Can Maximum-a-Posteriori Reconstruction Improve Imaging Quality on Liver Malignant Tumor PET/CT Imaging Compared with Ordered Subsets Expectation Maximization Reconstruction?

Abstract:1584 Objectives To evaluate the performance of maximum-a-posteriori (MAP) reconstruction on PET/CT image quality of liver malignant tumour compared to ordered subsets expectation maximization (OSEM) reconstruction. Methods 20 patients with liver malignant tumour were examined by FDG PET/CT (uMI 510,Shanghai, China) before surgery. Six image series were reconstructed, including one for OSEM and 5 for MAP with different reconstruction strengths from 1 to 5. SUVmax of tumour, SUVmax of normal liver tissue, and the tumor to normal ratio (T/N), were recorded. Subjective scores of image quality (IQ) were also assessed by 2 nuclear medicine physicians. It was rated by a 5-point scale which 5 score was the best IQ. Results A total of 31 malignant tumors were histopathologically confirmed by surgery in 20 patients. The SUVmax of the liver malignant tumor decreased with the increasing of MAP reconstruction strength and was higher than OSEM reconstruction except MAP4 and MAP5. There were significant differences between MAP reconstruction and OSEM reconstruction (P=0.00) on the value of SUVmax. But there was no significant difference between MAP and OSEM reconstruction on T/N ratio (P>0.05). Among the overall 6 datasets, the dataset of MAP3 obtained the best image quality scores of 4.6. And there was significant differences between MAP3 reconstruction and OSEM reconstruction on IQ score (P=0.001). Conclusions Maximum-a-Posteriori reconstruction can significantly improve PET/CT imaging quality of liver malignant tumor compared to OSEM. MAP with reconstruction strength 3 was a pertinent choice for PET/CT imaging.
