Abstract:Introduction: An epidermoid cyst in an intrapancreatic accessory spleen (ECIPAS) is a rare non-neoplastic cyst, typically occurring in the pancreatic tail. It is difficult to preoperatively differentiate ECIPAS from other types of pancreatic neoplastic cysts. Case presentation: We herein report a case of a 32-year-old man with a cystic tumor in the tail of the pancreas. The patient underwent a laparoscopic spleen-preserving distal pancreatectomy, and histological examination revealed the presence of ECIPAS. In addition, we also performed a literature review of 42 case reports of ECIPAS. Conclusion: Although the preoperative diagnosis of ECIPAS is relatively difficult, familiarity with the imaging features, the clinical presentation and the location of the cyst could lead to a correct preoperative diagnosis of ECIPAS, which might thereby reduce the number of unnecessary resections.

Abstract:We describe herein a 68-year-old woman who was diagnosed with a quite rare entity of intraductal papillary mucinous neoplasms (IPMNs) occurring simultaneously in the left lateral lobe of liver and the tail of pancreas.Abdominal computed tomography and magnetic resonance cholangiopancreatography showed a cystic dilatation of the pancreatic duct in the pancreatic tail, which suggested an IPMN, and multiple intrahepatic duct stones in the left lateral lobe.The patient underwent a laparoscopic left lateral hepatolobectomy and spleenpreserving distal pancreatectomy.Intra-operative finding of massive mucin in the dilated bile duct implied an intraductal mucinous tumor in the liver.The diagnosis of synchronous IPMNs in the liver and pancreas was confirmed by pathological examination.The patient was followed up for 6 mo without signs of recurrence.Although several cases of IPMN of liver without any pancreatic association have been reported, the simultaneous occurrence of IPMNs in the liver and pancreas is very rare.To the best of our knowledge, it is the first reported case treated by laparoscopic resection.

Abstract:BACKGROUND:Standard treatment for nonampullary duodenal tumors has not yet been established. In case of tumors originated from the muscularis propria (MP) layer and adherent to the serosa layer, the lesions can not be completely removed by ESD. However, with the development of the endoscopic suture technique, endoscopic full-thickness resection (EFTR) of duodenal subepithelial lesions has become possible.METHODS:We retrospectively analyzed 32 patients with nonampullary duodenal subepithelial lesions who underwent EFTR between February 2012 and January 2017. The suturing method, complications that occurred during and after the operations, perioperative management, tumor characteristics, and pathological findings were analyzed in all patients.RESULTS:The complete resection rate was 100%; all patients successfully received EFTR except for one patient who required conversion to open surgery. Severe abdominal pain was observed after the operation in one patient who then received laparoscopic exploration, and the possibility of delayed perforation was considered. Another patient showed a decline in blood oxygen saturation (SO2) and was transferred to the intensive care unit (ICU) for further management. Delayed bleeding and fistula were not observed. All patients achieved complete remission.CONCLUSION:EFTR is a safe, minimally invasive treatment modality that ensures complete eradication of the duodenal subepithelial lesions.
Application of Endoscopic Technique in Completely Occluded Anastomosis with Anastomotic Separation after Radical Resection of Colon Cancer: a Case Report and Literature Review

Abstract:Background Anastomosis-related complications are common after the radical resection of colon cancer. Among such complications, severe stenosis or completely occluded anastomosis (COA) are uncommon in clinical practice, and the separation of the anastomosis is even rarer. For such difficult problems as COA or anastomotic separation, clinicians tend to adopt surgical interventions, and few clinicians try to solve them through endoscopic operations. Case presentation In this article, we present a case of endoscopic treatment of anastomotic closure and separation after radical resection for sigmoid carcinoma. After imaging examination and endoscopic evaluation, we found that the patient had a COA accompanied by a 3–4 cm anastomotic separation. With the aid of fluoroscopy, we attempted to use the titanium clip marker as a guide to perform an endoscopic incision and successfully achieved recanalization. We used a self-expanding covered metal stent to bridge the intestinal canal to resolve the anastomotic separation. Finally, the patient underwent ileostomy takedown, and the postoperative recovery was smooth. The follow-up evaluation results showed that the anastomotic stoma was unobstructed. Conclusions We reported the successful application of endoscopic technique in a rare case of COA and separation after colon cancer surgery, which is worth exploring and verifying through more clinical studies in the future.
Successful Appendiceal Incision by Endoscopic Submucosal Dissection to Allow Endoscopic Removal of an Encapsulated Fecalith.

