Abstract:We report a patient with atrioventricular reentrant tachycardia (AVRT) with bidirectional conduction over an anteroseptal accessory pathway (AP) who underwent successful ablation in the non-coronary aortic sinus (AS). In three previous attempts, the intracardiac recordings showed an anteroseptal, AP with antegrade and retrograde conduction that failed to be ablated in spite of radiofrequency (RF) applications from the right and left anteroseptal regions. During the study, the earliest atrial. activation during tachycardia was recorded in the non-coronary AS preceding the atrial activation at the His bundle (HB) region by 24 ms, and the anteroseptal AP was successfully blocked by one single ablation in the non-coronary AS. These data strongly suggest that careful mapping of an anteroseptal. AP in the non-coronary AS may provide an alternative ablation approach in patients with previously failed ablation.
The changes of electrophysiological characteristics in atrioventricular accessory pathways induced by incomplete ablation

Abstract:Objective Radio-frequency ablation of common atrioventricular accessory pathways may result in slow accessory pathways, and subsequently occurrence of permanent junctional reciprocating tachycardia(PJRT).The purpose of the study was to analyze this phenomenon. Method Six cases of tachycardia with above phenomenon were analyzed by the following features: characteristics of tachycaridia, target graph, decrement conduction and distribution of slow accessory pathways.Result All 6 slow accessory pathways resulted from non-thorough radio-frequency ablation and had the features of decrement conduction, VA non-fusion in thetarget graph [retro-conductive time was 60-160 (104.8±38.3)ms and A wave appeared 9-20 (15.3±4.9)ms earlier than the earliest control point].Conclusions Except congenital origin, slow accessory pathways can be resulted from non-thorough radio-frequency ablation.Therefore, slow accessory pathways can be classified into two subtypes: congenital and acquired according to the origin.
Ablation of Atrial Tachycardia Originating from the Noncoronary Sinus: Case Report and Literature Review

Abstract:This report describes a patient with atrial tachycardia who underwent radiofrequency catheter ablation. Two applications of radiofrequency energy were received at different sites within 1 week. Initially, right atrium mapping indicated that the earliest activation site was located at the His-bundle region (HBR). However, the first application of radiofrequency near HBR appeared to be ineffective. Moreover, a degree III atrioventricular block and inferior ST-segment elevation was temporally observed during the therapy, which recovered after drug treatment. As later aortic sinus mapping showed that the activation site located at the noncoronary sinus (NCS) preceded the one at HBR, and this activation terminated at atrial tachycardia, a second application of radiofrequency was performed at NCS without any complications. In addition, a literature review of the clinical features of atrial tachycardia at the NCS is also summarized in the present report.

Abstract:OBJECTIVE: To investigate the correlation between RFCA catheter cumulative energy and autonomic nerve injury. METHODS: Forty-one patients with paroxysmal supraventricular tachycardia were enrolled, Patients were excluded if they had Diabetes, Hypertension, Congestive Heart Failure or other organic heart disease. HRV and biochemical markers were measured before and after the RFCA. RESULTS: Compared with pre-ablation values,there was significantly decrease in post-ablation low frequency (LF) and high frequency (HF). This was noted in both the septal group (AVNRT and septal pathway) and free wall group (free wall accessory pathway).Post-procedure,the sensitivity of cardiac troponin I(cTnI) for myocardial injury detection was 58.3%, AST was 41.7%. This was significantly higher than other markers(CK:4.2%, CK-MB:10.4%, LDH:20.8%). The post-ablation sensitivity of cTnI was 54.2%, 6.3% and 52.1%at 1 hour, 12 hours, and 24 hours respectively. A significant correlation between cumulative energy and delta HF(r=0.688,P=0.01) or delta LF (r=0.462, P<0.05).was noted in free wall group.(delta HF=pre-ablation HF-post-ablation HF/pre-ablation HF x 100%). There was no significant correlation between biochemical markers and either delta HF or delta LF. CONCLUSION: RFCA induced injury on cardiac autonomic nerves related to both cumulative energy and ablation site,but not size of myocardial injury as determined by cTnI measurement. cTnI is an excellent biochemical marker of myocardial injury.
Effects of Radio Frequency Ablation on Non-inducible Left Posterior Fascicle Ventricular Tach-ycardia

