Abstract:Governments across low-income and middle-income countries have pledged to achieve universal health coverage by 2030, which comes at a time where healthcare systems are subjected to multiple and persistent pressures, such as poor access to care services and insufficient medical supplies. While the political willingness to provide universal health coverage is a step into the right direction, the benefits of it will depend on the quality of healthcare services provided. In this analysis paper , we ask whether there are any lessons that could be learnt from the English National Health Service, a healthcare system that has been providing comprehensive and high-quality universal health coverage for over 70 years. The key areas identified relate to the development of a coherent strategy to improve quality, to boost public health as a measure to reduce disease burden, to adopt evidence-based priority setting methods that ensure efficient spending of financial resources, to introduce an independent way of inspecting and regulating providers, and to allow for task-shifting, specifically in regions where staff retention is low.

Lessons from New Zealand for England's NHS Foundation Trusts

Abstract:The legislation to devolve responsibility for the management and operation of England's top-performing NHS hospitals to community-owned NHS Foundation Trusts raises several issues relating to the challenges posed to governance structures by private non-profit ownership and control of assets used to provide government-financed services. Building upon the lessons learned from devolution of public hospital governance in New Zealand to boards at arm's-length from central control during the 1990s, this paper analyses the English NHS hospital changes. Whilst local political accountability and competition between hospitals indicate that the English reforms may be more successful in meeting patients' needs more efficiently than the New Zealand reforms, the English proposals may be compromised by the ability of staff to become members of Trusts, boards bearing risks of decisions outside their control whilst simultaneously being insulated from the consequences of their decisions by a 'soft budget constraint', and conflicts of interest as boards simultaneously act as agents of both central regulators and local beneficiaries.
Universal coverage and cost control: the United Kingdom National Health Service

Abstract:The UK NHS has a number of important strengths. Its costs are relatively low compared to the health care systems of other developed countries due in part to cash limited central budgeting. It is extremely popular with the electorate and surveys show overall satisfaction with the NHS despite some dissatisfaction with waiting lists and a public perception of underfunding. The NHS model of general medical care provided by independent contractors has been acclaimed as "a British success" (General Medical Services Council, 1983). The role of the UK GP combines providing primary care and acting as a gatekeeper to secondary care. This increases equitable access to care for the population and assists in cost containment. As a model, it is currently being emulated in other countries including Sweden and US Health Maintenance Organizations but, as in these countries, the UK primary care model has been evaluated poorly. There are of course continuing weaknesses in the UK health care system. There is insufficient knowledge upon which to base health care services and increase efficiency. In the future, if a knowledge-based health care service is to be created, a considerable amount of research and evaluation is required to identify "what works" in health care (i.e., what is effective) and also the cost effective ways of altering provider behaviour to maximise the amount of health gain which can be achieved using a limited budget. The NHS reforms created a lot of enthusiasm and energy but its effects are difficult to disentangle from the simultaneous increases in funding. There is little evidence from the UK or elsewhere that competition in health care produces efficiency or improvements in resource allocation. Evaluation is required to identify which of the reforms are increasing efficiency. Competition needs to be used with caution and recognised as a mean and not an end in itself. It is remarkable how both clinical practice and health policy reform, in the UK and elsewhere, is poorly evaluated. Medical practice varies substantially locally, regionally, and internationally, e.g., patients with similar age and stage of cancer receive very different levels of radiotherapy across Europe. For most interventions, the appropriate level of treatment may be asserted but is not based on cost effectiveness knowledge. Health policy analysts, like clinicians, make assertions about competition and other health care reforms which are value- rather than knowledge-based. Both groups of decision-makers should be more cautious, informing their choices with research rather than relying on unsubstantiated optimism!
The British NHS at 75: Past, Contemporary, and Future Challenges

Abstract:This article assesses how and why the National Health Service (NHS) has remained at the forefront of British politics and society for 75 years amid significant economic pressures, organizational strain, shifting ideological trends, ongoing reforms, and an unprecedented public health emergency. The postwar "years of consensus" evolved into alleged decline and ostensible neglect during the 1980s, while New Labour sought to rejuvenate this core public service after 1997, featuring investment and often controversial reforms that challenged the party's social democratic values. Amid the New Labour era, NHS powers filtered down to devolved administrations, while from 2008 retrenchment and austerity ensued, fueled by global recession. Austerity eventually subsided, yet from early 2020 the NHS swiftly faced the extreme conditions of the globalized Covid-19 pandemic. The service continues to face challenges regarding its longer-term viability, and this article analyzes this scenario, within the context of the NHS's 75-year historical legacy, its contemporary status, comparative international trends, and likely future evolution.
Achieving a High-Performance Health Care System with Universal Access: What the United States Can Learn from Other Countries

