Abstract:Objective: To investigate the clinical application of endoscopic dissection technique for submucosal tumors (SMTs) in the esophagogastric junction (EGJ). Methods: A total of 43 cases of SMTs in the EGJ were treated by endoscopic resection in the Endoscopy Center of the First Affiliated Hospital, College of Medicine, Zhejiang University between January 2010 and October 2013. The clinical and histopathologic features, surgical approach, en bloc resection rate, complications, and postoperative follow-up information were evaluated. Results: There were 19 males and 24 females with a mean age of 52.0±11.0 years old. The en bloc resection rate was 100%. The mean lesion size was 27.81±19.51 mm. The mean procedure time was 66.97±30.53 minutes and the mean bleeding volume during operation was 4.65±12.12 mL. Perforations occurred in 4 patients, pneumoperitoneum in 2 patients, and esophago-mediastinal fistula in 1 patient. All of them were successfully managed with conservational therapy. One patient with severe pneumothorax was treated with thoracic drainage. Pathologic examination showed that the lesions were leiomyoma (n = 19), gastrointestinal stromal tumor (n = 14), unknown pathological type of spindle cell tumor (n = 5) and intermuscular lipoma (n = 3), and mucinous cystadenoma (n = 1). No local recurrence and distant metastasis were noted during the follow-up. Conclusion: Various endoscopic resection methods based on endoscopic submucosal dissection seem to be safe and effective for SMTs in the EGJ. DOI:10.3781/j.issn.1000-7431.2014.04.008

Abstract:OBJECTIVES: Esophageal leiomyoma is the most common benign tumor of the esophagus, and it originates from mesenchymal tissue. This study analyzed the clinicopathological characteristics of esophageal leiomyoma and aimed to evaluate the role of endoscopic ultrasonography in the diagnosis and treatment selection for these lesions. METHODS: Two hundred and twenty-five patients who had suspected esophageal leiomyomas in endoscopic ultrasonography were enrolled at the Endoscopy Center of The First Affiliated Hospital, Zhejiang University from January 1st, 2009 to May 31th, 2015. The main outcomes included the demographic and morphological characteristics, symptoms, comparisons of diagnosis and treatment methods, adverse events, and prognosis. RESULTS: One hundred and sixty-seven patients were diagnosed as having an esophageal leiomyoma by pathological examination. The mean patient age was 50.57±9.983 years. In total, 62.9% of the lesions originated from the muscularis mucosa, and the others originated from the muscularis propria. The median distance to the incisors was 30±12 cm. The median diameter was 0.72±0.99 cm. As determined by endoscopic ultrasonography, most existing leiomyomas were homogeneous, endophytic, and spherical. The leiomyomas from the muscularis mucosa were smaller than those from the muscularis propria and much closer to the incisors (p<0.05). SMA (smooth muscle antibody) (97.2%) and desmin (94.5%) were positive in the majority of patients. In terms of treatments, patients preferred endoscopic therapies, which led to less adverse events (e.g., intraoperative bleeding, local infection, pleural effusion) than surgical operations (p<0.05). The superficial leiomyomas presented less adverse events and better recovery (p<0.05) than deep leiomyomas. CONCLUSION: Endoscopic ultrasonography has demonstrated high accuracy in the diagnosis of esophageal leiomyomas and provides great support in selecting treatments; however, EUS cannot completely avoid misdiagnosis, so combining it with other examinations may be a good strategy to solve this problem.

Abstract:BACKGROUND Esophageal bronchogenic cyst（EBC） is a rare congenital disease that is difficult to diagnose preoperatively, and treatment remains controversial.CASE SUMMARY We report a 53-year-old Chinese woman hospitalized in our hospital following the discovery of a submucosal protruding mass of the esophagus by upper endoscopy. A preliminary diagnosis of EBC was made by endoscopic ultrasonography（EUS）, and treatment was accomplished by endoscopic submucosal tunnel dissection（ESTD）. The pathological results verified the diagnosis. No scar changes or cystic lesion within the original lesion were found under EUS after a 3-mo follow-up.CONCLUSION EUS is valuable for the preliminary diagnosis of EBC and surveillance. ESTD is a safe and effective treatment for EBC. Further evaluation of complications and long-term follow-ups are required.

