Abstract:This study explored and reviewed the logistic regression (LR) model, a multivariable method for modeling the relationship between multiple independent variables and a categorical dependent variable, with emphasis on medical research. Thirty seven research articles published between 2000 and 2018 which employed logistic regression as the main statistical tool as well as six text books on logistic regression were reviewed. Logistic regression concepts such as odds, odds ratio, logit transformation, logistic curve, assumption, selecting dependent and independent variables, model fitting, reporting and interpreting were presented. Upon perusing the literature, considerable deficiencies were found in both the use and reporting of LR. For many studies, the ratio of the number of outcome events to predictor variables (events per variable) was sufficiently small to call into question the accuracy of the regression model. Also, most studies did not report on validation analysis, regression diagnostics or goodness-of-fit measures; measures which authenticate the robustness of the LR model. Here, we demonstrate a good example of the application of the LR model using data obtained on a cohort of pregnant women and the factors that influence their decision to opt for caesarean delivery or vaginal birth. It is recommended that researchers should be more rigorous and pay greater attention to guidelines concerning the use and reporting of LR models.

Abstract:Family medicine has traditionally prioritised patient care over research. However, recent recommendations to strengthen family medicine include calls to focus more on research including improving research methods used in the field. Binary logistic regression is one method frequently used in family medicine research to classify, explain or predict the values of some characteristic, behaviour or outcome. The binary logistic regression model relies on assumptions including independent observations, no perfect multicollinearity and linearity. The model produces ORs, which suggest increased, decreased or no change in odds of being in one category of the outcome with an increase in the value of the predictor. Model significance quantifies whether the model is better than the baseline value (ie, the percentage of people with the outcome) at explaining or predicting whether the observed cases in the data set have the outcome. One model fit measure is the count- R 2 , which is the percentage of observations where the model correctly predicted the outcome variable value. Related to the count- R 2 are model sensitivity—the percentage of those with the outcome who were correctly predicted to have the outcome—and specificity—the percentage of those without the outcome who were correctly predicted to not have the outcome. Complete model reporting for binary logistic regression includes descriptive statistics, a statement on whether assumptions were checked and met, ORs and CIs for each predictor, overall model significance and overall model fit.

Abstract:Logistic regression is one of the most commonly used models to account for confounders in medical literature. The article introduces how to perform purposeful selection model building strategy with R. I stress on the use of likelihood ratio test to see whether deleting a variable will have significant impact on model fit. A deleted variable should also be checked for whether it is an important adjustment of remaining covariates. Interaction should be checked to disentangle complex relationship between covariates and their synergistic effect on response variable. Model should be checked for the goodness-of-fit (GOF). In other words, how the fitted model reflects the real data. Hosmer-Lemeshow GOF test is the most widely used for logistic regression model.
Logistic Regression: Limitations in the Estimation of Measures of Association with Binary Health Outcomes

Abstract:Introduction: Logistic regression models are frequently used to estimate measures of association between an exposure, health determinant or intervention, and a binary outcome. However, when the outcome is frequent (> 10%), model estimates for relative risks and prevalence ratios might be biased. Despite the availability of several alternatives, many still rely on these models, and a consensus is yet to be reached. We aimed to compare the estimation and goodness-of-fit of logistic, log-binomial and robust Poisson regression models, in cross-sectional studies involving frequent binary outcomes. Methods: Two cross-sectional studies were conducted. Study 1 was a nationally representative study on the impact of air pollution on mental health. Study 2 was a local study on immigrants' access to urgent healthcare services. Odds ratios (OR) were obtained through logistic regression, and prevalence ratios (PR) through log-binomial and robust Poisson regression models. Confidence intervals (CI), their ranges, and standard-errors (SE) were also computed, along with models' relative goodness-of-fit through Akaike Information Criterion (AIC), when applicable. Results: In Study 1, the OR (95% CI) was 1.015 (0.970 - 1.063), while the PR (95% CI) obtained through the robust Poisson mode was 1.012 (0.979 - 1.045). The log-binomial regression model did not converge in this study. In Study 2, the OR (95% CI) was 1.584 (1.026 - 2.446), the PR (95% CI) for the log-binomial model was 1.217 (0.978 - 1.515), and 1.130 (1.013 - 1.261) for the robust Poisson model. The 95% CI, their ranges, and the SE of the OR were higher than those of the PR, in both studies. However, in Study 2, the AIC value was lower for the logistic regression model. Conclusion: The odds ratio overestimated PR with wider 95% CI and higher SE. The overestimation was greater as the outcome of the study became more prevalent, in line with previous studies. In Study 2, the logistic regression was the model with the best fit, illustrating the need to consider multiple criteria when selecting the most appropriate statistical model for each study. Employing logistic regression models by default might lead to misinterpretations. Robust Poisson models are viable alternatives in cross-sectional studies with frequent binary outcomes, avoiding the non-convergence of log-binomial models.