Abstract:This position paper concerns improving health care in the United States. Unlike previous highly focused policy papers by the American College of Physicians, this article takes a comprehensive approach to improving access, quality, and efficiency of care. The first part describes health care in the United States. The second compares it with health care in other countries. The concluding section proposes lessons that the United States can learn from these countries and recommendations for achieving a high-performance health care system in the United States. The articles are based on a position paper developed by the American College of Physicians' Health and Public Policy Committee. This policy paper (not included in this article) also provides a detailed analysis of health care systems in 12 other industrialized countries. Although we can learn much from other health systems, the College recognizes that our political and social culture, demographics, and form of government will shape any solution for the United States. This caution notwithstanding, we have identified several approaches that have worked well for countries like ours and could probably be adapted to the unique circumstances in the United States.

The Gospel of Wealth and the National Health: The Rockefeller Foundation and Social Medicine in Britain's NHS, 1945-60

Abstract:This article examines the Rockefeller Foundation's (RF) engagement with the British National Health Service (NHS) between 1945 and 1960. It argues that the organization morally invested in the success of the NHS because, to them, it offered a world-inspiring model for how to provide medical care following the tenets of social medicine. The RF administratively and financially supported two health centers, in Edinburgh and Manchester, to help realize these ambitions. While the development of both centers exposed conflicting understandings of social medicine, these facilities later became important examples when British health centers expanded in number after the mid-1960s. The article also considers how the shift toward more communal forms of general practice provoked unease regarding privacy among patients. However, strategies used by medical professionals to offset these anxieties helped facilitate public acceptance of forms of care that aligned with the communitarian values of social democracy. The connections between American private philanthropy and British state planning show how a routine visit to the doctor in Edinburgh or Manchester in the 1950s was implicated in the broader politics of postwar global health.

Abstract:It has been argued that "consumerism" has been one of the main influences on health policy in many countries over the last decade or so. This article focuses on the role of consumerism in U.K. health policy during this period through the introduction of changes in National Health Service provision such as introduction of the quasi-market, creation of the new managerialism, and new organizational structures set up under the recent Labour government. It considers the consequences of these changes for the users of health care and for citizenship rights, in particular the extent to which the changes have empowered these users and citizens.
Lessons learned from China's healthcare system and nursing profession

Abstract:Purpose: To examine the healthcare policy and the nursing profession in China and to share insights learned from that country, which can be used to inform health-policy decision makers in the US. Organizing construct: Despite their significant cultural, political, and historical differences, many healthcare issues are the same for China and the US. Both countries face rising healthcare costs and widening disparities between wealthy and poor individuals, and between urban and rural sectors. Conclusions: Some of the ideas that can be gleaned from China's healthcare system include rewarding physicians for prescribing preventive services; using trained laypeople as gatekeepers to the healthcare system in rural areas; communicating to the public and to health-policy decision makers the importance of nurses so that staffing, work conditions, and professional status can be improved; and including nurses in health-policy decisions. Clinical relevance: Lessons learned from China's healthcare policy and healthcare system have implications for public health policy change that could lead to improved patient outcomes in the US. The current changing global health environment calls for healthcare providers to learn from and work with our international neighbors to improve health outcomes for all patients.

Abstract:There is a growing global interest in integrating health and care through digitalisation. However, many ambitious digitalisation initiatives in the healthcare sector fail to achieve their intended outcomes. One contributing factor is the failure to apply lessons learned from past endeavours. We here leverage the experiences gained from large-scale digitalisation efforts within the National Health Service (NHS) in England to distil valuable insights for strategic decision-makers who are embarking on the development and implementation of initiatives aimed at integrating health and social care through digitalisation. While not exhaustive, our compilation of eight key lessons serves as a foundational resource to inform such initiatives, seeking ultimately to contribute to realising maximum benefits for health and care organisations and service users.

