Abstract:Objective To investigate the contribution of occupational exposure to dusts/gases/fumes to chronic obstructive pulmonary disease(COPD) and respiratory symptoms in China.Methods Based on the cross-sectional survey of COPD which was conducted in urban and rural areas of Beijing,Shanghai,Guangdong,Liaoning,Tianjin,Chongqing and Shanxi for residents aged 40 years or older,the association between the occupational exposure to dusts/gases/fumes and COPD and respiratory symptoms was analyzed.The recruited populations were interviewed with questionnaire and were tested with spirometry.The post-bronchodilators FEV1/FVC<70% was used as diagnostic criteria of COPD.Having any cough,sputum,wheezing and dyspnea was defined as having respiratory symptoms.Results The prevalence of occupational exposure to dusts/gases/fumes was 20.5%.As shown by multiple-variables Logistic regression analyses,occupational exposure to dusts/fumes/gases [OR=1.20(1.04,1.39)] and dusts of grain [1.48(1.18,1.86)]were associated with COPD;occupational exposure to dusts/fumes/gases [OR=1.37(1.25,1.49)],hard-rock mining [OR=2.31(1.67,3.20)],coal mining [OR=1.71(1.09,2.70)],dusts of cement [OR=1.92(1.47,2.52)],chemical or plastics manufacturing [OR=1.58(1.37,1.83)],spray painting [OR=1.46(1.16,1.84)],and other dusts or fumes [OR=1.46(1.29,1.64)]were associated with the respiratory symptoms.Smoking and occupational exposure to dusts/gases/fumes had synergic effects on the increasing risk of respiratory symptoms.The population-attributable risk(PAR) of exposure to dusts/gases/fumes was 3.94% and 7.05% for COPD and respiratory symptoms respectively.Conclusions Occupational exposure to dusts/gases/fumes is associated with COPD and respiratory symptoms.Smoking and occupational exposure to dusts/gases/fumes may have synergic effects on respiratory symptoms.

Abstract:Objective: In China, significant levels of environmental pollution, substandard worksite quality and high rates of smoking predispose the population to potentially high risks of respiratory illnesses and other diseases. We assessed the prevalence of respiratory symptoms and their risks in relation to personal, occupational and environmental risk factors in a heavy-industry province of northeastern China. Methods: Lifestyle, health, residential and occupational data were obtained in 2002 from 31,704 adults of six cities in Liaoning, China, using self-assessment questionnaires. General linear and multi-level models were used to evaluate prevalence rates and risks of respiratory symptoms, related to both individual and combined exposures to environmental and occupational risk factors. Results: The crude prevalence rates (PRs) for persistent cough, persistent phlegm, wheeze and asthma were 2.3, 3.8, 2.1 and 1.0%, respectively. The odds ratios (ORs) of at[ four respiratory symptoms examined were increased by: smoking (ORs from 2.06 to 5.02), occupational dust (ORs from 1.35 to 1.72), occupational gas (ORs from 1.48 to 1.72) and presence of irritating smoke during cooking (ORs from 1.54 to 2.22). An index combining proximity of residence to road, factory or chimney, indoor coat use and presence of irritating smoke during cookingwas associated with up to 3.9-fold increased risks of all symptoms. Increasing values of each risk factor were generally associated with dose-response trends in prevalence rates and risks (at[ p for trend <0.01). Conclusion: The crude PRs of symptoms were tower than those reported by European and American studies but closer to those of previous Chinese studies. The risks of respiratory symptoms in this population were increased by smoking, occupational exposures to dust and gas, and combined residence-related exposures such as living close to a main road, factory or chimney, indoor coal use and the presence of irritating smoke during cooking, among other risk factors. Abstract:OBJECTIVE:to explore effects of airborne fine particulate matter exposure on human respiratory symptoms and pulmonary function.METHODS:one hundred and seven field traffic policemen were recruited as airborne fine particulate matter high-exposure group and one hundred and one male residents as common exposure group. The individual sampler was used to measure fine particulate matter exposure levels of the two groups. To obtain personal information, especially respiratory symptoms such as cough, sputum, etc. a questionnaire survey was used. The pulmonary ventilation function was detected: forced expiratory vital capacity (FVC), the first 1 second forced expiratory volume (FEV1.0), FVC/FEV1.0% and peak flow values (PEF), and the difference of fine particulate matter exposure level and respiratory function of the two groups was compared.RESULTS:24 h individual average fine particulate matter exposure concentration of traffic police and residents were respectively (115.4 ± 46.17) microg/m(3) and (74.94 ± 40.09) microg/m(3), the traffic police PM2.5 exposure levels were significantly higher than the residents. In the incidence of respiratory symptoms, compared with high-exposure group and common exposure group, coughing, expectoration, throat unwell, asthma, short of breath and nose discomfort, traffic police group was higher than residents group (P < 0.05). The abnormal rate of lung ventilation function indexes, such as FVC and FEV1.0 was 25.23% and 12.15% respectively in high-exposure group, 11.88% and 2.97% in common exposure group, there was no statistical difference between two groups. Besides, the abnormal rate of FVC and FEV1.0, showed rising trend in high-exposure group with seniority.CONCLUSION:long-term higher levels of airborne fine particulate matter exposure, may impact respiratory health and impair pulmonary function.

