Abstract:Abstract Background Image reconstruction algorithm is one of the important factors affecting the quantitative parameters of PET/CT. The purpose of this study was to investigate the effects of time of flight (TOF) and point spread function (PSF) on quantitative parameters of lung lesions in 18 F-FDG PET/CT. Methods This retrospective study evaluated 60 lung lesions in 39 patients who had undergone 18 F-fluoro-deoxy-glucose (FDG) PET/CT. All lesions larger than 10 mm in diameter were included in the study. The PET data were reconstructed with a baseline ordered-subsets expectation–maximization (OSEM) algorithm, OSEM + PSF, OSEM + TOF and OSEM + TOF + PSF respectively. The differences of maximum standard uptake value (SUVmax), mean standard uptake value (SUVmean), metabolic tumor volume (MTV), total lesion glycolysis (TLG)and signal to noise ratio (SNR)were compared among different reconstruction algorithms. Results Compared with OSEM reconstruction, using OSEM + TOF + PSF increased SUVmean and SUVmax by 23.73% and 22.71% respectively, and SNR increased by 70.18%, MTV decreased by 23.84% ( p 15.31). The difference of TLG among various reconstruction algorithms is relatively small, the highest value is − 6.48% of OSEM + TOF + PSF, and the lowest value is 0.81% of OSEM + TOF. Conclusion TOF and PSF significantly affected the quantitative parameters of lung lesions in 18 F-FDG PET/CT. OSEM + TOF + PSF can significantly increased SUVmax, SUVmean and SNR, and significantly reduce MTV, especially in small lesions and low contrast lesions. TLG can be relatively stable in different reconstruction algorithms.

Exploration of Optimal Reconstruction Parameters for 18F-FDG Total-body PET/CT with Ultra-low Activity Injection

Abstract:PURPOSE To explorer the optimal reconstruction parameters in oncologic 18 F-FDG total-body PET/CT studies with ultra-low activity injection. METHODS A total of 204 reconstructed PET images of 34 patients with a total of 58 lesions were analyzed by two experienced nuclear medicine physicians. Images were reconstructed with ordered subset expectation maximization (OSEM) algorithm (2 and 3 iterations) including time-of-flight (TOF) and point spread function (PSF) corrections and regularization ordered subset expectation maximization (ROSEM) (b-values of 0.3, 0.4, 0.5, and 0.6). General image quality was assessed using the five-point method including overall image quality, image clarity, noise, and lesion conspicuity. Image noise, signal-to-noise ratio, lesion size, SUVmax, SUVpeak and T/N were quantitatively analyzed by the third reader who did not participate in subjective image assessment. RESULTS In objective image quality indicators, noise decreased and a continuous increase of SNR with incremental β-values (0.3,0.4, 0.5 and 0.6) compared with OSEM3. In subjective image quality, OSEM2, ROSEM0.5 and ROSEM0.6 scored higher (all P<0.001) in overall image quality, image contrast and noise. The scores of ROSEM reconstructions were all higher in lesion conspicuity compared with OSEM3 (all P<0.001). In lesion detectability, SUVmax, SUVpeak and T/N increase with β value of ROSEM increase. Compared with OSEM3, there was a negative correlation between lesion size and the percentage increase of SUVpeak in OSEM2 and ROSEM reconstructions (all P<0.01). CONCLUSION In clinical practice, we recommended OSEM reconstruction with 3 iterations with a relatively short reconstruction time and we recommend ROSEM algorithm with b of 0.5 when reconstruction time is not considered.
Changes of [ 18 F]FDG-PET/CT quantitative parameters in tumor lesions by the Bayesian penalized-likelihood PET reconstruction algorithm and its influencing factors

Abstract:Background To compare the changes in quantitative parameters and the size and degree of 1 8 F - fluorodeoxyglucose ([ 18 F]FDG) uptake of malignant tumor lesions between Bayesian penalized-likelihood (BPL) and non-BPL reconstruction algorithms. Methods Positron emission tomography/computed tomography images of 86 malignant tumor lesions were reconstructed using the algorithms of ordered subset expectation maximization (OSEM), OSEM + time of flight (TOF), OSEM + TOF + point spread function (PSF), and BPL. [ 18 F]FDG parameters of maximum standardized uptake value (SUVmax), SUVmean, metabolic tumor volume (MTV), total lesion glycolysis (TLG), and signal-to-background ratio (SBR) of these lesions were measured. Quantitative parameters between the different reconstruction algorithms were compared, and correlations between parameter variation and lesion size or the degree of [ 18 F]FDG uptake were analyzed. Results After BPL reconstruction, SUVmax, SUVmean, and SBR were significantly increased, MTV was significantly decreased. The difference values of %ΔSUVmax, %ΔSUVmean, %ΔSBR, and the absolute value of %ΔMTV between BPL and OSEM + TOF were 40.00%, 38.50%, 33.60%, and 33.20%, respectively, which were significantly higher than those between BPL and OSEM + TOF + PSF. Similar results were observed in the comparison of OSEM and OSEM + TOF + PSF with BPL. The %ΔSUVmax, %ΔSUVmean, and %ΔSBR were all significantly negatively correlated with the size and degree of [ 18 F]FDG uptake in the lesions, whereas significant positive correlations were observed for %ΔMTV and %ΔTLG. Conclusion The BPL reconstruction algorithm significantly increased SUVmax, SUVmean, and SBR and decreased MTV of tumor lesions, especially in small or relatively hypometabolic lesions.

