Effect of Modified Interscalene Approach for Continuous Brachial Plexus Block and Patient-Controlled Postoperation Analgesia on Upper Limb Surgery

Abstract:Objective To study the feasibility of anesthesia and analgesia in upper limb surgery by modified interscalene approach for continuous brachial plexus block through a perivasculer catheter.Methods Eighty American Society of Anesthesiologists(ASA)Ⅰ~Ⅱpatients aged 19~67 years for upper limb surgery were randomly divided into 2 groups.In the control group,classical single-shot interscalene anesthesia and patientcontrolled subcutaneous analgesia(PCSA) with Sufentanyl were used in postoperation.In the experiment group,modified approach for continuous interscalene block and continuous perineural block analgesia (CPNA) with Ropivacaine in postoperation.Operative anesthsia effects and postoperative analgesia with visual analog scale(VAS) were observed at 0 h,2 h,4 h,8 h,16 h in two groups.Ramesay sedation scores and incident complication about dyspnea,nausea and vomit were recorded.Results Both groups showed good anesthsia effects in operation.Compared with the control group,VAS was higher in experiment group at 0h and 2 h(P0.05 ) while it had no differences at 4 h,8 h,16 h.Ramesay sedation scores had no differences between two groups at 0 h,2 h but at 4 h,8 h,16 h postoperation it were much lower in the experiment group(P0.05 ).Compared with the control group,incident complication about nausea and vomit was lower than those in the experiment group at 8 h,16 h postoperation(P0.05 ).Conclusion The modified approach for continous interscalene block has good operative anesthesia effects satisfied analgesia effects and lower complication in postoperation.It is a good、safe and reliable method and deserved to spread this approachs in upper limb surgery.
