Abstract:We appreciate the thoughtful and constructive comments by Dr. Wael C. Hanna from Canada (1) and Dr. Abbas E. Abbas from America (2) on our article “ Robotic Assisted Right Middle Lobectomy ”. In this article, we described our experience with robotic assisted right middle lobectomy through a specific case (3). As both Dr. Hanna and Dr. Abbas mentioned in their insightful comments, they preferred to line up all of the robotic ports in the same intercostal space (8 th intercostal space) to minimize postoperative pain. In robot-assisted thoracic surgery, the incision positions are flexible and their placement primarily depends on the practice and preference of the surgeon. Many surgeons place the ports in multiple locations in the intercostal space (4-6). Another important factor that we could not ignore is racial difference. Caucasians have wider chests than Chinese; the Chinese chest surface is 93% of the chest surface of Caucasians (7).

Abstract:AbstractOver the past decade, management of the T1 renal mass has focused on nephron‐sparing surgery. Robotic partial nephrectomy has played an increasing role in the technique of preserving renal function by decreasing warm ischemia time, as well as optimizing outcomes of hemorrhage and fistula. Robot‐assisted partial nephrectomy is designed to provide a minimally‐invasive nephron‐sparing surgical option utilizing reconstructive capability, decreasing intracorporeal suturing time, technical feasibility and safety. Ultimately, its benefits are resulting in its dissemination across institutions. Articulated instrumentation and three‐dimensional vision facilitate resection, collecting system reconstruction and renorrhaphy, leading to decreased warm ischemia time while preserving oncological outcomes. The aim of the present review was to present our surgical sequence and technique, as well as review the current status of robot‐assisted partial nephrectomy.

Abstract:Robotic-assisted surgery is now well established and has been introduced into the field of thoracic surgery. We are going to share the experience of robotic surgery for left inferior lobectomy. A 44-year-old patient was underwent robotic-assisted thoracic surgery for a pulmonary nodule. The patient was discharged on postoperative day 3 without any perioperative complications. The pathological stage was T1aN0M0 (stage IA). Our result showed the robotic-assisted thoracoscopic surgery was a safe and feasible surgical approach for lobectomy.

Abstract:We are grateful for the reviewers’ comments on our manuscript entitled “Robotic thoracic surgery: S1+2 segmentectomy of the left upper lobe” (1). An advantage of robotic assisted thoracic surgery (RATS) is that the robotic arms are flexible and allow the surgeon to move instruments freely inside the chest cavity as needed without geometric limitations. Compared with single-incision video-assisted thoracic surgery (VATS), the flexibility is one of the primary advantages of robotic surgery. Although the literature revealed that RATS does not seem to offer any advantages over VATS with regards to complications, postoperative pain, hospital stay, and oncological outcome for early-stage lung cancer (2,3), the benefit of RATS is that the surgeon feels more comfortable and confident when performing RATS.

The patient was discharged on postoperative day 6 without any perioperative complications. The pathological stage was T1aN0M0 (stage IA). Our result showed the robotic-assisted thoracoscopic surgery was a safe and feasible surgical approach for non-small cell lung cancer (NSCLC).
Robotic-assisted laparoscopic anatomic hepatectomy in China: initial experience

