Abstract:Objective: To evaluate dual-energy CT (DECT) findings of pulmonary ischaemic-reperfusion injury (PIRI) and its pathophysiological correlation in the canine model.Methods: APIRI model was established in 11 canines, utilizing closed pectoral balloon occlusion. Two control canines were also included. For the PIRI model, the left pulmonary artery was occluded with a balloon, which was deflated and removed after 2h. DECT was performed before, during occlusion and at 2, 3 and 4h thereafter and was utilized to construct pulmonary perfusion maps. Immediately after the CT scan at the fourth hour post reperfusion, the canines were sacrificed, and lung specimens were harvested for pathological analysis. CT findings, pulmonary artery pressure and blood gas results were then analysed.Results: Data at every time point were available for 10 animals (experimental group, n = 8; control group, n = 2). Quantitative measurements from DECT pulmonary perfusion maps found iodine attenuation values of the left lung to be the lowest at 2h post embolization and the highest at 1 h post reperfusion. In the contralateral lung, perfusion values also peaked at 1 h post reperfusion. Continuous hypoxia and acid-based disorders were observed during PIRI, and comprehensive analysis showed physiological changes to be worst at 3 h post reperfusion.Conclusion: DECT pulmonary perfusion mapping demonstrated pulmonary perfusion of the bilateral lungs to be the greatest at 1 h post reperfusion. These CT findings corresponded with pathophysiological changes.Advances in knowledge: DECT pulmonary perfusion mapping can be used to evaluate lung ischaemia-reperfusion injury.

Abstract:Objective To evaluate CT findings and associated pathophysiological basis of pulmonary ischemic reperfusion injure(PIRI) in dog models with the closed pectoral balloon embolization method.Materials and Methods Eleven health hybrid dogs were selected for establishing PIRI model.Right femoral vein was punctured with Seldinger method and left pulmonary artery was blocked with the balloon for 2 hours.CT plain scan was performed,pulmonary arterial pressure were measure and blood sample was collected at 6 time points,including pre embolisation,2 h after establism,1 h,2 h,3 h,4 h after reperfusion.Immediately after CT scan at 4 h after reperfusion,the dogs were sacrificed and lung specimens were harvested.Dynamic CT findings,pulmonary artery pressure,blood gas results and histopathological findings were analyzed.Results PIRI models were successfully made in 10 dogs,8 dogs were induced into final analysis.Bilateral lungs were normal on pre embolisation CT images.At 2h after embolisation,the left lungs had decreased opacification and the right lungs occurred different degree of ground glass opacification in 8 dogs.During reperfusion process,bilateral lungs appeared to different degree of ground glass opacification in 8 dogs,and did not recover until 4h after reperfusion.The contralateral lung did not show homogeneous change after reperfusion,while the embolized lungs showed the most serious damage at 3-4 h after reperfusion(n=7).Comprehensive evaluation showed the left lung lesions were slightly heavier than the right lung in 4 dogs,the right lung lesions slightly heavier than the left lung in other 4 dogs.Pathological examination showed the widened alveolar interval and hyperemia capillaries in bilateral lung tissues.Pathological findings agreed with CT imaging on severity of lung lesions.Continuous hypoxia and acid base disorders were represented during PIRI.But from embolisation to 4h after reperfusion,blood gas results had no statistically significant(P0.05),comprehensive analysis showed the pathophysiological changes were worst at 3 h after reperfusion.Conclusion Different degree of ground-glass opacification was main CT finding of PIRI in canine models,which will persist at least 4h after reperfusion.These CT findings were concord with pathophysiological changes,such as pulmonary capillary hyperemia,hypoxemia,and acid base disorders.