Abstract:[Objective] To explore the middle/long‐term effects of radio frequency (RF) ablation on non‐inducible left posterior fascicle ventricular tachycardia .[Methods] A total of 27 patients with recurrent tachy‐cardia were admitted into our department from 2003~2012 .There were 16 males and 11 females .On the re‐cording of electrocardiogram (ECG ) , tachycardia referred to left posterior fascicle ventricular tachycardia (right bundle branch block morphology & left axis deviation ) .The symptoms occurred at an average of 8 ± 5 episodes .And structural abnormality was excluded .Electrophysiologic evaluations were performed after in‐formed consents upon a withdrawal of antiarrhythmic drugs .Left ventricle endocardial mapping during sinus rhythm revealed Purkinje potential (PP) and diastolic potential (DP) .Cross linear ablation was applied per‐pendicular and parallel to left posterior fascicle at the earliest DP .When DP mapping was unsuccessful ,cross linear ablation was applied perpendicular and parallel to left posterior fascicle at middle‐poster septum .A cath‐eter was employed for recording PP .Ablation was stimulated for non‐induced tachycardia .[Results]DP map‐ping was successful in 22 patients .During a follow‐up period of 27 ± 8 months ,4 patients had a recurrence of tachycardia without any other complication .The success rate was 85 .2% during one single procedure .[Con‐clusion]During sinus rhythm ,ablation is both effective and safe for non‐inducible patients under the guidance of DP and PP .

Abstract:A 73-year-old man, who was admitted to our hospital with idiopathic ventricular tachycardia, underwent radiofrequency catheter ablation(RFCA) without any immediate signs of complications. He complained of sudden onset of chest pain shortly after the RFCA procedure. The diagnosis of PCIS was supported by slight fever, pericardial effusion, bilateral pleural effusion and elevated inflammatory markers. One should be aware of PCIS since there will be increasing performance of RFCA procedures and patients may present with this previously rare postoperative complications.
Acute Injury of Radiofrequency Catheter Ablation to the Heart and Autonomic Nerves

Abstract:ObjectiveTo study the acute injury of Radiofrequency Catheter Ablation (RFCA) to the heart and it's autonomic nerves. MethodsSuperoxide Dismutase(SOD) and Heart Rate Variation(HRV) were measured in 120 cases with paroxysmal supraventricular tachycardia (PSVT) pre and post RFCA. ResultsSOD post RFCA increased obviously than that of pre-operation(881.68±328.16) vs (319.77±176.34)ng/mL, P0.01). HRV includes Very Low Frequency (VLF), Low Frequency(LF), High frequency(HF), R-R interval, root mean standard deviation of successive (rMSSD)and the percentage of NN interval deviation were greater than 50ms (PNN50). Changes were significant between pre and post RFCA operation(P0.05,0.01,0.01,0.05,0.05, irrespectively).But HRV changes were not evidently related to pre and post RFCA operation with SOD change .ConclusionRFCA operation can cause injury to the heart and it's autonomic nerves. HRV decrement post RFCA was resulted from acute injury of Cardiac vagus by radiofrequency current but not resulted from injury of myocardium. 
[Effects of Short-Term Rapid Atrial Pacing on Electrophysiological Characteristics of Atrium in Hyperthyroidism].

Abstract:OBJECTIVE:To investigate the effects of short-term rapid atrial pacing on the electrophysiological characteristics of atrium in hyperthyroidism.METHODS:Forty-six adult rabbits were randomly divided into 4 groups: normal control group (n=10), pacing group (n=10), hyperthyroidism group (n=14), hyperthyroidism/pacing group (n=12). Baseline AERP and AERPs after pacing 2, 4, 6 h were determined in all groups at driver cycle length (DCL) of 200 ms, 150 ms and 130 ms.RESULT:In pacing group, AERPs at different DCL (200 ms, 150 ms and 130 ms) were shortened after rapid pacing 2, 4, 6 h when compared with before pacing and control group (P<0.01). AERPs (at DCL of 200 ms, 150 ms and 130 ms) in hyperthyroidism group were shorter than those in control group at all time points (P<0.01). AERPs (at DCL of 200 ms, 150 ms and 130 ms) in hyperthyroidism/pacing group after rapid pacing 2, 4, 6 h were shorter than those in pacing 0 h (P<0.01) and hyperthyroidism group (P<0.05). AERP200-150 and AERP200-130 in pacing group after rapid pacing 2, 4, 6 h were significantly different from at pacing 0 h and control group (P<0.01). AERP200-150 and AERP200-130 in hyperthyroidism and hyperthyroidism/pacing group were significantly different from control group at all time points (P<0.01). No differences were observed in AERP200-150 and AERP200-130 between hyperthyroidism group and hyperthyroidism/pacing group.CONCLUSION:Hyperthyroidism and short-term atrial pacing in the presence of hyperthyroidism can lead to remodeling of atrial electrophysiology.