Abstract:Logistic regression model is one of the most widely used modeling techniques in clinical medicine, owing to the widely available statistical packages for its implementation, and the ease of interpretation. However, logistic model training requires strict assumptions (such as additive and linearity) to be met and these assumptions may not hold true in real world. Thus, clinical investigators need to master some advanced model training methods that can predict more accurately. TensorFlow™ is a popular tool in training machine learning models such as supervised, unsupervised and reinforcement learning methods. Thus, it is important to learn TensorFlow™ in the era of big data. Since most clinical investigators are familiar with the logistic regression model, this article provides a step-by-step tutorial on how to train a logistic regression model in TensorFlow™, with the primary purpose to illustrate how the TensorFlow™ works. We first need to construct a graph with tensors and operations, then the graph is run in a session. Finally, we display the graph and summary statistics in the TensorBoard, which shows the changes of the accuracy and loss value across the training iterations.

Abstract:BACKGROUND: Regression analysis is a standard method in medical research. It is often not clear, however, how the individual components of regression models are to be understood and interpreted. In this article, we provide an overview of this type of analysis and discuss its special features when used in observational studies.METHODS: Based on a selective literature review, the individual components of a regression model for differently scaled outcome variables (metric: linear regression; binary: logistic regression; time to event: Cox regression; count variable: Poisson or negative binomial regression) are explained, and their interpretation is illustrated with respect to a study on multiple sclerosis. The prerequisites for the use of each of these models, their applications, and their limitations are described in detail.RESULTS: Regression analyses are used to quantify the relation between several variables and the outcome variable. In randomized clinical trials, this flexible statistical analysis method is usually lean and prespecified. In observational studies, where there is a need to control for potential confounders, researchers with knowledge of the topic in question must collaborate with experts in statistical modeling to ensure high model quality and avoid errors. Causal diagrams are an increasingly important basis for evaluation. They should be constructed in collaboration and should differentiate between confounders, mediators, and colliders.CONCLUSION: Researchers need a basic understanding of regression models so that these models will be well defined and their findings will be fully reported and correctly interpreted.

Abstract:The purpose of this article is to provide researchers, editors, and readers with a set of guidelines for what to expect in an article using logistic regression techniques. Tables, figures, and charts that should be included to comprehensively assess the results and assumptions to be verified are discussed. This article demonstrates the preferred pattern for the application of logistic methods with an illustration of logistic regression applied to a data set in testing a research hypothesis. Recommendations are also offered for appropriate reporting formats of logistic regression results and the minimum observation-to-predictor ratio. The authors evaluated the use and interpretation of logistic regression presented in 8 articles published in The Journal of Educational Research between 1990 and 2000. They found that all 8 studies met or exceeded recommended criteria.

Abstract:Variables measured in medicine are usually continuous, such as blood pressure, serum glucose, and drug dosage. There are statistical methods to confirm whether these variables act as independent variables that cause an outcome. Most of these methods are types of regression analysis. In this article, starting with a simple correlation coefficient, linear regression, multi-factor regression analysis, analysis of covariance (ANCOVA), logistic regression, and modified Poisson regression will be explained. This article shall also cover when and how to use these regression analysis techniques. The following explanations can be used as a statistical background of research using regression analysis. It can also promote the understanding of medical researchers who would like to conduct advanced regression analysis.