Abstract:A 53-year-old man was admitted to our gastroenterology department for investigation of a colonic lump. Ultrasonic colonoscopy revealed a lump (about 0.8 cm) blocking the opening of the appendix and an encapsulated fecalith between the submucosa and the muscularis propria ([Fig. 1] and [Fig. 2] a). Removal of the encapsulated fecalith was difficult using only an endoscope. Therefore, the mucosa and submucosa were incised in the middle of the lump via endoscopic submucosal dissection (ESD); however, a scar was detected during the mucosal and submucosal incision ([Video 1]). The scar was incised with a DualKnife, subsequently revealing a hard yellow encapsulated fecalith ([Fig. 2] b). The lump had originated from inflammatory hyperplasia caused by the encapsulated fecalith. The encapsulated fecalith (about 1×1 cm) was removed using an endoscope ([Fig. 3]). Some bleeding at the incision site was detected endoscopically, and hemostatic clips were applied to stop this. Appendicography showed no residual encapsulated fecalith. The patient experienced slight abdominal distension at the end of the therapeutic endoscopy. He was discharged from the hospital 5 days later with no symptoms.

Abstract:Objectives: Endoscopic full-thickness resection (EFTR) has proven effective and economical for patients with gastric submucosal tumors (SMTs). However, the poor operative field of view, the risk of massive hemorrhage, and the difficulties in defect closure have limited its widespread application. Herein, we described a modified EFTR technique developed to simplify the dissection and defect closure procedures using common and economical endoscopic accessories. Methods: Forty-two patients who underwent the modified EFTR for gastric SMTs in the Shenzhen Guangming District People's Hospital were enrolled in the case series. Following a cross incision to expose the intraluminal surface the tumors were captured by suction through a transparent cap and the roots were ligated using a loop. The tumors and part of the suction tissue were removed along the ligated root. A tension-relieving closure was performed by clipping the raised plica in four quadrants outside the ligated root. Patient demographics, tumor characteristics, and therapeutic outcomes were evaluated retrospectively. Results: All tumors had an R0 resection. The median procedure time was 51.8 min (IQR 34.25 min). No severe perioperative adverse events occurred. No residual lesion or recurrence was reported during the follow-up period of 9.84 months (IQR 5.0 months). Conclusion: The safety and practicability of Modified-EFTR could allow for wide clinical application in patients with micro-gastric SMTs.

Abstract:A 67-year-old man underwent an investigation of epigastric pain and weight loss. Preoperative imaging findings suggested the presence of a tumor, which developed as an epidermoid cyst and originated from an intrapancreatic accessory spleen; however, the possibility of malignancy could not be ruled out. We therefore performed a laparoscopic-assisted distal pancreatectomy with a splenectomy for both diagnostic and treatment purposes. Five laparoscopic ports were created. After the spleen and pancreatic tail were dissected from the retroperitoneum laparoscopically, they were pulled out through a 7-cm left subcostal incision and resected with an endoscopic linear stapler. The operative time was 227 min and the blood loss was 400 ml. The postoperative course was uneventful. The final pathological diagnosis was in agreement with the preoperative diagnosis. This case demonstrates that the minimally invasive approach of laparoscopic surgery can be used safely and successfully for difficult-to-diagnose pancreatic tumors. This is the first report describing a laparoscopic resection of an epidermoid cyst originating from an intrapancreatic accessory spleen.