Abstract:Objective To observe the changes in the transforming growth factor-beta 1 (TGF-β1 ) mRNA expression in the lung in a dog model of cardiopulmonary bypass(CPB)-induced acute lung injury. Methods Thirty-six healthy adult mongrel dogs of both sexes weighing 15-16 kg were randomly assigned into control group and CPB group ( n = 18 each) . Lung injury was produced by CPB according to the method described by Williams. Six animals were killed at each of the following time points: before CPB (T0 ) and 30 and 60 min after termination of CPB (T1 , T2) in each group. Lung specimens were obtained for microscopic examination and determination of TGF-β1 mRNA expression (by RT-PCR) and MDA content. The lungs were lavaged and the protein concentration in the brancho-alveolar lavage fluid (BALF) was determined and pulmonary permeability index (PPI) was calculated. Results Microscopic examination showed massive inflammatory cell infiltration, alveolar capillary dilatation, congestion, widened alveolar septum, massive RBC in the alveolar space and focal atelectasis in the lung in CPB group. The TGF-β1 mRNA expression and MDA content and PPI were significantly higher in CPB group than in control group. The TGF-β1 mRNA expression and MDA was positively correlated to PPI (MDA: r = 0.867, P < 0.01; PPI: r = 0.821, P < 0.01) . Conclusion TGF-β1 mRNA expression in the lung is significantly up-regulated after CPB and is an important factor contributing to CPB-induced acute lung injury.

Abstract:Objective To observe the QRS morphology of electrocardiographic Ⅱ lead in ischemia-reperfusion(I/R)injured canine myocardium by silk suture ligation,and to explore its significance in stem cell transplantation research.Method 24 healthy adult dogs were randomly divided into 3 groups and myocardial ischemia-reperfusion injuries were established by ligating the middle-distal 1/3 segment of left anterior discending artery(LAD)followed by opening the blood flow at 30,60 and 90 minutes later,respectively.Electrocardiogram(ECG)was measured from the beginning of the LAD ligation to more than 30 min after LAD reperfusion by using a MPA-2000 biosignal analytical system.Results Absence of Rs complexes in the upright QRS was showed at the beginning of the LAD ligation.After the LAD reperfusion,the QRS morphology evolved from the rS or QS complexes with wide and deep S waves,which occurred at the beginning of the LAD reperfusion in 83.3%(20/24)of the dogs,into the upright Rs complexes with normal QRS interval gradually.The evolvement rate were 25.00%(6/24)at 15 min after LAD reperfusion,41.67%(10/24)at 30 min after LAD reperfusion and 16.67%(4/24)at more than 30 min after LAD reperfusion,respectively.There was a statistically significant correlation between the evolvement rate of the QRS complex and the LAD blocked period.Conclusion There are certain regularly evolutionary changes of electrocardiogram in I/R injured canine myocardium.The QRS wave changes may become an useful direct indicator in the judgment of protective effect and functional recovery of the injured myocardium.
Protective Effects of Ischemic Precondition on Graft Lung in Canine

Abstract:Using canine lung transplantation model, the protective effects of ischemic precondition on ischemic reperfusion lung injury were investigated. Twelve adult donor canines were randomly divided into control group (n=6) and ischemic precondition group (n= 6). The donor left lung hilum in ischemic precondition group was subsequently clamped for 10 rain, and then released for 15 min, but not in control group. After being preserved in 4 ℃ modified Euro-Collins solution,the donor lung was transplanted into the left chest of allograft recipients. The results showed that, as compared with control group, the content changes in superoxide dismutase (SOD) and malondialdehyde (MDA) were significant in statistics 2 h after transplantation. It was suggested that lung ischemic precondition might increase endogenous anti-oxidative ability, and inhibit the production of oxygen free radical following ischemic reperfusion. It was also demonstrated that ischemic precondition combined with cold Euro-Collins solution exerted an evidently protective effect on graft lung from canine donor.

Abstract:Objective: To study the effect of ischemia-reperfusion injury（IRI） and hepatic artery ischemia（HAI） on CD14 expression in canine auto-transplantation livers. Methods:Liver orthotopic auto-transplantation models were applied with 30 healthy male Xi’an canines which were randomly divided into a control group, simultaneous reperfusion（SR） group and HAI group. CD14 protein expression, Malonaldehyde （MDA） Contents in hepatic tissues and ALT values in serum were detected, and the pathological changes of hepatic tissues was investigated under the light microscopy. Results:The level of CD14 protein expression in SR and HAI group tended to be time-dependent and both higher than controls with statistical significance（P ＜ 0.01）; The peak values of these two groups both occurred at 4 h, but the level in HAI group （11.94± 0.43） was evidently higher than that in SR group（3.04± 0.34）. MDA contents in liver tissue, ALT values in serum and pathological changes showed the same changing tendency as CD14 expression. Conclusion:① Up-regula- tion of CD14 expression may be the receptor-mechanism of Kupffer cells（KCs） activation in liver transplantation. ② HAI can up- regulate CD14 expression after portal vein reperfusion, improve the activity of KCs further more, increase OFRs production and coop- erate with portal reperfusion, and finally aggravate the grafts injury.
Computer Tomographic Assessment of Partial Liquid Ventilation in Dogs with Inhalation Injury