Abstract:BackgroundF18-fluorodeoxyglucose positron-emission tomography (FDG-PET) reconstruction algorithms can have substantial influence on quantitative image data used, e.g., for therapy planning or monitoring in oncology. We analyzed radial activity concentration profiles of differently reconstructed FDG-PET images to determine the influence of varying signal-to-background ratios (SBRs) on the respective spatial resolution, activity concentration distribution, and quantification (standardized uptake value [SUV], metabolic tumor volume [MTV]).MethodsMeasurements were performed on a Siemens Biograph mCT 64 using a cylindrical phantom containing four spheres (diameter, 30 to 70 mm) filled with F18-FDG applying three SBRs (SBR1, 16:1; SBR2, 6:1; SBR3, 2:1). Images were reconstructed employing six algorithms (filtered backprojection [FBP], FBP + time-of-flight analysis [FBP + TOF], 3D-ordered subset expectation maximization [3D-OSEM], 3D-OSEM + TOF, point spread function [PSF], PSF + TOF). Spatial resolution was determined by fitting the convolution of the object geometry with a Gaussian point spread function to radial activity concentration profiles. MTV delineation was performed using fixed thresholds and semiautomatic background-adapted thresholding (ROVER, ABX, Radeberg, Germany).ResultsThe pairwise Wilcoxon test revealed significantly higher spatial resolutions for PSF + TOF (up to 4.0 mm) compared to PSF, FBP, FBP + TOF, 3D-OSEM, and 3D-OSEM + TOF at all SBRs (each P < 0.05) with the highest differences for SBR1 decreasing to the lowest for SBR3. Edge elevations in radial activity profiles (Gibbs artifacts) were highest for PSF and PSF + TOF declining with decreasing SBR (PSF + TOF largest sphere; SBR1, 6.3%; SBR3, 2.7%). These artifacts induce substantial SUVmax overestimation compared to the reference SUV for PSF algorithms at SBR1 and SBR2 leading to substantial MTV underestimation in threshold-based segmentation. In contrast, both PSF algorithms provided the lowest deviation of SUVmean from reference SUV at SBR1 and SBR2.ConclusionsAt high contrast, the PSF algorithms provided the highest spatial resolution and lowest SUVmean deviation from the reference SUV. In contrast, both algorithms showed the highest deviations in SUVmax and threshold-based MTV definition. At low contrast, all investigated reconstruction algorithms performed approximately equally. The use of PSF algorithms for quantitative PET data, e.g., for target volume definition or in serial PET studies, should be performed with caution - especially if comparing SUV of lesions with high and low contrasts.

Comparison among Reconstruction Algorithms for Quantitative Analysis of 11C-Acetate Cardiac PET Imaging

Abstract:Objective Kinetic modeling of dynamic 11C-acetate PET imaging provides quantitative information for myocardium assessment. The quality and quantitation of PET images are known to be dependent on PET reconstruction methods. This study aims to investigate the impacts of reconstruction algorithms on the quantitative analysis of dynamic 11C-acetate cardiac PET imaging. Methods Suspected alcoholic cardiomyopathy patients (N = 24) underwent 11C-acetate dynamic PET imaging after low dose CT scan. PET images were reconstructed using four algorithms: filtered backprojection (FBP), ordered subsets expectation maximization (OSEM), OSEM with time-of-flight (TOF), and OSEM with both time-of-flight and point-spread-function (TPSF). Standardized uptake values (SUVs) at different time points were compared among images reconstructed using the four algorithms. Time-activity curves (TACs) in myocardium and blood pools of ventricles were generated from the dynamic image series. Kinetic parameters K1 and k2 were derived using a 1-tissue-compartment model for kinetic modeling of cardiac flow from 11C-acetate PET images. Results Significant image quality improvement was found in the images reconstructed using iterative OSEM-type algorithms (OSME, TOF, and TPSF) compared with FBP. However, no statistical differences in SUVs were observed among the four reconstruction methods at the selected time points. Kinetic parameters K1 and k2 also exhibited no statistical difference among the four reconstruction algorithms in terms of mean value and standard deviation. However, for the correlation analysis, OSEM reconstruction presented relatively higher residual in correlation with FBP reconstruction compared with TOF and TPSF reconstruction, and TOF and TPSF reconstruction were highly correlated with each other. Conclusion All the tested reconstruction algorithms performed similarly for quantitative analysis of 11C-acetate cardiac PET imaging. TOF and TPSF yielded highly consistent kinetic parameter results with superior image quality compared with FBP. OSEM was relatively less reliable. Both TOF and TPSF were recommended for cardiac 11C-acetate kinetic analysis.
The impact of reconstruction algorithms and time of flight information on PET/CT image quality