Radiofrequency current catheter ablation in 561 cases with tachyarrhythimas

Abstract:Five hundred and sixty one cases with tachyarrhythmias were treated by radiofrequency current (RFC) catheter ablation. The patient population consisted of 273 males and 288 females. There were 413 patients with 429 atrioventricular (AV) accessory pathways (APs), including 343 left-sided APs, 86 right-sided APs and 4 APs with slow, decremental and retrograde-only conduction properties. One hundred and forty two cases had AV nodal reentrant tachycardias (AVNRT), there were 3 cases with atrial tachycardia (AT), 2 cases with atrial flutter (AF) and one case with ventricular tachycardia (VT) from right ventricular outflow tract (RVOT) and without organic heart disease. The catheter in coronary sinus was used as a landmark of AP mapping. The ablation catheter was manipulated to map the target site of ablation accurately. The characteristics of local electrocardiogram at the ablation target site of overt APs were short AV interval (�� 40ms), long V-delta interval (�� 20ms) and A/V ratio < 1. When concealed APs were ablated, AP mapping was performed during ventricular pacing or inducdd atrioventricular reentrant tachycardias (AVRT). The characteristics of the local electrocardiogram were short VA interval (��40ms) and A/V ratio < 1. AP potential might also be of value in predicting successful ablation. Transseptal approach was the alternative procedure for left-sided APs with anterior location, or complicated aortic stenosis and patients with mitral stenosis in whom the balloon valvuplasty was scheduled in the same section. The procedure was applied in 6 cases and all were successful. AV nodal modification. Slow pathway ablation was attempted by either inferior or posterior approach. The advantages and disadvantages of both were evaluated. The successful rate of AP (the number of APs was counted), AVNRT, AT, AF and VT ablations were 98%, 98%, 3/3, 2/2, 1/1, respectively. Complete AV block occurred in 2 patients (0.36%) requiring permanent pacemaker implantation. There were 14 recurrences (2.6%,), all of which were re-ablated successfully.
Reversible Tachycardia Mediated Cardiomyopathy after Radiofrequency Ablation of Idiopathic Left Ventricular Tachycardia

Abstract:A 20-year-old girl was admitted to our hospital with a chief complain of 12-days of palpitation and progressive shortness of breath. The symptoms of heart failure were consistent with New York Heart Association (NYHA) grade III. She had fever and cold symptoms 2-days before the onset of palpitation and heart failure symptoms but recovered soon, without other significant medical history. Physical examination demonstrates a heart rate of 170 beats per minute (bpm), a blood pressure of 70/40 mm Hg and bibasilar rales. ECG revealed a relatively narrow QRS tachycardia of 170 bpm with right bundle branch block (RBBB) morphology and a superior left axis configuration (Fig. 1a ). At the emergency department, empirical administration of intravenous verapamil was implemented and sinus rhythm was restored within 3 min with inverted T wave in leads II, III, aVF and V3–6 (Fig. 1b). A chest X-ray revealed bilateral plural effusion and increased cardiothoracic ratio (Fig. 2a ). Transthoracic echocardiography performed demonstrated marginal left ventricular dysfunction (left ventricular ejection fraction [LVEF] of 50%), with a normal LV end-diastolic diameter (LVEDD) of 38 mm and cardiac effusion with a fluid sonolucent area of 6 mm. Electrophysiological study performed in the following day after administration induced narrow QRS tachycardia which did not replicate the clinical morphology and indicated atrio-ventricular tachycardia via left atrio-ventricular bypass (Fig. 3a ), while the tachycardia of clinical morphology was not induced. The atrio-ventricular bypass was successfully ablated and the pre-systolic Purkinje potentials (Fig. 3b) were targeted, resulting in a new Q wave in inferior leads of ECG which mimics left posterior fascicular block after ablation (Fig. 1c). Chest X-ray performed in the 2nd day (Fig. 2b), the 3rd day (Fig. 2c) and the 40th day (Fig. 2d) revealed decreased and demission of plural effusion and decrease cardiothoracic ratio. Transthoracic echocardiography performed in the 3rd day demonstrated normalization of left ventricular dysfunction (LVEF of 76.6%) and cardiac effusion with a fluid sonolucent area of 6 mm, while in the 40th day the cardiac effusion was diminished. ECG in the 40th day revealed T wave restored upright (Fig. 1d). The patient remained free from arrhythmia for 2 months, without anti-arrhythmic medication.
Clinical Analysis of Radiofrequency Catheter Ablation on Paroxysmal Supraventricular Tachycardia