Abstract:Objective To analyze CT and pathologic findings of lung in dogs with inhalation injury,and to provide reliable basis to explore the mechanism of partial liquid ventilation (PLV) treatment on inhalation injury.Methods Respiratory tract injury models in 14 dogs were made by steam inhalation and they were divided into 2 groups.The control group was treated by high frequency ventilation (HFV) and the treatment group was treated by partial liquid with perfluorocarbon plus HFV.CT scan of the lung in dogs with inhalation injury was performed before and after perfluorocarbon (PFC) PLV,and then the histopathologic pieces with isolation lung were obtained.Analysis and comparison of the results among them was conducted.Results After steam inhalation injury,CT appeared as exudation with patchy and mass in 6 dogs,cotton-wool spots in 2 dogs,ground-glass opacity in 2 dogs,and consolidation in 4 dogs.CT appeared as mass in 5 dogs with inhalation injury of perfluorocarbon-filled lungs during partial liquid ventilation,patchy in 2 dogs,and homogeneous in 1 dog.The distribution of perfluorocarbon was in the posterior region of lung.The CT findings were in accordance with those in histopathology.Conclusion Perfluorocarbon is distributed predominantly in the dependent regions of the lung and the distribution of perfluorocarbon is gravity dependent.It is benefit for the inhalation injury.
Canine Study on Myocardial Ischemic Memory with 18F-FDG PET/CT Imaging

Abstract:Objective To explore whether the existence and duration of ischemia measured by dynamic 18F-FDG PET/CT imaging correlated with the extent of myocardial ischemia in a canine model of myocardial ischemia-reperfusion.Methods Canine coronary artery occlusion was carried out for 20 min (n =4) and for 40 min (n =4) followed by 24 h of open-artery reperfusion.All dogs underwent dynamic 18F-FDG PET/CT and 99Tcm-MIBI SPECT imaging at baseline and 1 h and 24 h after reperfusion.Quantitative analysis of myocardial 18F-FDG uptake was performed using Carimas Core software,and the extraction ratio of 18F-FDG (K) was calculated by the ratio of 18F-FDG uptake rate in the ischemic area (kischemia) and normoperfused region (knormoperfused).Echocardiographic data were also acquired between each PET/CT imaging study to detect the wall motion in the ischemic and normoperfused myocardium.Paired t test and non-parametric statistical tests,measured by SPSS 19.0,were used to analyze the data.Results Coronary occlusion produced sustained,abnormal wall motion in the ischemic region for more than 1 h.Similar K values were demonstrated between the 20 min and 40 min groups at baseline (1.02 ±0.06 and 1.03 ±0.05,Z =-0.29,P ＞0.05).At 1 h after reperfusion,the reperfusion regions showed normal perfusion but with increased 18F-FDG uptake,which was higher in the 40 min ischemic group than in the 20 min ischemic group (2.31 ±0.13 and 1.87 ±0.09,Z=-2.31,P＜0.05).At 24 h after reperfusion,however,only the 40 min ischemic group showed slightly higher 18F-FDG uptake than baseline (1.15 ± 0.02 and 1.03 ±0.05,t =4.32,P ＜ 0.05),whereas no significant difference was found in the 20 min ischemic group (1.05 ± 0.04and 1.02 ± 0.06,t =0.87,P ＞ 0.05).Histological examination of the ischemic myocardium from both groups revealed neatly arranged cells without interstitial edema,hemorrhage nor inflammatory response.Conclusions Myocardial ischemic memory was correlated with the extent of myocardial ischemia.18F-FDG imaging of ischemic memory may have important value in the diagnosis and treatment of patients with acute coronary syndrome. Consecutive CT-guided Core Needle Tissue Biopsy of Lung Lesions in the Same Dog at Different Phases of Radiation-Induced Lung Injury