Abstract:An inflammatory myofibroblastic tumour is a mesenchymal neoplasm that mostly involves the lung and rarely involves the oesophagus. Surgery has been most commonly used for the treatment of oesophageal inflammatory myofibroblastic tumours but there are no definite guidelines for their diagnosis and treatment. We describe the case of a 60-year-old woman presenting with dysphagia and poor appetite who was diagnosed with a submucosal oesophageal tumour by contrast enhanced computed tomography and ultrasonography endoscopy. She was treated successfully by endoscopic submucosal dissection with no complications. The final diagnosis was confirmed by pathological examination.

Abstract:BACKGROUNDPrimary schwannoma is a rare submucosal tumor of the esophagus, which is most often benign, and surgery is the only effective treatment. So far, only a few cases have been reported. Herein, we reported a single case diagnosed with primary esophageal schwannoma that was totally removed by submucosal tunneling endoscopic resection (STER).CASE SUMMARYA 62-year-old man presented to the hospital with a history of resection of a malignant gastric tumor and mild dysphagia. Endoscopic examination revealed a large submucosal elevated lesion in the esophagus 25-30 cm from the incisors. Endoscopic ultrasonography detected a 45 mm x 35 mm x 31 mm hypoechoic lesion; chest computed tomography showed a mass of approximately 55 mm x 35 mm x 29 mm. A preliminary examination showed features suggestive of a stromal tumor. Pathological findings indicated esophageal schwannoma. Next, STER alone was performed to completely resect the mass, and the patient recovered well post-surgery. Afterward, the patient was discharged and showed no tumor recurrence at 33 mo of follow-up.CONCLUSIONEndoscopic resection is still an effective treatment for large esophageal schwannomas (> 30 mm) under meticulous morphological evaluation.

You can register here Esophageal leiomyoma is the most common benign intramural tumor of the esophagus. Despite being the most common benign tumor in its category, esophageal leiomyomas constitute only 1.2% of all esophageal tumors. While esophageal leiomyoma itself is uncommon, the occurrence of multiple esophageal leiomyomas is even rarer, and the coexistence of high-grade squamous intraepithelial neoplasia (HGIN), involving both epithelial and mesenchymal tissues, is exceedingly rare. This case report describes a patient with multiple esophageal leiomyomas and localized HGIN, diagnosed using endoscopic ultrasonography, which identified a submucosal lesion within the muscularis mucosae. The lesion was successfully treated using endoscopic high-frequency electrocoagulation resection. This minimally invasive approach proved to be precise, safe, and effective, offering therapeutic outcomes comparable to those of traditional surgical resection, as confirmed by postoperative pathological analysis.

Unusual Case of a Protruding Mass in the Esophagus Successfully Treated by Endoscopic Submucosal Dissection

Abstract:A 65-year-old man presented with a history of progressive dysphagia for 1 month. His physical examination and blood tests showed no abnormalities. He underwent upper gastrointestinal endoscopy three times, which showed a bulky protruding mass in the esophagus at 34 – 39 cm from the incisors ([Fig. 1]). Pathologic findings all revealed inflammatory hyperplasia of epithelial cells and subepithelial granulation tissue. A contrast-enhanced computed tomography (CT) scan showed an intraluminal mass in the lower esophagus ([Fig. 2]), with no evidence of mediastinal lymphadenopathy or any metastases in the chest or abdominal organs. After a multidisciplinary team discussion, we performed endoscopic submucosal dissection (ESD) ([Video 1]), and the lesion was successfully resected en bloc ([Fig. 3]).

Abstract:A 35-year-old female complained of slight dysphagia for 3 months. Her physical examination and laboratory tests were unremarkable. Esophagogastroduodenoscopy (EGD) revealed a submucosal tumor (SMT) in the lower esophagus. Then, endoscopic ultrasonography (EUS) revealed that a hypoechoic echo lesion (10mm*12mm) originated from muscularis propria. Subsequently, ligation-assisted endoscopic resection was performed to remove the esophageal lesion. The steps were briefly described as follows: Marking dots in the SMT, and injecting submucosally beneath the marking dots. Incising apical mucosal surface around the marking dots Assembling an endoloop and ligation device (MAJ-339; Olympus). Ligating the SMT with endoloop. Cold snare the SMT.Ligating the defect using another endoloop. Histopathology confirmed a leiomyoma). After 2 months follow-up, EGD revealed healing of the esophageal lesion.