Abstract:Abstract Background. The aim of the study was to explore the influence of various time-of-flight (TOF) and non-TOF reconstruction algorithms on positron emission tomography/computer tomography (PET/CT) image quality. Materials and methods. Measurements were performed with a triple line source phantom, consisting of capillaries with internal diameter of ~ 1 mm and standard Jaszczak phantom. Each of the data sets was reconstructed using analytical filtered back projection (FBP) algorithm, iterative ordered subsets expectation maximization (OSEM) algorithm (4 iterations, 24 subsets) and iterative True-X algorithm incorporating a specific point spread function (PSF) correction (4 iterations, 21 subsets). Baseline OSEM (2 iterations, 8 subsets) was included for comparison. Procedures were undertaken following the National Electrical Manufacturers Association (NEMA) NU-2-2001 protocol. Results. Measurement of spatial resolution in full width at half maximum (FWHM) was 5.2 mm, 4.5 mm and 2.9 mm for FBP, OSEM and True-X; and 5.1 mm, 4.5 mm and 2.9 mm for FBP+TOF, OSEM+TOF and True-X+TOF respectively. Assessment of reconstructed Jaszczak images at different concentration ratios showed that incorporation of TOF information improves cold contrast, while hot contrast only slightly, however the most prominent improvement could be seen in background variability - noise reduction. Conclusions. On the basis of the results of investigation we concluded, that incorporation of TOF information in reconstruction algorithm mostly affects reduction of the background variability (levels of noise in the image), while the improvement of spatial resolution due to incorporation of TOF information is negligible. Comparison of traditional and modern reconstruction algorithms showed that analytical FBP yields comparable results in some parameter measurements, such as cold contrast and relative count error. Iterative methods show highest levels of hot contrast, when TOF and PSF corrections were applied simultaneously.

Performance of the Iterative OSEM and HYPER Algorithm for Total-body PET at SUVmax with a Low 18F-FDG Activity, a Short Acquisition Time and Small Lesions

Abstract:Objective: The primary objective of this comparative investigation was to examine the qualitative attributes of image reconstructions utilizing two distinct algorithms, namely OSEM and HYPER Iterative, in total-body 18F- FDG PET/CT under various acquisition durations and injection activities. Methods: An initial assessment was executed using a NEMA phantom to compare image quality engendered by OSEM and HYPER Iterative algorithms. Parameters such as BV, COV, and CRC were meticulously evaluated. Subsequently, a prospective cohort study was conducted on 50 patients, employing both reconstruction algorithms. The study was compartmentalized into distinct acquisition time and dosage groups. Lesions were further categorized into three size-based groups. Quantifiable metrics including SD of noise, SUVmax, SNR, and TBR were computed. Additionally, the differences in values, namely ΔSUVmax, ΔTBR, %ΔSUVmax, %ΔSD, and %ΔSNR, between OSEM and HYPER Iterative algorithms were also calculated. Results: The HYPER Iterative algorithm showed reduced BV and COV compared to OSEM in the phantom study, with constant acquisition time. In the clinical study, lesion SUVmax, TBR, and SNR were significantly elevated in images reconstructed using the HYPER Iterative algorithm in comparison to those generated by OSEM (p < 0.001). Furthermore, an amplified increase in SUVmax was predominantly discernible in lesions with dimensions less than 10 mm. Metrics such as %ΔSNR and %ΔSD in HYPER Iterative exhibited improvements correlating with reduced acquisition times and dosages, wherein a more pronounced degree of enhancement was observable in both ΔSUVmax and ΔTBR. Conclusion: The HYPER Iterative algorithm significantly improves SUVmax and reduces noise level, with particular efficacy in lesions measuring ≤ 10 mm and under conditions of abbreviated acquisition times and lower dosages.

