Abstract:There is little evidence that market reforms have had any appreciable effect on the NHS’s performance in England, the Nuffield Trust and the Health Foundation have found in a new comparison of health services in the four countries of the United Kingdom.1 Although performance in England had improved, the study found the same to be true of Scotland and, until recently, Wales—neither of which had undergone the same reforms. Spending more money and setting tough targets seemed to have been the main cause of the improvements rather than patient choice, competition between providers, or private sector involvement. Abstract:The number of hospital deaths has fallen more quickly in England than in Scotland since 1997, an analysis of administrative data has shown.1The differences are substantial, said Maria Jose Aragon and Martin Chalkley of the Centre for Health Economics at the University of York, and apply to both emergency and elective care. Abstract:International studies show that relative levels of regional (de)centralisation are associated with more or less balanced patterns of economic growth, well-being and resilience. Alongside supporting specific levels and types of devolution, prior studies emphasise the quality of local institutions as a key factor underlying balanced growth. This study empirically confirms the relative lack of devolution alongside large and growing disparities across England's regions. Drawing on an interview-based case study of the West Midlands, we then identify a Catch-22, with devolution predicated on high-quality local institutions caught in a highly centralised system that undermines the development of the required capacity and competency. We identify specific decision-making and resource allocation mechanisms, appraisal and control systems which underpin the dominance of central government agencies in an ad hoc and asymmetric approach to devolution. In combination these limit progress towards locally-driven 'levelling-up'.

Abstract:The legislation to devolve responsibility for the management and operation of England's top-performing NHS hospitals to community-owned NHS Foundation Trusts raises several issues relating to the challenges posed to governance structures by private non-profit ownership and control of assets used to provide government-financed services. Building upon the lessons learned from devolution of public hospital governance in New Zealand to boards at arm's-length from central control during the 1990s, this paper analyses the English NHS hospital changes. Whilst local political accountability and competition between hospitals indicate that the English reforms may be more successful in meeting patients' needs more efficiently than the New Zealand reforms, the English proposals may be compromised by the ability of staff to become members of Trusts, boards bearing risks of decisions outside their control whilst simultaneously being insulated from the consequences of their decisions by a 'soft budget constraint', and conflicts of interest as boards simultaneously act as agents of both central regulators and local beneficiaries.

Abstract:The UK NHS has a number of important strengths. Its costs are relatively low compared to the health care systems of other developed countries due in part to cash limited central budgeting. It is extremely popular with the electorate and surveys show overall satisfaction with the NHS despite some dissatisfaction with waiting lists and a public perception of underfunding. The NHS model of general medical care provided by independent contractors has been acclaimed as "a British success" (General Medical Services Council, 1983). The role of the UK GP combines providing primary care and acting as a gatekeeper to secondary care. This increases equitable access to care for the population and assists in cost containment. As a model, it is currently being emulated in other countries including Sweden and US Health Maintenance Organizations but, as in these countries, the UK primary care model has been evaluated poorly. There are of course continuing weaknesses in the UK health care system. There is insufficient knowledge upon which to base health care services and increase efficiency. In the future, if a knowledge-based health care service is to be created, a considerable amount of research and evaluation is required to identify "what works" in health care (i.e., what is effective) and also the cost effective ways of altering provider behaviour to maximise the amount of health gain which can be achieved using a limited budget. The NHS reforms created a lot of enthusiasm and energy but its effects are difficult to disentangle from the simultaneous increases in funding. There is little evidence from the UK or elsewhere that competition in health care produces efficiency or improvements in resource allocation. Evaluation is required to identify which of the reforms are increasing efficiency. Competition needs to be used with caution and recognised as a mean and not an end in itself. It is remarkable how both clinical practice and health policy reform, in the UK and elsewhere, is poorly evaluated. Medical practice varies substantially locally, regionally, and internationally, e.g., patients with similar age and stage of cancer receive very different levels of radiotherapy across Europe. For most interventions, the appropriate level of treatment may be asserted but is not based on cost effectiveness knowledge. Health policy analysts, like clinicians, make assertions about competition and other health care reforms which are value- rather than knowledge-based. Both groups of decision-makers should be more cautious, informing their choices with research rather than relying on unsubstantiated optimism!
The impact of devolution on local health systems: Evidence from Greater Manchester, England