Abstract:Objective: To assess the feasibility and safety of robotic-assisted laparoscopic anatomic hepatectomy. Background: The development of minimally invasive surgery has led to an increase in the use of laparoscopic hepatectomy. However, laparoscopic hepatectomy remains technically challenging and is not widely developed. Robotic surgery represents a recent evolution in minimally invasive surgery that is being used increasingly for complex minimally invasive surgical procedures. Herein, we report our initial experience with robotic-assisted laparoscopic anatomic hepatectomy in 13 consecutive patients. Patients and methods: Between April and July 2009, 13 consecutive patients underwent robotic-assisted laparoscopic anatomic hepatectomies for benign and malignant hepatic diseases. Major hepatectomies were performed in 9 patients, left lateral sectionectomies in 4 patients. Eight major hepatectomies were for malignant diseases and 5 hepatectomies (1 left hepatectomy and 4 left lateral sectionectomies) were for benign diseases. All the robotic-assisted hepatectomy procedures were performed anatomically with hilum dissection. Prior to starting the parenchymal transaction, vascular control of the portal vessels was carried out whenever possible. These robotic-assisted laparoscopic anatomic hepatectomies were compared with 20 traditional laparoscopic hepatectomies and 32 open resections that were contemporaneous and cohort-matched. Results: All 13 robotic-assisted laparoscopic anatomic hepatectomies were performed successfully in the manner of pure laparoscopic resection. No conversion to laparotomy or hand-assisted laparoscopic resection occurred. Despite its longer operative time (338 minutes) and higher hospital cost ($12,046), robotic liver surgery compared favorably with traditional laparoscopic hepatectomy and open resection in blood loss (280 vs. 350, 470 mL), transfusion requirement (0 vs. 3 of 20, 4 of 32), use of the Pringle maneuver (0 vs. 3 of 20, 6 of 32) and overall operative complications (7.8% vs. 10%,12.5%). Neither ascites nor transient hepatic decompensation occurred in the robotic group. The surgical margins in all 8 patients with malignant lesions were negative and as yet, no intrahepatic recurrences or metastases have been observed in the robotic group. The mean postoperative stay was shorter with the traditional laparoscopic procedure (5.2 days) than with robotic (6.7 days)or open surgery (9.6 days). Conversions from traditional laparoscopic to open and hand-assisted laparoscopic resection occurred in 2 patients (10.0%) who underwent right hemihepatectomy and left hepatectomy, respectively. Conclusions: These preliminary results show that robotic-assisted laparoscopic anatomic hepatectomy is safe and feasible with a much lower complication and conversion rate than traditional laparoscopic hepatectomy or open resection. The robotic surgical system may broaden the indications for laparoscopic hepatactomy, and it enabled the surgeon to perform precise laparoscopic liver resection which required hylum dissection, hepatocaval dissection, endoscopic suturing, and microanastamosis. However, more long-term, evidence-based outcomes will be necessary to prove its efficacy, and further research on its cost-effectiveness is still required.

Abstract:The robotic approach in thoracic surgery has rapidly gained popularity in recent years. As with the introduction of any new technology, this warrants not only adaptation of the operative technique itself, but also the evolution of appropriate troubleshooting strategies. A selected number of helpful tips and technical procedural manoeuvres have been compiled to prevent intraoperative problems, as well as to overcome challenging situations that can arise during robotic lobectomies. In robotic surgery, as opposed to open surgery or video-assisted thoracic surgery, these tips serve an important purpose for the operating surgeon, as well as the entire surgical team involved in the procedure. All the assembled recommendations have proved their effectiveness and have been successfully used by the authors in many procedures. Furthermore, these manoeuvres have been found to be of great importance in the training and proctoring of thoracic surgeons, fellows and residents (bed-side assistants). This guide of clearly arranged tips and troubleshooting strategies offers surgeons a useful tool to overcome difficult situations in robotic lobectomy and preferably improve the reproducibility and safety of their procedures.
Initial experience with a robotic hepatectomy program at a high-volume laparoscopic center: single-center experience and surgical tips

Abstract:Background: Despite the development of laparoscopic surgery, there are still inherent limitations associated with conventional laparoscopic instruments such as restrictions in movement and an inability for articulation. Robotic surgery may help to overcome the limitations of conventional laparoscopic surgery. The aim of this study was to present our initial experience with robotic hepatectomy (RH) and discuss the steps required to develop an RH program at a high-volume laparoscopic hepatectomy (LH) center. Methods: We retrospectively reviewed prospectively collected data for 14 consecutive patients who underwent RH between 2017 and 2018. Clinicopathological characteristics and perioperative outcomes were compared with those reported in previous studies. The operation time of each procedure was analyzed to assess RH proficiency based on experience. Results: Of the 14 patients, 12 patients (85.7%) underwent robotic major hepatectomy. Median patient age was 54.5 years, while median body mass index (BMI) was 25.2 kg/m2. The median operation time was 360 (range: 145-544) min. The median estimated blood loss (EBL) was 300 (range: 50-1,400) mL. Conversion to open surgery was not required in any case. The median length of hospital stay was 5 (range: 4-14) days. Major complications occurred in 2 patients (14.2%), although both recovered without sequelae. The time required for hilar dissection, docking, and parenchymal transection gradually decreased after the first two cases of RH. Conclusions: From our initial experience, RH might be considered as a feasible procedure in the liver resection, even in major hepatectomy. In addition, surgeons with sufficient experience in LH could rapidly adapt for RH. However, we have to make a system for education and monitoring of this innovative surgery for the patients' safety.