Abstract:2011 Purpose: To evaluate the effect of different methods of reconstructions and number of iterations on the quantitative analysis of cardiac function using cardiac gated PET. Materials and Methods: 15 patients underwent 18F-FDG cardiac PET/MR examination, PET and MRI data were collected simultaneously. The filtered back projection, the ordered subsets expectation maximization (OSEM) and the OSEM based on the point spread function (PSF-OSEM) were combined with ECG-gating to reconstruct the images. The number of iterations was defined as 1 to 8 times. PET/MR cardiac software of SIEMENS Singo via workstation were used for PET and cine image processing to obtain the left ventricular ejection fraction (LVEF) and the percentage of left ventricular wall thickness of AHA 1~16 segment from end-diastolic to end-systolic stage(Thickening). ANOVA was used to compare the difference among PET images obtained by different reconstruction methods. Pearson correlation analysis was used for the analysis of the correlation between PET and cine images. Results: There was no significant difference in the LVEFPET obtained by different reconstruction methods (P>0.5), which were in good correlation (r=0.788~0.973, P<0.05). Two iterations OSEM method achieved the highest correlation (r=0.973), which was overestimated by (0.08±17.07)% compared to LVEFMR. Only between AHA 2 and 3 segment thickeningPET obtained by different reconstruction methods was found significant difference(F=3.69,2.34,P<0.05) There was good correlation with AHA 1 and 4~16 thickeningMR(r=0.45~0.791, P<0.05). Also 2 iterations OSEM method had the highest correlation (r=0.791), overestimated by(9.08±22.69)% compared with thickeningMR. Conclusions: There was no significant difference between PET images of the LVEFPET, AHA 1 and 4~16 segments using different reconstruction methods. Cardiac PET images of LVEFPET, AHA 1 and 4~16 segments are more accurate by using 2 iteration OSEM.
Phantom and clinical assessment of small pulmonary nodules using Q.Clear reconstruction on a silicon-photomultiplier-based time-of-flight PET/CT system

Abstract:Abstract To evaluate the quantification accuracy of different positron emission tomography-computed tomography (PET/CT) reconstruction algorithms, we measured the recovery coefficient (RC) and contrast recovery (CR) in phantom studies. The results played a guiding role in the partial-volume-effect correction (PVC) for following clinical evaluations. The PET images were reconstructed with four different methods: ordered subsets expectation maximization (OSEM), OSEM with time-of-flight (TOF), OSEM with TOF and point spread function (PSF), and Bayesian penalized likelihood (BPL, known as Q.Clear in the PET/CT of GE Healthcare). In clinical studies, SUVmax and SUVmean (the maximum and mean of the standardized uptake values, SUVs) of 75 small pulmonary nodules (sub-centimeter group: < 10 mm and medium-size group: 10–25 mm) were measured from 26 patients. Results show that Q.Clear produced higher RC and CR values, which can improve quantification accuracy compared with other methods (P 0.05). The SUVs of sub-centimeter fludeoxyglucose (FDG)-avid pulmonary nodules with Q.Clear illustrated highly significant differences from those reconstructed with other algorithms (P < 0.001). After performing the PVC, highly significant differences (P < 0.001) still existed in the SUVmean measured by Q.Clear comparing with those measured by the other algorithms. Our results suggest that the Q.Clear reconstruction algorithm improved the quantification accuracy towards the true uptake, which potentially promotes the diagnostic confidence and treatment response evaluations with PET/CT imaging, especially for the sub-centimeter pulmonary nodules. For small lesions, PVC is essential.

Abstract:Background: Bayesian penalized likelihood reconstruction for PET (e.g., GE Q.Clear) aims at improving convergence of lesion activity while ensuring sufficient signal-to-noise ratio (SNR). This study evaluated reconstructed spatial resolution, maximum/peak contrast recovery (CRmax/CRpeak) and SNR of Q.Clear compared to time-of-flight (TOF) OSEM with and without point spread function (PSF) modeling. Methods: The NEMA IEC Body phantom was scanned five times (3 min scan duration, 30 min between scans, background, 1.5-3.9 kBq/ml F18) with a GE Discovery MI PET/CT (3-ring detector) with spheres filled with 8-, 4-, or 2-fold the background activity concentration (SBR 8:1, 4:1, 2:1). Reconstruction included Q.Clear (beta, 150/300/450), "PSF+TOF4/16" (iterations, 4; subsets, 16; in-plane filter, 2.0 mm), "OSEM+TOF4/16" (identical parameters), "PSF+TOF2/17" (2 it, 17 ss, 2.0 mm filter), "OSEM+TOF2/17" (identical), "PSF+TOF4/8" (4 it, 8 ss, 6.4 mm), and "OSEM+TOF2/8" (2 it, 8 ss, 6.4 mm). Spatial resolution was derived from 3D sphere activity profiles. RC as (sphere activity concentration [AC]/true AC). SNR as (background mean AC/background AC standard deviation). Results: Spatial resolution of Q.Clear150 was significantly better than all conventional algorithms at SBR 8:1 and 4:1 (Wilcoxon, each p < 0.05). At SBR 4:1 and 2:1, the spatial resolution of Q.Clear300/450 was similar or inferior to PSF+TOF4/16 and OSEM+TOF4/16. Small sphere CRpeak generally underestimated true AC, and it was similar for Q.Clear150/300/450 as with PSF+TOF4/16 or PSF+TOF2/17 (i.e., relative differences < 10%). Q.Clear provided similar or higher CRpeak as OSEM+TOF4/16 and OSEM+TOF2/17 resulting in a consistently better tradeoff between CRpeak and SNR with Q.Clear. Compared to PSF+TOF4/8/OSEM+TOF2/8, Q.Clear150/300/450 showed lower SNR but higher CRpeak. Conclusions: Q.Clear consistently improved reconstructed spatial resolution at high and medium SBR compared to PSF+TOF and OSEM+TOF, but only with beta = 150. However, this is at the cost of inferior SNR with Q.Clear150 compared to Q.Clear300/450 and PSF+TOF4/16/PSF+TOF2/17 while CRpeak for the small spheres did not improve considerably. This suggests that Q.Clear300/450 may be advantageous for the 3-ring detector configuration because the tradeoff between CR and SNR with Q.Clear300/450 was superior to PSF+TOF4/16, OSEM+TOF4/16, and OSEM+TOF2/17. However, it requires validation by systematic evaluation in patients at different activity and acquisition protocols.
Harmonization of Standard Uptake Values Across Different Positron Emission Tomography/computed Tomography Systems and Different Reconstruction Algorithms: Validation in Oncology Patients