Abstract:BackgroundEvidence regarding the cardiopulmonary effects of indoor coarse particulate matter (PM2.5-10) and its chemical constituents and sources remains limited.MethodsFive consecutive daily visits (200 person-visits in total) were completed among 40 male patients with chronic obstructive pulmonary disease (COPD) in this panel study from November 2015 to May 2016 in Beijing, China. Daily indoor PM2.5-10 was collected and twenty-one elements were measured using inductively coupled plasma mass spectrometry. Lung function and blood pressure were measured at daily visit. Principal component analysis (PCA) was used for source appointment. Bayesian kernel machine regression (BKMR) and linear mixed-effect models were used to establish the exposure-response relationships.ResultsAn interquartile range (IQR, 23.12μg/m3) increase in 1-day average concentration of indoor PM2.5-10 was associated with changes of 0.88 (95CI: -0.25, 2.01) mmHg in systolic blood pressure (SBP), 1.15 (95CI: 0.25, 2.06) mmHg in diastolic blood pressure (DBP), -2.29% (95CI: -4.07%, -0.51%) in forced expiratory volume in the first second (FEV1) and -1.68% (95CI: -3.41%, 0.06%) in peak expiratory flow (PEF). According to the results of PCA, soil/dust, coal combustion, and natural Ni/Cr sources were identified as three main sources of indoor PM2.5-10 inorganic elements. BKMR analysis revealed a significant association of all measured constituents with elevated SBP and a negative trend of FEV1. Coal combustion-related constituents posed a stronger joint effect on blood pressure than the other two sources. Stronger associations of indoor PM2.5-10 constituents with elevated blood pressure and decreased PEF were observed during the heating period than during the non-heating period. And the adverse effects of coal combustion-related constituents on blood pressure and lung function were also stronger in the heating season.ConclusionsIndoor PM2.5-10 constituents were associated with impaired cardiopulmonary health of COPD patients, and those from coal combustion source may play a dominant role, especially during the heating period.

Abstract:BACKGROUND:The cardiopulmonary effects of chemical constituents and sources of indoor fine particulate matter (PM2.5) remain unclear.OBJECTIVES:To examine the individual and joint effects of constituents of indoor PM2.5 on cardiopulmonary function of patients with chronic obstructive pulmonary disease (COPD) and the role of identified sources.METHODS:This panel study recruited 43 stable COPD patients from November 2015 to May 2016 in Beijing, China. Daily indoor and outdoor PM2.5 were collected for five consecutive days simultaneously. Twenty-four elements were measured and principal component analysis was used for source appointment. Pulmonary function and blood pressure (BP) were also measured at daily visit. The linear mixed-effect models were used to estimate the effect of each constituent and source. Bayesian kernel machine regression (BKMR) models were used to estimate the overall effect of all measured constituents.RESULTS:The combustion, indoor soil/dust and road dust sources were identified as the main sources of indoor PM2.5 and combustion sources contributed over 40% during the heating season. Most constituents were significantly associated with elevated BP of COPD patients and the joint effects of mixed exposures were also significant especially during the heating season. Most associations of chemical constituents with pulmonary function were negative but not statistically significant during the heating season, as was the joint effect. Few associations were observed during the non-heating season. Further, we observed combustion sources throughout the study period and road dust sources during the heating season were significantly associated with increased BP but not decreased pulmonary function.CONCLUSION:The combustion and road dust sources and their related constituents of indoor PM2.5 could cause adverse effects on cardiovascular function of COPD patients especially during the heating season, but the effect on pulmonary function still needs to be further studied.