Abstract:Devolution and decentralisation policies involving health and other government sectors have been promoted with a view to improve efficiency and equity in local service provision. Evaluations of these reforms have focused on specific health or care measures, but little is known about their full impact on local health systems. We evaluated the impact of devolution in Greater Manchester (England) on multiple outcomes using a whole system approach. We estimated the impact of devolution until February 2020 on 98 measures of health system performance, using the generalised synthetic control method and adjusting for multiple hypothesis testing. We selected measures from existing monitoring frameworks to populate the WHO Health System Performance Assessment framework. The included measures captured information on health system functions, intermediatory objectives, final goals, and social determinants of health. We identified which indicators were targeted in response to devolution from an analysis of 170 health policy intervention documents. Life expectancy (0.233 years, S.E. 0.012) and healthy life expectancy (0.603 years, S.E. 0.391) increased more in GM than in the estimated synthetic control group following devolution. These increases were driven by improvements in public health, primary care, hospital, and adult social care services as well as factors associated with social determinants of health, including a reduction in alcohol-related admissions (-110.1 admission per 100,000, S.E. 9.07). In contrast, the impact on outpatient, mental health, maternity, and dental services was mixed. Devolution was associated with improved population health, driven by improvements in health services and wider social determinants of health. These changes occurred despite limited devolved powers over health service resources suggesting that other mechanisms played an important role, including the allocation of sustainability and transformation funding and the alignment of decision-making across health, social care, and wider public services in the region.

Abstract:New indicators show that primary care clinical commissioning groups (CCGs) in England vary widely in their ability to deliver healthcare equitably to rich and poor people. Some inner city areas, such as Tower Hamlets in east London, have done well on the new measure, while others, such as central Manchester, did poorly. A large gap could also be seen in affluent areas, with south Cheshire performing relatively badly while east Surrey did well. Abstract:This article assesses how and why the National Health Service (NHS) has remained at the forefront of British politics and society for 75 years amid significant economic pressures, organizational strain, shifting ideological trends, ongoing reforms, and an unprecedented public health emergency. The postwar "years of consensus" evolved into alleged decline and ostensible neglect during the 1980s, while New Labour sought to rejuvenate this core public service after 1997, featuring investment and often controversial reforms that challenged the party's social democratic values. Amid the New Labour era, NHS powers filtered down to devolved administrations, while from 2008 retrenchment and austerity ensued, fueled by global recession. Austerity eventually subsided, yet from early 2020 the NHS swiftly faced the extreme conditions of the globalized Covid-19 pandemic. The service continues to face challenges regarding its longer-term viability, and this article analyzes this scenario, within the context of the NHS's 75-year historical legacy, its contemporary status, comparative international trends, and likely future evolution.

Abstract:So much for decentralisation: England’s health secretary can’t keep his hands off the NHS It hasn’t taken long for former health secretary Andrew Lansley’s plan for a “hands-off” NHS to hit the rocks. His successor, Jeremy Hunt, is behaving as if the Health and Social Care Act 2012, with its careful separation of powers, had never passed into law. If there’s a problem—and there are plenty—it isn’t long before Hunt comes up with a new wheeze for dealing with it, as health secretaries always have. From pressure on emergency care to hospital quality or the supposed iniquities of the general practitioner contract, he’s there with an opinion and an initiative. His junior minister, Dan Poulter, has echoed his master’s voice with a plan of his own, suggesting with no great originality that the NHS could save money by better procurement.1 The merits of these interventions matter less than their frequency and specificity, which tell us that the Lansley plan is in tatters. Abstract:Under recent reforms to the National Health Service (NHS) in England, NHS organizations have been given new objectives to contribute to social and economic development. Health systems in other high-income countries are pursuing related approaches. This paper analyses national policy documents to understand the framing of the NHS's new policy priorities on social and economic development. We focus on the role of NHS integrated care systems—area-based planning bodies responsible for managing NHS resources and coordinating local services. National policy is vague about what social and economic development means in practice. There is limited guidance on which approaches local organizations should prioritize, and which organisations are responsible for implementation. Greater clarity from national policymakers and an overarching framework to guide local action is needed to reduce the risk of inaction, poorly targeted interventions, and missed opportunities for learning and evaluation. Policymakers and health system leaders also need to be realistic about the limits of local action given the importance of national public policy choices in shaping the social determinants of health. Coordinated policy action and investment across government is needed to address underlying social and economic conditions.

