Abstract:Medical waste management is of great importance due to its infectious and hazardous nature that can cause undesirable effects on humans and the environment. The objective of this study was to analyze and evaluate the present status of medical waste management in the light of medical waste control regulations in Nanjing. A comprehensive inspection survey was conducted for 15 hospitals, 3 disposal companies and 200 patients. Field visits and a questionnaire survey method were implemented to collect information regarding different medical waste management aspects, including medical waste generation, segregation and collection, storage, training and education, transportation, disposal, and public awareness. The results indicated that the medical waste generation rate ranges from 0.5 to 0.8 kg/bed day with a weighted average of 0.68 kg/bed day. The segregated collection of various types of medical waste has been conducted in 73% of the hospitals, but 20% of the hospitals still use unqualified staff for medical waste collection, and 93.3% of the hospitals have temporary storage areas. Additionally, 93.3% of the hospitals have provided training for staff; however, only 20% of the hospitals have ongoing training and education. It was found that the centralized disposal system has been constructed based on incineration technology, and the disposal cost of medical waste is about 580 US$/ton. The results also suggested that there is not sufficient public understanding of medical waste management, and 77% of respondents think medical waste management is an important factor in selecting hospital services. The problematic areas of medical waste management in Nanjing are addressed by proposing some recommendations that will ensure that potential health and environmental risks of medical waste are minimized.

Abstract:Furthering healthcare reform calls for public hospitals to curb unreasonable growth of medical expenses,reduce drug proportion,and lower the costs of medical materials consumed per 100 RMB medical income.In view of the hospital,the authors established a data-based monitoring system for key indicators of medical consumables,formulated an evaluation scheme of rational use,and implemented the corresponding control measures,which provide a reference for scientific control of clinical rational use of the medical consumables.

Abstract:OBJECTIVEThis paper adopts advanced information technology to manage medical supplies, in order to improve the medical supplies management level and reduce material cost.METHODSIt develops a Medical Supplies Management System with B/S and C/S mixed structure, optimizing material management process, building large equipment performance evaluation model, providing interface solution with HIS, and realizing real-time information briefing of high value material's consumption.CONCLUSIONThe medical materials are managed during its full life-cycle. The material consumption of the clinical departments is monitored real-timely. Through the closed-loop management with pre-event budget, mid-event control and after-event analysis, it realizes the final purpose of management yielding benefit.

Abstract:Currently,medical consumables management,to focus on high-value medical consumables,has become hot research area of hospital,regulatory and industry.Its management level directly affects the medical quality and economic efficiency of the hospital.The original static and extensive mode of medical consumables management has been difficult to meet the rapid development of hospital.This paper discusses how to establish an entire regulation management system of high-value Consumables,based on the theory of supply chain management.It realizes dynamic tracking by the combination of bar code and information technology and promotes the integration of material flow,capital flow and information flow.So that the high-value consumables management tends to be scientific and effective,and provides adequate protection to continuously improve the level of medical safety and quality management.

Abstract:Objective:This paper aims at ensuring safe use of medical materials to establish a management platform for tracing back, reduce medical cost, easing patient burden by strengthening scientific management on medical materials. Methods:A series of strategies such as establishing medical materials access system, choosing suppliers, executing into and out of library strictly, establishing a management platform for tracing back, and monitoring the adverse events of medical apparatus have been taken. Results:The goal of standard and professional management on medical materials has been basically achieved. Conclusion:Medical materials information is an important basis for hospital decision-making. Scientific, professional and standard management assures the quality of medical treatment.
Effectiveness of the special-grade antimicrobial stewardship based on information technology

Abstract:Objective:To evaluate the effectiveness of the information management system on the clinical application of special-grade antimicrobial.Methods:Using the established knowledge database, a computer program was designed and developed, which was embedded in the electronic medical record to intervene the clinical use of the special-grade antimicrobial since 2015. The basic information of all discharged patients from the Second Affiliated Hospital of Zhejiang University School of Medicine from January 2013 to December 2020 were extracted from the HIS system, including the medical orders for antibiotics and the drug storehouse dispensing data.The trend analysis was carried out on the changes of the use rates and antibiotic use density (AUD) of the special-grade antimicrobials in the whole hospital and intensive care units (ICU). The data were processed and analyzed using SPSS 24.0.Results:From 2013 to 2015, except for meropenem and amphotericin B, the usage rate of all special-grade antimicrobials in the whole hospital showed an upward trend ( P<0.05). The proportion of special-grade antimicrobials used in the hospital increased year by year ( χ2=7 804.081, P<0.01). The total usage rate of special-grade antimicrobials in ICU showed an upward trend year by year ( χ2=67.028, P<0.01). Since the implementation of the special-grade antimicrobial information management system in 2015, the total use rate of special-grade antimicrobials in the hospital, the use rate of various antibiotics except linezolid, amphotericin B and posaconazole, and the proportion of special-grade antimicrobials used in the hospital have all shown a downward trend year by year ( P<0.01). The total usage rate and total AUD of special-grade antimicrobials in ICU showed a decreasing trend year by year ( χ2=343.514, P<0.01, β=-0.963, P=0.002). Conclusion:The information management system for special-grade antimicrobial can effectively reduce the utilization rate and AUD of most special-grade antibiotics in hospitalized patients including ICU, and has a good clinical application value in antimicrobial stewardship.

