Description #### Background - **The impact of COVID - 19**: COVID - 19 has become one of the leading causes of death globally, especially in urban environments, and Social Determinants of Health (SDoH) have a significant impact on the spread and mortality of the epidemic. - **The importance of SDoH**: SDoH includes economic stability, access to and quality of education, community and living environment, access to and quality of healthcare, and social and community context. These factors have an important impact on health outcomes during the epidemic. #### Objectives - **Redefine the SDoH classification**: In order to better respond to COVID - 19, researchers have redefined the SDoH classification of Healthy People 2030, dividing it into six domains: 1. SDoH affecting resource access 2. SDoH increasing disease exposure, susceptibility, and severity 3. SDoH affecting compliance with laws and policies 4. SDoH of community characteristics 5. SDoH enhancing awareness, knowledge dissemination, and health education 6. SDoH of neighborhood and built environment affecting COVID - 19 - related comorbidities - **Design and develop the UPHO platform**: This platform aims to integrate data from multiple sources, including individual - level COVID - 19 indicators, multi - dimensional population - level SDoH indicators, SDoH and epidemiological data, clinical data, and patient - reported outcomes. The platform provides near - real - time analysis and visualization tools through the data analysis layer and the application layer. #### Methods - **Data collection and integration**: Collect data from multiple sources, including regional registries, the US Census Bureau, the Department of Agriculture, and PolicyMap, etc. - **Data classification**: Classify the collected data into drivers and outcomes. - **Data analysis**: Use data science techniques to calculate measurable indicators and study the extent of identification or development of interventions. - **Platform architecture**: The UPHO platform consists of a data layer, an analysis layer, and an application layer, which are responsible for data collection, data analysis, and result presentation respectively. #### Results - **Generation and classification of SDoH indicators**: Generate and classify SDoH indicators related to COVID - 19. - **Platform prototype design**: Demonstrate the functions of the UPHO platform and provide four usage scenario examples for different users. #### Conclusions - **The application value of UPHO**: UPHO can be used as a tool for implementing effective interventions and as a global platform for monitoring chronic and infectious diseases. This platform provides new methods and insights for immediate and long - term public health policy responses, which helps to reduce health inequalities, achieve health equity, and improve the health of urban populations. Through these objectives and methods, this study aims to provide tools for public health organizations and policy - makers to respond more effectively to COVID - 19 and other future public health crises.