Abstract:A 66-year-old female patient underwent esophagogastroduodenoscopy (EGD) in our center because of occasional dysphagia, and a large submucosal tumor (SMT) was identified 32–36 cm from the incisors, occupying nearly half of the luminal circumference (Figure 1A). Mini-probe endoscopic ultrasonography (EUS) revealed a hypoechoic submucosal lesion, measuring 20 15 mm and originating from the superficial muscularis propria layer (Figure 1B). It was covered by normal-appearing mucosa. The patient underwent EUS (SU-9000, Fujifilm, Japan) for further investigation (Figure 1C). EUS elastography showed high tissue stiffness with a predominantly blue pattern (Figure 1D) and there was no enhancement after injection of intravenous contrast. Overall, these findings were consistent with those of a submucosal tumor (SMT) of the lower esophagus originating in the muscularis propria, possibly a leiomyoma. The patient underwent en bloc resection of the tumor by submucosal tunneling endoscopic resection (STER). The first step of the procedure consisted of a submucosal injection 5 cm proximally to the lesion. A longitudinal mucosal incision and subsequent submucosal tunneling were performed using the Hybrid Knife (ERBE, Germany). Endoscopic en bloc resection was then carried out using both the Hybrid Knife and the IT Knife (Olympus, Japan). After retrieval of the endoscopic specimen measuring 40 x 20 mm (Figure 1E) and adequate hemostasis of the tunnel, the incision site was closed using hemostatic clips. However, histology revealed a poorly differentiated neuroendocrine carcinoma (small-cell carcinoma) invading the submucosal layer with lymphovascular invasion (Figure 1F). Immunohistochemistry staining was positive for CD56, synaptophysin, CgA and CK-PAN and negative for SSTR2, SSTR5, and p53. The Ki-67 score exceeded 60%. The patient was suggested to receive further surgical treatment. However, she did not seek any treatment. On a 3-month follow-up, investigation revealed distant metastases to both liver lobes and the left chest wall so the patient was referred for palliative care.

Abstract:Although most large submucosal tumors in the esophagus such as leiomyomas are benign, obstructive symptoms may develop owing to their size [1]. Endoscopic resection of large tumors in the esophagus remains difficult [2], and exophytic tumors in the mid-esophagus are the worst. The main problem is the presence of important adjacent organs such as bronchus, aorta, and spine. We report a novel concept for facilitating endoscopic resection: 3D-printed model in the guidance of tumor resection (3DM-GTR). The 3D-printed model, based on enhanced computed tomography, could clearly display the tumor anatomy and details of adjacent structures, playing a role in planning and implementing endoscopic resection.

Abstract:Objective To study the value of endoscopic submucosal excavation (ESE) for esophageal submucosal tumors originating from the muscularis propria layer.Methods Data of 27 patients with 29 lesions in esophageal muscularis propria treated with ESE from Dec.2008 to Dec.2010 were retrospected.Feasibility,effects and safety were evaluated accordingly.Results Of 27 patients,there were 17 males and 10 females.Mean age was 50(22 ～62)yrs,and mean diameter of the lesions was 1.25 ±0.70 (0.5 ～3.0)cm.Resection rate was 96.3％ (26/27).One failed case with tumor residual after ESE received additional operation.The median procedure time was 74 (30-120) min.Pathological examination confirmed leiomyoma in 26 cases and gastrointestinal stromal tumor (GISTs) in 1 case.Perforation during operation occurred in 2 cases,accompanied with pneumothorax.They were treated with closed thoracic drainage,without surgery.The median follow-up time was 12 months (3 to 27 months),and no recurrence was found.Conclusion ESE is a safe and effective therapy for the esophageal tumor smaller than 3.0 cm from the muscularis propria.