Abstract:Objective This study was to analyze the outcomes of patients with paroxysmal supravetricu1ar tachycardia after therapeutic radiofrequency catheter ablation for further clinical operating works.Methods Clinical data of 382 patients with different supravetricu1ar tachycardia received radiofrequency ablationtherapy were retrospectively analyzed.Results There were 174 patients with atrioventricular reentrant tachycardia.Among them,there were 113 cases with left accessory pathways,61 cases with right accessory pathways.There were 208 patients with atrioventricular nodal reentrant tachycardia.The overallsuccess rate was 97.4%,in which radiofrequency catheter ablation of paroxysmal supraventricular tachycardia mediated by dual atrioventricular nodal and left accessory pathway had a higher success rate(98.4%).The overall recurrence rate was 2.6% and the incidence of complications was 1.8% after radiofrequency catheter ablation.Conclusion Radiofrequency catheter ablation is an effective and safe therapeutic method with high successful rate,low incidence of complications for patients with paroxysmal supravetricu1ar tachycardia.The patients with right accessory pathway had a higher recurrence rate.
Retrospective Analysis of Radiofrequency Catheter Ablation of Tachycardia Caused by Multiple Accessory Pathways

Abstract:Objective: To summarize electrophysiological characteristics through analyzing radiofrequency catheter ablation of tachycardia caused by multiple accessory pathways in 33 cases. Methods: This study included 33 patients, male 22, female 11, mean age [40.2±9.7(15～68)] years old. Electrophysiological examination and radiofrequency catheter ablation were finished one time. Results: Among 33 patients, double pathways 32 cases (left double pathways 18 cases, right double pathways 4 cases, bilateral pathways 10 cases); triple pathways 1 case (left pathway and right double pathways); 33 cases of all patients with 67 pathways were ablated successfully 40 times respectively after final diagnosis of tachycardia caused by multiple through electrophysiological examinations; rate of success was 100 percent; tachycardia with different shape and different frequency were evoked in 15 cases; Tachycardia with two different frequency in 6 cases was mapped and ablated respectively; another pathway in 5 cases, which was again confirmed after one pathway was ablated, was ablated; 6 cases of double pathways and 1 case of triple pathways had neither evidence of accessory pathway by electrophysiological examination, nor tachycardia excited after the first successfully ablation. However, tachycardia recurred from 1st month to 24th month after operation, confirmed by the electrophysiological reexamination, and ablated successfully again. During 5～72 months of follow-up, no complications was found. Conclusion: Diagnosis of multiple accessory pathways was basis of electrophysiological examination but manipulative complexity of electrophysiological multiple accessory pathways lead to practical difficulty. Bamboo-peeling method of ablation should be adopted. Sequences of multiple pathways ablation are: first, dominant accessory pathways are ablated before dormant accessory pathways done; second, left and right target points are mapped at the same time; third, lateral walls are ablated before septum done.
Very Long-Term Outcome of Catheter Ablation of Post-Incisional Atrial Tachycardia: Role of Incisional and Non-Incisional Scar

Abstract:BACKGROUND:The arrhythmogenicity of right atrial (RA) incisional scar after cardiac surgery could result in atrial tachycardia (AT). Radiofrequency catheter ablation is effective in the treatment of such tachycardia. However, data regarding long-term outcomes are limited.METHODS AND RESULTS:A total of 105 patients with prior RA incision who underwent radiofrequency catheter ablation of AT were included. In the first procedure, electroanatomic mapping (EAM) revealed a total of 139 ATs in 105 patients, including 88 cavotricuspid isthmus dependent atrial flutters (IDAFs), 5 mitral annulus reentrant tachycardias (MARTs), 44 intra-atrial reentrant tachycardias (IARTs) and 2 focal ATs (FATs). AT was successfully eliminated in 101 (96.1%) patients. During a mean follow-up period of 90 ± 36 months, recurrent AT was observed in 23 patients and 21 underwent a second ablation. A total of 23 ATs were identified in redo procedures including 4 IDAFs, 2 MARTs, 12 IARTs and 5 FATs. The time to recurrence was significantly different among various AT types. Acute success was achieved in 20 of 23 redo procedures. Taking a total of 21 patients presenting atrial fibrillation during follow-up into account, 85 patients (81.9%) were in sinus rhythm. No complications except for a case of RA compartmentation occurred.CONCLUSION:RA incisional scar played an essential role in promoting both IDAF and IART, while non-incisional scar contributed to a substantial rate of late recurrent AT in forms of both macroreentry and small reentry. Catheter ablation using EAM system resulted in a high success rate during long-term follow-up.
Participation of a nodoventricular fiber presenting with bidirectional conduction property in the genesis of narrow QRS tachycardia