Abstract:BACKGROUND Gastric stromal tumors, originating from mesenchymal tissues, are one of the most common tumors of the digestive tract. For stromal tumors originating from the muscularis propria, compared with conventional endoscopic submucosal dissection (ESD), endoscopic full-thickness resection (EFTR) can remove deep lesions and digestive tract wall tumors completely. However, this technique has major limitations such as perforation, postoperative bleeding, and post-polypectomy syndrome. Herein, we report a case of postoperative serous surface bleeding which formed an encapsulated hemoperitoneum in a patient with gastric stromal tumor that was treated with exposed EFTR. Feasible treatment options to address this complication are described. CASE SUMMARY A 47-year-old male patient had a hemispherical protrusion found during gastric endoscopic ultrasonography, located at the upper gastric curvature adjacent to the stomach fundus, with a smooth surface mucosa and poor mobility. The lesion was 19.3 mm × 16.1 mm in size and originated from the fourth ultrasound layer. Computed tomography (CT) revealed no significant evidence of lymph node enlargement or distant metastasis. Using conventional ESD technology for mucosal pre-resection, exposed EFTR was performed to resect the intact tumor in order to achieve a definitive histopathological diagnosis. Based on its morphology and immunohistochemical expression of CD117 and DOG-1, the lesion was proven to be consistent with a gastric stromal tumor. Six days after exposed EFTR, CT showed a large amount of encapsulated fluid and gas accumulation around the stomach. In addition, gastroscopy suggested intracavitary bleeding and abdominal puncture drainage indicated serosal bleeding. Based on these findings, the patient was diagnosed with serosal bleeding resulting in encapsulated abdominal hemorrhage after exposed EFTR for a gastric stromal tumor. The patient received combined treatments, such as hemostasis under gastroscopy, gastrointestinal decompression, and abdominal drainage. All examinations were normal within six months of follow-up. CONCLUSION This patient developed serous surface bleeding in the gastric cavity following exposed EFTR. Serosal bleeding resulting in an encapsulated hemoperitoneum is rare in clinical practice. The combined treatment may replace certain surgical techniques.

Abstract:With advancements in endoscopic technology and the integration of artificial intelligence, detection rates of hidden polyps, adenomas, and early colorectal cancer have significantly improved. These advancements have notably enhanced the prevention of colorectal cancer. Endoscopic mucosal resection (EMR), an endoscopic technique developed from polypectomy and mucosal injection methods, is suitable for lesions smaller than 20 mm, as well as flat or laterally spreading tumors. [ 1 ] The colon is the most common site for EMR, and over the past decade, EMR has become the fundamental technique for removing colorectal polyps. It effectively and completely removes polyps with a high R0 (complete) resection rate. [ 2 ] Large prospective studies have demonstrated that EMR is a safe, effective, and cost-efficient procedure. Abstract:With the advancement in endoscopic technology, the discovery of novel endoscopic techniques such as endoscopic full-thickness resection (EFTR) provides new contents for natural orifice transluminal endoscopic surgery (NOTES) and endoscopic therapeutic options for patients with gastrointestinal lesions. Here, we present transcolonic endoscopic appendectomy as an expansion of full thickness resection with extraluminal dissection of the appendix for minimally invasive treatment of the sessile serrated lesions (SSLs) deeply involving the appendiceal orifice; final closure is done by endoloop and clip placement. The procedure was successful in four patients with good follow-up results at the 3 months colonoscopic control. Generally, transcolonic endoscopic appendectomy extends endoscopic resection technique and will promisingly replace combined endoscopic and laparoscopic surgery technique. This technique should be studied further.\n\nSSLs are colorectal lesions with malignant potential1 and should be removed as accurately as possible.2 In dilemma, more than half of the appendiceal adenomas are SSLs3 4 and therapeutic methods are controversial: endoscopic resection, extended laparoscopic appendectomy or combined laparoscopic appendectomy is traditionally optional.1 Surgical resection remains the standard and endoscopic resection is sometimes performed as an alternative to surgical intervention. However, conventional endoscopic resection of SSLs involving the appendiceal orifice has very critical limitations: endoscopic resection into the appendiceal orifice deeply itself carries a risk of appendicitis and it cannot guarantee a negative deep margin for lesions growing into the lumen of the appendix.2 To overcome these disadvantages, we present a novel NOTES technique, transcolonic endoscopic appendectomy, for the SSL involving the appendiceal orifice. This technique takes advantage of EFTR technique, which includes the endoscopic full-thickness incision and defect closure techniques. Abstract:With microtrauma becoming a consensus, in order to improve surgical treatment capability, the clinical application of laparoscopic multiorgan resection is becoming more and more complicated and diversified. Recently, we successfully presented a case of transvaginal specimen extraction surgery that included laparoscopic anatomical left hemihepatectomy combined with laparoscopic total hysterectomy and bilateral adnexectomy and the pelvic and para-aortic lymphadenectomy. The patient, a 75-year-old woman, was hospitalized with abnormal vaginal discharge and bleeding. The pathologic diagnosis of uterine curettage was endometrioid adenocarcinoma. After completing examinations such as color Doppler ultrasound, CEUS, MRCP and thoracoabdominal enhanced spiral CT, preoperative diagnosis was considered as endometrial cancer and a space-occupying lesion in the liver (primary or secondary site)?. No lymphatic or distant metastasis had been found. We also excluded Lynch syndrome by digestive endoscopy and gene sequencing. After a multidisciplinary consultation, the patient underwent surgery under general anesthesia on 24 September 2021. The operation was completed uneventfully in 6 hours, then the patient was transferred to the ICU for follow-up monitoring. The patient began to eat and was able to leave bed on the 4th postoperative day. According to immunohistochemistry, the patient's postoperative diagnosis was intrahepatic cholangiocarcinoma (ICC) and endometrial cancer. Compared with open surgery, laparoscopic multiorgan resection with natural orifice specimen extraction surgery (NOSES) has many advantages such as fewer traumas, shorter recovery time, and better postoperative quality of life. However, combined large-scale laparoscopic surgeries of different organs can be challenging for surgeons and anesthesiologists. No similar cases have been searched.