Lessons from the Basque Country; Contrasting Basque Integrated Care Health Policy with the New NHS Long Term Plan

Abstract:IntroductionBasque country health reforms are credited with development of one of the most successful integrated care strategies in Europe. These reforms are based on “Health the People’s Right, Everyone’s Responsibility. Health Policy in the Basque Country 2013-2020” (HPBC).England’s NHS has stressed the need for effective integrated care to manage chronic disease and multi-morbidity. The NHS’s approach has been outlined in the “Long Term Plan” (LTP), published January 2019. The LTP’s effectiveness and implementation will be essential to the NHS’s ability to deliver effective integrated care.ObjectiveWe contrast HPBC (proven highly effective in practice) against the new LTP. Our methodological approach is a structured comparison of policy documents, semi-structured interviews with subject-matter experts and evaluation of published and grey literature.Target PopulationThe LTP covers 55.62 million people. HPBC covers 2.2 million. Whilst there is a difference in population size, both exhibit similar baseline challenges (identified within the policies themselves) including multi-morbidity, health inequalities, unhealthy lifestyles and changing demographics.HighlightsDevelopment of both policies took a similar approach of cross-sector engagement. Both approaches stress significant baseline progress in life expectancy, efficiency and outcomes, whilst recognising chronic disease challenges. There are similar focuses on priority diseases including diabetes, cancer, obesity, domestic violence and mental health.A crucial difference is that HPBC outlines conceptual frameworks on which service transformation is based. The frameworks cover health, social determinants and implementation. The LTP does not reframe the conceptual framework, and whilst reduction of health inequality features heavily, it doesn’t have the same prominence. In contrast the LTP focuses more on leveraging digital health interventions.Building on the Basque conceptual frameworks there is the establishment of health as a personal asset and macroeconomic factor. There is a far stronger focus on civic “co-responsibility”. This extends to the Basque "Health in All Policies" approach demanding support from other social structures, whilst the LTP in contrast looks outwards explaining how it “Supports Wider Social Goals.”A key contrast is HPBC’s structured and itemised listing of quantitative indicator goals comparing status quo with end-of-plan targets, this includes health outcomes, core structural and intermediate determinants. The LTP expresses no such quantitative targets, but does include financial targets.TransferabilityThis evaluation has international importance. For the NHS it identifies potential pitfalls and opportunities; for the Basque county it will help develop its next plan; for healthcare globally it allows a comparison of two contrasting approaches to integrated care delivery, providing support to the development of their own local plans.ConclusionsBoth healthcare systems outline ambitious plans for development of sustainable integrated care. The Basque model is centred on conceptual frameworks and an overriding focus on health inequalities alongside quantitative outcome targets. The LTP acknowledges the importance of inequality and focuses more on digital delivery of economically sustainable integrated care. It will be important to evaluate progress against these aims alongside the next iteration of the Basque Health Plan in 2020.This work forms part of an EASD/EFSD Trainee Albert Renold Fellowship

Abstract:Abstract The English National Health Service (NHS) is a public sector organisation with a longstanding objective to deliver high quality healthcare that is free at the point of use. In order to achieve this, the NHS has endured an evocative and controversial theme of reform across many decades. Despite such high levels of reform, the recent Operating Efficiency Framework declared that the NHS is about to enter its toughest ever financial climate. This paper will illustrate the complexities and tensions of implementing service improvement in the NHS in a climate of persistent policy reform, reduced budgets and tough regulation. The paper reports findings of three case studies of hospital trusts in the UK in relation to the implementation of Lean improvement methodologies, highlighting key complexities of a hospital context and the corresponding tension with service improvement activity.

Abstract:AIM This paper reviews the origins, core principles and impact of New Public Management (NPM) in the UK public services, with particular attention paid to the emergence of NPM in the National Health Service (NHS). BACKGROUND NPM represents a distinct break from traditional forms of public sector administration. Whilst its origins are grounded in economic crisis and New Right ideology, NPM philosophies and practices have since found wider acceptance. ORIGINS OF INFORMATION Selective review of key journal papers, books and policy documents. KEY ISSUES NPM is characterized by: managers being 'free to manage'; public sector disaggregation; competition; concern for efficiency; concern for outcomes; concern for quality; individualism rather than collectivism; and identification of the public service user as 'customer', 'consumer' or 'citizen'. CONCLUSIONS NPM has taken root in the NHS. Future reform of the health service is likely to modify some aspects of NPM, though many of the core themes of NPM in the NHS look likely to remain for the foreseeable future.