Abstract:This project aimed to set up a Beagle dog model of radiation-induced lung injury in order to supply fresh lung tissue samples in the different injury phases for gene and protein research. Three dogs received 18 Gy X-ray irradiation in one fraction, another three dogs received 8 Gy in each of three fractions at weekly intervals, and one control dog was not irradiated. Acute pneumonitis was observed during the first 3 months after radiation, and chronic lung fibrosis was found during the next 4-12 months in all the dogs exposed to radiation. CT-guided core needle lung lesion biopsies were extracted from each dog five times over the course of 1 year. The dogs remained healthy after each biopsy, and 50-100 mg fresh lung lesion tissues were collected in each operation. The incidence of pneumothorax and hemoptysis was 20% and 2.8%, respectively, in the 35 tissue biopsies. A successful and stable radiation-induced lung injury dog model was established. Lung lesion tissue samples from dogs in acute stage, recovery stage and fibrosis stage were found to be sufficient to support cytology, genomics and proteomics research. This model safely supplied fresh tissue samples that would allow future researchers to more easily explore and develop treatments for radiation-induced lung injury.
Impact of lung ischemic preconditioning on myocardium in canine cardiopulmonary bypass

Abstract:Objactive To explore the mechanism of protection effect of lung ischemic preconditioning on myocardium in canine cardiopulmonary bypass(CPB).Methods 12 adult mongrel canines(12-15 kg)were randomly divided into two groups,6 for Control(Group C),in which the CPB was established by abdominal aorta and right atrium cannulation,experimental group was treated with ischemic preconditioning,in which left lung hilus were clamped for two cycles of five minutes followed five minutes released before cross-clamping of the aorta in 6 canines(Group IP).CPB was controlled running for 1 hour in both groups.The myocardial tissue were sampled at the time of pre-CPB and 120 minutes after reperfusion to test myocardial water content(MWC),maleic dialdehyde(MDA)and superoxid dismutase(SOD).Myocardial biopsies were obtained before and after CPB in both groups for microscope pathological examination.Results After reperfusion,the MWC and MDA of myocardium in Group IP was significantly lower than that in Group C;SOD activity of myocardium in Group IP was significantly higher than Group C;pathological examination of myocardial tissue revealed that there were more obvious intraalveolar hemorrhage,leukocyte adherence and interstitial edema in Control group compared with group IP.Conclusion The study indicates that lung ischemic preconditioning may have protective effect on myocardium after CPB.The possible mechanism was that ischemic preconditioning reduced the production of ischemia-reperfusion injury.
Diagnosis of Acute Myocardial Ischemic Reperfusion Injury by 320-Row Dynamic Volume CT Perfusion Imaging:An Experimental Study in Swine

Abstract:Objective To investigate whether 320-row CT myocardial perfusion imaging(CTP)is an accurate diagnosis method of acute myocardial ischemia reperfusion injury in a swine model.Methods The distal left anterior descending coronary artery(LAD)of 8 swine heart were clogged half an hour by balloon catheter and opened up 1.5 hours,and then the acute myocardial ischemic reperfusion injury model was created and was detected by CTP.Pigs were sacrificed after the inspection,and the hearts were removed,triphenyhetrazolium chloride staining was performed.The CT numbers of injuries and non-injuries regions were measured.In the short axis of the left ventricle,the ventricular wall was divided into 17 segments for analysis,segments with myocardial perfusion defect in myocardial transmural perfusion ratio(TPR)maps,two-dimension imaging were determined and compared with histopathology.The sensitivity,specificity and inter-modality agreement of CTP in detecting myocardial injury were calculated.Paired t test was used to analyze the differences of the CT number between injuries and normal regions in two-dimension imaging.Results Partial sparse or defective perfusion in the apical anterior and septal wall were demonstrated in TPR maps.The CT number of injured myocardium [(56.23±8.68)HU]was significantly lower than that of normal myocardium [(120.57±15.36)HU]in two-dimension imaging.The sensitivity,specificity of TPR maps were 89.3%(25/28),93.5%(101/108),and Kappa value was 0.79.The sensitivity,specificity of two-dimension imaging were 85.7%(24/28),92.6%(100/108),and Kappa value was 0.74.Conclusions DVCT myocardial perfusion imaging can detect acute myocardial ischemic reperfusion injury in a swine model and have a good correlation with histopathology.
FR167653 ameliorates pulmonary damage in ischemia-reperfusion injury in a canine lung transplantation model