Abstract:Classifying a measurable clinical outcome as a dichotomous variable often involves difficulty with borderline cases that could fairly be assigned either of the two binary class memberships. In such situations the indicated class membership is often highly subjective and subject to, for instance, a measurement error. In other situations the intermediate level of a three-level ordinal factor may sometimes be explicitly reserved for cases which could likely belong to either of the two binary classes. Such indefinite readings are often eliminated from the statistical analysis. In this article we review conceptual and methodological aspects of employing proportional odds logistic regression for a three level ordinal factor as a suitable alternative to ordinary logistic regression when dealing with limited uncertainty in classifying clinical outcome as a binary variable.

Abstract:A biomarker is a measured indicator of a variety of processes, and is often used as a clinical tool for the diagnosis of diseases. While the developmental process of biomarkers from lab to clinic is complex, initial exploratory stages often focus on characterizing the potential of biomarkers through utilizing various statistical methods that can be used to assess their discriminatory performance, establish an appropriate cut-off that transforms continuous data to apt binary responses of confirming or excluding a diagnosis, or establish a robust association when tested against confounders. This review aims to provide a gentle introduction to the most common tools found in diagnostic biomarker studies used to assess the performance of biomarkers with an emphasis on logistic regression.

Abstract:Objectives: In medical research, the study's design and statistical methods are pivotal, as they guide interpretation and conclusion. Selecting appropriate statistical models hinges on the distribution of the outcome measure. Count data, frequently used in medical research, often exhibit over-dispersion or zero inflation. Occasionally, count data are considered ordinal (with a maximum outcome value of 5), and this calls for the application of ordinal regression models. Various models exist for analyzing over-dispersed data such as negative binomial, generalized Poisson (GP), and ordinal regression model. This study aims to examine whether the GP model is a superior alternative to the ordinal logistic regression (OLR) model, specifically in the context of zero-inflated Poisson models using both simulated and real-time data. Methods: Simulated data were generated with varied estimates of regression coefficients, sample sizes, and various proportions of zeros. The GP and OLR models were compared using fit statistics. Additionally, comparisons were made using real-time datasets. Results: The simulated results consistently revealed lower bias and mean squared error values in the GP model compared to the OLR model. The same trend was observed in real-time datasets, with the GP model consistently demonstrating lower standard errors. Except when the sample size was 1000 and the proportions of zeros were 30% and 40%, the Bayesian information criterion consistently favored the GP model over the OLR model. Conclusions: This study establishes that the proposed GP model offers a more advantageous alternative to the OLR model. Moreover, the GP model facilitates easier modeling and interpretation when compared to the OLR model.

Abstract:Up to now I have introduced most steps in regression model building and validation. The last step is to check whether there are observations that have significant impact on model coefficient and specification. The article firstly describes plotting Pearson residual against predictors. Such plots are helpful in identifying nonlinearity and provide hints on how to transform predictors. Next, I focus on observations of outlier, leverage and influence that may have significant impact on model building. Outlier is such an observation that its response value is unusual conditional on covariate pattern. Leverage is an observation with covariate pattern that is far away from the regressor space. Influence is the product of outlier and leverage. That is, when influential observation is dropped from the model, there will be a significant shift of the coefficient. Summary statistics for outlier, leverage and influence are studentized residuals, hat values and Cook's distance. They can be easily visualized with graphs and formally tested using the car package.

Abstract:Measurement error in continuous, normally distributed data is well known in the literature. Measurement error in a binary outcome variable, however, remains under-studied. Misclassification is the error in categorical data in which the observed category is different from the underlying one. In this study, we show, through a Monte Carlo simulation study, that there are non-ignorable biases in parameter estimates if the misclassification is ignored. To deal with the influence of the misclassification, we introduce a model with false-positive and false-negative misclassification parameters. Such a model can not only estimate the underlying association between the dependent and independent variables, but it also provides information on the extent of the misclassification. To estimate the model, a maximum likelihood estimation method based on a Fisher scoring algorithm is utilized. A simulation study is conducted to evaluate the performance, and a real data example is given to demonstrate the usefulness of the new model. An R package is developed to aid the application of the model.

