Abstract:We are grateful for the reviewers’ comments on our manuscript entitled “Robotic thoracic surgery: S1+2 segmentectomy of the left upper lobe” (1). An advantage of robotic assisted thoracic surgery (RATS) is that the robotic arms are flexible and allow the surgeon to move instruments freely inside the chest cavity as needed without geometric limitations. Compared with single-incision video-assisted thoracic surgery (VATS), the flexibility is one of the primary advantages of robotic surgery. Although the literature revealed that RATS does not seem to offer any advantages over VATS with regards to complications, postoperative pain, hospital stay, and oncological outcome for early-stage lung cancer (2,3), the benefit of RATS is that the surgeon feels more comfortable and confident when performing RATS.

Abstract:From the first Laparoscopic pancreaticoduodenectomy (LPD) in 1994 to the first Da Vinci robotic pancreaticoduodenectomy(RPD) in 2003, minimally invasive pancreaticoduodenectomy has achieved a breakthrough after 20-year exploration and development.It is a well-established procedure for periampullary carcinomas in some specialized centers now.Some outstanding reports including large clinical trials more than 100 cases of L/RPD in a single center and review analysis about the short and long outcomes between L/RPD and open PD for large cases have been published.For the sake of increasing popularity and acceptance of L/RPD, it is very important for researchers to update the current status of L/RPD and summarize experience and development.

Abstract:We appreciate the thoughtful and constructive comments by Dr. Wael C. Hanna from Canada (1) and Dr. Abbas E. Abbas from America (2) on our article “ Robotic Assisted Right Middle Lobectomy ”. In this article, we described our experience with robotic assisted right middle lobectomy through a specific case (3). As both Dr. Hanna and Dr. Abbas mentioned in their insightful comments, they preferred to line up all of the robotic ports in the same intercostal space (8 th intercostal space) to minimize postoperative pain. In robot-assisted thoracic surgery, the incision positions are flexible and their placement primarily depends on the practice and preference of the surgeon. Many surgeons place the ports in multiple locations in the intercostal space (4-6). Another important factor that we could not ignore is racial difference. Caucasians have wider chests than Chinese; the Chinese chest surface is 93% of the chest surface of Caucasians (7).

Abstract:Minimally invasive techniques have been widely applied in general thoracic surgery. Compared with video-assisted thoracoscopic surgery (VATS), due to its theoretic superiority, robotic surgery is challenging the traditional position of VATS. With its unique advantages, including 3D vision and a high-freedom endowrist, it leads to easier lymph node dissection, more convenient blood vessel dissection, a shorter learning curve and competence for the completion of complex surgery. However, as a new surgical technology, the safety and efficacy of robotic-assisted thoracoscopic surgery (RATS) still need to be further verified. Thus, in this article, we review and summarize the application of RATS versus VATS in general thoracic surgery.

Abstract:AbstractOver the past decade, management of the T1 renal mass has focused on nephron‐sparing surgery. Robotic partial nephrectomy has played an increasing role in the technique of preserving renal function by decreasing warm ischemia time, as well as optimizing outcomes of hemorrhage and fistula. Robot‐assisted partial nephrectomy is designed to provide a minimally‐invasive nephron‐sparing surgical option utilizing reconstructive capability, decreasing intracorporeal suturing time, technical feasibility and safety. Ultimately, its benefits are resulting in its dissemination across institutions. Articulated instrumentation and three‐dimensional vision facilitate resection, collecting system reconstruction and renorrhaphy, leading to decreased warm ischemia time while preserving oncological outcomes. The aim of the present review was to present our surgical sequence and technique, as well as review the current status of robot‐assisted partial nephrectomy.

Abstract:Robotic-Assisted Thoracic Surgery (RATS) is considered one of the main issues of present thoracic surgery. RATS is a minimally invasive surgical technique allowing enhanced view, accurate and complex movements, and high ergonomics for the surgeon. Despite these advantages, its application in lung procedures has been limited, mainly by its costs. Since now many different approaches have been proposed and the experience in RATS for lungs ranges from wedge resection to pneumonectomy and is mainly related to lung cancer. The present narrative review explores main approaches and outcomes of RATS lobectomy for lung cancer. A non-systematic review of literature was conducted using the PubMed search engine. An overview of lung robotic surgery is given, and main approaches of robotic lobectomy for lung cancer are exposed. Initial experiences of biportal and uniportal RATS are also described. So far, retrospective analysis reported satisfactory robotic operative outcomes, and comparison with VATS might suggest a more accurate lymphadenectomy. Some Authors might even suggest better perioperative outcomes too. From an oncological standpoint, no definitive prospective study has yet been published but several retrospective analyses report oncological outcomes comparable to those of VATS and open surgery. Literature suggests that RATS for lung procedures is safe and effective and should be considered as a valid additional surgical option.

Abstract:Therapeutic endoscopic resection has gained favor for its ability to achieve high en bloc and histologically complete resection rates via a minimally invasive approach. The main technical difficulties faced by interventionists are first the lack of traction causing suboptimal visualization of the dissection field and second, the lack of triangulation using existing therapeutic apparatuses. These challenges can be overcome with the use of robots and the multiple degrees of freedom afforded by the robotic wrists. Nevertheless, complications such as bleeding and perforation can occur. It is hence beneficial for the robotic device to be equipped with additional abilities such as suturing. Once the robotic prototypes have been fully optimized and marketed, a structured program should be instituted to ensure proper and adequate training of the end-users. The future of robotics should then explore the possibility of developing a soft robot or a robot with more natural human-like movements. A robot with a force feedback mechanism would be superior and improve safety. Eventually, a supervised autonomous robot may perform interventions with greater precision and accuracy than an expert procedurist. This review describes the benefits of robot-assisted endoscopic resections, recent developments aimed at managing iatrogenic complications and future directions for robotic endoscopy.

Abstract:Background: Robotic hepatectomy represents an appealing treatment modality for resectable hepatocellular carcinoma (HCC). A contemporary review of robotic hepatectomy compared to laparoscopic/open hepatectomy is necessary. Methods: We performed a literature review to identify studies between 2018–2024 comparing robotic to laparoscopic/open hepatectomy for HCC with measurable outcomes. Results: A total of 10 studies were identified, including 943 patients undergoing robotic hepatectomy compared to 1678 patients undergoing laparoscopic/open hepatectomy. Generally, while similar short/long-term survival was noted across all resection modalities, robotic hepatectomy was associated with longer operative time, shorter length of stay, and less post-operative complications. An additional 4 studies were evaluated in the context of HCC, reviewing the prognostic value of robotic hepatectomy margins, robotic hepatectomy in the context of metabolic syndrome, "huge" (>10 cm) HCCs, and robotic hepatectomy vs. microwave ablation. Conclusions: Robotic hepatectomy is a safe alternative to laparoscopic/open hepatectomy for HCC that provides similar oncological/long-term outcomes, while potentially decreasing post-operative complications and length of stay.