Abstract:OBJECTIVE:This study aims to determine the occupational health status of workers exposed to dust and the risk factors of lung function decline, to provide a basis for formulating corresponding occupational disease-prevention strategies.METHODS:Data on 2045 workers exposed to dust, including their age, gender, exposure time, chest X-ray test results, and pulmonary function test results, were obtained from a key occupational disease monitoring project in Chongqing, China, in 2021. Chi-square tests and multifactorial logistic regression, and other methods, were used for statistical analysis.RESULTS:The prevalence of pneumoconiosis-like changes was 0.83% (17/2045), and the prevalence of abnormal forced expiratory volume in one second (FEV1), forced vital capacity (FVC), and FEV1/FVC was 4.25% (87/2045), 12.81% (262/2045), and 1.47% (30/2045), respectively. With increasing worker's age, the prevalence of abnormal pneumoconiosis-like changes (p = 0.0065), FEV1 (p = 0.0002), FVC (p < 0.0001), and FEV1/FVC (p = 0.0055) all increased. Factors such as age, exposure duration, enterprise size, and dust exposure concentration were associated with abnormal lung function.CONCLUSIONS:Workers exposed to occupational dust have a high rate of abnormal lung function. The government, enterprises, and individuals should pay attention to occupational dust exposure, and various effective measures should be actively taken to protect the life and health of workers.

Abstract:Fine particulate matter (PM2.5) is an important environmental factor affecting human health. However, most studies on PM2.5 and health have used data from fixed monitoring sites to assess PM2.5 exposure, which may have introduced misleading information on the exposure–response relationship. We aimed to assess the effect of short-term personal PM2.5 exposure on lung function in patients with chronic obstructive pulmonary disease (COPD) and asthma. To achieve this, we conducted a longitudinal panel study among 37 COPD patients and 45 asthma patients from Beijing, China. The COPD group and the asthma group completed 148 and 180 lung function tests, respectively. We found that in COPD patients, for every 10-μg/m3 increase in PM2.5 exposure at lag2, the FEV1, FVC and DLco decreased by −0.014 L (95% CI −0.025, −0.003), −0.025 L (95% CI −0.050, −0.003) and −0.089 mmol/min/kPa (95% CI −0.156, −0.023), respectively. There was also a decrease of −0.023 L/s (95% CI −0.042, −0.003) and −0.017 L/s (95% CI −0.032, −0.002) in MMEF at lag3 and lag03, respectively. In the asthma group, every 10-μg/m3 increase in PM2.5 exposure led to a reduction of −0.012 L (95% CI −0.023, −0.001), −0.042 L (95% CI −0.081, −0.003) and −0.061 L/s (95% CI −0.116, −0.004) in the FEV1, FVC and PEF at lag3, respectively. Our findings suggest that PM2.5 exposure may primarily affect both airway function and lung diffusion function in COPD patients, and airway function in asthma patients.

Association between dust exposure and lung function levels in steelworkers: mediation analysis of inflammatory biomarkers

