Abstract:BACKGROUND AND PURPOSE: MR imaging provides critical information about fetal brain growth and development. Currently, morphologic analysis primarily relies on manual segmentation, which is time-intensive and has limited repeatability. This work aimed to develop a deep learning?based automatic fetal brain segmentation method that provides improved accuracy and robustness compared with atlas-based methods. MATERIALS AND METHODS: A total of 106 fetal MR imaging studies were acquired prospectively from fetuses between 23 and 39?weeks of gestation. We trained a deep learning model on the MR imaging scans of 65 healthy fetuses and compared its performance with a 4D atlas-based segmentation method using the Wilcoxon signed-rank test. The trained model was also evaluated on data from 41 fetuses diagnosed with congenital heart disease. RESULTS: The proposed method showed high consistency with the manual segmentation, with an average Dice score of 0.897. It also demonstrated significantly improved performance (P?<?.001) based on the Dice score and 95% Hausdorff distance in all brain regions compared with the atlas-based method. The performance of the proposed method was consistent across gestational ages. The segmentations of the brains of fetuses with high-risk congenital heart disease were also highly consistent with the manual segmentation, though the Dice score was 7% lower than that of healthy fetuses. CONCLUSIONS: The proposed deep learning method provides an efficient and reliable approach for fetal brain segmentation, which outperformed segmentation based on a 4D atlas and has been used in clinical and research settings.

Abstract:Super-resolution techniques provide a route to studying fine scale anatomical detail using multiple lower resolution acquisitions. In particular, techniques that do not depend on regular sampling can be used in medical imaging situations where imaging time and resolution are limited by subject motion. We investigate in this work the use of a super-resolution technique for anisotropic fetal brain MR data reconstruction without modifying the data acquisition protocol. The approach, which consists of iterative motion correction and high resolution image estimation, is compared with a previously used scattered data interpolation-based reconstruction method. To optimize acquisition time, an evaluation of the influence of the number of input images and image noise is also performed. Evaluation on simulated MR images and real data show significant improvements in performance provided by the super-resolution approach.

Abstract:Super-resolution reconstruction (SRR) of fetal brain MRI from motion-corrupted thick-slice stacks can provide high-resolution isotropic 3Dimages that are vital for prenatal examination and quantification of brain development. Existing fetal brain SRRmethods generally rely on a two-stage optimization procedure by performing rigid slice-to-volume registration and volumetric reconstruction in an alternating manner. Despite their advantages, these methods have not considered additional guidance from external anatomical priors, resulting in unsatisfactory performance in various challenging cases. To address this issue, we propose a novel Prior Anatomical Knowledge guided fetal brain Super-Resolution Reconstruction method, namely PAK-SRR. In PAK-SRR, we consider two key kinds of prior anatomical information. First, we integrate the anatomical prior provided by tissue segmentation into both the slice-tovolume registration and volumetric reconstruction to enforce registration consistency on boundaries, effectively alleviating misregistration caused by blurry tissue boundaries of brain. Second, to enrich the structural details of the reconstructed images, we further employ longitudinal fetal brain atlases to guide volumetric reconstruction. Extensive experiments on multi-site clinical datasets demonstrate that our PAK-SRR significantly outperforms the state-of-the-art SRR methods for fetal brain MRI, quantitatively and qualitatively. Abstract:High-resolution volume reconstruction from multiple motion-corrupted stacks of 2D slices plays an increasing role for fetal brain Magnetic Resonance Imaging (MRI) studies. Currently existing reconstruction methods are time-consuming and often require user interactions to localize and extract the brain from several stacks of 2D slices. We propose a fully automatic framework for fetal brain reconstruction that consists of four stages: 1) fetal brain localization based on a coarse segmentation by a Convolutional Neural Network (CNN), 2) fine segmentation by another CNN trained with a multi-scale loss function, 3) novel, single-parameter outlier-robust super-resolution reconstruction, and 4) fast and automatic high-resolution visualization in standard anatomical space suitable for pathological brains. We validated our framework with images from fetuses with normal brains and with variable degrees of ventriculomegaly associated with open spina bifida, a congenital malformation affecting also the brain. Experiments show that each step of our proposed pipeline outperforms state-of-the-art methods in both segmentation and reconstruction comparisons including expert-reader quality assessments. The reconstruction results of our proposed method compare favorably with those obtained by manual, labor-intensive brain segmentation, which unlocks the potential use of automatic fetal brain reconstruction studies in clinical practice.

Abstract:Magnetic resonance images of neonates, compared with toddlers, exhibit lower signal-to-noise ratio and spatial resolution. In this paper, we propose a novel method for super-resolution reconstruction of neonate images with the help of toddler images, using residual-structured sparse representation with convex regularization. Specifically, we introduce a two-layer image representation, consisting of a base layer and a detail layer, to cater to signal variation across scanners and sites. The base layer consists of the smoothed version of the image obtained via Gaussian filtering. The detail layer is the difference between the original image and the base layer. High-frequency details in the detail layer are borrowed across subjects for super-resolution reconstruction. Experimental results on T1 and T2 images demonstrate that the proposed algorithm can recover fine anatomical structures, and generally outperform the state-of-the-art methods both qualitatively and quantitatively.