Abstract:Background: Interleukin (IL)-1 and tumor necrosis factor-alpha (TNF-alpha) are recognized as important factors in ischemia-reperfusion (I/R) injury. FR167653 has been characterized as a potent suppressant of IL-1 and TNF-alpha production. We previously reported that FR167653 suppressed the expression of IL-1 beta mRNA after reperfusion and ameliorated pulmonary I/R injury following 3-hour left lung warm ischemia in dogs. The aim of this study was to investigate the effects of FR167653 on I/R injury in a canine left, single, lung transplantation model. Methods: We used 10 pairs of weight-matched dogs. We assigned 5 pairs to the FR group, in which each animal received FR167653 (1 mg/kg/hr) IV from 30 minutes before ischemia until 2 hours after reperfusion; we treated the transplanted lungs with FR167653 after the onset of reperfusion. The others were assigned to the control group. After 8-hour preservation with 4 degrees C Euro-Collins solution, orthotopic left, single, lung transplantation was performed. During a 5-minute clamping test at the right pulmonary artery of each recipient, the left (transplanted) pulmonary arterial pressure (L-PAP), left (transplanted) pulmonary vascular resistance (L-PVR), arterial oxygen pressure (PaO(2)), and alveolar-arterial oxygen pressure difference (A-aDO(2)) were measured. We harvested transplanted lung specimens for histologic study, and we counted polymorphonuclear neutrophils (PMNs), which were identified by staining with naphthol AS-D cholroacetate esterase. Pulmonary perfusion and ventilation scintigraphy (Tc-99m-MAA and Xe-133) were performed. We observed the animals for 3 days after transplantation. Results: The PAP, L-PVR, PaO(2), and A-aDO(2) revealed significantly (p < 0.05) better function in the FR group than in the control group. Histologically, lung edema was milder, and PMN infiltration was significantly (p < 0.05) lower in the FR group than in the control group. Xe-133 and Tc-99m-MAA were widely distributed throughout the graft lung in the FR group. Three-day survival rates in FR and control groups were 60% and 20%, respectively. Conclusions: FR167653 appears to generate a protective effect on I/R injury in lung transplantation in dogs.
Acute Myocardial Infarct Detection with Dual Energy CT: Correlation with Single Photon Emission Computed Tomography Myocardial Scintigraphy in a Canine Model.

Abstract:BACKGROUND:Dual-energy CT (DECT) has been used to detect myocardial infarct. However, few comparable studies with histopathological findings as gold standard have been published.PURPOSE:To investigate the accuracy of DECT iodine maps for detecting acute myocardial infarction compared with single photon emission computed tomography (SPECT) in a canine model using histopathological findings as the reference standard.MATERIAL AND METHODS:A model of myocardial ischemia was created by ligating the left anterior descending (LAD) coronary artery after thoracotomy in six dogs, while another three dogs undergoing thoracotomy without LAD ligature served as a control group. Contrast-enhanced DECT scans of the heart were performed, followed by resting 99mTc-MIBI SPECT myocardial perfusion imaging in all nine dogs before and 3 h after the procedure. Triphenyltetrazolium chloride (TTC) staining was performed and analyzed. In the short axis of the left ventricle, the wall surface was divided into 17 segments, which were assessed for infarcted myocardium on conventional CT from average-weighted data, DECT myocardial iodine maps, conventional CT plus DECT, SPECT, and histopathology. Inter-observer and inter-modality agreement for conventional CT, DECT myocardial iodine maps, and SPECT were calculated. CT value of infracted and non-infracted areas was measured.RESULTS:With the histopathological results as the reference standard, the sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and accuracy were 75.0% (30/40), 92.0% (104/113), 76.9% (30/39), 91.2% (104/114), 87.6% (134/153) for conventional CT, 85.0% (34/40), 84.1% (95/113), 65.4% (34/52), 94.1% (95/101), 84.3% (129/153) for DECT myocardial iodine maps; 87.5% (35/40), 92.9% (105/113), 81.4% (35/43), 95.5% (105/110), 91.5% (140/153) for conventional CT plus DECT; 82.5% (33/40), 90.3% (102/113), 75.0% (33/44), and 93.6% (102/109), 88.2% (135/153) for SPECT, respectively. Excellent inter-observer agreement (Kappa value >0.8) and good inter-modality agreement (Kappa value >0.6) for each modality were found. CT values of infarcted myocardium (26 ± 22 HU, 36 ± 33 HU, 34 ± 16 HU) were lower than those of non-infarcted myocardium (115 ± 16 HU, 121 ± 28 HU, 123 ± 11 HU) on images of 140 kVp, 80 kVp, and average-weighted 120 kVp images (all P < 0.05).CONCLUSION:With histopathology as the reference standard, DECT myocardial iodine maps can detect acute myocardial infarction with diagnostic accuracy comparable to resting SPECT myocardial perfusion imaging in a canine model. DECT plus conventional CT had a potential to improve the detection of acute myocardial infarction.
Temporal Changes of Inflammatory Cytokine Profiles in Epithelial Lining Fluid in a Canine Lung Transplant Model