Abstract:Purpose: This investigation aimed to examine the mediating effect of inflammatory biomarkers on the relationship between dust exposure and lung function levels among steelworkers. Methods: The study comprised 2,315 front-line workers employed at an iron and steel company in Tangshan, who underwent occupational health assessments through cluster sampling. Demographic and lifestyle data were collected via a self-administered questionnaire, while physical examinations measured parameters such as height and weight. Lung function was assessed using a portable pulmonary function tester (CHEST). Blood cell counts were uniformly analyzed using a Mindray fully automated biochemistry analyzer (BS-800). Inflammatory biomarkers, including leukocyte count, neutrophil count, lymphocyte count, and platelet count, were assessed, and the neutrophil-to-lymphocyte ratio and systemic immune inflammation index were computed. Generalized linear models and Spearman rank correlation analyses were employed to explore the interplay among dust exposure, inflammatory biomarkers, and alterations in lung function. A mediation analysis model was constructed to elucidate how inflammatory biomarkers mediate the relationship between dust exposure and lung function levels. Results: After adjusting for covariates, dust exposure was significantly associated with reduced lung function levels, with statistically significant differences observed between dust-exposed and non-exposed groups across various lung function indicators (P < 0.001). In the dust-exposed group, inflammatory biomarkers were elevated, showing significant correlations with FVC and FEV1 (P < 0.05). However, the correlation between FEV1/FVC and various inflammatory biomarkers was insignificant (P > 0.05). Mediation analysis revealed that white blood cells and neutrophils partially mediated the association between dust exposure and FVC, with proportions of 1.75% and 1.09%, respectively. Similarly, white blood cells, neutrophils, and the systemic immune inflammation index partially mediated the association between dust exposure and FEV1, with proportions of 1.15%, 0.82%, and 0.82%, respectively. Conclusion: In conclusion, dust exposure poses a risk for decreased lung function levels. Inflammatory biomarkers derived from blood cells offer a valuable and easily obtainable means of identifying changes in lungfunction levels. Among these biomarkers, white blood cells, neutrophils, and the systemic immune inflammation index significantly mediate the association between dust exposure and lung function levels, although further exploration is needed to understand their underlying mechanisms.
Effects of Rush Mat Dust Exposure and Smoking on Workers' FEV1.0

Abstract:OBJECTIVE:To study the effect of smoking and rush-mat dust exposure on pulmonary function.METHOD:122 rush-mat dust exposed workers and 118 controls without dust exposure were selected. Questionnaire and FEVY1.0 measurement were performed on them.RESULTS:For dust exposure, FEV1.0% of cases (81.8% +/- 13.9%) was statistically lower than controls (95.9% +/- 15.3%) (t = -7.49, P < 0.01), the abnormal rate of FEV1.0% was 46.7% (57/122) for cases, statistically higher than controls (8.5%, 10/118), chi2 = 43.6, P < 0.01. For smoking, FEV1.0% of cases (82.02% +/- 12.9%) was statistically lower than controls (93.33% +/- 12.4%), t = -5.18, P < 0.05, the abnormal rate of FEV1.0% is 55.9% for cases, statistically higher than controls (16.7%), chi2 = 22.2, P < 0.05. The exposing ages of workers has negative correlation with FEV1.0% (R(S1) = -0.299, P = 0.0008). Exposure and smoking could affect FEV1.0% (Fe = 259.06, P < 0.01, F(s) = 42.42, P < 0.01), and their interactive effect on FEV1.0% was greater than single ones (F = 34.70, P < 0.01).CONCLUSION:Rush-mat dust exposure, smoking and their interaction have influence on FEV1.0%.

Abstract:Rationale: It remains unknown whether long-term ozone exposure can impair lung function. Objectives: To investigate the associations between long-term ozone exposure and adult lung function in China. Methods: Lung function results and diagnosis of small airway dysfunction (SAD) were collected from a cross-sectional study, the China Pulmonary Health Study (N = 50,991). We used multivariable linear and logistic regression models to examine the associations of long-term ozone exposure with lung function parameters and SAD, respectively, adjusting for demographic characteristics, individual risk factors, and longitudinal trends. We then performed a stratification analysis by chronic obstructive pulmonary disease (COPD). Measurements and Main Results: We observed that each 1 SD (4.9 ppb) increase in warm-season ozone concentrations was associated with a 14.2 ml/s (95% confidence interval [CI], 8.8-19.6 ml/s] decrease in forced expiratory flow at the 75th percentile of vital capacity and a 29.5 ml/s (95% CI, 19.6-39.5 ml/s) decrease in mean forced expiratory flow between the 25th and 75th percentile of vital capacity. The odds ratio of SAD was 1.09 (95% CI, 1.06-1.11) for a 1 SD increase in warm-season ozone concentrations. Meanwhile, we observed a significant association with decreased FEV1/FVC but not with FEV1 or FVC. The association estimates were greater in the COPD group than in the non-COPD group. Conclusions: We found independent associations of long-term ozone exposure with impaired small airway function and higher SAD risks, while the associations with airflow obstruction were weak. Patients with COPD appear to be more vulnerable.