Abstract:Superresolution T2-weighted fetal-brain magnetic-resonance imaging (FBMRI) traditionally relies on the availability of several orthogonal low-resolution series of 2-dimensional thick slices (volumes). In practice, only a few low-resolution volumes are acquired. Thus, optimization-based image-reconstruction methods require strong regularization using hand-crafted regularizers (e.g., TV). Yet, due to in utero fetal motion and the rapidly changing fetal brain anatomy, the acquisition of the high-resolution images that are required to train supervised learning methods is difficult. In this paper, we sidestep this difficulty by providing a proof of concept of a self-supervised single-volume superresolution framework for T2-weighted FBMRI (SAIR). We validate SAIR quantitatively in a motion-free simulated environment. Our results for different noise levels and resolution ratios suggest that SAIR is comparable to multiple-volume superresolution reconstruction methods. We also evaluate SAIR qualitatively on clinical FBMRI data. The results suggest SAIR could be incorporated into current reconstruction pipelines.

Application of Automatic Segmentation on Super-Resolution Reconstruction MR Images of the Abnormal Fetal Brain

Abstract:Background and purpose: Fetal brain MR imaging is clinically used to characterize fetal brain abnormalities. Recently, algorithms have been proposed to reconstruct high-resolution 3D fetal brain volumes from 2D slices. By means of these reconstructions, convolutional neural networks have been developed for automatic image segmentation to avoid labor-intensive manual annotations, usually trained on data of normal fetal brains. Herein, we tested the performance of an algorithm specifically developed for segmentation of abnormal fetal brains. Materials and methods: This was a single-center retrospective study on MR images of 16 fetuses with severe CNS anomalies (gestation, 21-39 weeks). T2-weighted 2D slices were converted to 3D volumes using a super-resolution reconstruction algorithm. The acquired volumetric data were then processed by a novel convolutional neural network to perform segmentations of white matter and the ventricular system and cerebellum. These were compared with manual segmentation using the Dice coefficient, Hausdorff distance (95th percentile), and volume difference. Using interquartile ranges, we identified outliers of these metrics and further analyzed them in detail. Results: The mean Dice coefficient was 96.2%, 93.7%, and 94.7% for white matter and the ventricular system and cerebellum, respectively. The Hausdorff distance was 1.1, 2.3, and 1.6 mm, respectively. The volume difference was 1.6, 1.4, and 0.3 mL, respectively. Of the 126 measurements, there were 16 outliers among 5 fetuses, discussed on a case-by-case basis. Conclusions: Our novel segmentation algorithm obtained excellent results on MR images of fetuses with severe brain abnormalities. Analysis of the outliers shows the need to include pathologies underrepresented in the current data set. Quality control to prevent occasional errors is still needed.

Abstract:The detailed sonographic assessment of the fetal neuroanatomy plays a crucial role in prenatal diagnosis, providing valuable insights into timely, well-coordinated fetal brain development and detecting even subtle anomalies that may impact neurodevelopmental outcomes. With recent advancements in artificial intelligence (AI) in general and medical imaging in particular, there has been growing interest in leveraging AI techniques to enhance the accuracy, efficiency, and clinical utility of fetal neurosonography. The paramount objective of this focusing review is to discuss the latest developments in AI applications in this field, focusing on image analysis, the automation of measurements, prediction models of neurodevelopmental outcomes, visualization techniques, and their integration into clinical routine.

Abstract:In this Technical note, the authors describe a how do‐we‐do it 3D ultrasound technique in the examination of the fetal face, either in normal as in pathological cases, using a conventional 3D surface application compared with the use of artificial intelligence (AI) platform. Legal considerations are also discussed. The use of artificial intelligence (AI) platforms is revolutionizing the performance in managing metadata and big data. Medicine is another field where AI is spreading. However, this technological advancement is not amenable to errors or fraudulent misconducts. International organization and recently the European Union have released principles and recommendations for an appropriate use of AI in healthcare. In prenatal ultrasound diagnosis, the use of AI in daily practice is having a revolutionary impact. Notwithstanding, the diagnostic enhancement should be regulated, and AI applications should be developed to guarantee correct imaging acquisition and further postprocessing.

Abstract:Objective: We aimed to compare three-dimensional (3D) fetal malformation images obtained using ultrasonography (US) and magnetic resonance imaging (MRI) on the same day during the third trimester of pregnancy. Methods: Total 33 fetuses were selected from cases evaluated for malformations. Morphological abnormalities were first scanned using 3DUS. MRI was used to confirm the previous preliminary 3DUS findings, and diagnoses were confirmed postnatally. 3DUS scans were performed transabdominally using an Rab (4-8 L) probe, Voluson 730 Pro/Expert and E8 (General Electric, Healthcare, Zipf, Austria). MRI was performed using a 1.5-T scanner (Magneton Avanto, Siemens, Erlangen, Germany) with a body coil. The 3D reconstruction of the structure of interest was manually performed from a True FISP sequence using an interactive pen tablet (Syngo multimodality 2009B, Siemens, Erlangen, Germany). Results: Despite recent advancements in 3DUS, the quality of 3D images obtained from MRI was superior during the third trimester. 3DUS had certain limitations, such as being influenced by the fetal position, the volume of amniotic fluid, and maternal obesity. Fetal movements during image acquisition were one of the main challenges for MRI. Conclusion: The quality of the 3D images obtained using MRI was superior to that of images obtained using US during the third trimester of pregnancy.