Abstract:Objectives: Ischemia-reperfusion injury resulting in post-transplant lung dysfunction involves a complicated network of cytokines that has yet to be fully investigated. We analyzed temporal changes in cytokine levels in epithelial lining fluid (ELF) from the distal airways of a canine lung transplantation model. Methods: Ischemic time was set to 18 hours to enhance lung injury in a left single-lung transplantation model. ELF was collected via bronchoscopic microsampling, a minimally-invasive technique allowing repeated sampling, hourly up to 5 hours after reperfusion started. We compared levels of pro-inflammatory cytokines in ELF with those at baseline (time zero), and with a sham-operated control group. Results: Concentrations of tumor necrosis factor alpha (TNF-α) and interleukin 6 (IL-6) significantly increased immediately after the start of lung reperfusion in the transplant group relative to the sham group (P < 0.005 and P < 0.05, respectively); both were sustained through the 5 hours. Interferon gamma (IFN-γ levels were significantly reduced 3 h after reperfusion started (P < 0.05). Conclusions: We found time- and cytokine-dependent changes in TNF-α, IL-6, and IFN-γ in ELF from the lung transplantation model. These specific changes in the cytokine profile may relate to the mechanism underlying post-transplant lung dysfunction.

Abstract:Objective To investigate the effect of partial liquid ventilation(PLV) lung histopathology in dogs with halation injuries.Methods Sixteen dogs with stream inhalation injury were randomly divided into control group and treatment group.All the dogs received conventional mechanical ventilation.In PLV group,12 ml/kg perfluorodecalin was slowly injected into lungs at 1 5 h after injury.All the dogs were killed at 3 h after injury.Lung histopathological changes were observed with the optical microscope and electron microscope.Results The pathological changes of congestion,edema and neurophile granulocyte infiltration of lung in PLV group was less than those in the control group.The PLV group had less lesios in angioendothelial cells and alveolar cells.Conclusion PLV may allieviate the histopathological changes after inhalation injury and has anti-inflammatory function.

Abstract:BACKGROUND:Utilize quantitative computed tomography (QCT) to detect and evaluate the severity of lung injury after successful cardiopulmonary resuscitation (CPR) in a porcine cardiac arrest (CA) model with different downtimes.METHODS:Twenty-one male domestic pigs weighing 38±3 kg were randomized into 3 groups: the sham group (n=5), the ventricular fibrillation (VF) 5 min (VF5) group (n=8), and the VF 10 min (VF10) group (n=8). VF was induced and untreated for 5 (VF5 group) or 10 (VF10 group) min before the commencement of manual CPR. Eight animals (8/8, 100%) in VF5 and 6 (6/8, 75%) in VF10 were successfully resuscitated. Chest QCT scans and arterial blood gas tests were performed at baseline and 6 h post-resuscitation. The QCT score, volume, and weight of ground-glass opacification (GGO), which was defined as poorly aerated regions with a CT value ranging from -500 Hounsfield units (HU) to -100 HU, and intense parenchymal opacification (IPO), which was defined as a non-aerated area with a CT value greater than -100 HU, were quantitatively measured.RESULTS:Significantly shorter durations of CPR and fewer defibrillations were observed in the VF5 group compared with the VF10 group [duration of CPR: VF5 (6±0 minutes) versus VF10 (8.3±1.5 minutes), P<0.05; numbers of defibrillation: VF5 (1±0) versus VF10 (2.2±0.8), P<0.05]. Compared with the baseline or sham animals, declining gas exchanges (end-tidal CO2, PO2, oxygen index) were observed in both VF groups; however, there were no significant differences in gas exchanges between the VF groups. Compared with the VF5 group, the GGO QCT score, volume, and weight were significantly greater in the VF10 group (P=0.002, 0.001, and 0.002 respectively), while no significant differences were found in the IPO QCT score, volume, or weight between two the VF groups (P=0.354, 0.447, and 0.512 respectively).CONCLUSIONS:QCT analysis enables unique non-invasive assessments of different lung injuries (IPO and GGO lesions) that can clearly distinguish heterogeneous lesions and allow for early detection and quantitative monitoring of the severity of lung injury following CPR. QCT could provide a basis for clinical early ventilation strategy management after CPR.

