Effect of 20% Lipid Emulsion on Pharmacokinetics of 0.5% Ropivacaine after Interscalene Nerve Block in Patients Undergoing Upper Limb Orthopacdic Surgery

Abstract:Objective To investigate the effect of 20% lipid emulsion on pharmacokinetics of 0.5%ropivacaine after interscalene nerve block in patients undergoing upper limb orthopacdic surgery.Methods Twenty-four ASA Ⅰ or Ⅱ patients of both sexes aged 18-60 yr weighing 65-80 kg undergoing elective upper limb orthopaedic surgery were randomly divided into 2 groups(n = 12 each): group Ⅰ-received 0.9% NaCl solution1.5 ml/kg iv over 10 min and group Ⅱ received 20% lipid emulsion 1.5 ml/kg iv over 10 min.Then 0.5%ropivacaine 30 ml was injected for ultrasound-guided interscalene nerve block in both groups.Blood samples were taken at 2,5,10,15,20,30,45,60,90 and 120 min after the block for determination of plasma concentrations of ropivacaine using HPLC.Pharmacokinetic parameters were calculated.Results Plasma concentrations of ropivacaine were higher in the group Ⅱ than in group Ⅰ(P< 0.05 or 0.01).The difference was statistically significant starting from 45 min after nerve block(P< 0.05).Conclusion 20 % Lipid emulsion can increase the distribution of ropivacaine in plasma,while decrease the distribution in tissues and reduce toxicity caused by ropivacaine in patients undergoing upper limb orthopacdic surgery.