Abstract:Objective To introduce the electrophysiological characteristics of nodoventricular fiber with bidirectional conduction property and the method of its catheter ablation.Methods The study case,female,34 years old,had the history of recurrent palpitation for more than 7 years.Baseling ECG was essentially normal and ECG during palpitation attack showed a narrow QRS tachycardia.Intravenous bolus injection of verapamil and propafenone could terminate the tachycardia.Physical examination and echocardiography excluded the evidence of structural heart disease.Results Baseline electrophysiological study showed AH interval of 73 ms and HV interval of 42 ms with normal QRS complex in sinus rhythm.The tachycardia could be induced by atrial increamental pacing and had the same morphology as that of sinus rhythm.Endocardial recording demonstrated ventriculoatrial(VA) dissociation during tachycardia and HV interval was idendical to that of sinus rhythm.Sometimes,VA 1:1 conduction was noticed and the earliest atrial activation was in His recording.VA dissociation was observed during right ventricular apical pacing with the cycle lenth of 400 ms.QRS complex was progressively pre-excited with HV interval shortening and H deflection merging into the QRS complex during atrial increamental pacing.The interval between pacing signal and the onset of the QRS complex was concurrently prolonged until the QRS was fully pre-excited.When further decreased the cycle length of atrial pacing,the pre-excited QRS complex was suddenly shifted to normal and narrow QRS taehycardia was initiated after the abrupt increase in the AH interval.ATP could terminate the tachycardia and disclose accessory pathway conduction.When completely pre-excited,the earliest ventricular activation with "QS"unipolar recording was found at 4 o' clock of tricuspid ring( LAO 45° ) and radiofrequency energy delivery successfully blocked the accessory pathway conduction.No junctional rhythm was observed during ablation and AV jump conduction could still be noticed after ablation.No tachycardia occurred after a follow-up of 18 months.Conclusion This tachycardia is mediated by nodoventricular pathway with bidirectional conduction property,which starts from the slow pathway of AV node and ends at the ventricular myocardium across the AV groove.The tachycardia incorperates His Purkinje system as the antegrade conduction limb and the nodoventricular fiber as the retrograde conduction limb.

Abstract:Objective To evaluate the electrophysiological characteristics of Mahaim type accessory pathway, and the experiences of radiofrequency catheter ablation. Methods Fourteen patients suffered from symptomatic Mahaim type accessory pathway tachycardia were included in the study according to cardiac electrophysiology and the results of radiofrequency catheter ablation. ECG and cardiac programmed stimulation were performed to determine the category. Results Eight subjects suffered from superventricular tachycardia with the forward conduction of atrialfascular accessory pathway, 2 with nodefascular accessory pathway, and 4 with atrialventricular accessory pathway. In the subjects of atrialfascular/ nodefascular accessory pathway , the V-H interval during HRA S1S1 and S1S2 stimulation, supreventricular tachycardia and atrial entrainment were significantly shorter than those of atrialventricular accessory pathway(P0.001). All the subjects were successfully ablated when radiofrequency energy was released at the location where obviously AP potential was recorded. Conclusion Careful electrophysiological examination is necessary to determine the pattern of Mahaim fiber and differentiate them from other analogous tachycardia and proceed successful radiofrequency catheter ablation.

Abstract:This study was undertaken to review the outcome of tachyarrhythmia with radiofrequency catheter ablation(RFCA). The data of 1 108 tachyarrhythmia cases with RFCA was summarized from 1993 to 2003, including achieve rate,recurrence rate ,complication and comparing to the results of 5 years in early stage and later stage. Results:The achieve rate of ablation was 96.7% for all arrhythmia types,98.3% in atrioventricular reentrant tachycardia(AVRT) and 97.5% in atrioventricular nodal reentrant tachycardia(AVNRT) respectively. The total recurrence rate was 3.7%, 3.1% in AVRT and 2.6% in AVNRT. Only 0.99% patients had complications. At the later stage,the mean procedure and radiological exposure time were significantly shorter than at early stage(1.1±0.6 h vs 4.2±0.8 h, P0.001; 20.1±7.1 min vs 40.3±10.3 min,P0.001 respectively). The achieve rate increased significantly (98.4% vs 93.2%,P0.05), while the reduction in the recurrence rate and complication was obvious. Conclusion:RFCA is a safe, effective method for patient with tachyarrhythmia.The procedure has low recurrence rate and complication, and the technique level improves rapidly in the increasing of the case number.
New access for radiofrequency catheter ablation of left-sided atrioventricular accessory pathways: safety and efficacy of the transradial approach