Exploration of Standard Management of High-value Medical Consumables under Global Budget System

Abstract:Objective: To standardize the use and management of high-value medical consumables and relieve patient burden and to adapt to the payment reform of global budget of medical insurance. Methods: Information technology was applied to establish the supply chain and material management platform integrating suppliers, clinical departments, material management departments and related medical insurance departments and price departments. The platform was also integrated with the HIS billing system to monitor and manage the use of high-value medical consumables. The transparency of high-value medical consumables was improved by auditing access of high-valuable consumables, adopting public bidding approach, promoting joint supervision of the health insurance department and price department, implementing the informed consent mechanism and applying bar code technologies. Results: 1/3 of duplicative consumable varieties were cut off the supply chain and the bidding price was reduced by 12%. Patients were able to choose medical consumables according to their own conditions, thus protecting their interests and benefits.

Abstract:Objective To reduce the using risk of medical consumables. Methods This paper analyzed the management risks of high value consumables through the analytic hierarchy process, and determined the weights. Meanwhile, the methods and techniques of bar code, health technology assessment and clinical diagnosis and treatment path management were adopted to implement risk management. Results The weight of risk factors for medical consumables management was obtained, and various countermeasures were developed to reduce the risk. Conclusion The safety and effectiveness of consumables can be guaranteed by controlling the effective risk of medical consumables.

Abstract:OBJECTIVE To standardize the classification,collection and transportation of the medical wastes in our hospital. METHODS A Manual of Administrative Measures for Medical Wastes was set according to the Regulations on Medical Waste Management stipulated by State Council.Field investigation,supervision,management and adjustment had been carried out.RESULTS The medical waste was strictly collected and separated in clinical departments.The dispensable sharp wastes should be collected in a special container.We adopted medical wastes preservation management in our hospital and records should be made when the staff exchange.The mixed medical wastes have been solved.While transporting the medical wastes,closed vans should be used in order to avoid wastes left on the road.After our measures were put in place good achievement was made.CONCLUSIONS Standardized training on separation,collection,transportation,preservation and settlement of medical wastes should be carried out among medical staff at all levels.The facilities for medical wastes settlement should be improved.Investigation,supervision and adjustment in this regard should be strengthened.

Application and Evaluation of SPD Based Logistics Management Model for Medical Consumables in Clinical Nursing Departments

Abstract:Background: Supply, processing, and distribution (SPD) model is sparingly used in hospitals in China. We evaluated its effects on the management efficiency, quality control, and operating costs of medical consumables (MCs) in the clinical nursing surroundings in a single Chinese hospital-Anhui Provincial Hospital from 2014 to 2015. Methods: Amount-based packages (ABP) and procedure-based packages (PBP) models were created. They were introduced the use of quick response (QR) code scanning for using in clinical nursing departments (CNDs). Questionnaires were prepared by referring to previous literature and using Delphi method repeatedly, further discussed and formalized. Partial results of the formal questionnaire were analyzed using SPSS. Results: Frequency of MCs claims reduced without any requirements of MCs in 70% of CNDs. Average time spent on the inventory per week decreased and the time required to procure MCs reduced. Moreover, the average satisfaction score with MCs management increased, reaching 100%. Average space occupied by MCs decreased significantly, reducing by 1.2444m3. Overall, 100% of the respondents concluded that the management of MCs improved effectively and the inventory turnover rate had accelerated. The cost of MCs decreased by 15% with more than 10% increase in in-hospital amount, and the average daily cost of MCs also showed decrease. Conclusion: SPD can improve the efficiency of MCs management in CNDs, reducing medical risks and disputes, saving hospital operating costs, and decreasing capital occupation.