Abstract:BackgroundRobotic surgery can enhance a surgeon’s laparoscopic skills through a magnified three-dimensional view and instruments with seven degrees of freedom compared to conventional laparoscopy.MethodsThis study reviewed a single surgeon’s experience of robotic liver resections in 30 consecutive patients, focusing on major hepatectomy. Clinicopathological characteristics and perioperative and short-term outcomes were analyzed.ResultsThe mean age of the patients was 52.4 years and 14 were male. There were 21 malignant tumors and 9 benign lesions. There were 6 right hepatectomies, 14 left hepatectomies, 4 left lateral sectionectomies, 2 segmentectomies, and 4 wedge resections. The average operating time for the right and left hepatectomies was 724 min (range 648–812) and 518 min (range 315–763), respectively. The average estimated blood loss in the right and left hepatectomies was 629 ml (range 100–1500) and 328 ml (range 150–900), respectively. Four patients (14.8%) received perioperative transfusion. There were two conversions to open surgery (one right hepatectomy and one left hepatectomy). The overall complication rate was 43.3% (grade I, 5; grade II, 2; grade III, 6; grade IV, 0) and 40% in 20 patients who underwent major hepatectomy. Among the six (20.0%) grade III complications, a liver resection–related complication (bile leakage) occurred in two patients. The mean length of hospital stay was 11.7 days (range 5–46). There was no recurrence in the 13 patients with hepatocellular carcinoma during the median follow-up of 11 months (range 5–29).ConclusionsFrom our experience, robotic liver resection seems to be a feasible and safe procedure, even for major hepatectomy. Robotic surgery can be considered a new advanced option for minimally invasive liver surgery.

Abstract:The optimized surgical procedure for right upper lobectomy by minimal invasive surgery was introduced, involving transection of interlobar fissures with the superior pulmonary vein and the ascending A2 artery simultaneously. Compared with the traditional method, this procedure achieved better clinical outcomes, including shorter operation times, fewer complications, less blood loss, shorter hospitalization times, and lower costs, while yielding similar disease‐free survival. Background This multi‐center study was aimed at retrospectively evaluating the feasibility, safety, clinical outcomes, and surgical learning curve of an optimized procedure for right upper lobectomy (RUL), which is challenging because of the anatomical structures and features of this lobe. Methods This study included 45 RUL cases of robot‐assisted thoracoscopy (RATS) in a pilot cohort and 187 RUL cases of video‐assisted thoracoscopy (VATS) in three cohorts. A total of 121 and 111 patients underwent traditional and optimized RUL, respectively. The optimized surgical procedure was performed to consecutively transect the superior arterial trunk and bronchus, and finally disconnect the pulmonary vein and posterior ascending artery with interlobar fissures. Clinical and radiological data were reviewed retrospectively. Results Optimized RUL can be performed successfully by RATS or VATS. The optimized procedure yielded better clinical outcomes than the traditional procedure, including shorter operation times, less blood loss, fewer complications, shorter hospital times, lower costs, and a lower likelihood of postoperative intermedius bronchial kinking. Additionally, for calcified interlobar lymph nodes, the optimized VATS group was less likely to be converted to thoracotomy than the traditional group. The skills required to perform optimized VATS RUL can be gained by surgeons after 12 to 15 cases. The two RUL procedures in the pilot cohort showed similar disease‐free survival. Conclusions The optimized RUL was safe, economical, and feasible, with a short learning curve and satisfactory disease‐free survival.