Abstract:Background EQ.PET is a software package that overcomes the reconstruction-dependent variation of standard uptake values (SUV). In this study, we validated the use of EQ.PET for harmonizing SUVs between different positron emission tomography/computed tomography (PET/CT) systems and reconstruction algorithms. Methods In this retrospective study, 49 patients with various cancers were scanned on a Biograph mCT (mCT) or Gemini TF 16 (Gemini) after [ 18 F]FDG injections. Three groups of patient data were collected: Group 1, patients scanned on mCT or Gemini with data reconstructed using two parameters; Group 2, patients scanned twice on different PET scanners (interval between two scans, 68.9 ± 41.4 days); and Group 3, patients scanned twice using mCT with data reconstructed using different algorithms (interval between two scans, 109.5 ± 60.6 days). The SUVs of the lesions and background were measured, and the tumor-to-background ratios (TBRs) were calculated. In addition, the consistency between the two reconstruction algorithms and confounding factors were evaluated. Results In Group 1, the consistency of SUV and TBR between different reconstruction algorithms improved when the EQ.PET filter was applied. In Group 2, by comparing ΔSUV, ΔSUV%, ΔTBR, and ΔTBR% with and without the EQ.PET, the results showed significant differences ( P < 0.05). In Group 3, Bland–Altman analysis of ΔSUV with EQ.PET showed an improved consistency relative to that without EQ.PET. Conclusions EQ.PET is an efficient tool to harmonize SUVs and TBRs across different reconstruction algorithms. Patients could benefit from the harmonized SUV, ΔSUV, and ΔSUV% for therapy responses and follow-up evaluations.
Exploration of the Total-Body PET/CT Reconstruction Protocol with Ultra-Low 18F-FDG Activity over a Wide Range of Patient Body Mass Indices.

Abstract:Abstract Purpose The purpose of this study was to investigate the image quality and diagnostic performance of different reconstructions over a wide range of patient body mass indices (BMIs) obtained by total-body PET/CT with ultra-low 18F-FDG activity (0.37 MBq/kg). Methods A total of 63 patients who underwent total-body PET/CT with ultra-low activity (0.37 MBq/kg) 18F-FDG were enrolled. Patients were grouped by their BMIs. Images were reconstructed with the following two algorithms: the ordered subset expectation maximization (OSEM) algorithm (2, 3 iterations), both with time of flight (TOF) and point spread function (PSF) corrections (hereinafter referred as OSEM2, OSEM3) and HYPER Iterative algorithm (β-values of 0.3, 0.4, 0.5, 0.6) embedded TOF and PSF technologies (hereinafter referred as HYPER0.3, HYPER0.4, HYPER0.5 and HYPER0.6, respectively). Subjective image quality was assessed by two experienced nuclear medicine physicians according to the Likert quintile, including overall image quality, image noise and lesion conspicuity. The standard deviation (SD) and signal-to-noise ratio (SNR) of the liver, and maximum standard uptake value (SUVmax), peak standard uptake value (SUVpeak), tumour background ratio (T/N) and the largest diameter of lesions were quantitatively analysed by a third reader who did not participate in the subjective image assessment. Results Increased noise was associated with increased BMI in all reconstruction groups. Significant differences occurred in the liver SNR among BMI categories of OSEM reconstructions (P < 0.001) but no difference was seen in the HYPER Iterative reconstructions between any of the BMI categories (P > 0.05). With the increase in BMI, overall image quality and image noise scores decreased significantly in all reconstructions, but there was no statistically significant difference of lesion conspicuity. The overall image quality score of the obese group was not qualified (score = 2.7) in OSEM3, while the others were qualified. The lesion conspicuity scores were significantly higher in HYPER Iterative reconstructions and lower in OSEM2 than in OSEM3 (all P < 0.05). The values of SUVmax, SUVpeak and T/N in HYPER0.3, HYPER0.4 and HYPER0.5 were higher than those in OSEM3. In different reconstructions, there was a correlation between lesion size (median, 1.55 cm; range, 0.7–11.0 cm) and SUVpeak variation rate compared to OSEM3 (r = 0.388, − 0.515, − 0.495, − 0.464, and − 0.423, respectively, and all P < 0.001). Conclusion Considering the image quality and lesion analysis in 18F-FDG total-body PET/CT with ultra-low activity injection, OSEM reconstructions with 3 iterations meet the clinical requirements in patients with BMI < 30. In patients with BMI ≥ 30, it is recommended that the HYPER Iterative algorithm (β-value of 0.3–0.5) be used to ensure consistent visual image quality and quantitative assessment.