Abstract:BackgroundThe wide-ranging program of reforms brought about by the Health and Social Care Act (2012) in England fundamentally changed the operation of the public health system, moving responsibility for the commissioning and delivery of services from the National Health Service to locally elected councils and a new national public health agency. This paper explores the ways in which the reforms have altered public health commissioning.MethodsWe conducted multi-methods research over 33 months, incorporating national surveys of Directors of Public Health and local council elected members at two time-points, and in-depth case studies in five purposively selected geographical areas.ResultsPublic health commissioning responsibilities have changed and become more fragmented, being split amongst a range of different organisations, most of which were newly created in 2013. There is much change in the way public health commissioning is done, in who is doing it, and in what is commissioned, since the reforms. There is wider consultation on decisions in the local council setting than in the NHS, and elected members now have a strong influence on public health prioritisation. There is more (and different) scrutiny being applied to public health contracts, and most councils have embarked on wide-ranging changes to the health improvement services they commission. Public health money is being used in different ways as councils are adapting to increasing financial constraint.ConclusionsOur findings suggest that, while some of the intended opportunities to improve population health and create a more joined-up system with clearer leadership have been achieved, fragmentation, dispersed decision-making and uncertainties regarding funding remain significant challenges. There have been profound changes in commissioning processes, with consequences for what health improvement services are ultimately commissioned. Time (and further research) will tell if any of these changes lead to improved population health outcomes and reduced health inequalities, but many of the opportunities brought about by the reforms are threatened by the continued flux in the system.

Abstract:The NHS in England balanced its books in 2017-18, but appearances can be deceptive, says the parliamentary Public Accounts Committee in its review of the NHS’s accounts.1 Striking the balance required some creative accounting, such as using money from the Sustainability and Transformation Fund to meet winter pressures, and the overall picture of stability conceals some local variation that could be affecting the care of patients. The Department of Health and Social Care spent £120.7bn (€134bn; $153bn) on running costs and £5.2bn on capital spending in 2017-18. Abstract:Background: Giving children the best start in life is critical for their future health and wellbeing. Political devolution in the UK provides a natural experiment to explore how public health systems contribute to children's early developmental outcomes across four countries. Method: A systematic literature review and input from a stakeholder group was used to develop a public health systems framework. This framework then informed analysis of public health policy approaches to early child development. Results: A total of 118 studies met the inclusion criteria. All national policies championed a 'prevention approach' to early child development. Political factors shaped divergence, with variation in national conceptualizations of child development ('preparing for life' versus 'preparing for school') and pre-school provision ('universal entitlement' or 'earned benefit'). Poverty and resourcing were identified as key system factors that influenced outcomes. Scotland and Wales have enacted distinctive legislation focusing on wider determinants. However, this is limited by the extent of devolved powers. Conclusion: The systems framework clarifies policy complexity relating to early child development. The divergence of child development policies in the four countries and, particularly, the explicit recognition in Scottish and Welsh policy of wider determinants, creates scope for this topic to be a tracer area to compare UK public health systems longer term.
A Review-Based Comparative Analysis of Health Care Delivery System in Nigeria and the United Kingdom

Abstract:Background: It is a general belief that the healthcare delivery system (HCDS) in Nigeria is bedevilled by frequent workers industrial actions, inter-professional rivalry among healthcare workers and insurance scheme mismanagement and corruption. Objective: This study was aimed at comparing the health care system in Nigeria (a lower middle-income country) and the United Kingdom (a high-income country). Methods: Using keywords including 'primary health care', 'secondary health care', 'tertiary health care', 'health care delivery system', 'Nigeria' and ''United Kingdom, PubMed was searched for studies on health care in Nigeria and the United Kingdom. Databases of the World Bank, World Health Organization, Central Intelligence Agency World Factbook and Index Mundi were also searched for information on health indices and organisational characteristics for Nigeria and the United Kingdom. Retrieved articles and data obtained were analysed and a two-country comparison was performed on HCDS features and health indices. Results: The average Life Expectancy in Nigeria is less than two third of that of the UK (54.8 years vs. 81.2 years) and the probability of dying between 15 and 60 years per thousands of population is 127.8% higher among Nigerians than among UK citizens. The Infant mortality rate per 1,000 live births and the under-five children mortality rate is 16 and 23 times higher in Nigeria than in the UK. The probability of dying on or before the first year of life and before the age of five is 177% and 183% higher respectively, in Nigeria than in the UK. While the UK ranked within the top 2% -10% on health system rating features such as achievement of goals, health care services distribution and overall goal achievement on HCDS, Nigeria is ranked within the bottom 2%-5% in these measures. Discussion: Using the HCDS in the UK as a model, it can be argued that a multi-faceted approach that includes attitudinal and behavioural changes among the healthcare workers, proactive health and social policy implementation and monitoring, preventive and corrective anti-corruption measures, population control and increased healthcare spending may be needed to address challenges and improve Nigeria's healthcare rating and ranking among countries.
