Abstract:BACKGROUND AND PURPOSE: MR imaging provides critical information about fetal brain growth and development. Currently, morphologic analysis primarily relies on manual segmentation, which is time-intensive and has limited repeatability. This work aimed to develop a deep learning?based automatic fetal brain segmentation method that provides improved accuracy and robustness compared with atlas-based methods. MATERIALS AND METHODS: A total of 106 fetal MR imaging studies were acquired prospectively from fetuses between 23 and 39?weeks of gestation. We trained a deep learning model on the MR imaging scans of 65 healthy fetuses and compared its performance with a 4D atlas-based segmentation method using the Wilcoxon signed-rank test. The trained model was also evaluated on data from 41 fetuses diagnosed with congenital heart disease. RESULTS: The proposed method showed high consistency with the manual segmentation, with an average Dice score of 0.897. It also demonstrated significantly improved performance (P?<?.001) based on the Dice score and 95% Hausdorff distance in all brain regions compared with the atlas-based method. The performance of the proposed method was consistent across gestational ages. The segmentations of the brains of fetuses with high-risk congenital heart disease were also highly consistent with the manual segmentation, though the Dice score was 7% lower than that of healthy fetuses. CONCLUSIONS: The proposed deep learning method provides an efficient and reliable approach for fetal brain segmentation, which outperformed segmentation based on a 4D atlas and has been used in clinical and research settings.
Automatic facial axes standardization of 3D fetal ultrasound images

Abstract:Craniofacial anomalies indicate early developmental disturbances and are usually linked to many genetic syndromes. Early diagnosis is critical, yet ultrasound (US) examinations often fail to identify these features. This study presents an AI-driven tool to assist clinicians in standardizing fetal facial axes/planes in 3D US, reducing sonographer workload and facilitating the facial evaluation. Our network, structured into three blocks-feature extractor, rotation and translation regression, and spatial transformer-processes three orthogonal 2D slices to estimate the necessary transformations for standardizing the facial planes in the 3D US. These transformations are applied to the original 3D US using a differentiable module (the spatial transformer block), yielding a standardized 3D US and the corresponding 2D facial standard planes. The dataset used consists of 1180 fetal facial 3D US images acquired between weeks 20 and 35 of gestation. Results show that our network considerably reduces inter-observer rotation variability in the test set, with a mean geodesic angle difference of 14.12$^{\circ}$ $\pm$ 18.27$^{\circ}$ and an Euclidean angle error of 7.45$^{\circ}$ $\pm$ 14.88$^{\circ}$. These findings demonstrate the network's ability to effectively standardize facial axes, crucial for consistent fetal facial assessments. In conclusion, the proposed network demonstrates potential for improving the consistency and accuracy of fetal facial assessments in clinical settings, facilitating early evaluation of craniofacial anomalies.

Reconstruction of the fetus face from three-dimensional ultrasound using a newborn face statistical shape model

Abstract:Background and objective: The fetal face is an essential source of information in the assessment of congenital malformations and neurological anomalies. Disturbance in early stages of development can lead to a wide range of effects, from subtle changes in facial and neurological features to characteristic facial shapes observed in craniofacial syndromes. Three-dimensional ultrasound (3D US) can provide more detailed information about the facial morphology of the fetus than the conventional 2D US, but its use for pre-natal diagnosis is challenging due to imaging noise, fetal movements, limited field-of-view, low soft-tissue contrast, and occlusions. Methods: In this paper, we propose the use of a novel statistical morphable model of newborn faces, the BabyFM, for fetal face reconstruction from 3D US images. We test the feasibility of using newborn statistics to accurately reconstruct fetal faces by fitting the regularized morphable model to the noisy 3D US images. Results: The results indicate that the reconstructions are quite accurate in the central-face and less reliable in the lateral regions (mean point-to-surface error of 2.35 mm vs 4.86 mm). The algorithm is able to reconstruct the whole facial morphology of babies from US scans while handle adverse conditions (e.g. missing parts, noisy data). Conclusions: The proposed algorithm has the potential to aid in-utero diagnosis for conditions that involve facial dysmorphology.

