Laboratory test results showed hyperlipidemia and an elevated level of cancer antigen 19-9 (61.3 U/mL). MR cholangiopancreatography showed stenosis of the primary biliary confluence with dilation of the intrahepatic bile ducts (Figure, A). Two-dimensional fluorine 18 (18F) fluorodeoxyglucose PET/CT showed multiple nodule-type lesions with intensive radiotracer uptake in the mesentery and omentum (Figure, B), without abnormal radiotracer uptake in the porta hepatis. At laparoscopic exploration, numerous small nodules with a yellowish-orange color were observed on the hemidiaphragm, peritoneal, and mesenteric surfaces (Figure, C). Photomicrography with hematoxylin-eosin stain of the specimen showed multinucleated giant histiocytic cells with foamy cytoplasm (Figure, D). Clinical findings, radiologic findings, and histologic features were consistent with xanthoma disseminatum. The patient subsequently was administered lipid-lowering agents for 2 months. Her cholesterol reduced to normal levels, and her appetite returned.Images in a 74-year-old woman with 1-month history of poor appetite. Extracutaneous involvement has been reported in less than 20% of patients (2). The most commonly observed extracutaneous lesions are in the respiratory tract, myocardium, oral cavity, and skeletal musculature. This case is an example of extracutaneous xanthoma mimicking peritoneal carcinomatosis from hilar cholangiocarcinoma.