Abstract:Background: The underlying mechanism on the susceptibility of chronic obstructive pulmonary disease (COPD) patients to air pollution has yet to be clarified. Objectives: Based on the COPD in Beijing (COPDB) study, we examined whether lung dysfunction contributed to pollutant-associated systemic inflammation in COPD patients. Methods: Proinflammatory biomarkers including interleukin-8 (IL-8) and tumor necrosis factor alpha (TNF alpha) were measured in serum samples collected from 53 COPD and 82 healthy participants. Concentrations of particulate matter with aerodynamic diameter <= 2.5 mu m (PM2.5), carbonaceous components in PM2.5, and PM size distribution were continuously monitored. Linear mixed effects models were used to examine the associations of biomarker differences with particle exposure, between COPD and healthy participants, and across subgroups with different levels of lung dysfunction. Results: COPD patients showed higher differences in IL-8 and TNF alpha levels associated with exposure to measured pollutants, comparing to healthy controls. In advanced analysis, particle-associated differences in IL-8 and TNF alpha levels were higher in participants with poorer lung ventilation and diffusion capacity, and higher ratio of residual volume. For example, an interquartile range increase in average PM2.5 concentration 2 weeks before visits was associated with a 15.7% difference in IL-8 level in participants with the lowest ratio of measured value to predicted value of forced expiratory volume in 1 s (FEV1%pred) (65.2%), and the association decreased monotonically with increasing FEV1%pred. Associations between differences in TNF alpha level and average ultrafine particle concentration 1 week before visits increased gradually with increasing ratio of measured value to predicted value of residual volume/total lung capacity. Conclusions: COPD patients, especially those with poorer lung function, are more susceptible to systemic inflammation associated with fine particle exposure. (C) 2021 Elsevier B.V. Abstract:Background: Exposure to fine particulate matter (PM2.5) has been linked with adverse cardiorespiratory health conditions. However, evidence for PM2.5 constituents is still scarce, especially among patients with chronic obstructive pulmonary disease (COPD). Objective: To investigate the associations of short-term exposure to different chemical constituents of PM2.5 with measures of cardiac and lung function in COPD patients. Methods: We conducted a retrospective panel study among 100 COPD patients who received repeated measures of left ventricular ejection fraction (LVEF), forced expiratory volume in 1 s (FEV1), forced vital capacity (FVC) and peak expiratory flow (PEF) in Shanghai, China from August 2014 to September 2019. Daily PM2.5 and PM2.5 constituents were obtained from fixed-site monitoring station. Linear mixed-effects models were used to estimate the associations of PM2.5. Results: We found water-soluble ions of PM2.5, mainly NO3-, SO42-, and NH4+ were robustly associated with reduced LVEF, and the reductions in LVEF associated with an IQR increase of them ranged from 1.8% to 2.0% (lag 1 d). Metal constituents such as Cu and As were associated with FEV1, FVC and PEF. The corresponding reductions in lung function parameters for an IQR increase of them ranged from 1.4% to 2.3% (lag 0 or 1 d). These associations remained relatively robust after adjusting for total PM2.5 mass and gaseous pollutants. Conclusions: Our results suggest that water-soluble ions and several metal/metalloid elements might be important constituents in PM2.5 that were associated with reduced cardiorespiratory function among COPD patients. Combustion-Derived Particulate PAHs Associated with Small Airway Dysfunction in Elderly Patients with COPD