Abstract:Rationale: Inflammatory fibroid polyp (IFP), also known as Vanek tumor, is a rare, benign gastrointestinal lesion characterized by its inflammatory and fibroid histological features. IFP is often discovered incidentally during endoscopic examinations. It is exceedingly rare for an IFP to prolapse into the duodenum and results in incomplete obstruction of the pylorus. Patient concerns: A 64-year-old male patient was admitted to the hospital with recurrent episodes of melena over a 6-month period, along with complaints of dizziness and fatigue in the past 10 days. Diagnoses: Gastroscopy showed a giant polypoid mass on the posterior wall of the gastric antrum, prolapsing into the duodenum. Abdominal computer tomography (CT) confirmed the tumor protruding into the duodenum. Pathologic examination of the resected specimen confirmed the IFP diagnosis. Interventions: The giant tumor was completely and successfully excised using endoscopic submucosal dissection (ESD). After the surgery, the patient underwent acid suppression and fluid replenishment therapy. Outcomes: The patient responded well to ESD and was discharged in stable condition. As of the submission of the case report, there has been no recurrence of the tumor after a 5-month follow-up, and the patient is still under follow-up. Lessons: While IFPs have traditionally been managed surgically, ESD demonstrates promising treatment outcomes, avoiding the need for surgical distal gastrectomy, and emerges as a safe and effective treatment option.