Abstract:The aim of this study was to investigate the effects of the point-spread function (PSF) and time-of-flight (TOF) on improving (18)F-FDG PET/CT images in relation to reconstruction parameters and noise-equivalent counts (NEC).METHODS: This study consisted of a phantom study and a retrospective analysis of 39 consecutive patients who underwent clinical (18)F-FDG PET/CT. The body phantom of the National Electrical Manufacturers Association and International Electrotechnical Commission with a 10-mm-diameter sphere was filled with an (18)F-FDG solution with a 4:1 radioactivity ratio compared with the background. The PET data were reconstructed with the baseline ordered-subsets expectation maximization (OSEM) algorithm, with the OSEM+PSF model, with the OSEM+TOF model, and with the OSEM+PSF+TOF model. We evaluated image quality by visual assessment, the signal-to-noise ratio of the 10-mm sphere (SNR(10 mm)), the contrast of the 10-mm sphere, and the coefficient of variance in the phantom study and then determined the optimal reconstruction parameters. We also examined the effects of PSF and TOF on the quality of clinical images using the signal-to-noise ratio in the liver (SNR(liver)) in relation to the NEC in the liver (NEC(liver)).RESULTS: In the phantom study, the SNR(10 mm) was the highest for the OSEM+PSF+TOF model, and the highest value was obtained at iteration 2 for algorithms with the TOF and at iteration 3 for those without the TOF. In terms of a postsmoothing filter full width at half maximum (FWHM), the high SNR(10 mm) was obtained with no filtering or was smaller than 2 mm for algorithms with PSF and was 4-6 mm for those without PSF. The balance between the contrast recovery and noise is different for algorithms with either PSF or TOF. A combination of PSF and TOF improved SNR(10 mm), contrast, and coefficient of variance, especially with a small-FWHM gaussian filter. In the clinical study, the SNR(liver) of the low-NEC(liver) group in the OSEM+PSF+TOF model was compared with that of the high-NEC(liver) group in conventional OSEM. The PSF+TOF improved the SNR(liver) by about 24.9% ± 9.81%.CONCLUSION: A combination of PSF and TOF clearly improves image quality, whereas optimization of the reconstruction parameters is necessary to obtain the best performance for PSF or TOF. Furthermore, this combination has the potential to provide good image quality with either lower activity or shorter acquisition time, thus improving patient comfort and reducing the radiation burden.

Abstract:Objective: The objective was to evaluate the impacts of different reconstruction methods [filtered back projection (FBP) and ordered subset expectation maximization (OSEM)] and different filters (Butterworth filter and Gaussian filter) on the image quality in cadmium-zinc-telluride (CZT)-based single photon emission computed tomography (SPECT)/computed tomography (CT) pulmonary perfusion imaging. Methods: A combinations including FBP with Butterworth filter, OSEM with Butterworth filter (OSEM + Butterworth filter ), and OSEM with Gaussian filter (OSEM + Gaussian filter) were used during SPECT image reconstruction. Visual and quantitative parameters [root mean square (RMS) noise, contrast and contrast-to-noise ratio (CNR)] were used to evaluate image quality. Results: The OSEM + Gaussian filter had better RMS noise and CNR than those of the FBP + Butterworth filter or OSEM + Butterworth filter, while the OSEM + Butterworth filter had the best contrast. The highest visual scores were obtained by OSEM + Gaussian filter ( P < 0.0001). In the lesion size <2 cm group, the contrast ( P < 0.01) and visual scores ( P < 0.001) of OSEM + Butterworth filter were better than those of the other two groups. In the lesion size ≥2 cm group, the RMS noise and visual scores of OSEM + Gaussian filter were better than those of the other two groups. Conclusion: In CZT SPECT/CT pulmonary perfusion imaging, this study recommended the clinical use of the OSEM + Gaussian filter combination for reconstruction in both conventional and larger lesions, the OSEM + Butterworth filter image postprocessing method might be advantageous in small lesions.
SUV Harmonization Between Different Hybrid PET/CT Systems