Abstract:Objective To assess the ability and limitation of detecting acute myocardial infarction with electron beam computed tomography (EBCT) and the importance of reprocessing the images with computer Methods The infarction was created in six dogs Enhancement scans were made with an EBCT scanner at 1 5, 6, 12 and 20 h after the infarction and CT sectional images of dog heart were gotten After the final scan the dogs were sacrificed The dog hearts were massively sectioned and stained with nitro blue tetrazolium (NBT) The massive sections were photographed before and after staining and the samples for optic microscopy were collected from it The CT images and pathologic photographs were loaded into computer for processing and infarction size was calculated and compared between the two methods Results The site and shape of acute myocardial infarction were clearly showed in the EBCT images The infarction size slightly lessened after ligation as time prolonging The infarction size calculated from the final EBCT scan was similar to that on the stained pathological sections with a correlation coefficient of 0 994 ( P 0 001) Conclusion The site, shape and size of acute myocardial infarction can be accurately assessed by EBCT The accuracy about the size of focus may be improved with computer reprocessing

Abstract:Objective To investigate the feasibility and accuracy of dual energy CT myocardial iodine maps in detecting acute myocardial infarction in canine model. Methods Myocardial ischemia model was made by ligaturing left anterior descending coronary arteries (LAD) after thoracotomy in six dogs, while another 3 dogs undergoing thoracotomy not ligaturing LAD as control group. Before and three hours after operation, dual-source CT (DSCT) was performed, followed by resting 99Tcm-MIBI single photon emission computed tomography myocardial perfusion imaging. Then, dogs were sacrificed, and the hearts were removed, triphenyltetrazolium chloride staining and conventional HE staining were performed. CT number of non-ischemic and ischemic regions were measured and analyzed. The wall of the left ventricle in the short axis was divided into 17 segments, the segments of myocardial perfusion defect in DSCT myocardial iodine maps, SPECT, and pathology were determined. Student t test was used to analyze the difference of CT number between infarcted and non-infarcted myocardium. Kappa test was used for the accuracy of DSCT myocardial iodine maps and SPECT in detecting myocardial ischemia according to the pathological results. Results No abnormal regions were detected using DSCT myocardial iodine maps in preoperative control and infarction group. After thoracotomy, partial sparse or defective perfusion was consistently noted in six dogs' apical anterior and partition wall in both DSCT myocardial iodine maps and SPECT. In the infarcted group, the attenuation of infarction region (34.75 ± 16.66) HU was significantly decreased compared with preoperative measurements ( 123. 18 ± 15.38 ) HU ( t = 10. 526, P ＜ 0. 01 ); decreased perfusion in the infarcted region was also noted in the DSCT myocardial iodine maps and SPECT. The sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of DSCT myocardial iodine maps and SPECT were 85.0% (34/40) , 84. 1% (95/113) ,65.4% (34/52) ,94. 0%(95/101) ,and 82. 5% (33/40), 90. 3% ( 102/113 ) ,75.0% (33/44) ,93.6% ( 102/109 ), respectively.Kappa values were 0. 63 and 0. 71 for the agreement of DSCT myocardial iodine maps and SPECT.Conclusion DSCT myocardial iodine maps is comparable diagnostic accuracy with rest SPECT myocardial perfusion imaging in detection of acute myocardial infarction in a canine model.