Abstract:BACKGROUND Colorectal anastomotic occlusion is a serious complication of colorectal cancer surgery. Although several treatment strategies have been proposed, the management of anastomotic occlusion remains challenging. In this report, we present a case of anastomotic occlusion recanalization performed using a novel technique involving two endoscopes, one for radial incision and the other serving as a guide light. This novel technique offers significant advantages in terms of operational feasibility, reduced invasiveness, rapid recovery, and shortened hospital stay. CASE SUMMARY A 37-year-old man underwent low anterior resection and prophylactic double-lumen ileostomy for rectal cancer in June, 2023. Two months later, complete anastomotic occlusion was observed on colonoscopy. Therefore, we developed a novel atresia recanalization technique. Two endoscopes were placed, one through the colonic anastomosis and the other through the anus. A radial incision was successfully made from the colonic side, guided by the light of the endoscope from the anal side. Atresia recanalization was performed within 20 minutes. Three weeks after recanalization, colonoscopy revealed that the diameter of the colorectal anastomosis was approximately 16 mm and the patient therefore underwent stoma reversal in September. During the follow-up period of approximately one year, the patient remained well and no stenosis or obstruction symptoms were observed. CONCLUSION Endoscopic atresia recanalization of colorectal anastomotic occlusion assisted by an opposing light source is safe and effective.
Endoscopic submucosal dissection for a large cecal adenoma covering an appendiceal orifice after appendectomy

Abstract:Endoscopic submucosal dissection (ESD) is recommended for colorectal tumors that cannot be completely removed by snare-based techniques. Endoscopic resection for appendiceal orifice lesions (Toyonaga's type 3: lesion entirely covers the orifice) is technically challenging because the appendiceal portion cannot be incised accurately [1]. Such lesions can be endoscopically removed if the patient has undergone an appendectomy, but submucosal fibrosis is a potential issue. Several reports have shown that ESD using a traction device or underwater strategies are useful for such lesions [2] [3] [4]. We demonstrate ESD for a lesion using the water pressure method ([Video 1]), which is a technique that uses the waterjet function of an endoscope to secure the working space [5]. A 62-year-old woman, who had undergone appendectomy for acute appendicitis 45 years previously, was diagnosed with a 35-mm laterally spreading tumor in the cecum on colonoscopy ([Fig. 1]). The scar of the appendiceal orifice could not be identified because the lesion was covering it. We performed ESD for the lesion with a FlushKnife BT-S (1.5 mm, DK2620J; Fujifilm Medical, Tokyo, Japan). A solution of sodium alginate was injected into the submucosa for thickening. The endoscope faced the lesion perpendicularly, and endoscope stability was poor due to respiratory fluctuations. To secure a stable field of view, the water pressure method was used while sucking air from the cecum. Severe fibrosis was partly visible in the submucosa during ESD ([Fig. 2]). En bloc resection was achieved while maintaining a good visual field using the water pressure method ([Fig. 3], [Fig. 4]). The procedure was completed without any adverse events. The patient was discharged on postoperative day 4. Histopathological examination revealed a high grade tubulovillous adenoma with negative margins. A large cecal adenoma covering the appendiceal orifice after appendectomy could be removed by ESD with the water pressure method. A surgical resection of giant perianal mass secondary to complex anal fistula: a case report

Abstract:In complex anal fistula, the patient's anus has multiple internal or external openings. The closure of the external opening can be broken again in other trips and another external opening. It is one of the refractory diseases in the field of anorectal surgery. In the treatment of a high recurrence rate, poor functional protection and other problems, surgical treatment such as incision and retention of sphincter hanging line, incision and suture internal opening drainage, fistula removal, and suture were used for different situations. After the failure of surgical treatment, it becomes chronic anal fistula or become cancerous tumor. In this case, the patient had a long time of illness, the mass was huge, the color was black, and it was suspected to be a secondary malignant tumor. This situation is extremely rare in clinical treatment. In our hospital, a case of complex anal fistula secondary to perianal huge tumor was found, and good therapeutic effect was achieved after complex anal fistula resection and tumor resection. In this case, the patient had complex anal fistula for about 5 years without timely treatment, and the perianal mass was rare and huge. It is shown by ultrasonography the secondary malignant tumor, which was extremely rare in clinical treatment. During the operation, it was found that three fistulas at the root of the tumor were connected with one of the anal fistulas. From the appearance, the tumor seems to have a malignant tendency. After surgical resection, the pathology was suppurative inflammation and granulation tissue hyperplasia. The patients recovered well after follow-up and did not relapse. This case report provides case reference and reference for clinical treatment of anorectal diseases.