More for less: best patient outcomes in a time of financial restraint

Abstract:In many countries, expenditure on health care has increased dramatically over recent years. There have been parallel improvements in many indicators of population health, but too many patients continue to be harmed by health care or receive care that is supply-sensitive, ineffective, or poorly aligned with their needs and values. In addition to human costs, this translates into substantial waste of resource. The world has recently faced economic challenges unseen since the great depression of the 1930s. The financial situation of a country can, like a business, be expressed in three sets of accounts: statements of financial position, financial performance, and cash flow. A key test of solvency is the ability to pay debts as they become due (whether from current account or further borrowing). In general, this is a function of public debt, which for many countries has become very high. However, private debt and net financial position are also relevant to a country's financial prospects. Ultimately, borrowing is not sustainable indefinitely and given limited prospects for growth in income in the coming years, most countries will likely need to reduce or at least constrain expenditure on health care. This implies obtaining better value from the resources that are available, and we suggest that the key to this lies in improving the quality of care and, in particular, reducing variation in health care. In the United States, new legislation promoting accountable care organizations may help to do this. Cardiac surgery can be particularly effective in extending patients' lives and in improving the quality of their lives. Our ability to continue to provide cardiac surgery in the face of constrained economic times will depend on engaging more actively in ensuring that what we do is the right thing: that our operations are effective and that they truly meet the needs and values of our patients. It will also depend on doing these operations right the first time.

That may sound improbable, but Ben Collins, of the King’s Fund health charity, made a persuasive case at the fund’s annual conference on November 19. Collins is the author of a new study1 of the Southcentral Foundation’s Nuka system of care run by Alaska Native people, which has evolved in the past 20 years into an international exemplar of the approach backed in NHS England’s Five Year Forward View 2: holistic care delivered by a tax based system that combines high quality, universal clinician and public approval, and affordable costs. Abstract:Julian Tudor Hart The Policy Press, 2010, PB, 336pp, £22.99, 978-1847427823 I'm tempted to suggest you shouldn't read this brilliant book, but only for selfish reasons. Reading this book makes me realise that everything I have written has been said before, and better, by Julian Tudor Hart. Tudor Hart, as you know, is a hero of general practice. He worked as a GP in the Welsh mining village of Glyncorrwg — describing himself in this book as 'a Glyncorrwgologist, the only one in the world.' He was the first GP in the world to know the blood pressure of every one of his patients, and he did this just from seeing his patients coming in for their regular health problems, without a 'health check' in sight. He is perhaps most famous for describing the inverse care law in 1971, which he describes here as 'a banal truism.' This book was written in 2010, the second edition of a 2006 book, and so there are a few sections that betray the 14-year gap between then and now. Journal of Comparative Policy Analysis: Research and Practice

Abstract:This contribution explores the relation between national values and national health policy. The article emphasizes the difficulty in describing “national values” in nations with heterogeneous populations. Values, we characterize as general, and as conflicting with one another. And the article briefly describes the factors besides ideas (including values) that shape national policy: interests and social institutions. The article demonstrates substantial differences in the OECD countries' institutional arrangements – differences in the extent of health insurance coverage, in how organized interest groups bargain about the rules of health financing policy, and which policy instruments are adopted, maintained, or changed over time. At the same time the article provides evidence of widespread public support among the populations of industrialized democracies for the principle of universal access for health care, with the sharing of costs according to ability to pay. This evidence supports the argument that a wide variety of institutions are consistent with the same set of values.

Abstract:Health resource distribution structure should agree with the health care demands of common people, as allocating most resource to primary care institutions.However, China's healthcare resource layout is Inverted Triangle, with less resource to primary care institutions, more to hospitals, and it affects the efficiency of China's health care system.This article studies on the mechanism innovation of British National Health Service, especially the general practice system involving four parts, cultivation of General Practitioner, salary incentive system, principal-agent relationship, and first-contact and referral system, which helps to form optimized healthcare resource structure.It offers important instruction for us to optimize China's healthcare resource allocation by establishing a feasible and systematic general practice system at the primary care level.Based on it, this article proposes some suggestions of China health care reform.