Abstract:At present, prenatal ultrasound is one of the important means for screening fetal malformations. In the process of prenatal ultrasound diagnosis, the accurate recognition of fetal facial ultrasound standard plane is crucial for facial malformation detection and disease screening. Due to the dense distribution of fetal facial images, no obvious structure contour boundary, small structure area, and large area overlap in the middle of the structure detection frame, this paper regards the fetal facial standard plane and its structure recognition as a universal target detection task for the first time, and applies real‐time YOLO v5s to the fetal facial ultrasound standard plane structure detection and classification task. First, we detect the structure of a single slice, and take the structure of a slice class as the recognition object. Second, we carry out structural detection experiments on three standard planes; then, on the basis of the previous stage, the images of all parts included in the ultrasound examination of multiple fetuses were collected. In the single‐class structure detection experiment and the structure detection and classification experiment of three types of standard planes, the overall recognition effect of Precision and Recall index data is better, with Precision being 98.3% and 98.1%, and Recall being 99.3% and 98.2%, respectively. The experimental results show that the model has the ability to identify fetal facial anatomy and standard sections in different data, which can help the physician to automatically and quickly screen out the standard sections of each fetal facial ultrasound.

Abstract:3D ultrasound imaging of fetal faces has been predominantly confined to qualitative assessment. Many genetic conditions evade diagnosis and identification could assist with parental counselling, pregnancy management and neonatal care planning. We describe a methodology to build a shape model of the third trimester fetal face from 3D ultrasound and show how it can objectively describe morphological features and gestational-age related changes of normal fetal faces. 135 fetal face 3D ultrasound volumes (117 appropriately grown, 18 growth-restricted) of 24-34 weeks gestation were included. A 3D surface model of each face was obtained using a semi-automatic segmentation workflow. Size normalisation and rescaling was performed using a growth model giving the average size at every gestation. The model demonstrated a similar growth rate to standard head circumference reference charts. A landmark-free morphometry model was estimated to characterize shape differences using non-linear deformations of an idealized template face. Advancing gestation is associated with widening/fullness of the cheeks, contraction of the chin and deepening of the eyes. Fetal growth restriction is associated with a smaller average facial size but no morphological differences. This model may eventually be used as a reference to assist in the prenatal diagnosis of congenital anomalies with characteristic facial dysmorphisms.

Abstract:During pregnancy, ultrasound examination in the second trimester can assess fetal size according to standardized charts. To achieve a reproducible and accurate measurement, a sonographer needs to identify three standard 2D planes of the fetal anatomy (head, abdomen, femur) and manually mark the key anatomical landmarks on the image for accurate biometry and fetal weight estimation. This can be a time-consuming operator-dependent task, especially for a trainee sonographer. Computer-assisted techniques can help in automating the fetal biometry computation process. In this paper, we present a unified automated framework for estimating all measurements needed for the fetal weight assessment. The proposed framework semantically segments the key fetal anatomies using state-of-the-art segmentation models, followed by region fitting and scale recovery for the biometry estimation. We present an ablation study of segmentation algorithms to show their robustness through 4-fold cross-validation on a dataset of 349 ultrasound standard plane images from 42 pregnancies. Moreover, we show that the network with the best segmentation performance tends to be more accurate for biometry estimation. Furthermore, we demonstrate that the error between clinically measured and predicted fetal biometry is lower than the permissible error during routine clinical measurements.