Abstract:Aim: Current PET/CT lutetium oxyorthosilicate (LSO) or lutetium-yttrium oxyorthosilicate crystal scanners are equipped with sophisticated softwares including point-spread function (PSF) and time-of-flight (TOF) image reconstruction. These softwares are associated with increased SUVs compared with 3D-OSEM reconstructions associated with older BGO PET/CT scanners. The European Association of Nuclear Medicine (EANM) identified the problem of SUV harmonization since 2010 through the EANM Research Ltd European Association of Nuclear Medicine Research Ltd [EARL] FDG PET/CT accreditation program. This required processing 2 reconstructions, one optimized for maximum spatial lesion detection and one for harmonized quantitation. We investigated an alternative single reconstruction method for both qualitative and quantitative analysis optimized to maximize spatial lesion detectability, followed by an intrinsic postreconstruction algorithm for SUV harmonization. Methods: Phantom and "in vivo" patient data analysis were acquired and analyzed on (a) a Siemens Biograph mCT system with LSO crystals and PSF and TOF algorithms, and on (b) a General Electric Discovery STE system with BGO crystals, without PSF and TOF. A dedicated algorithm (EQ.filter) was tested to harmonize SUV between the 2 scanners compared with EANM/EARL specifications. NEMA IQ phantom and a Jaszczak cylindrical phantom equipped with small fillable spheres (lesion to background ratios of 8:1 and 4:1) were used. Phantom data were validated on 7 oncologic patients with 39 hyperactive lesions ranging from 3 mm to 26 mm. Results: The main benefit of PSF + TOF LSO PET/CT systems was increased contrast for small active lesions. Recovery coefficients measured according to NEMA standards exceeded those obtained by 3D-OSEM reconstruction. SUVmax discrepancies between the 2 PET/CT systems were as high as 149%, dropping to below 10% when optimized by EQ.filter. Conclusions: A single reconstruction optimized by EQ.filter for maximum spatial lesion detectability is an easy and precise solution to harmonize SUVs between different PET/CT scanners, avoiding a second reconstruction with an additional smoothing filter as requested by EANM/EARL.
Quantitative evaluation of PSMA PET imaging using a realistic anthropomorphic phantom and shell-less radioactive epoxy lesions

Abstract:Background: Positron emission tomography (PET) with prostate specific membrane antigen (PSMA) have shown superior performance in detecting metastatic prostate cancers. Relative to [18F]fluorodeoxyglucose ([18F]FDG) PET images, PSMA PET images tend to visualize significantly higher-contrast focal lesions. We aim to evaluate segmentation and reconstruction algorithms in this emerging context. Specifically, Bayesian or maximum a posteriori (MAP) image reconstruction, compared to standard ordered subsets expectation maximization (OSEM) reconstruction, has received significant interest for its potential to reach convergence with minimal noise amplifications. However, few phantom studies have evaluated the quantitative accuracy of such reconstructions for high contrast, small lesions (sub-10 mm) that are typically observed in PSMA images. In this study, we cast 3 mm-16-mm spheres using epoxy resin infused with a long half-life positron emitter (sodium-22; 22Na) to simulate prostate cancer metastasis. The anthropomorphic Probe-IQ phantom, which features a liver, bladder, lungs, and ureters, was used to model relevant anatomy. Dynamic PET acquisitions were acquired and images were reconstructed with OSEM (varying subsets and iterations) and BSREM (varying β parameters), and the effects on lesion quantitation were evaluated. Results: The 22Na lesions were scanned against an aqueous solution containing fluorine-18 (18F) as the background. Regions-of-interest were drawn with MIM Software using 40% fixed threshold (40% FT) and a gradient segmentation algorithm (MIM's PET Edge+). Recovery coefficients (RCs) (max, mean, peak, and newly defined "apex"), metabolic tumour volume (MTV), and total tumour uptake (TTU) were calculated for each sphere. SUVpeak and SUVapex had the most consistent RCs for different lesion-to-background ratios and reconstruction parameters. The gradient-based segmentation algorithm was more accurate than 40% FT for determining MTV and TTU, particularly for lesions [Formula: see text] 6 mm in diameter (R2 = 0.979-0.996 vs. R2 = 0.115-0.527, respectively). Conclusion: An anthropomorphic phantom was used to evaluate quantitation for PSMA PET imaging of metastatic prostate cancer lesions. BSREM with β = 200-400 and OSEM with 2-5 iterations resulted in the most accurate and robust measurements of SUVmean, MTV, and TTU for imaging conditions in 18F-PSMA PET/CT images. SUVapex, a hybrid metric of SUVmax and SUVpeak, was proposed for robust, accurate, and segmentation-free quantitation of lesions for PSMA PET.

