Abstract:Computational modeling, owing to its advantages over traditional in vivo or in vitro measurements in high-precision quantification of key variables, integrative analysis of multiple factors, and flexible adaptability to various clinical scenarios, represents a promising means for addressing biomechanical problems associated with the diagnosis or treatment of cardiovascular/musculoskeletal diseases. The advancements in computational methods and substantial improvement of computing capability in the past decades have greatly promoted the conversion of the objectives of numerical studies from investigating general biomechanical phenomena to providing quantitative insights for aiding patient-specific diagnosis/treatment or medical device design. In this special issue, we collect some recent works in this field. The papers in the special issue address a wide range of problems spanning the diagnosis or treatment of vascular diseases, optimization of artificial heart valves, and evaluation of orthokeratology lens therapy. Among the eight papers focused on biomechanical problems, two papers are devoted to improving the modeling methods for predicting coronary fractional flow reserve based on medical images1, 2; two papers investigate the influences of vascular anatomy, operative method, or device selection on the risk of complications following interventional treatments of aortic arch aneurysms or abdominal aortic aneurysms3, 4; two papers address hemodynamic changes induced by the interventional treatment or drug therapy of venous diseases5, 6; one paper is dedicated to optimizing the fatigue performance of polymeric heart valve7; and one paper explores the biomechanical responses of cornea to orthokeratology lens therapy.8 In addition, there is one paper dealing with medical image processing where a new algorithm is proposed to segment pelvic bone tumors based on multimodal images.9 Most studies in this special issue build the computational models based on patient-specific clinical data, which helps strengthen the clinical implications of the numerical results. Nevertheless, there is still much space for further improvement. Challenging issues in this field include, but are not limited to, how to improve the level of model personalization, how to accurately predict the prognosis of disease, how to minimize the gap between model-based theoretical findings and requirements in clinical practice, and how to build digital twin models on the timeline of disease progression to capture the key factors in play.