Abstract:Over the past decade, deep-learning (DL) algorithms have become a promising tool to aid clinicians in identifying fetal head standard planes (FHSPs) during ultrasound (US) examination. However, the adoption of these algorithms in clinical settings is still hindered by the lack of large annotated datasets. To overcome this barrier, we introduce FetalBrainAwareNet, an innovative framework designed to synthetize anatomically accurate images of FHSPs. FetalBrainAwareNet introduces a cutting-edge approach that utilizes class activation maps as a prior in its conditional adversarial training process. This approach fosters the presence of the specific anatomical landmarks in the synthetized images. Additionally, we investigate specialized regularization terms within the adversarial training loss function to control the morphology of the fetal skull and foster the differentiation between the standard planes, ensuring that the synthetic images faithfully represent real US scans in both structure and overall appearance. The versatility of our FetalBrainAwareNet framework is highlighted by its ability to generate high-quality images of three predominant FHSPs using a singular, integrated framework. Quantitative (Fréchet inception distance of 88.52) and qualitative (t-SNE) results suggest that our framework generates US images with greater variability compared to state-of-the-art methods. By using the synthetic images generated with our framework, we increase the accuracy of FHSP classifiers by 3.2% compared to training the same classifiers solely with real acquisitions. These achievements suggest that using our synthetic images to increase the training set could provide benefits to enhance the performance of DL algorithms for FHSPs classification that could be integrated in real clinical scenarios.

Abstract:Abstract Objective. This work investigates the use of deep convolutional neural networks (CNN) to automatically perform measurements of fetal body parts, including head circumference, biparietal diameter, abdominal circumference and femur length, and to estimate gestational age and fetal weight using fetal ultrasound videos. Approach. We developed a novel multi-task CNN-based spatio-temporal fetal US feature extraction and standard plane detection algorithm (called FUVAI) and evaluated the method on 50 freehand fetal US video scans. We compared FUVAI fetal biometric measurements with measurements made by five experienced sonographers at two time points separated by at least two weeks. Intra- and inter-observer variabilities were estimated. Main results. We found that automated fetal biometric measurements obtained by FUVAI were comparable to the measurements performed by experienced sonographers The observed differences in measurement values were within the range of inter- and intra-observer variability. Moreover, analysis has shown that these differences were not statistically significant when comparing any individual medical expert to our model. Significance. We argue that FUVAI has the potential to assist sonographers who perform fetal biometric measurements in clinical settings by providing them with suggestions regarding the best measuring frames, along with automated measurements. Moreover, FUVAI is able perform these tasks in just a few seconds, which is a huge difference compared to the average of six minutes taken by sonographers. This is significant, given the shortage of medical experts capable of interpreting fetal ultrasound images in numerous countries.

Abstract:In this Technical note, the authors describe a how do‐we‐do it 3D ultrasound technique in the examination of the fetal face, either in normal as in pathological cases, using a conventional 3D surface application compared with the use of artificial intelligence (AI) platform. Legal considerations are also discussed. The use of artificial intelligence (AI) platforms is revolutionizing the performance in managing metadata and big data. Medicine is another field where AI is spreading. However, this technological advancement is not amenable to errors or fraudulent misconducts. International organization and recently the European Union have released principles and recommendations for an appropriate use of AI in healthcare. In prenatal ultrasound diagnosis, the use of AI in daily practice is having a revolutionary impact. Notwithstanding, the diagnostic enhancement should be regulated, and AI applications should be developed to guarantee correct imaging acquisition and further postprocessing.

Agent Based Fetal Face Segmentation for Standard Plane Localization in 3D Ultrasound

Abstract:In practice, fetal 3D ultrasound can have difficulty in accurately detecting labels for auxiliary standard cut plane localization because of mass loss. Therefore, in this paper, we propose a new segmentation-based reinforcement learning framework for automatically localizing the standard plane of the face: in 3D fetal ultrasound, the initial plane is localized based on anatomical landmarks of mass and geometric relationships, agents navigate through visual segmentation to automatically localize the standard plane, and bound ultrasound views are presented to show the resultant plane. This study was extensively validated on an in-house large dataset. The accuracy of this automatic localization of 3D ultrasound standard planes with sonographer-calibrated median sagittal views of the face, horizontal transverse views of both eyeballs, and coronal views of the nasolabial was 6.64 °/5.65mm, 7.04°/3.58mm, and 5.14 ° 14.26mm, respectively, with success rates of 66.67 %, 78.38 %, and 80.41 %, respectively. The experimental results verify that this system can effectively improve navigation performance.