Abstract:Purpose: To evaluate the feasibility and value of dual-energy computed tomography myocardial iodine maps in the diagnosis of acute myocardial infarction.Materials and Methods: In 6 dogs, arterial-phase myocardial dual-energy computed tomography imaging were performed 1 day prior to and 3 hours after the surgical ligation of the left anterior descending artery to generate 100 kVp, 140 kVp, average weighted images, and dual energy myocardial iodine maps. For each of the 17 segments of the left ventricle (LV, 102 total segments), the presence or absence myocardial infarction was determined by histopathology and correlated to blinded reader determination of infarcted and noninfarcted myocardium at computed tomography (CT). Statistical analysis for diagnostic accuracy of aforementioned techniques and interreader agreement was performed.Results: The LV myocardial contrast enhancement at the average weighted images and iodine maps were uniform in all 6 dogs before surgery. Following anterior descending artery ligation, histopathology showed 40 infarcted left ventricular segments and 62 noninfarcted segments. For the postligation CT scans, 100 kVp, 140 kVp, average weighted images, and myocardial iodine maps showed 33, 28, 33, 34 infarcted segments and 53, 56, 56, 52 noninfarcted segments for both readers; corresponding to per-segment sensitivities of 83%, 70%, 80%, 92% and specificities of 85%, 90%, 92%, 80% for detecting myocardial infarction. No statistical difference was found for diagnostic accuracy of 100 kV, 140 kV, weighted average images, and iodine maps to detect myocardial infarct segments (all P > 0.05 for both readers). Good inter-reader agreement was seen for myocardial infarct detection using iodine maps (kappa = 0.80).Conclusions: Myocardial single-and dual-energy CT imaging shows high per-segment sensitivity and moderate specificity for detecting acute myocardial infarction in a canine model with histopathology as the standard of reference.

Abstract:Objectives To explore the protection effect and mechanism of lung ischemic preconditioning on lung and myocardium in canine cardiopulmonary bypass(CPB). Methods 12 adult mongrel canines(12-15 kg) were randomly divided into two groups, 6 for control group(group C), in which the CPB was established by abdominal aorta and right atrium cannulation, experimental group was treated with ischemic preconditioning, in which left lung hilus were clamped for two cycles of five minutes followed five minutes released before cross-clamping of the aorta in six canines(croup IP). The technique of myocardium protection and cardiac arrest was the same in both groups(cold crystalline cardioplegia). CPB was controlled running for 1 hour in both groups. The lung and myocardial tissue were sampled at the time of pre- CPB and 120 minutes after reperfusion to test the wet/dry ratio of lung tissue and myocardial water content(MWC), maleic dialdehyde(MDA )and superoxid dismutase(SOD) of both tissue. Lung and myocardial biopsies were obtained before and after CPB in both groups for microscope pathological examination. Results ⑴After reperfusion, the wet/dry ratio of lung tissue in group IP was significantly lower than that in group C(3.4867±0.6776 vs 5.3067±0.3034,P<0.05). The MDA of lung tissue in group IP was significantly lower than that in group C[(5.3517±1.0769)nmol/mgprot vs (7.4002±1.1569)nmol/mgprot,P<0.05). SOD activity of lung tissue in group IP was significantly higher than group C[(127.4512±18.88)U/mgprot vs (103.6103±9.5299)U/mgprot,P<0.05). ⑵After reperfusion, the MWC of myocardium in group IP was significantly lower than that in group C(64.6173±4.1682 vs 77.3333±3.0807,P<0.05). The MDA of myocardium in group IP was significantly lower than that in group C[(5.3233±1.5304)nmol/mgprot vs(7.5517±1.0236)nmol/mgprot,P<0.05);SOD activity of myocardium in group IP was significantly higher than group C [(166.0104±6.3458)U/mgprotvs(140.4514±13.9413)U/mgprot,P<0.05). ⑶ pathological examination of lung and myocardial tissue revealed that in control group there were more obvious intraalveolar hemorrhage, leukocyte adherence and interstitial edema compared with group IP. Conclusion The study indicates that lung ischemic preconditioning may have protective effect on lung and myocardium after CPB. The possible mechanism was that ischemic preconditioning reduced the production of ischemia-reperfusion injury.