Abstract:Evidence of the respiratory effects of ambient organic aerosols (e.g., polycyclic aromatic hydrocarbons, PAHs) among patients with chronic diseases is limited. We aimed to assess whether exposure to ambient particle-bound PAHs could worsen small airway functions in patients with chronic obstructive pulmonary disease (COPD) and elucidate the underlying mechanisms involved. Forty-five COPD patients were recruited with four repeated visits in 2014-2015 in Beijing, China. Parameters of pulmonary function and pulmonary/systemic inflammation and oxidative stress were measured at each visit. Linear mixed-effect models were performed to evaluate the associations between PAHs and measurements. In this study, participants experienced an average PAH level of 61.7 ng/m3. Interquartile range increases in exposure to particulate PAHs at prior up to 7 days were associated with reduced small airway functions, namely, decreases of 17.7-35.5% in forced maximal mid-expiratory flow. Higher levels of particulate PAHs were also associated with heightened lung injury and inflammation and oxidative stress. Stronger overall effects were found for PAHs from traffic emissions and coal burning. Exposure to ambient particulate PAHs was capable of impairing small airway functions in elderly patients with COPD, potentially via inflammation and oxidative stress. These findings highlight the importance of control efforts on organic particulate matter from fossil fuel combustion emissions.

Abstract:Background: Chronic obstructive pulmonary disease (COPD) is a leading cause of death worldwide. There is no clear evidence of whether COPD patients are more susceptible to respiratory inflammation associated with short-term exposure to air pollutants than those without COPD. Objectives: This study directly compared air pollutant-associated respiratory inflammation between COPD patients and healthy controls. Methods: This study is based on the COPDB panel study (COPD in Beijing). Fractional exhaled nitric oxide (FeNO) was repeatedly measured in 53 COPD patients and 82 healthy controls at up to four clinical visits. Concentrations of carbon monoxide (CO), nitrogen monoxide, nitrogen dioxide (NO2), sulfur dioxide (SO2), fine particulate matter (PM2.5), black carbon (BC), Id traline particles (UFPs), and accumulated-mode particles (Acc) were monitored continuously at a fixed-site monitoring station. Linear mixed-effects models were used to compare the associations between In-transformed FeNO and average 1-23 h concentrations of air pollutants before the clinical visits. Results: FeNO was positively associated with interquartile range (IQR) increases in average concentrations of CO, NO2, SO2, BC. UFPs, and Acc in all participants, with the strongest associations in different time-windows (range from 6.6% for average 1 h NO2 exposure to 32.1% for average 7 h SO2 exposure). Associations between FeNO and average 13-23 h PM2.5 exposure differed significantly according to COPD status. Increases in FeNO associated with average 1-2 h NO exposure were significant in COPD patients (range 8.9-10.2%), while the associations were nonsignificant in healthy controls. Associations between leNO and average 1-23 h CO and SO2 exposure tended to be higher in COPD patients than in healthy controls, although the differences were not significant. UFPs-associated respiratory inflammation was robust in both subgroups. Conclusions: COPD patients are more susceptible to respiratory inflammation following PM2.5, NO, CO. and SO2 exposure than individuals without COPD. (C) 2020 Elsevier B.V. All rights reserved.

Abstract:Refractory ceramic fibers (RCFs) are increasingly used as heating‐insulated materials in various industries. However, toxicological and epidemiological studies focusing on the adverse effects of RCFs were still insufficient, particularly in China. We conducted a cross‐sectional study to evaluate comprehensively the associations between occupational exposure to RCFs and respiratory health effects among Chinese workers. We measured and calculated cumulative RCFexposure levels of RCFs workers from the biggest RCFs factory in China. In total, 430 RCF‐exposed workers and 121 controls were enrolled in this study. Physical examinations of the respiratory system were performed and serum levels of biomarkers including Clara cell protein 16 (CC16), surfactant protein D (SP‐D), transforming growth factor β1 (TGF‐β1), and 8‐hydroxy‐2′‐deoxyguanosine (8‐OHdG) were determined among all subjects. RCF exposure workers showed a higher prevalence rate of respiratory symptoms (cough: 11.9%) and lower levels of small airways function indices (V50%: 82.71 ± 20.01, maximal mid expiratory flow (MMEF)%: 81.08 ± 19.56) compared with the control group (cough: 5.0%, V50%: 90.64 ± 24.36, MMEF%: 88.83 ± 24.22). RCFs workers showed higher levels of TGF‐β1 (31.04 ng/mL) and 8‐OHdG (130.72 ng/mL) and lower levels of CC16 (3.68 ng/mL) compared with the controls (TGF‐β1: 26.63 ng/mL, 8‐OHdG: 106.86 ng/mL, CC16: 5.65 ng/mL). After adjusting for covariates, cumulative RCF exposure levels showed significant positive associations with the levels of TGF‐β1 and 8‐OHdG and negative association with the level of CC16. Occupational RCF exposure could induce adverse respiratory health effects, including cough and small airways damage, which may correlate to the altered levels of lung damage markers (CC16 and TGF‐β1) and oxidative markers (8‐OHdG).