Abstract:The performance of two classification techniques, logistic regression and Support Vector Machines (SVMs), in assessing vaccination data is investigated in this study. The model was trained based on leave-out-one cross validation to obtain an accurate result. Simulated with ten thousand replications, a life data set was used to establish a better model. The findings from the simulation revealed that the logistic regression model slightly outperformed the SVM while the life data shows that the tuned SVM outperformed both the logistic and the SVM. This demonstrates the practical utility of advanced approaches such as SVMs in difficult categorization scenarios such as vaccination prediction. The study emphasizes the superiority of the customized SVM model in this setting, as well as the potential of machine learning approaches to increase comprehension of complicated healthcare scenarios and guide data-driven decision-making for influencing vaccination plans and public health. The study recommends the use of logistic regression if the data point is high.

Abstract:Significance Logistic regression is a popular model in statistics and machine learning to fit binary outcomes and assess the statistical significance of explanatory variables. Here, the classical theory of maximum-likelihood (ML) estimation is used by most software packages to produce inference. In the now common setting where the number of explanatory variables is not negligible compared with the sample size, we show that classical theory leads to inferential conclusions that cannot be trusted. We develop a theory that provides expressions for the bias and variance of the ML estimate and characterizes the asymptotic distribution of the likelihood-ratio statistic under some assumptions regarding the distribution of the explanatory variables. This theory can be used to provide valid inference.

Abstract:: Linear regression is among the most popular statistical models in social sciences research, and researchers in various disciplines use linear probability models (LPMs)—linear regression models applied to a binary outcome. Surprisingly, LPMs are rare in the IS literature, where researchers typically use logit and probit models for binary outcomes. Researchers have examined specific aspects of LPMs’ but not thoroughly evaluated their practical pros and cons for different research goals under different scenarios. We perform an extensive simulation study to evaluate the advantages and dangers of LPMs, especially with respect to big data, which is now common in IS research. We evaluate LPMs for three common uses of binary outcome models: inference and estimation, prediction and classification, and selection bias. We compare its performance to logit and probit under different sample sizes, error distributions, and more. We find that coefficient directions, statistical significance, and marginal effects yield results similar to logit and probit. In addition, LPM estimators are consistent for the true parameters up to a multiplicative scalar. This scalar, although rarely required, can be estimated assuming an appropriate error distribution. For classification and selection bias, LPMs are on par with logit and probit models in terms of class separation and ranking and is a viable alternative in selection models. LPMs are lacking when the predicted probabilities are of interest because predicted probabilities can exceed the unit interval. We illustrate some of these results by modeling price in online auctions using data from eBay.

Abstract:Variable or feature selection is one of the most important steps in model specification. Especially in the case of medical-decision making, the direct use of a medical database, without a previous analysis and preprocessing step, is often counterproductive. In this way, the variable selection represents the method of choosing the most relevant attributes from the database in order to build a robust learning models and, thus, to improve the performance of the models used in the decision process. In biomedical research, the purpose of variable selection is to select clinically important and statistically significant variables, while excluding unrelated or noise variables. A variety of methods exist for variable selection, but none of them is without limitations. For example, the stepwise approach, which is highly used, adds the best variable in each cycle generally producing an acceptable set of variables. Nevertheless, it is limited by the fact that it commonly trapped in local optima. The best subset approach can systematically search the entire covariate pattern space, but the solution pool can be extremely large with tens to hundreds of variables, which is the case in nowadays clinical data. Genetic algorithms (GA) are heuristic optimization approaches and can be used for variable selection in multivariable regression models. This tutorial paper aims to provide a step-by-step approach to the use of GA in variable selection. The R code provided in the text can be extended and adapted to other data analysis needs.