Abstract:Background Positron emission tomography (PET) response criteria in solid tumours (PERCIST 1.0) is a systematic assessment of therapy response with fluorine 18 fluorodeoxyglucose ([18F]FDG) PET. The ordered subset expectation maximization (OSEM) and block sequential regularized expectation maximization (BSREM) reconstruction algorithms are both commonly used. The impact of using OSEM or BSREM on PERCIST 1.0 is not fully determined. This study aimed to compare treatment response evaluation between the reconstructions OSEM and BSREM according to PERCIST 1.0. Methods The highest peak standardized uptake value corrected for lean body mass (SULpeak) in a lesion were measured in PET‐CT scans from 90 patients with metastatic breast cancer or malignant melanoma, who had undergone two [18F]FDG PET with computer tomography (CT) scans for treatment evaluation purpose. SUL measurements in the reference organ were also calculated. All scans were reconstructed with OSEM and BSREM. Bland Altman plots and Spearman correlation coefficient were used for comparing measurements obtained from OSEM and BSREM images. Patients were categorized as a complete metabolic response (CMR), partial metabolic response (PMR), stable metabolic disease (SMD), or progressive metabolic disease (PMD). Results The mean standard deviation (SD) of SULmean in the reference organ was slightly lower for BSREM images, resulting in a slightly lower threshold. Lesion SULpeak was slightly higher for BSREM images compared with OSEM images. Only 5 out of 90 patients had different response evaluations for the different reconstructions. Conclusion PERCIST 1.0 serves as a robust response evaluation criterion and is only marginally dependent on the reconstruction used.

Abstract:Background The maximum likelihood activity and attenuation (MLAA) reconstruction algorithm has been proposed to jointly estimate tracer activity and attenuation at the same time, and proven to be a promising solution to the CT attenuation correction (CT-AC) artifacts in PET images. This study aimed to perform a quantitative evaluation and clinical validation of the MLAA method. Methods A uniform cylinder phantom filled with 18 F-FDG solution was scanned to optimize the reconstruction parameters for the implemented MLAA algorithm. 67 patients who underwent whole-body 18 F-FDG PET/CT scan were retrospectively recruited. PET images were reconstructed using MLAA and clinical standard OSEM algorithm with CT-AC (CT-OSEM). The mean and maximum standardized uptake values (SUVmean and SUVmax) in regions of interest (ROIs) of organs, high uptake lesions and areas affected by metal implants and respiration motion artifacts were quantitatively analyzed. Results In quantitative analysis, SUVs in patient’s organ ROIs between two methods showed R 2 ranging from 0.91 to 0.98 and k ranging from 0.90 to 1.06, and the average SUVmax and SUVmean differences between two methods were within 10% range, except for the lung ROI, which was 10.5% and 16.73% respectively. The average SUVmax and SUVmean differences of a total of 117 high uptake lesions were 7.25% and 7.10% respectively. 20 patients were identified to have apparent respiration motion artifacts in the liver in CT-OSEM images, and the SUVs differences between two methods measured at dome of the liver were significantly larger than measured at middle part of the liver. 10 regions with obvious metal artifacts were identified in CT-OSEM images and the average SUVmean and SUVmax differences in metal implants affected regions were reported to be 52.90% and 56.20% respectively. Conclusions PET images reconstructed using MLAA are clinically acceptable in terms of image quality as well as quantification and it is a useful tool in clinical practice, especially when CT-AC may cause respiration motion and metal artifacts. Moreover, this study also provides technical reference and data support for the future iteration and development of PET reconstruction technology of SUV accurate quantification.

Abstract:Objective To evaluate the image quality of SPECT Hoffmann phantom using different reconstruction algorithms.Methods Data sampling of line-source and Hoffmann phantom were acquired with parallel-hole low energy and high resolution collimator by a dual-head SPECT system.The images were reconstructed by both filtered backprojection (FBP) and ordered-subsets expectation maximization (OSEM) algorithms.The spatial resolution of line resources was evaluated by the full width at half maximum (FWHM) value.The image quality of Hoffmann phantom was visually accessed by 3 doctors.The reconstruc-tion time was recorded and region of interest (ROI) was indentified on the reconstructed Hoffmann phantom images.Independent samples t-test was used to compare the ratios of counts/pixel of target ROI to cerebellum ROI by different reconstruction algorithms using software SPSS 15.0.Results The FWHM values for FBP and OSEM were 18.77 mm and 12.62 mm, respectively.The visual assessment suggested that OSEM could provide better overall image quality, better edge delineation of candate, putamen and striatum than FBP.The reconstruction time of FBP and OSEM was 80 s and 100 s respectively, which was acceptable in routine clini-cal procedure.The ratios of counts/pixel of target ROI to cerebellum ROI generated by FBP and OSEM showed no significant difference (t=-0.332, P=0.750).Conclusion OSEM algorithm could provide bet-ter spatial resolution and image quality than FBP with an acceptable increase in reconstruction time.