Abstract:Background: Occupational health is closely related to harmful factors in the workplace. Dust is the primary contributing factor causing impaired lung ventilation function among employees with dust exposure, and their lung ventilation function may also be influenced by other factors. We aimed at assessing the status and influencing factors of lung ventilation function among employees exposed to dust in the enterprises of the Eighth Division located in the Xinjiang Production and Construction Corps (XPCC), China. Methods: Employees exposed to dust in enterprises of the Eighth Division located in the XPCC in 2023 were selected as the subjects of this cross-sectional study. Their lung ventilation function indicators were extracted from health examination records, and an on-site electronic questionnaire survey was conducted among them. Binary logistic regression analyses were conducted to evaluate the factors influencing lung ventilation function. Results: According to the fixed value criteria, the abnormal rates of forced expiratory volume in 1 s (FEV 1 ), forced vital capacity (FVC), and FEV 1 /FVC were 31.6, 1.4, and 0.4%, respectively. The lower limit of normal (LLN) criteria could overestimate the rate of abnormal lung ventilation function. Several factors were related to impaired lung ventilation function, including gender, age, education level, marital status, body mass index (BMI), smoking status, physical activity, the type of dust, industry, enterprise scale, occupation, length of service, working shift, monthly income, and respiratory protection. Conclusions: A relatively low abnormal rate of lung ventilation function was observed among employees exposed to dust in enterprises of the Eighth Division, XPCC, and their lung ventilation function was associated with various factors. Effective measures should be taken urgently to reduce the effects of adverse factors on lung ventilation function, thereby further protecting the health of the occupational population.

Abstract:PDS 72: Environmental health issues in LMIC, Exhibition Hall (PDS), Ground floor, August 28, 2019, 10:30 AM - 12:00 PM Background/Aim: Epidemiological studies have suggested that certain subgroups of the population, such as individuals with Chronic Obstructive Pulmonary Disease (COPD), are susceptible to the health effects of air pollution, probably due to differences in physiological structure, immune level, exposure pattern, and disease condition. However, the evidence remains inadequate, and it is not clear if air pollution exacerbates COPD development via certain biological pathways. Methods: A nested case-control panel study on susceptibility of individuals with COPD to air pollution in Beijing (COPDB) was conducted. 53 stable COPD patients and 82 healthy subjects had undergone four clinical visits during May, 2016 to September, 2017. Samples of exhaled air, exhaled breath condensation, blood, and urine were collected for measuring biomarkers of inflammation, endothelial function, glycolipid metabolism, and insulin resistance. Multi-omics analysis, such as whole blood transcriptomics and metabolomics were conducted to further reveal underlying mechanisms. Gaseous air pollutants along with detailed chemical components and size distribution of particulate matter were continuously measured before and during each visits. Linear mixed-effect model was fitted to estimate the association. Results: Increase in IL-6 in COPD patients could be significantly higher than those in healthy controls with the increase of the concentrations of CO, NOx, K, Cr, Mn, Fe, Ni, Cu, Ba, and UFPs, depending on the length of exposure. Increase in health endpoints of systematic inflammation, metabolism, ventilation, and resistance in COPD patients were significantly higher than those in healthy controls with the increase of various air pollutants, after 9 days accumulative exposure. Conclusions: this study provided comprehensive evidences about the susceptibility of COPD individuals to the health effects of air pollution in aspects of systematic inflammation, metabolism, ventilation, and resistance.