Abstract:Background: The safety and efficacy of the transradial approach for radiofrequency catheter ablation (RFCA) of left-sided atrioventricular accessory pathways (APs) was evaluated in the present study. Methods and results: Included were 40 consecutive patients with type A Wolff-Parkinson-White (WPW) syndrome who underwent RFCA via the radial artery route, and 30 patients with type A WPW syndrome who underwent RFCA via the transfemoral approach (controls) were retrospectively chosen for control. All 45 APs in the 40 patients were successfully ablated: 35 APs were successfully blocked with 1 ablation attempt, and the other 10 APs were ablated after 2-4 attempts. Compared with the transfemoral approach, the total procedure time for the transradial approach was longer (40 +/- 7.7 vs 32.4 +/- 8.7 min, P<0.05) and the fluoroscopic time was similar (7.2 +/- 2.2 vs 7.9 +/- 3.9 min, P>0.05). There were no vascular complications in the transradial group, but 2 patients in the transfemoral developed local hematoma. There was no recurrence of arrhythmia in either group. Conclusions: The transradial approach is a safe and effective access for RFCA of left-sided APs.

Abstract:Introduction: This study sought to investigate electrophysiologic characteristics and radiofrequency ablation (RFA) in patients with focal atrial tachycardia (AT) arising from the left atrial appendage (LAA). Methods: This study included seven patients undergoing RFA with focal AT. Activation mapping was performed during tachycardia to identify an earlier activation in the left atria and the LAA. The atrial appendage angiography was performed to identify the origin in the LAA before and after RFA. Results: AT occurred spontaneously or was induced by isoproterenol infusion rather than programmed extrastimulation and burst atrial pacing in any patient. The tachycardia demonstrated a characteristic P-wave morphology and endocardial activation pattern. The P wave was highly positive in inferior leads in all patients. Lead V-1 showed upright or biphasic (+/-) component in all patients. Lead V-2-V-6 showed an isoelectric component in five patients or an upright component with low amplitude (< 0.1 mV) in two patients. Earliest endocardial activity occurred at the distal coronary sinus (CS) ahead of P wave in all seven patients. Mean tachycardia cycle length was 381 +/- 34 msec and the earliest endocardial activation at the successful RFA site occurred 42.3 +/- 9.6 msec before the onset of P wave. RFA was acutely successful in all seven patients. Long-term success was achieved in seven of the seven over a mean follow-up of 24 +/- 5 months. Conclusions: The LAA is an uncommon site of origin for focal AT (3%). There were consistent P-wave morphology and endocardial activation associated with this type of AT. The LAA focal ablation is safe and effective. Long-term success was achieved with focal ablation in all patients.
Characterization, Mapping and Catheter Ablation of Atrial Tachycardia with Prior Atriotomy

Abstract:Introduction: The aim of this study was to develop bidirectional block of intra-atrial reentrant tachycardias (IARTs) late after the repair of congenital heart disease (CHD) using pace-map and entrainment mapping.Methods and results: There were 106 patients enrolled in this study. These patients were divided into two groups as giant right atrium (RA) group and non-giant RA group based on their RA volume measured by Carto. All patients have performed activation and entrainment mapping among these patients, 24 (22.6%) of Giant RA group had dual-loop reentry and 82 patients had single loop reentry. 94 patients (88.7%) found out the gaps by pace-map in SR. Linear ablation was successful in 105 patients (99%). During a mean follow-up period of 105 ± 34 months the success rate of IARTs was 66%, there were significantly different in these two groups(P<0.01). Seven (6.6%) patients in Giant RA group undergone redo procedure.Conclusion: Entrainment mapping in tachycardia and pace-map combined with three-dimensional electroanatomic mapping can elaborate the mechanism of complex re-entry circuits and critical isthmuses as targets for ablation. Pace-map in SR may be a good method for seeking the gap of ablation line to be bidirectional block for linear ablation IARTs.
